Despite widespread enthusiasm for broader participation in health policy and programming, little is known about the ways in which multi-sector groups address the challenges that arise in pursuing this goal. Based on the experience of Peru's National Multi-sector Health Coordinating Body (CONAMUSA), this article characterizes these challenges and identifies organizational strategies the group has adopted to overcome them. Comprising nine government ministries, nongovernmental organizations, academia, religious institutions, and international cooperation agencies, CONAMUSA has faced three principal challenges: 1) selecting representatives, 2) balancing membership and leadership across sectors, and 3) negotiating role transition and conflict. In response, the group has instituted a rotation system for formal leadership responsibiliti es, and professionalized management functions; created electoral systems for civil society; and developed conflict of interest guidelines. This case study offers lessons for other countries trying to configure multi-sector groups, and for donors who mandate their creation, tempering unbridled idealism toward inclusive participation with a dose of healthy realism and practical adaptation.

A pesar del entusiasmo generalizado por la mayor participación en las políticas y programas sanitarios, poco se sabe sobre las formas de afrontar los retos que se plantean en la consecución de este objetivo por parte de los grupos multisectoriales. Este artículo parte de la experiencia de la Coordinadora Nacional Multisectorial en Salud del Perú (CONAMUSA) para caracterizar dichos retos e identificar las estrategias de organización que ha adoptado el grupo a fin de superarlos. CONAMUSA, formada por nueve ministerios del gobierno, organizaciones no gubernamentales, instituciones académicas, organizaciones religiosas y agencias de cooperación internacional, se ha enfrentado con tres retos fundamentales: 1) elegir a los representantes, 2) encontrar el equilibrio entre la representación de los miembros y el liderazgo en los distintos sectores y 3) negociar el cambio de roles y los conflictos. Para responder a estos retos el grupo ha establecido un sistema rotatorio para las responsabilidades formales de liderazgo y ha profesionalizado las funciones de gestión, se han creado sistemas electorales para la sociedad civil y se han elaborado pautas para los conflictos de intereses. Este estudio de casos aporta lecciones para otros países que estén tratando de configurar grupos multisectoriales, así como para los organismos de ayuda que dirigen su creación, suavizando los idealismos extremos con una dosis de realismo saludable y de adaptación práctica para lograr una participación inclusiva.

Age and growth for the scalloped hammerhead shark (Sphyrna lewini) were determined from opaque bands (OBs) on postcephalic vertebrae from 109 organisms (44 females, 52 cm to 276 cm total length (TL) and 65 males, 47 cm to 245 cm TL) obtained bimonthly from commercial fisheries off the southern coast of Sinaloa state (23°45'25"N and 106°05'15"W to 21°52'N and 105°54'W) from January 2003 to February 2005. The Bowker test of symmetry and the Index of Average Percent Error, suggest that this ageing method represents an unbiased and precise age assessment. Results show that immediately after birth (in summer), the first OB was formed and in the next winter showed the second OB. Later it was observed that two OBs were formed each year, one during summer and the other during winter, influenced by the sea surface temperature (SST). Based on the comparison of five back-calculation methods, the best methods were Fraser-Lee. The parameters of the von Bertalanffy growth function were, for females: L∞ = 376 cm, K = 0.1 year-1, t0 = -1.16 years, b = 3 and W∞ = 222 kg; for males: L∞= 364 cm, K = 0.123 year-1, t0 = 1.18 years, b = 3 and W∞ = 193 kg. The standard index growth (Φ') was 4.2 (s = 0.1). According to these results the largest sharks observed, a female of 280 cm TL would be 12.5 years old and a male of 281 cm TL would be 11 years old.

Se estimó la edad y el crecimiento del tiburón martillo (Sphyrna lewini) a través de bandas opacas (BOs) en las vértebras postcefálicas de 109 organismos (44 hembras, 52 cm a 276 cm de longitud total (LT) y 65 machos, 47 cm a 245 cm LT) colectados bimensualmente en la pesca comercial en la costa sur del estado de Sinaloa (23°45'25"N y 106°05'15"W a 21°52'N y 105°54'W) de enero 2003 a febrero 2005. La prueba de simetría de Bowker y el índice del error promedio porcentual sugieren que los métodos usados fueron los correctos para determinar la edad. Los resultados muestran que inmediatamente después de nacer (verano) se forma la primera BO y en el siguiente invierno aparece la segunda BO. Posteriormente se forman dos BOs anuales: una en verano y otra en invierno por la influencia de la temperatura superficial del mar (SST). Al comparar cinco diferentes métodos de retrocálculo, se encontró que el mejor método fue el de Fraser-Lee. Los parámetros de la ecuación de crecimiento de von Bertalanffy fueron, para hembras: L∞ = 376 cm, K = 0.1 años-1, t0 = -1.16 años, b = 3 and W∞ = 222 kg; y para machos: L∞ = 364 cm, K = 0.123 años-1, t0 = 1.18 años, b = 3 and W∞ = 193 kg. El índice estándar de crecimiento (Φ') fue 4.2 (s = 0.1). De acuerdo con estos resultados una hembra de 280 cm TL tendría 12.5 años de edad y un macho de 281 cm, 11 años de edad en promedio.

Objective: To evaluate use of the Internet for seeking sex partners and information on HIV and/or sexually transmitted infections (STIs) among people living with HIV/AIDS (PLWHA). Materials and methods: We interviewed consecutive PLWHA attending at Vía Libre, in Lima, Peru, between May and June 2004. Results: Of 100 PLWHA, (46 men who had sex with men [MSM], 18 heterosexual men and 36 women), 59% reported using the Internet in the last 12 months. Of these, the majority (73%) accessed the Internet at a public place; 16 (27,1%) reported having gone online to search for sexual partners; and five (8,5%) reported having had sex with a partner found over the Internet. All sex seekers were men, of them a greater percentage were MSM rather than heterosexual (94% vs. 6%, p= 0,032). All five respondents who reported having had sex with a partner found online were MSM. Of those who accessed the Internet in the past year, 76% (47/59) had used it to find information on HIV/AIDS, and 39% (23/59) had used it to seek information on other STIs. Conclusions: Among PLWHA interviewed more than half reported Internet access. More than a quarter -almost exclusively MSM- had used it to search for sex partners, however, the majority had used the Internet to look for information about HIV/AIDS. These findings suggest that the Internet offers a convenient tool to engage high-risk MSM in online HIV/STI prevention.

Objetivo: Evaluar el uso de Internet para buscar parejas sexuales e información sobre VIH/SIDA o infecciones de transmisión sexual (ITS) entre personas viviendo con VIH/SIDA (PVVS). Materiales y métodos: Entrevistamos a las PVVS que acudieron consecutivamente a Vía Libre en Lima Perú entre mayo y junio de 2004. Resultados: De las 100 PVVS (46 hombres que tienen sexo con hombres [HSH], 18 hombres heterosexuales y 36 mujeres), 59% reportó el uso de Internet en los últimos 12 meses. De ellos, la mayoría (73%) accedió a Internet en un lugar público; 16 (27,1%) reportaron haber buscado parejas sexuales en línea y cinco (8,5%) reportaron haber tenido sexo con una pareja que conocieron por Internet. Todas las PVVS que buscaron sexo en Internet fueron hombres, siendo el porcentaje de HSH significativamente mayor que la de heterosexuales (94% frente a 6%; p=0,032). Los cinco que reportaron haber tenido sexo con una pareja que conocieron en línea fueron HSH. De los que accedieron a Internet en el último año, 76% (45/59) lo hizo para buscar información sobre VIH/SIDA y 39% (23/59) para buscar información sobre otras ITS. Conclusiones: Entre las PVVS encuestadas, más de la mitad reportó acceso a Internet. La mayoría lo hizo para buscar información sobre VIH/SIDA y más de la cuarta parte - casi exclusivamente HSH - lo hizo para buscar parejas sexuales. Estos hallazgos sugieren que Internet es una herramienta conveniente para brindar mensajes de prevención en línea sobre VIH/ITS a HSH de alto riesgo.
