Abstract Objective To evaluate the growth trajectory of head circumference and neurodevelopment, and to correlate head circumference with cognitive, language, and motor outcomes during the first two years. Method Prospective cohort study in a tertiary hospital including 95 newborns under 32 weeks or 1500 g. Neonates who developed major neonatal morbidities were excluded. The head circumference was measured at birth, at discharge, and at term-equivalent age, 1, 3, 5, 12, 18, and 24 months of corrected age, and the Bayley Scales (Bayley-III) were applied at 12, 18 and 24 months of corrected age to assess cognitive, language and, motor domains. Scores below 85 were classified as mild/moderate deficits and scores below 70 as severe deficits. The association between head circumference Z score and Bayley scores was assessed using Pearson's correlation. The study considered a significance level of 0.05. Results There was a decrease of -0.18 in the head circumference Z score between birth and discharge and the catch-up occurred between discharge and 1 month (an increase of 0.81 in the Z score). There was a positive correlation between head circumference and Bayley scores at 18 months. There was also a positive correlation between head circumference at discharge and at 5 months with the three domains of the Bayley. Conclusion Serial measurements of head circumference provide knowledge of the trajectory of growth, with early catch-up between discharge and 1 month, as well as its association with neurodevelopment. Abstract Background Analyses of extensive registries indicate adverse outcomes for women undergoing coronary artery bypass grafting (CABG) surgery, while randomized studies often lack representativeness. Objective To compare adjusted hospital outcomes between men and women undergoing CABG. Methods From July 2017 to June 2019, 3991 patients underwent primary isolated CABG, both electively and urgently, in 5 hospitals in the state of São Paulo, Brazil. To mitigate demographic differences between men and women, populations were adjusted using propensity score matching (PSM). The outcomes considered for analysis were those used by the STS Adult Database. The analyses were performed using R software, with a significance set at p<0.05. Results After PSM (1:1), each group included 1089 patients. Regarding intraoperative variables, men exhibited longer cardiopulmonary bypass (CPB) time (p<0.001), surgical time (p<0.001), a higher number of distal anastomoses (p<0.001), and increased use of arterial grafts. Regarding outcomes, women had a higher incidence of deep sternal wound infection (p=0.006), prolonged Intensive Care Unit stay (p=0.002), increased need for an intra-aortic balloon pump (p=0.04), higher blood transfusion rates (p<0.001), higher 30-day hospital readmission rates after surgery (p=0.002) and higher mortality rate (p=0.03). Conclusions Although men had longer CPB times, a greater number of arterial grafts, and a greater number of distal anastomoses, immediate results after CABG were poorer in women. Resumo Fundamento Análises em grandes registros apontam desfechos desfavoráveis para mulheres submetidas à cirurgia de revascularização do miocárdio (CRM), enquanto estudos randomizados sofrem com a falta de representatividade. Objetivo Comparar os resultados hospitalares ajustados entre homens e mulheres submetidos à CRM. Métodos Entre julho de 2017 e junho de 2019, 3991 pacientes foram submetidos à CRM primária isolada, tanto de forma eletiva como de urgência, em 5 hospitais de estado de São Paulo, Brasil. Para equilibrar as diferenças entre homens e mulheres, as populações foram ajustadas utilizando o Propensity Score Matching. Os desfechos considerados para análise foram os utilizados pelo STS Adult Database. As análises foram conduzidas no software R, considerando significância valores de p < 0,05. Resultados Após o Propensity Score Matching (1:1), cada grupo incluiu 1089 pacientes. Em relação às variáveis intraoperatórias os homens apresentaram maior tempo de CEC (p<0,001), tempo cirúrgico (p<0,001), número de anastomoses distais (p<0,001) e uso de enxertos arteriais. Em relação aos desfechos as mulheres apresentaram maior incidência de infecção de ferida profunda (p=0,006), tempo prolongado na Unidade de Terapia Intensiva (p=0,002), maior necessidade do uso de balão intraórtico (p=0,04), maior taxa de transfusão sanguínea (p<0,001), maior readmissão hospitalar em até 30 dias após a cirurgia (p=0,002) e maior taxa de óbitos (p=0,03). Conclusões Apesar dos homens terem apresentado um maior tempo de CEC, maior número de enxertos arteriais e maior número de anastomoses distais, os resultados imediatos após CRM foram piores em mulheres. Abstract Human development is influenced by the interaction between biological and social factors. This study aimed to verify the moderating effect of social risk on the relationship between biological risk and child development. Data were collected on 201 children, aged 6 to 72 months. The independent variable was measured by the biological risk index, and the moderator variable by the social risk index was assessed by the Denver II test. Linear regression, effect size, and analysis of moderation were used to verify the relationship between BRI and the child development (Denver II), and the moderating effect of the SRI. BRI was negatively associated with child development, the interaction between the BRI and SRI increased the explained variance in the Denver II result to 14%. The SRI was also a significant moderator of the Language and Gross Motor domains. This research evidence that social risk moderates the relationship between biological risk and child development, the more social risk factors, the stronger this relationship becomes. Este estudo teve como objetivo verificar o efeito moderador do risco social na relação entre risco biológico e desenvolvimento infantil. Finos de carvão vegetal são coprodutos da carbonização da madeira, indesejados por diminuir o rendimento gravimétrico em carvão vegetal. São produzidos devido ao processo de carbonização, durante o armazenamento e transporte do carvão vegetal. A pelletização de finos aumenta a densidade energética, produzindo um combustível homogêneo e com menor teor de umidade, possibilitando a queima em equipamentos com alta eficiência energética, além de propriedades desejáveis em produtos energéticos como alto teor de carbono fixo e baixo teor de umidade. Este estudo avaliou o efeito do teor de umidade e do tempo de secagem na produção de black pellets a partir de finos de carvão vegetal ligados com amido de arroz (Oryza sativa) nas características físico-químicas e mecânicas. Os black pellets foram produzidos utilizando duas proporções de água (10 ou 20%) e tempo de secagem de 4 ou 6 horas. Os black pellets foram avaliados quanto à análise química imediata, composição elementar, teor de umidade, tamanho, densidade aparente, poder calorífico e densidade energética. A adição de água não afetou o teor de voláteis, cinzas e carbono fixo, enquanto o tratamento com adição de 20% de água e quatro horas de secagem apresentou maior teor de umidade (3,34%). A adição de água permitiu melhor condução de calor e disposição das partículas, produzindo pellets com durabilidade superior a 98%. A melhor disposição promovida pela adição de 20% de água também aumentou a densidade dos pellets, resultando em maior densidade energética. Os black pellets podem ser considerados uma boa alternativa para aquecimento residencial, com tratamento T3 (20% de umidade - 4h de secagem) apresentando elevada densidade energética e melhor desempenho em termos de propriedades desejáveis para utilização e comercialização de acordo com a norma EN 14961-6.
Charcoal fines are co-products of the wood carbonization process, unwanted because they reduce the gravimetric yield of the main product, charcoal. The better arrangement promoted by the addition of 20% water also increased the density of the pellets, resulting in greater energy density. Black pellets can be considered an alternative for residential heating, with T3 treatment (20% - 4h) with a high energy density and better performance in terms of desirable properties for use and commercialization according to the standard EN 14961-6.

Overview of global healthcare policies for patients with chronic kidney disease: an integrative literature review disease

ABSTRACT Introduction Chronic kidney disease is a progressive and irreversible loss of kidney function and considerably affects the lives of patients and their families. Its high incidence necessitates efficient public policies for prevention and treatment. However, policies for chronic kidney disease education and awareness are scarce. Objective To evaluate global public policies for the prevention and treatment of chronic kidney disease adopted in various regions, aiming to comprehend the differences between various models. Methods This integrative review followed PRISMA recommendations and included papers published between 2016 and 2021 across several databases. Results The 44 selected articles were categorized into three themes: structural and financial aspects of the organization of renal healthcare, access to renal healthcare or management of chronic kidney disease, and coping strategies for chronic kidney disease or kidney health. Critical analysis of the papers revealed global neglect of kidney disease in political agendas. Considerable policy variations exist between different countries and regions of the same country. Our research highlighted that free and universal health coverage, especially for the most vulnerable patients, is crucial for accessing treatment owing to the prohibitively high treatment costs. Abstract This study develops briquettes from fines generated in the processing of manganese ore from a strategic manganeferous district of Minas Gerais-Brazil. The briquettes were produced based on the following variables: compaction pressure (20 and 25MPa), curing time (7 and 28 days), percentage by mass of binder (5% and 10%), and replacement with tailings (0% and 10%). Technological tests evaluated the compressive, abrasion, impact strength and water absorption. The best conditions for briquette included 10% Portland cement, 10% Mn tailings, compaction pressure of 25MPa and 7 days of curing, as well as 10% Portland cement, compaction pressure of 25MPa and 28 days of curing. The briquettes primarily contained the spessartine, todorokite, quartz, and pyrolusite minerals. The storage time tests indicated that the briquettes responded satisfactorily to storage for three months in a dry environment and at room temperature. The amino-quinazoline scaffold stands out as a privileged structure in Medicinal Chemistry, due to its pleiotropic pharmacological profile. It has been explored in the design and development of novel drug candidates for several therapeutic applications, including antihypertensive, anti-inflammatory, antipsychotic, anti-Alzheimer, anticancer, antiviral, antibiotic, and antiparasitic treatments, among others. The therapeutic value of amino-quinazoline drugs was first demonstrated with the use of alpha 1-adrenoceptor antagonists, such as prazosin, doxazosin and terazosin. These drugs were initially approved for the treatment of hypertension and later for the treatment of benign prostatic hyperplasia. Several amino-quinazoline kinase inhibitors were introduced into the clinic as innovative therapeutic alternatives for cancer treatment after the U.S. Food and Drug Administration approved gefitinib in 2003 as a novel epidermal growth factor receptor inhibitor drug for non-small cell lung cancer. More recently, in 2021, the kinase inhibitor belumosudil was launched as a third-line therapy for adult and pediatric patients with chronic graft-versus-host disease. Este estudo avaliou o efeito do teor de umidade e do tempo de secagem na produção de black pellets a partir de finos de carvão vegetal ligados com amido de arroz (Oryza sativa) nas características físico-químicas e mecânicas. Os black pellets foram produzidos utilizando duas proporções de água (10 ou 20%) e tempo de secagem de 4 ou 6 horas. Os black pellets foram avaliados quanto à análise química imediata, composição elementar, teor de umidade, tamanho, densidade aparente, poder calorífico e densidade energética. A adição de água não afetou o teor de voláteis, cinzas e carbono fixo, enquanto o tratamento com adição de 20% de água e quatro horas de secagem apresentou maior teor de umidade (3,34%). A adição de água permitiu melhor condução de calor e disposição das partículas, produzindo pellets com durabilidade superior a 98%. A melhor disposição promovida pela adição de 20% de água também aumentou a densidade dos pellets, resultando em maior densidade energética. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Background Autoimmune encephalitis (AIE) is a group of inflammatory diseases characterized by the presence of antibodies against neuronal and glial antigens, leading to subacute psychiatric symptoms, memory complaints, and movement disorders. The patients are predominantly young, and delays in treatment are associated with worse prognosis. Objective With the support of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, ABN) and the Brazilian Society of Child Neurology (Sociedade Brasileira de Neurologia Infantil, SBNI), a consensus on the diagnosis and treatment of AIE in Brazil was developed using the Delphi method. Methods A total of 25 panelists, including adult and child neurologists, participated in the study. Results The panelists agreed that patients fulfilling criteria for possible AIE should be screened for antineuronal antibodies in the serum and cerebrospinal fluid (CSF) using the tissue-based assay (TBA) and cell-based assay (CBA) techniques. Children should also be screened for anti-myelin oligodendrocyte glucoprotein antibodies (anti-MOG). Treatment should be started within the first 4 weeks of symptoms. The first-line option is methylprednisolone plus intravenous immunoglobulin (IVIG) or plasmapheresis, the second-line includes rituximab and/or cyclophosphamide, while third-line treatment options are bortezomib and tocilizumab. Most seizures in AIE are symptomatic, and antiseizure medications may be weaned after the acute stage. In anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis, the panelists have agreed that oral immunosuppressant agents should not be used. Patients should be evaluated at the acute and postacute stages using functional and cognitive scales, such as the Mini-Mental State Examination (MMSE), the Montreal Cognitive Assessment (MoCA), the Modified Rankin Scale (mRS), and the Clinical Assessment Scale in Autoimmune Encephalitis (CASE). Conclusion The present study provides tangible evidence for the effective management of AIE patients within the Brazilian healthcare system. Resumo Antecedentes Encefalites autoimunes (EAIs) são um grupo de doenças inflamatórias caracterizadas pela presença de anticorpos contra antígenos neuronais e gliais, que ocasionam sintomas psiquiátricos subagudos, queixas de memória e distúrbios anormais do movimento. A maioria dos pacientes é jovem, e o atraso no tratamento está associado a pior prognóstico. Objetivo Com o apoio da Academia Brasileira de Neurologia (ABN) e da Sociedade Brasileira de Neurologia Infantil (SBNI), desenvolvemos um consenso sobre o diagnóstico e o tratamento da EAIs no Brasil utilizando a metodologia Delphi. Métodos Um total de 25 especialistas, incluindo neurologistas e neurologistas infantis, foram convidados a participar. Resultados Os especialistas concordaram que os pacientes com critérios de possíveis EAIs devem ser submetidos ao rastreio de anticorpos antineuronais no soro e no líquido cefalorraquidiano (LCR) por meio das técnicas de ensaio baseado em tecidos (tissue-based assay, TBA, em inglês) e ensaio baseado em células (cell-based assay, CBA, em inglês). As crianças também devem ser submetidas ao rastreio de de anticorpo contra a glicoproteína da mielina de oligodendrócitos (anti-myelin oligodendrocyte glycoprotein, anti-MOG, em inglês). O tratamento deve ser iniciado dentro das primeiras 4 semanas dos sintomas, sendo as opções de primeira linha metilprednisolona combinada com imunoglobulina intravenosa (IGIV) ou plasmaférese. O tratamento de segunda linha inclui rituximabe e ciclofosfamida. Bortezomib e tocilizumab são opções de tratamento de terceira linha. A maioria das crises epilépticas nas EAIs são sintomáticas, e os fármacos anticrise podem ser desmamadas após a fase aguda. Em relação à encefalite antirreceptor de N-metil-D-aspartato (anti-N-methyl-D-aspartate receptor, anti-NMDAR, em inglês), os especialistas concordaram que agentes imunossupressores orais não devem ser usados. Abstract This bibliometric study aimed to identify and analyze the 100 most cited articles about orofacial trauma in children and adolescents. The search was conducted in the Web of Science Core Collection (WoS-CC) using a combined search strategy. Two researchers collected the following data from each article: year of publication, country, journal, number and density of citations, author, institutions, study design, type of trauma, and keywords. The VOSviewer and SPSS version 22.0 softwares were used for data analyses. The articles were published from 1968 to 2012 and the number of citations ranged from 49 to 176. Europe was the continent with most articles (40 articles; 3,408 citations). Brazil was the country that made the largest contribution (20 articles; 1,741 citations) and the Universidade do Sul de Santa Catarina (Brazil) was the institution with most articles (5 articles; 492 citations). Marcenes W was the most productive author (8 articles; 968 citations). The cross-sectional study design was the most common (50 articles; 3,978 citations). The most frequent field was epidemiology (73 articles; 5,971 citations). The most widely used criteria for trauma diagnosis were the Andreasen (18 articles; 1,505 citations) and Le Fort (3 articles; 260 citations). Strong positive correlations were found in the number of citations between WoS-CC and Google Scholar (r = 0.929; p < 0.001), WoS-CC and Scopus (r = 0.976; p < 0.001), and Google Scholar and Scopus (r = 0.903; p < 0.001). Abstract Golden-Dewdrop (Duranta erecta L.), known for its landscaping applications, undergoes successive pruning to maintain its ornamental structure, a practice known to induce numerous physiological and chemical responses in the plant. Therefore, this study aimed to evaluate the influence of pruning and different extraction methods on the allelopathic potential of Golden-Dewdrop on the germination and initial growth of lettuce. Leaf extracts were obtained by decoction (hot) and maceration (cold) at concentrations of 20%, 40%, 60%, 80%, and 100%, with distilled water as a negative control. Phytochemical screening of the extracts was performed for phenolic compounds, flavonoids, anthocyanidins, tannins, cardioactive heterosides, saponins, alkaloids, and terpenes. For the bioassay, germinative and morphological parameters were measured and characterized. The data were subjected to analysis of variance (ANOVA), and for quantitative data, regression models were adjusted, while for qualitative data, means were compared using the Scott-Knott test with 5% significance. Significant differences were observed between extraction methods and management practices, with the hot aqueous extract from pruned plants significantly reducing germination at higher concentrations. The initial growth of lettuce was severely affected, presenting numerous morphological abnormalities, with severe damage to the root system at all concentrations. This study suggests that pruning practices have a modulating effect on the allelopathic activity of Golden-Dewdrop, providing valuable information for landscaping projects and botanical studies considering its evaluation and production of phytotoxic phytochemicals. Resumo Pingo-de-ouro (Duranta erecta L.), conhecida por suas aplicações paisagísticas, é frequentemente submetida a sucessivas podas para manter sua estrutura ornamental, sabendo-se que essa prática gera inúmeras respostas fisiológicas e químicas na planta. Portanto, este estudo teve como objetivo avaliar a influência da poda e de diferentes métodos de extração sobre o potencial alelopático da Pingo-de-ouro na germinação e crescimento inicial da alface. Os extratos das folhas foram obtidos por decocção (quente) e maceração (frio) em concentrações de 20%, 40%, 60%, 80% e 100%, tendo água destilada como controle negativo. Foi realizada a triagem fitoquímica dos extratos para identificar compostos fenólicos, flavonoides, antocianidinas, taninos, heterosídeos cardiotônicos, saponinas, alcaloides e terpenos. Para o bioensaio, foram mensurados e caracterizados parâmetros germinativos e morfológicos. Os dados foram submetidos à análise de variância (ANOVA) e, para os dados quantitativos, foram ajustados modelos de regressão, enquanto para os dados qualitativos, as médias foram comparadas pelo teste de Scott-Knott com 5% de significância. Foram observadas diferenças significativas entre os métodos de extração e as práticas de manejo, sendo que o extrato aquoso quente das plantas podadas reduziu significativamente a germinação em concentrações mais elevadas. O crescimento inicial da alface foi gravemente prejudicado, apresentando inúmeras anormalidades morfológicas, com danos severos ao sistema radicular em todas as concentrações. ABSTRACT In 2000, Cleide Costa published a paper presenting the state of knowledge of the Neotropical Coleopte ra, with a focus on the Brazilian fauna. Twenty-four years later, thanks to the development of the Coleoptera section of the Taxonomic Catalog of the Brazilian Fauna (CTFB - Catálogo Taxonômico da Fauna do Brasil) through the collaboration of 100 coleopterists from all over the globe, we can build on Costa’s work and present an updated overview of the state of knowledge of the beetles from Brazil. There are currently 35,699 species in 4,958 genera and 116 families known to occur in the country, including representatives of all extant suborders and superfamilies. Our data show that the Brazilian beetle fauna is the richest on the planet, concentrating 9% of the world species diversity, with some estimates accounting to up to 15% of the global total. The most diverse family in numbers of genera is Cerambycidae (1,056 genera), while in number of species it is Chrysomelidae (6,079 species). Conotrachelus Dejean, 1835 (Curculionidae) is the most species-rich genus, with 570 species. The French entomologist Maurice Pic is the author who has contributed the most to the naming of species recorded from Brazil, with 1,794 valid names in 36 families, whereas the Brazilians Ubirajara R. Martins and Maria Helena M. Galileo are the only ones among the top-ten authors to have named species in the 21st century. Abstract Background: The analysis of indicators such as hospital readmission rates is crucial for improving the quality of services and management of hospital processes. Objectives: To identify the variables correlated with hospital readmission up to 30 days following coronary artery bypass grafting (CABG). Methods: Cross-sectional cohort study by REPLICCAR II database (N=3,392) from June 2017 to June 2019. Retrospectively, 150 patients were analyzed to identify factors associated with hospital readmission within 30 days post-CABG using univariate and multivariate logistic regression. Analysis was conducted using software R, with a significance level of 0.05 and 95% confidence intervals. Results: Out of 3,392 patients, 150 (4,42%0 were readmitted within 30 days post-discharge from CABG primarily due to infections (mediastinitis, surgical wounds, and sepsis) accounting for 52 cases (34.66%). Other causes included surgical complications (14/150, 9.33%) and pneumonia (13/150, 8.66%). The multivariate regression model identified an intercept (OR: 1.098, p<0.00001), sleep apnea (OR: 1.117, p=0.0165), cardiac arrhythmia (OR: 1.040, p=0.0712), and intra-aortic balloon pump use (OR: 1.068, p=0.0021) as predictors of the outcome, with an AUC of 0.70. Conclusion: 4.42% of patients were readmitted post-CABG, mainly due to infections. Factors such as sleep apnea (OR: 1.117, p=0.0165), cardiac arrhythmia (OR: 1.040, p=0.0712), and intra-aortic balloon pump use (OR: 1.068, p=0.0021) were predictors of readmission, with moderate risk discrimination (AUC: 0.70). Resumo Fundamento: A análise de indicadores como taxa de readmissão hospitalar é crucial para aprimorar a qualidade dos serviços e gestão em processos hospitalares. Objetivo: Identificar as variáveis correlacionadas a readmissão hospitalar até 30 dias após cirurgia de revascularização miocárdica (CRM). Métodos: Estudo de coorte transversal no banco de dados Registro Paulista de Cirurgia Cardiovascular II (REPLICCAR II)(N=3.392), de junho de 2017 a junho de 2019. Avaliaram-se retrospectivamente 150 pacientes para identificar os fatores correlacionados a readmissão hospitalar até 30 dias após-CRM via regressão logística univariada e multivariada. As análises foram realizadas no software R, com significância de 0,05 e intervalos de confiança de 95%. Resultados: Cento e cinquenta pacientes foram readmitidos até 30 dias após a alta hospitalar de CRM (150/3.392, 4,42%) principalmente por infecções (mediastinite, ferida operatória e sepse) totalizando 52 casos (52/150, 34,66%), outras causas foram: complicações cirúrgicas (14/150, 9,33%) e pneumonia (13/150, 8,66%). Os preditores de readmissão identificados foram: O modelo de regressão multivariada apontou intercepto (OR: 1,098, p<0,00001), apneia do sono (OR: 1,117, p=0,0165), arritmia cardíaca (OR: 1,040, p=0,0712) e uso de balão intra-aórtico (OR: 1,068, p=0,0021) como preditores do desfecho, com uma AUC de 0,70. Conclusão: 4,42% dos pacientes foram readmitidos pós-CRM, principalmente por infecções. Fatores como apneia do sono (OR: 1,117, p=0,0165), arritmia cardíaca (OR: 1,040, p=0,0712) e uso de balão intra-aórtico (OR: 1,068, p=0,0021) foram preditores de readmissão, com uma discriminação de risco moderada (AUC: 0,70).
