Abstract This article aims to analyze, from the perspective of state administrators, the implementation of the health regionalization process in Rio Grande do Sul, beginning in the 2000s. This is a qualitative-descriptive study, conducted in the State Health Department. We obtained information from ten semi-structured interviews with state administrators from the central level. We interpreted the data using content analysis, thematic method. As results, we highlight the improvement in healthcare quality, the progress in the organization of the state’s health system, the change in the role of the state administration, the development of the technology and the acknowledgment of pact-making spaces for decision-making. We note that the system’s fragmentation and the historical planning insufficiency hamper regional governance. In conclusion, implementing a culture of strategic management is necessary, with the backing of adequate resource allocation and enhancement of governance in the health regions.

Resumo Este artigo teve como objetivo analisar, através da perspectiva dos gestores estaduais, como ocorreu a implementação do processo da regionalização da saúde no Rio Grande do Sul, a partir dos anos 2000. Trata-se de um estudo qualitativo descritivo, desenvolvido na Secretaria da Saúde do Estado do Rio Grande do Sul. As informações foram obtidas através de dez entrevistas semiestruturadas. Para a interpretação dos dados levantados, utilizou-se análise de conteúdo, modalidade temática. Como resultados, destacaram-se a melhoria na qualidade da atenção à saúde, o progresso na organização do sistema de saúde do estado, a mudança na função da gestão estadual, o desenvolvimento da tecnologia e o reconhecimento das instâncias de pactuação para a tomada de decisão. Identificou-se que a governança regional foi dificultada pela fragmentação do sistema e a histórica insuficiência de planejamento. Concluiu-se que se faz necessário implementar uma cultura de gestão estratégica, amparada pela alocação adequada de recursos e qualificação da governança nas regiões de saúde.

Abstract Background Previous studies have established normal and reference values for Pulse Wave Velocity (PWV). However, the PWV value that has the strongest association with cardiovascular biomarkers remains poorly understood. Objective This study aimed to determine the PWV value more likely to be associated with left ventricular hypertrophy (LVH), increased intima-media thickness (IMT), and presence of carotid plaques in patients with hypertension. Methods This cross-sectional study included 119 patients. Analysis of receiver operating characteristic (ROC) curves was performed for each cardiovascular biomarker. Statistical significance was set at p < 0.05. Results According to the ROC curve analysis, the PWV values were 8.1 m/s, 8.2 m/s, and 8.7 for the LVH, IMT, and presence of carotid plaques, respectively. A PWV value of 8.2 m/s was identified as the best parameter to determine the three TOD biomarkers. PWV above 8.2 m/s was associated with increased CIMT (p = 0.004) and the presence of carotid plaques (p = 0.003) and LVH (p<0.001). PWV above 8.2 showed greater sensitivity for increased CIMT (AUC = 0.678, sensitivity = 62.2), LVH (AUC = 0.717, sensitivity = 87.2), and the presence of plaques (AUC = 0.649, sensitivity = 74.51) in the ROC curve analysis. Conclusion The PWV value 8.2 m/s was more sensitive in early identifying the existence of cardiovascular biomarkers of TOD.

Resumo Fundamento Estudos prévios estabeleceram valores de normalidade e de referência da Velocidade de Onda de Pulso (VOP). Porém, qual valor de VOP que apresenta a associação mais forte com biomarcadores cardiovasculares ainda é pouco conhecido. Objetivo Identificar o valor de VOP com maior possibilidade de estar associado com hipertrofia ventricular esquerda (HVE), aumento da espessura íntima-média carotídea (EIMC), e presença de placas carotídeas em pacientes hipertensos. Métodos Este é um estudo transversal de 119 pacientes. Análise de curvas características de operação do receptor (ROC) foi realizada para cada biomarcador cardiovascular. A diferença estatística foi estabelecida em p<0,05. Resultados Segundo análises das curvas ROC, valores de VOP de 8,1m/s para HVE, 8,2m/s para EMIC aumentada e 8,7m/s para a presença de placa carotídea foram encontrados, respectivamente. O valor de VOP de 8,2m/s foi definido como melhor o parâmetro para encontrar os três biomarcadores de LOA. A VOP acima de 8,2m/s associou-se ao aumento da EMIC (p = 0,004), à presença de placas carotídeas (p = 0,003) e à HVE (p < 0,001). A VOP acima de 8,2m/s apresentou maior sensibilidade para EMIC aumentada (AUC = 0,678, sensibilidade 62,2), HVE (AUC = 0,717, sensibilidade 87,2), e presença de placas (AUC = 0,649, sensibilidade 74,51) na análise das curvas ROC. Conclusão O valor de 8,2m/s de VOP foi mais sensível em identificar, precocemente, a existência de biomarcadores cardiovasculares de LOA.

BACKGROUND The high demand for adequate material for the gold standard reverse transcription real-time polymerase chain reaction (RT-qPCR)-based diagnosis imposed by the Coronavirus disease 2019 (COVID-19) pandemic, combined with the inherent contamination risks for healthcare workers during nasopharyngeal swab (NP) sample collection and the discomfort it causes patients, brought the need to identify alternative specimens suitable for the diagnosis of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). OBJECTIVES The aim of this work was to compare saliva and gingival fluid swabs to NP swabs as specimens for RT-qPCR-based SARS-CoV-2 diagnosis. METHODS We compared gingival fluid swabs (n = 158) and saliva (n = 207) to the rayon-tipped NP swabs obtained from mild-symptomatic and asymptomatic subjects as specimens for RT-qPCR for SARS-CoV-2 detection. FINDINGS When compared to NP swabs, gingival fluid swabs had a concordance rate of 15.4% among positive samples, zero among inconclusive, and 100% among negative ones. For saliva samples, the concordance rate was 67.6% among positive samples, 42.9% among inconclusive, and 96.8% among negative ones. However, the concordance rate between saliva and NP swabs was higher (96.9%) within samples with lower cycle threshold (Ct) values (Ct > 10 ≤ 25). MAIN CONCLUSIONS Our data suggests that whereas gingival fluid swabs are not substitutes for NP swabs, saliva might be considered whenever NP swabs are not available or recommended.

ABSTRACT Multidimensional scaling is a multivariate analysis technique that can be used to exploit genetic diversity, aiming at the selection of Capsicum genotypes with desirable characteristics for in-pot ornamental purposes. This work aimed to select genotypes with ornamental potential within F4 populations of ornamental pepper plants. Three F4 families were used (17.18, 30.16, and 56.8). The genotype distance matrices were estimated based on qualitative and quantitative descriptors, separately, combining the standardized distances of Gower and Mahalanobis, respectively. The relation of the distance between genotypes was graphically studied through non-metric multidimensional scaling. Kruskal' Stress was used as the measured misadjustment of the nMDS solution. There is genetic diversity within the analyzed families, allowing to practice selection. The selection in family 17.18 of genotypes 6 and 32 is recommended, as well as in family 30.16 of genotypes 22 and 4, and family 56.8 of genotypes 15 and 36, since they present important characteristics for ornamental purposes. The selection of genotypes is more efficient when using mixed data since it provides a more complete genetic diversity in an improvement program.

Abstract Plant breeding of ornamental peppers (Capsicum spp.) can be supported by biotechnological tools. The objective of this study was to evaluate the efficiency of an in vitro culture of immature zygotic embryos (IZE) to reduce the breeding cycle of ornamental pepper (C. annuum) in comparison to the conventional system. Three ornamental pepper genotypes were used: UFPB 001, UFPB 004, and UFPB 099. Embryos at 30 days after selfing were inoculated in MS ½-strength culture medium, and at the same time, seeds were placed to germinate in a commercial substrate. Approximately 215 days are required from selfing until fruit ripening in the conventional system, whereas the IZE system requires an average of 153 days, a decrease of approximately 30% per selection cycle, corresponding to 496 days considering 8 selfing cycles. A decrease in time, labor, and inputs makes the IZE system a suitable tool for shortening the breeding program of ornamentals peppers.

RESUMO Objetivo: analisar as habilidades auditivas, de linguagem, motoras e sociais de crianças e propor uma triagem do desenvolvimento infantil. Métodos: participaram 129 pré-escolares, de ambos os sexos, com idades entre três e seis anos, matriculadas em instituições de educação infantil e 25 educadores do ensino infantil oriundos de instituições públicas e particulares. Todas as crianças com curvas timpanométricas tipo A e presença de reflexos acústicos. Nas crianças, aplicou-se o teste de desenvolvimento neuropsicomotor, Denver II, e a avaliação da localização sonora e da ordenação temporal de três sons verbais e não verbais. Com as educadoras, foi aplicado a Escala SAB. Resultados: verificou-se que a grande maioria dos participantes com SAB normal apresentaram habilidades auditivas ou Denver II normal; enquanto no grupo de SAB alterado, grande parte dos participantes apresentou alteração no Denver II ou nos testes de habilidades auditivas. Foi constatado, ainda, que parte das crianças com Denver II normal foram apontados, pelos educadores, como com comportamento inadequado no SAB. Conclusão: a combinação entre os achados do Denver II, de testes auditivos de localização sonora e ordenação temporal e a Escala SAB são úteis na caracterização do desenvolvimento infantil, assim, recomenda-se o uso dos três instrumentos como triagem.

ABSTRACT Purpose: to analyze the hearing, language, motor and social skills of children and propose a screening of child development. Methods: 129 preschool children of both sexes, aged between three and six years old, enrolled in educational institutions and 25 teachers of kindergarten from public and private institutions, with no history of hearing disorders, with type A tympanometric curves and the presence of acoustic reflexes participated. For the children, the neuropsychomotor test, Denver II, and the evaluation of sound localization and temporal ordination of three verbal and non-verbal sounds were applied. For the educators responsible for the children, the Scale of Auditory Behaviors (SAB), was used. Results: most participants with normal SAB presented hearing abilities or standard Denver II; while in the amended SAB group, most participants presented alterations in Denver II or in the auditory abilities tests. It was found, also, that part of the children with standard Denver II were pointed, by the educators, as misbehaving in SAB. Conclusion: the combination of the findings of the Denver II, hearing tests of sound localization and temporal ordination and the SAB Scale is useful in the characterization of child development and, thus, the use of these three instruments for screening in this age group is recommended.
