Abstract Background Migraine affects 1 billion people worldwide and > 30 million Brazilians; besides, it is an underdiagnosed and undertreated disorder. Objective The need to disseminate knowledge about the prophylactic treatment of migraine is known, so the Brazilian Headache Society (SBCe, in the Portuguese acronym) appointed a committee of authors with the objective of establishing a consensus with recommendations on the prophylactic treatment of episodic migraine based on articles from the world literature as well as from personal experience. Methods Meetings were held entirely online, with the participation of 12 groups that reviewed and wrote about the pharmacological categories of drugs and, at the end, met to read and finish the document. The drug classes studied in part II of this Consensus were: antihypertensives, selective serotonin reuptake inhibitors, serotonin and norepinephrine reuptake inhibitors, calcium channel blockers, other drugs, and rational polytherapy. Results From this list of drugs, only candesartan has been established as effective in controlling episodic migraine. Flunarizine, venlafaxine, duloxetine, and pizotifen were defined as likely to be effective, while lisinopril, enalapril, escitalopram, fluvoxamine, quetiapine, atorvastatin, simvastatin, cyproheptadine, and melatonin were possibly effective in prophylaxis of the disease. Conclusions Despite an effort by the scientific community to find really effective drugs in the treatment of migraine, given the large number of drugs tested for this purpose, we still have few therapeutic options.

Resumo Antecedentes Migrânea afeta um bilhão de pessoas em todo o mundo e mais de 30 milhões de brasileiros; além disso, é um distúrbio subdiagnosticado e subtratado. Objetivo Sabe-se sobre a necessidade de difundir o conhecimento sobre o tratamento profilático da migrânea; por isso, a Sociedade Brasileira de Cefaleias (SBCe) nomeou um comitê de autores com o objetivo de estabelecer um consenso com recomendações sobre o tratamento profilático da migrânea episódica com base em artigos da literatura mundial, assim como da experiência pessoal. Métodos As reuniões foram realizadas inteiramente online, com a participação de 12 grupos que revisaram e escreveram sobre as categorias farmacológicas dos medicamentos e, ao final, reuniram-se para a leitura e conclusão do documento. As classes de medicamentos estudadas na parte II deste Consenso foram: anti-hipertensivos, inibidores seletivos de recaptação de serotonina, inibidores de recaptação de serotonina e noradrenalina, bloqueadores dos canais de cálcio, outros medicamentos e politerapia racional. Resultados Desta lista de medicamentos, apenas o candesartan foi estabelecido como eficaz no controle da migrânea episódica. Flunarizina, venlafaxina, duloxetina e pizotifeno foram definidos como provavelmente eficazes, enquanto lisinopril, enalapril, escitalopram, fluvoxamina, quetiapina, atorvastatina, sinvastatina, ciproheptadina e melatonina foram possivelmente eficazes na profilaxia da doença. Conclusões Apesar do esforço da comunidade científica em encontrarmedicamentos realmente eficazes no tratamento da migrânea, dado o grande número de medicamentos testados para este fim, ainda dispomos de poucas opções terapêuticas.

Abstract The Brazilian Headache Society (Sociedade Brasileira de Cefaleia, SBCe, in Portuguese) nominated a Committee of Authors with the aim of establishing a consensus with recommendations regarding prophylactic treatment for episodic migraine based on articles published in the worldwide literature, as well as personal experience. Migraine affects 1 billion people around the world and more than 30 million Brazilians. In addition, it is an underdiagnosed and undertreated disorder. It is well known within the medical community of neurologists, and especially among headache specialists, that there is a need to disseminate knowledge about prophylactic treatment for migraine. For this purpose, together with the need for drug updates and to expand knowledge of the disease itself (frequency, intensity, duration, impact and perhaps the progression of migraine), this Consensus was developed, following a full online methodology, by 12 groups who reviewed and wrote about the pharmacological categories of the drugs used and, at the end of the process, met to read and establish conclusions for this document. The drug classes studied were: anticonvulsants, tricyclic antidepressants, monoclonal anti-calcitonin gene-related peptide (anti-CGRP) antibodies, beta-blockers, antihypertensives, calcium channel inhibitors, other antidepressants (selective serotonin reuptake inhibitors, SSRIs, and dual-action antidepressants), other drugs, and polytherapy. Hormonal treatment and anti-inflammatories and triptans in minimum prophylaxis schemes (miniprophylaxis) will be covered in a specific chapter. The drug classes studied for part I of the Consensus were: anticonvulsants, tricyclic antidepressants, monoclonal anti-CGRP antibodies, and beta-blockers.

Resumo A Sociedade Brasileira de Cefaleia (SBCe) nomeou um Comitê de Autores com o objetivo de estabelecer um consenso com recomendações sobre o tratamento profilático da enxaqueca episódica com base em artigos da literatura mundial e da experiência pessoal. A enxaqueca é um distúrbio subdiagnosticado e subtratado que acomete um bilhão de pessoas no mundo e mais de 30 milhões de brasileiros. É conhecido na comunidade médica de neurologistas e, sobretudo, dos especialistas em cefaleia, a necessidade de se divulgar o conhecimento sobre o tratamento profilático da enxaqueca. Com esta finalidade, aliada às necessidades de atualizações de drogas e de se aumentar o conhecimento sobre a doença em si (frequência, intensidade, duração, impacto e talvez a progressão da enxaqueca), foi elaborado este Consenso, com metodologia totalmente on-line, por 12 grupos que revisaram e escreveram sobre as categorias farmacológicas das drogas e, ao final, reuniram-se para a leitura e conclusão do documento. As classes de drogas estudadas para este Consenso foram: anticonvulsivantes, antidepressivos tricíclicos, anticorpos monoclonais do antipeptídeo relacionado ao gene da calcitonina (peptídeo relacionado ao gene da calcitonina — anti-CGRP), betabloqueadores, anti-hipertensivos, inibidores dos canais de cálcio, outros antidepressivos (inibidores seletivos de recaptação de serotonina, ISRSs, e antidepressivos de ação dual), outras drogas, e politerapia. O tratamento hormonal, bem como anti-inflamatórios e triptanas em esquema de profilaxia mínima (miniprofilaxia), será abordado em um capítulo próprio. As classes de drogas estudadas na parte I do Consenso foram: anticonvulsivantes, antidepressivos tricíclicos, anticorpos monoclonais anti-CGRP, e betabloqueadores.

RESUMO Introdução: A busca pela aprendizagem de técnicas cirúrgicas dentro da sala de operação está vinculada a dificuldades, como a redução do tempo de ensino pelos cirurgiões e problemas éticos. Já foram elaborados modelos para facilitar a prática de técnicas cirúrgicas, contudo de custo elevado, difícil acesso e com complicações éticas e morais. O presente trabalho tem como objetivo apresentar um modelo sintético, inédito e prático para o treinamento das técnicas de retalho cutâneo, formulado para ser de fácil reprodução e de baixo custo, permitindo sua exequibilidade. Métodos: No modelo foi utilizado malha, esponja para lavagem de carro, elástico de látex, pincel de ponta fina, bisturi e instrumentos cirúrgicos de sutura. A malha é fixada pelo elástico sobre a superfície da esponja, simulando pele e subcutâneo. Desenha-se, então, o retalho a ser feito na superfície do tecido. Resultados: O modelo criado mostrou-se satisfatório, visto que aprimora o manuseio de instrumentos cirúrgicos e o aprendizado da técnica de retalho proposta, além de ter demonstrado boa elasticidade e resistência a tração. Nas faculdades de medicina percebe-se uma carência na abordagem de temas importantes da cirurgia plástica. Ressalta-se a importância de modelos de baixo custo e de fácil execução, como o supracitado, para facilitar a aprendizagem de estudantes interessados no assunto, buscando cumprir a função educacional sem romper princípios éticos. Conclusão: O modelo proposto é uma excelente forma de treinamento por apresentar benefícios logísticos e instrutivos, facilitando a aprendizagem, sem causar prejuízo aos animais. ABSTRACT Introduction: The search for learning surgical techniques within the operating room is linked to difficulties, such as reducing teaching time by surgeons and ethical problems. Models have already been developed to facilitate the practice of surgical techniques, however, with high cost, difficult access, and ethical and moral complications. The present work aims to present a synthetic model, unpublished and practical for the training of skin flap techniques, formulated to be easy to reproduce and low cost, allowing its feasibility. Methods: In the model, fabric, sponge for car washing, latex elastic, fine-tipped brush, scalpel, and surgical suture instruments were used. The fabric is fixed by the elastic on the surface of the sponge, simulating skin and subcutaneous. The flap to be made on the surface of the fabric is then drawn. Results: The model created was satisfactory, since it improves the handling of surgical instruments and the learning of the proposed flap technique, besides having demonstrated good elasticity and tensile strength. In medical schools, there is a lack of approach to essential topics in plastic surgery. The importance of low-cost and easy-to-execute models, such as the above, is emphasized to facilitate the learning of students interested in the subject, seeking to fulfill the educational function without breaking ethical principles. Conclusion: The proposed model is an excellent form of training because it presents logistical and instructive benefits, facilitating learning, without causing harm to animals. Resumo Objetivo: avaliar critérios diagnósticos e propor pontos de corte para as circunferências da cintura (CC), do braço (CB) e da perna (CP) e relação cintura/altura (RCA), para a predição de excesso de peso em adolescentes. Métodos: censo com todos os indivíduos na idade de 13-19 anos (N=1.075) do município de Caracol-PI, Brasil; o excesso de peso foi definido como índice de massa corporal (IMC) acima de um escore-z, conforme critério da Organização Mundial da Saúde. Resultados: os pontos de corte (em centímetros) de CC, CB, CP e RCA para predizer excesso de peso foram, respectivamente, 76,4, 26,4, 34,0 e 0,460 para o sexo masculino, e 74,6, 27,0, 34,5 e 0,475 para o feminino; a área sob a curva ROC ficou acima de 0,70 (sexo masculino) e de 0,90 (sexo feminino). Conclusão: os indicadores antropométricos examinados atingiram valores diagnósticos satisfatórios para predição de excesso de peso em adolescentes do semiárido brasileiro.

Abstract Objective: to assess the diagnostic criteria and propose cutoff points for waist circumference (WC), arm circumference (AC), leg circumference (LC) and waist/height ratio (WHR) for overweight prediction in adolescents. Methods: a census with all individuals aged 13-19 years old (N=1,075) from the municipality of Caracol-PI, Brazil was carried out; overweight was defined as body mass index (BMI) above one z-score, according to the criterion of the World Health Organization. Results: the cutoff points (in centimeters) of WC, AC, LC and WHR for overweight prediction were, respectively, 76.4, 26.4, 34.0 and 0.460 in males, and 74.6, 27.0, 34.5 and 0.475 in females; the area under the ROC curve was above 0.70 (males) and 0.90 (females). Conclusion: the anthropometric indicators assessed reached satisfactory diagnostic values for overweight prediction in adolescents from the Brazilian semi-arid region.

Métodos: censo con todos los individuos de 13 a 19 años de edad (N=1.075) del municipio de Caracol-PI, Brasil; el exceso de peso se define como un índice de masa corporal (IMC) por encima de uno escore-z, conforme criterio de la Organización Mundial de la Salud. ABSTRACT Introduction The aim of this study was to determine the prevalence of GBS colonization in pregnant women in a public health service. Methods A study of 496 pregnant women at 35-37 gestational weeks was conducted from September 2011 to March 2014 in 21 municipalities of the 18th Health Region of Paraná State. Vaginal and anorectal samples of each woman were plated on sheep blood agar, and in HPTH and Todd-Hewitt enrichment broths. Results Of the 496 pregnant women, 141 (28.4%) were positive for GBS based on the combination of the three culture media with vaginal and anorectal samples. The prevalence was 23.7% for vaginal samples and 21.9% for anorectal ones. Among the variables analyzed in this study, only urinary infection was a significant factor (0.026) associated with GBS colonization in women. Conclusions Based on these results, health units should performs universal screening of pregnant women and hospitals should provide adequate prophylaxis, when indicated.

ABSTRACT The triterpene lupeol (1) and some of its esters are secondary metabolites produced by species of Celastraceae family, which have being associated with cytotoxic activity. We report herein the isolation of 1, the semi-synthesis of eight lupeol esters and the evaluation of their in vitro activity against nine strains of cancer cells. The reaction of carboxylic acids with 1 and DIC/DMAP was used to obtain lupeol stearate (2), lupeol palmitate (3) lupeol miristate (4), and the new esters lupeol laurate (5), lupeol caprate (6), lupeol caprilate (7), lupeol caproate (8) and lupeol 3’,4’-dimethoxybenzoate (9), with high yields. Compounds 1-9 were identified using FT-IR, 1H, 13C-NMR, CHN analysis and XRD data and were tested in vitro for proliferation of human cancer cell activity. In these assays, lupeol was inactive (GI50> 250µg/mL) while lupeol esters 2 -4 and 7 - 9 showed a cytostatic effect. The XRD method was a suitable tool to determine the structure of lupeol and its esters in solid state. Compound 3 showed a selective growth inhibition effect on erythromyeloblastoid leukemia (K-562) cells in a concentration-dependent way. Lupeol esters 4 and 9 showed a selective cytostatic effect with low GI50 values representing promising prototypes for the development of new anticancer drugs.

OBJETIVO: verificar o desempenho da sucção nutritiva, na mamadeira, em recém-nascidos pré-termo submetidos à estimulação sensório-motora-oral. MÉTODOS: pesquisa do tipo ensaio clínico controlado de intervenção. 20 recém-nascidos pré-termo foram distribuídos em grupo estimulado e grupo controle. Estes grupos foram submetidos à avaliação fonoaudiológica em dois diferentes momentos: na liberação da alimentação por via oral; e quando atingiam alimentação plena por via oral num período de 24 horas. As avaliações foram filmadas, e os resultados foram analisados pela pesquisadora. Para análise dos dados utilizou-se o Teste exato de Fishe r e o Teste "T" de Studen t do software estatístico STATA 10.0. Considerou-se p < 0,05. RESULTADOS: recém-nascidos do grupo estimulado, na segunda avaliação, apresentaram resultados com diferença estatística significante quanto a força de sucção forte (p=0,003), presença dos três reflexos adaptativos (p=0,001), coordenação da sucção/deglutição/respiração (p=0,003), no tempo total de sucções (p=0,01) e na frequência de sucção (p<0,01). O número total de sucções apresentou diferença estatística significante (p<0,05) entre as avaliações do grupo controle. Quanto ao ritmo e a relação entre o volume de leite prescrito e ingerido não apresentou resultados com diferença estatística significante. CONCLUSÃO: a estimulação sensório-motora oral parece favorecer o desempenho nas funções de sucção nutritiva de recém-nascidos pré-termo.

PURPOSE: to check the nutritive sucking performance in preterm infants that underwent sensory-motororal stimulation. METHODS: a controlled clinical trial of intervention was performed in 20 preterm infants divided into experimental and control group. The infants received a speech therapist evaluation in two different times: at the beginning of the oral feeding (first speech therapist evaluation) and when they reached full oral feeding (second evaluation). Statistical data analyses used STATA 10 package and they were analyzed for treatment group differences with Fisher's exact Test and Student's unpaired t-test (p<0.05). RESULTS: infants in the experimental group, in the second evaluation, differed statistically in the pressure of strong sucking (p=0.003), presence of the three adaptive reflexes (p=0.001), coordination among sucking, swallowing and breathing (p=0.003), the total time for sucking (p=0.01) and of number of sucking/time (p<0.01). The total number of sucks showed a statistically significant difference (p<0.05) among the evaluations of the control group. No differences were observed among the groups as for sucking rhythm, and prescribed and ingested milk volume. CONCLUSION: in this study, sensory-motor-oral stimulation contributed for a better sucking performance, during bottle feeding, in preterm infants.

OBJETIVOS: avaliar a influência da estimulação sensório-motora-oral (SMO) sobre o ganho de peso, tempo para realizar a transição da sonda para a via oral e, indiretamente, sobre a alta hospitalar de pré-termos. MÉTODOS: estudo de intervenção, do tipo ensaio clínico controlado, que incluiu 24 pré-termos, internados na UTI Neonatal do Hospital Universitário de Santa Maria, Rio Grande do Sul, Brasil, entre maio de 2007 e março de 2008, alocados, por sorteio, em grupo estimulado (GE) e controle (GC). A análise das diferenças entre os grupos foi testada utilizando o teste t-Student independente e exato de Fisher (valor de p<0,05). RESULTADOS: não houve diferença estatística para as medidas antropométricas, embora o incremento diário de peso tenha sido maior no GE (p=0,16). O GE realizou a transição completa da sonda para a via oral em média 1,6 dias antes que o GC, sendo que 83,3% das crianças do GE fizeram essa transição em até sete dias, enquanto apenas 38,9% do GC alcançaram esse prazo. Não houve diferença no tempo de permanência hospitalar entre os grupos (p=0,48). CONCLUSÕES: a estimulação SMO favoreceu uma transição mais rápida da sonda para a via oral, não comprometendo o ganho de peso de recém-nascidos pré-termo. Não foi possível observar sua influência sobre o tempo de permanência hospitalar.

OBJECTIVES: to evaluate the influence of sensory-motor-oral (SMO) stimulation on weight gain, the time taken to make the transition from tube-to oral feeding and, indirectly, on the duration of hospitalization of preterm infants. METHODS: a controlled clinical intervention study was carried out with 24 preterm infants hospitalized in the Neonatal ICU of the Santa Maria University Hospital in the State of Rio Grande do Sul, Brazil, between May 2007 and March 2008, divided randomly into a treatment (TG) and control group (CG). The analysis of the differences between the groups was tested using Student's independent t-test and Fisher's exact test (with a p value <0.05). RESULTS: there was no statistical difference in terms of anthropometrical measurements, although the daily weight gain was higher in the TG (p=0.16). The TG group made a full transition from tube-to oral feeding on average 1.6 days earlier than the CG, 83.3% of the children in the TG making the transition within seven days, compared to only 38.9% of the control group. There was no difference in terms of the length of stay in hospital between the two groups (p=0.48). CONCLUSIONS: SMO stimulation was likely to lead to a swifter transition from tube-to oral feeding, without any adverse effect on weight gain in preterm newborns. No influence on the length of stay in hospital was observed.

Neste trabalho um controlador preditivo não linear multivariável foi desenvolvido para um processo de fermentação alcoólica extrativa. O modelo interno do controlador foi representado por duas redes do tipo Functional Link (FLN), identificadas usando dados de simulação gerados a partir de um modelo validado experimentalmente. A estrutura FLN apresenta como vantagem o treinamento rápido e convergência garantida, já que a estimação dos seus pesos é um problema de otimização linear. Além disso, a eliminação de pesos não significativos gera modelos parsimoniosos, o que permite a rápida execução em algoritmos de controle preditivo baseado em modelo. Os resultados mostram que o algoritmo proposto tem grande potencial para identificação e controle de processos não lineares.

In this work a MIMO non-linear predictive controller was developed for an extractive alcoholic fermentation process. The internal model of the controller was represented by two MISO Functional Link Networks (FLNs), identified using simulated data generated from a deterministic mathematical model whose kinetic parameters were determined experimentally. The FLN structure presents as advantages fast training and guaranteed convergence, since the estimation of the weights is a linear optimization problem. Besides, the elimination of non-significant weights generates parsimonious models, which allows for fast execution in an MPC-based algorithm. The proposed algorithm showed good potential in identification and control of non-linear processes.
