Summary: The hydrological region 21 coast of Oaxaca has an active tourist dynamic, it generates significant economic income, and it is considered the fourth most important socioeconomic region in the state of Oaxaca. Similarly, hydrological region 23 coast of Chiapas, has an economy governed by tertiary sector activities and is comprised of the Istmo-Costa and Soconusco socioeconomic regions, the latter is the main region that contributes to the gross domestic product of the state of Chiapas. It should be noted that this socioeconomic growth has required a greater demand for water resources, which has led to water pollution. The deterioration of the area’s water resources is caused by the discharge of wastewater, solid waste and agrochemicals into natural waterways. In order to assess the state of the water resources of hydrological regions 21 and 23, 31 sampling sites were established in the spring of 2021 distributed in the main streams. Each sample was analyzed for chemical properties: pH, electrical conductivity (EC), major anions and cations (CO3 2-, HCO3-, Cl-, SO4 2-, Ca2+, Mg2+, Na+ and K+), total dissolved solids (TDS), percentage of sodium (%Na), adsorption ratio of sodium (RAS), residual sodium carbonate (RSC), permeability index (PI), as well as a hydrochemical evaluation. The waters of the hydrological regions 21 and 23 present as dominant facies the calcium-magnesium bicarbonate, whose possible origin is a consequence of the dilution process of the geological material (water-rock interaction) with low and medium concentrations. With respect to salinity, 45.16% are low salinity waters and 93.55% of the sites have low sodium, therefore, they are considered recommended for irrigation with moderate restrictions.

Resumen: La región hidrológica 21 costa de Oaxaca, presenta una activa dinámica turística, en ella se genera un ingreso económico importante y es considerada la cuarta región socioeconómica más importante del estado de Oaxaca. De igual manera, la Región Hidrológica 23 costa de Chiapas, tiene una economía regida por las actividades del sector terciario, está comprendida por las regiones socioeconómicas Istmo-Costa y Soconusco, ésta última es la principal región que aporta al producto interno bruto del estado de Chiapas. Cabe señalar, que dicho crecimiento socioeconómico ha exigido una mayor demanda del recurso agua, que ha ocasionado la contaminación del mismo. El deterioro de los recursos hídricos de la zona tiene su origen en la descarga de aguas residuales, de desechos sólidos y de agroquímicos a los cauces naturales. Con la finalidad de valorar el estado de los recursos hídricos de las regiones hidrológicas 21 y 23, se establecieron 31 sitios de muestreo en la primavera de 2021 distribuidos en las corrientes principales. En cada muestra se analizaron las propiedades químicas: pH, conductividad eléctrica (CE), aniones y cationes principales (CO3 2-, HCO3-, Cl-, SO4 2-, Ca2+, Mg2+, Na+ y K+), sólidos totales disueltos (STD), porcentaje de sodio (%Na), relación de adsorción de sodio (RAS), carbonato de sodio residual (CSR) e índice de permeabilidad (IP), así como una evaluación hidroquímica. Las aguas de las regiones hidrológicas 21 y 23 presentan como facie dominante la bicarbonatada cálcico-magnésica, cuyo posible origen es consecuencia del proceso de dilución del material geológico (interacción agua-roca) con concentraciones bajas y medias. Con respecto a la salinidad el 45.16% son aguas de salinidad baja y el 93.55% de los sitios tiene baja sodicidad, por lo tanto, se consideran recomendables para el riego con restricciones moderadas.

RESUMEN Introducción y objetivos: Se describen los resultados del Registro Radcong-21 del Grupo de Trabajo de Hemodinámica de la Sociedad Española de Cardiología Pediátrica y Cardiopatías Congénitas con el objetivo de analizar los datos, establecer parámetros de referencia actualizados y compararlos con otros registros. Métodos: Registro multicéntrico, observacional, transversal y retrospectivo de pacientes con cardiopatías congénitas tratados con cateterismo cardiaco en 2021. Cada sala de hemodinámica remite los últimos 100 casos realizados hasta enero de 2022. Análisis descriptivo de variables antropomórficas, procedimientos (agrupados por tipo y riesgo estimado de radiación [RER]), características técnicas y parámetros dosimétricos. Revisión adicional de todos los valores fuera del intervalo de confianza del 95% de la mediana. Resultados: Se analizaron 1.090 procedimientos en 11 salas de hemodinámica de 10 centros hospitalarios. De los pacientes, el 22,8% fueron < 1 año, el 60,7% entre 1 y 18 años, y el 16,4% > 18 años. Los valores del producto dosis-área (PDA)/kg y del PDA/kg/fluoroscopia fueron muy similares independientemente del tipo de sala, al igual que ocurre con la edad, el peso y el grupo RER en la mayor parte de las salas pediátricas. El PDA/kg fue superior en los grupos RER I y III comparado con otros países con registros y programas de mejora activos en este campo (un 78 y un 8,3%, respectivamente). Conclusiones: Se obtienen datos representativos de los parámetros dosimétricos por edad y procedimientos en los cateterismos cardiacos congénitos en España en el año 2021. El PDA/kg es el que tiene menor dispersión en la muestra total. Existen áreas de mejora en comparación con otros países con programas de optimización.

ABSTRACT Introduction and objectives: The results of the Radcong-21 Registry of the Spanish Society of Pediatric Cardiology and Congenital Heart Disease Working Group on Hemodynamics are described to analyze data, establish updated reference parameters, and compare them to other registries. Methods: Retrospective, cross-sectional, observational, multicenter registry of patients with congenital heart disease undergoing cardiac catheterization in 2021. Each cath lab sent the last 100 cases performed prior to January 2022. A descriptive analysis was conducted of anthropomorphic variables, procedural (grouped by type and radiation exposure categories [REC]) and technical characteristics, and dosimetric parameters with additional review of all values outside the 95%CI of the median. Results: A total of 1090 procedures performed in 11 cath lab of 10 hospital centers were analyzed. Age distribution: 22.8% < 1 year, 60.7% between 1-18 years, and 16.4% > 18 years. In dose area product (DAP)/Kg and DAP/Kg/fluoroscopy, the distribution was very similar regardless of the type of cath lab as is the case with most pediatric patients in terms of age, weight, and REC group. The DAP/Kg was higher in the REC I and III groups compared to other countries with registries and improvement programs in this area (78% and 8,3%, respectively). Conclusions: Representative data of dosimetric parameters by age and procedures in congenital cardiac catheterizations were obtained in Spain back in 2021. DAP/Kg is the parameter with the lowest dispersion in the sample. There is room for improvement compared to other countries with optimization programs in this area.

RESUMEN Introducción y objetivos: La Asociación de Cardiología Intervencionista de la Sociedad Española de Cardiología (ACI-SEC) y el Grupo de Trabajo de Hemodinámica de la Sociedad Española de Cardiología Pediátrica y Cardiopatías Congénitas (GTH-SECPCC) presentan su informe anual de actividad hemodinámica en cardiopatías congénitas correspondiente al año 2021. Métodos: Se invitó a participar a los centros españoles con laboratorio de hemodinámica y actividad intervencionista en cardiopatías congénitas. La recogida de datos fue telemática y su análisis lo efectuó una empresa externa junto con miembros de la ACI-SEC y el GTH-SECPCC. Resultados: Participaron 16 centros (15 públicos y 1 privado) que acumulan 34 salas de hemodinámica con actividad en cardiopatías congénitas, 7 (20,5%) de ellas con dedicación exclusiva a pacientes pediátricos. Se registraron 1.094 estudios diagnósticos (5,1% más que en 2020) y 1.553 cateterismos intervencionistas (6,4% más que en 2020). Las técnicas más frecuentes fueron el cierre de comunicación interauricular (336 casos), la angioplastia de ramas pulmonares (231 casos) y el cierre de ductus (228 casos). Respecto al año anterior se incrementaron significativamente las técnicas de implantación de válvulas percutáneas (29,3%), cierre de ductus (20,6%) y angioplastia aórtica (12,8%). La tasa de éxito en los procedimientos intervencionistas fue del 95%, con una tasa de complicaciones mayores del 2,7% y una mortalidad intrahospitalaria del 0,2%. Conclusiones: Este trabajo es la segunda publicación del Registro Español de Intervencionismo en Cardiopatías Congénitas. Se ha constatado un aumento significativo de los procedimientos diagnósticos y terapéuticos, destacando el incremento en la implantación de válvulas percutáneas, el cierre de ductus y la angioplastia aórtica. La mayoría de las técnicas intervencionistas siguen demostrando excelentes datos de seguridad y eficacia.

ABSTRACT Introduction and objectives: This is the 2021 annual activity report from the Interventional Cardiology Association of the Spanish Society of Cardiology (ACI-SEC), and the Interventional Working Group of the Spanish Society of Pediatric Cardiology (GTH-SECPCC). Methods: All Spanish centers with cath lab capabilities and interventional activity in congenital heart diseases were invited to participate. Data were collected online, analyzed by an external company, and ACI-SEC and GTH-SECPCC members. Results: A total of 16 centers participated 15 public and 1 private including 34 cath labs with experience in congenital heart diseases, 7 of them (20.5%) exclusively dedicated to pediatric patients. A total of 1094 diagnostic studies (4.5% more than 2020) and 1553 interventional catheterizations (5.8% more than 2020) were registered. The most common procedures were atrial septal defect closure (336 cases), pulmonary branch artery angioplasty (231 cases), and percutaneous closure of the patent ductus arteriosus (228 cases). Interventional procedures were considered successful in 95% of the cases with rates of major procedural complication and in-hospital mortality of 2.7% and 0.2%, respectively. Conclusions: This is the second Spanish Cardiac Catheterization in Congenital Heart Diseases Registry report. A significant increase of diagnostic and interventional procedures was reported with a special increase of percutaneous valve implantation, ductus arteriosus closure, and aortic angioplasty. Most interventional techniques continue to demonstrate excellent safety and efficacy outcomes.

Resumen Introducción: Es un error común pensar que los dientes anteroinferiores, por lo general, tienen un solo conducto radicular con una sola raíz. Sin embargo, un estudio realizado por Vertucci et. al., (1974), mostraron una alta prevalencia (13%) de dos conductos radiculares en los dientes anteroinferiores, lo que estimuló más investigaciones. Objetivo: El propósito de este estudio fue determinar la prevalencia del segundo conducto radicular en los dientes anteroinferiores en una población nicaragüense, estos fueron detectados por medio de tomografía computadorizada (Cone Beam). Materiales y Métodos: En el estudio se analizaron 293 piezas dentales, de canino a canino de la arcada inferior. Para realizar el análisis se utilizó el software libre Radiant DICOM Viewer 2021.2.2, se realizaron cortes sagitales, axiales y coronales para ver la prevalencia del segundo conducto radicular. Resultados: De las 293 piezas dentarias analizadas se encontró que 259 presentaban un solo conducto que correspondía al 88.4% y 34 dientes presentaban dos conductos que correspondían al 11.6%. De acuerdo con el análisis tomográfico, se encontró que en los cortes axiales y sagitales fue donde se observó la presencia del segundo conducto. Con respecto a la presencia del segundo conducto de acuerdo al tercio del canal radicular se identificó que la mayoría se presentó en el tercio medio (52.94%), seguido por coronal (29.41%) y por último el tercio apical (17.65%). De acuerdo con la clasificación de Vertucci se encontró que se presenta un mayor porcentaje del tipo I con 88.40%, seguido por el tipo III con 4.44%, después el tipo V con 3.41%, y el tipo II con 2.39%. El de menor porcentaje fue el tipo VI con 1.37%, mientras que, en las piezas analizadas, no se encontraron los tipos IV, VII y VIII. Conclusión: Basados en los resultados obtenidos en este estudio, la prevalencia de un segundo conducto en dientes anteroinferiores fue de 11.6%.

Title The prevalence of a second root canal in mandibular anterior teeth using Cone Beam Computed Tomography. Abstract Introduction: It is a common misconception that the mandibular anterior teeth usually have a single root canal with a single root. However, a study by Vertucci et. al., (1974), showed a high prevalence (13%) of two root canals in the lower anterior teeth, which stimulated further investigations. Objective: The purpose of this study was to determine the prevalence of the second root canal in the mandibular anterior teeth in a Nicaraguan population, these were detected by means of computed tomography (Cone Beam). Materials and methods: In the study, 293 teeth were analyzed, from canine to canine of the mandibular teeth. To perform the analysis, the free software Radiant DICOM Viewer 2021.2.2 was used, sagittal, axial and coronal views were made to see the prevalence of the second root canal. Results: Of the 293 teeth analyzed, it was found that 259 had a single root canal corresponding to 88.4%, and 34 teeth had two root canals corresponding to 11.6%. According to the tomographic analysis, it was found that the presence of the second root canal was observed only in the axial and sagittal views. Regarding the presence of the second canal according to the third of the root canal, it was identified that the majority presented in the middle third (52.94%), followed by coronal (29.41%) and finally the apical third (17.65%). According to the Vertucci classification, it was found that there is a higher percentage of type I with 88.40%, followed by type III with 4.44%, then type V with 3.41%, and type II with 2.39%. The one with the lowest percentage was type VI with 1.37%, while in the pieces analyzed, types IV, VII and VIII were not found. Conclusion: Based on the results obtained in this study, the prevalence of a second root canal in lower anterior teeth was 11.6%.

Abstract The pandemic generated multiple biopsychosocial challenges that significantly impacted the health of trans women. The aim of this study is to characterize emotional states and resilience in trans women in Colombia and their relationship to COVID-19 adherence behaviors and prevention behaviors during lockdown. The study employed a cross-sectional descriptive correlational design, with an anonymous online questionnaire that was given to 100 trans women in five cities in Colombia. During the pandemic, more than half of trans women experienced negative emotional states such as depression, anxiety and somatization, and some positive ones such as resilience and personal growth. The latter two contribute to reinforcing adherence and prevention behaviors. This study demonstrates a clear need to give priority and recognition to trans women and their health needs, in order to protect their rights, their well-being, and their health.

Resumen La pandemia generó múltiples desafíos biopsicosociales que impactaron significativamente en la salud de las mujeres trans. El objetivo del presente estudio fue caracterizar los estados emocionales y resiliencia en mujeres trans de Colombia y su relación con las conductas de adherencia y prevención a la COVID-19 durante el confinamiento. Se realizó un diseño trasversal correlacional descriptivo, administrando un cuestionario online anónimo a 100 mujeres trans en cinco ciudades de Colombia. Se encontró que más de la mitad de las mujeres trans han experimentado estados emocionales negativos como depresión, ansiedad y somatización y algunos positivos como resiliencia y crecimiento personal. Los dos últimos contribuyen a fortalecer las conductas de adherencia y prevención de la COVID-19. Es prioritario dar reconocimiento a las mujeres trans y a sus necesidades de salud, desde diversos frentes de acción con el fin de proteger sus derechos y su salud integral.

Resumo A pandemia gerou múltiplos desafios biopsicossociais que impactaram significativamente a saúde das mulheres trans. O objetivo deste estudo é caracterizar estados emocionais e resiliência em mulheres trans da Colômbia e sua relação com os comportamentos de aderência à COVID-19 e os comportamentos de prevenção durante o confinamento. Foi realizado um desenho descritivo, transversal e correlacional, foi administrado um questionário online anônimo a 100 mulheres trans em cinco cidades da Colômbia. Durante a pandemia, mais da metade das mulheres trans experimentaram estados emocionais negativos como depressão, ansiedade e somatização, e alguns positivos como a resiliência e o crescimento pessoal. Estes dois últimos contribuem para reforçar os comportamentos de adesão e de prevenção. Em situações como a pandemia, é prioritário dar reconhecimento às mulheres trans e às suas necessidades de saúde, a fim de proteger os seus direitos e a sua saúde integral.

Resumen Córdoba es uno de los departamentos de la región Caribe colombiana con mayor atención en el estudio de los mamíferos, lo cual ha permitido ampliar el listado previo que incluía 117 especies. Gracias al desarrollo de nuevos trabajos de campo en zonas no exploradas, revisión de colecciones biológicas y los cambios taxonómicos, la diversidad de mamíferos de Córdoba se incrementa a 141 especies, adicionando el registro de 24 nuevas especies. Se incluyen 14 cambios taxonómicos y la extensión de distribución geográfica de 42 especies dentro del departamento. Los órdenes Chiroptera, Cetacea, Rodentia y Didelphiomorphia presentan la mayor cantidad de cambios y adiciones.

Abstract Córdoba is one of the departments of the Colombian Caribbean region with the greatest attention for the study of mammals, which has allowed the previous list of 117 species of mammals to be increased. Thanks to new fieldwork in previously unexplored areas, and to taxonomic changes, the diversity of mammals in Córdoba expands to 141 species, adding 24 new species, including 14 taxonomic changes and the geographical distribution extension of 42 species within the department. The Chiroptera, Cetacea, Rodentia and Didelphiomorphia orders have the greatest changes and additions.

BACKGROUND Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a virus of zoonotic origin that can bind to ACE2 receptors on the cells of many wild and domestic mammals. Studies have shown that the virus can circulate among animals mutate, lead to animal-to-human zoonotic jump, and further onward spread between humans. Infection in pets is unusual, and there are few human-to-pet transmission reports worldwide. OBJECTIVE To describe the SARS-CoV-2 infection in a domestic animal in Córdoba, Colombian Caribbean region. METHODS A cross-sectional molecular surveillance study was carried out, oral and rectal swabs were taken from cats and dogs living with people diagnosed with coronavirus disease 2019 (COVID-19). RESULTS SARS-CoV-2 was found in a cat living with a person with COVID-19. Genome sequencing showed that the B.1.111 lineage caused the infection in the cat. The owner’s sample could not be sequenced. The lineage is predominant in Colombia, and this variant is characterised by the presence of the D614D and Q57H mutation. CONCLUSION The present work is the first report of an infected cat with SARS-CoV-2 with whole-genome sequencing in Colombia. It highlights the importance of detecting SARS-CoV-2 mutations that could promote the transmissibility of this new coronavirus. There is still a significant information gap on human-to-cat-to-human infection; we encourage self-isolation measures between COVID-19 patients and companion animals. The findings of this study give a preliminary view of the current panorama of SARS-CoV-2 infection in animals in Colombia.

Resumen Introducción: San Juan de Dios nace en Portugal en 1495. En el Real Monasterio de Santa María de Guadalupe, en Cáceres, se forma en el cuidado de los enfermos. Regresa a Granada y comienza a dedicarse al cuidado de los pobres sin hogar y los enfermos, instituyendo un hospital para tales fines. Muere el 8 de marzo de 1550. El primer sello postal que conmemora a San Juan de Dios es emitido en Portugal en 1949 con ocasión del IV Centenario de su muerte. Metodología: Para llevar a cabo el análisis del “TU SELLO”, se ha utilizado el método iconográfico-iconológico de Panofsky. Resultados: Se presenta el “TU SELLO” emitido en conmemoración del “430 Aniversario de la Orden de San Juan de Dios en Palencia”. El diseño se basa en el grupo escultórico creado por Óscar Aragón, que se inauguró el 4 de diciembre de 2017, con motivo del 425 aniversario de la presencia de la Orden en Palencia. En el sello aparece un arco que simboliza la puerta del hospital de San Juan de Dios de Palencia y, por otro lado, dos figuras que representan a San Juan de Dios abrazando y ayudando, en un gesto de protección, a otra figura que es una representación alegórica de la enfermedad. Discusión: La utilización de los sellos personalizados es una manera de dar a conocer, a través de la filatelia, acontecimientos, personajes, hechos relevantes o efemérides que de otra manera no tendrían su reflejo en el sello postal, dada la limitada y exigente política de los servicios filatélicos de los distintos países para incluir en sus emisiones todo lo que de relevante puede ocurrir a lo largo de un año. Este ha sido, y no otro, el objetivo de emitir este “TU SELLO” homenaje al 430 Aniversario de la Orden de San Juan de Dios en Palencia.

Abstract Introduction: St. Juan de Dios was born in Portugal in 1495. In the Royal Monastery of Santa María de Guadalupe, in Cáceres, he was trained in the care of the sick. He returns to Granada and begins to dedicate himself to the care of the poor homeless and the sick, establishing a hospital for such purposes. He died on March 8, 1550. The first postage stamp commemorating St. Juan de Dios was issued in Portugal in 1949 on the IV Centenary of his death. Methodology: To carry out the analysis of “ TU SELLO “, the iconographic-iconological method of Panofsky has been used. Results: The “TU SELLO” issued in commemoration of the “430th Anniversary of the Order of St. Juan de Dios in Palencia” is presented. The design is based on the sculptural group created by Óscar Aragón, which was inaugurated on December 4, 2017, on the 425th anniversary of the presence of the Order in Palencia. In the stamp appears an arch that symbolizes the door of the hospital of St. Juan de Dios in Palencia and, on the other side, two figures that represent St. Juan de Dios, embracing and helping, in a gesture of protection, another figure that is an allegorical representation of the disease. Discussion: The use of personalized stamps is a way of making known, through philately, events, characters, relevant facts, or ephemeris that otherwise would not be reflected in the postage stamp, given the limited and demanding policy of philatelist services from different countries to include in their issues every relevant event that may occur over the course of a year. This has been, and no other, the objective of issuing this “TU SELLO” tribute to the 430th Anniversary of the Order of St. Juan de Dios in Palencia.

Resumen Objetivo: la finalidad de este estudio fue evaluar las asociaciones entre los perfiles de los genes NUDT15 y TPMT con los efectos adversos del tratamiento de mantenimiento en pacientes pediátricos con Leucemia Linfoblástica Aguda atendidos en un hospital de referencia durante el 2017. Métodos: Este fue un estudio observacional analítico, de corte longitudinal en el que los genotipos de los genes de interés fueron determinados mediante PCR de discriminación alélica con sondas TaqMan® en pacientes que estaban recibiendo quimioterapia de mantenimiento en la Unidad de Oncohematología Pediátrica durante el 2017. Los datos clínicos y sociodemográficos correspondientes a los primeros 6 meses de sus tratamientos de mantenimiento fueron colectados, y se evaluó la correlación entre los genotipos identificados y el desarrollo de efectos secundarios en estos pacientes. Resultados: setenta pacientes fueron incluidos en el estudio, de estos, los análisis genéticos para NUDT15 y TPMT (rs1800462 and rs1800460) fueron realizados en 68 pacientes, en tanto que para el polimorfismo rs1142345 se logró la tipificación en 42 pacientes. 4/68 pacientes fueron heterocigotos para NUDT15 y el mismo número de pacientes fueron heterocigotos para rs1800462 and rs1142345, mientras que para rs1800460, 6 pacientes heterocigotos fueron identificados. No se identificaron asociaciones estadísticamente significantes entre las variants genéticas y los resultados clínicos de interés. Conclusiones: Estos hallazgos resaltan la importancia de realizar estudios de este tipo con un mayor número de sujetos de estudio, así como plantean la necesidad de evaluar otras variantes genéticas que podrían tener algún impacto en el desarrollo de efectos secundarios durante la quimioterapia de mantenimiento.

Abstract Objective: This study aimed to correlate the genetic profile of the NUDT15 and TPMT genes with the side effects of the treatment of pediatric patients with acute lymphoid leukemia who were undergoing maintenance therapy at a tertiary care hospital in 2017. Methods: This was an analytical, longitudinal, observational study in which the genotypes of the genes of interest were determined by PCR allelic discrimination with TaqMan® probes in patients receiving chemotherapy during the maintenance phase in the Pediatric Hematology and Oncology Unit in 2017. Sociodemographic and clinical data corresponding to the first six months of their maintenance chemotherapy were collected, and the correlation between the genotypes obtained and the development of side effects during the maintenance phase of chemotherapy in these patients was evaluated. Results: Seventy pediatric patients were included in the study. Genetic analyses were carried out of these for NUDT15 and TPMT (rs1800462 and rs1800460) on 68 patients, while for the rs1142345 polymorphism, typing was achieved in 42 patients. 4/68 patients were heterozygous for NUDT15, and the same number of patients were heterozygous for rs1800462 and rs1142345, while for rs1800460, 6 heterozygous patients were identified. No statistically significant association was identified between the genetic variants and the outcomes of interest. Conclusion: Studies with a larger population size are needed and the evaluation of other genetic variants that may influence the development of side effects during maintenance chemotherapy.

Resumen La ruptura uterina es una complicación obstétrica poco frecuente con una alta morbilidad y mortalidad materna e infantil. El diagnóstico precoz y el tratamiento inmediato son factores pronósticos importantes tanto para la madre como para el feto. Se presenta el caso de una paciente multigestante, con alto riesgo por diabetes gestacional compensada y déficit de proteína S sin manejo, con embarazo a término y adecuado control prenatal, que ingresa hemodinámicamente estable, con cambios cervicales iniciales, feto en presentación longitudinal, cefálico con monitoreo fetal categoría ACOG 1 para conducción del trabajo de parto, y presenta ruptura uterina espontánea, dando lugar a un hemoperitoneo materno y sufrimiento fetal agudo con posterior tratamiento quirúrgico de urgencia mediante una cesárea e histerectomía, por atonía uterina sin control del sangrado con maniobras farmacológicas. Este caso es muy significativo por la falta de factores de riesgo y su presentación clínica atípica, tanto en síntomas y signos como en la ubicación de la ruptura. MÉD. UIS.2021;34(1): 107-12.

Abstract Uterine rupture is a rare obstetric complication with high maternal and infant morbidity and mortality. Early diagnosis and immediate treatment are important prognostic factors for both mother and fetus. The study presents the case of a multigravida patient, with high risk for compensated gestational diabetes, and uncontrolled protein S deficiency. The patient, who was full-term and had adequate prenatal control, was admitted hemodynamically stable, with initial cervical changes, fetus in longitudinal presentation, andcephalic with monitoring category ACOG 1. The patient spontaneous uterine rupture, which leads to maternal hemoperitoneum and acute fetal distress. Cesarean section and hysterectomy were performed as emergency surgical treatments due to uterine atony without bleeding control with pharmacological maneuvers. This case is great significance due to the lack of risk factors and the atypical clinical presentation, evidenced in the signs and symptoms and the rupture’s location. MÉD.UIS.2021;34(1): 107-12.

Resumen La Formación Tibasosa es la principal fuente de calizas en Boyacá. Esta unidad se depositó entre el Valanginiano-Albiano según el contenido fósil en la Cuenca Cordillera Oriental, registrando la primera incursión del mar Cretácico en Firavitoba. Los análisis de facies y estratigráficos de una sucesión carbonática-siliciclástica de ~12 m de espesor de la Formación Tibasosa Superior indican sistemas mareales/carbonáticos. Diez facies/microfacies se agruparon en dos asociaciones de facies (AFs): AF1, planicie mareal consiste en areniscas/limolitas laminadas y floatstones con un solo tipo de organismo dominante (conchas de bivalvos); y AF2 comprende rudstones, floatstones, packstones y wackstones, representando una plataforma carbonática. La descripción petrográfica determinó la textura/génesis y la secuencia diagenética con características de la eodiagénesis, mesodiagénesis y telodiagénesis sugiriendo un origen primario de estos carbonatos. El análisis mediante catodoluminiscencia (CL), espectrometría de energía dispersa (EDS) y microscopía electrónica de barrido (MEB) permitió identificar diferencias composicionales, fases de cementación y características morfológicas en diferentes procesos como micritización, neomorfismo, porosidad, piritización, compactación, cementación, fracturamiento. y meteorización. La interpretación de facies/microfacies indicó una deposición en una plataforma poco profunda con condiciones hidráulicas fluctuantes, aguas cálidas y eventos de tormentas/tsunamis que fragmentaron los bioclastos. Este sistema deposicional en la Cuenca Cordillera Oriental durante el Cretácico indica un gran evento transgresivo que inundó cientos de kilómetros, revelando conexión con el Océano Pacífico antes del levantamiento Andino. Los principales eventos diagenéticos corresponden a la micritización, cementación de calcita y compactación mecánica/química como resultado de la actividad microbiana, disolución, precipitación en la zona vadosa/freática y diagénesis de enterramiento. La secuencia diagenética revela la incidencia de procesos marinos y meteóricos que redujeron la porosidad y corroboran la actividad microbiana en el carbonato precipitado. Esta nueva interpretación permite comprender las plataformas carbonatadas en la Cuenca Cordillera Oriental para futuras correlaciones del mar Cretácico en Colombia.

Abstract The Tibasosa Formation is the main source of limestones in Boyacá. This unit corresponds to a Valanginian-Albian age according to the fossil content in the Eastern Cordillera Basin, recording the first incursion of the Cretaceous sea in Firavitoba. Outcrop-based facies and stratigraphic analyzes of the ~12 m-thick siliciclastic-carbonate succession of the uppermost Tibasosa Formation indicate tidal and carbonate systems. Ten facies/microfacies are grouped into two facies associations (FAs): FA1, tidal flat deposits consist of laminated sandstones/siltstones and floatstones with a single organism dominance (bivalve shells); and FA2 comprises fossiliferous rudstones, floatstones, packstones, and wackstones, representing a carbonate platform. The petrographic description determined rock textures/genesis and the diagenetic sequence with features of the eodiagenesis, mesodiagenesis, and telodiagenesis suggesting a primary origin of these carbonates. The analysis using cathodoluminescence (CL), energy disperse spectrometry (EDS), and scanning electron microscopy (SEM) allowed identify compositional differences, cementation phases, and morphological features in different processes as micritization, neomorphism, porosity, pyritization, compaction, cementation, fracturing, and weathering. The interpretation of facies and microfacies indicated a deposition mainly in a shallow platform with variation in the hydraulic conditions, warm waters, and episodic events of storms/tsunamis that fragmented the bioclasts. A shallow marine system in the Eastern Cordillera Basin during Cretaceous indicates a large transgressive event that flooded hundreds of kilometers, being a link with the Pacific Ocean before the Andes uplift. The main diagenetic events correspond to micritization, cementation of calcite, and mechanical/chemical compaction as a result of microbial activity, dissolution, precipitation in the vadose/phreatic zone, and burial diagenesis. The diagenetic sequence events reveal the incidence of marine and meteoric process that reduced porosity and attest to the microbial activity in carbonate precipitated. This new interpretation allows the understanding of carbonate platforms in the Eastern Cordillera Basin for future correlations of the Cretaceous sea in Colombia.

Resumen Antecedentes: Las leucemias son la principal causa de cáncer infantil. Los avances en el tratamiento han llevado a los pacientes a una supervivencia global hasta del 80%. Cerca del 10% de los niños con cáncer tienen toxicidad cardiovascular sintomática, la dosis acumulada de antraciclinas es un factor de riesgo para afección cardíaca. Objetivo: Describir la frecuencia de afectación cardíaca temprana en niños con leucemias agudas que recibieron tratamiento antineoplásico. Métodos: Estudio prospectivo observacional, de pacientes <18 años con diagnóstico confirmado de leucemia aguda. Fueron evaluados con electrocardiograma, ecocardiograma bidimensional y biomarcadores séricos en diferentes momentos durante el tratamiento. Resultados: Se evaluaron 94 pacientes con leucemia linfoide aguda y 18 con leucemia mieloide aguda. 20 pacientes (17.9%) tuvieron disfunción cardiaca de inicio temprano. Se observaron diferencias estadísticamente significativas, después de recibir 150 mg/m2 de antraciclinas, entre la evaluación del ecocardiograma basal y evaluaciones posteriores de la fracción de eyección ventricular izquierda por Teicholz p 0.05, fracción de eyección ventricular izquierda por Simpson p 0.018 y la deformación longitudinal global p 0.004. No se encontraron alteraciones en los niveles séricos de las troponinas y péptido natriurético cerebral. Conclusiones: La disfunción cardíaca relacionada con quimioterapia estuvo directamente relacionada con las dosis acumuladas de antraciclinas. En este estudio el uso del ecocardiograma como método de seguimiento permitió identificar factores predictores de riesgo para disfunción cardiaca temprana.

Abstract Background: Acute leukemias are the most frequent malignancies in children. Advances in treatment have improved the overall survival to 80%. Almost 10% of children with cancer develop clinical cardiac toxicity. Total anthracycline cumulative dose is a risk factor for early-onset cardiotoxicity. Objective: To describe the incidence of early-onset cardiotoxicity in children with acute leukemia treated with chemotherapy. Methods: A prospective descriptive study of patients >1 y and <18 years diagnosed with acute leukemia. Assessed with electrocardiograma, echocardiography, and blood biomarkers at diagnosis and during the follow-up. Results: 94 patients with acute lymphoblastic leukemia and 18 with acute myeloid leukemia were included. 20 patients (17.9%) developed early-onset cardiotoxicity. Statistically significant data was seen after anthracycline dose >150 mg/m2, between the first echocardiographic evaluation and posterior analyses in the left ventricular fraction ejection with Teicholz p 0.05, Simpson p 0.018 and GLS p 0.004. In this study, there was no relation between blood biomarkers and cardiotoxicity. Conclusions: Cancer therapeutic-related cardiac dysfunction is related to anthracycline cumulative dose. In this study, echocardiographic follow-up was useful to predict risk factors for early cardiac dysfunction.

Resumen Introducción: Las personas con diálisis de mantenimiento se consideran una población en alto riesgo de infección por SARS-CoV-2, complicaciones y muerte. La periodicidad de la diálisis, la organización y la demanda en las unidades de diálisis y las limitaciones de alfabetización en salud poblacional limitan el cumplimiento del aislamiento y el distanciamiento social. Objetivo: Desarrollar, mediante un consenso de expertos, recomendaciones informadas en evidencia para la prevención, el diagnóstico y el manejo de la infección por SARS-CoV-2/COVID-19 en pacientes con enfermedad renal crónica. Materiales y métodos: Se realizó una revisión rápida de literatura en Pubmed, Embase y sociedades científicas. La calidad de evidencia fue evaluada según el tipo de estudio incluido. El acuerdo se definió para cada recomendación con umbral de al menos 70% de aprobación. La fuerza de las recomendaciones fue graduada como fuerte o débil. Resultados: El colectivo fue consultado entre el 17 y 19 de mayo de 2020. Se obtuvo respuesta de 44 expertos clínicos que declararon conflicto de interés previo a la consulta. El acuerdo de las recomendaciones estuvo entre 70,5 y 100%. Se presentan las recomendaciones de un colectivo experto para la prevención, el diagnóstico y el manejo de infección por SARS-CoV-2/COVID-19 en pacientes con enfermedad renal crónica. Conclusión: Debido a la reciente aparición de la infección por SARS-CoV-2 y las incertidumbres respecto a la prevención, el diagnóstico y el manejo, las recomendaciones presentadas se conciben como un estándar colombiano que permita garantizar un cuidado centrado en las personas con enfermedad renal crónica y la protección de los profesionales de la salud.

Abstract Introduction: People with chronic dialysis are considered a population at high risk of SARS CoV2 infection and its derived complications and death. The need to go to strict dialysis schedules, the high demand in the kidney facilities and the difficulties derived from the time and space organization in the rooms in the face to the pandemic added to the difficult learning, teaching and adapting new protocols manifest the needed of standard recommendation according to this problem in people who couldn't have an ideal isolation. Objective: Develop through an expert consensus, evidence-informed recommendations for the prevention, diagnosis, and management of SARS-CoV-2/COVID-19 infection in patients with chronic kidney disease on Dialysis. Materials and methods: We carried out a quick literature review, PubMed, Embase and scientific societies were consulted. The quality of the evidence was considered according to the type of study included. The agreement threshold defined for each recommendation was > 70% approval among experts. The strength of the recommendations was rated as strong or weak. Results: Between May 17 and 19, 2020, was conformed a team of 44 clinical experts who declared their interest conflict prior to the consultation. The agreement of the recommendations was between 70.5% and 100%. The recommendations were separated in prevention, early identification, and diagnostic, isolation in hemodialysis facilities and peritoneal dialysis, and team protection. Ethical considerations also were included. Conclusion: Due to the recent appearance of SARS-CoV-2 infection and the uncertainties regarding prevention, diagnosis and management, the recommendations presented are conceived as a Colombian standard that allows guaranteeing focused care for people with chronic kidney disease and the protection of health team.

Abstract Introduction: Acute kidney injury is a frequent complication in patients with COVID-19 and its occurrence is a potential indicator of multi-organ dysfunction and disease severity. Objective: Develop, through an expert consensus, evidence-based recommendations for the prevention, diagnosis, and management of acute kidney injury in patients with SARS CoV2 / COVID-19 infection. Materials and methods: Based on a rapid systematic review in Embase and Pubmed databases and documents from scientific societies, we made preliminary recommendations and consulted with an expert group through an online tool. Then we defined agreement after at least 70 % consensus approval. Quality evidence was evaluated according to the type of document included. The strength of the recommendations was graded as strong or weak. Results: Fifty clinical experts declared their conflict of interest; the consultation took place between May 2 and 29, 2020. The range of agreement ranged from 75.5 % to 100 %. Recommendations for prevention, diagnosis and management of acute kidney injury in patients with SARS CoV2 infection are presented. Conclusions: Although the good quality information available regarding acute kidney injury in patients with COVID-19 is scarce, the recommendations of clinical experts will guide clinical decision-making and strategies around patients with this complication, guaranteeing care focused on the people, with high quality standards, and the generation of safety, health and wellness policies for multidisciplinary care teams.
