Response of a bainitic steel to plasma nitriding in terms of core preservation

Abstract New processing routes and alternatives have been studied in the manufacture of automotive parts aiming for lower energy consumption and less environmental impact. In this context, continuous cooling bainitic steels have emerged to address this demand by eliminating the quenching and tempering treatments. In substitution to heat treatments to improve wear resistance on the surface, plasma nitriding is a candidate. However, the effects on the core of bainitic microstructures over long time processes at subcritical temperatures are limited and should be further investigated. Especially in case of unexpected effects on the core of steels depending on time and temperature, compromising performance in service. The aim of the present study was to evaluate the effects of two plasma nitriding conditions at 500 ºC on the core of DIN 18MnCrSiMo6-4 continuous cooling bainitic steel, for 20 h and 30 h of treatment. For this purpose, characterization techniques, such as optical microscopy and SEM were applied to evaluate microstructural changes on the core followed by the Vickers hardness test to estimate possible softening effects. The research found a decrease of austenite fraction in the nitride conditions in comparison to the initial microstructure of the bainitic steel, a reduction of dislocation density of ferrite measured by Williamson–Smallman relation and the hardness dropped about 10% after 20 h and 20% after 30 h of treatment. The conclusion pointed out alteration on the core for the plasma nitriding treatments for long time cycles at a temperature of 500 °C, similarly to effects observed in tempering treatments.

Abstract An aneurysm of the anterior tibial artery is always a challenge because it does not provoke symptoms and when a pulsating mass is detected, the aneurysm diameter is already enlarged and there is a greater likelihood of rupture or thrombosis. We report the case of a healthy, 53-year-old, female patient diagnosed with an aneurysm of the anterior tibial artery at the transition to the dorsalis pedis artery, which was identified during ultrasonography performed to investigate plantar fasciitis. The aneurysm was treated by coil embolization and there were no complications.
Resumo O aneurisma de artéria tibial anterior é sempre um desafio, pois não apresenta sintomas e, quando há a percepção de uma massa pulsátil, o aneurisma já apresenta diâmetro aumentado, com uma maior chance de romper ou de trombosar. Relatamos o caso de uma paciente do sexo feminino, 53 anos, hígida, que apresentou o diagnóstico de aneurisma na artéria tibial anterior na transição com a artéria dorsal do pé, sendo identificado a partir de ultrassonografia realizada para investigação de fascite plantar. O aneurisma foi tratado através de embolização com molas e não apresentou complicações.

Fenofibrate attenuates renal lipotoxicity in uninephrectomized mice with high-fat diet-induced obesity

Abstract Introduction: The objective of this study was to investigate the role of fenofibrate, a peroxisome proliferator-activated receptor-α agonist, in obesity-induced kidney damage (lipotoxicity) in mice with uninephrectomy. Methods: C57BL/6 mice underwent uninephrectomy and sham surgeries and were fed normocaloric or high-fat diets. After 10 weeks, obese mice were administered 0.02% fenofibrate for 10 weeks. Kidney function and morphology were evaluated, as well as levels of inflammatory and fibrotic mediators and lipid metabolism markers. Results: High-fat diet-fed mice developed characteristic obesity and hyperlipidemia, with subsequent renal lipid accumulation and damage, including mesangial expansion, interstitial fibrosis, inflammation, and proteinuria. These changes were greater in obese uninephrectomy mice than in obese sham mice. Fenofibrate treatment prevented hyperlipidemia and glomerular lesions, lowered lipid accumulation, ameliorated renal dysfunction, and attenuated inflammation and renal fibrosis. Furthermore, fenofibrate treatment downregulated renal tissue expression of plasminogen activator inhibitor-1, monocyte chemoattractant protein-1, and local expression of fibroblast growth factor-21. Conclusion: Peroxisome proliferator-activated receptor-α activation by fenofibrate, with subsequent lipolysis, attenuated glomerular and tubulointerstitial lesions induced by renal lipotoxicity, thus protecting the kidneys of uninephrectomy mice from obesity-induced lesions. The study findings suggest a pathway in the pharmacological action of fenofibrate, providing insight into the mechanisms involved in kidney damage caused by obesity in kidney donors.
Resumo Introdução: O objetivo deste estudo foi investigar o papel do fenofibrato, um agonista do receptor ativado por proliferadores de peroxissoma-α, na lesão renal induzida por obesidade (lipotoxicidade) em camundongos submetidos à uninefrectomia. Métodos: Camundongos C57BL/6 foram submetidos a uninefrectomia e cirurgias simuladas (sham) e alimentados com dietas normocalóricas ou rica em gordura. Após 10 semanas, os camundongos obesos receberam fenofibrato a 0,02% por 10 semanas. Avaliamos função e morfologia renal, bem como níveis de mediadores inflamatórios e fibróticos e marcadores do metabolismo lipídico. Resultados: Camundongos alimentados com dieta rica em gordura desenvolveram obesidade e hiperlipidemia características, com subsequente acúmulo de lipídios e danos renais, incluindo expansão mesangial, fibrose intersticial, inflamação e proteinúria. Essas alterações foram maiores em camundongos obesos uninefrectomizados do que em camundongos obesos sham. O tratamento com fenofibrato preveniu hiperlipidemia e lesões glomerulares, reduziu o acúmulo de lipídios, melhorou a disfunção renal e atenuou a inflamação e fibrose renal. Além disso, o tratamento com fenofibrato reduziu a expressão no tecido renal do inibidor do ativador do plasminogênio-1, da proteína quimiotática de monócitos-1 e a expressão local do fator de crescimento de fibroblastos-21. Conclusão: A ativação do receptor ativado por proliferadores de peroxissoma-α pelo fenofibrato, com subsequente lipólise, atenuou lesões glomerulares e tubulointersticiais induzidas por lipotoxicidade renal, protegendo, assim, os rins de camundongos uninefrectomizados contra lesões induzidas por obesidade. Os achados do estudo sugerem uma via na ação farmacológica do fenofibrato, fornecendo insights sobre os mecanismos envolvidos no dano renal causado pela obesidade em doadores renais.

Abstract: Despite all the knowledge generated about grafting techniques in plant species, there is little research that evaluates the results found. The objective of this work was to analyze, through bibliometric techniques, the information published in scientific articles in which a grafting technique was evaluated, to identify analysis methodologies, types of grafts, plant species, and the application of the results found. The keyword used for the search of scientific articles was ‘grafting’, and publications up to December 2022 were considered. From 1994 to 2022, 293 scientific articles were identified, the frequency of their publication showed an exponential trend (R2 = 0,54). It was found that variables associated with rootstock, graft and survival of the newly formed plant are evaluated, with an increasing use of multivariate techniques in order to detect the most significant variables. Eleven grafting techniques were documented: bark grafting, cleft grafting, splice grafting, whip and tongue grafting, budding, in vitro grafting, micrografting, T-budding, side-veneer grafting, chip budding, and tube grafting. Fruit (citrus), horticultural (tomato), forest (walnut), and ornamental (rose) crops were the most widely analyzed. However, 63,47% of the scientific articles were limited to descriptive analyses on success rate, compatibility and survival rate of vegetative materials. This provides the opportunity to conduct research on genetic improvement processes in the search for agronomic traits of interest (fruit and growth), or on genetic diversity aimed to improve productivity and/or conservation.
Resumen: A pesar de todo el conocimiento generado en torno a las técnicas de injerto en especies vegetales, existe poca investigación que evalúe los resultados encontrados. El objetivo de este trabajo fue analizar, mediante técnicas bibliométricas, la información publicada en artículos científicos donde se evalúo alguna técnica de injerto, para identificar metodologías de análisis, tipos de injertos, especies vegetales y la aplicación de los resultados encontrados. La palabra clave utilizada para la búsqueda de los artículos científicos fue ‘grafting’, y se consideraron publicaciones hasta diciembre de 2022. De 1994 a 2022 se identificaron 293 artículos científicos, la frecuencia de su publicación mostró una tendencia exponencial (R2 = 0,54). Se encontró que se evalúan variables asociadas al portainjerto, injerto y sobrevivencia de la nueva planta formada, que son evaluadas cada vez con mayor frecuencia con técnicas multivariadas por el interés de detectar las variables más significativas a partir de un número elevado de variables analizadas. Se documentaron 11 técnicas de injerto: púa, hendidura, empalme, inglés, yema, in vitro, microinjerto, en T, chapado, chip y entube. Las especies de mayor análisis correspondieron a frutícolas (cítricos), hortícolas (tomate), forestal (nogal), y ornato (rosa). Sin embargo, el 63,47% de los artículos científicos se limitaron a hacer análisis descriptivos sobre prendimiento, compatibilidad y sobrevivencia de los materiales vegetativos. Lo anterior, deja un área de oportunidad para desarrollar investigación sobre los procesos de mejora genética en la búsqueda de caracteres agronómicos de interés (fruto y crecimiento), o bien en la diversidad genética orientada a la productividad y/o conservación.

Quantification and mapping of erosion in the Sergipe–Alagoas Basin using sonic logs and vitrinite reflectance data

Abstract Net erosion recorded in lithological columns of hundreds of wells from the proximal domain of the Sergipe–Alagoas Basin was quantified through palaeoburial indicators: transit time sonic logs (DT) and vitrinite reflectance profiles (VRe). The preferred determinations with DT logs of pelitic rocks compound a large database and are less subject to variations in sampling and laboratory procedures. The sonic log profiles of 485 wells have been used for the calculation of the net erosion grid, while just 60 VRe profiles have been used for this purpose. Net erosion varies typically from 100 to 700 m in the Sergipe Sub-basin to the south, probably as a result of isostatic compensation to the sedimentary loads of the post-rift stratigraphic sequences. The Alagoas Sub-basin to the north stands as the most eroded. The average net erosion in the Alagoas SB is around 750 m but reaches more than 1,200 m in the north. The main cause of the erosion of the syn-rift strata in the Alagoas SB is probably the long-time sub-aerial exposure of uplifted rift flanks. Late Albian tectonic inversion, focused on northern Alagoas SB, enhanced erosion as evidenced in seismic sections, especially those parallel to the coast.

Bioactive TiO2 Fibers Prepared by Solution Blow Spinning: A Promising Approach for Microbial Control

PEO/TiP fibers were obtained using the Solution Blow Spinning (SBS) apparatus and heat treated to produce TiO2 fibers. The morphological and structural characteristics were assessed using SEM and X-ray diffraction. The fibers, with a thickness of 12 μm, showed a change in crystalline structure with heat treatment. At temperatures as low as 800 °C, only the anatase phase was identified, while at 900 °C, both anatase and rutile phases coexisted. The addition of TiP to the polymer matrix reduced the initial breakdown temperature, and the DSC curves showed exothermic peaks due to the amorphous phase transition to TiO2/anatase. The fibers' photocatalytic capacity was tested, revealing that TiO2-fibers in the anatase phase achieved 97% degradation of Rhodamine-B dye in 40 minutes. The study found that the biocide efficacy of TiO2-fibers depends on their heat treatment. Fibers with anatase/rutile or pure rutile phases did not show significant efficiency. However, fibers treated at 600°C with pure anatase phase were more effective in eliminating E. coli and total coliforms. Finally, we can state that the TiO2 fibers obtained in this work using the SBS technique can be used to produce filters to purify water contaminated by pathogens dangerous to human health or even to purify the air.

ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others.

Objective: To investigate the association between depressive symptoms and social support among a representative sample of the Brazilian population aged 50 years or older. Methods: Cross-sectional study of 8,074 participants of the second wave of the Brazilian Longitudinal Study on Aging (ELSI-Brasil). Depressive symptoms were screened with the Center for Epidemiologic Studies Depression Scale (CES-D8) instrument, and social support was investigated in its structural and functional dimensions. Sociodemographic variables and health conditions were considered for adjustment in investigating the association between social support and depressive symptoms using the Poisson regression model. Results: The prevalence of depressive symptoms was estimated at 19.1% (95%CI 16.7-21.7). In the analysis adjusted for possible confounding factors, depressive symptoms were independently associated with negative social support in the items “not married” (prevalence ratio [PR] = 1.24; 95%CI 1.07-1.44), “not having someone to trust” (PR = 1.31; 95%CI 1.10-1.56) and “not having someone to borrow money or an object from, in case of need” (PR = 1.46; 95%CI 1.21-1.75). Conclusion: The present findings highlight the importance of social relations in determining the presence of depressive symptoms and reinforce the need to implement public policies aimed at strengthening social networks to minimize this public health problem.

ABSTRACT Introduction: Blood transfusion is one of the most common medical practices worldwide. However, current scientific literature has shown that the immunomodulatory effects of blood transfusion are associated with an increased likelihood of infection, prolonged hospitalization, and morbimortality. Also, it means high costs for healthcare systems. Methods: In this context, acknowledging that blood transfusions are essentially heterologous cell transplantations, the use of therapeutic options has gained strength and is collectively known as the patient blood management (PBM) program. PBM is an approach based on three main pillars: (1) treating anemias and coagulopathies in an optimized manner, especially in the preoperative period; (2) optimizing perioperative hemostasis and the use of blood recovery systems to avoid the loss of the patient's blood; (3) anemia tolerance, with improved oxygen delivery and reduced oxygen demand, particularly in the postoperative period. Results: Current scientific evidence supports the effectiveness of PBM by reducing the need for blood transfusions, decreasing associated complications, and promoting more efficient and safer blood management. Thus, PBM not only improves clinical outcomes for patients but also contributes to the economic sustainability of healthcare systems. Conclusion: The aim of this review was to summarize PBM strategies in a comprehensive, evidence-based approach through a systematic and structured model for PBM implementation in tertiary hospitals. The recommendations proposed herein are from researchers and experts of a high-complexity university hospital in the network of the Sistema Único de Saúde, presenting itself as a strategy that can be followed as a guideline for PBM implementation in other settings.

Abstract The diabetic foot interacts with anatomical, vascular, and neurological factors that challenge clinical practice. This study aimed to compile the primary scientific evidence based on a review of the main guidelines, in addition to articles published on the Embase, Lilacs, and PubMed platforms. The European Society of Cardiology system was used to develop recommendation classes and levels of evidence. The themes were divided into six chapters (Chapter 1 - Prevention of foot ulcers in people with diabetes; Chapter 2 - Pressure relief from foot ulcers in people with diabetes; Chapter 3 -Classifications of diabetic foot ulcers; Chapter 4 - Foot and peripheral artery disease; Chapter 5 - Infection and the diabetic foot; Chapter 6 - Charcot's neuroarthropathy). This version of the Diabetic Foot Guidelines presents essential recommendations for the prevention, diagnosis, treatment, and follow-up of patients with diabetic foot, offering an objective guide for medical practice.
Resumo O pé diabético corresponde a uma interação entre fatores anatômicos, vasculares e neurológicos que representam um desafio na prática clínica. O objetivo deste trabalho foi compilar as principais evidências científicas com base em uma revisão das principais diretrizes, além de artigos publicados nas plataformas Embase, Lilacs e PubMed. O sistema da Sociedade Européia de Cardiologia foi utilizado para desenvolver classes de recomendação e níveis de evidência. Os temas foram divididos em seis capítulos (Capítulo 1-Prevenção de úlceras nos pés de pessoas com diabetes; Capítulo 2-Alívio da pressão de úlceras nos pés de pessoas com diabetes; Capítulo 3-Classificações das úlceras do pé diabético; Capítulo 4-Pé diabético e a doença arterial periférica; Capítulo 5-Infecção e o pé diabético; Capítulo 6-Neuroartropatia de Charcot). A versão atual das Diretrizes sobre pé diabético apresenta importantes recomendações para prevenção, diagnóstico, tratamento e seguimento dos pacientes com pé diabético, oferecendo um guia objetivo para prática médica.

Abstract Objective To measure the average time for the diagnosis and for the therapeutic prescription of Continuous Positive Airway Pressure (CPAP) at a hospital in Botucatu Medical School - State University São Paulo, UNESP. Method A retrospective observational study was carried out by collecting data from the electronic medical records of patients over 18-years of age, who had a diagnostic polysomnography testing scheduled between January and December 2017. Result Of the 347 patients eligible for the study, 94 (27.1%) missed follow-up and 103 (29.7%) had a referral for CPAP use. Until February 2021, only 37 (35.9%) of these patients had already acquired and were using the device, the remaining 66 (64.1%) were waiting or gave up the therapy. The mean value of the waiting time interval between the referral of the diagnostic test and its performance was equivalent to 197 days (6.5 months). The mean time between diagnostic polysomnography and CPAP prescription was 440-days (14.5-months), with a total mean time of 624 days (21-months). Conclusion As in other services, the diagnostic-therapeutic flow proved to be highly inefficient, with a long waiting period, difficult access to treatment and a high dropout rate. These findings highlight the need to establish new patient-centered strategies with measures that speed up the flow and facilitate access to CPAP, in order to reduce the morbidity and mortality associated with this condition. Resumen Antecedentes: Las poblaciones de Magnolia pugana están disminuyendo, asimismo la precipitación disminuirá 10 % y la temperatura aumentará (2-4 °C) en su área de distribución, de acuerdo al escenario de cambio climático más severo, lo que puede comprometer su germinación. Hipótesis: La interacción del aumento de temperatura y disminución del potencial hídrico reduce y retrasa la germinación de Magnolia pugana. Especie de estudio: Magnolia pugana, endémica del occidente de México, en peligro de extinción. Sitio y año de estudio: Sur de Zacatecas y centro de Jalisco, México. Abril, 2019. Materiales y métodos: Se evaluó la viabilidad de semillas con la prueba de tetrazolio. Se analizó el efecto de temperatura (24, 28 y 37 °C), potencial hídrico (con PEG 8000) (0, -0.3, -0.6, -0.9, -1.2 MPa) y su interacción sobre porcentaje y tiempo medio de germinación, con ANOVA factoriales. Resultados: La viabilidad fue 80 %. Las interacciones entre temperatura y potencial hídrico sobre porcentaje y tiempo medio de germinación fueron significativas. No hubo germinación a 37 °C. El porcentaje de germinación más alto (78 %) se produjo a 24 °C y -0.3 MPa, el menor (3 %) a 28 °C y -0.6 MPa. A 24 °C y 0 MPa germinaron en menor tiempo (23.9 ± 1.5 días), a 28 °C y -0.9 MPa se retrasó 1.5 veces. Conclusiones: La interacción entre bajo potencial hídrico y aumento de temperatura redujo la germinabilidad de Magnolia pugana, condiciones previstas por futuros escenarios de cambio climático, con esto su vulnerabilidad a la extinción podría aumentar.
Abstract Background: Magnolia pugana populations are declining, in addition, it is estimated that precipitation will decrease by up to 10 % and temperatures will increase (2-4 °C) under the most severe climate change scenario, which may affect germination. Hypotheses: The interaction of increased temperature and decreased water potential reduces and delays the germination of Magnolia pugana seeds. Studied species: Magnolia pugana, an endangered species endemic to western Mexico. Study site and dates: Southern Zacatecas and central Jalisco, Mexico. April 2019. Materials and methods: Seed viability was evaluated with tetrazolium test. In addition, the effects of temperature (24, 28, and 37 °C), water potential with PEG 8000 (0, -0.3, -0.6, -0.9, -1.2 MPa) and its interaction on the percentage and mean germination time were analyzed with factorial ANOVA tests. Results: Viability was 80 %. The interactions between temperature and water potential on germination percentage and mean germination time were significant. There was no germination at 37 °C. The highest germination (78 %) was at 24 °C and -0.3 MPa, while the lowest (3 %) was at 28 °C and -0.6 MPa. The control treatment (24 °C and 0 MPa) facilitated germination in a shorter time (23.9 ± 1.5 days). At 28 °C and -0.9 MPa, the mean germination time was slowed by 1.5 times. Conclusions: Magnolia pugana germinability was reduced by the interaction between low water potentials and increased temperature, conditions predicted by future climate change scenarios, therefore its vulnerability to extinction could increase.

ABSTRACT Objective: To report outcomes from the largest multicenter series of penile cancer patients undergoing video endoscopic inguinal lymphadenectomy (VEIL). Materials and Methods: Retrospective multicenter analysis. Authors of 21 centers from the Penile Cancer Collaborative Coalition-Latin America (PeC-LA) were included. All centers performed the procedure following the same previously described standardized technique. Inclusion criteria included penile cancer patients with no palpable lymph nodes and intermediate/high-risk disease and those with non-fixed palpable lymph nodes less than 4 cm in diameter. Categorical variables are shown as percentages and frequencies whereas continuous variables as mean and range. Results: From 2006 to 2020, 210 VEIL procedures were performed in 105 patients. Mean age was 58 (45-68) years old. Mean operative time was 90 minutes (60-120). Mean lymph node yield was 10 nodes (6-16). Complication rate was 15.7%, including severe complications in 1.9% of procedures. Lymphatic and skin complications were noted in 8.6 and 4.8% of patients, respectively. Histopathological analysis revealed lymph node involvement in 26.7% of patients with non-palpable nodes. Inguinal recurrence was observed in 2.8% of patients. 10y- overall survival was 74.2% and 10-y cancer specific survival was 84.8%. CSS for pN0, pN1, pN2 and pN3 were 100%, 82.4%, 72.7% and 9.1%, respectively. Conclusion: VEIL seems to offer appropriate long term oncological control with minimal morbidity. In the absence of non-invasive stratification measures such as dynamic sentinel node biopsy, VEIL emerged as the alternative for the management of non-bulky lymph nodes in penile cancer.

Abstract Objectives: To review and provide evidence-based recommendations for the diagnosis and treatment of otosclerosis. Methods: Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on otosclerosis were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results: The topics were divided into 2 parts: 1) Diagnosis – audiologic and radiologic; 2) Treatment – hearing AIDS, pharmacological therapy, stapes surgery, and implantable devices - bone-anchored devices, active middle ear implants, and Cochlear Implants (CI). Conclusions: The pathophysiology of otosclerosis has not yet been fully elucidated, but environmental factors and unidentified genes are likely to play a significant role in it. Women with otosclerosis are not at increased risk of worsening clinical condition due to the use of contraceptives or during pregnancy. Drug treatment has shown little benefit. If the patient does not want to undergo stapedotomy, the use of hearing aids is well indicated. Implantable systems should be indicated only in rare cases, and the CI should be indicated in cases of profound deafness.

Resumo Introdução o pé diabético é uma das complicações mais sérias do diabetes mellitus. Cerca de 50% das amputações não traumáticas ocorrem nesses pacientes. Além disso, é um importante problema de saúde pública por ser um distúrbio metabólico crônico e complexo que se caracteriza pelo comprometimento do metabolismo da glicose associada a outras complicações em órgãos essenciais para manutenção vital. Objetivo avaliar a sensibilidade e especificidade para neuropatia diabética da autoavaliação e do exame físico de Michigan nos diabéticos tipo 1 e tipo 2. Método trata-se de um estudo transversal. Foi utilizada a classificação “Michigan Neuropathy Screening Instruments” para avaliação do grau de neuropatia periférica, em que os participantes responderam ao questionário e foram avaliados quanto a presença de lesões nos pés. Todos os participantes foram estratificados quanto ao risco de desenvolver úlcera nos pés de acordo com o protocolo do IWGDF. Resultados a amostra contou com 200 participantes. Quanto à classificação do IWGDF, 23 pacientes foram classificados como risco moderado (11,50%) e 61 como alto risco para o desenvolvimento de úlceras nos pés (30,50%). Utilizando-se um corte de 2,5 na pontuação do exame físico para diagnosticar a neuropatia, foi obtida uma sensibilidade de 97,62% e uma especificidade de 47,41%. Utilizando-se uma pontuação maior ou igual a 6 na autoavaliação para o diagnóstico de neuropatia, foi obtida uma sensibilidade de 50,00% e uma especificidade de 94,83%. Conclusão a associação do exame físico de Michigan (alta sensibilidade) com a autoavaliação (alta especificidade) tem melhor acurácia para o diagnóstico de neuropatia diabética.
Abstract Introduction the diabetic foot is one of the most serious complications of diabetes mellitus. About 50% of non-traumatic amputations occur in these patients. In addition, it is an important public health problem and constitutes a chronic and complex metabolic disorder that is characterized by impaired metabolism of glucose and other complications in essential organs for the maintenance of life. Objective to evaluate the sensitivity and specificity of diabetic neuropathy using the Michigan self-assessment and physical examination in type 1 and type 2 diabetics. Methods this is a cross-sectional study. The “Michigan Neuropathy Screening Instruments” classification was used to assess the degree of peripheral neuropathy, in which participants answered the questionnaire and were evaluated for the presence of foot lesions. All participants were stratified by the risk of developing foot ulcers according to the IWGDF protocol. Results the sample had 200 participants. Regarding the IWGDF classification, 23 patients were classified as moderate risk (11.50%) and 61 as high risk for developing foot ulcers (30.50%). Using a cutoff of 2.5 on the physical examination score to diagnose neuropathy, a sensitivity of 97.62% and a specificity of 47.41% were obtained. Using a score greater than or equal to 6 in the self-assessment for the diagnosis of neuropathy, a sensitivity of 50.00% and a specificity of 94.83% were found. Conclusion the association of the Michigan physical examination (high sensitivity) with self-assessment (high specificity) increases the accuracy for the diagnosis of diabetic neuropathy.
