Abstract Fibromatosis colli is the most severe presentation of congenital muscular torticollis and a relatively rare cause of neck swelling in neonates and infants. We present a case of a 6 weeks old male infant who was admitted to the emergency department with a mass in the left lateral-cervical region. Two soft tissue ultrasounds were performed showing multiple adenopathy and one week later a new ultrasound was performed by a radiologist experienced in pediatric imaging that was suggestive of fibromatosis colli. Ultrasound is the diagnosis imaging modality of choice if there is any doubt in the diagnosis. It shows focal or diffuse enlargement of the sternocleidomastoid muscle in a fusiform configuration.

Resumo A Fibromatosis colli é a apresentação mais grave do torcicolo muscular congénito e uma causa rara de tumefação cervical em recém-nascidos e lactentes. Apresentamos o caso de um lactente do sexo masculino com 6 semanas de vida que deu entrada no serviço de urgência com uma massa na região látero-cervical esquerda. Foram realizadas duas ecografias de partes moles que mostraram múltiplas adenopatias e após uma semana foi realizada uma nova ecografia por um radiologista experiente em pediatria que era sugestiva de fibromatosis colli. A ecografia é a modalidade de diagnóstico de escolha se existir dúvida no diagnóstico. Normalmente mostra aumento focal ou difuso do músculo esternocleidomastóideo em configuração fusiforme.

Resumo Há uma enorme disparidade entre os países de alta renda e outros em termos de acesso a dispositivos médicos cardíacos, como marca-passos e desfibriladores implantáveis. Os custos são uma das principais barreiras ao uso de dispositivos cardíacos nesses países. Existem iniciativas internacionais que visam reduzir essa disparidade, e o reuso de marca-passos tem sido discutido como uma possível alternativa. Abstract There is a gap between high-income countries and others in terms of access to medical cardiac devices, such as pacemakers and implantable cardioverter defibrillators. Costs are one of the main barriers to the use of cardiac devices in these countries. There are international initiatives that aim to reduce the gap. The reuse of pacemakers has been discussed as a possible alternative to this problem. The concept of reusing pacemakers is not new; however, recent studies have proven to be safe, ethical, and effective for those who need cardiac implantable electronic devices and cannot afford them. Abstract Chronic kidney disease is a worldwide public health problem, and end-stage renal disease requires dialysis. Most patients requiring renal replacement therapy have to undergo hemodialysis. Therefore, vascular access is extremely important for the dialysis population, directly affecting the quality of life and the morbidity and mortality of this patient population. Since making, managing and salvaging of vascular accesses falls within the purview of the vascular surgeon, developing guideline to help specialists better manage vascular accesses for hemodialysis if of great importance. Abstract Obesity affects several areas of the human body, leading to increased morbidity and mortality and the likelihood of other diseases, such as type 2 diabetes mellitus, cardiovascular diseases and musculoskeletal disorders. These conditions predispose to bone fractures and sarcopenic obesity, defined by the presence of an obesity-associated decrease in muscle mass and strength. Both bone fragility and sarcopenic obesity disease are consequences of several factors, such as a low degree of chronic inflammation, insulin resistance, hormonal changes, nutritional deficiencies, ectopic fat deposits and sedentary lifestyle. The diagnosis of obesity-related musculoskeletal disorders is limited by the lack of sarcopenia criteria and lower accuracy of bone mineral density measurement by dual-energy X-ray absorptiometry in overweight people. Reducing body weight provides undeniable benefits to this population, however treating cases of severe obesity with bariatric surgery can cause even greater damage to bone and muscle health, especially in the long term. The mechanisms involved in this process are not yet fully understood, but factors related to nutrient malabsorption and mechanical discharge as well as changes in gut hormones, adipokines and bone marrow adiposity should be taken into account. Depending on the surgical technique performed, greater musculoskeletal damage may occur, especially in cases of malabsorptive surgeries such as Roux-en-Y gastric bypass, when compared to restrictive techniques such as sleeve gastrectomy. This difference is probably due to greater weight loss, nutrient malabsorption and important hormonal changes that occur as a consequence of the diversion of intestinal transit and loss of greater absorptive surface. Thus, people undergoing bariatric procedures, especially malabsorptive ones, should have their musculoskeletal health supervised to allow early diagnosis and appropriate therapeutic interventions to prevent osteoporotic fractures and preserve the functionality of the skeletal muscles. RESUMO Introdução: Nas últimas décadas houve uma grande evolução na superfície de revestimento dos implantes mamários, o que resultou na diminuição das complicações. No pós-operatório a inflamação é uma constante e pode ser avaliada pelo hemograma, pois é um exame rápido, barato e com alta disponibilidade. O presente estudo tem como objetivo avaliar os biomarcadores sanguíneos em ratas submetidas à colocação de mini-implantes de silicone nanotexturizados e revestidos por espuma de poliuretano. Métodos: Foram utilizadas 60 ratas Wistar divididas em dois grupos para utilização de mini-implantes nanotexturizados e revestidos com espuma de poliuretano, subdivididos em subgrupos de acordo com a eutanásia dos animais nos 30, 60 e 90 dias. No momento da eutanásia, as amostras de sangue foram obtidas por punção cardíaca e foi analisado o hemograma. Resultados: A hemoglobina, o hematócrito, a hemoglobina corpuscular média, os leucócitos, os neutrófilos, os linfócitos e as plaquetas tiveram os resultados muito semelhantes em todos os subgrupos avaliados (30, 60 e 90 dias). Entretanto, quando os diferentes subgrupos foram comparados entre si dentro de cada grupo, obteve-se resultados estatisticamente significantes na hemoglobina corpuscular média (nanotexturizado p=0,032 e poliuretano p=0,007) e nos leucócitos (nanotexturizado p=0,038 e poliuretano p=0,034). Sobre as alterações dos biomarcadores sanguíneos no pós-operatório encontrou-se anemia hipocrômica, contagem de leucócitos normais, neutrofilia, linfopenia e trombocitopenia. Conclusão: Após a colocação de mini-implantes de silicone, as ratas de ambos os grupos evoluíram com anemia hipocrômica, contagem de leucócitos normais às custas de neutrofilia e linfopenia, e trombocitopenia.

ABSTRACT Introduction: In recent decades, there has been a great evolution in breast implants' lining surface, which has resulted in decreased complications. In the postoperative period, the inflammation is constant and can be evaluated by the blood count, as it is a fast, inexpensive, and highly available examination. The present study evaluates blood biomarkers in rats submitted to the placement of nanotextured silicone implants and implants coated with polyurethane foam. Methods: 60 Wistar rats were used divided into two groups for nanotextured mini-implants and others mini-implants coated with polyurethane foam, subdivided into subgroups according to the animals' euthanasia in the 30, 60, and 90 days. At the time of euthanasia, blood samples were obtained by cardiac puncture and the blood count was analyzed. Results: Hemoglobin, hematocrit, mean corpuscular hemoglobin, leukocytes, neutrophils, lymphocytes, and platelets had very similar results in all subgroups evaluated (30, 60, and 90 days). However, when the different subgroups were compared within each group, statistically significant results were obtained in the mean corpuscular hemoglobin (nanotextured p=0.032 and polyurethane p=0.007) and leukocytes (nanotextured p=0.038 and polyurethane p=0.034). Changes in postoperative blood biomarkers were hypochromic anemia, normal leukocyte count, neutrophilia, lymphopenia, and thrombocytopenia. Conclusion: After the placement of mini-silicone implants, the rats of both groups evolved with hypochromic anemia, normal leukocyte count at the expense of neutrophilia and lymphopenia, and thrombocytopenia.

Abstract The mesoeucrocodylian record from the Bauru Group (Paraná Basin, Brazil) comes from three formations: Adamantina (Late Turonian–Early Maastrichtian), Presidente Prudente (Early Campanian–Early Maastrichtian) and Marília (Maastrichtian). These records are restricted to the Early Coniacian–Early Maastrichtian. Here, we report a new crocodyliform record from the Bauru Group, which was the first of the Araçatuba Formation, from a new locality of Coronel Goulart district, in Álvares Machado municipality. Coronelsuchus civali gen. nov. et sp. nov. comprises two related specimens - FFP PG 13 and FFP PG 14 recovered close to each other and on the same muddy sandstone level. We perform a phylogenetic analysis with 392 characters (five new added) and 88 taxa. Our analyses supported the new clades: Sphagesauria, Sphagesaurinae and Caipirasuchinae. Our phylogenetic results suggest C. civali as a basal Sphagesauria. Regarding the Crocodyliform Assemblage Zones (CAZ), the new species is correlated to the CAZ 1, together with small sphagesaurians, Caryonosuchus and itasuchids. The C. civali reveals an older appearing (pre-Turonian) and diversification of Sphagesauria. The specimens also expand the stratigraphic range for Notosuchia in the Bauru Group.

Resumo Este estudo teve como objetivo investigar as propriedades psicométricas do Inventário de Personalidade para o DSM-5 (PID-5) para amostras clínica e comunitária brasileiras. Esta pesquisa incluiu 1.210 pessoas, sendo 554 delas com indicadores de psicopatologia. Os participantes responderam ao PID-5 e ao Self-Reporting Questionnaire, além de um questionário sobre dados sociodemográfico e de saúde que incluía os seis itens do módulo de risco de suicídio Mini International Neuropsychiatric Interview. Os resultados mostraram que as 25 facetas do PID-5 apresentaram coeficientes de confiabilidade adequados e evidências de unidimensionalidade. A estrutura de cinco fatores do instrumento foi replicada com altos níveis de congruência com a amostra representativa da América do Norte. Os escores PID-5 foram estatisticamente diferentes entre os grupos clínicos e comunitários e foram positivamente correlacionados com medidas de risco de suicídio e sintomas psicopatológicos. Este estudo apresenta as propriedades psicométricas do PID-5 e sua adequação para uso na população brasileira.

Abstract This study aimed to investigate the psychometric properties of the Personality Inventory for DSM-5 (PID-5) in Brazilian community and clinical samples. This research included 1,210 people, 554 of them with psychopathology indicators. The participants answered the PID-5 and the Self-Reporting Questionnaire, in addition to a sociodemographic and health data questionnaire that included the six items of the suicidality module of the Mini International Neuropsychiatric Interview. The results showed that the 25 facets of the PID-5 had adequate reliability coefficients and evidence of unidimensionality. The instrument’s five-factor structure was replicated with high levels of congruence with the representative sample from North America. PID-5 scores were statistically different between clinical and community groups and were positively correlated with measures of suicide risk and psychopathological symptoms. This study presents the psychometric properties of PID-5 and its suitability for use in the Brazilian population.

Resumen Este estudio tuvo como objetivo investigar las propiedades psicométricas del Inventario de Personalidad para el DSM-5 (PID-5) en muestras clínicas y comunitarias de Brasil. Esta investigación incluyó a 1.210 personas, 554 de ellas con indicadores de psicopatología. Los participantes respondieron al PID-5 y al Self-Reporting Questionnaire, además de un cuestionario de datos sociodemográficos y de salud que incluía los seis ítems del módulo de suicidio de la Mini Entrevista Neuropsiquiátrica Internacional. Los resultados mostraron que las 25 facetas del PID-5 presentaron coeficientes de confiabilidad adecuados y evidencias de unidimensionalidad. La estructura de cinco factores del instrumento se replicó con altos niveles de congruencia con la muestra representativa de Norteamérica. Las puntuaciones de PID-5 fueron estadísticamente diferentes entre los grupos clínicos y comunitarios, y se correlacionaron positivamente con las medidas de riesgo de suicidio y síntomas psicopatológicos. Este estudio presenta las propiedades psicométricas del PID-5 y su idoneidad para su uso en la población brasileña.

The first Jurassic theropod from the Sergi Formation, Jatobá Basin, Brazil

Abstract Archosaur osteological remains are abundant in Brazil, particularly from the Triassic and Cretaceous strata, but in Jurassic, the record is predominantly represented by ichnofossils. The Upper Jurassic archosaur records comprise the Paralligatoridae Batrachomimus pastosbonensis, from the Pastos Bons Formation (Parnaíba Basin), remains of Mesoeucrocodylia from the Aliança Formation (Jatobá Basin), and fragments of Dinosauria from the Brejo Santo Formation (Araripe Basin) with a dubious assignment. Here, we present the first undoubted Dinosauria record for the Jurassic of Brazil, MCT 2670-LE, a middle to distal caudal vertebra belonging to a theropod. MCT 2670-LE was excavated in the 60s by prof. Ignacio Machado Brito, being primarily attributed to strata of the Aliança Formation but the rock matrix involving the specimen allows us to correlate MCT 2670-LE to Sergi Formation, marking this specimen as the first archosaur record of this unit. The caudal vertebra is assigned to Carcharodontosauria based on the hourglass shaped centrum and the deep and narrow ventral groove. The specimen provides new data to the paleogeographic distribution of carcharodontosaurians before the South America-Africa break-up, as well as understanding the theropod faunal turnover of paleoenvironments during the Jurassic-Cretaceous transition in Brazil.

RESUMO CONTEXTO: Hérnias de parede abdominal são patologias cirúrgicas frequentes. O surgimento da cirurgia minimamente invasiva levantou questionamentos sobre a melhor técnica a ser aplicada, considerando a possível redução de dor pós-operatória e de complicações, e retorno precoce às atividades habituais. OBJETIVO: Avaliar frequência de hernioplastias abertas e laparoscópicas no Brasil entre os anos de 2008 e 2018, analisar taxas de cirurgias urgentes e eletivas, mortalidade, custos e o impacto do treinamento em cirurgia laparoscópica no sistema público de saúde. MÉTODOS Análise de banco de dados do registro de saúde pública (DATASUS) entre 2008 e 2018 para coleta dos dados e parâmetros selecionados. RESULTADOS: Foram realizadas 2.671.347 hernioplastias no período, média de 242.850 cirurgias/ano (99,4% abertas, 0,6% laparoscópicas). Predominou a faixa etária economicamente ativa (20-59 anos), 54,5% do total. Houve redução significativa das cirurgias abertas (P<0,01), sem aumento compensatório da laparoscopia. 22,3% das cirurgias foram de urgência, com aumento significativo da mortalidade em relação às cirurgias eletivas (P<0,01). Observou-se distribuição heterogênea da videolaparoscopia, diretamente relacionada com o número de cirurgiões digestivos. CONCLUSÃO: Este estudo apresenta pela primeira vez os dados populacionais das cirurgias de hérnia no Brasil. As técnicas minimamente invasivas representam uma parcela pouco significativa das hernioplastias. As cirurgias urgentes apresentam percentual elevado comparado a outros países, com aumento significativo na mortalidade, reforçando a necessidade de políticas que permitam aumento da oferta do serviço, treinamento especializado e equalização na distribuição dos procedimentos em todas as regiões.

ABSTRACT BACKGROUND: Abdominal wall hernia is one of the most common surgical pathologies. The advent of minimally invasive surgery raised questions about the best technique to be applied, considering the possibility of reducing postoperative pain, a lower rate of complications, and early return to usual activities. OBJECTIVE: To evaluate the frequency of open and laparoscopic hernioplasties in Brazil from 2008 to 2018, analyzing the rates of urgent and elective surgeries, mortality, costs, and the impact of laparoscopic surgical training on the public health system. METHODS: Nationwide data from 2008 to 2018 were obtained from the public health registry database (DATASUS) for a descriptive analysis of the selected data and parameters. RESULTS: 2,671,347 hernioplasties were performed in the period, an average of 242,850 surgeries per year (99.4% open, 0.6% laparoscopic). The economically active population (aged 20-59) constituted the dominant group (54.5%). There was a significant reduction (P<0.01) in open surgeries, without a compensatory increase in laparoscopic procedures. 22.3% of surgeries were urgent, with a significant increase in mortality when compared to elective surgeries (P<0.01). The distribution of laparoscopic surgery varied widely, directly associated with the number of digestive surgeons. CONCLUSION: This study presents nationwide data on hernia repair surgeries in Brazil for the first time. Minimally invasive techniques represent a minor portion of hernioplasties. Urgent surgeries represent a high percentage when compared to other countries, with increased mortality. The data reinforce the need for improvement in the offer of services, specialized training, and equalization in the distribution of procedures in all regions.

RESUMO CONTEXTO: A síndrome hepatopulmonar (SHP) é uma complicação associada à cirrose que pode contribuir para piora da capacidade de exercício e menor sobrevida após o transplante hepático (TxH). OBJETIVO: Avaliar a capacidade de exercício, as complicações e a sobrevida após TxH em cirróticos com SHP e comparar com os resultados de cirróticos sem esse diagnóstico. MÉTODOS: Estudo de coorte prospectivo, composto inicialmente por 178 pacientes, dos quais 90 foram submetidos ao TxH (42 com SHP e 48 sem SHP). Foi realizada uma avaliação prévia composta pelo teste de caminhada dos seis minutos (TC6M), teste ergométrico e manovacuometria. Os submetidos ao TxH tiveram avaliados o tempo de ventilação mecânica (VM), uso de ventilação não invasiva (VNI), e a sobrevida dois anos após o procedimento. Na análise estatística utilizamos os testes de Kolmogorov-Smirnov, o teste t de Student, o teste do quadrado de associação linear, a curva de sobrevida de Kaplan Meier. Os dados foram analisados no programa SPSS 16.00 sendo considerado significativo P<0,05. RESULTADOS: O grupo SHP apresentou menor pico de consumo de oxigênio (VO2pico) (14,2±2,3 vs 17,6±2,6) P<0,001, e menor distância percorrida no TC6M (340,8±50,9 vs 416,5±91,4) P<0,001 antes do TxH. Os pacientes com SHP transplantados permaneceram mais horas em VM (19,5±4,3 vs 12,5±3,3) P=0,02, necessitaram mais de VNI (12 vs 2) P=0,01, e tiveram menor sobrevida dois anos após o procedimento (P=0,01). CONCLUSÃO: Pacientes com SHP apresentaram pior capacidade de exercício antes do TxH, mais complicações e menor sobrevida após a realização desse procedimento.

ABSTRACT BACKGROUND: Hepatopulmonary syndrome (HPS) is a complication associated with cirrhosis that may contribute to worsening exercise capacity and reduced survival after liver transplantation (LT). OBJECTIVE: To evaluate exercise capacity, complications and survival after LT in patients with cirrhosis and HPS and to compare these results with the results of patients with cirrhosis without HPS. METHODS: A prospective cohort study, consisting initially of 178 patients, of whom 90 underwent LT (42 with HPS and 48 without HPS). A previous evaluation consisted of the six-minute walk test (6MWT), an exercise test and manovacuometry. Those who underwent LT were evaluated for the mechanical ventilation time (MV), noninvasive ventilation (NIV) use, and survival two years after the procedure. In the statistical analysis, we used the Kolmogorov-Smirnov test, Student’s t-test, the linear association square test, and the Kaplan-Meier survival curve. The data were analyzed with the SPSS 16.00 program and considered significant at P<0.05. RESULTS: The HPS group demonstrated a lower peak of oxygen consumption (VO2peak) (14.2±2.3 vs 17.6±2.6) P<0.001 and a shorter distance walked on the 6MWT (340.8±50.9 vs 416.5±91.4) P<0.001 before LT compared with the non-HPS group. The transplanted patients with HPS remained longer hours in MV (19.5±4.3 vs 12.5±3.3) P=0.02, required more NIV (12 vs 2) P=0.01, and had lower survival two years after the procedure (P=0.01) compared with the transplanted patients without HPS. CONCLUSION: Patients with HPS had worse exercise capacity before LT, more complications and shorter survival after this procedure than patients without HPS.

ABSTRACT Hypovitaminosis D is a common condition with a negative impact on health. This statement, prepared by experts from the Brazilian Society of Endocrinology and Metabolism and the Brazilian Society of Clinical Pathology/Laboratory Medicine, includes methodological aspects and limitations of the measurement of 25-hydroxyvitamin D [25(OH)D] for identification of vitamin D status, and identifies individuals at increased risk for deficiency of this vitamin in whom 25(OH)D measurement is recommended. For the general population, 25(OH)D levels between 20 and 60 ng/mL are considered normal, while individuals with levels below 20 ng/mL are considered to be vitamin D deficient. This statement identifies potential benefits of maintaining 25(OH)D levels > 30 ng/mL in specific conditions, including patients aged > 65 years or pregnant, those with recurrent falls, fragility fractures, osteoporosis, secondary hyperparathyroidism, chronic kidney disease, or cancer, and individuals using drugs with the potential to affect the vitamin D metabolism. This statement also calls attention to the risk of vitamin D intoxication, a life-threatening condition that occurs at 25(OH)D levels above 100 ng/mL

Resumo Fundamento A síndrome de takotsubo (takotsubo) é uma forma de cardiomiopatia adquirida. Dados nacionais sobre essa condição são escassos. O Registro REMUTA é o primeiro a incluir dados multicêntricos dessa condição no nosso país. Objetivo Descrever as características clínicas, prognóstico, tratamento intra-hospitalar e mortalidade hospitalar e em 1 ano de seguimento. Métodos Estudo observacional, retrospectivo, tipo registro. Incluídos pacientes internados com diagnóstico de takotsubo ou que desenvolveram esta condição durante internação por outra causa. Os desfechos avaliados incluíram fator desencadeador, análise dos exames, uso de medicações, complicações e óbito intra-hospitalar e em 1 ano de seguimento. O nível de significância adotado foi de 5%. Resultados Foram incluídos 169 pacientes, em 12 centros no Estado do Rio de Janeiro. A idade média foi de 70,9 ± 14,1 anos e 90,5% eram do sexo feminino; 63% dos casos foram de takotsubo primário e 37% secundário. Troponina I foi positiva em 92,5% dos pacientes e a mediana de BNP foi de 395 (176,5; 1725). Supradesnivelamento do segmento ST esteve presente em 28% dos pacientes. A fração de ejeção do ventrículo esquerdo teve mediana de 40 (35; 48)%. Observamos taxa de 25,7% de ventilação mecânica invasiva e 17,4% de choque. Suporte circulatório mecânico foi utilizado em 7,7%. A mortalidade intra-hospitalar foi de 10,6% e a mortalidade ao final de 1 ano foi de 16,5%. Takotsubo secundário e choque cardiogênico foram preditores independentes de mortalidade. Conclusão Os resultados do REMUTA mostram que takotsubo não se trata de patologia benigna como se pensava, especialmente no grupo de takotsubo secundário que acarreta elevada taxa de complicações e de mortalidade. Abstract Background Takotsubo syndrome (TTS) is an acquired form of cardiomyopathy. National Brazilian data on this condition are scarce. The Takotsubo Multicenter Registry (REMUTA) is the first to include multicenter data on this condition in Brazil. Objective To describe the clinical characteristics, prognosis, in-hospital treatment, in-hospital mortality, and mortality during 1 year of follow-up. Methods This is an observational, retrospective registry study including patients admitted to the hospital with diagnosis of TTS and patients admitted for other reasons who developed this condition. Evaluated outcomes included triggering factor, analysis of exams, use of medications, complications, in-hospital mortality, and mortality during 1 year of follow-up. A significance level of 5% was adopted. Results The registry included 169 patients from 12 centers in the state of Rio de Janeiro, Brazil. Mean age was 70.9 ± 14.1 years, and 90.5% of patients were female; 63% of cases were primary TTS, and 37% were secondary. Troponin I was positive in 92.5% of patients, and median BNP was 395 (176.5; 1725). ST-segment elevation was present in 28% of patients. Median left ventricular ejection fraction was 40 (35; 48)%. We observed invasive mechanical ventilation in 25.7% of cases and shock in 17.4%. Mechanical circulatory support was used in 7.7%. In-hospital mortality was 10.6%, and mortality at 1 year of follow-up was 16.5%. Secondary TTS and cardiogenic shock were independent predictors of mortality. Conclusion The results of the REMUTA show that TTS is not a benign pathology, as was once thought, especially regarding the secondary TTS group, which has a high rate of complications and mortality. Abstract The objective of the present study was to characterize the physicochemical composition and to quantify the fatty acids of chocolates produced from different cocoa varieties (Theobroma cacao L.) cultivated in the Southern Region of Bahia. Nine samples of monovarietal chocolates were evaluated, according to their physical, thermal and fatty acid profile. The results were subjected to tests of variance and multivariate statistics. The chocolates PH16 and TSH1188 presented higher melting point. It was possible to identify a linear correlation between the fat content of the chocolates and the break force parameter. The chocolates showed higher concentration of saturated fatty acids, mainly palmitic acid (16:0) and stearic acid (18:0), followed by the unsaturated fatty acids among which oleic acid (18:1n-9) and linoleic acid (18:6) were predominant. The principal component analysis allowed to discriminate the different varieties of chocolate according to the profile of fatty acids. The thermogravimetric analysis of the samples showed a similar profile. The differences found in the samples of chocolates were attributed to factors such as the fruit maturation process, the cocoa variety and to which they were subjected during processing.

Em que medida o Sistema de Justiça exerce protagonismo no atual processo de conflito institucional do Brasil? A partir de uma análise crítica do conceito de Judicialização da Megapolítica, proposto por Ran Hirschl, discutimos o papel das instituições do Sistema de Justiça na atual crise política brasileira. Mostramos que Hirschl é impreciso acerca do problema da legitimidade no conflito entre poderes e omisso acerca da função que a grande mídia tem de deslocar a legitimidade dos poderes eleitos para as instituições do Sistema de Justiça. Para testar nossa hipótese, comparamos a evolução da aprovação popular dos poderes de Estado com a cobertura que os jornais Folha de S. Paulo, Estado de S. Paulo e o Globo, além do Jornal Nacional dedicaram a elas, de 2014 até 2018, usando os dados da pesquisa do Manchetômetro. Concluímos sugerindo aprimoramentos ao estudo da judicialização e particularmente ao conceito de judicialização da megapolítica.

What is the role of the Justice System in the current process of institutional conflict in Brazil? Through a critical analysis of the concept of the judicialization of megapolitics, proposed by Ran Hirschl, we discuss the role of the Judiciary and Public Prosecutor’s Office in the current Brazilian political crisis. We call attention to the lack of clarity in Hirschl’s analysis about the fundamental problem of legitimacy in the conflict between governmental branches, and then we show how there is strong evidence in the Brazilian case that the mainstream media had the function of shifting legitimacy from the elected powers to the institutions of the System of Justice. To test our hypothesis, we compare the evolution of the popular approval of the main institutions of Brazilian democracy with the coverage that the quality newspapers Folha de S. Paulo, Estado de S. Paulo and Globo, and the television news program Jornal Nacional have dedicated to them from the beginning of 2014 until now, using data from the Manchetometro project. We conclude by suggesting improvements to the study of judicialization and particularly to the concept of the judicialization of the megapolitics.

Dans quelle mesure le système de justice joue-t-il un rôle de premier plan dans le processus actuel de conflit institutionnel au Brésil? À partir d’une analyse critique du concept de Judicialisation Megapolitique, proposée par Ran Hirschl, nous avons discuté du rôle du pouvoir judiciaire et du ministère public dans la crise politique brésilienne actuelle. Nous attirons l’attention sur le manque de clarté dans l’analyse de Hirschl sur le problème fondamental de la légitimité dans le conflit entre les pouvoirs, puis nous montrons comment il existe de fortes preuves dans l’affaire brésilienne selon laquelle les médias traditionnels ont pour fonction de transformer la légitimité des pouvoirs élus en institutions des Système de justice. Afin de prouver notre hypothèse, nous comparons l’évolution de l’approbation populaire des principales institutions de la démocratie brésilienne avec la couverture que les journaux Folha de S. Paulo, État de S. Paulo et Globo et le journal Jornal Nacional leur ont consacré dès le début de 2014 jusqu’à présent, en utilisant les données d’enquête du Manchetomètre. Nous concluons en suggérant quelques améliorations à l’étude de la judiciarisation et en particulier à la notion de judiciatisation de la mégapolitique.

Resumo O modelo alternativo de transtornos da personalidade introduzido na quinta edição do Manual Diagnóstico e Estatístico de Transtornos Mentais oferece um sistema diagnóstico que supostamente deve corresponder ao bem conhecido modelo de diagnóstico categórico dos transtornos da personalidade. O presente estudo tem por objetivo incrementar os conhecimentos sobre a relação entre os traços de personalidade e seus correspondentes tipos de personalidade. Uma amostra brasileira de 1.162 pessoas participou deste estudo. Os resultados indicaram algum nível de continuidade entre os dois modelos quando as variáveis foram consideradas dimensionais. Por outro lado, existe uma falta de evidência científica que justifique a manutenção da abordagem categórica. Recomenda-se a exclusão da abordagem categórica de sistemas diagnósticos de transtornos da personalidade.

Abstract The alternative model of personality disorders introduced in the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders provides a diagnostic system that is expected to correspond to the well-known categorical approach of personality disorder diagnoses. The current study aims to improve knowledge about the relationship between pathological personality traits and their corresponding personality types. A Brazilian sample of 1,162 people took part in this study. The results point to some level of continuity between the two models when the variables were treated as dimensional. Contrariwise, there is a lack of strong scientific evidence to justify the maintenance of the categorical approach. We recommend the exclusion of the categorical approach from personality disorder diagnosis systems.
