Abstract Understanding the dynamic behavior of soils is essential to the study of the influence of seismic loads on the instability of submarine slopes, an important issue in Brazil and other countries. The shear modulus and the damping ratio are two fundamental parameters for the study of this behavior. Determining these parameters for Speswhite kaolin clay is the object of the present study using resonant column tests and dynamic centrifuge tests with accelerometers and pairs of bender elements. The curves obtained in the laboratory are compared with empirical curves and comparable data in the literature. Good agreement was observed between experimental data and the empirical prediction for the degradation curve of the normalized shear modulus. The damping curve for very low strains, obtained with resonant column tests, was consistent with the empirical curve. However, consistent with a trend observed in the literature, the centrifuge test results presented considerable scatter (dispersion), attributable to the difficulty in modelling damping dissipation mechanisms in the centrifuge.

ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

ACUTE EFFECTS OF DIFFERENT INTENSITIES RESISTANCE TRAINING ON INFLAMMATORY MARKERS OF WOMEN WITH SEVERE OBESITY

RESUMO Este estudo objetivou comparar os efeitos agudos entre uma sessão de treinamento de força de alta versus moderada intensidade sobre marcadores inflamatórios em mulheres com obesidade severa. Estudo experimental do tipo cross-over, realizado com mulheres adultas com obesidade severa (IMC ≥ 40,0 kg/m²; idade ≥ 18 anos). Cada participante realizou uma sessão de treinamento de força de alta e de moderada intensidade (HIRT e MIRT, respectivamente) com intervalo de 7 dias entre as sessões. Quatro exercícios foram realizados em ambos os protocolos, com volume total de treino equalizado. O MIRT consistiu em 3 séries de 8 repetições cada, com intensidade de 75% de 1RM, enquanto o HIRT consistiu em 2 séries de 6 repetições, seguidas de um descanso de 20 segundos, com nova execução de 2 a 3 repetições, com mais um descanso de 20s e finalização com mais 2 a 3 repetições, com intensidade de 85% de 1RM. Amostras de sangue foram coletadas antes, 15 minutos após e 24 horas após ambos os protocolos de treino. Os marcadores inflamatórios analisados foram IL-2, IL-4, IL-6, IL-10, TNF-α e IFN-γ. Nove participantes completaram a intervenção (n=9; 35,2 ± 10,93 anos; IMC = 48,3 ± 5,06 kg/m²). O teste U de Mann-Whitney mostrou que não houve diferença significativa entre HIRT e MIRT na concentração dos marcadores. O teste de Friedman não mostrou diferenças significativas entre as medidas intragrupo para HIRT e MIRT. Esta pesquisa sugere que uma única sessão de treinamento de força de alta ou moderada intensidade não altera o estado inflamatório de mulheres com obesidade severa.
ABSTRACT This study objective to compare the acute effects between one resistance training bout with high versus moderate intensities on inflammatory markers in women with severe obesity. Experimental crossover study, performed with adult women with severe obesity (BMI ≥ 40.0 kg/m²; age ≥ 18 years old). Each participant performed a high and moderate intensity resistance training (HIRT and MIRT, respectively) with an interval of 7 days between bouts. Four exercises were performed in the both protocols, with total training volume equalized. MIRT consisted of 3 sets of 8 repetitions each, with an intensity of 75% of 1RM, while HIRT consisted of 2 sets of 6 repetitions, followed by a 20-second rest, with a new performance of 2 to 3 repetitions, with one more rest of 20s and finishing with 2 to 3 more repetitions, at an intensity of 85% of 1RM. Blood samples were collected before, 15 minutes after and 24 hours after both training protocols. The inflammatory markers analyzed were IL-2, IL-4, IL-6, IL-10, TNF-α and IFN-γ. Nine participants have completed the intervention (n=9; 35.2 ± 10.93 years old; BMI = 48.3 ± 5.06 kg/m²). The Mann-Whitney U test showed that there was no significant difference between the HIRT and MIRT in the concentration of markers. Friedman's test did not report significant differences between intragroup measures for the HIRT as well as MIRT. This research suggests that a single session of high or moderate intensity resistance training does not change the inflammatory status of women with severe obesity.

Catastrophic thinking and functional disability in Primary Health Care chronic pain patients

RESUMO JUSTIFICATIVA E OBJETIVOS: Os pensamentos catastróficos são fatores avaliativos importantes porque interferem no gerenciamento da dor e no grau de incapacidade, uma vez que mediam as respostas à dor. O estudo objetivou investigar a relação entre pensamentos catastróficos e as dimensões de incapacidade funcional em portadores de dores crônicas de origem musculoesquelética no âmbito da atenção básica a saúde. MÉTODOS: Estudo descritivo transversal que incluiu 50 pacientes de ambos os sexos, com idade entre 18 e 59 anos, portadores de dor crônica musculoesquelética, com intensidade de 3 a 7 pela escala analógica visual. Para avaliação da catastrofização da dor foi utilizada a Escala de Pensamentos Catastróficos sobre a Dor (B-PCS) e a interferência da dor na funcionalidade foi avaliada pelo Índice de Incapacidade Relacionada à Dor (PDI). RESULTADOS: A maioria dos pacientes foi do sexo feminino (94%), com média etária de 42±10,3 anos, mediana da dor 7 [6-7], nível fundamental de escolaridade (62%), sem emprego (66%) e sedentários (76%). Catastrofização (56%). A maior prevalência de dor foi identificada nas regiões toracolombar (38%), anterior dos joelhos (32%), lombossacral e face (30%). Houve correlação direta e significativa entre os escores da PCS e do PDI (ρ=0,56; p<0,01) e entre os escores do PDI e os subdomínios ruminação (ρ=0,34; p<0,01), magnificação (ρ=0,57; p<0,01) e desesperança (ρ=0,53; p<0,01). CONCLUSÃO: Os portadores de dor crônica apresentaram orientação mental em direção aos aspectos mais desagradáveis da dor, acarretando menor participação nas atividades cotidianas, com repercussão sobre a capacidade funcional.
ABSTRACT BACKGROUND AND OBJECTIVES: Catastrophic thoughts are an important evaluative factor because they interfere with pain management and degree of disability since they affect responses to pain. The study aimed to investigate the relationship between catastrophic thoughts and the dimensions of functional disability in patients with chronic musculoskeletal pain in primary health care. METHODS: A cross-sectional descriptive study that included 50 patients of both genders, aged 18 to 59 years, with chronic musculoskeletal pain, with intensity from 3 to 7 by the visual analog scale. To evaluate pain catastrophization, the Brazil validated Pain Catastrophizing Scale (B-PCS) was used and the interference of pain on function was assessed by the Pain Disability Index (PDI). RESULTS: The majority of patients were female (94%), mean age 42±10.3 years, median pain 7 [6-7], basic schooling level (62%), unemployed (66%), and sedentary (76%). Catastrophizing (56%). The regions with the highest prevalence of pain were thoracolumbar (38%), anterior knees (32%), lumbosacral and face (30%). There was a direct and significant correlation between the PCS and PDI scores (ρ=0.56; p<0.01), as well as between the PDI scores and the subdomains: rumination (ρ=0.34; p <0.01), magnification (ρ=0.57; p <0.01) and helplessness (ρ=0.53; p <0.01). CONCLUSION: People with chronic pain demonstrate a tendency towards the most unpleasant aspects of pain, resulting in less participation in daily activities, with impacts on functional capacity.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

RESUMO Objetivos: Avaliar a relação entre hipossalivação, doenças sistêmicas e uso de medicamentos, sintomas bucais, experiência com cárie, fluxo e pH salivar. Métodos: Realizou-se estudo transversal com 50 participantes com xerostomia, selecionados e distribuídos aleatoriamente em dois grupos: 25 com hipossalivação e 25 sem hipossalivação, pareados em idade e sexo. Determinou-se o fluxo salivar em repouso (FSR), índice de dentes cariados, perdidos e obturados (CPO-D) e pH salivar. Aplicou-se teste de Mann-Whitney e teste qui-quadrado, considerando significantes valores de p<0,05. Resultados: No grupo de participantes com hipossalivação 88% usavam medicamentos e 96% tinham doença sistêmica e, entre os sem hipossalivação, 48% usavam medicamentos e 64% tinham doenças sistêmicas. Aqueles com hipossalivação tiveram os maiores percentuais de disgeusia (60%) e ardor bucal (36%). Houve diferenças estatisticamente significantes para as medianas de FSR (0,08 ml/minuto / 0,2 ml/minuto) (p=0,000), pH (6/7) (p=0,000) e CPO-D (22/17) (p=0,004) obtidas dos participantes com hipossalivação e sem hipossalivação, respectivamente. Apenas no grupo com hipossalivação houve associação estatisticamente significante do fluxo salivar em repouso com faixa etária (p=0,035), tipo de doença sistêmica (p=0,049) e pH (p=0,032) e, o CPO-D teve associação com doenças sistêmicas (p=0,015). Conclusão: Os resultados sugerem que a hipossalivação piora a condição dental, favorece a presença de sintomas bucais e, o fluxo salivar em repouso sofre influência de doenças sistêmicas e faixa etária.
ABSTRACT Objective: The aim of this study was to evaluate the relationship among the following features: hyposalivation, systemic diseases and drug use, oral symptoms, dental condition, salivary flow and salivary pH, as well. Methods: A cross-sectional study was performed with 50 participants diagnosed with xerostomia, randomly selected and distributed in two groups: 25 with hyposalivation and 25 without hyposalivation, paired in age and sex. Unstimulated Salivary Flow Rate (USFR), Decayed, Missing, Filled, Teeth (DMFT) index and salivary pH were determined. The Mann-Whitney test and chi-square test were applied, considering significant for p-values <0.05. Results: Among the participants with hyposalivation, 88% used drugs and 96% presented systemic disease. And among those without hyposalivation, 48% used drugs and 64% presented systemic disease. The ones with hyposalivation showed the highest levels of dysgeusia (60%) and burn mouth (36%). There were statistically significant differences for the medians of USFR (0.08ml/minute / 0.2ml/minute) (p = 0.000), pH (6/7) (p = 0.000) and DMFT (22/17) (p = 0.004) obtained from participants with hyposalivation and without hyposalivation, respectively. Only in the group with hyposalivation there was a statistically significant association of unstimulated salivary flow rate with age (p = 0.035), type of systemic disease (p = 0.049) and pH (p=0.032) and DMFT demonstrated an association with systemic diseases (p = 0.015). Conclusion: The research results have suggested that hyposalivation worsens dental status triggering oral symptoms, and that salivary flow is influenced by the type of systemic disease and age group.

Abstract Our aim was to evaluate the sensorial acceptance and consumption intent of pasteurized milks and coalho cheeses produced with milk with low (80,000 cells/mL) and high (480,000 cells/mL) somatic cell count (SCC) level. Low SCC pasteurized milk (LCC) obtained statistically higher scores on the aroma, consistency, flavor and overall liking attributes in comparison with high SCC pasteurized milk. LCC mean scores for all attributes were statistically higher than high SCC Coalho Cheese. High SCC in raw milk has a detrimental effect on the quality of pasteurized milk and coalho cheeses as it presented lower sensory acceptance, failing to meet the consumer’s desired expectations.

RESUMO: Um pônei mestiço de 48 horas de vida foi encaminhado ao hospital para avaliação por desconforto abdominal, diminuição do reflexo de sucção e prostração. Durante a investigação clínica, tratamentos de suporte e sintomáticos foram necessários, e radiografia abdominal foi realizada revelando cólon maior repleto de fezes e grandes quantidades de gás, além de fluido livre na cavidade. Após três dias de tratamento, o potro ainda não havia defecado, sendo a laparotomia exploratória indicada, mas os proprietários recusaram. Portanto, o potro foi humanamente eutanasiado. O exame post-mortem revelou ausência completa da flexura pélvica. Porções subseqüentes dos cólons dorsal, cólon transverso e cólon menor se apresentaram intensamente reduzidas. Além disso, na cavidade abdominal havia 850 ml de líquido translúcido amarelado. Esses achados são compatíveis com o hidroperitônio e a atresia coli tipo III na flexura pélvica, associados ao subdesenvolvimento dos cólons dorsal, transverso e menor.
ABSTRACT: A 48-hour-old mixed breed pony colt was referred to the hospital because of abdominal discomfort, weak suckling reflex and prostration. During clinical investigation, supportive and symptomatic treatments were necessary, and an abdominal radiography was performed revealing a large intestine filled with feces and large amounts of gas, in addition to free fluid in the cavity. After 3 days of treatment, the foal had not yet defecated, thus exploratory abdominal surgery was indicated, but the owners declined. Therefore, the foal was humanly euthanized. Post-mortem examination revealed complete absence of the pelvic flexure. Subsequent portions of the dorsal, transverse and small colons were intensely reduced. In addition, at the abdominal cavity there was 850 ml of yellowish translucent liquid. These findings are compatible with hydroperitoneum and type III atresia coli at the pelvic flexure, associated with underdevelopment of the dorsal, transverse and small colons.

Abstract The objective of this study was to evaluate the influence of the milking method, the storage conditions and the SCC (Somatic Cell Count) increase on the quality of raw milk. Monthly evaluations were performed out over a year in 21 tanks by monitoring the refrigeration temperature and the storage time of the milk in the tank. The tanks were grouped into three temperature levels. Milk storage time intervals were established in each tank: up to 24 h of storage; between 24 and 48 h; and above 48 h. The effect of SCC on the composition was evaluated in three categories: Low SCC; Medium SCC; High SCC. In the analyzed period, 10.8 % presented low SCC, followed by 46.5 % with medium SCC, while 42.7 % had high SCC. There was a positive correlation between SCC and protein, and a negative correlation between SCC and lactose. It is concluded that the milking method does not influence the microbial contamination of the milk; however, longer storage time and increased temperature influenced an increase in microorganism counts in milk. In evaluating the hygienic/sanitary quality of the milk, 42.7 % had high SCC and the total bacterial counts presented values above the values recommended by legislation.
Resumen Las condiciones del ordeño y almacenamiento pueden afectar a la calidad de la leche cruda. Se hizo una evaluación de la influencia del método de ordeño, las condiciones de almacenamiento y el aumento en el recuento de células somáticas (RCS) en la calidad de la leche cruda. Se realizaron evaluaciones mensuales durante un año en 21 tanques mediante el monitoreo de la temperatura de refrigeración y el tiempo de almacenamiento de la leche en el tanque. Los tanques se agruparon en tres rangos de temperatura. Se establecieron intervalos de tiempo de almacenamiento de leche en cada tanque: hasta 24 h; de 24 a 48 h; y más de 48 h. El efecto de RCS sobre la composición se evaluó en tres categorías: RCS bajo; RCS intermedio; y RCS alto. En el período analizado, solo el 10.8 % de los tanques tuvieron un RCS bajo, el 46.5 % tuvieron uno intermedio, y el 42.7 % uno alto. Hubo una correlación positiva entre el RCS y el nivel de la proteína, y una correlación negativa entre el RCS y el nivel de la lactosa. Los resultados indican que el método de ordeño no influye en la contaminación microbiana de la leche. En contraste, un mayor tiempo de almacenamiento y el aumento de la temperatura causaron un aumento en el RCS y el conteo microbiano total. Las prácticas de higiene en los hatos y los tanques estudiados requieren de mejoramiento, ya que el 42.7 % de los tanques presentaron un RCS alto y los conteos bacterianos totales presentaron valores superiores a los recomendados por la legislación.

OBJECTIVE: This study aimed to determine the relationship between rs17576 (MMP-9) polymorphism and increased cancer risk in a Brazilian breast cancer cohort. METHODS: This study included 141 women (71 breast cancer patients and 70 controls without breast cancer) who donated 3 mL of their peripheral blood for genomic DNA extraction. This DNA was then genotyped using a real-time polymerase chain reaction. RESULTS: The AG (rs17576) genotype was identified in 26 (18.43%) participants in the case group and in 22 (15.60%) participants in the control group (p=0.274), while the GG genotype was identified in ten (7.09%) participants in the case group and in one (0.70%) participant in the control group (p<0.003 - OR (95% CI) 13.13 (1.73, 593.08). No significant difference in the incidence rates was observed for AG or GG rs17576 genotypes in premenopausal women, p=0.813 and p=0.556, respectively. However, in postmenopausal women, the AG genotype was shown to occur in 14 (22.5%) participants in the case group and in 4 (6.45%) participants in the control (p<0.043), while GG genotype occurred in eight (12.90%) of the individuals in the case group and in none of the individuals in the control group (p<0.006). CONCLUSION: In this study, the MMP-9 rs17576 GG polymorphic variant was shown to be significantly associated with breast cancer risk in premenopausal women, while the AG and GG genotypes were associated with increased cancer risk in postmenopausal women.

Resumo: Embora os girinos de anuros sejam amplamente distribuídos e abundantes em habitats aquáticos tropicais, não há chaves taxonômicas suficientes para as espécies da Mata Atlântica. Aqui, desenvolvemos uma chave dicotômica para identificar os girinos de todas as espécies com fase larval conhecida e já registradas nesse bioma, ao norte do rio São Francisco. Nós analisamos características discretas de 1.042 girinos pertencentes a 63 espécies (28 gêneros) de 32 localidades. Esta chave de identificação inclui ilustrações e imagens, sendo um passo significativo para um melhor conhecimento dos girinos da Mata Atlântica.
Abstract: Although anuran tadpoles are widely distributed and abundant in tropical aquatic habitats, there is a lack of taxonomic keys for the Atlantic Forest. Herein, we developed a dichotomous key for identifying the tadpoles for all species with known larval phase and already recorded in the Atlantic Forest north of the São Francisco River. We analyzed discrete characteristics of 1,042 tadpoles encompassing 63 species of 28 genera from 32 localities. The user-friendly key includes illustration and pictures, and it is a significant step towards improving our knowledge of tadpoles of the Atlantic Forest.

Abstract Objective: To evaluate whether there is any difference on the results of patients treated with coronary artery bypass grafting (CABG) or percutaneous coronary intervention (PCI) in the setting of ischemic heart failure (HF). Methods: Databases (MEDLINE, Embase, Cochrane Controlled Trials Register [CENTRAL/CCTR], ClinicalTrials.gov, Scientific Electronic Library Online [SciELO], Literatura Latino-americana e do Caribe em Ciências da Saúde [LILACS], and Google Scholar) were searched for studies published until February 2019. Main outcomes of interest were mortality, myocardial infarction, repeat revascularization, and stroke. Results: The search yielded 5,775 studies for inclusion. Of these, 20 articles were analyzed, and their data were extracted. The total number of patients included was 54,173, and those underwent CABG (N=29,075) or PCI (N=25098). The hazard ratios (HRs) for mortality (HR 0.763; 95% confidence interval [CI] 0.678-0.859; P<0.001), myocardial infarction (HR 0.481; 95% CI 0.365-0.633; P<0.001), and repeat revascularization (HR 0.321; 95% CI 0.241-0.428; P<0.001) were lower in the CABG group than in the PCI group. The HR for stroke showed no statistically significant difference between the groups (random effect model: HR 0.879; 95% CI 0.625-1.237; P=0.459). Conclusion: This meta-analysis found that CABG surgery remains the best option for patients with ischemic HF, without increase in the risk of stroke.

Abstract Objectives: This study sought to evaluate the impact of prosthesis-patient mismatch (PPM) on the risk of early-term mortality after transcatheter aortic valve implantation (TAVI). Methods: Databases (Medical Literature Analysis and Retrieval System Online [MEDLINE], Excerpta Medica dataBASE [EMBASE], Cochrane Controlled Trials Register [CENTRAL/CCTR], ClinicalTrials.gov, Scientific Electronic Library Online [SciELO], Latin American and Caribbean Literature on Health Sciences [LILACS], and Google Scholar) were searched for studies published until February 2019. PPM after TAVI was defined as moderate if the indexed effective orifice area (iEOA) was between 0.85 cm2/m2 and 0.65 cm2/m2 and as severe if iEOA ≤ 0.65 cm2/m2. Results: The search yielded 1,092 studies for inclusion. Of these, 18 articles were analyzed, and their data extracted. The total number of patients included who underwent TAVI was 71,106. The incidence of PPM after TAVI was 36.3% (25,846 with PPM and 45,260 without PPM). One-year mortality was not increased in patients with any PPM (odds ratio [OR] 1.021, 95% confidence interval [CI] 0.979-1.065, P=0.338) neither in those with moderate PPM (OR 0.980, 95% CI 0.933-1.029, P=0.423). Severe PPM was separately associated with high risk (OR 1.109, 95% CI 1.041-1.181, P=0.001). Conclusion: The presence of severe PPM after TAVI increased early-term mortality. Although moderate PPM seemed harmless, the findings of this study cannot not rule out the possibility of it being detrimental, since there are other registries that did not address this issue yet.

Abstract Prosthesis-patient mismatch (PPM) after surgical aortic valve replacement (SAVR) is an issue that has been overlooked (not to say neglected). Cardiac surgeons must bear in mind that this is a real problem that we must tackle. The purpose of this paper is to be a wake-up call to the surgical community by giving a brief overview of what PPM is, its incidence and impact on the outcomes. We also discuss the increasing role played by imaging for predicting and assessing PPM after SAVR (with which surgeons must become more acquainted) and, finally, we present some options to avoid PPM after the surgical procedure.
