Resumo Diferentes formulações têm dado destaque à Atenção Primária à Saúde como porta de entrada preferencial do sistema de saúde, base das redes de atenção e função de coordenação do cuidado. A regulação assistencial (ou do acesso), por sua vez, vem sendo considerada, no Brasil, uma estratégia e tecnologia central de gestão das redes. Este artigo objetivou caracterizar a interface entre atenção primária e regulação assistencial nas formulações e diretrizes nacionais sobre regulação assistencial do SUS, considerando o período de 2002 a 2017. Adotou-se a metodologia da análise documental, utilizando fontes de domínio público, notadamente portarias e manuais técnicos do Ministério da Saúde, organizando os conteúdos extraídos nas dimensões “conceitos de regulação”, “organização e gestão da regulação” e “estratégias e ferramentas de regulação”, dentro das quais se buscou caracterizar o lugar da atenção primária. Observou-se maior destaque para a interface entre regulação assistencial ambulatorial e atenção primária a partir de 2011 com a noção de microrregulação, de modo mais marcante nos documentos e iniciativas relacionados à gestão da atenção primária do que nos específicos da regulação, enfocando a elaboração de dispositivos de regulação a partir da atenção primária, destacando-se o Telessaúde.

The methodology was the documentary analysis, using public domain sources, notably ordinances and technical manuals of the Ministry of Health, organizing the extracted contents in the dimensions “concepts of regulation”, “organization and management of regulation” and “strategies and tools of regulation”, which sought to characterize the place of primary care. Greater emphasis was given to the interface between health care regulation and primary care from 2011, more prominently in the documents and initiatives related to primary care management than in the specifics of regulation, focusing on the ambulatory dimension of access regulation and in the elaboration of regulation devices based on primary care, highlighting Telehealth.

Resumo Introdução: No Brasil, a toxoplasmose congênita (TC) é a principal causa de deficiência visual na infância. É causa de retinocoroidite, que pode levar à cegueira. Minas Gerais apresenta prevalência de um neonato com TC para cada 770 nascidos vivos. Objetivo: Avaliar funcionalidade visual e tarefas do autocuidado de crianças com TC classificadas em grupos de acordo com a acuidade visual. Método: Estudo transversal com 96 pré-escolares com TC. Realizado exame oftalmológico e avaliada a funcionalidade por dois instrumentos: O teste Avaliação da Visão Funcional (AVIF-2 a 6 anos) e Inventário de Avaliação Pediátrica de Incapacidade (PEDI-versão brasileira). Resultados: As crianças foram classificadas em três grupos conforme a acuidade visual: perda visual moderada/grave (n=16), perda leve (n=39) e visão normal (n=41). Houve diferença significativa no escore total do AVIF-2 a 6 anos entre os três grupos (p=0,001), e entre os grupos com perda visual moderada/grave e visão normal (p<0,0001). Os escores do domínio seguimento visual apresentaram pior resultado (p=0,022). O teste PEDI não mostrou diferença significativa entre os grupos. Não houve correlação entre os escores dos testes aplicados. Conclusão: Crianças com TC e perda visual moderada/grave apresentaram comprometimento da funcionalidade visual com maior prejuízo no seguimento visual. O teste AVIF-2 a 6 anos demonstrou esse comprometimento entre os grupos com diferentes acuidades visuais. O teste PEDI (autocuidado) não mostrou diferença estatística significativa dos escores entre os grupos. O teste AVIF- 2 a 6 anos pode contribuir para intervenção mais objetiva na habilitação visual de crianças com TC e baixa visão.

Abstract Introduction: In Brazil, congenital toxoplasmosis (CT) is the main cause of visual impairment in childhood. It causes retinochoroiditis, which should lead to blindness. Minas Gerais has a prevalence of 1 newborn with congenital toxoplasmosis for every 770 live births. Objective: To evaluate visual functionality and tasks of self-care with TC classified in groups according to a visual acuity. Method: A cross-sectional study with 96 preschoolers with CT. Ophthalmologic examination and assessment of some functionalities were performed: The Functional Vision Evaluation (AVIF-2 at 6 years) and the Pediatric Disability Assessment Inventory (PEDI-Brazilian version). Results: Children were classified into three groups according to visual acuity: moderate / severe visual loss (n=16), low level (n=39) and normal vision (n=41). The chance of difference is not greater than AVIF-2 at 6 years among the three groups (p=0.001), being (p <0.0001) between the groups with moderate/severe visual and normal vision. The domain scores are displayed as the end result (p=0.022). The test PEDI is not able to make a difference between the groups. It is not different between the scores of the tests carried out. Conclusion: Children with CT without moderate / severe visual impairment, compromising visual functionality with greater impairment without visual accompaniment. The AVIF-2 at 6 years test demonstrated the impairment between groups with different visual acuities. The PEDI (self-care) test was not performed with a significant difference in scores between the groups. The AVIF-2 at 6 years test may contribute to the more effective intervention in the visual habilitation of children with CT and low vision.

RESUMO: O presente estudo investigou a morfologia e patologia associada com a ocorrência de cistos de Myxobolus sp. no intestino do peixe siluriforme Rhamdia quelen . Comparações com outras espécies de Myxobolus que têm infectado intestinos de carpas e bagres de água doce demonstraram que os esporos piriformes de Myxobolus sp. são similares no formato dos anteriormente descritos para M. miyairii, M. duodenalis e M. cunhai , diferindo, no entanto, quanto ao tamanho. As análises morfométricas revelaram que os esporos maduros de Myxobolus sp. (10,9 x 5,1μm) são menores do que os da maioria das espécies, exceto para M. cunhai (10,0 x 5,0μm), que foi descrito infectando o bagre brasileiro Clarias pimelodus , embora os esporos sejam diferentes morfologicamente em relação à forma das cápsulas polares. Futuras pesquisas são necessárias para esclarecer as relações taxonômicas e filogenéticas entre espécies congêneres.

ABSTRACT: The present study investigated the morphology and pathology associated with the occurrence of cysts caused by Myxobolus sp. in the intestine of the silver catfish Rhamdia quelen . Comparisons with the other Myxobolus species that infect the intestines of cyprinids and siluriforms revealed that the pyriform spores of Myxobolus sp. are similar in shape to those described previously in M. miyairii, M. duodenalis and M. cunhai, but different in size. Morphometric analyses revealed that mature spores of Myxobolus sp. (10.9 x 5.1μm) are smaller than those of most species except M. cunhai (10.0 x 5.0μm), which has been described infecting the Brazilian catfish Pimelodus clarias , although the spores differ morphologically in relation to the shape of the polar capsules. Further research is necessary in order to clarify the taxonomic and phylogenetic relationships among these congeneric species.

Esse texto aborda os resultados de uma pesquisa qualitativa financiada pela FAPEMIG/PUC - Minas que tem como objetivo investigar os efeitos do método "canguru" na relação entre mãe e bebê prematuro. A pesquisa de campo teve seus dados coletados através de entrevistas semiestruturadas junto às mães de bebês prematuros egressas do programa Canguru da Maternidade Municipal Aideé Conroy Espejo, de Betim. Utilizamos a análise de conteúdo por categorias temáticas para a análise dos dados. Os resultados apontam a complexidade da maternidade no interior da prematuridade, atravessada pela vivência de trauma, pelo medo de perder o bebê, pela visão da fragilidade da criança. Nesse contexto, o programa mãe-canguru possibilita efetivamente a conexão entre mãe e bebê prematuro, colaborando para a vinculação da díade. Concluímos ainda que a mãe, os familiares e a equipe podem constituir um ambiente facilitador a partir do momento em que assumam uma postura ativa na mediação da relação do prematuro com o mundo.

This text approaches the results of the qualitative research financed by FAPEMIG and PUC-MINAS and aims to investigate the effects of the kangaroo mother method in the relationship between the mother and the newborn baby. The field research had the data collected by semi structural interviews conducted with the mothers of newborn babies egressed from the Kangaroo program of Betim's Public Maternity named Maternidade Municipal Aidée Conroy Espejo. We utilized the content analyses with thematic categories on data assessment. The results showed the complexity of motherhood inside the prematurity, crossed by the trauma experience, by the fear of losing the baby and by the vision of the child's fragility. In this context, the Kangaroo-Mother Program facilitates the relationship between the mother and the premature baby, contributing to the dyadic's bound. We concluded that mother, family members, and staff can provide a facilitating environment as they take on an active attitude in the mediation between the premature relationship and the world.

Este texto enfoca los resultados de la pesquisa cualitativa financiada pela FAPEMIG / PUC - Minas que tiene como objetivo investigar los efectos del 'método Canguro' en la relación entre madre y bebé prematuro. La pesquisa de campo tuvo sus datos colectados a través de entrevistas semi-estructuradas junto a las madres de bebés prematuros egresas del Programa Ganguro de la Maternidad Municipal Aideé Conroy Espejo, de Betim, Minas Gerais, Brasil. Utilizamos el análisis de contenido por categorías temáticas para el estudio de los datos. Los resultados apuntan para la complejidad de la maternidad en el interior de la prematuridad, atravesada por la vivencia de trauma, por el miedo de perder el bebé, por la visión de la fragilidad del niño. En este contexto, el Programa Madre-canguro posibilita efectivamente la conexión entre madre y bebé prematuro, colaborando para la vinculación de la díada. Concluimos, además, que la madre, familiares, el equipo pueden crear un ambiente facilitador a partir del momento en que asumen una postura activa en la mediación de la relación del prematuro con el mundo.

O presente artigo pretende trabalhar com o conceito de clínica psicológica e tenta definir esse campo de atuação do psicólogo através de uma análise histórica do surgimento dessa prática e de uma reflexão sobre os limites do modelo de clínica como prática individual de consultório, em contraposição a um modelo de clínica chamada social, que sustenta novas atuações no campo da Psicologia, no Brasil. Assim, iremos, primeiramente, buscar as origens etimológicas, históricas e reflexivas do termo clínica. Em seguida, pretendemos refletir sobre as articulações entre a Psicologia e o individualismo moderno, e, ainda, sobre os limites da clínica entendida como prática liberal, privada e individualizante. Por fim, defenderemos, para além da cisão entre clínica e política, presente no modelo tradicional, uma definição de clínica social como prática ética e política das intervenções, comprometida com a promoção da saúde e engajada na realidade social brasileira.

This article intends to work with the clinical psychological concept trying to define the psychologist field across a historical analysis of the beginning of this practice and a reflection about the model limits of clinic as an individual practice at the office, in opposite to a model of clinic called social, that supports new performances on the Psychology field in Brazil. First, we will look for the etymological, historical and reflexive origins of the term clinic. Then, we intend to reflect about the articulations between Psychology and the modern individualism and the limits of the clinic understood as a liberal and private practice. In the end, we will defend beyond the division between clinic and politics, in the tradicional model, a definition of social clinic as a political and ethical position of interventions engaged with the promotion of health and with the Brazilian social reality.
