FUNDAMENTO: Parâmetros derivados do Doppler tecidual correlacionam-se com a pressão diastólica final do ventrículo esquerdo (VE) e podem servir como índice prognóstico na insuficiência cardíaca. OBJETIVO: Determinar se parâmetros do Doppler tecidual podem predizer eventos em longo prazo em pacientes ambulatoriais com disfunção sistólica do VE. MÉTODOS: Estudo retrospectivo envolvendo 73 pacientes (60,9±12,1 anos) que realizaram ecocardiograma Doppler entre março de 2001 e maio de 2004. O evento primário pesquisado foi morte ou hospitalização por piora da insuficiência cardíaca. RESULTADOS: O período de seguimento médio foi de 1.367±665 dias. Após análise logística multivariada "stepwise" incluindo os parâmetros ecocardiográficos, a razão entre as velocidades máximas de enchimento do VE e miocárdica no início da diástole (razão E/E'; p=0,0007) e a fração de ejeção do VE (FE; p=0,01) permaneceram como preditores do evento primário. Os pontos de corte ótimos para a previsão do evento primário para a razão E/E' (AUC 0,77; p=0,0001) e FE (AUC 0,68; p=0,006) foram, respectivamente, 12,7% e 30%. Assim, pacientes com razão E/E' > 12,7 (razão de risco=3,8, p=0,001) ou FE <30% (razão de risco=2,3, p=0,03) demonstravam pior prognóstico pela análise da curva de sobrevida. Importante salientar que 47% dos pacientes com FE acima do ponto de corte, mas razão E/E' elevada, apresentaram eventos durante o período estudado. CONCLUSÃO: Em pacientes ambulatoriais com disfunção sistólica do VE, a razão E/E' é um importante e independente indicador prognóstico em longo prazo de morte ou hospitalização. Portanto, recomenda-se incluir a medida dessa variável na avaliação rotineira desses pacientes.

BACKGROUND: Tissue Doppler parameters correlate with left ventricular (LV) filling pressure and can be useful as prognostic indexes for patients with heart failure. OBJECTIVE: Determine whether tissue Doppler parameters can predict events during long term follow-up of outpatients with LV systolic dysfunction. MeTHODS: Retrospective study with 73 patients (aged 60.9±12.1 years) who underwent Doppler echocardiogram between March 2001 and May 2004. The primary endpoint studied was death or hospitalization due to heart failure worsening. RESULTS: The mean follow-up period was 1,367±665 days. After logistic stepwise multivariate analysis, including echocardiographic parameters, the ratio of maximal early diastolic filling wave velocity to maximal early diastolic myocardial velocity (E/E` ratio; p=0.0007), and LV ejection fraction (EF; p=0.01) remained significant predictors of the primary outcome. The optimal cutoffs for primary endpoint prediction for E/E' ratio (AUC 0.77; p=0.0001) and EF (AUC 0.68, p=0.006) were respectively 12.7 and 30%. Accordingly, patients with E/E' ratio > 12.7 (hazard ratio=3.8, p =0.001) or EF <30% (hazard ratio=2.3, p=0.03) had a poorer outcome by survival curve analysis. It is noteworthy that 47% of the patients with EF above the optimal cutoff point, but with high E/E' ratio, presented events during follow-up. CONCLUSION: E/E' ratio is an important independent long-term prognostic index of death or hospitalization due to worsening heart failure in outpatients with LV systolic dysfunction. Therefore, we recommend the measurement of this variable in the routine evaluation of such patients.

Visando avaliar o intervalo QTc e sua relação com variáveis clínicas, laboratoriais e com suscetibilidade da LDL à oxidação in vitro em pacientes com DM1, estudamos 40 diabéticos e 33 não diabéticos com idades de 24,83 ± 10,21 e 23,51 ± 7,28 anos, respectivamente, pareados por sexo, idade e índice de massa corporal (IMC). Avaliamos controle metabólico, apolipoproteínas A e B, coeficiente de oxidação da LDL por espectrofotometria e eletrocardiograma (ECG). O intervalo QTc foi calculado pela fórmula de Bazett. Não houve diferença no QTc entre os grupos dos DM1 e dos não diabéticos (394,43 ± 19,98 ms vs. 401,31 ± 17,83 ms; p = 0,2065). Cinco diabéticos apresentavam QTc aumentado (396,76 ± 14,63 ms vs. 429,75 ± 1,89 ms; p < 0,001) e menores níveis de apolipoproteína A que os demais diabéticos (74,60 ± 25,42 mg/dL vs. 113,64 ± 29,79 mg/dL; p = 0,011). Na amostra total, houve correlação entre QTc e IMC (rho = -0,288; p = 0,045), glicemia pós-prandial (rho = 0,357; p = 0,016) e coeficiente de oxidação 3 h (Cox3h) (r = -0,293; p = 0,039). Nos diabéticos, encontramos correlação entre QTc e triglicerídeos (rho = -0,420; p = 0,023) e Cox3h (r = -0,427; p = 0,021). Embora não tenhamos encontrado diferença entre o QTc dos diabéticos e não diabéticos estudados, houve correlação com marcadores de risco para a doença aterosclerótica. Entretanto, ainda são necessários mais estudos para estabelecer o real valor preditivo do QTc para esta doença nos pacientes com DM1.

To evaluate the QTc interval and its relation with clinical, laboratorial variables and LDL susceptibility to in vitro oxidation in patients with type 1 DM, we studied 40 diabetics and 33 non diabetics with 24.83 ± 10.21 and 23.51 ± 7.28 years old, respectively matched by sex, age and body mass index (BMI). We evaluated metabolic control, A and B apolipoproteins, LDL oxidation coefficient for spectrophotometry and electrocardiogram (ECG). Interval QTc was calculated by the Bazett’s formula. There was no difference in QTc between diabetic and non diabetic groups (394.43 ± 19.98 ms versus 401.31 ± 17.83 ms; p = 0.2065). Five diabetics showed increased QTc (396.76 ± 14.63 ms versus 429.75 ± 1.89 ms; p < 0.001) and lesser A apolipoprotein levels than rest of diabetic group (74.60 ± 25.42 mg/dL versus 113.64 ± 29.79 mg/dL; p = 0,011). In pooled sample, there was correlation between QTc and BMI (rho = -0.288; p = 0.045), pot-prandial glycemia (rho = 0.357; p = 0.016) and 3 h oxidation coefficient (OxC3h) (r = -0.293; p = 0.039). In diabetics, there was correlation between QTc and triglycerides (rho = -0.420; p = 0.023) and OxC3h (r = -0.427; p = 0.021). Although there was no difference between QTc of diabetics and the non diabetics subjects studied, there was correlation with risk factors for the atherosclerotic disease. Further studies are necessary to establish the real predictive value of QTc for this type of disease in the patients with type 1 DM.

OBJETIVO: Avaliar a atividade da acetil-hidrolase do fator ativador de plaquetas (PAF-AH) e sua relação com variáveis clinicodemográficas, com o controle metabólico, os níveis de apolipoproteínas A e B e a suscetibilidade da lipoproteína de baixa densidade (LDL) à oxidação in vitro em pacientes com DM tipo 1 (DM 1). MÉTODOS: Foram avaliados 42 pacientes com DM1 (27 mulheres) e 48 não-diabéticos (16 mulheres), pareados por sexo, idade e índice de massa corporal (IMC). Os exames realizados foram: glicemia de jejum (GJ) e pós-prandial (GPP), lipidograma, ácido úrico (AU), hemoglobina glicosilada (HbA1c) e coeficiente de oxidação da lipoproteína de baixa densidade (LDL) por espectrofotometria. A análise da atividade da PAF-AH foi realizada por espectrofotometria (Cayman Chemical). RESULTADOS: A análise da atividade da PAF-AH mostrou haver maior atividade enzimática nos pacientes com DM 1 do que nos não-diabéticos (0,0150 ± 0,0051 versus 0,0116 ± 0,0041; p < 0,001). Nos pacientes com DM 1, encontramos correlação direta entre a atividade da PAF-AH e a idade e a LDL, e inversa entre a PAF-AH e a HbA1c e a lipoproteína de alta densidade (HDL). CONCLUSÃO: A PAF-AH, na amostra estudada, apresentou maior atividade nos pacientes com DM 1, fator que pode estar relacionado ao maior risco de desenvolver doenças cardiovasculares apresentados por portadores dessa doença. Ainda são necessários novos estudos para avaliar a real participação dessa enzima no risco de desenvolvimento das doenças ateroscleróticas nos pacientes com DM 1.

OBJECTIVE: To evaluate platelet-activating factor acetylhydrolase (PAF-AH) activity and its relationship with clinical and demographic variables, metabolic control, apolipoprotein A and B levels and the susceptibility of low-density lipoprotein (LDL) to in vitro oxidation in patients with type 1 diabetes mellitus (DM 1). METHODS: Forty two patients with DM 1 (27 females) and 48 control subjects (16 females) matched for gender, age and body mass index (BMI) were evaluated. The following tests were performed: fast plasma glucose (FG) and postprandial plasma glucose (PPG), lipid profile, uric acid (UA), glycosylated hemoglobin (HbA1c), and low-density lipoprotein (LDL) oxidation rate using colorimetric assay. The PAF-AH activity was analyzed using colorimetric assay (Cayman Chemical). RESULTS: The analysis of PAF-AH activity showed a higher enzyme activity in patients with DM 1 than in control subjects (0.0150 ± 0.0051 vs. 0.0116 ± 0.0041; p < 0.001). In patients with DM 1, a direct correlation between PAF-AH activity and age and LDL, and an inverse correlation between PAF-AH and HbA1c and high-density lipoprotein (HDL) were found. CONCLUSION: In the sample studied, PAF-AH showed a higher activity in patients with DM 1, a factor that may be related to the higher risk of developing cardiovascular diseases observed in these patients. Further studies are necessary to evaluate the real participation of this enzyme in the risk of development of atherosclerotic diseases in patients with DM 1.

Visando avaliar o valor do índice de massa corporal (IMC) como preditor de circunferência abdominal de risco cardiovascular (CARCV) e de diagnóstico de síndrome metabólica (CASM) em pacientes com DM tipo 2 (DM 2), avaliamos o IMC e a CA em 753 pacientes com DM 2 (472 mulheres) com 23 ± 8 anos. Todos os participantes foram divididos em grupos de acordo com a presença de CARCV e CASM, sendo avaliado o melhor ponto de corte de IMC preditor destas alterações nas mulheres e nos homens. Nas mulheres, o IMC > 25,0 kg/m² foi o melhor preditor da CARCV, sendo a área sob a curva ROC e IC 95% de 0,7202 (0,6753 - 0,7652) e de CASM [0,8318 (0,7928 - 0,8708)]. Nos homens, o IMC > 25,0 kg/m² foi melhor preditor da presença de CARCV [0,8527 (0,8098 - 0,8955)], sendo o IMC > 30,0 kg/m² melhor preditor da CASM [0,9071 (0,8708 - 0,9433)]. Concluímos que o cálculo do IMC pode ser uma maneira prática para avaliação de síndrome metabólica e risco cardiovascular em países onde nem sempre estão disponíveis material e treinamento adequado para avaliação da CA. Estudos prospectivos são necessários para avaliar necessidade de mudança nos pontos de corte adotados como indicativos destas alterações nos diabéticos.

To evaluate the value of body mass index (BMI) as predictor of waist circumference of cardiovascular risk (CRWC) and diagnostic of metabolic syndrome (MSWC) in patients with type 2 diabetes mellitus (DM 2), we assessed BMI and WC in 753 patients with DM 2 (472 women) with 23 ± 8 years. The participants had been divided in groups in accordance with the presence or absence of ACCR or ACMS. The best BMI cut-off to predict such disturbances was evaluated in women and men. In females, BMI > 25.0 kg/m² was the best predictor of CRWC. Area under ROC curve and IC 95% were 0.7202 (0.6753 - 0.7652) for CRWC and of [0.8318 (0.7928 - 0.8708)] for MSWC. In males, IMC > 25.0 kg/m² was better predictor for CRWC presence [0.8527 (0.8098 - 0.8955)], while BMI > 30.0 kg/m² for MSWC [0.9071 (0.8708 - 0.9433)]. We conclude that BMI can be a simple way to evaluate metabolic syndrome and cardiovascular risk where there were not material and prepared professionals for the WC evaluation. We need prospective studies to evaluate if it is necessary to change the BMI cut-off adopted as indicative of these disturbances in the diabetic population.

Para verificar a variabilidade do controle glicêmico em um estudo não controlado de diabéticos durante um ano de acompanhamento, nós avaliamos retrospectivamente 113 pacientes diabéticos. Com os valores das HBA1c (hemoglobina glicada) dosadas em 1998 calculamos um índice de controle que foi associado à idade, duração do diabetes, IMC e dose de insulina. A HBA1c foi maior em pacientes DM1 do que DM2, respectivamente [7,9 (4,4-13,3) vs. 7,0 (4,4-13,4)%; p= 0,007]. Nos 90 pacientes com no mínimo duas HBA1c, 68 (75,6%) mantiveram o controle: 51 (76,1%) em bom, 8 (11,1%) em regular e 9 (11,9%) em péssimo controle. Nenhum manteve todas as HBA1c na faixa da normalidade, sendo que 26 pacientes (28,9%) tiveram pelo menos uma HBA1c normal. No grupo geral, 44 pacientes (48,9%) apresentaram aumento da HBA1c, 41 (45,6%) diminuição e 5 pacientes (5,6%) mantiveram o mesmo valor, sem diferença entre DM1 e DM2 (p= 0,77). Observamos diferença na HBA1c e tempo de duração de diabetes entre os pacientes com DM2 que tratavam com dieta, hipoglicemiante oral, terapia combinada e monoterapia com insulina, respectivamente (5,4±0,5 vs. 6,3±1,3 vs. 7,6±1,4 vs. 8,4±2,0%; p= 0,001) e (8,5±9,9 vs. 5,3±4,2 vs. 14,1±9,6 vs. 16,9±8,1 anos; p= 0,003). O coeficiente de variação intraindividual da HBA1c foi de 11,6±7,4% (p= 0,0000), sendo de 12,8±7,6% (p= 0,0000) em DM1 e 10,4±7,2% em DM2 (p= 0,0000) sem diferença entre ambos. Concluímos que em nossa amostra a maioria dos pacientes manteve um bom controle apesar da variabilidade intraindividual e da dificuldade em normalizar os níveis de HBA1c.

To evaluate the variability of glicemic control in diabetic patients followed during one year, we conducted a retrospective study in which the record data of 113 diabetic patients were analyzed. The value of HBA1c (glycated hemoglobin) measured during a year were registered and an index of glicemic control was calculated. This index was associated with duration of diabetes, BMI and dose of insulin. HBA1c were higher in type 1 diabetes than type 2, respectively, [7.9 (4.4-13.3) vs. 7.0 (4.4-13.4)%; p= 0.007]. In 90 patients with at least two HBA1c measurements, 68 (75.6%) had no change in glicemic control: 51 (76.1%) had good, 8 (11.1%) regular and 9 (11.9%) poor control. None of them sustained all the HBA1c in the normal range: 26 (28.9%) had at least one normal HBA1c value. In general group, 44 patients (48.9%) showed increase, 41 (45.6%) decreased and 5 (5.6%) remained with the same level of HBA1c, without difference between DM1 and DM2 (p= 0,77). A difference in HBA1c and diabetes duration among patients with DM2 treated with diet, oral hipoglycemic agents, combined therapy and monotherapy with insulin, was noted respectively (5.4±0.5 vs. 6.3±1.3 vs. 7.6±1.4 vs. 8.4±2.0%; p= 0.001) and (8.5±9.9 vs. 5.3±4.2 vs. 14.1±9.6 vs. 16.9±8.1 years; p= 0,003). The intraindividual coefficient of variation of HBA1c was 11.6±7.4% (p= 0.0000) being 12.8±7.6 (p= 0.0000) in type 1 and 10.4±7.2% in type 2 (p= 0.0000) without difference between both groups. In conclusion: the majority of the patients in our study maintained a good control despite the intraindividual variability of HBA1c and the difficulty to keeping it in the normal range.
