SUMMARY OBJECTIVE: Increasing evidence suggests that exercise programs are of great value in the rehabilitation and survivorship of patients with cancer. However, challenges remain regarding maintaining patients more physically active. This study aimed to evaluate the impact of a supervised exercise program on quality of life, fatigue, physical performance, and levels of physical activity of patients with cancer. METHODS: An observational longitudinal study, with a 1-year prospective follow-up, was developed. SETTING: This is a university-based outpatient rehabilitation program in a high-complexity cancer care center in Sao Paulo. RESULTS: After the program, patients showed a significant gain in quality of life (p<0.0001), physical performance (p<0.0001), and improvement in fatigue (p<0.0001). After 12 months, 81.1% of the patients remained active, and only 4.5% declared themselves to be sedentary. CONCLUSION: The results of this study confirm that exercise programs are an important tool in the rehabilitation of patients with cancer and that an initial supervised exercise program, in combination with follow-ups, can help increase the levels of physical activity of this population. Sendo assim, estes passaram por importantes adaptações durante a pandemia de COVID-19. Porém, não se sabe ao certo como os equipamentos de saúde adaptaram suas estratégias e quais foram os resultados. O estudo investigou como os atendimentos em reabilitação foram afetados durante a pandemia e quais foram as estratégias para a manutenção dos serviços prestados. Entre junho de 2020 e fevereiro de 2021, realizaram-se 17 entrevistas semiestruturadas com profissionais de saúde da área da reabilitação do Sistema Único de Saúde (SUS), que atuam em um dos três níveis de atenção, nas cidades de Santos e São Paulo, Estado de São Paulo, Brasil. Os discursos foram gravados, transcritos e analisados por meio da análise de conteúdo. Abstract: Several healthcare processes, including rehabilitation, require prompt initiation and cannot be interrupted. The professionals reported organizational changes in their services, with the initial interruption of appointments and, subsequently, the adoption of new sanitary protocols and the gradual return to in-person and/or remote appointments. Working conditions were directly impacted, as there was a need for staffing, training, increased workloads, as well as physical and mental exhaustion among professionals. The pandemic caused a series of changes in healthcare services, some of which were interrupted due to the suspension of numerous services and appointments. Some in-person appointments were maintained exclusively for patients who presented a risk of short-term deterioration. Preventive sanitary measures and strategies for continuity of care were adopted.

MÉTODOS Realizou-se uma revisão de escopo estabelecendo-se a perguntanorteadora: “Quais são as principais barreiras que as pessoas com deficiência enfrentam no acesso a serviços de saúde?” O levantamento dos artigos foi realizado em julho de 2019, em seis bases de dados de literatura científica. Dos 1.155 documentos identificados nas buscas, após seleção por título e resumo, foram lidas na íntegra 170 publicações e, após leitura, 96 artigos foram incluídos e categorizados conforme referencial teórico. RESULTADOS As principais barreiras indicadas pelos usuários do serviço foram: comunicação falha entre profissionais e paciente/cuidador; limitações financeiras; questões atitudinais/comportamentais; oferta de serviços escassa; barreiras organizacionais e de transporte. As principais barreiras apresentadas pelos prestadores de serviços foram: falta de treinamento/capacitação aos profissionais; falha do sistema de saúde; barreiras físicas; falta de recursos/tecnologia e barreiras de idioma. CONCLUSÕES Ficou evidente que as pessoas com deficiência enfrentam diversas barreiras ao tentarem acesso aos serviços de saúde de que necessitam e que usuários e profissionais de saúde têm visões distintas e complementares sobre as dificuldades.

Some patients with coronavirus disease (COVID-19) present with severe acute respiratory syndrome, which causes multiple organ dysfunction, besides dysfunction of the respiratory system, that requires invasive procedures. On the basis of the opinions of front-line experts and a review of the relevant literature on several topics, we proposed clinical practice recommendations on the following aspects for physiotherapists facing challenges in treating patients and containing virus spread: 1. personal protective equipment, 2. conventional chest physiotherapy, 3. exercise and early mobilization, 4. oxygen therapy, 5. nebulizer treatment, 6. non-invasive ventilation and high-flow nasal oxygen, 7. endotracheal intubation, 8. protective mechanical ventilation, 9. management of mechanical ventilation in severe and refractory cases of hypoxemia, 10. prone positioning, 11. cuff pressure, 12. tube and nasotracheal suction, 13. humidifier use for ventilated patients, 14. methods of weaning ventilated patients and extubation, and 15. equipment and hand hygiene. These recommendations can serve as clinical practice guidelines for physiotherapists. This article details the development of guidelines on these aspects for physiotherapy of patients with COVID-19.

RESUMO O objetivo desse estudo foi determinar o tempo de oclusão necessário para avaliar a pressão inspiratória máxima (PIMáx) obtida pelo método da válvula expiratória unidirecional em sujeitos sem via aérea artificial. Foram avaliados 31 sujeitos, com idade entre 18 e 60 anos. A PIMáx foi avaliada pelo método convencional (PIMáxconv) e pelo método da válvula expiratória unidirecional (PIMáxuni), sendo a ordem de avaliação definida por meio de sorteio. Para a medida da PIMáxuni, um manovacuômetro digital foi acoplado a uma válvula expiratória unidirecional e máscara orofacial por 20 segundos de oclusão. Nesse período, todos os sujeitos foram encorajados a realizar esforços inspiratórios máximos. Para definir a ótima duração da manobra, o tempo de esforço foi dividido a cada intervalo de 5 segundos (0-5s, 0-10s, 0-15s, 0-20s). Os intervalos de tempo para obtenção da PIMáxuni foram comparados por meio do teste de ANOVA One-way. Para comparação das médias dos valores de PIMáxconv e PIMáxuni, foi utilizado o teste t de Student. O nível de significância foi de 5%. A média dos valores da PIMáxconv foi de -102,5±23,9 cmH2O, enquanto que a PIMáxuni foi de -117,3±24,8 cmH2O (p<0,001). O valor absoluto máximo da PIMáxuni foi alcançado dentro do intervalo de 0-20 segundos, que foi significativamente superior ao valor absoluto máximo obtido nos primeiros 5 segundos (p=0,036). O tempo de oclusão necessário para avaliar a PIMáx pelo método da válvula expiratória unidirecional em sujeitos colaborativos sem via aérea artificial deve ser de pelo menos 20 segundos.

ABSTRACT The aim of this study was to determine how much occlusion time is necessary to obtain maximal inspiratory pressure (MIP) by the unidirectional expiratory valve method in subjects without artificial airway. Thirty-one subjects aged 18-60 years were evaluated. MIP was evaluated by the standard method (MIPstan) and by the unidirectional expiratory valve method MIPuni, with the order of evaluation determined randomly by lot. For MIPuni measurement, a digital vacuum manometer was attached to a unidirectional expiratory valve and an orofacial mask for 20 seconds of occlusion. During this period, all subjects were encouraged to make maximal respiratory efforts. To define the optimum duration of the maneuver, the 20 seconds of effort were partitioned at every five-second interval (0-5s, 0-10s, 0-15s, 0-20s). The time intervals for obtaining MIPuni were compared with the one-way ANOVA test. The mean values of the standard method and the unidirectional expiratory valve method were compared using the paired Student’s t-test. The significance level was established at 5%. The mean values for the MIPstan (-102.5±23.9 cmH2O) presented a statistically significant difference as compared to the mean values for MIPuni (-117.3±24.8 cmH2O; p<0.001). Maximal peak values for MIPuni were achieved within the 20-second time window, which differed significantly from the peak values obtained during the first five seconds (p=0.036). The occlusion time necessary to record MIP by the unidirectional expiratory valve method in collaborative subjects without artificial airway should be of at least 20 seconds.

RESUMEN Este estudio busca determinar cuánto tiempo de oclusión es necesario para obtener la presión inspiratoria máxima (PIMáx) por medio del método de la válvula espiratoria unidireccional en individuos sin vía aérea artificial. Se evaluaron 31 sujetos de entre 18 y 60 años de edad. La PIMáx se evaluó mediante el método estándar (PIMáxest) y el método de válvula espiratoria unidireccional (PIMáxuni), siendo que el orden de evaluación se estableció por medio de un sorteo. Para el PIMáxuni, un manovacuómetro digital se ha conectado a una válvula espiratoria unidireccional y una máscara orofacial durante 20 segundos de oclusión. Durante este período, se alentó a los individuos a hacer esfuerzos respiratorios máximos. Para definir la óptima duración de la maniobra, el tiempo de esfuerzo se dividió en intervalos de cinco segundos (0-5s, 0-10s, 0-15s, 0-20s). Los intervalos del tiempo para el PIMáxuni se compararon mediante la prueba ANOVA one-way. Las medias de los valores de PIMáxest y de PIMáxuni se compararon mediante la prueba pareada t de Student. El nivel de significancia se estableció en el 5%. La media de los valores de PIMáxest (-102,5±23,9 cmH2O) presentó una diferencia estadísticamente significativa en comparación con la media de los valores de PIMáxuni (-117,3±24,8 cmH2O, p<0,001). El valor absoluto máximo obtenido de PIMáxuni estaba dentro del intervalo de 0-20 segundos, que fue significativamente superior del valor absoluto máximo durante los primeros 5 segundos (p=0,036). RESUMO Objetivo: Avaliar a evolução funcional dos pacientes submetidos a um protocolo de reabilitação precoce do paciente grave da admissão até a alta da unidade de terapia intensiva. Métodos: Foi conduzido um estudo transversal retrospectivo, incluindo 463 pacientes adultos com diagnóstico clínico e/ou cirúrgico, submetidos a um protocolo de reabilitação precoce. A força muscular global foi avaliada na admissão da unidade de terapia intensiva por meio da escala Medical Research Council. De acordo com a pontuação da Medical Research Council os pacientes foram alocados em um dos quatro planos de intervenção, de acordo com a adequação ou não desses parâmetros, com a escala crescente do plano significando melhor status funcional. Os pacientes não colaborativos foram alocados nos planos de intervenção, conforme seu status funcional. A força muscular global e/ou o status funcional foram reavaliados na alta da unidade de terapia. Por meio do comparativo entre o plano de Intervenção na admissão (Planoinicial) e na alta (Planofinal). Os pacientes foram categorizados em três grupos, de acordo com a melhora ou não do status funcional: respondedores 1 (Planofinal > Planoinicial), respondedores 2 (Planofinal = Planoinicial) e não respondedores (Planofinal < Planoinicial). Resultados: Dos 463 pacientes submetidos ao protocolo, 432 (93,3%) pacientes responderam positivamente à estratégia de intervenção, apresentando manutenção e/ou melhora do status funcional inicial. Os pacientes clínicos classificados como não respondedores apresentaram idade superior (74,3 ± 15,1 anos; p = 0,03) e maior tempo de internação na unidade de terapia intensiva (11,6 ± 14,2 dias; p = 0,047) e no hospital (34,5 ± 34,1 dias; p = 0,002). Conclusão: A manutenção e/ou melhora do status funcional admissional esteve associada com menor tempo de internação na unidade de terapia intensiva e hospitalar. Os resultados sugerem que o tipo de diagnóstico, clínico ou cirúrgico, não é definidor da resposta positiva ao protocolo de reabilitação precoce.

ABSTRACT Objective: Evaluation of the functional outcomes of patients undergoing an early rehabilitation protocol for critically ill patients from admission to discharge from the intensive care unit. Methods: A retrospective cross-sectional study was conducted that included 463 adult patients with clinical and/or surgical diagnosis undergoing an early rehabilitation protocol. The overall muscle strength was evaluated at admission to the intensive care unit using the Medical Research Council scale. Patients were allocated to one of four intervention plans according to the Medical Research Council score, the suitability of the plan’s parameters, and the increasing scale of the plan expressing improved functional status. Uncooperative patients were allocated to intervention plans based on their functional status. The overall muscle strength and/or functional status were reevaluated upon discharge from the intensive care unit by comparison between the Intervention Plans upon admission (Planinitial) and discharge (Planfinal). Patients were classified into three groups according to the improvement of their functional status or not: responsive 1 (Planfinal > Planinitial), responsive 2 (Planfinal = Planinitial) and unresponsive (Planfinal < Planinitial). Results: In total, 432 (93.3%) of 463 patients undergoing the protocol responded positively to the intervention strategy, showing maintenance and/or improvement of the initial functional status. Clinical patients classified as unresponsive were older (74.3 ± 15.1 years of age; p = 0.03) and had longer lengths of intensive care unit (11.6 ± 14.2 days; p = 0.047) and hospital (34.5 ± 34.1 days; p = 0.002) stays. Conclusion: The maintenance and/or improvement of the admission functional status were associated with shorter lengths of intensive care unit and hospital stays. The results suggest that the type of diagnosis, clinical or surgical, fails to define the positive response to an early rehabilitation protocol.
