ABSTRACT Latin America was during November 2020 one of the most affected regions by the COVID-19 pandemic in terms of prevalence and deaths attributed to the virus. Therefore, the start of vaccine testing in citizens of the region was received with high expectations about its effectiveness. Therefore, it is essential to take into account certain fundamental principles that should guide this process ensuring its correct implementation. The objective of this article is to provide considerations on the process of implementing clinical trials and post-trial access to COVID-19 vaccines in Latin America, in particular those related to respect for the dignity of participants, the historical global and regional tensions regarding the political economy of clinical trials, some aspects of global governance and the role of the World Health Organization in the pandemic, and the need to ensure post-trial access to the vaccine. It is concluded that the pandemic is an opportunity to strengthen bonds of cooperation and solidarity between Latin American countries. Clinical trials are a key instance to strengthen local and regional governance with a view to consolidate transparency and monitoring of the correct implementation of public-private partnerships in the development of therapies, in which the World Health Organization and the Pan American Health Organization have an important role for the implementation of integration strategies and access to vaccines.

RESUMEN Latinoamérica fue durante noviembre de 2020 una de las regiones más afectada por la pandemia de COVID-19 en cuanto a prevalencia y muertes atribuidas al virus. Por ello, el inicio de testeo de vacunas en ciudadanos de la región fue recibido con altas expectativas sobre su efectividad. Frente a lo cual es esencial tener en cuenta ciertos principios fundamentales que deben guiar este proceso asegurando su correcta implementación. El objetivo de este artículo es ofrecer consideraciones sobre el proceso de implementación de ensayos clínicos y acceso posensayo a vacunas para COVID-19 en Latinoamérica. En particular los relacionados con el respeto a la dignidad de los participantes, las tensiones históricas globales y regionales en materia de economía política de ensayos clínicos, algunos aspectos de la gobernanza global y el rol de la Organización Mundial de la Salud en la pandemia, y la necesidad de asegurar el acceso posensayo a la vacuna. Se concluye que la pandemia es una oportunidad para estrechar lazos de cooperación y solidaridad entre países latinoamericanos. Los ensayos clínicos son una instancia clave para reforzar la gobernanza local y regional con miras a fortalecer la transparencia y la vigilancia de la correcta realización de alianzas público-privadas en el desarrollo de terapias, en los que la Organización Mundial de la Salud y la Organización Panamericana de la Salud, tienen un rol importante para la implementación de estrategias de integración y acceso a vacunas.

Abstract: Objective: To describe a cohort of critically ill adult patients suffering from COVID-19, admitted to a pediatric intensive care unit managed by a pediatric intensive care team (ICU-MP). Patients and Method: Retrospective observational study of adults admitted to the ICU-MP due to COVID-19 from May 11 to July 26, 2020. Demographic, clinical, biochemical, ventilatory support characteris tics, and complications were recorded. Disease severity was characterized by Acute Physiology and Chronic Health Evaluation II score (APACHE II) using data from the first 24 hours of admission to the ICU-MP. Results: Ninety-three patients over 18 years with suspected or confirmed COVID-19 were admitted to the ICU-MP. The median age was 60.3 years (SD 13.9), and 59 (63.4%) patients were male. Eighty-two (88.1%) patients had at least 1 medical comorbidity. The median APACHE II score was 9.4 points (SD 5.6). Fifty-one (54.8%) patients were invasively ventilated, for a median of 13.7 days (SD 17.9). Inotropic support was used in 45 (48%) patients. Thirty-three (35.5%) patients presented acute kidney injury (AKI) and 14 (15.1%) patients received continuous renal replacement therapy. Twenty-nine (31.2%) patients had healthcare-associated infections. The median ICU-MP stay was 10.8 days (SD 11.8). 25 (26.9%) patients died, ten of them (40%) had adequacy of thera peutic effort. Conclusions: The mortality rate of critically ill patients with COVID-19 is high. Older patients (> 70 years), those who require invasive mechanical ventilation and who develop AKI are at increased risk of death. Although this is not a comparative study, our mortality rate and complica tions seem to be similar to those reported in adult case series.

Resumen: Objetivo: Describir una cohorte de pacientes adultos críticos con COVID-19, ingresados en una Uni dad de Cuidados Intensivos Pediátrica manejada por pediatras intensivistas (UCI-MP). Pacientes y Método: Estudio observacional retrospectivo de adultos ingresados en UCI-MP por COVID-19 del 11 de mayo al 26 de julio de 2020. Se registraron datos demográficos, clínicos, bioquímicos, carac terísticas del soporte ventilatorio y complicaciones. La gravedad de la enfermedad se caracterizó por el puntaje en el “Acute Physiology and Chronic Health Evaluation II score” (APACHE II) utilizando datos de las primeras 24 horas de ingreso a la UCI-MP. Resultados: Noventa y tres pacientes mayores de 18 años con COVID-19 sospechado o confirmado ingresaron en la UCI-MP. La media de edad fue de 60,3 años (DE 13,9) y 59 pacientes (63,4%) eran varones. Ochenta y dos pacientes (88,1%) tenían al menos una comorbilidad médica. La puntuación media de APACHE II fue de 9,4 puntos (DE 5,6). Cincuenta y un pacientes (54,8%) fueron ventilados de forma invasiva, durante una mediana de 13,7 días (DE 17,9). Se utilizó soporte inotrópico en 45 pacientes (48%). Treinta y tres pacientes (35,5%).presentaron falla renal aguda (FRA) y 14 (15,1%) recibieron terapia de reemplazo renal continua. Veintinueve pacientes (31,2%) tuvieron infecciones asociadas a la asistencia sanitaria. La estancia media en UCI-MP fue de 10,8 días (DE 11,8). Murieron 25 pacientes (26,9%). En 10 (40%) de los 25 pacientes que fallecieron, se estableció adecuación del esfuerzo terapéutico. Conclusiones: La tasa de mortalidad de los pacientes críticamente enfermos con COVID-19 es alta. Los pacientes de edad avanzada (> 70 años), los que requieren ventilación mecánica invasiva y los que desarrollan FRA tienen un mayor riesgo de muerte. Aunque no se trata de un estudio comparativo, la tasa de morta lidad y complicaciones de este estudio parecen ser similares a las reportadas en las series de casos de pacientes manejados por intensivistas de adultos.

Health systems do not have the capacity to finance all services. The impact of choosing one option or another is important in order to prioritize health resources. Citizen participation can help to set priorities or to select the interventions that will receive public funding. We reviewed the literature searching for articles that reported mechanisms to gather information about citizens’ values or preferences about health system coverage. We identified 363 publications, 18 articles were analyzed in full, and 7 articles were included in the review. Three articles were European, two were from Australia and two from Latin America. The most commonly used mechanisms to gather information were interviews and surveys. We conclude that there is a limited number of articles with examples of tools to capture information about values and preferences in health decision processes. The main barrier observed was the lack of standardized processes to collect the values and preferences of the community.

Abstract: Vitamin D (VD) is essential for calcium and phosphorus metabolism. Its deficiency can cause rickets. In Chile, newborns receive 400UI/day supplementation from the first day of age until the first year. Objective: To describe the VD plasma levels in healthy infants who received supplementation and secondarily to correlate this with seasonality and nutritional status. Subjects and Method: Cross sectional study. Infants on exclusive or mixed breastfeeding, with monthly pediatric checkups recei ving 400 UI VD supplementation were evaluated, measuring VD plasma levels at 6 months of age, weight, and length, and their nutritional status was classified according to the WHO growth referen ces (weight/age and weight/length). The VD cut-off concentration values were ≤ 20 ng/ml, 21- 29 ng/ ml, and ≥ 30 ng/ml considered as deficiency, insufficiency, and sufficiency, respectively. Results: 40 infants were studied, 40% had insufficient levels and 40% presented deficiency. Season and nutritio nal status were variables significantly related to lower VD values (Winter-Spring p = 0.007; at risk of malnutrition p = 0.038). Conclusions: The population who received supplementation presented a high frequency of VD deficiency and insufficiency which increases during winter and spring and in subjects at risk of malnutrition.

Resumen: La vitamina D (VD) es indispensable para el metabolismo del calcio y fósforo, su deficiencia puede causar raquitismo. En Chile se suplementa con 400 UI /diarias desde el primer mes hasta el año de vida. Objetivo: Describir las concentraciones plasmáticas de VD en lactantes sanos suplementados y secundariamente, evaluar la asociación del estatus de VD con estacionalidad y estado nutricional. Sujetos y Método: Estudio de corte transversal. Se evaluó a lactantes con lactancia materna exclusiva o mixta que acudían a controles pediátricos mensuales y recibían suplementación de VD a dosis de 400 UI diarias. A los 6 meses de edad se midió la concentración plasmática de VD, el peso y la talla. Posteriormente se clasificó el estado nutricional (peso/edad y peso/talla) según los referentes de la OMS. De acuerdo a la concentración plasmática de VD, se definió como normal ≥ 30 ng/ml, insuficiencia si esta fue entre 20 y 29 ng/ml y deficiencia < 20 ng/ml. Resultados: Se estudiaron 40 lactantes, 40% tuvieron concentracio nes de VD insuficientes y 40% deficientes. En el modelo de regresión lineal múltiple, las variables que se asociaron, de modo significativo, con una menor concentración de VD fueron: el periodo del año invierno-primavera (p = 0,007) y el riesgo de desnutrición (p = 0,038). Conclusiones: La frecuencia de deficiencia e insuficiencia de VD es elevada en nuestra población suplementada, dicho déficit es mayor durante el invierno y la primavera y en sujetos con riesgo de desnutrición.

Abstract: Introduction: Prader-Willi Syndrome (PWS) is the most common cause of genetic obesity. Hyperphagia and obe sity are the most associated concepts with this condition. However, undernutrition secondary to severe hypotonia and feeding difficulties is the predominant initial feature. Objective: to reprodu ce and communicate the nutritional phases on a series of Chilean cases with PWS. Patients and Method: Cross-sectional study in which clinical records of PWS individuals under nutritional con trol at the Clínica Santa María in Santiago, Chile between 2017 and 2018 were analyzed. The anthro pometric references of the World Health Organization were used to carry out the nutritional as sessment. The classification into nutritional phases was according to the Miller criteria. Results: 24 patients from infants to adults were included. All children aged under 9 months were in phase I and had malnutrition or were eutrophic; those between 9 and 25 months were classified in phase 2a; pa tients between 2.1 and 4.5 years were distributed between phases 1 and 2 and 66% were eutrophic; those between 4.5 to 8 years, 80% were in phase 2a and 2b and obesity begins to appear, and patients over 8 years of age, 75% were in phase 3 and all are overweight or obese. There was an association bet ween nutritional phase and age but not between it and nutritional status. Conclusions: In our series, the nutritional phases described according to age were reproduced according to those internationally described. There was no association between nutritional status and age.

Resumen: Introducción: El Síndrome de Prader-Willi (SPW) es la causa más frecuente de obesidad de origen genético, siendo obesidad e hiperfagia los conceptos más asociados a esta enfermedad. Sin embargo, la desnutrición secundaria a una hipotonía marcada y dificultad alimentaria, es la característica inicial predominan te. Objetivo: Reproducir y comunicar las fases nutricionales en una serie de casos chilenos de SPW. Pacientes y Método: Estudio transversal en que se analizaron registros clínicos de individuos con SPW bajo control nutricional en Clínica Santa María, Chile durante 2017 y 2018. Se utilizaron los referentes de la Organización Mundial de la Salud para la evaluación nutricional. La clasificación en fases nutricionales se basó en los criterios de Miller. Resultados: Fueron incluidos 24 individuos des de lactantes hasta adultos. Todos los menores de 9 meses estaban en fase 1 y eutróficos o desnutridos, entre los 9 y 25 meses aparecen niños en fase 2a. Entre los 2,1 y los 4,5 años se dispersan entre las fases 1 o 2 y 66% estaba eutróficos. Entre los 4,5 a 8 años, 80% está en la fase 2a y 2b y empieza a aparecer la obesidad, sobre los 8 años un 75% está en fase 3 y todos tienen malnutrición por exceso. Se demostró una asociación entre fase nutricional, pero no entre estado nutricional y grupo etario. Conclusiones: En nuestra serie de SPW se reprodujeron las fases nutricionales acorde a la edad descritas internacio nalmente. No se demostró una asociación entre el estado nutricional y la edad.

Background: Breast cancer (BC) has a high mortality rate in developing countries due to a scarcity of early detection. Risk communication is critical to support women who face the decision to undertake BC screening. Thus, they can balance their perceived and real risk, and make informed choices. Aim: To describe experts’ views on how the provision of information related to BC screening should be made. Material and Methods: A qualitative study with focus groups with national experts was conducted. Open coding was performed. Results: Four categories on the way information about BC screening should be provided emerged: to communicate about the need of the exam; the pros and cons of the test; fear as a barrier for understanding; and involving women in the decision-making process. Conclusions: These findings emphasize the need to include risk communication strategies in the patient-provider relationship and encourage and respect women's autonomy when facing the BC screening decision.

RESUMEN Un principio del pensamiento abismal es que el conocimiento científico no se distribuye equitativamente. La relación universidad-escuela puede leerse bajo este principio donde existe un sujeto de conocimiento, el saber académico y un objeto de conocimiento, el saber docente-escolar. Más específicamente, conectamos el problema de la (de)colonialidad del poder y el saber con la educación en ciencias a través de un trabajo de cuidado epistémico. Se presenta el caso de CIDSTEM-PUCV como un centro experimental que desafía el rol tradicional de la investigación para incluir la diversidad epistémica y cultural que existe en escuelas y sus comunidades. Reflexionamos sobre los desafíos de una educación científica decolonial y latina capaz de reconocer la pluralidad de saberes con que se relaciona.

ABSTRACT A principle of abyssal thinking is that scientific knowledge is not equitably distributed. The university-school relationship may be read under this principle where there is a subject of knowledge (academic knowledge), and an object of knowledge (teachers and school knowledge). More specifically, we connect the problem of the (de)coloniality of power and knowledge with science education through a work of epistemic care. The case of CIDSTEM-PUCV is presented as an experimental center that challenges the traditional role of research by including the epistemic and cultural diversity that exists in schools and its communities. We reflect on the challenges of a decolonial and Latina science education capable of recognizing the plurality of knowledge with which it is related.

Abstract This aim of this study is to understand how high school counselors (also referred to as head teachers) interpret educational policies in Chile, specifically through analysis and execution of PACE (Program for Assistance and Effective Access to Higher Education), an affirmative action program that provides low-income students the possibility to continue their college education. Counselor subjectivity was highlighted as the basis to understanding how they help design, implement and assess educational policies and their participation in executing the PACE program. The qualitative approach for this analysis was based on a case study that was developed, in-depth interviews and a focus group to collect data. Results show that the PACE program reflects educational policy that focuses on specific stakeholders within the educational community, seeking to subsidize social rights without altering the structure of the educational system. The program is also characterized by the low level of involvement of the counselors despite the pivotal role they carry out in the student´s journey through high school to college.

Resumen El estudio busca comprender los significados elaborados por el profesorado en jefe respecto a la política educativa en Chile, a partir del análisis de la implementación del Programa de Acompañamiento y Acceso Efectivo a la Educación Superior (PACE), el cual otorga un ingreso alternativo a la educación superior a estudiantes en condición de vulnerabilidad. La subjetividad del profesorado jefe se articuló como eje central para comprender el rol que ejerce en el diseño, implementación y evaluación de la política educativa; de su participación en la implementación del PACE y de las particularidades de su función. La investigación se planteó desde un enfoque cualitativo, como un estudio de caso, aplicando la entrevista en profundidad y el grupo focal como técnicas de recolección de datos. Los resultados muestran al PACE como ejemplo de políticas educativas focalizadas en actores específicos de la comunidad educativa; subsidiándoles derechos sociales sin alterar la estructura del sistema educativo. Además, se caracteriza por la escasa participación docente, en este caso de profesores jefes, pese al rol preponderante que cumplen en el tránsito hacia la educación superior del estudiantado.
