Abstract Objective: To describe differences in Case Fatality Rate (CFR) for Covid-19 among healthcare subsystems in Mexico City between March and December 2020. Materials and methods: This is a retrospective secondary data analysis from the National Epidemiological Surveillance System data of Covid-19 cases. Information about health provider institutions was retrieved from the Catalogue of Health Establishments (CLUES). Logistic regressions were fitted to determine the association between health subsystems and mortality associated to Covid-19. The analyses were divided between hospitalized and ambulatory patients. Results: The probability of dying from Covid-19 was higher among those treated at Instituto Mexicano del Seguro Social (IMSS) (Hospitalized:OR=5.11, Ambulatory:OR=36.57), Instituto de Seguridad y Servicios Sociales de los Trabajadores del Estado (ISSSTE) (Hospitalized:OR=2.10, Ambulatory:OR=9.19), Secretaría de Salud (SS) (Hospitalized:OR=1.94, Ambulatory:OR=5.29) or other public institutions (Hospitalized: OR=1.70, Ambulatory: OR=9.56) than in those treated in private institutions. Conclusions: Differences in healthcare quality and access between health subsystems are profound. It is imperative to increase the capacity and quality of the different health subsystems to improve health outcomes.

Resumen Objetivo: Describir diferencias en letalidad por Covid-19 entre subsistemas de salud en la Ciudad de México entre marzo y diciembre de 2020. Material y métodos: Análisis secundario retrospectivo del Sistema Nacional de Vigilancia Epidemiológica sobre casos Covid-19. La información sobre instituciones proveedoras de salud fue obtenida del Catálogo de Establecimientos de Salud. Se ajustaron regresiones logísticas para determinar la asociación entre los subsistemas de salud y la mortalidad atribuida a Covid-19. Los análisis se dividieron entre pacientes hospitalizados y ambulatorios. Resultados: La probabilidad de morir fue mayor entre aquéllos atendidos en el Instituto Mexicano del Seguro Social (IMSS) (Hospitalizados: OR=5.11, Ambulatorios: OR=36.57), Instituto de Seguridad y Servicios Sociales de los Trabajadores del Estado (ISSSTE) (Hospitalizados: OR=2.10, Ambulatorios: OR=9.19), Secretaría de Salud (SS) (Hospitalizados: OR=1.94, Ambulatorio: OR=5.29) u otras instituciones públicas (Hospitalizados: OR=1.70, Ambulatorio: OR=9.56) que en los atendidos en instituciones privadas. Conclusiones: Las diferencias en calidad y acceso a la atención médica entre subsistemas de salud son profundas. Urge aumentar la capacidad y la calidad de los subsistemas de salud para mejorar los resultados en salud.

Early childhood caries (ECC) is a particular destructive form of caries that destroys dental structures of children during the first 3 years of life. Objective: To find out if there is an association between caries prevalence and socioeconomical level in children who attended the pediatric dental clinic at Universidad Nacional Autónoma de México (UNAM) during the school year 2006-2007. Material and methods: A transversal study was performed with a sample of 100 patients ranging from 12 to 48 months of age. An oral examination was carried out to determine the presence of caries. A questionnaire was applied to the parents to establish their socioeconomical level. Results: The prevalence of early childhood caries in the sample was 59.5%. A statistical significant difference by gender and age (χ2 = 43.27 p =0.001) was found. In relation to the average of teeth affected in children it was greater (2.8 ± 1.75) when the father had technical or professional studies (F = 4.5, p = 0.01). And also when the mother had studied junior high or high school (3.05 ± 2.69) (F = 3.9, p = 0.02). When children consumed 3 or more sodas or soft drinks daily, there was an increase in the number of teeth to be extracted (1.91 ± 2.06) (F = 3.3 p = 0.03) that also occur when children ate candies during twice the day (2.09 ± 2.07) (F = 4.6 p = 0.12). Conclusions: The prevalence of ECC was high, and the educational level of the parents and dietary habits of the children were directly related to the presence of tooth decay of the surveyed underage patients.

La caries de la infancia temprana (CIT) es una forma particularmente destructiva de las estructuras dentarias que afecta principalmente a niños menores de tres años de edad. Objetivo: Determinar la prevalencia de caries de la infancia temprana y su asociación con el nivel socioeconómico familiar en niños que asistieron a consulta por primera vez a la Clínica de Odontopediatría de la DEPeI, UNAM durante el periodo escolar 2006-2007. Material y métodos: Se realizó un estudio transversal, con una muestra de 100 pacientes infantiles de 12 a 48 meses de edad. Mediante un examen bucal se determinó la presencia de caries de la infancia temprana y se aplicó una encuesta a los padres de los pacientes para conocer su nivel socioeconómico. Resultados: La prevalencia de caries de la infancia temprana en la población examinada fue de 59.5%. Se encontró diferencia estadística significativa por género y edad en el total de los niños y niñas (χ² = 43.27 p = 0.001). Respecto al promedio de dientes afectados por caries en la población infantil examinada fue mayor (2.8 ± 1.75) cuando el padre tenía estudios técnicos o profesionales (F = 4.5, p = 0.01) y también cuando la madre tenía estudios de secundaria y preparatoria. (3.05 ± 2.69) (F = 3.9, p = 0.02). Cuando los niños consumieron 3 o más refrescos diarios, se incrementó el promedio de dientes con necesidad de extracción (1.91 ± 2.06) (F = 3.3 p = 0.03). Al consumir 2 o más golosinas diarias también se incrementó el número de extracciones (2.09 ± 2.07) (F = 4.6 p = 0.12). Conclusiones: La prevalencia de CIT fue alta y la escolaridad de los padres y la dieta infantil se relacionaron directamente con la presencia de caries de la infancia temprana en los pacientes examinados.

OBJECTIVE: Translate, culturally adapt, and validate a new Spanish version of the Oral Health Impact Profile (OHIP) for older persons in Mexico City, Mexico. METHODS: A validation study of the new version of OHIP-Mx-49 was carried out through interviews and clinical examinations of people aged 60 or older in the southern area of Mexico City. The following variables were analyzed: sociodemographic (age, sex, marital status, level of schooling, and whether the subject lived alone), clinical (current number of teeth, coronal and root caries, dental hygiene, and the use of removable prostheses and dentures), and self-perception (of the need for dental and general health care). The OHIP-Mx-49 value was calculated as the sum of the scores in seven dimensions: functional limitation, pain, psychological discomfort, physical disability, psychological disability, social disability, and handicap. The internal consistency (Cronbach's alpha coefficient), replicability (intraclass correlation coefficient), and convergent and discriminant validity (Mann-Whitney and Kruskal-Wallis tests) were assessed. RESULTS: Of the 131 people interviewed (average age: 73.8 ± 8.3 years), 77.9% were women. High internal consistency values were obtained for the OHIP-Mx-49 (0.96) and the dimensions (0.79-0.86), and replicability in the instrument (0.877) and all the dimensions except social disability (0.176). The average OHIP-Mx-49 score was 37.1 ± 35.3; higher scores were observed in people with 1-9 teeth (P = 0.02), people who did not have coronal caries (P = 0.02), and people who perceived a need for dental care (P = 0.01). CONCLUSIONS: The OHIP-Mx-49 is a reliable and valid instrument that can be used with older Mexicans.

OBJETIVOS: Traducir, adaptar culturalmente y validar una nueva versión en español del instrumento denominado perfil de impacto de la salud bucal (OHIP) en adultos mayores de la Ciudad de México, México. MÉTODOS: Estudio de validación de la nueva versión OHIP-Mx-49 mediante entrevistas y exámenes clínicos a personas de 60 años o más del sur de la Ciudad de México. Se analizaron variables sociodemográficas (edad, sexo, estado civil, nivel de escolaridad y si vive solo), clínicas (número de dientes presentes, caries coronal y radicular, higiene de la dentadura, y uso de prótesis removible y dentadura total) y de autopercepción (necesidad de tratamiento dental y de salud general). Como valor del OHIP-Mx-49 se tomó la suma de las puntuaciones alcanzadas en sus siete dimensiones: limitación funcional, dolor, incomodidad psicológica, inhabilidad física, inhabilidad psicológica, inhabilidad social e incapacidad. Se evaluó la consistencia interna (coeficientes a de Cronbach), la repetibilidad (coeficiente de correlación intraclase) y la validez convergente y discriminativa (pruebas de Mann-Whitney y de Kruskal-Wallis). RESULTADOS: De las 131 personas entrevistadas (edad promedio: 73,8 ± 8,3 años), 77,9% eran mujeres. Se obtuvieron valores elevados de consistencia interna, tanto del OHIP-Mx-49 (0,96) como de las dimensiones (0,79-0,86), y de repetibilidad en el instrumento (0,877) y todas las dimensiones, excepto inhabilidad social (0,176). La puntuación promedio del OHIP-Mx-49 fue de 37,1 ± 35,3; se observaron mayores puntuaciones en las personas con 1-9 dientes (P = 0,02), las que no presentaron caries coronal (P = 0,02) y las que percibían necesitar tratamiento dental (P = 0,01). CONCLUSIONES: El OHIP-Mx-49 es un instrumento confiable y válido que puede aplicarse en adultos mayores mexicanos.
