RESUMO Comparamos o efeito do treinamento de marcha em esteira versus piscina no equilíbrio e na marcha em 12 sobreviventes crônicos de AVC isquêmico separados aleatoriamente nos grupos piscina ou esteira. A escala de Berg (EEB) e timed up and go test (TUG) foram aplicados antes e após as intervenções. A EMG de superfície dos músculos do lado parético e não parético foi registrada na caminhada em esteira. Três janelas de 100 ms foram extraídas da EMG relacionada às fases da marcha: aceitação de peso; propulsão; e pré-contato do pé. Para cada fase, calculou-se o RMS do sinal EMG. Os participantes treinaram 9 semanas (3 vezes/semana, 40 minutos/sessão). O grupo piscina fez marcha na água com cinto de natação. O grupo esteira fez treinamento de marcha na na velocidade máxima confortável. A Manova comparou o efeito do treinamento, grupo, lado, músculos e fase da marcha na EMG. A Anova testou o efeito do treinamento, lado do grupo e fase da marcha nas variáveis BBS, TUG e EMG. Os grupos piscina e esteira aumentaram o equilíbrio e a agilidade. O maior EMG RMS ocorreu no lado parético, no grupo esteira e após o treinamento. Os músculos: tibial anterior, gastrocnêmio lateral, vasto lateral e bíceps femoral apresentaram o maior RMS para o lado não parético; enquanto para os músculos reto femoral e semitendíneo, o lado parético apresentou o maior RMS. Assim, os dois tipos de exercício levaram a adaptações funcionais semelhantes com diferentes ativações musculares durante a caminhada.

ABSTRACT We compared the effect of gait training on treadmill versus deep water on balance and gait in 12 ischemic stroke chronic survivors randomly sorted to the Pool or Treadmill Groups. Berg Scale (BBS) and timed up and go test (TUG) were applied before and after the interventions. Just one person applied all tests and she was blinded for the aims of the study. Surface EMG of the paretic and non-paretic (NP) side muscles were recorded during walking on a treadmill. Three 100-ms epochs were extracted from the EMG related to gait phases: weight acceptance; propulsion; and pre-strike. For each epoch, we calculated the RMS of the EMG signal. Participants did gait training for 9 weeks (3 times/week, 40 minutes/session). The Pool group did the deep-water walking with a swimming belt. The Treadmill group walked on the treadmill at the maximum speed they could stand. The Manova group compared the effect of training, group, side, muscles, and gait phase into the EMG. Anova was used to test the effect of training, group side, and gait phase into BBS, TUG and EMG variables. Pool and Treadmill had increased balance and agility. The highest EMG RMS occurred at the paretic side, for the Treadmill and after training. The mm. tibialis anterior, gastrocnemius lateralis, vastus lateralis, and biceps femoris presented the highest RMS for the NP side; while for mm. rectus femoris and semitendinosus, the paretic side presented the highest RMS. Thus, the both types of exercise lead to similar functional adaptations with different muscular activations during walking.

RESUMEN Se comparó el efecto del entrenamiento de la marcha en cinta de correr y en piscina en el equilibrio y la marcha de 12 sobrevivientes crónicos de accidente cerebrovascular isquémico, quienes fueron clasificados aleatoriamente en los grupos piscina o cinta de correr. Se aplicaron la Escala de Equilibrio de Berg (BBS) y la Timed up and go test (TUG) antes y después de las intervenciones. Solo una persona aplicó todas las pruebas con evaluación ciega para los objetivos del estudio. Se registró la EMG de superficie de los músculos laterales paréticos y no paréticos durante la caminata en la cinta. Se extrajeron tres momentos de 100 ms de la EMG relacionadas con las etapas de la marcha: aceptación de peso; propulsión; y precontacto del pie. Para cada momento, se calculó el RMS de la señal EMG. Los participantes realizaron entrenamientos de marcha durante 9 semanas (3 veces/semana, 40 minutos/sesión). El grupo piscina caminó con un cinturón de natación. El grupo cinta de correr caminó en la cinta a la velocidad máxima confortable. Utilizando la Manova se comparó el efecto del entrenamiento, el grupo, los músculos laterales y la fase de la marcha en la EMG. Con la ANOVA se probó el efecto del entrenamiento, el lado grupal y la fase de marcha en las variables BBS, TUG y EMG. El equilibrio y la agilidad aumentaron en ambos grupos. El EMG RMS más alto ocurrió en el lado parético del grupo cinta de correr y después del entrenamiento. Los músculos tibial anterior, gastrocnemio lateral, vasto lateral y bíceps femoral presentaron el RMS más alto en el lado no parético; mientras que en los músculos recto femoral y semitendinoso, el lado parético presentó el RMS más alto. Por lo tanto, los dos tipos de ejercicio conducen a adaptaciones funcionales similares con diferentes activaciones musculares durante la marcha.

RESUMO Objetivo Avaliar a eficácia da reabilitação cognitiva em pacientes com a doença de Alzheimer em estágio leve a moderado por meio de uma revisão sistemática. Métodos O levantamento de artigos se deu pelas bases de dados MEDLINE (acessado via PubMed), EMBASE e LILACS, seguindo a Colaboração Cochrane e a recomendação PRISMA, por meio dos critérios de elegibilidade: ensaios clínicos randomizados e que apresentaram o uso da reabilitação cognitiva como intervenção não farmacológica em pacientes com a doença de Alzheimer em estágio leve a moderado, devendo haver pelo menos um grupo de intervenção e um grupo de comparação. Resultados Foram selecionados seis ensaios clínicos randomizados para a realização desta revisão. Em todos os estudos analisados, foi observada melhora no desempenho dos pacientes nos instrumentos de avaliação, principalmente no que diz respeito à realização de atividades da vida diária. Conclusão A reabilitação cognitiva é capaz de prolongar a independência do paciente na realização de atividades da vida diária e de gerar alterações funcionais e estruturais. Como consequência da constatação de poucos trabalhos que atingiram os critérios de elegibilidade, também se constatou a necessidade de se realizarem mais estudos controlados e com maior controle de aplicação, considerando o alto risco de viés nos estudos utilizados.

ABSTRACT Objective To evaluate the efficacy of Cognitive Rehabilitation in patients with mild to moderate Alzheimer’s disease through a systematic review. Methods The survey of articles was done by MEDLINE (accessed via PubMed), EMBASE and LILACS, following the Cochrane Collaboration and the PRISMA recommendation, using the following eligibility criteria: randomized clinical trials that presented the use of cognitive rehabilitation as non-pharmacological intervention in patients with mild to moderate Alzheimer’s disease, with at least one intervention group and one comparison group. Results Six randomized controlled trials were selected for this review. In all of the studies analyzed, improvement in the performance of patients in the assessment instruments was observed, mainly in relation to the performance of activities of daily living. Conclusion Cognitive rehabilitation is able to prolong the independence of the patient in performing activities of daily living and generate functional and structural changes. As a consequence of the finding of few studies that met the eligibility criteria, we also verified the need for more controlled studies and greater application control, considering the high risk of bias in the studies used.

RESUMO O objetivo desta pesquisa foi mensurar de forma objetiva a propriocepção, em diferentes situações (com e sem o auxílio da visão, com e sem tarefa cognitiva concomitante e de forma ativa ou passiva), em indivíduos idosos e adultos. Participaram do estudo dez adultos saudáveis e dez idosos saudáveis. Foram estudados os efeitos de diferentes restrições no desempenho do movimento por meio dos erros absoluto (precisão) e relativo (consistência). A mensuração do senso de posicionamento do tornozelo foi realizada com o dinamômetro isocinético Biodex Sistem 3. A análise de variância de quatro fatores e o teste post hoc de Tukey foram utilizados para analisar os parâmetros. Os resultados mostraram que o fator sujeito interfere no erro absoluto, pois os grupos apresentaram diferença significativa: os idosos erram mais quando comparados com os adultos. Os demais fatores (visão, tarefa cognitiva e tipo de movimento) não apresentaram diferença significativa. No erro relativo os resultados mostraram que a tarefa cognitiva concomitante ao movimento foi capaz de produzir diferença significante; entretanto os demais fatores (indivíduos, visão e tipo de movimentação) não foram capazes de produzir uma diferença significativa. A idade afeta a precisão do sentido proprioceptivo, independentemente da situação. Não houve diferença entre a realização do movimento com e sem o auxílio da visão, e a dupla tarefa (motora associada à cognitiva) afeta a consistência do movimento.

ABSTRACT The objective of this research was to measure objectively the proprioception in different situations (with and without the aid of vision, with or without a concomitant cognitive task, and actively and passively), in elder and adult individuals. Ten healthy adults and ten healthy seniors participated in the study. The effects of different restrictions were studied in the development of movement through absolute (accuracy) and relative (consistency) errors. The sense of ankle positioning was measured with a Biodex System 3isokinetic dynamometer. Four-factor analysis of variance and the Tukey post hoc test were used to analyze the parameters. The results showed that the subject factor interferes in the absolute error, since the groups showed significant difference: the elderly make more mistakes when compared with adults. The other factors (vision, cognitive task and type of movement) did not show significant difference. In the relative error, results showed that the cognitive task concomitant to movement was capable of producing significant difference; however, the other factors (individuals, vision and type of movement) were not. Age affects the accuracy of the proprioceptive sense, regardless of the situation. There was no difference between performing the movement with and without the aid of vision, and the double task (motor associated with cognitive) affects the consistency of the movement.

RESUMEN En este estudio se propone medir objetivamente la propiocepción en situaciones diferentes (con y sin la ayuda de la visión, con y sin la tarea cognitiva concomitante y de forma activa o pasiva), en adultos y adultos mayores. Del estudio han participado diez adultos sanos y diez adultos mayores sanos. Se analizaron los efectos de diversas restricciones en el funcionamiento del movimiento por medio de los errores absoluto (precisión) y relativo (consistencia). La medición del sentido de posicionamiento del tobillo se llevó a cabo por medio del dinamómetro isocinético Biodex System 3. Se utilizaron el análisis de varianza de cuatro factores y la prueba posthoc de Tukey, para analizar los parámetros. Los resultados demostraron que el factor sujeto interfiere en el error absoluto, ya que los grupos presentaron una diferencia significativa: los adultos mayores cometen más errores que los adultos. Los otros factores (visión, tarea cognitiva y tipo de movimiento) no demostraron diferencias significativas. En el error relativo, los resultados desvelan que la tarea cognitiva y el movimiento fueron capaces de producir una diferencia estadísticamente significativa; sin embargo, otros factores (personas, visión y tipo de movimiento) no fueron capaces de producirla. La edad afecta a la exactitud del sentido propioceptivo, independientemente de la situación. No hubo diferencias entre el rendimiento del movimiento con y sin la ayuda de la visión, y la doble tarea -motora relacionada con la cognitiva- impacta en la consistencia del movimiento.

Abstract AIMS (Stroke patients often present sensory-motor alterations and less aerobic capacity. Joint position sense, which is crucial for balance and gait control, is also affected in stroke patients). To compare the effect of two exercise training protocols (walking in deep water and on a treadmill) on the knee position sense of stroke patients. METHODS This study was designed as a randomized controlled clinical trial. Twelve adults, who suffered a stroke at least one year prior to the start of the study, were randomly assigned to one of two groups: 1) pool group submitted to aerobic deep water walking training; and 2) the treadmill group which was submitted to aerobic walk on a treadmill. Measurements: The position sense, absolute error and variable error, of the knee joint was evaluated prior to and after nine weeks of aerobic training. RESULTS The pool group presented smaller absolute (13.9o versus 6.1o; p < 0.05) and variable (9.2o versus 3.9o; p < 0.05) errors after nine-weeks gait training than the treadmill group. CONCLUSIONS Nine-week aerobic exercise intervention in aquatic environment improved precision in the position sense of the knee joint of stroke patients, suggesting a possible application in a rehabilitation program.

OBJECTIVES: Acute post-stroke patients present cardiovascular autonomic dysfunction, which manifests as lower heart rate variability and impaired baroreflex sensitivity. However, few studies performed to date have evaluated cardiovascular autonomic function in chronic post-stroke patients. The aim of this study was to evaluate cardiovascular autonomic modulation in chronic post-ischemic stroke patients. METHODS: The seventeen enrolled subjects were divided into a stroke group (SG, n=10, 5±1 years after stroke) and a control group (CG, n=7). Non-invasive curves for blood pressure were continuously recorded (Finometer®) for 15 minutes while the subject was in a supine position. Heart rate variability and blood pressure variability were analyzed in the time and frequency domains. RESULTS: No differences were observed in systolic and diastolic pressure and heart rate between post-stroke patients and healthy individuals. The SG group had lower indexes for heart rate variability in the time domain (standard deviation of normal to normal R-R intervals, SDNN; variance of normal to normal R-R intervals, VarNN; and root mean square differences of successive R-R intervals, RMSSD) and a lower high-frequency band for heart rate variability than was observed in the CG. Systolic blood pressure variability and the low-frequency band for systolic pressure were higher in post-stroke patients, while the alpha index was lower in the SG than in the CG. CONCLUSION: After ischemic stroke, affected patients present chronically reduced heart rate variability, impaired cardiac vagal modulation, increased systolic blood pressure variability and higher sympathetic vascular modulation along with impaired baroreflex sensitivity, which can increase the risk of cardiovascular events, despite adequate blood pressure control.

RESUMO O objetivo desta revisão é avaliar estudos sobre intervenções com treino de marcha e exercícios específicos sobre marcha e equilíbrio postural, aplicados a pacientes após acidente vascular encefálico em fase crônica. Os estudos incluídos nesta revisão foram ensaios clínicos randomizados, incluindo apenas indivíduos pós-acidente vascular encefálico em fase crônica que avaliaram marcha e equilíbrio postural, com uma pontuação em escala PEDro ≥ 7.0. Oito estudos foram selecionados. Os resultados sugerem que a marcha e o equilíbrio somente são afetados em pacientes crônicos pós-acidente vascular encefálico se as sessões de treinamento tiverem duração mínima de 30 minutos, forem repetidos três vezes por semana, e mantidos durante pelo menos cinco semanas. O treino de marcha e os exercícios afetam a forma como os indivíduos pós-acidente vascular encefálico em fase crônica andam. Eles provavelmente andarão mais rápido e com menor risco de quedas; no entanto, não está claro se as consequências destes procedimentos afetam a qualidade de vida.

The aim of this review is to evaluate studies about gait training and exercise interventions applied to patients following chronic stroke on gait and balance. The studies included in this review were random clinical trials, including only chronic post-stroke individuals that evaluated gait and balance outcomes and with a PEDro scale score ≥ 7.0. Eight studies were selected. The results suggest gait and balance will only be affected in chronic post-stroke patients if training sessions last at least 30 minutes, are repeated three times a week, and maintained for at least five weeks. Gait training affects how chronic post-stroke individuals walk. They will probably walk faster and with a lower risk of falling; however, it is unclear whether the consequences of these procedures affect the quality of life.

A quantidade de torque aplicado na articulação é uma medida de aptidão física importante para crianças com paralisia cerebral. O presente estudo analisou parâmetros cinéticos na articulação do cotovelo em crianças saudáveis e com paralisia cerebral. Participaram 10 crianças com paralisia cerebral e 10 crianças sem comprometimento neurológico. Avaliou-se a média do pico de torque, média do ângulo do pico de torque, coeficiente de variação do torque e aceleração angular do movimento de flexo-extensão do cotovelo nas velocidades com um dinamômetro isocinético. A média de pico de torque (extensão), aceleração (flexão) e coeficiente de variação (flexão e extensão) são diferentes entre grupos. Conclui-se que o torque e aceleração sofreram interferências no movimento de flexo-extensão; as principais diferenças encontradas foram entre os extremos das velocidades; não houve diferenças no ângulo do pico de torque. A espasticidade não interferiu na força dos músculos agonistas do movimento de flexão da articulação do cotovelo.

The joint torque is an important variable related to children with cerebral palsy. The present study analyzed kinetic parameters during elbow flexion and extension movements in healthy and cerebral palsy children. Ten healthy and 10 cerebral palsy children participated of the study. An isokinetic dynamometer was used to measure the elbow mean peak torque, mean angle peak torque, coefficient of variation and acceleration during flexion and extension movements at different angular speeds. The mean peak torque on extension movement in healthy children group was significant higher compared to the cerebral palsy group. The coefficient of variation on both flexion and extension movements was significantly higher in cerebral palsy group. However there were significantly difference on both groups compared the lowest and highest velocities. Although the results showed no difference in flexor peak torque, the acceleration is significantly lower in lowest and highest angular velocity.

Os pontos gatilhos miofasciais (PGMs) são manifestações comumente encontradas na prática clínica e estão relacionados à alteração de tônus e à síndrome dolorosa miofascial (SDM). OBJETIVO: Avaliar a capacidade da EMG de superfície e detectar alterações da atividade neuromuscular no PGM, em situações de repouso e de contração isométrica voluntária máxima. MÉTODO: Participaram do estudo 56 indivíduos distribuídos em dois grupos: grupo Saudável com 28 indivíduos, que necessariamente não possuíam PGM, e o grupo Dor, constituído de 28 indivíduos que possuíam necessariamente PGM no músculo escolhido. O grupo Dor apresentava indivíduos com PGM latente e ativo, e com e sem fenômenos autonômicos (FA). RESULTADOS: O sinal EMG da porção muscular com PGM mostrou-se significativamente maior quando comparado com a porção muscular sadia do grupo Dor, e do grupo Saudável durante o repouso (26,56 ± 44,54, 5,39 ± 6,29 e 1,56 ± 0,76, respectivamente, p = 0,0001). Os indivíduos com PGM ativo obtiveram maior intensidade do sinal EMG do que aqueles que apresentaram PGM latente (17,85 ± 30,25 versus 3,74 ± 1,52, p = 0,04). Além disso, os indivíduos que apresentaram fenômenos autonômicos tiveram maior intensidade do sinal EMG do que aqueles que não os apresentaram (16,78 ± 28,44 versus 3,51 ± 3,65), na condição de repouso. CONLUSÃO: A EMG de superfície é capaz de mensurar a atividade do PGM, principalmente na condição de repouso.

There are examples of common clinical conditions that clinical signals are related to alterations in muscle tone, including myofascial pain syndrome. OBJECTIVE: to discuss the application of surface EMG to detect effect of miofascial trigger point (MTP) on neuromuscular activity at rest and maximum voluntary contraction of the trapezoid muscle. METHODS: Fifty-six subjects participated in the study and were divided into two groups: Healthy group (n = 28), with subjects who necessarily did not present MTP, and PAIN group (n = 28), with subjects who necessarily had MTP. RESULTS: The EMG signal was significantly higher in the muscle portion containing MTP than in the healthy muscle portion in PAIN group and than Healthy group (26,56 ± 44,54, 5,39 ± 6,29 and 1,56 ± 0,76, respectively, p = 0,0001) during rest. In addition, a higher intensity of the EMG signal was observed in subjects with active MTP than in latent MTP (17,85 ± 30,25 versus 3,74 ± 1,52, p = 0,04. Besides, the subjects who presented autonomic phenomena had higher EMG signal than those who had no autonomic phenomena (16,78 ± 28,44 versus 3,51 ± 3,65. CONCLUSION: The surface EMG is reliable to measure the muscular activity of the MTP, mainly at rest condition.

O desenvolvimento da Fisioterapia, aliado às mudanças na educação e na saúde, faz com que a autonomia e os dilemas éticos do fisioterapeuta sejam maiores a cada dia, expandindo seu papel no cuidado do paciente. Para além do Código de Ética, as noções de Bioética são centrais no preparo de graduandos para os desafios profissionais. Este estudo verificou a capacidade de tomar decisões éticas de alunos no último ano de Fisioterapia de duas universidades na cidade de São Paulo, das quais uma oferece no currículo a disciplina Bioética e a outra não. Cinqüenta alunos de cada universidade responderam a um questionário sobre dilemas éticos correspondentes a artigos do Código de Ética; cada questão oferecia três alternativas de resposta: uma bioética, outra referente ao Código e uma não-ética. As respostas foram analisadas estatisticamente. Os resultados mostraram um bom preparo ético em pouco mais de metade dos alunos das duas universidades, sugerindo que ambas oferecem preparo ético similar aos graduandos, independente da diferença curricular. No entanto, os alunos da universidade que oferece Bioética tiveram resultados significantemente melhores nos quesitos referentes à relação com outros profissionais da saúde, sugerindo que essa disciplina oferece melhores condições para o relacionamento interprofissional, fator importante tanto na prática clínica quanto para elevar o status da Fisioterapia.

Physical therapy recent expansion, along with changes in education and health, enhance physical therapists' role in patient care, while increasing both their autonomy and the ethical challenges they face in daily clinic. In addition to knowledge of the professional ethical code, notions of Bioethics are thought to be central to students training. This study analysed the ability to make ethical decisions among 100 physical therapy undergraduate students at two universities in São Paulo, of which only one offers the discipline Bioethics. Fifty students of each university answered a questionnaire of dilemmas related to items of the Physical Therapy Code of Ethics; for each question there was a choice between three answers, bioethical, in accordance with the ethical code, or non-ethical. Answers were statistically analysed. About half the students of both universities seemed to be satisfactorily prepared to act ethically, thus suggesting that both offer similar ethical training, regardless of curricula differences. However, students from the university that offers Bioethics had significantly better results in questions concerning the relationship with other health providers, thereby suggesting that this discipline may provide better student training for inter-professional relationship, which is important both to clinical practice and to raise physical therapy status.
