ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

Resumo A recuperação das pessoas acometidas pela COVID-19 é um processo que continua para além do quadro agudo da infecção pelo Sars-CoV-2. O comprometimento de diversos sistemas corporais pode acarretar impactos funcionais e demandar assistência fisioterapêutica contínua tanto na atenção ambulatorial quanto na atenção primária à saúde (APS). Neste ensaio, busca-se discutir os desafios da organização e da oferta de assistência fisioterapêutica em resposta à pandemia da COVID-19 no Brasil. A análise foi sumarizada em três dimensões: oferta da assistência fisioterapêutica na APS e na atenção ambulatorial, e oferta de atenção fisioterapêutica por telessaúde. Conclui-se que: a reabilitação funcional depende da capacidade de resposta da APS; existe um déficit de serviços de reabilitação que antecede a pandemia, e pode comprometer respostas satisfatórias às demandas do atual contexto epidemiológico; faz-se necessária a articulação entre as equipes de fisioterapia ambulatorial e da APS; embora a telessaúde seja um recurso e uma oportunidade para ampliar o acesso da população à reabilitação funcional, sua utilização exige cautela; o fisioterapeuta possui papel crucial em todo o continuum de cuidados da COVID-19.

The impairment of several body systems can cause functional impacts and demand continuous physical therapy assistance both in outpatient care and in Primary Health Care (PHC). In this essay, we seek to discuss the challenges of organizing and offering Physical Therapy assistance in response to the COVID-19 pandemic in Brazil. The analysis was summarized in three dimensions: offering physical therapy assistance in PHC, outpatient care, via telehealth. It is concluded that functional rehabilitation depends on the PHC’s response capacity; there is a shortage of rehabilitation services that precedes the pandemic, and may not respond satisfactorily to the demands of the current epidemiological context; it is necessary to articulate the outpatient physiotherapy and PHC teams; although telehealth is a resource and an opportunity to increase the population’s access to functional rehabilitation, its use requires caution; the physiotherapist plays a crucial role in the entire COVID-19 care continuum.

Abstract The objective was to produce flour from sorghum grains, to determine the physicochemical and microbiological characteristics, and to develop different formulations of cookies, with total and partial replacement of rice flour, evaluating the moisture content, color, expansion factor and texture profile - TPA. Five formulations were processed, as follows: control - 0%; F1 - 25%; F2 - 50%; F3 - 75% and F4 - 100% sorghum flour. The sorghum flour showed physical, chemical and microbiological characteristics in accordance with current legislation, and can be inserted into human food. The different cookie formulations after baking had a moisture content below 7%. With the addition of sorghum flour, the cookies showed lower light F4 – 32.35 L*. The addition of sorghum flour did not affect the expansion factor of the cookies, with average value of 2.13. The different concentrations of sorghum flour, showed interference in analysis of the Texture-TPA profile, the F3 formulation, presented higher values of hardness 25594.44 N, fracturability 251 N, cohesiveness 0.27. Elasticity has not been compromised. The highest values of gumminess 53.08 N, chewability 1471.15 mJ were found in the formulation F4 – 100% FS. These results may help improve the quality of sorghum-based gluten-free foods for celiac consumers.

RESUMO O DC de Neuroimunologia da ABN e o BCTRIMS trazem, nesse documento, as recomendações sobre vacinação da população com doenças desmielinizantes do sistema nervoso central (SNC) contra infecções em geral e contra o coronavírus da síndrome respiratória aguda grave 2 (SARS-CoV-2), causador da COVID-19. Destaca-se a gravidade do atual momento frente ao avanço da COVID-19 em nosso País, o que torna mais evidente e importante a criação de guia de referência para orientação aos médicos, pacientes e autoridades de saúde pública quanto à vacinação, meio efetivo e seguro no controle de determinadas doenças infecciosa. O DCNI/ABN e o BCTRIMS recomendam que os pacientes com doenças desmielinizantes do SNC (ex., EM e NMOSD) sejam constantemente monitorados, quanto a atualização do seu calendário vacinal, especialmente, no início ou antes da mudança do tratamento com uma droga modificadora de doença (DMD). É importante também salientar que as vacinas são seguras e os médicos devem estimular o seu uso em todos os pacientes. Evidentemente, deve ser dada especial atenção às vacinas com vírus vivos atenuados. Por fim, é importante que os médicos verifiquem qual DMD o paciente está em uso e quando foi feita a sua última dose, pois cada fármaco pode interagir de forma diferente com a indução da resposta imune.
ABSTRACT The Scientific Department of Neuroimmunology of the Brazilian Academy of Neurology (DCNI/ABN) and Brazilian Committee for Treatment and Research in Multiple Sclerosis and Neuroimmunological Diseases (BCTRIMS) provide recommendations in this document for vaccination of the population with demyelinating diseases of the central nervous system (CNS) against infections in general and against the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which causes COVID-19. We emphasize the seriousness of the current situation in view of the spread of COVID-19 in our country. Therefore, reference guides on vaccination for clinicians, patients, and public health authorities are particularly important to prevent some infectious diseases. The DCNI/ABN and BCTRIMS recommend that patients with CNS demyelinating diseases (e.g., MS and NMOSD) be continually monitored for updates to their vaccination schedule, especially at the beginning or before a change in treatment with a disease modifying drug (DMD). It is also important to note that vaccines are safe, and physicians should encourage their use in all patients. Clearly, special care should be taken when live attenuated viruses are involved. Finally, it is important for physicians to verify which DMD the patient is receiving and when the last dose was taken, as each drug may affect the induction of immune response differently.

RESUMO: Este relato descreve a placentite necrotizante causada por Bacillus cereus em uma vaca associada a aborto e mortalidade materna. O diagnóstico etiológico de placentite por B. cereus foi baseado na histopatologia da placenta, citologia e isolamento bacteriano partir do líquido aminiótico em placenta retida e identificação do patógeno pela técnica de MALDI-TOF. Embora abortos por B. cereus sejam esporádicos, a bactéria tem a capacidade de liberar toxinas necrotizantes que podem levar a placentite e aborto.
ABSTRACT: This report described a case of necrotizing placentitis caused by Bacillus cereus in a cow associated with abortion and maternal lethality. The etiological diagnosis of placentitis by B. cereus was based on histopathology of placenta, cytology and bacterial isolation from intrauterine aminiotic fluid in retained placenta and further characterization of the pathogen by the MALDI-TOF. Although, B. cereus abortions are sporadic, the bacterium has the ability to release necrotizing toxins that can lead to placentitis, fetal death and abortion.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

Resumo Objetivo Avaliar a prevalência da atividade física de lazer entre idosos brasileiros e sua associação com conhecimento e participação em programas públicos de promoção dessa prática. Método Estudo transversal com dados de 11.177 pessoas idosas entrevistadas na Pesquisa Nacional de Saúde 2013. A variável resposta foi a prática de atividade física de lazer. As variáveis explicativas foram conhecimento e participação em programas públicos de promoção de atividade física, características sociodemográficas e de estilo de vida. A associação bruta e ajustada foi estimada pela regressão de Poisson com variância robusta. Resultados A maioria das pessoas idosas era do sexo feminino e tinha entre 60 e 69 anos; 17% conheciam, mas não participavam dos programas públicos de promoção de atividade física e somente 3,2% conheciam e participavam. A prevalência de ativos no lazer foi de 13,3%, e na análise ajustada, foi maior entre as pessoas idosas que conheciam, mas não participavam e conheciam e participavam de programas públicos de promoção a atividade física, nos homens, na faixa etária de 60 a 69 anos, com 12 anos ou mais de escolaridade; das regiões Nordeste e Centro-Oeste, com 2 doenças crônicas, que realizaram três ou mais consultas médicas nos 12 últimos meses e consumiam frutas e hortaliças cinco ou mais dias por semana. Conclusão O estudo mostrou uma baixa adesão das pessoas idosas à prática de atividade física, e pouco conhecimento e participação em programas públicos. Esforços são necessários para ampliar a divulgação desses programas, promover o envolvimento da população nessas práticas e contribuir para o envelhecimento ativo.
Abstract Objective To evaluate the prevalence of leisure-time physical activity among elderly Brazilians and its association with knowledge and participation in public programs to promote this practice. Method Cross-sectional study with data from 11,177 older adults from the 2013 National Health Survey. The response variable was the practice of leisure-time physical activity. The explanatory variables were knowledge and participation in public programs to promote physical activity, sociodemographic and lifestyle characteristics. The crude and adjusted association was estimated by Poisson regression with robust variance Results Most of the elderly were female and were between 60 and 69 years old; 17% knew but did not participate in public programs to promote physical activity, and only 3.2% knew and participated. The prevalence of active leisure time was 13.3%. In the adjusted analysis, it was higher among older adults who knew but did not participate and kew and participated in public programs to promote physical activity, in men, aged 60 to 79 years old, with five years old or more education; from the Northeast and Midwest regions, with chronic diseases, who had three or more medical appointments in the last 12 months and consumed fruits and vegetables five or more days a week. Conclusion The study showed a low adherence of the elderly to the practice of physical activity, and little knowledge and participation in public programs. Efforts are needed to expand the dissemination of these programs, promote the involvement of the population in these practices, and contribute to active aging.

RESUMO Objetivo: Determinar a idade em que a puberdade começa em meninos e coletar dados antropométricos de participantes em diferentes fases da puberdade. Métodos: Trata-se de um estudo transversal no qual foram avaliados 430 meninos, uma amostra aleatória representativa da população total de 48.390 estudantes de escolas públicas e particulares de Uberaba, Minas Gerais. Os critérios de inclusão foram: ser do sexo masculino, ter idade de 5 a 18 anos e ausência de doenças prévias. Os participantes e seus responsáveis preencheram um questionário semiestruturado com perguntas pertinentes à sua puberdade e à de seus pais. Os dados foram considerados significantes para p<0,05, e os intervalos de confiança calculados foram de 95%. Resultados: A média de idade encontrada no estágio G2 foi de 11,2±1,8 anos, sendo que 95% dos participantes em G2 tinham entre 9,2 a 13,4 anos. Quanto à pubarca, a média de idade foi de 11,0±1,6 anos, e 95% dos participantes apresentaram pubarca entre 8,0 e 14,0 anos. Quando tais resultados foram comparados aos intervalos de confiança de dois estudos clássicos sobre o tema, houve tendência à pubarca mais precoce. Além disso, a média de idade da pubarca nos pais das crianças foi de 12,1±1,4 anos, significantemente maior em relação à dos filhos (p<0,001). Conclusões: Os resultados indicam uma tendência secular em direção à diminuição da idade da pubarca e um possível início mais precoce da puberdade. É muito importante considerar esses parâmetros para estabelecer políticas públicas destinadas a prevenir esses eventos iniciais.
ABSTRACT Objective: To determine the age of puberty onset in boys and collect anthropometric data of participants at different puberty stages. Methods: This is a cross-sectional study that assessed 430 boys in a random sample representing 48,390 students from public and private schools from the city of Uberaba, Southeast Brazil. The inclusion criteria were males, aged between 5 and 18 years, and absence of previous diseases. Participants and their guardians filled a semistructured questionnaire with questions relevant to their and their parents’ puberty. We set the significance at p<0.05 and calculated the 95% confidence intervals. Results: The mean age found in the puberty stage G2 was 11.2±1.8 (95% of participants in stage G2 were 9.2-13.4 years old). Pubarche data showed a mean of age of 11.0±1.6 years (95% of the participants experienced pubarche when they were 8.0-14.0 years old). When compared to the confidence intervals of two classical studies on the subject, our results showed a trend toward earlier pubarche. In addition, the mean age of this event in the children’s parents was of 12.1±1.4 years, which was significantly higher than the age of the children’s pubarche (p<0.001). Conclusions: These results indicate a secular decreasing trend in pubarche age and an earlier puberty onset. Considering these parameters, is important to design public policies aimed at preventing these early events.

Physical activity among women of low socioeconomic status living with HIV in two major cities of Brazil and Mozambique: A cross-sectional comparative study

OBJECTIVES: Physical activity (PA) may reduce cardiovascular risk and preserve functional capacity of people living with human immunodeficiency virus (HIV). However, only limited research objectively measured PA in patients with low socioeconomic status (SES) in low-income countries, particularly in South America and sub-Saharan Africa. This study compared the PA assessed by accelerometers in women of low SES living with HIV under common antiretroviral therapy (cART) from two major cities in Brazil (Rio de Janeiro, n=33; 40.1±6.1 years) and Mozambique (Maputo, n=50; 38.8±8.7 years). METHODS: Eligible women wore triaxial accelerometers during seven consecutive days, to estimate their habitual PA and daily energy expenditure. RESULTS: The proportion of participants with overweight/obesity was greater in Rio than Maputo (57% vs. 30%; p=0.021), as well as those classified as sedentary based on steps/day (45% vs. 22%; p=0.02). Sedentary time was prevalent (Median±IQD: Rio-1236±142 vs. Maputo-1192±135 min/day; p=0.15). Time spent in PA was short, but Brazilians exhibited lower amount of light (111±56 vs. 145±51 min/day; p<0.001) and moderate-to-vigorous PA (88±3 vs. 64±36 min/day; p=0.001) vs. Mozambicans. The proportion of patients performing 60 min/day of moderate-to-vigorous PA were 58% (Rio) and 82% (Maputo), respectively. Despite of this, estimated daily energy expenditure was equivalent in both groups (1976±579 vs. 1933±492 kcal; p=0.731). CONCLUSIONS: Women with low SES living with HIV in Maputo were more active vs. patients from Rio de Janeiro. Albeit sedentary behavior was prevalent, the proportion of patients complying with the minimum recommended PA for health was higher than values usually reported in developed countries.

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital’s electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. ClinicalTrials.gov: NCT04378582. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

Resumen El objetivo fue identificar el acceso a la información sobre sustancias psicoactivas y el consumo de éstas por agentes prisionales. Se trata de un estudio cualitativo exploratorio. Fue realizado en una cadena pública en el estado de Mato Grosso, Brasil entre agosto y septiembre de 2017, por medio de una entrevista semiestructurada.Se observó que los profesionales, en su mayoría, usan sustancias psicoactivas, exclusivamente alcohol y tabaco, con bajo consumo. Entre los medios más utilizados, prevaleció la electrónica. Se verificó que, cuanto mayor tiempo con algunos medios, mayor será la oportunidad de acceso a propagandas sobre sustancias psicoactivas. Aunque este acceso no ha representado aprendizajes que ayuden a la adhesión a estilos de vida más saludables, sin el uso de esas sustancias. Se concluye que la inclusión de actividades educativas en ese servicio permitirá que las informaciones sobre esas sustancias, así como otros contenidos importantes del cotidiano institucional o vehiculados en los medios impactan de forma positiva en la calidad de vida de ese grupo.
Resumo O objetivo foi identificar o acesso às informações sobre substâncias psicoativas e o consumo destas por agentes prisionais. Trata-se de estudo exploratório e qualitativo. Foi realizado em umacadeia pública no estado de Mato Grosso, Brasil entre agosto e setembro de 2017, por meio de entrevista semiestruturada. Observou-se que os profissionais, em suamaioria, usam substâncias psicoativas, exclusivamente álcool e tabaco, combaixo consumo. Entre as mídiasmais utilizadas, prevaleceu a eletrônica. Verificou-se que, quanto maior tempo comalgumamídia, maior será a oportunidade de acesso a propagandas sobre substâncias psicoativas. Embora ese acesso não tenhare presentado aprendizados que auxiliem a adesão para estilos de vida mais saudáveis, sem o uso dessassubstâncias. A inclusão de atividades educativas nesseserviço permitirá que as informações sobre esas substâncias, bem como outros conteúdos importantes do cotidiano institucional ou veiculadosnas mídias impactem de forma positiva naqualidade de vida desse grupo.
Abstract Theo bjective wasto identify the accessto information about psychoactive substances and their consumption by prison agents. Thisis a qualitative and exploratory study. It was carriedout in a publicjail in thes tateof Mato Grosso, Brazil between August and Septembervof 2017, through a semi-structured interview. It was observed that professionals, for the most part, use psychoactive substances, exclusively alcohol and tobacco, with low consumption. Among the mos tused media, electronics prevailed. It has been found that the more time with some media, the greater the opportunity of Access to advertisements on psychoactive substances. Although this Access did not represent learning that supports adherence to healthier lifestyles, without the use of these substances. The inclusión of educational activities in this service will allow the information about these substances, as well as other important contents of the institutional daily or transmitted in the media, to positively impact the quality of life of this group.

Abstract Introduction: Coronary artery bypass grafting (CABG) is the most frequently performed heart surgery in Brazil. Recent international guidelines recommend that national societies establish a database on the practice and results of CABG. In anticipation of the recommendation, the BYPASS Registry was introduced in 2015. Objective: To analyze the profile, risk factors and outcomes of patients undergoing CABG in Brazil, as well as to examine the predominant surgical strategy, based on the data included in the BYPASS Registry. Methods: A cross-sectional study of 2292 patients undergoing CABG surgery and cataloged in the BYPASS Registry up to November 2018. Demographic data, clinical presentation, operative variables, and postoperative hospital outcomes were analyzed. Results: Patients referred to CABG in Brazil are predominantly male (71%), with prior myocardial infarction in 41.1% of cases, diabetes in 42.5%, and ejection fraction lower than 40% in 9.7%. The Heart Team indicated surgery in 32.9% of the cases. Most of the patients underwent cardiopulmonary bypass (87%), and cardioplegia was the strategy of myocardial protection chosen in 95.2% of the cases. The left internal thoracic artery was used as a graft in 91% of the cases; the right internal thoracic artery, in 5.6%; and the radial artery in 1.1%. The saphenous vein graft was used in 84.1% of the patients, being the only graft employed in 7.7% of the patients. The median number of coronary vessels treated was 3. Operative mortality was 2.8%, and the incidence of cerebrovascular accident was 1.2%. Conclusion: CABG data in Brazil provided by the BYPASS Registry analysis are representative of our national reality and practice. This database constitutes an important reference for indications and comparisons of therapeutic procedures, as well as to propose subsequent models to improve patient safety and the quality of surgical practice in the country.
