Abstract Trust in institutions and democracy. The cases of young Spanish and Latin Americans This article examines data from the Jóvenes en Iberoamérica 2021 survey, coordinated by the SM Foundation of Spain, which interviewed 13,500 young people aged between 15 and 29 from Argentina, Brazil, Chile, Colombia, Ecuador, Spain, Mexico, Peru and the Dominican Republic between March 2019 and April 2020. The study aimed to measure the perception of these young people regarding the determinants of trust in relation to political and social institutions that constitute, in contemporary societies, the fundamental bases for the exercise of democratic life. The data shows, in general, a low level of trust in institutions among young people in these countries.

Resumo Confiança nas instituições e democracia. Os casos dos jovens espanhóis e latinoamericanos Este artigo examina dados da pesquisa Jóvenes en Iberoamérica 2021, coordenada pela Fundação SM da Espanha, que entrevistou 13.500 jovens com idade entre 15 e 29 anos da Argentina, Brasil, Chile, Colômbia, Equador, Espanha, México, Peru e República Dominicana entre março de 2019 e abril de 2020. O estudo teve como objetivo medir a percepção desses jovens a respeito dos determinantes da confiança em relação a instituições políticas e sociais que constituem, nas sociedades contemporâneas, as bases fundamentais para o exercício da vida democrática. Os dados mostram, em geral, um baixo nível de confiança nas instituições entre os jovens desses países.

Resumen Este artículo examina datos de la encuesta Jóvenes en Iberoamérica 2021, coordinada por la Fundación SM, de España, que entrevistó a 13.500 jóvenes de entre 15 y 29 años de Argentina, Brasil, Chile, Colombia, Ecuador, España, México, Perú y República Dominicana, entre marzo de 2019 y abril de 2020. El estudio tuvo como objetivo medir la percepción de estos jóvenes sobre los determinantes de la confianza en relación con las instituciones políticas y sociales que constituyen, en las sociedades contemporáneas, las bases fundamentales para el ejercicio de la vida democrática. Los datos muestran, en general, un bajo nivel de confianza en las instituciones entre los jóvenes de estos países.

ABSTRACT Objective To evaluate the time interval and possible delay in transportation to referral units for the treatment of testicular torsion. Methods We retrospectively analyzed all cases of spermatic cord torsion surgically treated at a university hospital between January 2018 to December 2021. We evaluated the time intervals, including pain onset until the first presentation (D1), interhospital transference time (D2), pain onset until urological evaluation in a tertiary service (D3), urological evaluation until surgery (D4), and time from pain onset to surgical treatment (D5). We analyzed demographic and surgical data, orchiectomy rates, and time intervals (D1–D5). Torsions presented to the first medical presentation within 6h were considered early for testicular preservation. Results Of the 116 medical records evaluated, 87 had complete data for the time interval analysis (D1 to D5) and were considered the total sample. Thirty-three had D1 ≤6h, 53 had D1 ≤24h (includes patients in the D1 ≤6h subgroup), and 34 had D1 >24h. The median time intervals of the total samples and subgroups D1 ≤6h, D1 ≤24h, and D1 >24h were D1 = 16h 42min, 2h 43min, 4h 14min and 72h, D2 = 4h 41min, 3h 39min, 3h 44min and 9h 59min; D3 = 24h, 6h 40min, 7h and 96h; D4 = 2h 20min, 1h 43min, 1h 52min and 3h 44min; D5 = 24h 42min, 8h 03min, 9h 26min and 99h 10min, respectively. Orchiectomy rates of the total sample, subgroups D1 ≤6h, D1 ≤24h, and D1 >24h were 56.32%, 24.24% (p<0.01), 32.08% (p<0.01), and 91.18% (p<0.01), respectively. Conclusion Late arrival at the emergency department or a long interhospital transference time determined a large number of patients who underwent orchiectomy. Thus, public health measures and preventive strategies can be developed based on the data from this study aiming to reduce this avoidable outcome.

ABSTRACT Organic fertilization favors the physical and chemical attributes improvement of the soil and promotes the use of waste that would be discarded in the environment. This study aimed to identify the appropriate levels of rabbit manure compost (RMC) from rabbit farming in the production of substrates for lettuce. The experiment was implanted in plastic pots using 5 treatments: 0%, 25%, 50%, 75% and 100% of RMC mixed with commercial substrate, in a completely randomized design with 10 replications. The analyzed variables were number of leaves per plant, shoot green matter, root green matter, shoot dry matter and root dry matter. There was an increase in number of leaves with the increase of the RMC doses incorporated into the substrate, up to the dose of 59.46%. The shoot green matter had its best response on 61.42% of RMC. In both variables there was a decline afterwards. The root green matter started with an increase up to the dose of 36.14% of RMC, followed by a decline, and another increase, not reaching the same values obtained at the beginning of the curve. An increase was observed in the shoot dry matter as the amount of RMC increased, up to the limit of 45.09%, and then declined. The root green matter showed an increase until approximately 25% of RMC, stabilizing on 75% of the compost. In general, values close to 60% of RMC mixed with the substrate presented the best results. We can conclude that the RMC can be used to promote a greater number of leaves, which are the economically viable parts of lettuce culture.

RESUMO A adubação orgânica favorece a melhoria de atributos físicos e químicos do solo e promove a utilização de dejetos que seriam descartados no meio ambiente. Este trabalho teve como objetivo identificar os níveis adequados do composto de esterco de coelhos (CEC) proveniente da cunicultura, na produção de substratos para a alface crespa. O experimento foi implantado em vasos plásticos utilizando 5 tratamentos: 0%, 25%, 50%, 75% e 100% de CEC misturado a substrato comercial, em delineamento inteiramente casualizado com 10 repetições. As variáveis analisadas foram número de folhas por planta, massa verde da parte aérea, massa verde da raiz, massa seca da parte aérea e massa seca da raiz. Para a variável número de folhas houve incremento com o aumento da dose de CEC incorporado ao substrato, até a dose de 59,46%. A massa verde de parte aérea teve sua melhor resposta na dose de 61,42%. Em ambas as variáveis houve declínio em seguida. Já a massa verde de raiz iniciou com incremento até a dose de 36,14% de CEC, seguido de declínio, e novamente aumento, não atingindo os mesmos valores obtidos no início da curva. Com relação à massa seca de parte aérea percebeu-se incremento das medidas conforme aumento da dose de CEC, até o limite de 45,09%, apresentando posterior declínio. Já a massa verde de raiz mostrou aumento até a dose de aproximadamente 25% de CEC, com estabilização a partir da dose 75% de composto. De modo geral, os valores próximos a 60% de CEC misturado ao substrato, apresentaram os melhores resultados, podendo concluir que o CEC pode ser utilizado para promover maior número de folhas, que são as partes economicamente viáveis da cultura da alface.

Objective: to describe the spatial distribution, treatment status and characteristics of cases of people infected with the human immunodeficiency virus HIV in Santa Cruz do Sul, RS, 2001 to 2020. Methods: descriptive study with data from individuals undergoing treatment for HIV/AIDS, in Santa Cruz do Sul, diagnosed from January 2001 to October 2020. Results: 708 (94.4%) cases were analyzed, of these (58.2%) were male, with a mean age of 39 years, the maximum incidence rate was in 2019 (59.4/100,000 in habitants), there was a high frequency of cases in the south and central region of the city, 92.9% of these individuals were still in active treatment and the dropout rate was 7.1% in the period. Conclusion: a higher incidence of HIV was observed in adult male, from the central and southern regions of the city, with a treatment rate close to the goals of the World Health Organization and a low dropout rate.

Objetivo: describir la distribución espacial, estado de tratamiento y características de los casos de personas infectadas por el virus de la inmunodeficiencia humana en Santa Cruz do Sul, RS, 2001 hasta 2020. Métodos: estudio descriptivo con datos de individuos en tratamiento por VIH/SIDA, en Santa Cruz do Sul, diagnosticados de enero de 2001 a octubre de 2020. Resultados: se analizaron 708 (94,4%) casos, de estos (58,2%) eran del sexo masculino, con una edad media de 39 años, la tasa de incidencia máxima fue en 2019 (59,4/100.000 habitantes), hubo una alta frecuencia de casos en la región sur y centro del municipio, el 92,9% de estos individuos seguían en tratamiento activo y la deserción fue del 7,1% en el período. Conclusión: se observó una mayor incidencia de VIH en hombres adultos, de las regiones centro y sur de la ciudad, con una tasa de tratamiento cercana a las metas de la Organización Mundial de la Salud y una baja tasa de deserción.

Objetivo: descrever a distribuição espacial, a situação do tratamento e as características dos casos de pessoas infectadas pelo vírus da imunodeficiência humana (HIV), em Santa Cruz do Sul/RS, de 2001 a 2020. Métodos: estudo descritivo com dados de indivíduos em tratamento para HIV/aids, em Santa Cruz do Sul, diagnosticados no período de janeiro de 2001 a outubro de 2020. Resultados: foram analisados 708 (94,4%) casos, dos quais 58,2% eram do sexo masculino, com idade média de 39 anos. A taxa de incidência máxima foi registrada em 2019 (59,4/100 mil habitantes), e observou-se frequência elevada de casos nas regiões sul e central do município; 92,9% dos indivíduos continuavam em tratamento ativo, e a taxa de abandono alcançou 7,1% no período. Conclusão: observou-se uma maior incidência de HIV entre adultos do sexo masculino, das regiões central e sul da cidade, com uma taxa de tratamento próxima às metas da Organização Mundial da Saúde (OMS) e baixa taxa de abandono.

Abstract This research aimed to investigate the association between the skin temperature variation of the skin identified by thermography and serum CK values in healthy subjects based on a systematic review with meta-analysis. A systematic literature search was performed in June 2021 in the SCOPUS, MEDLINE, and Web of Science databases (2000 to 2021). MeSH and DeCS platforms were used to find the descriptors in English referring to the terms “thermography” and “creatine kinase” with their synonyms (nine and 13, respectively) to find studies, following the PIRT strategy: Participants (healthy subjects); Index Test (thermography); Reference Test (CK collection); Target Condition (having analyzed the results of the Index and Reference Test). A meta-analysis was performed to calculate the association between variables. A total of 483 files were retrieved from the databases, and 13 were selected for analysis (five met the criteria for meta-analysis). Considering the 13 studies reviewed, there is a general balance in obtaining results of CK increase along with skin temperature versus studies that didn’t observe any association between the measures. However, the meta-analysis showed no significant association between the variables analyzed (r= –0.05; p= 0.06; 95%CI –0.11 – 0.00). This result suggests that the result of one test cannot estimate the result of the other. It was concluded that there is no good association between CK levels and thermography results; it’s recommended the use of thermal images in addition to other evaluation methods in order to assist in the determination of special differential diagnoses of musculoskeletal injuries.

Abstract Background The Parkinson disease (PD) is frequently associated with autonomic dysfunctions. However, data regarding the influence of PD on the autonomic responses to postural changes is limited. Objective To analyze and compare the autonomic responses, evaluated through linear and non-linear methods of heart rate variability, and cardiorespiratory parameters in two groups: Parkinson disease (PDG) and control (CG), at rest and during the active tilt test. Methods A total of 48 participants were analyzed (PDG: n = 25;73.40 ± 7.01 years / CG: n = 23;70.17 ± 8.20 years). The autonomic modulation and cardiorespiratory parameters were evaluated at rest and during the active tilt test. To assess the autonomic modulation the linear indices, at the time (rMSSD, SDNN) and frequency (LF, HF, LF/HF) domains, and the non-linear indices, obtained through the Poincaré plot (SD1, SD2, SD1/SD2), were calculated. The cardiorespiratory parameters evaluated were heart rate (HR), systolic (SBP), and diastolic blood pressure (DBP), peripheral oxygen saturation (SpO2), and respiratory rate. Results At rest, the PDG presented significantly lower values of rMSSD, SDNN, LF, HF, SD1, SD2, and DBP, and higher values of SpO2. During test, in the PD group, modifications were observed in HR, and SBP, besides a reduced parasympathetic response, and an increased global modulation. The qualitative analysis of the Poincaré plot showed that the PDG has a lower dispersion of the RR intervals during rest and the active tilt test. Conclusion Individuals with PD present reduced global variability and parasympathetic modulation at rest, and reduced parasympathetic response and damage in HR regulation when performing the active tilt test, compared with controls.

Resumo Antecedentes A doença de Parkinson (DP) está frequentemente associada a disfunções autonômicas. Porém, dados sobre a influência da DP nas respostas autonômicas às mudanças posturais são limitados. Objetivos Analisar e comparar as respostas autonômicas, avaliadas por métodos lineares e não lineares de variabilidade da frequência cardíaca e parâmetros cardiorrespiratórios em dois grupos: DP (GDP) e controle (CG), em repouso e durante o tilt test ativo. Métodos Foram analisados 48 participantes (GDP: n = 25;73,40 ± 7,01 anos/GC: n = 23; 70,17 ± 8,20 anos). A modulação autonômica e os parâmetros cardiorrespiratórios foram avaliados em repouso e durante o tilt test ativo. Para avaliar a modulação autonômica foram calculados os índices lineares, nos domínios do tempo (rMSSD, SDNN) e frequência (LF, HF, LF/HF), e os índices não lineares, obtidos através do plot de Poincaré (SD1, SD2, SD1/SD2). Os parâmetros cardiorrespiratórios avaliados foram frequência cardíaca (FC), pressão arterial sistólica (PAS), diastólica (PAD), saturação periférica de oxigênio (SpO2) e frequência respiratória. Resultados Em repouso, o GDP apresentou valores menores de rMSSD, SDNN, LF, HF, SD1, SD2 e PAD, e maiores valores de SpO2. No teste, foram observadas modificações na FC e na PAS, redução da resposta parassimpática e aumento da modulação global no GDP. A análise qualitativa do plot de Poincaré mostrou que o GDP apresentou menor dispersão dos intervalos RR no repouso e no tilt test ativo. Conclusão Indivíduos com DP apresentam redução da variabilidade global e modulação parassimpática em repouso, redução da resposta parassimpática e prejuízo na regulação da FC ao realizar o tilt test ativo, em comparação aos controles.

ABSTRACT Objective: Evaluating the prevalence of sarcopenia in women submitted to bariatric surgery – Roux-en-Y gastric bypass. Design: Observational, cross-sectional study. Subjects and methods: Women (18-65 years old) who underwent bariatric surgery (BG) ≥ 2 years and reached stable weight ≥ 6 months, were investigated. Control group (CG) comprised non-operated matched women with obesity. Body composition was determined through dual-energy X-ray absorptiometry. Low lean mass (LLM) was defined as appendicular lean mass index (ALM kg/height m2) < 5.5 kg/m2. Physical strength was assessed through dynamometer and sit-to-stand test (SST), whereas performance was assessed through 4-m gait speed and Short Physical Performance Battery Tests (SPPB). Sarcopenia was diagnosed in the presence of LLM and low strength. Results: One-hundred and twenty women (60 in each group, 50 ± 9.7 years old) were investigated. All anthropometric and body composition parameters were lower in BG than in CG, whereas strength and performance were similar between groups. Women with reduced strength presented high total fat mass and low physical activity level (p < 0.005). LLM was observed in 35% of BG and in 18.3% of CG (p = 0.04), whereas sarcopenia was diagnosed in 28.3% of BG and in 16.6% of CG (p = 0.12). Sarcopenic women in BG had better performance both in SST (p = 0.001) and SPPB (p = 0.004). Total lean mass (OR:1.41, 95% CI [1.18; 1.69], p < 0.001) and obesity (OR: 38.2 [2.27; 644.12], p < 0.001) were associated with sarcopenia in the multivariate analysis. Conclusion: Despite great weight loss, sarcopenia prevalence did not increase in BG and its presence was influenced by total lean mass and obesity.

Resumo O artigo se insere na linha de pesquisas sobre participação eleitoral em disputas políticas locais. A partir de um modelo multivariado, tem o objetivo de analisar os efeitos de variáveis políticas associadas ao efeito da pandemia da Covid-19 na abstenção eleitoral das eleições municipais de 2020 no Brasil. São consideradas as disputas para prefeito de todos os 5.568 municípios com eleições municipais para responder à pergunta: onde e quem mais se absteve nas eleições de 2020? Os resultados mostram que, do ponto de vista agregado, municípios com mais mortes por Covid-19 tenderam a ter maior abstenção. Porém, o número de candidatos, usado como indicador de competição eleitoral, exerceu forte efeito contrário. No nível individual, os dados oficiais do Tribunal Superior Eleitoral (TSE) indicam que homens, com baixa escolaridade e idade acima de 40 anos tenderam a participar menos das eleições de 2020.

Abstract In the present research we examines the electoral abstention. The paper is about the voter participation in political disputes. The objective is to analyze the effects of political variables associated with the effect of the Covid-19 pandemic on electoral abstention in the 2020 local elections in Brazil. The disputes of all 5,568 municipalities are considered to answer the question: who and where else abstained in the 2020 elections? The aggregate results show that municipalities with more deaths from Covid-19 tended to have higher abstention. However, the number of candidates, used as an indicator of electoral competition, exerted a strong opposite effect - in favor of turnout. At the individual level, the data indicate that men, low schooling and age over 40 years tended to participate less in the 2020 elections.

Resumen En Brasil, la Ley no. 11.340 de 2006, conocida popularmente como Ley Maria da Penha, tiene como objetivo educar a la sociedad acerca de la violencia doméstica y familiar contra las mujeres, además de combatirla y prevenirla. El objetivo de esta investigación es ubicar la referida ley en el campo de las políticas públicas brasileñas. La metodología utilizada fue cualitativa de carácter básico descriptivo. El procedimiento adoptado fue una revisión bibliográfica acerca de la violencia intrafamiliar, políticas públicas y de género, así como una investigación documental de la legislación pertinente. En 1983, Maria da Penha Maia Fernandes sufrió dos intentos de homicidio por parte de su pareja; con el apoyo de movimientos feministas, remitió una denuncia a la Comisión Interamericana de Derechos Humanos, dando a conocer internacionalmente la violencia que estaba siendo perpetrada por el Estado brasileño. Su lucha fue fundamental para el reconocimiento público de los derechos de las mujeres.

Resumo O Brasil possui a Lei no. 11.340 de 2006, popularmente conhecida como Lei Maria da Penha, que visa o combate, educação e prevenção da violência doméstica e familiar contra a mulher. O objetivo desta pesquisa é situar a referida lei no campo das políticas públicas brasileiras. A metodologia empregada foi de abordagem qualitativa, de natureza básica e com o objetivo de ser descritiva. O procedimento adotado foi levantamento bibliográfico em livros e artigos sobre violência doméstica, políticas públicas e gênero, bem como pesquisa documental na legislação pertinente. Em 1983 Maria da Penha Maia Fernandes sofreu duas tentativas de homicídio perpetradas pelo seu próprio companheiro. Maria da Penha, com o apoio de movimentos feministas, encaminhou uma denúncia à Comissão Interamericana de Direitos Humanos levando ao conhecimento internacional a violência que estava sendo perpetrada pelo Estado brasileiro. A sua luta foi fundamental para o reconhecimento público do direito das mulheres.

Abstract In Brazil, the 2006 Act no. 11.340, popularly known as the Maria da Penha Act, seeks to educate society about domestic and family violence against women, in addition to combating and preventing it. The purpose of this research is to place the aforementioned act in the field of Brazilian public policies. Basic descriptive qualitative methodology was used. The procedure involved a bibliographic review of domestic violence, public and gender policies, together with documentary research on related legislation. In 1983, Maria da Penha Maia Fernandes’s partner attempted to kill her on two occasions. With the support of feminist movements, she submitted a complaint to the Inter-American Commission on Human Rights, internationally publicizing the violence that was being perpetrated by the Brazilian State. Her struggle was instrumental in the public recognition of women’s rights.

Abstract The objective of this study was to prepare three donkey milk yoghurt formulations mixed with goat, cow and buffalo milk. The formulations were developed with 50% donkey milk and 50% cow, buffalo or goat milk. Concentrated mango pulp was added for flavouring. An evaluation was performed on the chemical composition of each milk sample and the formulated yoghurts. The donkey milk showed the lowest values for fat, protein, casein, total solids and non-fat solids. In turn, goat milk reached higher levels of fat and total solids. Furthermore, buffalo milk stood out with the highest values of protein and non-fat solids. Yoghurt made with donkey and buffalo milk showed higher concentrations of protein, lactose, total solids and non-fat solids. In contrast, the yoghurt that contained donkey and cow milk in its formulation obtained a higher fat percentage and a lower percentage of lactose, total solids and non-fat solids. The low content of fat and total solids in donkey milk, in addition to the structure of the casein micelles and small fat globules favor low consistency in fermented products. Thus, mixing donkey milk with milks from other animal species can result in better chemical composition and better consistency.

Resumo O artigo analisa a violência política durante o ciclo das eleições municipais brasileiras de 2020, com dois objetivos: discutir teoricamente a violência eleitoral como um tipo específico de violência política; e medir a incidência de cinco modalidades de violência (homicídios, atentados, ameaças, agressões e sequestros) contra lideranças políticas locais, a partir do monitoramento diário da imprensa. No total, foram identificados 485 casos entre janeiro de 2019 e dezembro de 2020. As vítimas são majoritariamente do sexo masculino, brancos, com alta escolaridade, dos pequenos municípios e filiados a partidos da centro-direita. No entanto, o modelo log-linear estimado sob o paradigma bayesiano revela que a chance de um homicídio ocorrer é maior contra políticos não brancos e de baixa escolaridade.

Abstract The article analyzes political violence during the 2020 municipal election cycle. The article pursues two objectives. The first theoretically discusses electoral violence as a specific kind of political violence, while the second aims to measure the incidence of five types of violence (homicides, attacks, threats, assaults and kidnappings) against local political leaders, based on daily press monitoring. In total, 485 cases were identified between January 2019 and December 2020. The victims are mostly male, white, highly educated, from small municipalities and affiliated to center-right parties. However, the log-linear model estimated under the Bayesian paradigm reveals that the chance of a homicide occurring is greater against non-white, low-educated politicians.

Abstract This study aimed to develop Greek style yoghurts with the addition of bovine colostrum. Four yoghurt formulations were produced with different colostrum content (0%, 10%, 20% and 30%). The fat, protein, total solids, ash, retinol levels were quantified as well as the colour was measured. A sensory analysis was conducted with 103 untrained panelists, evaluating the attributes of appearance, aroma, consistency, flavour, overall acceptance and the purchase intention. Colostrum addition increased the fat and protein levels in the yoghurt formulations. The Greek style yoghurts with bovine colostrum reached sensory acceptance above 70% for all attributes.

Resumo Este artigo analisa a propaganda negativa como estratégia eleitoral. O objetivo é avaliar três questões básicas: quem atacar, quando atacar e de que maneira atacar. Para isso, aplicamos um survey a um grupo de consultores políticos brasileiros a fim de verificar como o uso da estratégia varia de acordo com as circunstâncias. Os resultados indicam que ataques ao adversário devem ser usados na parte final da campanha, tendo como foco suas propostas e, preferencialmente, via redes sociais. A decisão sobre quem atacar é influenciada pela expectativa de vitória e posicionamento nas pesquisas, mas mudanças nas intenções de voto modificam o alvo dos ataques e a intensidade com que a propaganda negativa deve ser usada.

Abstract This article analyzes negative advertising as an electoral strategy. The objective is to assess three basic questions: who to attack, when to attack and how to attack. To do so, we conducted a research survey with a sample of Brazilian political consultants in order to understand how negative campaign strategies vary according to the circumstances. The results indicate that the decision to attack the opponent should be used in the final stage of the campaign, focusing on the opponent’s proposals and preferably via social networks. The decision about who to attack is influenced by the expectation of victory and positioning in the polls, but the dynamics of voting intentions change the target of the attacks and the intensity with which negative advertising should be used.

Resumen Este artículo analiza la publicidad negativa como estrategia electoral. El objetivo es evaluar tres cuestiones básicas: ¿a quién atacar, cuándo atacar y cómo atacar? Con este fin, aplicamos una encuesta a un grupo de consultores políticos brasileños para verificar cómo varía el uso de las estrategias de campaña negativas según las circunstancias. Los resultados indican que los ataques al oponente deben ser utilizados en la parte final de la campaña, que debe estar centrada en las propuestas del oponente y debe realizarse preferiblemente a través de las redes sociales. La decisión sobre a quién atacar está influenciada por la expectativa de victoria y el posicionamiento en las encuestas, pero los cambios en las intenciones de voto cambian el objetivo de los ataques y la intensidad con la que se debe utilizar la publicidad negativa.

RESUMO Objetivo. Examinar a viabilidade do uso de inteligência artificial como um método sensível e específico de triagem de COVID-19 em pacientes com afecções respiratórias, empregando imagens obtidas por exame de tomografia do tórax e uma plataforma de telemedicina. Métodos. Entre março de 2020 e junho de 2021, foi realizado um estudo observacional descritivo multicêntrico sobre a viabilidade do uso de inteligência artificial (IA) para a triagem de COVID-19, empregando imagens do tórax de pacientes com afecções respiratórias atendidos em hospitais da rede pública. O diagnóstico das imagens obtidas em tomografia do tórax foi realizado por meio de uma plataforma de IA e, em seguida, cotejado com o diagnóstico molecular (RT-PCR) para determinar a concordância entre os métodos utilizados e analisar a viabilidade deste processo para a triagem de pacientes com suspeita de COVID-19. As imagens e os resultados do exame diagnóstico foram disponibilizados em uma plataforma de telemedicina. Resultados. Foi realizada a triagem de 3 514 pacientes com suspeita de COVID-19 atendidos em 14 hospitais de todo o país. Os pacientes, na sua maioria, tinham entre 27 e 59 anos de idade ou pertenciam à faixa etária acima de 60 anos, com média de idade de 48,6 anos, sendo que 52,8% eram do sexo masculino. Os achados mais comuns foram pneumonia grave, pneumonia bilateral com derrame pleural, enfisema pulmonar bilateral e opacidade difusa em vidro fosco, entre outros. Verificou-se, em média, 93% de concordância e 7% de discordância entre as imagens analisadas com uso de IA e os resultados do exame de RT-PCR, com uma sensibilidade de 93% e especificidade de 80% desse sistema de triagem. Conclusões. Demonstrou-se que o uso de um sistema de IA sensível e específico é viável nos hospitais públicos do Paraguai para a detecção rápida estratificada de COVID-19 em pacientes com afecções respiratórias, empregando imagens de exame de tomografia do tórax e uma plataforma de telemedicina.

ABSTRACT Objective. Study the feasibility of using artificial intelligence as a sensitive and specific method for COVID-19 screening in patients with respiratory conditions, using chest CT scan images and a telemedicine platform. Methods. From March 2020 to June 2021, the authors conducted an observational descriptive multicenter feasibility study based on artificial intelligence (AI) for COVID-19 screening using chest images of patients with respiratory conditions who presented at public hospitals. The AI platform was used to diagnose chest CT scan images; this was then compared with molecular diagnosis (RT-PCR) to determine whether they matched and to analyze the feasibility of AI for screening patients with suspected COVID-19. A telemedicine platform was used to send images and diagnostic results. Results. Screening of 3 514 patients with a suspected COVID-19 diagnosis was performed in 14 hospitals around the country. Most patients were aged 27 to 59 years, followed by those over 60. The average age was 48.6 years; 52.8% were male. The most frequent findings were severe pneumonia, bilateral pneumonia with pleural effusion, bilateral pulmonary emphysema, and diffuse ground glass opacity, among others. There was an average of 93% matching and 7% mismatching between images analyzed by AI and RT-PCR. Sensitivity and specificity of the AI system, obtained by comparing AI and RT-PCR screening results, were 93% and 80% respectively. Conclusions. The use of sensitive and specific AI for stratified rapid detection of COVID-19 in patients with respiratory conditions by using chest CT scan images and a telemedicine platform in public hospitals in Paraguay is feasible.

RESUMEN Objetivo. Estudiar la factibilidad de utilización de la inteligencia artificial como método sensible y específico para el cribado de COVID-19 en pacientes con afecciones respiratorias empleando imágenes de tórax obtenidas con tomógrafo y una plataforma de telemedicina. Métodos. Entre marzo del 2020 y junio del 2021 se realizó un estudio observacional descriptivo multicéntrico de factibilidad basada en inteligencia artificial (IA) para el cribado de COVID-19 en imágenes de tórax de pacientes con afecciones respiratorias que acudieron a hospitales públicos. El diagnóstico de las imágenes tomográficas de tórax se realizó a través de la plataforma de IA; luego, se comparó con el diagnóstico molecular (RT-PCR) para determinar la concordancia entre ambos y analizar su factibilidad para el cribado de pacientes con sospecha de COVID-19. Las imágenes y los resultados diagnóstico se enviaron a través de una plataforma de telemedicina. Resultados. Se realizó el cribado de 3 514 pacientes con sospecha diagnóstica de COVID-19, en 14 hospitales a nivel nacional. La mayoría de los pacientes tenían entre 27 y 59 años, seguidos por los mayores de 60 años. La edad promedio fue de 48,6 años; el 52,8% eran de sexo masculino. Los hallazgos más frecuentes fueron neumonía grave, neumonía bilateral con derrame pleural, enfisema pulmonar bilateral y opacidad difusa en vidrio esmerilado, entre otros. Se determinó un promedio de 93% de concordancia y 7% de discordancia entre las imágenes analizadas mediante IA y la RT-PCR. La sensibilidad y especificidad del sistema de IA, obtenidas comparando el resultado del cribado obtenido por IA con la RT-PCR, fueron de 93% y 80% respectivamente. Conclusiones. Es viable la utilización de IA sensible y específica para la detección rápida estratificada de COVID-19 en pacientes con afecciones respiratorias utilizando imágenes obtenidas mediante tomografía de tórax y una plataforma de telemedicina en los hospitales públicos de Paraguay.
