Dois inquéritos sorológicos empregando testes imunoenzimáticos (ELISA) anti-Toxocara foram realizados em moradores da periferia de Campinas (SP, Brasil) em janeiro de 1999 e janeiro de 2000, envolvendo, respectivamente, 138 e 115 indivíduos escolhidos aleatoriamente. Dos 75 indivíduos examinados em ambas as ocasiões, 67 apresentaram resultados negativos em 1999, observando-se soroconversão em 12 no segundo inquérito. Tais resultados indicam taxa anual de incidência de 17,9% para infecção por Toxocara na região.

With the aim of estimating the incidence of infection by Toxocara among residents in the outskirts of Campinas (State of São Paulo, Brazil) two serological surveys, using ELISA anti-Toxocara tests, were performed in January 1999 and January 2000, involving, respectively, 138 and 115 individuals, 75 of which examined in both occasions. Among this group 67 individuals did not show the presence of anti-Toxocara antibodies in 1999, and 12 presented seroconversion in the second survey, revealing an annual incidence rate of 17.9%.

Estudou-se a ocorrência de infecção humana por Toxocara em três bairros da periferia do município de Campinas (Jardim Santa Mônica, Jardim São Marcos e Jardim Campineiro). Em 1999 sortearam-se aleatoriamente 40 domicílios e 138 moradores foram submetidos a inquérito soroepidemiológico, que incluiu coleta de sangue para teste imunoenzimático (E.L.I.S.A.) visando pesquisa de anticorpos anti-Toxocara e hemograma, além de aplicação de questionário semi-estruturado para avaliação de dados de interesse epidemiológico. Observou-se presença de anticorpos anti-Toxocara em 23,9% das amostras, sem ocorrência de diferença significativa quando se considerou a idade ou sexo dos indivíduos examinados. Em 57 amostras de solo da mesma região, coletadas em dezembro de 1998 e novamente em julho de 1999, estudou-se a contaminação ambiental por ovos de Toxocara, verificando-se sua presença em, respectivamente, 12,3% e 14,0% das amostras. Análises univariada e multivariada por regressão logística dos dados obtidos a partir dos questionários e dos resultados da sorologia anti-Toxocara mostraram influência significativa de variáveis de ordem sócio-econômica na prevalência de infecção humana por Toxocara, nas condições presentes na área estudada.

The occurrence of human Toxocara infection was evaluated in three neighborhoods of the periphery of the Campinas municipality (Jardim Santa Mônica, Jardim São Marcos and Jardim Campineiro) in 1999. Forty residences and 138 residents were randomly selected by drawing lots and were submitted to a seroepidemiological survey, which included blood collection for the immunoenzymatic detection (ELISA) of anti-Toxocara antibodies and a blood count, and the application of a semi-structured questionnaire for the evaluation of epidemiological data. Significant levels of anti-Toxocara antibodies were detected in 23.9% of the 1999 samples. No significant difference in the frequency of infection according to age was observed. Environmental contamination with Toxocara eggs was observed in 12.3 and 14.0% of 57 soil samples collected in the same region in December 1998 and July 1999, respectively. Univariate analysis and multiple logistic regression of the data obtained from the questionnaires and of the results of the serological tests, suggest a significant influence of socioeconomic variables on the frequency of human infection with Toxocara under the conditions prevalent in the study area.

A gravidade da leptospirose parece estar aumentando principalmente quanto ao envolvimento pulmonar. Essa maior gravidade poderia ser decorrente de uma resposta imunológica mais intensa, já que tanto a insuficiência renal, quanto a plaquetopenia e as lesões pulmonares são encontradas na fase imune da doença. Para investigar esta hipótese foram detectados por ELISA anticorpos IgM, IgG e IgA específicos em amostras de sangue de 35 pacientes internados com síndrome de Weil colhidas a partir de cerca de 15 dias do início dos sintomas até 1 ano após a alta. Os pacientes foram divididos em 2 grupos conforme o seu título de IgG na primeira amostra, colhida com até 2 semanas de sintomas: grupo 1 (n = 13) com título >1:400 (positivo) e grupo 2 (n = 22) com título <= 1:400 (negativo). Os anticorpos IgG no grupo 1 na primeira amostra mostraram alta avidez pelo antígeno o que sugere reinfecção. Mesmo em amostras mais tardias os títulos de IgG e IgA no grupo 1 foram mais elevados que no grupo 2. Os pacientes do grupo 1 eram mais idosos, permaneceram com febre por mais tempo e tiveram maior comprometimento renal e pulmonar. Estes resultados sugerem que a gravidade da leptospirose pode estar associada à intensidade da resposta humoral.

Leptospirosis severity may be increasing, with pulmonary involvement becoming more frequent. Does this increase result from an intense immune response to leptospire? Notice that renal failure, thrombocytopenia and pulmonary complications are found during the immune phase. Thirty-five hospitalized patients with Weil's disease had 5 blood samples drawn, from the 15th day to the 12th month of symptoms, for ELISA-IgM, -IgG and -IgA specific antibody detection. According their 1st IgG titer, the patients were divided into: group 1 (n = 13) titer > 1:400 (positive) and group 2 (n = 22) titer <=1:400 (negative). Early IgG antibodies in group 1 showed high avidity which may indicate reinfection. Group 1 was older, had worse pulmonary and renal function, and fever for a longer period than group 2. Throughout the study, IgG and IgA titers remained higher in group 1. In conclusion, the severity of Weil's disease may be associated with the intensity of the humoral immune response to leptospire.

Seropositivity for Chagas disease was evaluated in 834 children aged between 7 and 14 from the Municipal Teaching System in the district of Londrina, State of Paraná. A seroprevalence rate of 0.1% was found through the use of an indirect immunofluorescent test and an enzyme-linked immunosorbent assay. This low rate of seroprevalence provides evidence that the vectorial transmission of Chagas disease has been eliminated in Londrina. The main reason for the elimination of vectorial transmission of Trypanosoma cruzi infection, as evaluated by serological tests, may be a remarkable change in the economic structure of the northern region of Paraná in the 1960's. At that time coffee production was almost completely replaced by soy beans, wheat and grazing in the rural areas. This change deeply affected the rural ecology and caused an exodus of the population from rural to urban areas as well as a decrease in the total number of the population of that region. The measures introduced for controlling the disease through the Program of Chagas Disease Control established by the Fundação Nacional de Saúde of the Brazilian Ministry of Health, certainly, had a positive impact on the reduction of American trypanosomiasis prevalence in the area under study. However, it does not seem that this was the most relevant factor responsible for the elimination of vectorial transmission of Chagas disease in Londrina.

Visceral Larva Migrans syndrome (VLM) results from the presence or migration of helminth larvae in humans, who nonetheless only play the role of paratenic hosts in the helminths' life cycle. In humans, VLM can be caused by larvae of various nematode species, chiefly those of the ascarid Toxocara canis, which can then be found at a variety of body sites, such as the liver, lungs, heart, and brain. Clinical and pathological manifestations depend primarily on larvae number and location, infection duration, reinfection occurrence, and host's immunological condition. Signs and symptoms may range from asymptomatic infection to severe disease. In humans, infection is acquired through ingestion of T. canis eggs present in soil, containing larvae in the infective stage7, 8, 9. Indeed, eggs of Toxocara sp. have been found in sandboxes in several public places in the city of Campo Grande, Mato Grosso do Sul state2. This study was carried out to detect the presence of anti-Toxocara antibodies in children attending the Pediatrics division of Hospital Universitário of Universidade Federal de Mato Grosso do Sul at Campo Grande, Brazil. Over the years 1992-94, 454 serum samples, obtained from children of 5.25 ± 3.28 years of mean age and selected at that hospital on the basis of eosinophil count greater than or equal to 1000/mm3 of blood, were tested for the presence of antibodies by means of the ELISA technique employing Toxocara canis larvae excretory-secretory antigens5. A high prevalence rate for toxocariasis (35.55%) was found, which was observed to be associated with eosinophil levels lower than those usually reported in literature. Furthermore, a higher frequency of positive serology among boys was also observed (13 cases in contrast to only 3 among girls), a result also reported by other authors

Foi desenvolvido o teste dot-ELISA para detecção de anticorpos em líquido cefalorraquiano (LCR) no diagnóstico imunológico da neurocisticercose humana, utilizando antígenos de membrana e escólex de Cysticercus cellulosae (M+E-Cc) e, alternativamente, membrana (M) e líquido vesicular (LV) de Cysticercus longicollis (Cl) covalentemente ligados a um novo suporte constituído de tecido de poliéster-resina de N-metilol-acrilamida (dot-TR). O teste foi realizado à temperatura ambiente, com tempos de incubação reduzidos e sem necessidade de cuidados na manipulação do suporte. A sensibilidade obtida foi de 95,1% para o antígeno Cc e 97,6% para o Cl. A especificidade foi de 90,6% quando o antígeno Cc foi usado, e 96,9% e 100% para M-Cl e LV-Cl, respectivamente. Não foi observada diferença significativa entre os antígenos homólogo e heterólogo. O baixo custo e a fácil execução do teste dot-TR empregando extratos antigênicos de Cysticercus longicollis indicam que pode ser empregado como alternativa no imunodiagnóstico da neurocisticercose humana.

A dot-ELISA was developed for the detection of antibodies in CSF in the immunologic diagnosis of human neurocysticercosis, using antigen extracts of the membrane and scolex of Cysticercus cellulosae (M+S-Cc) and, alternately, membrane (M) and vesicular fluid (VF) of Cysticercus longicollis (Cl) covalently bound to a new solid phase consisting of polyester fabric treated with N-methylol-acrylamide resin (dot-RT). The test was performed at room temperature, with reduced incubation times and with no need for special care in the manipulation of the support. The sensitivity rates obtained were 95.1% for antigen Cc and 97.6% for antigen Cl. Specificity was 90.6% when Cc was used, and 96.9% and 100% when M-Cl and VF-Cl were used, respectively. No significant differences in titer were observed between tests carried out with homologous and heterologous antigens. The low cost and easy execution of the dot-RT test using antigen extracts of Cysticercus longicollis indicate the test for use in the immunodiagnosis of human neurocysticercosis.

Reagente novo destinado ao teste de hemaglutinação indireta, THAI IAL, foi padronizado, utilizando hemácias de ganso, como suporte inerte, para o diagnóstico de campo da tripanosomíase americana. O objetivo foi o de substituir o reagente liofilizado ou congelado de THAI produzido rotineiramente, empregando hemácias humanas, no Instituto Adolfo Lutz (São Paulo, Brasil). O reagente padronizado apresentou longa estabilidade em suspensão líquida, e foi avaliado em 137 amostras de soros de pacientes chagásicos e não chagásicos, em THAL IAl. O desempenho diagnóstico deste teste foi semelhante ao de THAI utilizando hemácias humanas e ao de THAl de procedência comercial. A sensibilidade foi 1,00, especificidade 0,98, valores de preditivo positivo 0,96, e negativo 1,00. As diferentes partidas de reagente sucessivamente produzidas demonstraram ser reprodutíveis em método de controle de qualidade. O novo reagente é mais econômico que o anterior, de fácil preparo e aplicável aos estudos soroepidemiológicos.

A new reagent was designed to the indirect hemagglutination test (IHAT IAL), utilizing goose red blood cells as inert matrix and standardized for the field diagnosis of American trypanosomiasis. The objective was to substitute the tyophilized or frozen reagent of IHAT produced routinely using human erythrocytes in the Adolfo Lutz Institute (São Paulo/Brazil). The standardized reagent presented a long stability in liquid suspension, and was evaluated in 137 serum samples from patient with and without Chagas disease, by IHAT ILA. The diagnostic performance of this lest was similar to the IHAT utilizing human erythrocytes and to that of a commercial IHA Tkit. The sensitivity was 1.00, specificity 0.98, predictive value of positive 0.96 and of negative 1.00. Different batches of reagent successively produced proved to be reproducible in a quality control method. The new reagent is more economic than the former reagent, it can be produced easily and may be applicable to the seroepidemiologic studies.

Com a finalidade de melhorar o diagnóstico da leptospirose humana, padronizou-se o teste dot-ELISA para a pesquisa de anticorpos específicos da classe IgM na saliva. Empregaram-se amostras de saliva e soro coletadas simultaneamente de 20 pacientes com a forma ictero-hemorrágica da doença, de 10 pacientes com outras patologias e 5 controles negativos. Culturas de Leptos-pira em meio EMJH, dos sorovares: icterohaemorrhagiae, canicola, hebdomadis, brasiliensis e cynopteri, foram utilizadas como antígeno, na concentração proteica individual de 0,2 µg/µl, misturadas em volumes iguais. Na fase sólida do teste empregou-se tecido de po-liéster impregnado com resina de N-metilol-acrilamida. O volume do antígeno para cada teste foi de 1µl, o de saliva 8µl, o de conjugado anti-IgM humana marcada com peroxidase, de 30µl. A leitura foi visual, após revelação em solução cromógena de preparo recente. O suporte de tecido é de fácil obtenção e manuseio, diferente do clássico de membrana de nitrocelulose. A saliva pode ser coletada diretamente sobre o suporte, o que facilita o método e diminui gastos e riscos inerentes à coleta e processamento do sangue.

In order to improve the diagnosis of human leptospirosis, we standardized the dot-ELISA for the search of specific IgM antibodies in saliva. Saliva and serum samples were collected simultaneously from 20 patients with the icterohemorrhagic form of the disease, from 10 patients with other pathologies and from 5 negative controls. Leptospires of serovars icterohaemorrhagiae, canicola, hebdomadis, brasiliensis and cynopteri grown in EMJH medium and mixed together in equal volumes, were used as antigen at individual protein concentration of 0.2 µg/µl. In the solid phase of the test we used polyester fabric impregnated with N-methylolacrylamide resin. The antigen volume for each test was 1µl, the saliva volume was 8 µl, and the volume of peroxidase-labelled anti-human IgM conjugate was 30 µl. A visual reading was taken after development in freshly prepared chromogen solution. In contrast to the classic nitrocellulose membrane support, the fabric support is easy to obtain and to handle. Saliva can be collected directly onto the support, a fact that facilitates the method and reduces the expenses and risks related to blood processing.

Foram estudadas 37 amostras de sangue de pacientes com leptospirose, forma icterohemorrágica, com intervalo de tempo de 2 a 12 dias entre o início dos sintomas e a coleta do material. Isolou-se leptospiras por hemocultura de 5 (13,5%) pacientes e em 4 destes, o agente etiológico pertencia ao sorogrupo Icterohaemorrhagiae sorovar copenhageni. O teste imunoenzimático ELISA-IgM apresentou reatividade em 35 (94,6%) pacientes, incluindo os 4 pacientes dos quais o agente etiológico foi isolado. Este teste demonstrou ser um importante recurso laboratorial para o diagnóstico da letospirose humana, mesmo no início da doença quando ainda na fase de leptospiremia.

Thirty-seven sera samples from patients with leptospirosis icterohaemorrhagic form were studied with a time interval of 2 to 12 days between the beginning of the symptoms and the collection blood samples. It was isolated leptospira of 5 patients' heinocultures (13.5%) and from 4 of these the etiological agent pertained to the serogroup Icterohaemorrhagiae serovar copenhageni. Thirty-five of them (94.6%), including the four patients whose the etiological agent was isolated, showed reactivity in the enzyme linked immunosorbent (ELISA) IgM test. By this way, it was demonstrated that this test is important for a rapid diagnosis of human leptospirosis, even in the beginning of the disease, when there is still leptospiraemic phase.

O teste imunoenzimático (ELISA) foi avaliado para detecção de anticorpos da classe IgA na leptospirose humana. Nas amostras de soro analisadas o teste demonstrou ser sensível e específico, quando comparado ao teste ELISA-IgM. Os resultados encontrados sugerem que os anticorpos da classe IgA aparecem tardiamente na leptospirose, e poderão ser um marcador sorológico evolutivo na progressão da doença

An Enzyme-linked immunosorbent assay ELISA was evaluated for the detection of IgA antibodies in the human leptospirosis. The assay proved to be sensitive and specific when compared with the ELISA-IgM, in the examinated serum samples. The results found suggest that IgA antibodies became positive later in leptospirosis, and will can be an evolutive indicator in the development of the disease

Padronizou-se o teste imunoenzimático dot-ELISA, empregando-se os antígenos somático (S) e excretor-secretor (ES) de Toxocara canis, para pesquisa de anticorpos específicos em 22 soros de pacientes com idades entre 5 a 15 anos, com dados clínicos de toxocaríase. Como grupo controle, foram estudados 14 soros de indivíduos supostamente normais e 28 soros de pacientes com outras patologias. Todas as amostras em estudo foram empregadas antes e após absorção com extrato de Ascaris suum. Os resultados obtidos foram avaliados comparativamente com o teste ELISA evidenciando, nos dois testes estudados, comportamento semelhante quanto à sensibilidade e maior especificidade para o dot-ELISA, com qualquer dos dois antígenos estudados. O teste dot-ELISA mostrou-se eficiente para o diagnóstico da toxocaríase humana, apresentando vantagens quanto ao rendimento, estabilidade, tempo e facilidade de execução e baixo custo.

The dot-enzyme-linked immunosorbent assay (dot-ELISA) was standardized using somatic (S) and excretory-secretory (ES) antigens of Toxocara-canis for the detection of specific antibodies in 22 serum samples from children aged 1 to 15 years, with clinical signs of toxocariasis. Fourteen serum samples from apparently normal individuals and 28 sera from patients with other pathologies were used as controls. All samples were used before and after absorption with Ascaris suum extract. When the results were evaluated in comparison with ELISA, the two tests were found to have similar sensitivity, but dot-ELISA was found to be more specific in the presence of the two antigens studied. Dot-ELISA proved to be effective for the diagnosis of human toxocariasis, presenting advantages in terms of yield, stability, time and ease of execution and low cost.

O teste ELISA-TETANOS (Biosys, France) foi utilizado para a titulação de antitoxina tetânica (sensibilidade = 0,0025 UI/ml) em soros humanos de doadores de sangue, 566 homens e 108 mulheres, idade entre 18 e 58 anos, média de 29 anos, residentes em São Paulo, Brasil. A Organização Mundial de Saúde aceita apenas o teste de soroneutralização em animais, método de referência, para os estudos de proteção contra o tétano, e preconisa o título mínimo protetor de 0,01 UI/ml. BOURLEAUD & HUET, propuseram o limite de 0,06 UI/ml quando se emprega o teste imunoenzimático ELISA, devido a discrepancias inevitáveis entre os dois métodos. Dos 674 soros estudados, 178 (26,41%) não apresentaram anticorpos detectáveis (< 0,0025 UI/ml); 413 (61,28%) apresentaram resultados iguais ou superiores a 0,01 UI/ml e, em 232 (34,42%) os títulos foram iguais ou superiores a 0,06 UI/ml. A porcentagem de indivíduos protegidos foi inversamente proporcional à idade: cerca de 50% do grupo mais jovem (homens de 18 a 35 anos e mulheres de 18 a 23 anos) contra 10% do grupo de mais de 42 anos apresentaram títulos seguramente protetores (> 0,06 UI/ml).

Le test ELISA-TÉTANOS (Biosys, France) a été utilisé pour le titrage des anti-corps tétaniques (sensibilité = 0,0025 UI/ml) en sérums humains de donneurs de sang, 566 hommes et 108 femmes, âges de 18 à 58 ans, moyenne de 29 ans, provenant de São Paulo, SP, Brésil. L'OMS, acceptant seulement la séroneutralisation sur souris (NT), la méthode de référence, pour les études sur la protection contre le tétanos, préconise le titre de 0,01 UI/ml comme minimum protecteur. BOURLEAUD & HUET ont proposé la limite de 0,06 UI/ml quand s'emploie le test ELISA, en attendant à une certaine discordance inévitable entre les méthodes. Parmi les 674 sérums étudiés, 178 (26,41%) n'ont pas présenté d'anticorps (< 0,0025 UI/ml); 413 (61,28%) ont présenté des résultats égaux ou supérieurs à 0,01 UI/ml et en 232 (34,42%) les titres ont été égaux ou supérieurs à 0,06 UI/ml. Le pourcentage d'individus protégés a été inversement proportionnel à 1'âge: environ 50% dans le groupe le plus jeune (hommes de 18 à 35 ans et femmes de 18 à 23 ans) contre environ 10% dans le groupe de plus de 42 ans ont présenté des titres sûrement protecteurs (> 0,06 UI/ml).

Relata-se a concomitância de leptospirose e síndrome da imunodeficiência adquirida (SIDA) em humanos. Nestes casos não foram observados aspectos clínicos diferentes que possam ser atribuídos à simultaneidade dessas patologias.

Two patients with concomitant leptospirosis and the acquired immunodeficiency syndrome (AIDS) are reported. In these cases different clinical aspects that can be attributed to simultaneity of these pathologies was not observed.

Foi desenvolvido o teste DOT-ELISA em tecido de poliéster tratado com resina polimerizada (NMA) para o imunodiagnóstico da neurocisticercose (NC) humana pesquisando anticorpos específicos em líquido cefalorraquiano (LCR). O teste pode ser empregado em levantamentos epidemiológicos e em laboratórios de média e baixa complexidade, já que todas as etapas são realizadas a temperatura ambiente, o antígeno é covalentemente ligado ao suporte permitindo obter um reagente estável, e o suporte sensibilizado não requer cuidados especiais no seu manuseio. Foram ensaiadas 30 amostras de LCR; todas as 15 amostras do grupo controle foram negativas e 14 (93,3%) das 15 amostras de LCR do grupo de pacientes com NC foram positivas (títulos de 1 a 128). Os estudos de sensibilidade, especificidade, reprodutibilidade, estabilidade e padronização do uso de soro como amostra estão em andamento.

A dot enzyme-linked immunosorbent assay (DOT-ELISA) was developed to detect specific antibodies in cerebrospinal fluid (CSF) for human neurocysticercosis immunodiagnosis, with Cysticercus cellulosae antigen dotted on a new solid-phase. This was represented by sheets of a synthetic polyester fabric impregnated with a polymerized resin (N-methylol-acrylamide). A very stable preparation was thus obtained, the antigen being covalently bound by cross-linking with free N-methylol groups on the resin. Since robust, no special care was necessary for handling the solid-phase. The test could be performed at room-temperature. From 30 CSF samples assayed, 14 were positive, from a group of 15 cases of neurocysticercosis, with titers from 1 to 128; 15 other samples, from normals or other neurological diseases, were all negative. Test characteristics seem to indicate it as adequate for epidemiological surveys. A more detailed study on sensitivity, specificity, reproducibility and the use in serum samples is being conducted.

Realizou-se estudo comparativo de diferentes sorotipos de Leptospira interrogans utilizados na preparação de antígenos empregados no teste ELISA, para a detecção de anticorpos da classe IgM, em amostras de soro na fase precoce e tardia da leptospirose humana. Foram utilizados dez sorotipos, escolhidos entre os que apresentaram maior reatividade na soroaglutinação microscópica (SAM), na cidade de São Paulo. Os cinco sorotipos que apresentaram melhores resultados individualmente no teste ELISA-IgM (canicola, hebdomadis, icterohaemorrhagiae, cynopteri e brasiliensis), foram também estudados em mistura antigênica. Os antígenos não tratados apresentaram maior reatividade do que os antígenos tratados com Triton X - 100 (4%) à temperatura de 50ºC, durante 4 horas. O teste ELISA-IgM empregando os sorotipos não tratados, isoladamente, e em mistura antigênica, mostrou-se altamente sensível, podendo ser empregado como teste de triagem para o diagnóstico precoce da leptospirose humana. Outra aplicação do teste é permitir a detecção do início de situações epidêmicas ou de surtos, possibilitando acionar medidas de vigilância epidemiológica.

A comparative study among different serovars of Leptospira interrogans was performed in order to prepare antigens to detect IgM antibodies by ELISA in early and late phase of human leptospirosis. Ten serovars were chosen among the most prevalent detected by microscopic seroagglutination (SAM) in São Paulo city. Using ELISA-IgM five of them showed better results (canicola, hebdomadis, icterohaemorrhagiae, cynopteri and brasiliensis). These ones were also studied in a pool. The non-treated antigens showed higher reactivity than the Triton X-100 (4%/50ºC/4h). ELISA-IgM using individually or pool of non treated antigens proved to be reliable with high sensitivity and should be used for an earlier diagnosis of leptospirosis, as a trial test. Faster diagnostic elucidation can be useful to detect epidemic situations, so, allowing epidemiological surveillance interventions.
