A pressão positiva intratorácica pode levar a alterações hemodinâmicas com repercussão no compartimento intracraniano, alterando a pressão intracraniana e a pressão de perfusão cerebral. Esse efeito pode se tornar mais intenso quando utilizados elevados valores de pressão positiva expiratória final. OBJETIVO: Medir o impacto que diferentes valores de pressão positiva expiratória final causam na pressão intracraniana, na pressão de perfusão cerebral e pressão arterial média. MÉTODO: O estudo foi desenvolvido em uma unidade de terapia intensiva neurológica envolvendo 25 pacientes adultos com acidente vascular cerebral hemorrágico, ventilados mecanicamente no modo com controle pressórico de vias aéreas. Foram instituídos valores de pressão positiva expiratória final variando de 0 a 14 cmH2O, de forma aleatória através de sorteio, utilizando valores pares. A monitorização das variáveis estudadas ocorreu após cinco minutos em cada patamar de pressão positiva expiratória final. RESULTADOS: O incremento da pressão positiva expiratória final aumentou a pressão intracraniana, (p < 0,001) sem causar alteração estatisticamente significativa na pressão arterial média ou na pressão de perfusão cerebral. CONCLUSÃO: Na população estudada, de pacientes com acidente vascular cerebral hemorrágico, os achados mostraram que valores de pressão positiva expiratória final até 14 cmH2O, não alteram a pressão de perfusão cerebral e a pressão arterial média, aumentando a pressão intracraniana, porém sem relevância clínica

Positive intrathoracic pressure may cause hemodynamic changes, which can be transmitted to the cranial compartment, changing intracranial pressure and cerebral perfusion pressure. This can be increased when high positive end-expiratory pressure values are used. OBJECTIVE: To measure the impact of different positive end-expiratory pressure levels on intracranial pressure, cerebral perfusion pressure and mean blood pressure. METHOD: This study was conducted in a neurological intensive care unit and included 25 adult hemorrhagic stroke patients who were mechanically ventilated on airway pressure control mode. Patients were subjected to various positive end-expiratory values ranging between 0 and 14 cmH2O. The order of these values were randomized, and the variables were assessed five minutes after each new positive end-expiratory pressure level was initiated. RESULTS: Incremental positive end-expiratory pressures led to increased intracranial pressure (p < 0.001), however, no statistically significant changes were observed in mean blood pressure or cerebral perfusion pressure. CONCLUSION: In this population of patients with hemorrhagic stroke, positive end-expiratory pressure values up to 14 cmH2O did not alter cerebral perfusion pressure or mean blood pressure. Increased intracranial pressures were noted, although these elevations were not clinically significant

OBJETIVO: Descrever as alterações nas variáveis da espirometria e seus distúrbios; estudar a existência de associação entre a extensão da lesão pulmonar residual ao final do tratamento da tuberculose e a alteração da função pulmonar aferida pela espirometria. MÉTODOS: Estudou-se uma série de 96 pacientes com diagnóstico de tuberculose pulmonar, atendidos em três unidades de saúde de Região Metropolitana do Recife, durante o período de janeiro de 2003 a novembro de 2005. Foram analisados pacientes de ambos os sexos, com idade a partir de 15 anos, cujas radiografias do tórax do final do tratamento foram classificadas pelo critério da National Tuberculosis Association (NTA). Os pacientes responderam questionário, no início da pesquisa, e foram submetidos à prova de função pulmonar, após o término do tratamento. RESULTADOS: Dos 96 pacientes estudados, concluímos que 89,6% apresentavam seqüelas radiográficas; 54% apresentavam seqüelas moderadas a graves. Estas alterações radiográficas correspondiam a 24,6% e 73,8%, respectivamente, de alterações na função pulmonar. CONCLUSÃO: Foram identificadas 89,6% de lesões residuais radiográficas, enquanto 66,7% das disfunções respiratórias alertam para o fato de que a assistência ao paciente com tuberculose pulmonar não deve se restringir apenas à cura baciloscópica da doença. A identificação de lesões pulmonares residuais e disfunções respiratórias em pacientes que concluem o tratamento da tuberculose permitirão instituir medidas precoces de tratamento dessas seqüelas para uma melhor qualidade de vida.

OBJECTIVE: To describe alterations in spirometric variables and its disturbances and to study the existence of a relationship between extent of the residual lung lesion at the end of treatment for tuberculosis and alteration of the lung function measured by spirometry. METHODS: Analyzed were 96 patients with diagnosis of pulmonary tuberculosis, treated in three health centers in the metropolitan area of Recife, from January, 2003 to November, 2005. Patients of both genders, 15 years of age or older were included, whose chest x-rays at end of the treatment were classified according to criteria of the National Tuberculosis Association (NTA) for extent of lung lesion. Patients replied to a questionnaire at the beginning of the survey, and were submitted to spirometry after conclusion of treatment. RESULTS: Of the 96 patients, 89.6% presented with radiographic sequels. About 54% had moderate to severe sequels. These radiographic alterations correspond to 24.6% and 73.8% respectively of alterations in the pulmonary function. CONCLUSION: The large number of residual radiographic lesionss (89.6%) and breathing dysfunction (66,7%) identified in this survey call attention to the fact that treatment of a patient with pulmonary tuberculosis must not be restricted to bacteriological healing of the disease. Identification of the residual lung lesions and of the breathing dysfunction in patients who conclude treatment will promote early actions to treat these sequels, improving the quality of life of these patients.

Este estudo teve como objetivo analisar características clínicas, epidemiológicas e bacteriológicas de 60 pacientes com tuberculose pulmonar infectados pelo HIV(+) comparando-as às de 120 pacientes com TB pulmonar/HIV(-). Analisou-se as características em relação ao sexo, idade, hábitos de vida, antecedentes de contato ou tratamento anterior para tuberculose, características clínicas e bacteriológicas e resultado do tratamento. Os doentes co-infectados foram predominantemente do sexo masculino (p=0,001), referiram com maior freqüência perda >10 quilos (p<0,001), apresentaram maior freqüência de teste tuberculínico não reatores (p<0,001), baciloscopia negativa (p=0,001) e cultura de escarro negativa (p<0,001). O insucesso do tratamento foi mais significante naqueles HIV(+) (p<0,001). Não se encontrou uma maior freqüência de resistência aos tuberculostáticos associada à co-infecção (p=0,407). A tuberculose extrapulmonar associada à tuberculose pulmonar foi mais freqüente nos soropositivos que nos HIV (-), 30% e 1,6% respectivamente. Estes achados evidenciam predominância de características clínico-laboratoriais atípicas nos pacientes com co-infecção, alertando para a possibilidade deste diagnóstico.

The aim of this study was to analyse the clinical, epidemiological and bacteriological features present in 60 pulmonary tuberculosis patients who were also infected with human immunodeficiency virus (HIV) and to compare these with 120 TB patients who were not infected with HIV. The patients with pulmonary tuberculosis and HIV coinfection were mostly male (p = 0.001), showed a higher frequency of weight loss >10 kilos (p <0.001), had a higher rate of non-reaction result to the tuberculin skin test (p <0.001), a higher frequency of negative sputum smear examination for acid-fast bacilli (p = 0.001) and negative sputum culture for Mycobacterium tuberculosis (p = 0.001). Treatment failure was more common in those who were HIV positive (p <0.000). No higher frequency of resistance to antituberculosis drugs was found to be associated with TB/HIV coinfection (p = 0.407). Association between extrapulmonary and pulmonary tuberculosis was more frequent in those seropositive to HIV than those without HIV virus, 30% and 1.6% respectively. These findings showed a predominance of atypical clinical laboratory features in co-infected patients, and suggest that health care personnel should consider the possibility this diagnosis.

Este trabalho objetivou comparar características radiológicas de doentes com tuberculose (TB) pulmonar, soropositivos para o vírus da imunodeficiência humana (HIV), com aquelas de doentes HIV-. Estudou-se, prospectivamente, 275 doentes com TB pulmonar, atendidos no Hospital das Clínicas da UFPE, entre janeiro de 1997 e março de 1999. Destes, 39 (14,2%) eram HIV+, com um predomínio maior de homens neste grupo (p=0,044). setenta e cinco por cento dos doentes HIV+ e 19% dos HIV- apresentavam reação negativa ao teste tuberculínico (PPD) (p<0,001). A proporção de baciloscopias positivas nos dois grupos foi semelhante. A característica radiológica mais fortemente associada à co-infecção tuberculose pulmonar/HIV+ foi a ausência de cavidades (p<0,001). Conclui-se que, diante de doentes com TB pulmonar, a ausência de cavidades na telerradiografia de tórax constitui-se num indicador útil da necessidade de se investigar a infecção pelo HIV. Esta conduta pode contribuir para aumentar a efetividade dos serviços locais de saúde, permitindo oferecer tratamento adequado a estes doentes.

This study aimed to compare the radiographic characteristics of patients with pulmonary tuberculosis (TB) and human immunodeficiency virus (HIV) infection with those of HIV-negative patients. In all, 275 TB patients attending the outpatients clinics at the University Hospital/UFPE, were studied from January 1997 to March 1999. Thirty nine (14.2%) of them were HIV+, with a higher frequency of males in this group (p=0.044). Seventy-five percent of the HIV+ patients and 19% of the HIV- had a negative tuberculin test (PPD) (p < 0.001). The proportion of positive sputum smears in the two groups was similar. The radiological finding most strongly associated with co-infection was absence of cavitation (p < 0.001). It may therefore be concluded that the lack of cavitation in patients with pulmonary TB may be considered a useful indicator of the need to investigate HIV infection. This approach could contribute to increasing the effectiveness of local health services, by offering appropriate treatment to co-infected patients.

Objetivos. O objetivo deste estudo caso-controle foi identificar fatores prognósticos para desfecho do tratamento da tuberculose pulmonar em 297 pacientes (Hospital das Clínicas, Universidade Federal de Pernambuco, Brasil) entre 1994 e 1999. Materiais e métodos. Foram considerados casos indivíduos com alta por óbito, abandono ou falência do tratamento. Os controles foram indivíduos com alta por cura. Foram realizadas análises uni e multivariada com as variáveis independentes sexo; idade; escolaridade; hábito de fumar; hábito de ingerir álcool; tratamento anterior para tuberculose; resposta ao teste tuberculínico; sorologia para HIV; grau de resistência aos antimicrobianos; resultado da pesquisa direta de bacilos álcool-ácido-resistentes; esquema terapêutico utilizado. Além disso, repetiram-se as análises uni e multivariada considerando como casos apenas os óbitos e os indivíduos com falência do tratamento. Resultados. A ingestão excessiva de álcool (OR = 2,58; P = 0,014), a co-infecção pelo HIV (OR = 3,40; P = 0,028), o a tratamento anterior para tuberculose (OR = 4,89; P < 0,001) e resistência a duas ou mais drogas antituberculose (OR = 3,49; P = 0,017) foram fatores de risco para o insucesso do tratamento. Na segunda análise multivariada, excluindo os casos de abandono, não houve associação entre a o ingestão excessiva de álcool e desfecho do tratamento, mantendo-se as demais associações, o que sugere uma estreita relação entre o abandono do tratamento e o etilismo. Conclusões. Os fatores prognósticos para insucesso do tratamento da tuberculose pulmonar entre os indivíduos estudados estão interrelacionados, sendo de natureza biológica, clínica e social. Devem ser identificados no início do tratamento para que sejam implementados procedimentos diferenciados de acompanhamento, tais como tratamento diretamente supervisionado, de forma a fortalecer o controle da tuberculose em nível local.

Objective. The objective of this case-control study was to identify prognostic factors for the outcome of pulmonary tuberculosis among 297 patients who were treated between 1994 and 1999 at the Federal University of Pernambuco Clinics Hospital, in Recife, Pernambuco, Brazil. Methods. The cases were defined as individuals whose treatment ended in death, dropout, or failure. The controls were persons whose treatment ended in cure. The following independent variables were submitted to uni- and multivariate analyses: sex, age, schooling, being a smoker, consuming alcohol, previous treatment for tuberculosis, response to the tuberculin test, HIV serology, resistance to antimicrobial agents, results of direct investigation of alcohol- and acid-resistant bacilli, and treatment approach used. After that, the uni- and multivariate analyses were repeated, including as cases only deaths and individuals with treatment failure. Results. Four risk factors for tuberculosis treatment failure were found: excessive alcohol intake (odds ratio (OR) = 2.58; P = 0.014), co-infection with HIV (OR = 3.40; P = 0.028), previous tuberculosis treatment (OR = 4.89; P < 0.001), and resistance to two or more antituberculosis drugs (OR = 3.49; P = 0.017). In the second multivariate analysis, which excluded dropout cases, no association was found between treatment outcome and excessive alcohol consumption, but the other associations remained. This result suggests a close relationship between alcoholism and treatment dropout. Conclusions. In the group that was studied the prognostic factors for failure of pulmonary tuberculosis treatment were interrelated and were of a biological, clinical, and social character. These factors should be identified at the beginning of treatment in order to allow implementation of specific follow-up procedures such as the strategy of directly observed treatment. This would strengthen tuberculosis control at the local level.

Objetivo: Este estudo descreve as características e analisa o acompanhamento de casos de retratamento ocorridos em residentes no município do Recife, no ano de 1997, com o objetivo de avaliar a importância deste grupo de doentes para a vigilância da tuberculose nos sistemas locais de saúde. Casuística e métodos: Foi realizado um estudo transversal com uma amostra de 240 casos de retratamento que representaram 16,2% dos casos de tuberculose notificados à Secretaria Estadual de Saúde do Estado de Pernambuco, em 1997. Resultados: O abandono do tratamento anterior foi o principal motivo para o retratamento (55,8%), seguido por casos de recidiva da doença (39,2%). Os doentes eram predominantemente do sexo masculino, com idade entre 30 e 49 anos, com baixa ou nenhuma escolaridade, e apresentavam um predomínio da forma pulmonar. Chama a atenção que 42,7% dos casos de retratamento tinham história de dois ou mais tratamentos anteriores. Em relação ao desfecho do retratamento, 52,5% dos casos apresentaram resultado desfavorável. Entre estes, 44,2% dos indivíduos abandonaram o esquema terapêutico e 39,6% deles fizeram-no até o primeiro mês. Os casos que voltaram a tratar-se após recidiva apresentaram maior percentual de desfecho favorável de retratamento (64%) quando comparados com os casos que voltaram a tratar-se após falência ou abandono do tratamento anterior e esta diferença foi estatisticamente significante (chi² = 19,55; p < 0,01). Entre os casos da forma pulmonar de tuberculose, 27,1% reiniciaram a quimioterapia sem realizar a pesquisa do bacilo de Koch no escarro e apenas 5% realizaram cultivo com teste de sensibilidade. Encontrou-se ainda que 30,2% dos casos voltaram a tratar-se apenas com as drogas do esquema I. Conclusões: Os resultados encontrados apontam para a necessidade de implementar as atividades de assistência e seguimento dos casos de retratamento de tuberculose, de forma rotineira, no âmbito dos serviços de saúde. O tratamento com drogas adequadas e o acompanhamento dos doentes até a cura são métodos simples e baratos para prevenir o aparecimento de cepas de bacilos resistentes às drogas. Devem, portanto, ser entendidos como estratégias de fortalecimento do sistema de vigilância epidemiológica à tuberculose no nível local.

Objectives: This study describes the epidemiological profile of cases of repeated treatment of tuberculosis occurring among residents of the city of Recife in the year of 1997. Material and methods: A cross-sectional study was performed and 240 cases of repeated treatment were studied, representing 16.2% of all cases of tuberculosis. Results: Interruption of previous treatment was the principal reason for repeated treatments (55.8%), followed by cases of recurrence of the disease (39.2%). Patients undergoing repeated treatment for tuberculosis are characterized as predominantly male, ages between 30 and 49, with little or no school education, with a significant predominance of pulmonary tuberculosis. It is striking that 42.7% of the cases of repeated treatment had a history of two or more previous courses of treatment. With regard to the outcome of the repeated treatment, 52.5% of the cases showed unfavorable results. Among these, 44.2% of the patients gave up the course of treatment and 39.6% followed it for the first month. The cases that returned for treatment following recurrence showed a higher percentage of successful outcomes (56.7%), in comparison to those that returned after interrupting previous treatment (40.2%), this difference being statistically significant (chi² = 19,55; p < 0,01). It was found that, among the cases of pulmonary tuberculosis, 27.1% resumed chemotherapy without undergoing the Koch bacillus catarrh test and only 27.1% carried out BK culture with sensitivity test. Conclusions: The results show the need to enhance tuberculosis treatment and follow-up procedures within the sphere of the health care service. An adequate treatment is the cheapest method to avoid drug resistance and should be understood as a strategy to improve the tuberculosis surveillance system.
