The objective of the present case report was to describe the aesthetic and functional rehabilitation of the upper anterior incisors of a patient with bruxism. A 23-year-old male patient, with bruxism, sought treatment for his upper anterior teeth due to diastemas and reduced lengths of the upper central incisors. The gingivoplasty and osteotomy were performed in the region of the upper central incisors to improve the positions of the gingival contours and increase the length of these teeth. After 90 days, teeth whitening was performed. After 1 month the composite resin veneers were made in the upper central incisors. During treatment, the needs and perspectives of the patient were considered to achieve clinical success. The treatments were finalized, and the final result was satisfactory. Therefore, an integrated treatment plan proved to be extremely important, especially when the patient is adequately informed of the available treatment options and collaborates with the treatment.

O objetivo do presente relato de caso clínico foi descrever a reabilitação estética e funcional dos incisivos anteriores superiores de um paciente com bruxismo. Paciente do gênero masculino, 23 anos de idade, bruxoma, procurou tratamento para os dentes anteriores superiores devido aos diastemas dentários e também por causa dos comprimentos reduzidos dos incisivos centrais superiores. Para isso, foi realizada a gengivoplastia e osteotomia na região dos incisivos centrais superiores com o objetivo de melhorar as posições dos contornos gengivais e aumentar o comprimento desses dentes. Após 90 dias, o clareamento dentário foi realizado. Depois de 1 mês as facetas de resina composta foram confeccionadas nos incisivos centrais superiores. Durante o tratamento as necessidades e perspectivas do paciente foram levadas em consideração para alcançar o sucesso clínico. Após a conclusão dos procedimentos, o resultado final foi satisfatório. Portanto, um plano de tratamento integrado se mostrou de extrema valia, principalmente quando o paciente é devidamente esclarecido das opções disponíveis de tratamento e colabora com o mesmo.

Abstract The aim of this study was to investigate the association between oral health status and central obesity (CO) in Brazilian independent-living elderly. A cross-sectional study was carried out in a sample of 489 elderly, who were participants of the Study on Aging and Longevity, in Londrina, state of Parana. The number of natural teeth and use of prostheses were evaluated according to the World Health Organization criteria. The presence of CO was assessed using measures of waist circumference (WC) and waist-hip ratio (WHR). Information concerning sociodemographic profile and some systemic conditions was also collected. Data were analyzed using stepwise logistic regression, α=5%. According to WC and WHR measures, the prevalence of central obesity was 79.3% and 76.1%, respectively. CO according to WC was not associated with oral status. Considering the WHR measure, the following oral conditions were associated to CO: having fewer natural teeth (OR = 2.61; 95%CI = 1.17–5.80), being edentulous and wearing both upper and lower complete dentures (OR = 2.34; 95%CI = 1.11–4.93), and being edentulous wearing only the upper complete denture (OR = 2.64; 95%CI = 1.01–6.95). Traditional risk factors for CO such as gender, dyslipidemia, hypertension and diabetes were associated with both measures. A poor oral health due to extensive tooth loss, whether partial or complete, even if rehabilitated by removable prostheses, may be considered a good predictor of CO in Brazilian independent-living elderly.

A clínica ampliada (CA) representa uma estratégia da Política Nacional de Humanização (PNH) para reflexão sobre o cuidado com o usuário do Sistema Único de Saúde (SUS). Impulsionados pelo Programa Pró-Saúde e pela PNH, docentes do curso de Odontologia da UEM promoveram uma reestruturação do atendimento clínico ofertado, resultando na implantação da CA. Este trabalho relata a experiência vivenciada na implantação dessa clínica, que visa a promover a interação e o vínculo do aluno com a comunidade e a equipe multiprofissional de saúde, de modo a obter o cuidado integral e humanizado. A CA está estruturada para acolher o usuário em um ambiente multiprofissional, direcionando-o dentro de um fluxograma até a sua alta, promovendo a autonomia na manutenção de sua saúde bucal. A CA tem dado subsídios para melhorar a visão do cuidado integral do usuário da clínica odontológica da UEM, fortalecendo a formação acadêmica e o cuidado prestado à comunidade. Nota-se que a construção coletiva agrega muito valor às ações, mas é um processo lento e depende da maturidade dos envolvidos.

Expanded clinical care is a strategy under the National Policy for Humanized Care for reflection on care for users of the Unified National Health System. Under the impetus of the Pró-Saúde program and the National Policy for Humanized Care, faculty from the School of Dentistry at the State University in Maringá (UEM) conducted a restructuring of clinical care, resulting in the implementation of expanded care. This study reports on the experience with the implementation of such expanded clinical care, aimed at promoting interaction between students, community, and the multi-professional health team, so as to obtain comprehensive and humanized care. Expanded care is structured to receive users in a multi-professional setting, directing them within a flowchart all the way to their discharge and fostering autonomy in the maintenance of their oral health. Expanded care has provided backing for improving the vision of comprehensive care for users of the UEM dental clinic, strengthening academic training and community care. The collective construction adds considerable value to the activities, but it is a slow process that requires maturity on the part of participants.
