RESUMO Objetivo O objetivo deste estudo foi avaliar o efeito do sistema CROS em fenômenos como efeito sombra da cabeça em usuários de implante coclear unilateral. Método Estudo transversal prospectivo, aprovado pelo conselho de ética sob protocolo 2.128.869. Onze adultos com surdez de instalação pós-lingual usuários de IC Advanced Bionics unilateral foram selecionados. O reconhecimento de fala foi avaliado com palavras gravadas apresentadas a 65dBA a 0o azimute e a (90o no lado contralateral ao IC), com ruído a 55dBA, usando somente o IC e IC+sistema CROS. Os resultados foram analisados usando teste t pareado com alfa de 0,05. Resultados Os escores médios de reconhecimento de fala foram significativamente melhores com IC + CROS em relação à condição apenas IC (p <0,05, p <0,005 e p <0,005 respectivamente). Na apresentação à frente não foram encontradas diferenças significantes. Conclusão Os usuários de IC unilateral sem resíduo útil para uso de prótese auditiva ou impossibilitados de submeter-se à cirurgia bilateral podem se beneficiar do dispositivo CROS para o reconhecimento de fala, sobretudo quando a fala for apresentada ao lado contralateral ao IC.

ABSTRACT Purpose The aim of this study was to evaluate the contribution of the CROS system on the head shadow effect in unilateral implant users. Methods Prospective cross-sectional study, approved by the ethics committee under protocol 2.128.869. Eleven adults with post-lingual deafness users of unilateral Advanced Bionics CI were selected. Speech recognition was evaluated with recorded words presented at 65dBA at 0o azimuth and at 90o on the side contralateral to the CI, with noise at 55dBA, using CI alone and CI + CROS system. The results were analyzed using paired t-test with a 0.05 alpha. Results The mean speech recognition scores were significantly better with CI + CROS in relation to the condition of CI alone (p <0.05, p <0.005 and p <0.005 respectively). In the presentation at 0o azimuth, no significant differences were found. Conclusion Users of unilateral CI without useful residual hearing for the use of hearing aids or unable to undergo bilateral surgery can benefit from the CROS device for speech recognition, especially when the speech is presented on the side contralateral to the CI.

Resumo Introdução: A utilização do implante coclear bilateral pode promover o desenvolvimento das vias auditivas centrais de maneira simétrica, beneficiando, assim, o desenvolvimento das habilidades auditivas e melhorando a localização sonora e a habilidade de percepção auditiva da fala em situações de ruído competitivo. Objetivo: Avaliar a habilidade de reconhecimento de fala em crianças e adolescentes usuários do implante coclear bilateral sequencial, considerando a relação das variáveis: idade cirúrgica, tempo de uso do dispositivo e intervalo entre as cirurgias. Método: Foram avaliados 14 indivíduos entre 10 e 16 anos, que receberam indicação cirúrgica para implante coclear bilateral sequencial como forma de intervenção no processo de habilitação auditiva. A habilidade de reconhecimento de fala foi avaliada por meio de listas de sentenças construídas na língua portuguesa, apresentadas em duas situações: no silêncio, com intensidade fixa de 60 dB NPS, e no ruído competitivo, com a relação sinal/ruído de + 15 dB. A avaliação foi feita nas seguintes condições: unilateral com o 1° implante coclear ativado, unilateral com o 2° implante coclear ativado e bilateral com ambos os dispositivos ativados. Resultado: Os resultados dos testes de reconhecimento de fala demonstraram melhor desempenho tanto no silêncio quanto no ruído para a condição implante coclear bilateral, quando comparado ao 1° implante coclear e 2° implante coclear isoladamente. Um pior resultado de reconhecimento de fala foi encontrado com o uso do 2° implante coclear isoladamente. Não foi encontrada correlação estatisticamente significante entre idade cirúrgica, intervalo entre as cirurgias e tempo de uso do 2° implante coclear e o desempenho de percepção auditiva da fala para todas as condições avaliadas. O uso do aparelho de amplificação sonora individual anterior ao 2° implante coclear beneficiou os resultados de percepção auditiva da fala com o 2° implante coclear, tanto no silêncio quanto no ruído. Conclusão: O implante coclear bilateral proporcionou melhor reconhecimento de fala em situações de silêncio e de ruído, quando comparado com o implante coclear unilateral, independentemente do intervalo entre as cirurgias, da idade cirúrgica e do tempo de uso do 2° implante coclear. O reconhecimento de fala com o 1° implante coclear foi significativamente melhor do que com o 2° implante coclear, tanto no silêncio quanto no ruído. O uso do aparelho de amplificação sonora individual anterior ao 2° implante coclear influenciou o desempenho de reconhecimento de fala com o 2° implante coclear, tanto no silêncio quanto no ruído.

Abstract Introduction: The use of the bilateral cochlear implants can promote the symmetrical development of the central auditory pathways, thus benefiting the development of auditory abilities and improving sound localization and the ability of auditory speech perception in situations of competitive noise. Objective: To evaluate the ability of speech perception in children and adolescents using sequential bilateral cochlear implants, considering the association of these variables: age at surgery, time of device use and interval between surgeries. Methods: A total of 14 individuals between 10 and 16 years of age, who demonstrated surgical indication for the use of sequential bilateral cochlear implants as intervention in the auditory habilitation process, were assessed. The speech perception ability was assessed through sentence lists constructed in the Portuguese language, presented in two situations: in silence, with fixed intensity of 60 dB SPL, and in competitive noise, with a signal-to-noise ratio of +15 dB. The evaluation was performed under the following conditions: unilateral with the first activated cochlear implant, unilateral with the second activated cochlear implant and bilateral with both devices activated. Results: The results of the speech perception tests showed better performance in both silence and in noise for the bilateral cochlear implant condition when compared to the 1st cochlear implant and the 2nd cochlear implant alone. A worse result of speech perception was found using the 2nd cochlear implant alone. No statistically significant correlation was found between age at the surgical procedure, interval between surgeries and the time of use of the 2nd cochlear implant, and the auditory speech perception performance for all assessed conditions. The use of a hearing aid prior to the 2nd cochlear implant resulted in benefits for auditory speech perception with the 2nd cochlear implant, both in silence and in noise. Conclusion: The bilateral cochlear implant provided better speech perception in silence and in noise situations when compared to the unilateral cochlear implant, regardless of the interval between surgeries, age at the surgical procedure and the time of use of the 2nd cochlear implant. Speech perception with the 1st cochlear implant was significantly better than with the 2nd cochlear implant, both in silence and in noise. The use of the hearing aid prior to the 2nd cochlear implant influenced speech perception performance with the 2nd cochlear implant, both in silence and in noise.

RESUMO Objetivo Analisar como os Potenciais Evocados Auditivos de Longa Latência (PEALL) variam de acordo com a idade na população infantil, por meio de revisão sistemática da literatura. Estratégia de pesquisa Depois da formulação da pergunta da pesquisa, o levantamento bibliográfico foi realizado em cinco bases de dados, com os seguintes descritores: Eletrofisiologia (Electrophysiology), Potenciais Evocados Auditivos (Auditory Evoked Potentials), Criança (Child), Plasticidade Neuronal (Neuronal Plasticity) e Audiologia (Audiology). Critérios de seleção Artigos com nível de evidência 1, publicados na íntegra entre os anos de 1995 e 2015 na língua Portuguesa Brasileira ou Inglesa. Análise dos dados Foram analisados os aspectos relacionados ao surgimento, morfologia e latência dos componentes P1, N1, P2 e N2. Resultados Foram localizados 388 estudos; contudo apenas 21 contemplaram os critérios de inclusão para análise. O componente P1 caracteriza-se como o de maior ocorrência em crianças pequenas, sendo observado por volta de 100-150 ms, o qual tende a diminuir com o decorrer da idade cronológica. O componente N2 mostrou-se como o segundo componente mais registrado em crianças, sendo observado por volta de 200-250 ms. Os demais componentes N1 e P2 mostram-se menos frequentes e passam a ser visualizados e registrados ao longo do processo maturacional. Conclusão A maturação dos PEALL acontece gradativamente, sendo o surgimento dos componentes P1, N1, P2 e N2 bem como seus valores de latência variáveis na infância. Os componentes P1 e N2 são os mais observados e descritos na população pediátrica. A diversidade de protocolos dificulta a comparação entre os estudos.

ABSTRACT Purpose To analyze how Auditory Long Latency Evoked Potentials (LLAEP) change according to age in children population through a systematic literature review. Research strategies After formulation of the research question, a bibliographic survey was done in five data bases with the following descriptors: Electrophysiology (Eletrofisiologia), Auditory Evoked Potentials (Potenciais Evocados Auditivos), Child (Criança), Neuronal Plasticity (Plasticidade Neuronal) and Audiology (Audiologia). Selection criteria Level 1 evidence articles, published between 1995 and 2015 in Brazilian Portuguese or English language. Data analysis Aspects related to emergence, morphology and latency of P1, N1, P2 and N2 components were analyzed. Results A total of 388 studies were found; however, only 21 studies contemplated the established criteria. P1 component is characterized as the most frequent component in young children, being observed around 100-150 ms, which tends to decrease as chronological age increases. The N2 component was shown to be the second most commonly observed component in children, being observed around 200-250 ms.. The other N1 and P2 components are less frequent and begin to be seen and recorded throughout the maturational process. Conclusion The maturation of LLAEP occurs gradually, and the emergence of P1, N1, P2 and N2 components as well as their latency values are variable in childhood. P1 and N2 components are the most observed and described in pediatric population. The diversity of protocols makes the comparison between studies difficult.

RESUMO Objetivo O objetivo deste estudo foi monitorar o surgimento e as mudanças nos componentes dos Potenciais Evocados Auditivos de Longa Latência (PEALL) em crianças com audição normal. Método Estudo longitudinal com crianças de ambos os gêneros, sendo: sete crianças com idade entre 10 e 35 meses, e oito crianças com idade entre 37 e 63 meses. A avaliação eletrofisiológica da audição consistiu na análise dos PEALL obtidos em campo sonoro com as caixas posicionadas a 90° azimute, por meio da sílaba /ba/ na intensidade de 70 dBnNA. Cada criança passou por uma avaliação inicial seguida por duas reavaliações após três e nove meses. Resultados Foi observado surgimento dos componentes dos PEALL ao longo dos nove meses de acompanhamento, sendo os componentes P1 e N2 os mais frequentes em crianças desta faixa etária. Não houve diferença estatística no que diz respeito à ocorrência dos componentes P1, N1, P2 e N2 entre as crianças mais novas ou mais velhas. No que tange aos valores de latência, as maiores diferenças ao longo dos nove meses foram observadas no componente P1 para as crianças mais novas e para o componente N2 para as crianças mais velhas. Somente o componente P1 apresentou diferença estatisticamente significante entre os grupos, sendo que foram observados maiores valores de latência entre as crianças mais novas. Conclusão A maturação dos PEALL ocorre gradualmente e o surgimento dos componentes do complexo parece estar mais relacionado à maturação do sistema nervoso auditivo central do que à idade cronológica.

ABSTRACT Purpose The purpose of this study was to monitor the emergence and changes to the components of the Long Latency Auditory Evoked Potentials (LLAEP) in normal hearing children. Methods This longitudinal study included children of both genders: seven aged between 10 and 35 months, and eight children between 37 and 63 months. The electrophysiological hearing evaluation consisted of analysis of LLAEP obtained in a sound field generated with loudspeakers positioned at an azimuth of 90°, through which the syllable /ba/ was played at an intensity of 70 dB HL. Each child underwent an initial evaluation followed by two re-evaluations three and nine months later. Results The emergence of LLAEP components across the nine-month follow-up period was observed. P1 and N2 were the most common components in children of this age range. There was no statistically significant difference regarding the occurrence of P1, N1, P2, and N2 components amongst younger and older children. Regarding latency values, the greatest changes overtime were observed in the P1 component for younger children and in the N2 component for older children. Only the P1 component significantly differed between the groups, with the highest latency values observed in younger children. Conclusion LLAEP maturation occurs gradually and the emergence of complex components appears to be related more to the maturation of the central auditory nervous system than to chronological age.

<sec><title>Introduction</title><p>Difficulties with telephone use by adult users of cochlear implants (CIs) are reported as a limitation in daily life. Studies to improve the speech understanding of CI users on the telephone are scarce in the Brazilian scientific literature.</p></sec><sec><title>Objective</title><p>To develop and evaluate the effectiveness of a training program of auditory abilities on the telephone for an adult CI user.</p></sec><sec><title>Resumed Report</title><p>The subject was a 55-year-old woman with a degree in accounting who used a CI for 24 months. The program consisted of three stages: pretraining evaluation, eight sessions of advanced auditory abilities training, and post-training evaluation. Auditory abilities with CI were evaluated before and after training in three conditions: sound field, telephone with the speech processor in the microphone function, and telephone with the speech processor in the telecoil function. Speech recognition was assessed by three different lists: one with monosyllabic and dissyllabic words, another with nonsense syllables, and another one with sentences. The Client Oriented Scale of Improvement (COSI) was used to assess whether the needs established by the CI user in everyday telephone use situations improved after training. The auditory abilities training resulted in a relevant improvement in the percentage of correct answers in speech tests both in the telephone use conditions and in the sound field condition. Conclusion The results obtained with the COSI inventory indicated a performance improvement in all situations presented at the beginning of the program.</p></sec>

OBJETIVO: Avaliar a qualidade de vida de crianças usuárias de implante coclear sob a perspectiva de seus pais.MÉTODOS: Estudo clínico e transversal com 15 pais de crianças usuárias de implante coclear, de ambos os gêneros e com idades entre 2 e 12 anos. Os pais responderam ao questionário "Crianças com Implante Coclear: Perspectiva dos Pais" (CCIPP). Foram levantados, nos prontuários das crianças, os dados referentes à categoria de audição e tempo de uso do dispositivo. Os percentuais dos domínios do CCIPP foram tabulados e analisados de forma descritiva e inferencial.RESULTADOS: Os domínios do questionário de qualidade de vida que apresentaram maior média foram autoconfiança (58,9%) e relações sociais (56,7%). Não foi verificada correlação entre o tempo (meses) de uso do implante coclear e os domínios do questionário. No entanto, as crianças com tempo de uso inferior ou igual a 24 meses apresentaram maior percentual no domínio comunicação do que aquelas com tempo de uso superior a 24 meses. Houve correlação negativa entre a categoria de audição e o domínio efeitos do implante.CONCLUSÃO: Na perspectiva dos pais, o uso do implante coclear melhora a qualidade de vida de seus filhos. Quanto menor o tempo de uso do implante coclear, maior o percentual da qualidade de vida; e, quanto mais desenvolvida a habilidade auditiva, menor o percentual de melhora da qualidade de vida.

PURPOSE:To evaluate the quality of life of children with cochlear implants from the perspective of their parents.METHODS:A clinical and cross-sectional study was conducted with 15 parents of children using cochlear implants of both genders aged between 2 and 12 years old. Parents of these children answered the questionnaire "Children with Cochlear Implants: Parental Perspective" (CCIPP). Data related to auditory category and time of cochlear implants use were collected from medical records of the children. The percentages of responses on the CCIPP domains were tabulated and descriptively and inferentially analyzed.RESULTS:The cochlear implants had a positive effect on the quality of life of children in the self-reliance (58.9%) and social relationships (56.7%) domains. No correlation was observed between the time of cochlear implants activation (months) and any of the CCIPP domains. However, children with 24 months or less of cochlear implant use presented higher percentages on the communication domain than those with more than 24 months of cochlear implants use. A negative correlation was observed between the auditory category and the effects of the implant domain.CONCLUSION:From the perspective of parents, the use of cochlear implants improves the quality of life of their children; the shorter the time of cochlear implants use, the higher the improvement in quality of life; and the more developed the auditory skills, the lower the percentage of quality of life improvement with the cochlear implants.

OBJETIVO:analisar a aquisição de vocábulos em um grupo de crianças usuárias de implante coclear, na fase inicial do desenvolvimento da língua oral.MÉTODOS:estudo do tipo clínico, prospectivo e longitudinal. Participaram cinco mães de crianças usuárias de implante coclear. As mães assinalaram os vocábulos produzidos espontaneamente pelas crianças, em situação natural, na Lista de Avaliação de Vocabulário Expressivo, adaptado do Language Development Survey. Esse instrumento foi administrado uma vez por mês, totalizando seis momentos por criança. Utilizou-se o teste-t pareado para realizar as comparações entre os momentos.RESULTADOS:a média dos vocábulos produzidos espontaneamente pelo grupo de crianças do primeiro para o sexto momento aumentou gradativamente: de 38,0 para 58,8 vocábulos. Houve diferença significante no número de vocábulos produzidos entre os Momento 1 e Momento 5 (p=0,016) e, entre Momento 1 e Momento 6 (p=0,010). As categorias mais produzidas pelas crianças foram: outros, pessoas, modificadores e ações. E, as categorias que mais se expandiram no período foram modificadores, partes de corpo e outros.CONCLUSÕES:o grupo de crianças usuárias de implante coclear que participaram deste estudo apresentou um aumento gradativo do número de vocábulos produzidos espontaneamente (principalmente outro, modificadores, pessoas e ações), num período de seis meses, sendo significativo após o quinto mês da intervenção fonoaudiológica e, conforme aumentou o tempo do programa de intervenção, o número de vocábulos aumentou com maior significância. Sugere-se aos fonoaudiólogos que administrem o formulário num intervalo de seis meses, por ser o ideal para evidenciar a aquisição lexical.

PURPOSE:to analyze word acquisition in early oral language development of a group of children with cochlear implants.METHODS:this consisted on a clinical, prospective and longitudinal study. Five mothers of children with cochlear implants participated. The mothers reported the words that were spontaneously produced by their children. The Language Development Survey adapted to Brazilian Portuguese was administered once a month, totaling six moments per child. The paired t-test was used on the longitudinal data analysis.RESULTS:the mean number of words spontaneously produced by the group of children gradually increased from the first to the sixth month: 38 to 58.8 words. There were significant differences in the number of words produced on Moment 1 and Moment 5 (p = 0.016) and between Moment 1 and Moment 6 (p = 0.010). The categories with the greatest number of words produced by children were other, people, modifiers and actions. The categories with the greatest increase were modifiers, body parts and other.CONCLUSIONS:the group of children with cochlear implants showed a gradual increase in the number of words spontaneously (mainly other, modifiers, people and actions) produced over a period of six months, being significant after the fifth month of Speech-Language therapy. The number of words produced increased with higher significance with longer intervention periods. It is suggested that Speech-Language Pathologists apply the formulary on a six-month interval as this is an ideal period to evidence lexical acquisition.

Introdução: A avaliação de potenciais evocados auditivos de longa latência em crianças usuárias de implante coclear tem se mostrado um método eficaz para avaliar a maturação cortical após estimulação elétrica. Objetivo: Analisar as modificações nos valores de latência do potencial evocado auditivo cortical antes e três meses após o uso do implante coclear. Material e método: Estudo de caso-controle em um grupo de cinco crianças usuárias do implante coclear que aguardavam a ativação dos eletrodos, e um grupo controle constituído por cinco crianças ouvintes. A avaliação eletrofisiológica da audição foi realizada por meio do registro dos potenciais evocados auditivos corticais em duas diferentes etapas: anterior à ativação do implante coclear e após três meses de adaptação. Resultados: Os resultados demonstraram diminuição significativa em relação ao tempo de latência do componente P1 no grupo estudo, cujos valores foram maiores daqueles do grupo controle. Quanto menor a idade na ativação, maior a redução no tempo de latência do componente P1. Conclusão: Modificações nas características dos potenciais evocados auditivos corticais podem ser observadas em crianças que recebem o implante coclear e estas modificações têm uma relação com a idade de intervenção, sugerindo uma rápida maturação das vias auditivas após estimulação elétrica.

Introduction: Evaluation of cortical auditory evoked potentials in children with cochlear implants has been proven to be an effective method for assessing cortical maturation after electrical stimulation. Objective: To analyze the changes in latency values of cortical auditory evoked potentials before and three months after cochlear implant use. Material and methods: This was a case-control study with a group of five children using cochlear implant awaiting activation of the electrodes, and a control group composed of five normal-hearing children. Auditory electrophysiological assessment was performed by the testing of the cortical auditory evoked potentials at two different periods: prior to cochlear implant activation and after three months of cochlear implant use. Results: A significant decrease in the latency time of the P1 component was observed after three months of stimulation via cochlear implant, whose values were higher than those from the control group. The younger the child was at electrode activation, the greater the reduction in latency of the P1 component. Conclusion: Changes in the characteristics of cortical auditory evoked potentials can be observed in children who receive cochlear implants; these changes are related to the age of intervention, suggesting a rapid maturation of the auditory pathways after electrical stimulation.

O objetivo do presente estudo foi analisar os resultados dos Potenciais Evocados Auditivos Corticais em crianças usuárias de Implante Coclear, por meio da revisão sistemática da literatura. Após formulação da pergunta da pesquisa e levantamento dos estudos em quatro bases de dados com os descritores: eletrofisiologia (electrophysiology), implante coclear (cochlear implantation), criança (child), plasticidade neuronal (neuronal plasticity) e audiologia (audiology), foram selecionados artigos (originais e completos) publicados entre 2002 e 2013 na língua portuguesa ou inglesa. Por meio disto, foram localizados 208 estudos; contudo, apenas 13 contemplaram os critérios estabelecidos e foram lidos na íntegra; foi realizada a extração de dados para análise da metodologia e conteúdo das pesquisas. Os resultados descritos sugerem rápidas modificações no componente P1 dos Potenciais Evocados Auditivos Corticais em crianças usuárias de implante coclear. Apesar dos poucos estudos sobre o tema, o implante coclear tem se mostrado capaz de gerar modificações efetivas nas vias auditivas centrais, principalmente em crianças implantadas antes dos 3 anos e 6 meses de idade.

The aim of this study was to analyze the findings on Cortical Auditory Evoked Potentials in children with cochlear implant through a systematic literature review. After formulation of research question and search of studies in four data bases with the following descriptors: electrophysiology (eletrofisiologia), cochlear implantation (implante coclear), child (criança), neuronal plasticity (plasticidade neuronal) and audiology (audiologia), were selected articles (original and complete) published between 2002 and 2013 in Brazilian Portuguese or English. A total of 208 studies were found; however, only 13 contemplated the established criteria and were further analyzed; was made data extraction for analysis of methodology and content of the studies. The results described suggest rapid changes in P1 component of Cortical Auditory Evoked Potentials in children with cochlear implants. Although there are few studies on the theme, cochlear implant has been shown to produce effective changes in central auditory path ways especially in children implanted before 3 years and 6 months of age.

OBJETIVO: Identificar e analisar os fatores que interferem na participação dos pais de crianças usuárias de implante coclear (IC) no processo (re)habilitação aurioral. ESTRATÉGIA DE PESQUISA: Formulação da pergunta da pesquisa, levantamento e seleção dos estudos em bases de dados, com os descritores cochlear implant (implante coclear) e parents (pais). CRITÉRIOS DE SELEÇÃO: Artigos originais e completos publicados na língua portuguesa brasileira ou inglesa, pais de crianças usuárias de IC como participantes diretos da pesquisa. ANÁLISE DOS DADOS: Leitura dos artigos na íntegra e extração de dados para a caracterização dos centros, da metodologia e conteúdo das pesquisas. RESULTADOS: Treze artigos foram selecionados, dos tipos transversal e caso-controle (técnica de entrevista). Foram identificados os fatores sobre a tomada de decisão em realizar ou não a cirurgia (conhecimento prévio sobre IC, expectativas dos resultados, qualidade e quantidade de informações, encaminhamentos realizados, aspectos biomédicos e éticos, inserção na reabilitação, contato com outros pais, apoio social e custos); os fatores preditivos relacionados aos resultados da reabilitação (tempo de uso diário do IC, modalidade aurioral, escola regular, comprometimentos associados, aspectos sócio-demográficos e efetividade do programa); os fatores relacionados aos benefícios e limitações do IC (modalidade comunicativa, evolução das habilidades auditivas e linguísticas, o aprendizado de uma segunda língua oral), bem como o grau de satisfação dos pais. CONCLUSÃO: O engajamento dos pais de crianças usuárias de IC na (re)habilitação aurioral depende de fatores relacionados a diferentes domínios, os quais os fonoaudiólogos devem compreender e considerar na elaboração do programa de intervenção.

PURPOSE: To identify and analyze factors that influence the participation of parents in the rehabilitation process of children with cochlear implants (CI). RESEARCH STRATEGY: Question formulation and articles selection in three databases using the following keywords: cochlear implant (implante coclear) and parents (pais). SELECTION CRITERIA: Complete original articles published in Brazilian Portuguese or English, with direct participation of parents of children with CI. DATA ANALYSIS: Articles were fully read. Data regarding characterization of the centers, research methodology and content were analyzed. RESULTS: Thirteen articles were selected based on the established criteria. The types of studies were cross-sectional and case-control (interview technique). The following influential factors were identified: pre-CI surgery factors (knowledge about CI, quality and quantity of information, specialist's advices, ethical and biomedical aspects, rehabilitation engagement, contact with experienced families, social service support and overall costs); rehabilitation aspects (CI use, oral communication modality, regular school, other disabilities, social and demographic aspects and rehabilitation program's effectiveness); other important influential processes (communication modality, auditory and language development, second oral language learning, as well as parent's behavior and satisfaction). CONCLUSION: The engagement of parents in the rehabilitation process of children with CI depends on several distinct influential factors which audiologists should understand and consider when elaborating a rehabilitation program.

INTRODUÇÃO: Quanto mais precoce o diagnóstico e intervenção da deficiência auditiva, menor será o impacto para o desenvolvimento das habilidades cognitivas, auditivas e de linguagem da criança. OBJETIVO: Caracterizar a idade no diagnóstico e no início da intervenção da perda auditiva e o acompanhamento de crianças atendidas em um serviço público de saúde auditiva brasileiro - Espaço Reouvir do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. MÉTODO: Estudo retrospectivo com informações de 166 prontuários de crianças no que se refere a: gênero; etiologia, tipo, grau e lateralidade da deficiência auditiva; idade do diagnóstico e da adaptação do Aparelho de Amplificação Sonora Individual (AASI) e acompanhamento no serviço. RESULTADOS: A amostra foi composta por 56% homens e 44% mulheres. A etiologia predominante foi a de origem multifatorial. A perda auditiva do tipo neurossensorial ocorreu em 88,6% dos casos. O grau de perda auditiva moderado foi o de maior ocorrência (30,7%), simetria entre as orelhas foi encontrada em 69,9% dos casos e perda auditiva unilateral em 2,4%. A idade média no diagnóstico foi de 5,46 anos e na intervenção de 6,86 anos. Um total de 96,98% das crianças já havia completado o processo de adaptação e 78,32% permaneciam em acompanhamento. CONCLUSÃO: O Programa Reouvir - HCFMUSP ainda recebe crianças, seja para o diagnóstico e/ou para a intervenção, de maneira tardia. Entretanto, ainda assim faz-se possível à realização do acompanhamento de um número significativo de crianças usuárias de AASI, possibilitando um processo de adaptação mais efetivo.

INTRODUCTION: The earlier the diagnosis and the intervention from the hearing impairment less will be the impact for the development of cognitive abilities, hearing and of speech from the child. OBJECTIVE: Characterize the age in the diagnosis and the beginning of intervention of the hearing loss and the accompaniment of the assisted children in a public Brazillian hearing health service - Espaço Reouvir from the Clinicas Hospital from the Medical School from the University of São Paulo. METHOD: Retrospective study with information from 166 medical records from children regarding the: gender; etiology, type, degree and laterality of the hearing impairment; age in the diagnosis and adaptation of the Hearing aids (HA) and accompaniment in the service. RESULTS: The sample was composed by 56% men and 44% women. The prevailing etiology was from multifactorial origin. The hearing loss from the neurosensory type occurred in 88,6% of the cases. The degree of moderate hearing loss was the most frequent (30,7%), symmetry in both ears was found in 69,9% of the cases and unilateral hearing loss in 2,4%. The average age in the diagnosis was of 5,46 years and in the intervention was of 6,86 years. A total of 96,98% of children had already completed the process of adaptation and 78,32% still remained in the accompaniment. CONCLUSION: The program Reouvir-HCFMUSP still receives children, both for diagnosis and or intervention in a late manner. However, still is possible the realization of the accompaniment of a significant number of users of the hearing aids, enabling a process of adaptation more effective.

A estimulação elétrica gerada pelo Implante Coclear (IC) pode ser capaz de melhorar a sincronia neural e contribuir para o desenvolvimento das habilidades auditivas de sujeitos portadores de Neuropatia Auditiva/Dessincronia Auditiva (NA/DA). OBJETIVO: Estudo de coorte prospectivo transversal para avaliar o desempenho auditivo e as características do Potencial de Ação Composto Eletricamente Evocado no Nervo Auditivo (ECAP) em 18 crianças portadoras de NA/DA e usuárias de IC. MATERIAL E MÉTODOS: Percepção auditiva e características do ECAP foram avaliadas respectivamente, por meio da determinação dos limiares tonais e testes de percepção de fala e pelas medidas de limiar e amplitude da resposta neural para as frequências de estimulação de 35 e 80Hz. Não foram encontradas diferenças estatisticamente significativas no desenvolvimento das habilidades auditivas entre os sujeitos avaliados. RESULTADOS: Não foram observadas modificações estatisticamente significativas nas características de limiar e amplitude do ECAP para as duas frequências de estimulação testadas. CONCLUSÕES: O IC caracterizou-se como um efetivo recurso para o desenvolvimento das habilidades auditivas em 94% dos sujeitos, sendo a estimulação elétrica capaz de compensar a alteração da sincronia neural decorrente da NA/DA. A avaliação criteriosa de cada caso de maneira específica deve ser realizada no momento anterior à indicação cirúrgica. Clinical Trials: NCT01023932

The electrical stimulation generated by the Cochlear Implant (CI) may improve the neural synchrony and hence contribute to the development of auditory skills in patients with Auditory Neuropathy/Auditory Dyssynchrony (AN/AD). AIM: Prospective cohort cross-sectional study to evaluate the auditory performance and the characteristics of the electrically evoked compound action potential (ECAP) in 18 children with AN/AD and cochlear implants. MATERIAL AND METHODS: The auditory perception was evaluated by sound field thresholds and speech perception tests. To evaluate ECAP's characteristics, the threshold and amplitude of neural response were evaluated at 80Hz and 35Hz. RESULTS: No significant statistical difference was found concerning the development of auditory skills. The ECAP's characteristics differences at 80 and 35Hz stimulation rate were also not statistically significant. CONCLUSIONS: The CI was seen as an efficient resource to develop auditory skills in 94% of the AN/AD patients studied. The auditory perception benefits and the possibility to measure ECAP showed that the electrical stimulation could compensate for the neural dyssynchrony caused by the AN/AD. However, a unique clinical procedure cannot be proposed at this point. Therefore, a careful and complete evaluation of each AN/AD patient before recommending a Cochlear Implant is advised.
