Topical Application of Orabase Bromelain Gel Reduces Inflammatory Process, Bone Loss and Steatosis in Experimental Periodontitis

Abstract This study aimed to develop a bromelain-based gel and evaluate its effects in a ligature-induced periodontitis model in rats. The bromelain-based gel was formulated in two concentrations (1% and 10%). Ten animals were used per group: Control, Periodontitis, Periodontitis + Neutral Gel, Periodontitis + 1% Bromelain Gel, Periodontitis + 10% Bromelain Gel and Periodontitis + 2% Chlorhexidine Gel. The clinical parameters of tooth mobility (TM), gingival bleeding index (GBI) and probing pocket depth (PPD) were evaluated and, after euthanasia, gingival and liver tissue were collected by biopsy for biochemical evaluation of myeloperoxidase (MPO), malonaldehyde (MDA), glutathione (GSH) and from the jaws for morphometric analysis of alveolar bone height. The clinical parameters of MT, GBI and PPD of the groups treated with bromelain gel showed a significant reduction (MT = 67.04%, GBI = 67.72% and PPD = 41.22%) compared to the group with periodontitis. The results for MPO dosage in the gingival tissue showed statistically significant differences between the groups when compared to the group with periodontitis, MDA and GSH showed promising results in the groups treated with gels in the dosages of gingival and liver tissue. The results for ABL showed significant differences when compared to the groups that received gel treatment. Therefore, the bromelain orabase gel evaluated in periodontitis showed a positive response for the following parameters TM, GBI and PPD, MPO, MDA and ABL of the oral tissues, preventing alveolar bone resorption caused by the disease, highlighting the potential as an adjuvant treatment of periodontitis, in addition to reducing the systemic effects on the liver tissue.

Resumo Objetivou-se identificar fatores associados à atenção à saúde infantil na atenção primária à saúde (APS), na perspectiva das ações preconizadas pela Rede de Cuidados à Pessoa Deficiência (RCPCD), quanto à identificação precoce de deficiências e acompanhamento do desenvolvimento infantil. Trata-se de estudo transversal, multicêntrico, realizado em oito estados brasileiros. A coleta de dados envolveu a aplicação de questionário estruturado com profissionais de saúde de nível superior atuantes na APS, com amostra aleatória estratificada por estado e município. Foram realizadas regressões de Poisson com variância robusta para duas variáveis resposta. Entre os 1.488 trabalhadores que compuseram a amostra final, 63,6% realizam ações de identificação precoce de deficiências e 49% efetuam acompanhamento do desenvolvimento infantil. As equipes de Saúde da Família (eSF) identificam e acompanham mais do que as equipes do modelo tradicional, e os Núcleos Ampliados de Saúde da Família e Atenção Básica realizam mais ações de acompanhamento do que essas equipes. Conclui-se que os fatores mais associados com a identificação e o acompanhamento de crianças com deficiências na APS foram aqueles relativos à formação profissional, à eSF e ao conhecimento de normativas relativas à RCPCD.

Family health teams performed early identification of disabilities and follow-up more than traditional model teams, and expanded family health teams provided developmental follow-up more than both these teams. The factors that showed the strongest association with identification and developmental follow-up were profession, working in a family health team and knowledge of the RCPCD.

ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others.

Resultados: el uso de cigarrillo en la vida estuvo fuertemente asociado al uso de alcohol (Odds Ratio=3,25; Intervalo de Confianza 95% 2,28-4,62), seguido del uso de alcohol por amigos en los últimos 30 días (Odds Ratio=2,15; Intervalo de Confianza 95% 1,52-3,05) y tener padre y madre que fuman (Odds Ratio=1,29; Intervalo de Confianza 95%1,13-148). Conclusión: son importantes acciones integradas y políticas públicas que involucren a jóvenes y adolescentes para garantizar la salud y el bienestar de adultos saludables.

ABSTRACT Purpose: To analyze the average time between submission and acceptance of national journals in seven Brazilian surgery journals from 2017 to 2022. Methods: It consists of a cross-sectional and observational study with a quantitative approach to analyze the acceptance time of articles approved by Brazilian journals on general surgery and its subspecialties, including Acta Cirúrgica Brasileira, Jornal Vascular Brasileiro, Arquivos Brasileiros de Cirurgia de Digestiva, Revista do Colégio Brasileiro de Cirurgiões, Journal of Coloproctology, Revista Brasileira de Cirurgia Plástica, and International Brazilian Journal of Urology. Results: The journals with the lowest average waiting times were Revista do Colégio Brasileiro de Cirurgiões, Acta Cirúrgica Brasileira, and Journal of Coloproctology, respectively, and, with the lowest interquartile range there is Acta Cirúrgica Brasileira. There was no significant difference between the pre-pandemic and pandemic periods. The study designs with the highest and lowest means were, respectively, ideas and innovations – also with the highest interquartile range – and expert opinion, while with the lowest interquartile range was technical skill. Conclusions: The acceptance time for articles in Brazilian surgery journals is extremely variable. Identifying these discrepancies highlights the importance of understanding editorial processes and seeking ways to improve consistency and efficiency in reviewing articles.

Resumo Introdução: A vida das pessoas com Doença Renal Crônica (DRC) é alterada em diversos aspectos. Dentre eles, o trabalho destaca-se como um dos que sofrem profundas consequências e comprometimento na sua continuidade. Embora existam estudos sobre as limitações impostas pelo tratamento da DRC nas atividades profissionais, a percepção dos pacientes em relação a essas limitações ainda é pouco explorada na literatura. Objetivo: Conhecer as percepções dos pacientes renais crônicos, em hemodiálise e transplantados renais, sobre as limitações impostas pelo tratamento nas suas atividades profissionais. Métodos: Estudo qualitativo, realizado com 21 pacientes com DRC (13 em tratamento de hemodiálise e 8 transplantados renais). A coleta de dados se deu por meio de entrevistas, a partir de um roteiro semiestruturado, e submetidas à análise de conteúdo, na modalidade temática. Resultados: Foram identificados três temas de análise: 1. Diagnóstico e acesso ao tratamento; 2. Limitações nas atividades diárias e no trabalho e 3. Mudanças relacionadas ao trabalho e apoio familiar. Entre os pacientes com hemodiálise, a necessidade de realizar sessões frequentes foi citado como um dificultador para a retomada das atividades laborais. Mesmo entre os pacientes transplantados, as limitações físicas e os cuidados pós-cirúrgicos afetaram a produtividade e a capacidade de retorno ao trabalho da maneira habitual. Conclusões: As mudanças experimentadas pelos entrevistados após o início do tratamento da DRC afetaram sua capacidade de retornar ao trabalho, e o apoio familiar foi considerado essencial para facilitar esse processo. A pesquisa qualitativa se mostrou uma ferramenta importante no entendimento de como as mudanças na vida do paciente renal crônico após o início do tratamento afetam sua capacidade de retornar ao trabalho.
Abstract Introduction: The lives of people with Chronic Kidney Disease (CKD) are changed in several ways, among them, work stands out as one of those that suffers profound consequences and compromises its continuity. Although there are studies on the limitations imposed by the treatment of CKD in professional activities, the perception of patients in relation to these limitations is still little explored in the literature. Objective: To know the perceptions of chronic kidney patients, on hemodialysis and kidney transplant patients, about the limitations imposed by the treatment in their professional activities. Methods: Qualitative study, carried out with 21 patients with CKD (13 on hemodialysis and 8 with kidney transplant). Data collection took place through interviews, based on a semi-structured script, and submitted to content analysis, in the thematic modality. Results: Three analysis themes were identified: 1. Diagnosis and access to treatment; 2. Limitations in daily activities and work and 3. Work-related changes and family support. Among hemodialysis patients, the need for frequent sessions was cited as a barrier to resuming work activities. Even among transplant patients, physical limitations and post-surgical care affected productivity and the ability to return to work as usual. Conclusions. The changes experienced by respondents after starting CKD treatment affected their ability to return to work, and family support was considered essential to facilitate this process. Qualitative research proved to be an important tool in understanding how changes in the life of chronic renal patients after starting treatment affect their ability to return to work.

RESUMO A aplicação de herbicidas dessecantes na cultura do crambe é importante para produção de sementes de qualidade. Objetivou-se avaliar o efeito de herbicidas dessecantes e diferentes volumes de calda aplicadas em plantas de crambe, cultivar FMS Brilhante, na fase de pré-colheita sobre a produtividade e qualidade de sementes. O delineamento experimental foi em blocos casualizados em esquema fatorial 2 x 5, sendo dois herbicidas dessecantes (Glyphosate – Roundup Original 360 g L-1 e Diquat – Reglone 200 g L-1) e cinco volumes de calda (0,0; 0,5; 1,0; 1,5; e 2,0 L ha-1), com quatro repetições. A aplicação dos dessecantes ocorreu aos 74 dias após a semeadura, baseada no estádio de maturação das sementes, verificado a partir da observação visual da porcentagem de sementes marrons (80%). Após a colheita, as sementes foram beneficiadas e avaliadas quanto ao teor de água, peso de mil sementes, produtividade, teor de óleo, germinação e vigor (primeira contagem de germinação, índice de velocidade de emergência, envelhecimento acelerado e condutividade elétrica). A dessecação na fase de pré-colheita das plantas de crambe com o herbicida Diquat, nos volumes de calda entre 1,3 e 1,5 L ha-1, proporcionou sementes de melhor qualidade fisiológica. O vigor das sementes foi reduzido pela dessecação das plantas com Glyphosate. A produtividade da cultura não foi afetada pelos herbicidas e nem pelos diferentes volumes de calda. ABSTRACT The application of burndown herbicides to crambe crops is important for production of quality seeds. The objective of this work was to evaluate the effect of burndown herbicides applied at pre-harvest, using different spray volumes, on seed yield and quality of crambe plants (Crambe abyssinica cv. FMS Brilhante). A randomized block experimental design was used, with four replications, in a 2 × 5 factorial arrangement consisted of two burndown herbicides [glyphosate (Roundup Original 360 g L-1) and diquat (Reglone 200 g L-1)] and five spray volumes (0.0, 0.5, 1.0, 1.5, and 2.0 L ha-1). The burndown herbicides were applied at 74 days after sowing, when the plants presented 80% brown seeds, based on visual evaluation of the seed maturation stage. After harvest, the seeds were processed and evaluated for water content, one-thousand seed weight, seed yield, oil contents, germination, and vigor (first germination counting, emergence speed index, accelerated aging, and electrical conductivity). The pre-harvest desiccation of crambe with the herbicide diquat, using spray volumes between 1.3 and 1.5 L ha-1, resulted in seeds with better physiological quality. Abstract The aim of the present study was to investigate the impact of oral health literacy (OHL) on conceptions of care and behaviors related to COVID-19. The sample came from two preliminary cross-sectional studies that determined the level of OHL of parents/guardians of six-to-12-year-old children in two major Brazilian cities (Curitiba and Belo Horizonte). Functional OHL was measured using the Brazilian version of the Rapid Estimate of Adult Literacy in Dentistry (BREALD-30) and the Health Literacy Dental Scale (HeLD-14) for the evaluation of interactive OHL. Participants were recruited through e-mail, social media, and telephone contact. The questionnaire on conceptions of care and behaviors related to COVID-19 was created based on the guidelines of the World Health Organization. Two hundred nineteen individuals participated in the study. There was no significant difference in socioeconomic and demographic variables and in the medians of BREALD and HeLD-14 between the two cities (P>0.05). Higher levels of functional OHL were associated with an appropriate conception that individual care affects collective care (P=0.038), but with an inappropriate conception of seeking medical assistance in cases of mild symptoms (P=0.030). Higher levels of interactive OHL were related to social distancing behavior in the city of Curitiba (P=0.049) and in the overall sample (P=0.040). It is concluded that functional OHL was associated with two of the investigated conceptions about COVID-19, while interactive OHL was associated with social distancing behavior. These data may suggest that different dimensions of the OHL can have an impact on different aspects of coping with the pandemic. (OHL COVID19. RESUMO Objetivo Verificar a adequação das oportunidades de ação no ambiente doméstico de crianças em risco de atraso no desenvolvimento e identificar fatores associados à sua frequência. Métodos Este estudo transversal incluiu 97 famílias que responderam ao Affordances in the Home Environment for Motor Development - Infant Scale (AHEMD-IS) para 3–18 meses (n=63) ou o Self Report (AHEMD-SR) para 18–42 meses (n=34). O teste de Mann-Whitney foi utilizado para identificar as diferenças entre as frequências de oportunidades de ação entre os grupos. A regressão linear múltipla foi utilizada para verificar a associação entre o sexo da criança, o estado civil, a escolaridade e nível socioeconômico da mãe, as idades da criança e da mãe, o número de residentes da casa, a renda per capita e os escores do AHEMD (α=0,05). Resultados A frequência das oportunidades de ação dos domicílios no AHEMD-IS variou de menos do que adequado a excelente; enquanto, no AHEMD-SR, a maior predominância foi de média frequência. A oferta de estímulos no AHEMD-IS foi significativamente maior. O maior nível socioeconômico e o número de moradores da casa foram associados a maiores oportunidades de ação. Abstract Objective: The aim of this study was to verify the adequacy of affordances in the home environment of children at risk of developmental delay and to identify factors associated with their frequency. Methods: The cross-sectional study included 97 families who responded to the Affordances in the Home Environment for Motor Development — Infant Scale (AHEMD-IS) for 3–18 months (n=63), or AHEMD – Self-Report (AHEMD-SR) for 18–42 months (n=34). The Mann-Whitney U test was used to identify the differences between the frequencies of affordances between the groups. Multiple linear regression was used to verify the association between the child’s sex, mother’s marital status, education, socioeconomic level, child and mother’s ages, house residents’ number, per capita income, and AHEMD scores (α=0.05). Results: The home affordances’ frequency in the AHEMD-IS ranged from less than adequate to excellent, while in the AHEMD-SR, the highest predominance was medium. The offer of stimuli in the AHEMD-IS was significantly higher. Higher socioeconomic level and house residents’ number were associated with greater affordances. Conclusions: The higher the socioeconomic level and house residents’ number, the greater the affordances in the homes of children at risk of delay. It is necessary to provide families with some alternatives to make their home environments richer in affordances that favor child development. Abstract Background Diagnosis of SS is a complex task, as no symptom or test is unique to this syndrome. The American-European Consensus Group (AECG 2002) and the American-European classification criteria of 2016 (ACR/EULAR 2016) emerged through a search for consensus. This study aims to assess the prevalence of Sjögren’s Syndrome (SS) in patients with Systemic Lupus Erythematosus (SLE), according to AECG 2002 and ACR-EULAR 2016 classifications, as well as clinical and histopathological features in this overlap. To date, there is no study that has evaluated SS in SLE, using the two current criteria. Methods This cross-sectional study evaluated 237 SLE patients at the outpatient rheumatology clinic between 2016 and 2018. Patients were submitted to a dryness questionnaire, whole unstimulated salivary flow (WUSF), “Ocular Staining Score” (OSS), Schirmer’s test I (ST-I), and labial salivary gland biopsy (LSGB). Results After verifying inclusion and exclusion criteria, a total of 117 patients were evaluated, with predominance of females (94%) and mixed ethnicity (49.6%). The prevalence of SS was 23% according to AECG 2002 and 35% to ACR- EULAR 2016. Kappa agreement between AECG 2002 and ACR-EULAR 2016 were 0.7 (p < 0.0001). After logistic regression, predictors for SS were: anti/Ro (OR = 17.86, p < 0.05), focal lymphocytic sialadenitis (OR = 3.69, p < 0.05), OSS ≥ 5 (OR = 7.50, p < 0.05), ST I positive (OR = 2.67, p < 0.05), and WUSF ≤ 0.1 mL/min (OR = 4.13, p < 0.05). Conclusion The prevalence of SS in SLE was 23% (AECG 2002) and 35% (ACR-EULAR 2016). The presence of glandular dysfunction, focal lymphocytic sialadenitis, and anti/Ro were predictors of SS in SLE. The greatest advantage of the new ACR-EULAR 2016 criteria is to enable an early diagnosis and identify the overlapping of these two diseases. ACR- EULAR 2016 criteria is not yet validated for secondary SS and this study is a pioneer in investigating prevalence based on the new criteria.

ABSTRACT The objective of this study was to evaluate the effects of cocoa meal in feed concentrate on the spermatogenesis in sheep. Twenty-five uncastrated males were divided into four groups and fed concentrated feed, supplemented with 0, 10, 20, and 30% cocoa meal, respectively, for 150 days. At the end of this period, the animals were slaughtered, the testicles were collected for histological slides, and testicular morphometric assessments were conducted, including cell type quantification in the seminiferous epithelium, intrinsic spermatogenesis yield, Sertoli-cell index, total number of Sertoli cells (TNSC), TNSC per testicular gram, daily sperm production (DSP), DSP per testicular gram, testicular sperm reserve (TSR), and TSR per testicular gram. Data were subjected to normality analysis using the Shapiro-Wilk test, followed by the tests for each condition, at a significance level of 5%. The administration of cocoa meal did not alter the germinal epithelial cell population, except for the number of type-A spermatogonia, which was lower in the control group than in the group that received 20% supplementation. A difference was found in the ratio between the number of type-A spermatogonia and primary spermatocytes in pre-leptotene/leptotene, and in the ratio between Sertoli cells and primary spermatocytes in pre-leptotene/leptotene, the control group and the group that received 10% cocoa meal significantly varied from each other, but not in relation to the other evaluated percentages. The TNSC, TNSC/g, DSP, and TSR levels were diminished in the group that consumed 30% cocoa meal. The TNSC, DSP, and TSR exhibited a negative quadratic tendency. Supplementation with 10% cocoa meal improved the TNSC, DSP, and TSR of sheep.

ABSTRACT. Unpaid caregivers of people living with dementia tend to suffer mental health problems as a result of the negative effects associated with the care tasks. Thus, psychosocial interventions for this population group are necessary. iSupport is an online support program for caregivers that was created by the World Health Organization. Objective: To describe the design of a randomized clinical trial to measure the efficacy of the iSupport-Brasil version on caregivers’ mental health and well-being. Methods: The participants will be randomized into Intervention Group (IG) (n=195) and Control Group (CG) (n=195). For three months, the IG will access the iSupport-Brasil platform, the CG will enter the electronic page of the Brazilian Alzheimer’s Association, and both groups will be emailed the preliminary version of the “Guia de cuidados para a pessoa idosa” e-book (a guide to providing care to the elderly) from the Ministry of Health. The data will be collected at three moments: baseline, and three and six months after the beginning of the intervention. Results: It is expected that it will be possible to provide diverse validity evidence about iSupport-Brasil as an online and free intervention alternative, as a preventive means and as a way to promote mental health among caregivers of people living with dementia. Conclusion: Through the evaluation protocol of this randomized clinical trial on the effects of the iSupport-Brasil program, it may become a reference for countries that plan to adapt and improve the iSupport program using digital health solutions.
RESUMO. Cuidadores informais de pessoas que vivem com demência tendem a sofrer problemas de saúde mental como resultado dos efeitos negativos associados às tarefas de cuidado. Assim, intervenções psicossociais para essa população são necessárias. O iSupport é um programa de apoio online para cuidadores criado pela Organização Mundial da Saúde. Objetivo: Descrever o desenho de um ensaio clínico randomizado para medir a eficácia da versão iSupport-Brasil na saúde mental e no bem-estar de cuidadores. Métodos: Os participantes serão randomizados em Grupo Intervenção (GI) (n=195) e Grupo Controle (GC) (n=195). Durante três meses, o GI acessará a plataforma iSupport-Brasil, o CG entrará na página eletrônica da Associação Brasileira de Alzheimer e ambos os grupos receberão por e-mail a versão preliminar do e-book Guia de cuidados para a pessoa idosa, do Ministério da Saúde. Os dados serão coletados em três momentos: linha de base, três e seis meses após o início da intervenção. Resultados: Espera-se que seja possível fornecer diversas evidências sobre os efeitos do iSupport-Brasil como alternativa de intervenção online e gratuita, de forma a promover a saúde mental entre os cuidadores de pessoas que vivem com demência. Conclusão: O protocolo de avaliação deste ensaio clínico randomizado sobre os efeitos do programa iSupport-Brasil pode se tornar uma referência para os países que planejam adaptar e melhorar esta intervenção, usando soluções digitais de saúde.

ABSTRACT Objective: To analyze the conceptions about COVID-19 among Brazilians who carry out commuting to work in clandestine mines located on the borders between Brazil, French Guiana and Suriname. Method: This is qualitative research, from an analytical perspective, based on Social Representation Theory. Semi-structured, audio-recorded interviews were carried out with 10 Brazilians who experience work routine in clandestine mining on the border between Brazil, French Guiana and Suriname. Results: Two analytical categories emerged: “The disease of otherness”; and “Health access dimension”. Conclusion: Disease severity was attributed to another or a human body organ, and not to individuals as a whole. Access to health services was established on issues of inequity, violence and illegal practices. The nature of a transient population, which carries out commuting and informal and clandestine work, demonstrates vulnerability to COVID-19 and a lower propensity to receive care.
RESUMO Objetivo: Analisar as concepções acerca da covid-19 entre brasileiros que realizam migração pendular para trabalhar em garimpos clandestinos situados nas fronteiras entre Brasil, Guiana Francesa e Suriname. Método: Pesquisa qualitativa, em uma perspectiva analítica, alicerçada na Teoria das Representações Sociais. Foram realizadas entrevistas semiestruturadas, áudiogravadas com 10 brasileiros que vivenciam a rotina de trabalho em garimpos clandestinos na fronteira entre Brasil, Guiana Francesa e Suriname. Resultados: Foram designadas duas categorias analíticas: “A doença da alteridade”; e “Dimensão do acesso à saúde”. Conclusão: A gravidade da doença foi atribuída ao outro ou a um órgão do corpo humano, e não ao indivíduo como um todo. O acesso aos serviços de saúde instituiu-se em questões de iniquidade, violência e práticas ilegais. O caráter de população transitória, que realiza migração pendular e de trabalho informal e clandestino, demonstra vulnerabilidade à covid-19 e menor propensão a receber cuidados.
RESUMEN Objetivo: Analizar las concepciones sobre el COVID-19 entre los brasileños que realizan desplazamientos para trabajar en minas clandestinas ubicadas en las fronteras entre Brasil, Guayana Francesa y Surinam. Método: Investigación cualitativa, desde una perspectiva analítica, basada en la Teoría de las Representaciones Sociales. Se realizaron entrevistas semiestructuradas y grabadas en audio a 10 brasileños que viven la rutina de trabajo en minas clandestinas en la frontera entre Brasil, Guayana Francesa y Surinam. Resultados: Se designaron dos categorías analíticas: “La enfermedad de la alteridad”; y “Dimensiones del acceso a la salud”. Conclusión: La gravedad de la enfermedad se atribuyó a otro o a un órgano del cuerpo humano, y no al individuo en su conjunto. El acceso a los servicios de salud se estableció sobre temas de inequidad, violencia y prácticas ilegales. La naturaleza de la población transitoria, que realiza desplazamientos y trabajos informales y clandestinos, demuestra vulnerabilidad al COVID-19 y una menor propensión a recibir atención.
