Elevated serum IL-6 levels predict treatment interruption in patients with moderate to severe psoriasis: a 6-year real-world cohort study

Abstract Background: Real-world, primary data on the treatment of psoriasis are scarce, especially concerning the role of soluble biomarkers as outcome predictors. Objective: The authors evaluated the utility of Th1/Th17 serum cytokines along with clinical characteristics as predictors of drug survival in the treatment of psoriasis. Methods: The authors consecutively included participants with moderate to severe psoriasis who were followed up for 6 years. Baseline interferon-α, tumor necrosis factor-α, and inter-leukin (IL)-2, IL-4, IL-6, IL-10, and IL-17A were measured using a cytometric bead array; clinical data were assessed. The authors calculated hazard ratios (HRs) for drug survival using a Cox proportional hazards model. Results: The authors included 262 patients, most of whom used systemic immunosuppressants or biologics. In the multivariate model, poor quality of life measured by the Dermatology Life Quality Index (HR = 1.04; 95% CI 1.01–1.07; p = 0.012) and elevated baseline IL-6 (HR = 1.99; 95% CI 1.29–3.08; p = 0.002) were associated with treatment interruption. Study limitations: The main limitation of any cohort study is the presence of confounders that could not be detected in clinical evaluation. Conclusions: Poor quality of life and elevated baseline serum IL-6 level predicted treatment interruption in patients with moderate to severe psoriasis. Although IL-6 is not the most important mediator of the inflammatory pathway in the skin environment, it is an interesting biomarker candidate for predicting psoriasis treatment response.

Abstract This study evaluated the effect of increasing the nutritional plane in finishing diets on meat quality traits of culled adult goats. Forty-one crossbred does were divided into four groups: baseline diet formulated to meet 100% of the nutritional requirements (BD); BD + 30% (BD30); + 60% (BD60); + 90% (BD90) of the nutritional level of BD. The goats received the experimental diets for 28 days. The protein profile of the longissimus lumborum muscle was evaluated using 1D SDS–PAGE and mass spectrometry. Cholesterol and albumin concentrations were highest in the BD group. There were no alterations in loin proximate composition or tissue composition. However, there was a significant reduction in the PUFA content in groups BD60 and BD90 compared with the group fed the baseline diet. Of 37 protein bands identified, 35 showed lower intensities in group BD90 than in BD and 22 bands showed lesser expression in groups BD60 and BD90 than in BD. The multivariate model identified 14 band proteins involved in group-diet segregation. Therefore, increasing the nutrient supply for finishing goats did not have marked positive effects on carcass. A lower expression of these proteins can have a profound impact on the qualitative aspects of the meat product.

Objetivo: Este estudo transversal objetiva examinar a associação entre marcadores inflamatórios e a ocorrência de hospitalizações entre idosos, considerando fatores predisponentes e facilitadores do uso de serviços de saúde e condições de saúde como potenciais fatores de confusão. Métodos: Foram utilizados dados de 1.393 participantes (≥ 60 anos) da linha de base da coorte de Bambuí. Os marcadores considerados foram dez citocinas e quimiocinas (interleucina (IL)-1, IL-6, IL-10, IL-12, fator de necrose tumoral (TNF), CCL2, CCL5, CXCL8, CXCL9 e CXCL10). A variável de desfecho foi a ocorrência de uma ou mais hospitalizações nos 12 meses precedentes. Resultados: Níveis séricos elevados da IL-6 apresentaram associações significantes com a ocorrência de hospitalizações (razão de prevalência - RP = 1,38; intervalo de confiança - IC95% 1,01 - 1,87; e RP = 1,38; IC95% 1,01 - 1,88, para os tercis intermediário e superior, respectivamente). Níveis elevados da CXCL9 também apresentaram associações independentes com o desfecho (RP = 1,38; IC95% 1,02 - 1,89 e RP = 1,46; IC95% 1,07 - 2,00, respectivamente). Os demais marcadores não apresentaram associações estatisticamente significantes com a ocorrência de hospitalizações. Conclusão: Entre os 10 marcadores examinados, IL-6 e CXCL9 apresentaram associação com a ocorrência de hospitalizações.

Factors Associated with Inadequate Management of Antiplatelet Agents in Perioperative Period of Non-Cardiac Surgeries

Resumo Fundamento: As diretrizes atuais apresentam recomendações para o manejo de antiagregantes plaquetários em perioperatório, entretanto, na prática clínica diária há falta de padronização das condutas médicas. Objetivos: Avaliar os fatores associados ao manejo inadequado de antiagregantes plaquetários em perioperatório de cirurgias não cardíacas. Métodos: Estudo transversal, realizado de outubro de 2014 a outubro de 2016, em hospital. A variável dependente do estudo foi a terapia divergente das recomendações das diretrizes da Sociedade Brasileira de Cardiologia (SBC). As variáveis independentes incluíram algumas características, os responsáveis pelo manejo e as causas de não adesão às diretrizes. As variáveis foram incluídas no modelo multivariado. A análise se baseou no valor de oddsratio (OR) e seu respectivo intervalo de confiança (IC) de 95%, estimados por regressão logística com um nível de significância de 5%. Resultados: A amostra foi composta de pacientes adultos submetidos a cirurgias não cardíacas e que faziam uso de ácido acetilsalicílico (AAS) ou clopidogrel (n = 161). O manejo esteve em desacordo com aquele preconizado pelas diretrizes em 80,75% da amostra. Os cirurgiões realizaram o maior número (n = 63) de orientações em desacordo. Após a análise multivariada, observou-se que os pacientes com nível de escolaridade superior (OR = 0,24; IC95% 0,07-0,78) e aqueles com episódio prévio de infarto agudo do miocárdio (IAM) (OR = 0,18; IC95% 0,04-0,95) possuem maior chance de utilizar a terapia em concordância com as recomendações. Conclusão: Associação positiva entre o nível de escolaridade dos pacientes ou histórico prévio de IAM com o manejo do uso de AAS e clopidogrel em perioperatório de cirurgias não cardíacas. Porém as divergências nas condutas reforçam a necessidade de definição de protocolos internos.
Abstract Background: The current guidelines dispose recommendations to manage antiplatelet agents in the perioperative period; however, the daily medical practices lack standardization. Objectives: To asses factors associated with inadequate management of antiplatelet agents in the perioperative period of non-cardiac surgeries. Methods: Cross-sectional Study conducted in hospital from October 2014 to October 2016. The study dependent variable was a therapy that did not comply with the recommendations in the Brazilian Association of Cardiology (SBC) guidelines. The independent variables included some characteristics, the people in charge of the management and causes of lack of adherence to those guidelines. Variables were included in the multivariate model. Analysis was based on the odds ratio (OR) value and its respective 95% confidence interval (CI) estimated by means of logistic regression with 5% significance level. Results: The sample was composed of adult patients submitted to non-cardiac surgeries and who would use acetylsalicylic acid (aspirin) or clopidogrel (n = 161). The management failed to comply with the recommendations in the guidelines in 80.75% of the sample. Surgeons had the highest number of noncomplying orientations (n = 63). After multivariate analysis it was observed that patients with a higher level of schooling (OR = 0.24; CI95% 0.07-0.78) and those with a previous episode of acute myocardial infarction (AMI) (OR = 0.18; CI95% 0.04-0.95) had a higher probability of using a therapy complying with the guidelines. Conclusion: Positive association between patients’ schooling level, or those with a history of previous AMI, with management of the use of aspirin and clopidogrel in the perioperative period of non-cardiac surgeries. However, diverging conducts stress the need of having internal protocol defined.

KSG estimation of reconstruction delay to detect vocal disorders in nonlinear dynamical analysis

Abstract Introduction This research investigates the applicability of a relatively new estimator of mutual information, KSG estimator, to find the reconstruction delay of phase space in dynamical systems. There are evidences that the KSG estimator is more accurate than the naive method commonly used. Methods In this paper we estimated mutual information between the voice signals and their delayed versions, with KSG method. The voice signals were obtained from a disordered voice database. Then, we found the reconstruction delay where mutual information reached its first minimum. We applied the encountered value of reconstruction delay in linear discriminant analysis, in order to discriminate between healthy and pathological voices or to discriminate between pathologies. Discrimination between voice pathologies using nonlinear measurements is still not much explored. Moreover, in this paper we used a single nonlinear measurement: reconstruction delay. Results The results show that the reconstruction delay obtained with KSG method has increased classification rates in most cases, in terms of accuracy, sensitivity and specificity, when compared to the naive estimator usually adopted. Conclusion The KSG estimator is a promising technique to improve the diagnosis of voice related pathologies.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Collared Pecary (tayassu tajacu) as a new model of renal ischemic injury induced by clamping the renal artery

PURPOSE:The use of the collared peccary as an experimental model for ischemic nephropathy.METHODS:A total of 12 collared peccary (Tayassu tajacu) was used and ischemic nephropathy was induced in six of these animals that constituted the experimental group (G1) while the other six formed the control group (G2). Ischemic nephropathy was induced surgically by partial occlusion of the left renal artery. The disease course was assessed by hematological tests, serum chemistry, urinalysis, ultrasound (US) and doppler ultrasound function of the renal artery before induction, and at five, 10, 15 and 20 days after surgery. Twenty days after the occlusion, unilateral nephrectomy and histopathological examination were performed to assess renal morphology.RESULTS:Statistical analysis by Fischer's test showed a significant difference (p<0.05) between the control group and the experimental group. The histopathological examination showed glomerular, tubular and interstitial lesions. In the experimental group, 83.3% (5 /6) showed moderate renal lesions and only 16.7% (1/6) were classified with no lesions. The ultrasound examination of the right kidney presented statistical difference between day 5 and day 10 post occlusion.CONCLUSION:The collared peccary as a good experimental model for ischemic renal disease, because it could be manipulated during the research time without death, with health conditions that permit any subsequent procedure for disease therapy.

Faculdades da alma e suas implicações para a educação: saberes divulgados no século XIX

Este artigo descreve resultados de uma pesquisa que buscou investigar como o tema das faculdades da alma e de suas implicações no desenvolvimento humano e na educação da pessoa foi divulgado, ao final do século XIX, pela imprensa periódica. Esperando contribuir para a história da psicologia e da educação, o artigo apresenta a análise de escritos sobre educação moral e educação estética de uma Secção Scientífica ou Pedagógica assinada por José Miguel de Siqueira e publicada no jornal O Baependyano (1877-1889). Inicialmente, são feitas breves descrições do periódico e de sua posição na conjuntura da época, bem como do lugar em que foi publicado. Em seguida, tratamos da definição de pedagogia apresentada pelo jornal, levando em consideração as apropriações de autores estrangeiros comumente presentes no campo da educação. A pedagogia considerada moderna é definida em tais escritos como a expansão e o desenvolvimento das forças animais, racionais e morais do ser humano; sua finalidade seria a formação do homem social pronto para o trabalho e para o serviço da sociedade e do Estado. Partindo dessa discussão, abordamos as concepções de educação espontânea, educação regular e instrução contidas no periódico. A educação é entendida como a unidade entre cultura e instrução, sendo a cultura a principal responsável pelo direcionamento das faculdades da alma e pela formação moral; a instrução, por sua vez, é tomada como aquisição de conhecimentos, mas também desempenha um papel importante na formação de hábitos e costumes.
The article describes the results of a research that investigated how the theme of the faculties of the soul and of their implications for human development and for the education of a person was made available in the late 19th century by the circulating press. In an attempt to contribute to the history of psychology and education, the article presents the analysis of writings on moral education and aesthetic education that appeared on a scientific or pedagogical section signed by José Miguel de Siqueira and published in the O Baependyano (1877-1889) periodical. Initially, brief descriptions of the periodical and of its position within the context of the time are made, as well as of the place at which it was published. Next, we deal with the definition of pedagogy presented by the periodical, taking into account the appropriations of foreign authors commonly present in the field of education. Pedagogy, regarded as modern, is defined in those writings as the expansion and development of the animal, rational and moral strengths of the human being; its purpose would be the formation of the social man ready for work and for the service of society and of the State. Based on this discussion, we investigate the conceptions of spontaneous education, regular education, and instruction contained in the periodical. Education is understood as the unity of culture and instruction, with the former being the chief responsible for directing the faculties of the soul and for moral formation; instruction, by its turn, is seen as the acquisition of knowledge, but also plays an important role in the formation of habits and customs.

É preocupante o crescimento do consumo de substâncias psicoativas entre a população, o que constitui um grave problema de saúde pública. Entre as substâncias psicoativas, o álcool e o tabaco merecem destaque, pois são drogas lícitas e socialmente aceitas. Este estudo buscou estabelecer a prevalência do uso de bebidas alcoólicas e cigarros entre estudantes da Faculdade de Medicina da Universidade Federal de Uberlândia (Famed/UFU), bem como caracterizar as situações propícias, a freqüência, a auto-avaliação sobre consumo e o tipo de bebida utilizado por esses estudantes. Os dados foram coletados de uma amostra de 303 acadêmicos matriculados na Famed/UFU, a partir de questionário anônimo. Constatou-se que 66,34% dos alunos entrevistados consomem bebidas alcoólicas. Há aumento de ingestão de bebidas no decorrer do curso, sendo que os homens consomem mais que as mulheres. Apenas 3,3% do total de estudantes são fumantes e 2% são ex-fumantes. Esses números sugerem que a consciência sobre os malefícios do cigarro é maior do que a existente sobre o consumo abusivo de bebidas alcoólicas, no qual um comportamento de risco é observado.
The significantly increasing use of psychoactive substances by the population has caused concern and poses a serious public health threat. Alcohol and tobacco deserve special attention among these substances for being licit and socially accepted drugs. The present paper provides data about the prevalence of alcohol and tobacco use among the students of the Faculty of Medicine of the Federal University of Uberlândia (Famed/UFU), situations that favor the use of these substances, frequency, self-evaluation of the users, and kind of alcohol used. The data were collected from a random sample of 303 students of Famed/UFU, using an anonymous questionnaire. The study indicated that 66.34% of the interviewed students consume alcohol. The use of alcohol increases during the course and is higher among males than among females. Only 3.3% of the students are smokers and 2% are ex-smokers. These numbers suggest a greater awareness of the harm caused by the cigarette than of the harm caused by abusive use of alcohol, where a risk behavior was observed.
