Abstract This article presents maps created with the homeless population in Florianópolis, Brazil and aims to create greater visibility for harm reduction (HR) strategies as a form of care provision on the streets. The first scenes draw on discussions during a HR workshop with homeless people, when they had their first contact with the strategy. The mapped scenes are the result of an encounter with Cigana and Alemão, a couple who met each other in the midst of scenes of crack use and, thereafter, through the agency of affect, were motivated to stop using the substance. The maps demonstrate that homeless people invent other ways to find themselves, creating lines of flight as a way of reaffirming their existence and using HR strategies as a form of care provision.

Resumo Este artigo apresenta as cartografias produzidas com a população de rua em Florianópolis, Brasil, e tem como objetivo dar visibilidade às estratégias de redução de danos (RD) como produção de cuidado nas ruas. As primeiras cenas partem dos debates em uma oficina de RD com a população de rua, quando se teve contato com a proposta da RD. Em seguida, as cenas cartografadas apresentam o encontro com Cigana e Alemão(c), um casal que se conheceu em cenas de uso de crack, e, a partir desse encontro, foram agenciados por afetos que os movimentaram a deixar o uso daquela substância. Essas cartografias demonstraram que a população de rua inventa outros jeitos de se encontrar, criando linhas de fuga como forma de reafirmar sua existência e utilizando estratégias de RD como produção de cuidado.

Resumo Este trabalho visou compreender a representação que a clínica assume nas práticas odontológicas, em um município de médio porte do estado de Santa Catarina. Estudo qualitativo, cujos participantes foram 20 cirurgiões-dentistas vinculados aos serviços públicos de saúde bucal. Os dados foram coletados por meio de três grupos focais. O material gravado por áudio foi transcrito e analisado pela hermenêutica-dialética. Na primeira categoria de análise - concepção sobre clínica -, os participantes demonstraram dificuldade para formular um conceito para a clínica, traduzindo-a como o desenvolvimento de procedimentos técnicos, entendidos como essenciais neste tipo de prática para os dentistas. Integralidade e resolubilidade apareceram como elementos centrais, mas assumem uma visão odontocentrada. Na categoria “recursos e organização da clínica odontológica”, observou-se as vantagens da utilização do prontuário eletrônico e as limitações da divisão de abas que restringe o acesso de informações. Na sistemática de atendimento, há pouco espaço para escuta e o odontograma desponta como sinônimo de plano de tratamento, reforçando o caráter dentarizado das práticas desenvolvidas. O estudo mostrou dificuldades diversas para incorporação da clínica, seja ela compreendida nos moldes tradicionais ou de maneira ampliada, nas práticas desenvolvidas pelos participantes do estudo.

Integrality and resolvability appeared as central elements, but assume a dentistry-centered view. In the category “resources and organization of the dental clinic,” the advantages of using the electronic medical records and the limitations of tab division that narrow the access to information were observed. In treatment systematics, there is little room for listening and the odontogram emerges as a synonym for treatment plan, reinforcing the tooth-focused character of the practices developed. The study showed diversified challenges to incorporating the clinic, whether understood in the traditional molds or in a broader way, in the practices developed by the study participants.

Resumo Este artigo tem como objetivo apresentar reflexões sobre a saúde da população em situação de rua, utilizando o conceito da determinação social do processo saúde-doença como método de análise. O artigo está estruturado em formato ensaístico, sendo organizado em duas seções: a primeira, que apresenta a discussão sobre a saúde dessa população, discorrendo sobre a organização dos serviços de saúde para dar assistência a essas pessoas, seus avanços e entraves. A segunda seção realiza uma análise do processo saúde-doença da população em situação de rua, utilizando o referencial teórico da Saúde Coletiva a partir do conceito da determinação social da saúde. O artigo argumenta que o modelo biomédico tem sido insuficiente para pensar na saúde da população em situação de rua, uma vez que desconsidera a complexidade dessa realidade social. A compreensão do processo saúde-doença como socialmente determinado localiza a saúde como resultando das condições materiais de existência dessa população, as quais são condicionadas pela forma de organização social no modo de produção capitalista. Assim, a determinação social opera como uma importante ferramenta de análise da saúde da população em situação de rua em uma perspectiva de totalidade.

Abstract This article aims to present reflections about the homeless people’s health, using the social determination of health-disease process concept as an analysis key. This article in essay format was organized in two sections: the first one presents the discussion about this population’s health, indicating to the organization of health services that assist these people, its advances and obstacles; the next section performs an analysis of the health-disease process of the homeless population using the theoretical reference of Collective Health based on the social determination of health concept. The article argues that the biomedical model has been insufficient to think about the health of the homeless population, once it disregards the complexity of this social reality. Understanding the health-disease process as socially determined approaches health as a result of the material conditions of existence of this population, which are conditioned by the form of social production organization in the capitalist production mode. Thus, social determination operates as an important tool to analyze the homeless people’s health from a perspective of totality.

Abstract Regular use of toothpaste with fluoride (F) concentrations of ≥ 1000 ppm has been shown to contribute to reducing caries increment. However, when used by children during the period of dental development, it can lead to dental fluorosis. Objective: In this study, we aimed to evaluate the in vitro effect of a toothpaste formulation with reduced fluoride (F) concentration (200 ppm) supplemented with sodium trimetaphosphate (TMP: 0.2%), Xylitol (X:16%), and Erythritol (E: 4%) on dental enamel demineralization. Methodology: Bovine enamel blocks were selected according to initial surface hardness (SHi) and then divided into seven experimental toothpaste groups (n=12). These groups included 1) no F-TMP-X-E (Placebo); 2) 16% Xylitol and 4% Erythritol (X-E); 3) 16% Xylitol, 4% Erythritol and 0.2%TMP (X-E-TMP); 4) 200 ppm F (no X-E-TMP: (200F)); 5) 200 ppm F and 0.2% TMP (200F-TMP); 200 ppm F, 16% Xylitol, 4% Erythritol, and 0.2% TMP (200F-X-E-TMP); and 7) 1,100 ppm F (1100F). Blocks were individually treated 2×/day with slurries of toothpastes and subjected to a pH cycling regimen for five days (DES: 6 hours and RE: 18 hours). Then, the percentage of surface hardness loss (%SH), integrated loss of subsurface hardness (ΔKHN), fluoride (F), calcium (Ca), and phosphorus (P) in enamel were determined. The data were analyzed by ANOVA (1-criterion) and the Student-Newman-Keuls test (p<0.001). Results: We found that the 200F-X-E-TMP treatment reduced %SH by 43% compared to the 1100F treatments (p<0.001). The ΔKHN was ~ 65% higher with 200F-X-E-TMP compared to 1100F (p<0.001). The highest concentration of F in enamel was observed on the 1100F treatment (p<0.001). The 200F-X-E-TMP treatment promote higher increase of Ca and P concentration in the enamel (p<0.001). Conclusion: The association of 200F-X-E-TMP led to a significant increase of the protective effect on enamel demineralization compared to the 1100F toothpaste.

Abstract Objective To evaluate the effects of fluoride (F) gels supplemented with micrometric or nano-sized sodium trimetaphosphate (TMPmicro and TMPnano, respectively) on the in vitro remineralization of caries-like lesions. Methodology Bovine enamel subsurface lesions (n=168) were selected according to their surface hardness (SH) and randomly divided into seven groups (n=24/group): Placebo (without F/TMP), 4,500 ppm F (4500F), 4500F + 2.5% TMPnano (2.5% Nano), 4500F + 5% TMPnano (5% Nano), 4500F + 5% TMPmicro (5% Micro), 9,000 ppm F (9000F), and 12,300 ppm F (Acid gel). The gels were applied in a thin layer for one minute. Half of the blocks were subjected to pH cycling for six days, whereas the remaining specimens were used for loosely- (calcium fluoride; CaF2) and firmly-bound (fluorapatite; FA) fluoride analysis. The percentage of surface hardness recovery (%SHR), area of subsurface lesion (ΔKHN), CaF2, FA, calcium (Ca), and phosphorus (P) on/in enamel were determined. Data (log10-transformed) were subjected to ANOVA and the Student-Newman-Keuls’ test (p<0.05). Results We observed a dose-response relation between F concentrations in the gels without TMP for %SHR and ΔKHN. The 2.5% Nano and 5% Micro reached similar %SHR when compared with 9000F and Acid gels. For ΔKHN, Placebo and 5% Nano gels had the highest values, and 5% Micro, 2.5% Nano, 9000F, and Acid gels, the lowest. All groups had similar retained CaF2 values, except for Placebo and Acid gel. We verified observed an increase in Ca concentrations in nano-sized TMP groups. Regarding P, TMP groups showed similar formation and retention to 9000F and Acid. Conclusion Adding 2.5% nano-sized or 5% micrometric TMP to low-fluoride gels lead to enhanced in vitro remineralization of artificial caries lesions.

Resumo Este artigo tem como objetivo compartilhar as reflexões sobre o cuidado em saúde bucal produzidas a partir da relação dialética entre a prática profissional e as pesquisas teóricas. Propomos a problematização da concepção de cuidado difundida pelo discurso biomédico, que o remete a uma certa tecnicidade voltada à cura. Apostamos no cuidado em sua dimensão ontológica, como característica intrínseca ao ser humano, a partir da qual os sujeitos se constituem e se realizam no mundo. Para tanto, utilizamos o conceito da bucalidade para localizar a boca humana enquanto território que está inserido no processo de produção e reprodução social, sendo completamente penetrado pela cultura e pelo psiquismo. Apresentamos, portanto, a boca que é socialmente produzida, um território-corpo dotado de subjetividade e que é atravessado por uma multiplicidade de experiências ao longo da vida. Por esse motivo, a ideia odontologizada de cuidado não dá conta de apreender o cuidado em saúde bucal em sua complexidade, tornando-se necessária a renúncia ao a priori odontológico, compreendendo que é na boca em que se materializam as realidades e experiências histórico-sociais dos sujeitos, abrindo-se espaço para o cuidado enquanto encontro intersubjetivo atravessado por afecções, com enorme potencial transformador.

Abstract This article shares reflections on oral health care that emerged from the dialect relation between professional practice and theoretical research. It interrogates the concept of care disseminated by the biomedical discourse, in which care is a technique geared towards cure. We believe care to be an ontological dimension, an intrinsic characteristic of the human being from which they constitute themselves in the world. As such, we use the notion of buccality to situate the mouth as a territory that take part in the social production and reproduction process, completely penetrated by the culture and psychism. The mouth is, therefore, socially produced-a body-territory endowed with subjectivity that is crossed by multiple experiences throughout life. Hence, dentistry’s concept of care fails to encompass oral health care in all its complexity, making it necessary to renounce an odontological a priori to understand that people’s socio-historical realities and experiences are materialized on their mouths, making room to understand care as an intersubjective encounter crossed by potent and transformative affections.

Abstract Objectives: To evaluate the mechanical, physicochemical, and antimicrobial properties of four different formulations containing micro- or nanoparticles of sodium trimetaphosphate (mTMP and nTMP, respectively). Methodology: Four experimental groups were used in this investigation: two mTMP groups and two nTMP groups, each containing zirconium oxide (ZrO2), and solution containing either chitosan or titanium oxide (TiO2) nanoparticles (NPs). Setting time, compression resistance, and radiopacity were estimated. The agar diffusion test was used to assess the antimicrobial activity of the formulations against five different microbial strains: Streptococcus mutans, Lactobacillus casei, Actinomyces israelii, Candida albicans, and Enterococcus faecalis. Parametric and nonparametric tests were performed after evaluating homoscedasticity data (p<0.05). Results: From the properties evaluated, nTMP cements required less setting time and showed greater resistance to compression. Cements containing TiO2 showed greater radiopacity for both nTMP and mTMP. All four cement formulations showed antimicrobial activity against S. mutans and L. casei Conclusion: Formulations containing nTMP have shorter setting times and higher compressive strength, and those with TiO2 nanoparticles showed antimicrobial activities. Clinical relevance: The cement containing nTMP, ZrO2, and TiO2 could be an alternative material for protecting the pulp complex.

Abstract A new sugarcane-derived cystatin (CaneCPI-5) showed anti-erosive properties when included in solutions and strong binding force to enamel, but the performance of this protein when added to gel formulations and its effect on surface free energy (SFE) requires further studies. Objective 1) to evaluate the protective effect of gels containing different concentrations of CaneCPI-5 against initial enamel erosion (Experiment 1); and 2) to analyze the SFE (γS) after treating the enamel surface with CaneCPI-5 solution (Experiment 2). Methodology In Experiment 1, 75 bovine enamel specimens were divided into five groups according to the gel treatments: placebo (negative control); 0.27%mucin+0.5%casein (positive control); 0.1 mg/mL CaneCPI-5; 1.0 mg/mL CaneCPI-5; or 2.0 mg/mL CaneCPI-5. Specimens were treated with the gels for 1 min, the AP was formed (human saliva) for 2 h and the specimens were incubated in 0.65% citric acid (pH=3.4) for 1 min. The percentage of surface hardness change (%SHC) was estimated. In Experiment 2, measurements were performed by an automatic goniometer using three probing liquids: diiodomethane, water and ethylene glycol. Specimens (n=10/group) remained untreated (control) or were treated with solution containing 0.1 mg/mL CaneCPI-5, air-dried for 45 min, and 0.5 µL of each liquid was dispensed on the surface to measure contact angles. Results Gels containing 0.1 and 1.0 mg/mL CaneCPI-5 significantly reduced %SHC compared to the other treatments (p<0.05). Treated enamel showed significantly lower γS than control, without changes in the apolar component (γSLW), but the polar component (γSAB=Lewis acid-base) became more negative (p<0.01). Moreover, CaneCPI-5 treatment showed higher γS - (electron-donor) values compared to control (p<0.01). Conclusions Gels containing 0.1 mg/mL or 1.0 mg/mL CaneCPI-5 protected enamel against initial dental erosion. CaneCPI-5 increased the number of electron donor sites on the enamel surface, which may affect AP formation and could be a potential mechanism of action to protect from erosion.

Resumo Neste artigo, o objetivo foi realizar uma análise crítica da relação entre saúde bucal e trabalho. A reflexão emergiu do aumento expressivo na busca por atendimentos de urgência odontológica em uma unidade básica de saúde durante a pandemia pelo coronavírus, em que os sujeitos apresentavam algo em comum: a demissão de seus empregos em fábricas de calçados. Com apoio na teoria social crítica, efetuou-se uma análise sobre como o trabalho, entendido como categoria fundante do ser social, tem o seu sentido esvaziado nas sociedades capitalistas, operando mais como um obstáculo à saúde bucal na dinâmica do trabalho assalariado e alienado. Isso porque pôde-se perceber que os sujeitos recém-desempregados acumulavam diversas necessidades em saúde bucal, como infecções e inflamações que causavam sofrimentos, até então internalizados e escondidos para que pudessem manter seus empregos em um contexto de desemprego iminente, possibilitando assim que pudessem seguir reproduzindo a sua vida material. Pensar a relação entre saúde bucal e trabalho nos aproxima da compreensão do processo saúde-doença como socialmente determinado, quando as relações de trabalho explorado se materializam nas ‘bocas trabalhadoras’. Desse modo, vislumbra-se a democratização do acesso à saúde bucal entendida como o direito a uma vida plena.

Abstract In this article, the objective was to carry out a critical analysis of the relationship between oral health and labor. The reflection emerged from the significant increase in the search for emergency dental care in a basic health unit during the coronavirus pandemic, in which the subjects had something in common: the termination of their jobs in shoe factories. Based on critical social theory, an analysis was carried out on how labor, understood as a founding category of the social being, has its meaning emptied in capitalist societies, operating more as an obstacle to oral health in the dynamics of hired and alienated work. This is because it was possible to perceive that the newly unemployed subjects accumulated several oral health needs, such as infections and inflammations that caused suffering, until then internalized and hidden so that they could keep their jobs in a context of imminent unemployment, thus enabling them to continue reproducing their material life. Thinking about the relationship between oral health and work brings us closer to understanding the health-disease process as socially determined, when exploited work relationships materialize in ‘working mouths’. In this way, the democratization of access to oral health is seen and understood as a right to a full life.

Resumen En este artículo, el objetivo fue realizar un análisis crítico de la relación entre la salud bucal y el trabajo. La reflexión surgió a partir del aumento significativo de la búsqueda de atención odontológica de urgencia en una unidad básica de salud durante la pandemia del coronavirus, en la que los sujetos tenían algo en común: el despido de sus trabajos en fábricas de calzados. Con base en la teoría social crítica, se realizó un análisis sobre cómo el trabajo, entendido como categoría fundante del ser social, se vacía de significado en las sociedades capitalistas, operando más como un obstáculo para la salud bucal en la dinámica del trabajo asalariado y alienado. Esto porque se pudo percibir que los sujetos recién desempleados acumulaban diversas necesidades de salud bucal, como infecciones e inflamaciones que les causaban sufrimiento, hasta entonces interiorizadas y ocultadas para que pudieran conservar sus puestos de trabajo en un contexto de desempleo inminente, permitiéndoles así seguir reproduciendo su vida material. Pensar en la relación entre salud bucal y trabajo nos acerca de la comprehensión del proceso salud-enfermedad como socialmente determinado, cuando las relaciones de labor explotado se materializan en ‘bocas trabajadoras’. De esta forma, se vislumbra la democratización del acceso a la salud bucal, entendida como el derecho a una vida plena.

Abstract The present study evaluated the ability of Bifidobacterium and Lactobacillus species associated with streptococci to increase insoluble extracellular polysaccharide (EPS) production and initial caries lesion progression. Bovine enamel blocks (n = 190; 4 mm x 4 mm) were prepared, selected according to initial surface hardness (SH), and divided into two groups: a) double combinations: S. mutans with Bifidobacterium or Lactobacillus, and b) triple combinations: S. mutans and S. sobrinus with Bifidobacterium or Lactobacillus species. The blocks were exposed to the bacterial associations for 7 days. Subsequently, quantity of EPS from biofilms and caries lesion depth were determined by means of colorimetric and cross-sectional enamel hardness (ΔKHN) analysis. The data were submitted to one-way analysis of variance, followed by the Bonferroni test (p < 0.05). S. mutans with B. animalis or B. dentium produced a higher quantity of EPS; S. mutans + B. animalis led to the highest ∆KHN. S. mutans + S. sobrinus + B. longum induced greater EPS and ∆KHN values. In conclusion, associations of B. animalis and B. longum with streptococci promoted EPS production and caries lesion progression.

Resumo Comemorando os 25 anos da Revista Ciência & Saúde Coletiva (C&SC), o presente artigo analisou 375 documentos publicados entre 2000-2019 neste periódico, como parte integrante da editoria de saúde bucal coletiva. A análise da produção visou compreender como o núcleo de saúde bucal aparece nas publicações e como poderia ter contribuído com o conhecimento sobre a saúde-doença da população, as políticas públicas específicas, o ensino e a gestão dos serviços de saúde bucal no SUS. O processo privilegiou a análise bibliométrica e a de documentos. Foi possível explicitar a distribuição institucional dos autores, sua expressiva rede de colaboradores e a dimensão das citações das publicações inclusive no plano internacional. Os estados brasileiros mais presentes nas publicações foram São Paulo e Minas Gerais, seguidos por autores de Pernambuco, Rio Grande do Sul e Santa Catarina. As citações foram mais frequentemente do Brasil (85,14%), seguido por Estados Unidos (2,31%), Portugal (1,34%) e Austrália (1,34%). Conclui-se que, mesmo com limitações, C&SC se mostrou, inequivocamente, um potente instrumento de divulgação da produção científica na perspectiva da saúde bucal coletiva, possibilitando a divulgação e o intercâmbio de informações, facilitando a integração entre os pesquisadores e possibilitando um caminho da sua consolidação.

Abstract Celebrating the 25 years of existence of the Journal Ciência & Saúde Coletiva (C&SC), this paper analyzed 375 documents published between 2000-2019 as an integral part of the editorial of collective oral health. The production analysis aimed to understand how oral health core appears in publications and how it could have contributed to knowledge on the population’s health-disease, specific public policies, education, and management of oral health services in the SUS. The process employed bibliometric and documental analysis. We could show the authors’ territorial distribution, their extensive collaboration network, and the dimension of citations in publications, including the international plan. The Brazilian states most present in the publications were São Paulo and Minas Gerais, followed by authors from Pernambuco, Rio Grande do Sul, and Santa Catarina. Citations were more frequent in Brazil (85.14%), followed by the United States (2.31%), Portugal (1.34%), and Australia (1.34%). We concluded that, despite the limitations, the C&SC showed unequivocally a powerful instrument for the dissemination of scientific production from the perspective of collective oral health, enabling the exchange of information and facilitating the integration between researchers and enabling a path to its consolidation.
