ABSTRACT Background: Several mechanisms, including excessive hunger, account for patients’ difficulties in maintaining weight loss and dietary changes after caloric restriction. Objective: To evaluate the effect of short-term high-fiber calorie-restricted diet in appetite-regulating hormones, and hunger and satiety sensations in women with obesity. Methds: In a randomized controlled trial study, thirty women with body mass index (BMI) higher than 30 kg/m2, and aged from 20 to 50 years were hospitalized following a calorie-restricted diet (1000 kcal/day) for three days. The experimental group (n=15) received high-fiber diet and the control group (n=15), conventional diet. Body weight, BMI, resting energy expenditure (REE), acylated and total ghrelin, leptin, insulin and glucose, and hunger and satiety sensations were evaluated. Linear regression models with mixed effects (fixed and random effects) helped to assess the variables between the two groups and within the groups. Results: Body weight and BMI decreased in both the experimental and control groups (P<0.001). After the high-fiber diet, postprandial acylated ghrelin (P=0.04), glucose (P<0.001), insulin (P=0.04), and leptin (P=0.03) levels as well as the HOMA-IR index (P=0.01) decreased, whereas satiety improved (P=0.02). Obese women that followed the conventional diet had increased body fat percentage (P=0.04) and lower REE (P=0.02). The two diets did not differ in terms of hunger sensation. Conclusion: A short-term high-fiber diet improves satiety sensations and metabolic parameters while suppressing postprandial acylated ghrelin (60 minutes) and maintaining the resting energy expenditure.

RESUMO Contexto: Vários mecanismos, incluindo a fome excessiva, são responsáveis pelas dificuldades dos pacientes em manter a perda de peso e mudanças na dieta após a restrição calórica. Objetivo: Avaliar o efeito da dieta de curta duração rica em fibras e com restrição calórica nos hormônios reguladores do apetite e nas sensações de fome e saciedade em mulheres com obesidade. Métodos: Em um estudo randomizado controlado, 30 mulheres com índice de massa corporal (IMC) superior a 30 kg/m2 e com idade entre 20 e 50 anos foram hospitalizadas seguindo dieta com restrição calórica (1000 kcal/dia) por 3 dias. O grupo experimental (n=15) recebeu dieta rica em fibras e o grupo controle (n=15), dieta convencional. Foram avaliados peso corporal, IMC, gasto energético de repouso (GER), grelina acilada e total, leptina, insulina e glicose e sensações de fome e saciedade. Modelos de regressão linear com efeitos mistos (efeitos fixos e aleatórios) ajudaram a avaliar as variáveis entre os dois grupos e dentro dos grupos. Resultados: O peso corporal e o IMC diminuíram tanto no grupo experimental quanto no controle (P<0,001). Após a dieta rica em fibras, os níveis de grelina acilada pós-prandial (P=0,04), glicose (P<0,001), insulina (P=0,04) e leptina (P=0,03), bem como o índice HOMA-IR (P=0,01) diminuiu, enquanto a saciedade melhorou (P=0,02). Mulheres obesas que seguiram a dieta convencional apresentaram aumento do percentual de gordura corporal (P=0,04) e menor GER (P=0,02). As duas dietas não diferiram em termos de sensação de fome. Conclusão: Uma dieta rica em fibras de curto prazo melhora as sensações de saciedade e os parâmetros metabólicos, suprimindo a grelina acilada pós-prandial (60 minutos) e mantendo o gasto energético de repouso.

Abstract The Handmaid’s Tale series (based on Margaret Atwood’s 1985 novel) has been established as one of the most impacting dramas over the last years. This show approaches dozens of present-day reproduction issues and ethical dilemmas… In a fictional near future, infertility plays a central role. Environmental deterioration, extreme pollution, and sexually transmitted diseases are to blame as both platitudes and mottos to establish a new theological state. The resulting patriarchal system disregards any consequences of male infertility. Female sexual violence, rape and coerced reproduction predominate. Slaves are reduced to their fertility duty, the government executes doctors known to have performed abortions, which are completely forbidden, and female genital mutilation is routinely performed. In The Handmaid’s Tale, the fertility and environmental crisis become a trigger, dramatically affecting the political/social pyramid. Under the shadows of “new laws”, doctors become less and less doctors but mere technicians, blindfolded and hindered of moral insight. Nowadays, standards of care and national/regional laws regarding female health reproduction are extremely wide-ranging. Doctors should reflect: regardless of the setting, is the core of our medical act still intact? Are we practicing a humanistic medicine? Should we be mere scientific practitioners oriented by new and everchanging mutant social obligations?

Resumo A série The Handmaid’s Tale (baseada no romance de Margaret Atwood de 1985) tem sido considerada uma das séries dramáticas mais impactantes dos últimos anos. Nesta, são abordados dezenas de tópicos e dilemas éticos da área da reprodução muito atuais... Num futuro fictício próximo, a infertilidade desempenha um papel crucial. A detioração ambiental, poluição extrema e doenças sexualmente transmissíveis são consideradas causas e motivações para o estabelecimento de um novo estado teológico. Nesse sistema patriarcal resultante, o papel da infertilidade de causa masculina é ignorado. A violência sexual feminina, violação e reprodução coagida predominam. As aias limitam-se à sua fertilidade, o governo executa médicos que levem a cabo abortamentos (que estão completamente proibidos) e a mutilação genital feminina é realizada de forma rotineira. Em The Handmaid’s Tale, a crise de fertilidade e ambiental são o gatilho que força uma dramática alteração da pirâmide política/social. Na sombra das “novas leis”, os médicos, cada vez menos médicos, tornam-se meros técnicos. Atualmente, a qualidade dos serviços e leis nacionais/regionais em saúde reprodutiva feminina são extremamente díspares. Os médicos deverão refletir: independentemente do seu contexto, estará o cerne do nosso ato médico intacto? Estaremos a praticar uma medicina humanística? Seremos meros praticantes de ciência orientados pelas novas e constantemente mutáveis obrigações sociais?

ABSTRACT Research from the last 20 years has provided important insights into the molecular pathogenesis of craniopharyngiomas (CPs). Besides the well-known clinical and histological differences between the subtypes of CPs, adamantinomatous (ACP) and papillary (PCP) craniopharyngiomas, other molecular differences have been identified, further elucidating pathways related to the origin and development of such tumors. The present minireview assesses current knowledge on embryogenesis and the genetic, epigenetic, transcriptomic, and signaling pathways involved in the ACP and PCP subtypes, revealing the similarities and differences in their profiles. ACP and PCP subtypes can be identified by the presence of mutations in CTNNB1 and BRAF genes, with prevalence around 60% and 90%, respectively. Therefore, β-catenin accumulates in the nucleus-cytoplasm of cell clusters in ACPs and, in PCPs, cell immunostaining with specific antibody against the V600E-mutated protein can be seen. Distinct patterns of DNA methylation further differentiate ACPs and PCPs. In addition, research on genetic and epigenetic changes and tumor microenvironment specificities have further clarified the development and progression of the disease. No relevant transcriptional differences in ACPs have emerged between children and adults. In conclusion, ACPs and PCPs present diverse genetic signatures and each subtype is associated with specific signaling pathways. A better understanding of the pathways related to the growth of such tumors is paramount for the development of novel targeted therapeutic agents.

RESUMO OBJETIVO Descrever o perfil clínico funcional dos idosos vinculados à atenção primária à saúde, por meio do Índice de Vulnerabilidade Clínico Funcional (IVCF-20) e realizar a espacialização dos de maior declínio funcional por unidades de atenção primária à saúde no município de Uberlândia-MG, no ano de 2022. MÉTODOS Estudo transversal com dados secundários da Secretaria Municipal de Saúde de Uberlândia-MG. As variáveis foram comparadas através de testes de t de Student, Mann Whitney, qui-quadrado de Pearson e regressão logística multinomial para obter o efeito independente de cada variável. O nível de significância adotado foi de 5% (p < 0,05). Utilizou-se o banco de dados georreferenciado no ArcGIS®. RESULTADOS Foram avaliados 47.182 idosos com idade média de 70,3 anos (60 a 113 anos), sendo 27.138 mulheres (57,52%) e observou-se franco predomínio de idosos de baixo risco ou robustos (69,40%). Todavia, 11,09% são idosos de alto risco e 19,52% estão em risco de fragilização. Idosos do sexo masculino tiveram chance independentemente inferior de risco moderado e alto comparados às mulheres idosas (OR = 0,53; p < 0,001). Foi observado alta prevalência de polifarmácia, 21,40% da população idosa, particularmente nos idosos frágeis, com prevalência de 63,08%. Houve maior distribuição dos idosos frágeis em torno da região central do município e nas unidades de saúde com maior área de abrangência. O IVCF-20 permitiu realizar o rastreio da fragilidade na atenção primária à saúde. CONCLUSÃO O instrumento é capaz de estratificar o risco dos idosos nas redes de atenção à saúde pela atenção primária à saúde, possibilitando a aplicação de intervenções preventivas, promocionais, paliativas ou reabilitadoras individualizadas, conforme o estrato clínico funcional do idoso e os domínios funcionais comprometidos. A estratificação de risco e a distribuição espacial dos idosos mais frágeis pode ser uma boa estratégia de qualificação dos profissionais de saúde com o objetivo de maximizar a autonomia e independência dos idosos.

ABSTRACT OBJECTIVE To describe the functional clinical profile of elderly people linked to primary health care, using the Functional Clinical Vulnerability Index (IVCF-20) and to spatialize those with the greatest functional decline by primary health care units in the municipality of Uberlândia, in the state of Minas Gerais (MG), in the year 2022. METHODS A cross-sectional study with secondary data from the Municipal Health Department of Uberlândia-MG. The variables were compared using Student’s t-test, Mann Whitney test, Pearson’s chi-square, and multinomial logistic regression to obtain the independent effect of each variable. The significance level adopted was 5% (p < 0.05). The georeferenced database in ArcGIS® was used. RESULTS 47,182 older adults were evaluated with a mean age of 70.3 years (60 to 113 years), 27,138 of whom were women (57.52%), with a clear predominance of low-risk or robust older adults (69.40%). However, 11.09% are high-risk older adults and 19.52% are at risk of frailty. Older men had independently lower odds of moderate and high risk compared to older women (OR = 0.53; p < 0.001). A high prevalence of polypharmacy was observed, 21.40% of the older adult population, particularly in frail older adults, with a prevalence of 63.08%. There was a greater distribution of frail older adults around the central region of the municipality and in health units with a larger coverage area. The IVCF-20 made it possible to screen frailty in primary health care. CONCLUSION The instrument is capable of stratifying the risk of older adults in health care networks through primary health care, enabling the application of individualized preventive, promotional, palliative, or rehabilitative interventions, according to the clinical functional stratum of the older adult and the compromised functional domains. Risk stratification and spatial distribution of the frailest older adults can be a good strategy for qualifying health professionals with the aim of maximizing the autonomy and independence of the older adults.

ABSTRACT Objective: Feeding restriction in rats alters the oscillators in suprachiasmatic, paraventricular, and arcuate nuclei, hypothalamic areas involved in food intake. In the present study, using the same animals and experimental protocol, we aimed to analyze if food restriction could reset clock genes ( Clock, Bmal1 ) and genes involved in lipid metabolism ( Pgc1a, Pparg, Ucp2 ) through nutrient-sensing pathways ( Sirt1, Ampk, Nampt ) in peripheral tissues. Materials and methods: Rats were grouped according to food access: Control group (CG, food ad libitum ), Restricted night-fed (RF-n, food access during 2 h at night), Restricted day-fed (RF-d, food access during 2 h in the daytime), and Day-fed (DF, food access during 12 h in the daytime). After 21 days, rats were decapitated at ZT3 (0900-1000 h), ZT11 (1700-1800 h), or ZT17 (2300-2400 h). Blood, liver, brown (BAT) and peri-epididymal (PAT) adipose tissues were collected. Plasma corticosterone and gene expression were evaluated by radioimmunoassay and qPCR, respectively. Results: In the liver, the expression pattern of Clock and Bmal1 shifted when food access was dissociated from rat nocturnal activity; this phenomenon was attenuated in adipose tissues. Daytime feeding also inverted the profile of energy-sensing and lipid metabolism-related genes in the liver, whereas calorie restriction induced a pre-feeding increased expression of these genes. In adipose tissues, Sirt1 expression was modified by daytime feeding and calorie restriction, with concomitant expression of Pgc1a , Pparg , and Ucp2 but not Ampk and Nampt . Conclusion: Feeding restriction reset clock genes and genes involved in lipid metabolism through nutrient-sensing-related genes in rat liver, brown, and peri-epididymal adipose tissues.

O repositório Web of Science foi a fonte das métricas de publicação. Selecionamos as medidas: total de publicações e citações, média de citações por ano e por item, índice H e histórico de citações entre 1950 e 2019. Usamos o nome do(a) docente como um proxy para a identidade de gênero. Usamos estatísticas descritivas para caracterizar as métricas. Avaliamos o efeito tesoura selecionando os(as) professores(as) com índice H alto. A série histórica de citações foi projetada até 2100. Realizamos análises para a população geral e subgrupos de tempo de trabalho: menos de 10 anos, de 10 a 20 anos e 20 anos ou mais. RESULTADOS Dos 8.325 docentes, incluímos 3.067 (36,8%). Dentre os incluídos, 1.893 (61,7%) eram professores e 1.174 (38,28%) professoras. O gênero masculino apresentou valores mais altos nas métricas de publicação (média de artigos: M = 67,0 versus F = 49,7; média de citações/ano: M = 53,9 versus F = 35,9) e índice H (M = 14,5 versus F = 12,4). Entre os 100 indivíduos com maior índice H (≥ 37), 83% são do gênero masculino. Na série histórica de citações, a curva masculina cresce mais rápido, abrindo uma diferença entre os grupos com afastamento confirmado pela projeção. DISCUSSÃO A produção científica na Universidade de São Paulo está sujeita a um viés de gênero. Dois terços do corpo docente são do gênero masculino, e as contratações das últimas décadas perpetuam esse padrão. A grande maioria dos membros com alto impacto no corpo docente é do sexo masculino. CONCLUSÃO Nossas análises sugerem que a Universidade de São Paulo não superará a desigualdade de gênero na produção científica sem uma ação afirmativa substancial. The male gender presented higher values in the publication metrics (average of articles: M = 67.0 versus F = 49.7; average of citations/year: M = 53.9 versus F = 35.9), and H-index (M = 14.5 versus F = 12.4). Among the 100 individuals with the highest H-index (≥ 37), 83% are male. The male curve grows faster in the historical series of citations, opening a difference between the groups whose separation is confirmed by the projection. DISCUSSION Scientific production at the Universidade de São Paulo is subject to a gender bias. Two-thirds of the faculty are male, and hiring over the past few decades perpetuates this pattern. The large majority of high impact faculty members are male. CONCLUSION Our analysis suggests that the Universidade de São Paulo will not overcome gender inequality in scientific production without substantive affirmative action. Development does not happen by chance but through choices that are affirmative, decisive, and long-term oriented.

Abstract: Anglophone authors who subscribe to the relational theory of society, like Charles Tilly, Mustafa Emirbayer, and Margaret Archer, have developed a critical reading of Durkheim’s work. This view asks the question, “How is it that society is, and how, from that possibility of being, can it be analyzed methodologically?” Although the critiques by the authors examined here defined the central points for constructing their own ontological-social position, this did not suppose a comprehensive critique with regard to his epistemological position. This article covers some critical points of these relational authors with regard to Durkheim’s work, contributing elements that seek to question his epistemological position and balance Emirbayer’s affirmation (2009) that the ontological proposal about the social should be accompanied by a resulting epistemological position.

Resumen: Autores de habla inglesa propios de la teoría relacional de la sociedad, como Charles Tilly, Mustafa Emirbayer y Margaret Archer, han generado una lectura crítica de la obra de Durkheim orientada hacia la respuesta de las preguntas: ¿cómo es que la sociedad es, y cómo, desde esa posibilidad de ser, puede analizarse metodológicamente? Aunque las críticas efectuadas por los autores analizados definieron los puntos centrales para construir su propio planteamiento ontológico-social, esto no supuso una crítica integral en torno a su posicionamiento epistemológico. En el presente artículo se abordan algunos puntos críticos de la lectura particular de los autores relacionales en torno a la obra durkheimiana, aportándose algunos elementos de crítica hacia su postura epistemológica, y buscando balancear la afirmación de Emirbayer (2009) en el sentido de que la propuesta ontológica de lo social debe suponer una postura epistemológica consecuente.

Objetivo : Este estudo avalia polimorfismos (SNPs) do NR3C1 na população brasileira, que possui origem étnica heterogênea. Materiais e métodos : SNPs do NR3C1 foram avaliados em brasileiros de ancestralidade caucasiana, africana ou japonesa (n = 380). Em um subgrupo (n = 40), os genótipos foram comparados à sensibilidade aos glicocorticoides (GC), previamente avaliada por cortisol plasmático (PF) e salivar (SF) após supressão com dexametasona (DEX), ensaio de afinidade do receptor ao GC (K d ) e inibição por DEX de 50% da proliferação de mononucleares estimulada por concanavalina-A (IC 50 ). Discriminação alélica de p.N363S (rs6195), p.ER22/23EK (rs6189-6190) e BclI (rs41423247) foi realizada por PCR em tempo real. Éxons 3 a 9 e transições éxon/íntron foram amplificados e sequenciados. Resultados : Frequências genotípicas (%) foram: rs6195 (n = 380; AA:96,6/AG:3,14/GG:0,26), rs6189-6190 (n = 264; GG:99,6/GA:0,4), rs41423247 (n = 264; CC:57,9/CG:34,1/GG:8,0), rs6188 (n = 155; GG:69,6/GT:25,7/TT:4,7), rs258751 (n = 150; CC:88,0/CT:10,7/TT:1,3), rs6196 (n = 176; TT:77,2/TC:20,4/CC:2,4), rs67300719 (n = 137; CC:99,3/CT:0,7), e rs72542757 (n = 137; CC:99,3/CG:0,7). Enquanto rs67300719 e rs72542757 foram exclusivos dos nipodescendentes, p.N363S e p.ER22/23EK estavam ausentes nesses indivíduos. p.ER22/23EK foi exclusivo dos descendentes de caucasianos. Equilíbrio de Hardy-Weinberg foi observado, exceto nos nipodescendentes para rs6188 e rs258751 e nos afrodescendentes para p.N363S. K d , IC 50 , PF ou SF basal ou após DEX foram semelhantes entre os genótipos. Entretanto, a dose média de DEX que suprimiu PF ou SF diferiu entre os genótipos BclI (P = 0,03), sendo maior nos carreadores GG (0,7 ± 0,2 mg) comparada aos GC (0,47 ± 0,2 mg) e CC (0,47 ± 0,1 mg). Conclusão : As frequências genotípicas dos SNPS de NR3C1 na população brasileira são semelhantes a outras populações. BclI foi associado à alteração da sensibilidade ao GC no eixo hipófise-adrenal.

Objective : The Brazilian population has heterogeneous ethnicity. No previous study evaluated NR3C1 polymorphisms in a Brazilian healthy population. Materials and methods : We assessed NR3C1 polymorphisms in Brazilians of Caucasian, African and Asian ancestry (n = 380). In a subgroup (n = 40), we compared the genotypes to glucocorticoid (GC) sensitivity, which was previously evaluated by plasma (PF) and salivary (SF) cortisol after dexamethasone (DEX) suppression tests, GC receptor binding affinity (K d ), and DEX-50% inhibition (IC 50 ) of concanavalin-A-stimulated mononuclear cell proliferation. p.N363S (rs6195), p.ER22/23EK (rs6189-6190), and BclI (rs41423247) allelic discrimination was performed by Real-Time PCR (Polymerase Chain Reaction). Exons 3 to 9 and exon/intron boundaries were amplified by PCR and sequenced. Results : Genotypic frequencies (%) were: rs6195 (n = 380; AA:96.6/AG:3.14/GG:0.26), rs6189-6190 (n = 264; GG:99.6/GA:0.4), rs41423247 (n = 264; CC:57.9/CG:34.1/GG:8.0), rs6188 (n = 155; GG:69.6/GT:25.7/TT:4.7), rs258751 (n = 150; CC:88.0/CT:10.7/TT:1.3), rs6196 (n = 176; TT:77.2/TC:20.4/CC:2.4), rs67300719 (n = 137; CC:99.3/CT:0.7), and rs72542757 (n = 137; CC:99.3/CG:0.7). The rs67300719 and rs72542757 were found only in Asian descendants, in whom p.N363S and p.ER22/23EK were absent. The p.ER22/23EK was observed exclusively in Caucasian descendants. Hardy-Weinberg equilibrium was observed, except in the Asian for rs6188 and rs258751, and in the African for p.N363S. The K d , IC 50 , baseline and after DEX PF or SF did not differ between genotype groups. However, the mean DEX dose that suppressed PF or SF differed among the BclI genotypes (P = 0.03). DEX dose was higher in GG- (0.7 ± 0.2 mg) compared to GC- (0.47 ± 0.2 mg) and CC-carriers (0.47 ± 0.1 mg). Conclusion : The genotypic frequencies of NR3C1 polymorphisms in Brazilians are similar to worldwide populations. Additionally, the BclI polymorphism was associated with altered pituitary-adrenal axis GC sensitivity.

OBJETIVO: Estimar a probabilidade pré-teste do diagnóstico de síndrome de Cushing (SC) por meio de julgamento clínico utilizando abordagem Bayesiana. MATERIAIS E MÉTODOS: Médicos responderam a três perguntas, em sete congressos de endocrinologia. Após obtenção da história clínica/exame físico, sem exames laboratoriais, apenas com base em sua experiência pessoal, qual a probabilidade de diagnosticar SC?; Há quanto tempo você pratica endocrinologia?; Onde você trabalha? Uma regressão beta Bayesiana, utilizando o software WinBugs, foi empregada. RESULTADOS: Foram obtidos 294 questionários. A estimativa Bayesiana da probabilidade média de diagnosticar SC foi 51,6% (IC 95%: 48,7-54,3). Houve relação direta entre probabilidade de diagnosticar SC e experiência da prática endócrina, porém não com o local de trabalho. CONCLUSÃO: A probabilidade pré-teste do diagnóstico de SC foi estimada utilizando uma metodologia Bayesiana. Embora a probabilidade pré-teste possa ser dependente do contexto, a experiência de anos de prática pode auxiliar no diagnóstico intuitivo da CS. Arq Bras Endocrinol Metab. 2012;56(9):633-7

OBJECTIVE: To estimate the pretest probability of Cushing's syndrome (CS) diagnosis by a Bayesian approach using intuitive clinical judgment. MATERIALS AND METHODS: Physicians were requested, in seven endocrinology meetings, to answer three questions: "Based on your personal expertise, after obtaining clinical history and physical examination, without using laboratorial tests, what is your probability of diagnosing Cushing's Syndrome?"; "For how long have you been practicing Endocrinology?"; and "Where do you work?". A Bayesian beta regression, using the WinBugs software was employed. RESULTS: We obtained 294 questionnaires. The mean pretest probability of CS diagnosis was 51.6% (95%CI: 48.7-54.3). The probability was directly related to experience in endocrinology, but not with the place of work. CONCLUSION: Pretest probability of CS diagnosis was estimated using a Bayesian methodology. Although pretest likelihood can be context-dependent, experience based on years of practice may help the practitioner to diagnosis CS. Arq Bras Endocrinol Metab. 2012;56(9):633-7

Apresentamos os dados clínicos e moleculares de dois pacientes com acromegalia tratados com octreotide LAR após cirurgia não curativa, com diferentes respostas a essa terapia medicamentosa. As expressões do receptor de somatostatina tipo 2 e 5 (SSTR2 e SSTR5) e da proteína de interação com o receptor aril hidrocarbono (AIP) foram analisadas por qPCR. Em ambos os casos, foi encontrada uma expressão elevada de SSTR2 e baixa do SSTR5. No entanto, o controle da doença foi obtido após tratamento com octreotide LAR em apenas um dos pacientes. Quando analisamos a expressão do AIP em ambos os casos, o paciente cuja doença foi controlada após a terapia medicamentosa apresentou uma expressão duas vezes maior do que a do paciente não controlado com o tratamento. Conclui-se que esses dois casos ilustram que, embora os análogos de somatostatina atualmente disponíveis se liguem preferencialmente ao SSTR2, alguns pacientes não respondem ao tratamento, apesar de uma elevada expressão desse receptor. Isso poderia ser explicado por alterações nas vias de sinalização pós-receptor, incluindo o envolvimento recentemente descrito da AIP. Arq Bras Endocrinol Metab. 2012;56(8):501-6

We present here the clinical and molecular data of two patients with acromegaly treated with octreotide LAR after non-curative surgery, and who presented different responses to therapy. Somatostatin receptor type 2 and 5 (SSTR2 and SSTR5), and aryl hydrocarbon receptor-interacting protein (AIP) expression levels were analyzed by qPCR. In both cases, high SSTR2 and low SSTR5 expression levels were detected; however, only one of the patients achieved disease control after octreotide LAR therapy. When we analyzed AIP expression levels of both cases, the patient whose disease was controlled after therapy exhibited AIP expression levels that were two times higher than the patient whose disease was still active. These two cases illustrate that, although the currently available somatostatin analogs bind preferentially to SSTR2, some patients are not responsive to therapy despite high expression of this receptor. This difference could be explained by differences in post-receptor signaling pathways, including the recently described involvement of AIP. Arq Bras Endocrinol Metab. 2012;56(8):501-6

OBJETIVO: Avaliar a modulação do eixo hipotálamo-hipófise-adrenal (HHA) sobre a secreção de prolactina após adrenalectomia (ADX). MATERIAIS E MÉTODOS: Quantificamos por RIE corticosterona, ACTH e prolactina plasmáticos em ratos após curtos (3 horas e 1 dia) e longos (3, 7 e 14 dias) períodos de ADX bilateral. RESULTADOS: Animais ADX mostraram níveis indetectáveis de corticosterona. As concentrações plasmáticas de ACTH apresentaram resposta trifásica: aumento transitório (3h), diminuição (1d) e novo aumento após longos períodos de ADX. Animais Sham mostraram aumento de corticosterona/ACTH após três horas de cirurgia, diminuindo posteriormente aos níveis basais. As concentrações plasmáticas de prolactina não se alteraram após ADX. CONCLUSÃO: Existem diferentes pontos de equilíbrio do eixo HHA após a remoção da retroalimentação negativa exercida pelos glicocorticoides. A secreção de prolactina não se alterou após curtos/longos períodos de ADX, sugerindo que os neurônios peptidérgicos essenciais para a liberação de prolactina não estão ativados durante os diferentes períodos de ADX.

OBJECTIVE: To evaluate the modulation of the hypothalamus-pituitary-adrenal axis (HPA) on prolactin secretion in rats after adrenalectomy (ADX). MATERIALS AND METHODS: Plasma corticosterone, ACTH, and prolactin concentrations were measured by radioimmunoassay in rats after bilateral ADX in the short- (3 hours and 1day) and long-term (3, 7, and 14 days). RESULTS: Animals that underwent ADX showed undetectable corticosterone levels and a triphasic ACTH response with a transient increase (3h), a decrease (1d), and further increase in the long-term after ADX. Sham animals showed a marked increase in corticosterone and ACTH levels three hours after surgery, with a decrease to basal levels thereafter. Plasma prolactin levels were not changed after ADX. CONCLUSION: There are different points of equilibrium in the HPA axis after the glucocorticoid negative feedback is removed. Prolactin plasma secretion is not altered in the short or long- term after ADX, suggesting that the peptidergic neurons essential for prolactin release are not activated after ADX.

OBJECTIVES: Idiopathic central precocious puberty and its postponement with a (gonadotropin-releasing hormone) GnRH agonist are complex conditions, the final effects of which on bone mass are difficult to define. We evaluated bone mass, body composition, and bone remodeling in two groups of girls with idiopathic central precocious puberty, namely one group that was assessed at diagnosis and a second group that was assessed three years after GnRH agonist treatment. METHODS: The precocious puberty diagnosis and precocious puberty treatment groups consisted of 12 girls matched for age and weight to corresponding control groups of 12 (CD) and 14 (CT) girls, respectively. Bone mineral density and body composition were assessed by dual X-ray absorptiometry. Lumbar spine bone mineral density was estimated after correction for bone age and the mathematical calculation of volumetric bone mineral density. CONEP: CAAE-0311.0.004.000-06. RESULTS: Lumbar spine bone mineral density was slightly increased in individuals diagnosed with precocious puberty compared with controls; however, after correction for bone age, this tendency disappeared (CD = -0.74 + 0.9 vs. precocious puberty diagnosis = -1.73 + 1.2). The bone mineral density values of girls in the precocious puberty treatment group did not differ from those observed in the CT group. CONCLUSION: There is an increase in bone mineral density in girls diagnosed with idiopathic central precocious puberty. Our data indicate that the increase in bone mineral density in girls with idiopathic central precocious puberty is insufficient to compensate for the marked advancement in bone age observed at diagnosis. GnRH agonist treatment seems to have no detrimental effect on bone mineral density.

Tumores do córtex adrenal (TCA) são mais frequentes em crianças, mas podem ocorrer em qualquer faixa etária. São classificados como funcionantes, não funcionantes (predominam no adulto), e mistos. O diagnóstico é baseado na avaliação clínica, hormonal e exames de imagem. Em crianças, o método de escolha para diferenciar entre benigno ou maligno é a classificação baseada no estadiamento do tumor. Alguns marcadores moleculares merecem destaque: além de mutações inativadoras no gene supressor tumoral TP53, há evidências de envolvimento do IGF2 em 90% de TAC malignos, e mutações no éxon 3 do gene CTNNB1 foram encontradas em 6% dos TAC pediátricos. Além disso, microRNAs podem atuar como reguladores negativos da expressão gênica e participar da tumorigênese adrenocortical. Métodos para análise da expressão gênica permitem identificar TCA com prognóstico bom ou ruim, e espera-se que esses estudos possam facilitar o desenvolvimento de drogas para tratar pacientes de acordo com as vias de sinalização específicas que estiverem alteradas.

Adrenocortical tumors (ACT) are more frequent during childhood, but they can appear at any age. ACTs can be classified in functioning, nonfunctioning (mainly observed in adults) and mixed. The diagnosis is based on clinical, biochemical findings and imaging. In children, in order to classify ACT as benign or malignant, tumor staging classification is recommended. Regarding molecular markers some studies should be taken into account: besides TP53 mutations, previous studies have also provided evidences of IGF2 involvement in 90% of the malignant ACT. Mutations altering exon 3 of CTNNB1 gene have been found in 6% of childhood ACTs. In addition, microRNAs can act as negative regulators of gene expression by targeting mRNA controlling cell growth, differentiation and apoptosis and have been implicated in adrenal tumorigenesis. High-throughput methods to analyze genome-wide expression have been developed over the last decade and identified a subset of tumors with good or poor prognosis. In the future, these studies can provide the basis of specific drug development, which can treat patients according to specific altered signaling pathway.

O objetivo deste trabalho foi descrever um caso de mielolipoma gigante associado à hiperplasia adrenal congênita (HAC) por deficiência da 21-hidroxilase (21OH). Paciente do sexo masculino, 57 anos de idade, encaminhado por achado ultrassonográfico de massa adrenal esquerda. Investigação bioquímica revelou hiperandrogenismo e exames de imagem revelaram grande lesão sólida em adrenal esquerda de aspecto heterogêneo, entremeada de tecido gorduroso, compatível com diagnóstico de mielolipoma, e um nódulo de 1,5 cm na adrenal direita. Os achados bioquímicos sugeriam o diagnóstico de carcinoma adrenocortical, indicando cirurgia para retirada da massa adrenal esquerda. O anatomopatológico confirmou mielolipoma e hiperplasia multinodular do córtex adrenal. A investigação subsequente diagnosticou HAC por deficiência da 21OH. Concluiu-se que a HAC tem sido descrita em associação com tumores adrenocorticais. Apesar de raro, o mielolipoma associado à HAC deve ser incluído nas possibilidades diagnósticas de massa adrenal. Adicionalmente, a HAC deve ser sempre afastada nos casos de massa adrenal de achado incidental, evitando cirurgias desnecessárias.

The objective of this study was to describe a case of giant myelolipoma associated with undiagnosed congenital adrenal hyperplasia (CAH) due to 21-hydroxylase (21OH) deficiency. Five seven year-old male patient referred with abdominal ultrasound revealing a left adrenal mass. Biochemical investigation revealed hyperandrogenism and imaging exams characterized a large heterogeneous left adrenal mass with interweaving free fat tissue, compatible with the diagnosis of myelolipoma, and a 1.5 cm nodule in the right adrenal gland. Biochemical correlation has brought concerns about differential diagnosis with adrenocortical carcinoma, and surgical excision of the left adrenal mass was indicated. Anatomopathologic findings revealed a myelolipoma and multinodular hyperplasic adrenocortex. Further investigation resulted in the diagnosis of CAH due to 21OH deficiency. Concluded that CAH has been shown to be associated with adrenocortical tumors. Although rare, myelolipoma associated with CAH should be included in the differential diagnosis of adrenal gland masses. Moreover, CAH should always be ruled out in incidentally detected adrenal masses to avoid unnecessary surgical procedures.
