Abstract Objective Few studies have addressed the use of sacral nerve stimulation (SNS) in the treatment of patients with multiple pelvic floor dysfunctions (PFD). So, we evaluated the functional outcomes and level of satisfaction with SNS in selected patients with one or multiples PFD. Methods A prospective database was used to collect information on eligible patients treated for PFD with SNS, and severity of symptoms was assessed with scores and satisfaction rates by visual analogue scale (VAS) at baseline and by the end of follow-up. Results We recruited 70 patients, 98.6% of whom responded positively during the evaluation period (Global Response Assessment ≥ 50% for at least one type of PFD), resulting in the implantation of a permanent SNS device. Additionally, 49 of the patients (71%) had a single PFD (fecal incontinence [FI] = 38; constipation/obstructed defecation syndrome [C/ODS] = 11), while 20 (29%) had more than one PFD (double incontinence/n = 12; double incontinence + C/ODS/n = 8). All scores improved significantly between baseline (pre-SNS) and the end of follow-up (post-SNS), as did VAS in all groups (single and multiple PFD). The pre-SNS scores were higher in patients with a single PFD, including FI (Cleveland clinic Florida incontinence score [CCF-FI]) and C/ODS (Cleveland clinic constipation score [C-CCF] and the Renzi ODS score). The pre-SNS impact of VAS scores was similar in all groups (single and multiple PFD), but the VAS (post-SNS) was significantly lower (better response) for FI alone compared with multiple PFD. Conclusion The SNS technique is an effective and safe option for patients with one or more PFD refractory to conservative measures. Response was positive for at least two PFD, based on reduced correspondent scores and satisfaction rate.

Abstract Background There is currently some discussion over the actual usefulness of performing preoperative upper airway assessment to predict difficult airways. In this field, modified Mallampati test (MMT) is a widespread tool used for prediction of difficult airways showing only a feeble predictive performance as a diagnostic test. We therefore aimed at evaluating if MMT test would perform better when used as a screening test rather than diagnostic. Methods An accuracy prospective study was conducted with 570 patients undergoing general anesthesia for surgical procedures. We collected preoperatively data on sex, age, weight, height, body mass index (BMI), ASA physical status, and MMT. The main outcome was difficult laryngoscopy defined as Cormack and Lahane classes 3 or 4. Bivariate analyses were performed to build three different predictive models with their ROC curves. Results Difficult laryngoscopy was reported in 36 patients (6.32%). Sex, ASA physical status, and MMT were associated with difficult laryngoscopy, while body mass index (BMI) was not. The MMT cut-off with the highest odds ratio was the class II, which also presented significantly higher sensitivity (94.44%). The balanced accuracy was 67.11% (95% CI: 62.78-71.44%) for the cut-off of class II and 71.68% (95% CI: 63.83-79.54) for the class III. Conclusion MMT seems to be more clinically useful when the class II is employed as the threshold for possible difficult laryngoscopies. At this cut-off, MMT shows the considerable highest sensitivity plus the highest odds ratio, prioritizing thus the anticipation of difficult laryngoscopies.

ABSTRACT: The present study aimed to identify and describe cardiac alterations in sheep experimentally poisoned with Palicourea marcgravii through analysis of serum cardiac biomarkers (serum troponin I and creatine kinase - CK-MB) and electro and echocardiographic assessments to contribute to a better understanding of the poisoning pathophysiology. P. marcgravii is the main plant within a group of 22 species that cause sudden death in Brazil; its toxic principle is sodium monofluoroacetate. Eight healthy crossbreed male sheep, aged between five and twelve months, weighing 14 to 27kg, were evaluated. The animals received 1g kg-1 of P. marcgravii plants orally. The sheep were evaluated before administering the plant (T0) through electro and echocardiography and blood collection to assess cardiac biomarkers (CK-MB and cTnI). Collections and analyses were repeated every four hours until the animal’s death. During the study, there was the presence of extravasation of serum troponin I carried out in a qualitative test, with positive values at time T4, and the serum CK-MB biomarker had a peak at T4 and slightly decreased at T8. The electro and echocardiographic examinations showed that the cause of death in these animals was due to acute heart failure characterized by arrhythmias, tachycardia/ventricular fibrillation, drop in cardiac output, left ventricular (LV) systolic dysfunction by the progressive decrease in the LV ejection fraction (EF), decrease in LV fractional shortening (FS), and decrease in aortic flow velocity and aortic flow gradient. This study seems to be the first to evaluate cardiac alterations in sheep poisoned by P. marcgravii through cardiac biomarkers and electro and echocardiographic exams.

Abstract A new concept of multisystem disease has emerged as a long-term condition following mild-severe COVID-19 infection. The main symptoms of this affection are breathlessness, chest pain, and fatigue. We present here the clinical case of four COVID-19 patients during hospitalization and 60 days after hospital discharge. Physiological impairment of all patients was assessed by spirometry, dyspnea score, arterial blood gas, and 6-minute walk test 60 days after hospital discharge, and computed tomographic scan 90 days after discharge. All patients had fatigue, which was not related to hypoxemia or impaired spirometry values, and interstitial lung alterations, which occurred in both mechanically ventilated and non-mechanically ventilated patients. In conclusion, identifying the prevalence and patterns of permanent lung damage is paramount in preventing and treating COVID-19-induced fibrotic lung disease. Additionally, and based on our preliminary results, it will be also relevant to establish long-term outpatient programs for these individuals.

Abstract Rhodolith beds are important marine benthic ecosystems, representing oases of high biodiversity among sedimentary seabed environments. They are found frequently and abundantly, acting as major carbonate 'factories' and playing a key role in the biogeochemical cycling of carbonates in the South Atlantic. Rhodoliths are under threat due to global change (mainly related to ocean acidification and global warming) and local stressors, such as fishing and coastal run-off. Here, we review different aspects of the biology of these organisms, highlighting the predicted effects of global change, considering the additional impact of local stressors. Ocean acidification (OA) represents a particular threat that can reduce calcification or even promote the decalcification of these bioengineers, thus increasing the eco-physiological imbalance between calcareous and fleshy algae. OA should be considered, but this together with extreme events such as heat waves and storms, as main stressors of these ecosystems at the present time, will worsen in the future, especially if possible interactions with local stressors like coastal pollution are taken into consideration. Thus, in Brazil there is a serious need for starting monitoring programs and promote innovative experimental infrastructure in order to improve our knowledge of these rich environments, optimize management efforts and enhance the needed conservation initiatives.

Abstract Knowledge about the Brazilian fungal diversity was, until 2010, recorded in few taxonomy and ecology publications, as well as in a handful of species lists. With the publication of the Catálogo de Plantas e Fungos do Brasil and the continued availability of an online list, it has been possible to aggregate this dispersed knowledge. The version presented here adds 2,111 species names to the 3,608 listed in 2010. A total of 5,719 species of fungi distributed in 1,246 genera, 102 orders and 13 phyla represents a considerable increase over the last five years, when only 924 genera and 78 orders were registered. Basidiomycota (2,741 species in 22 orders) and Ascomycota (1,881 species in 41 orders) predominate over other groups. The Atlantic Rainforest has the largest number of records, with 3,017 species, followed by Amazon Rainforest (1,050), Caatinga (999), Cerrado (638) and Pampa and Pantanal with 84 and 35 species, respectively. The Northeast region has the greatest richness (2,617 species), followed by Southeast (2,252), South (1,995), North (1,301) and Central-West (488 species). Regarding the States of the Federation, São Paulo with 1,846 species, Pernambuco with 1,611 and Rio Grande do Sul with 1,377 species are the most diverse.

Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Abstract: Morphometric parameters of the digestive tract are required for an understanding of the digestive processes of the food in the animal organism, besides indicating the feeding preference of specie. This study aimed to describe morphologically the small and large intestines, organs of the digestive system of representatives of Xenarthra order to provide data for the evaluation of diet and conduct clinical procedures in these animals, whether free-living or captive. At this research, were used in total 7 specimens from three-toed sloths (Bradypus torquatus), nine-banded armadillo (Dasypus novemcinctus) and giant anteater (Myrmecophaga tridactyla). The intestines of B. torquatus were short and simple, but at the specimens of D. novemcintus and M. tridactyla the intestines were long and had some peculiarities. We notice the presence of Brunner's glands and structures to increase the surface absorption at the duodenum of all specimens. Only in B. torquatus, we notice that the mesentery remains the jejune attached to the dorsal wall of the abdominal cavity. The ileum represented the lower portion of the intestines in all studied specimens except in M.tridactyla. The cecum in D. novemcinctus and M. tridactyla showed considerable size, glands at the mucosa and was full of food debris, indicating that it was functional. In the mucosa of the colon of all specimens had crypts of Lieberkühn, being more numerous in D. novemcinctus and M. tridactyla. Only in B. torquatus, the rectum showed greater diameter and stiffness compared to the colon. In all species studied, we notice a large glandular surface and lots of goblet cells that produce mucus to facilitate defecation. Our results demonstrate that the conformation and structure of the digestive system reflects more the kind of diet and digestive needs of the animal, than to the family he belongs.

The feeding habits of Macrodon ancylodon were studied based on 280 specimens from the Maranhão coast, northeast Brazil. The diet showed a predominance of fishes, crustaceans and occasionally cephalopods. The diets of males and females were similar and difference was found in the proportion of prey items among juveniles, sub adults, and adults (P > 0.05). During rainy season the prey items were more diverse than dry season. In the dry season M. ancylodon feeds more Penaeidae (Penaeus sp. and Penaeus schmitti), Callinectes sp. and Engraulidae (Cetengraulis sp.). An increase of food spectrum was observed in rainy season, preyed upon 5 species of fish (Anchovia clupeoides, Cetengraulis edentulus, Macrodon ancylodon, Mugil curema and Sardinella brasiliensis), 3 species of Crustacea (Penaeus sp., Penaeus schmitti, Parapenaeus politus, and Callinectes sp.) and 2 Cephalopoda (Loligo sp. and Loligo plei). Differences in the ingestion of prey items were found during the year, where in the rainy season this species presented larger stomach contents. Cannibalism was encountered more intensively for mature fish.

OBJECTIVES: There is evidence that outdoor workers exposed to high levels of air pollution exhibit airway inflammation and increased airway symptoms. We hypothesized that these workers would experience increased airway symptoms and decreased nasal mucociliary clearance associated with their exposure to air pollution. METHODS: In total, 25 non-smoking commercial motorcyclists, aged 18-44 years, were included in this study. These drivers work 8-12 hours per day, 5 days per week, driving on urban streets. Nasal mucociliary clearance was measured by the saccharine transit test; airway acidification was measured by assessing the pH of exhaled breath condensate; and airway symptoms were measured by the Sino-nasal Outcome Test-20 questionnaire. To assess personal air pollution exposure, the subjects used a passive-diffusion nitrogen dioxide (NO2) concentration-monitoring system during the 14 days before each assessment. The associations between NO2 and the airway outcomes were analyzed using the Mann-Whitney test and the Chi-Square test. Clinicaltrials.gov: NCT01976039. RESULTS: Compared with clearance in healthy adult males, mucociliary clearance was decreased in 32% of the motorcyclists. Additionally, 64% of the motorcyclists had airway acidification and 92% experienced airway symptoms. The median personal NO2 exposure level was 75 mg/m3 for these subjects and a significant association was observed between NO2 and impaired mucociliary clearance (p = 0.036). CONCLUSION: Non-smoking commercial motorcyclists exhibit increased airway symptoms and airway acidification as well as decreased nasal mucociliary clearance, all of which are significantly associated with the amount of exposure to air pollution.

Trichogramma galloi Zucchi is an important parasitoid of Diatraea saccharalis (Fabricius), a major pest of sugarcane. As various chemical products are used in the management of this crop, this study aimed to evaluate the repellence of some pesticides on adults of the parasitoid T. galloi, and its effect on the emergence of their offspring. The tests were carried out in a climatic chamber at 23 ± 2 ºC, with a relative humidity of 60 ± 10% and a photo period of 14 hours. The products evaluated were: insecticides clorantraniliprole, ethiprole, thiamethoxam, triflumuron, lambda-cyhalothrin+thiamethoxam, fipronil and Metarhizium anisopliae; the herbicides clomazone and diuron+hexazinone; and the plant-growth regulators methyl-sulfometuron, and ethyl-trinexapac, at the highest concentrations recommended for sugarcane. To evaluate the repellence of each product, a free-choice test was carried out with, where 30 eggs of D. saccharalis were immersed in distilled water and another 30 in a mixture of the product to be tested. Each mass of eggs from the two treatments was transferred to a glass tube, these being connected by a paper connector with a central hole through which a smaller tube was attached to allow the release of three females of T. galloi which had previously been fed. The experimental design was completely randomized, with fifteen repetitions for each product evaluated. Repellence, average number of parasitised eggs per female and emergence rates were all evaluated. Clorantraniliprole, triflumuron and methyl-sulfometuron did not repel the parasitoid nor affect the number of parasitized eggs. As for emergence, it was found that M. anisopliae, thiamethoxam, clomazone, diuron+hexazinone and methyl-sulfometuron did not affect this biological characteristic.

OBJECTIVE: To translate into Portuguese and to culturally adapt the malnutrition screening tool for hospitalized children, Strongkids. METHODS: This study documents the translation of the tool from the original version (English) into Portuguese. The translation and cultural adaptation of the content of this tool consisted of six stages, according to the methodology proposed by Beaton et al (initial translation, synthesis of translations, back translation, verification of the cultural equivalence process, pre-test, and evaluation of the cultural adaptation process). In the first stage, translation was performed by two independent translators, followed by their synthesis and reconciliation; in the third one, the reconciled version was back translated and, then, a pre-final one that retained all linguistic equivalence was developed. In the fifth step, a pre-test of the pre-final version was performed in order to verify the understanding of the items and a final version of the tool was developed. RESULTS: The pre-final version of the tool was applied to 30 parents/guardians and to 20 healthcare professionals in order to verify its understanding by both. The main alterations were the adaptation of technical terms in order to meet the recommendations of health professionals, and the adjustment of terms for parents/guardians understanding. CONCLUSIONS: The Portuguese translation of the tool was easily understood by parents/guardians and health professionals, and it should be useful to screen the risk of malnutrition in hospitalized children.

This paper describes an outbreak of pneumonia in a sheep herd in the central region of Minas Gerais, Brazil. Clinically, the animals presented apathy, exhibited respiratory difficulty during 2 to 3 days or sudden death. The animals were not medicated and found dead. Grossly, the pulmonary findings were similar in all sheep. The pulmonary cranial lobes and the ventral portion of caudal lobes were consolidated and purulent exsudate streamed out of the airways. In the parenchyma of the cranial lobes there were white slightly prominent multifocal to coalescent areas with 0.2 to 0.5cm in diameter intercalated with dark red areas. Consolidated lesions occupied 70 to 80% of the lungs. Fibrinous pleuritis was observed in sheep 1, 2 and 3. Microscopically, the findings were fibrinopurulent bronchopneumonia with intense hyperemia, areas with intra-alveolar hemorrhage and thickening of interlobular septa with numerous neutrophils, cellular rests and scattering fibrin. Multifocal areas with liquefaction necrosis containing numerous bacterial colonies were observed in sheep 1, 2 and 3. In the cranial lobes of these sheep, there were areas with degenerated neutrophils forming clusters of basophilic cells with alongated nuclei ("oat cells") associated with bacterial colonies. The histological findings were characteristic of pneumonia caused by Mannheimia (M.) haemolytica. Samples of the cranial lobes were sent for bacterial culture, and M. haemolytica was isolated and identified in all animals. This is the first report correlating pathological findings and the isolation of M. haemolytica as cause of bronchopneumonia in sheep in the country.

The anti-inflammatory and antinociceptive activities of the standardized leaves extract (HBSE) of Hypericum brasiliense (Guttiferae) were evaluated in animal models. Male Wistar rats were treated with H. brasiliense extract (50, 250 and 500 mg/kg, p.o.) in 3% Tween 80 0.9% saline solution. The treatment of the edema induced by carrageenin with HBSE (500 mg/kg) showed significant inhibition when compared to the control group. At this dose, the edema decreased by 31.25% in the third hour after treatment (edema peak), but the dose of 50 mg/kg has inhibited the edema by 53.13% (p < 0.05). At the dose of 50 mg/kg, the decrease of the edema induced by dextran was similar to that caused by cyproheptadine. The decrease of the formation of granulomatous tissue (6.6%) was comparable to the control group. The HBSE inhibited the abdominal constrictions induced by acetic acid. At a dose of 50 mg/kg, the inhibition of the abdominal constrictions (46.4%) was comparable to that produced by indomethacin (42.9%). A dose of 250 mg/kg inhibited these constrictions by 70.66% when compared to control (p < 0.001). In the hot-plate test, an increase in the latency time was observed at a 50 mg/kg dose. These data suggest that HBSE has anti-inflammatory activity on acute process, developed principally by arachdonic acid derivates and analgesic effect due to its probable involvement in the Central Nervous System.
