Resumen (Introducción): La capacidad productiva de una plantación forestal depende principalmente de las características fisicoquímicas del suelo, así como del manejo silvicultural necesario para un óptimo desarrollo de los árboles. (Objetivo): El objetivo de esta investigación fue la evaluación del efecto de la fertilización sobre plantaciones forestales clonales de Gmelina arborea Roxb. ex Sm., de tres años, en el Pacífico Norte y Sur de Costa Rica. (Metodología): El estudio se realizó en 2 ensayos experimentales del Instituto de Investigación y Servicios Forestales ubicados en La Esperanza, Cóbano (ensayo 1) y en Rancho Quemado, Bahía Drake (ensayo 2). Se establecieron 5 bloques completos, al azar, con 4 tratamientos de fertilización con el abono 19-4-19-2-0,1(B)-1,8(S)-,1(Zn). Se realizó un análisis de varianza y comparación de medias de los incrementos medios anuales del DAP, la altura y el volumen total de los árboles utilizando la prueba de Tukey. (Resultados): Los resultados muestran que el sitio con mayor fertilidad natural presenta aumento en el crecimiento de los árboles; sin embargo, no existen diferencias estadísticamente significativas entre todos los tratamientos aplicados. El sitio de menor fertilidad natural se ubica en Bahía Drake y presentó diferencias estadísticamente significativas en algunos tratamientos en términos de volumen total. (Conclusiones): La fertilización continua tienen un impacto positivo en el crecimiento dasométrico en plantaciones de G. arborea que presenten bajos contenidos de calcio (< 4.5 cmol L-1), valores bajos en capacidad de intercambio catiónico efectiva (< 7.8 cmol L-1) y alta saturación de acidez (> 19.4 %).

Abstract (Introduction): The productive capacity of a forest plantation depends mainly on the physicochemical characteristics of the soil, as well as the necessary silvicultural management for optimal tree development. (Objective): The objective of this research was the evaluation of the effect of fertilization on clonal forest plantations of Gmelina arborea Roxb. ex Sm. 3 years old, in the North and South Pacific of Costa Rica. (Methodology): The study was carried out in 2 experimental trials of the Forestry Research and Services Institute located in La Esperanza, Cóbano (trial 1) and in Rancho Quemado, Drake Bay (trial 2). Five complete blocks were established at random with four fertilization treatments with the fertilizer 19-4-19-2-0.1(B)-1.8(S)-.1(Zn). An analysis of variance and comparison of means of the mean annual increments of DBH, height and total volume of the trees was carried out, using the Tukey test. (Results): The results show that the site with the highest natural fertility presents a greater growth of the trees, however, there are no statistically significant differences between all the treatments applied. The site with the lowest natural fertility is located in Bahía Drake and it did present statistically significant differences in some treatments in terms of total volume. (Conclusions): Continuous fertilization have a positive impact on the dasometric growth in G. arborea plantations that present low calcium contents (< 4.5 cmol L-1), low values of effective cation exchange capacity (<7.8 cmol L-1) and high acidity saturation (> 19.4 %).

Resumen Antecedentes: El síndrome de Down es la anomalía cromosómica más frecuente y se asocia con defectos cardiacos congénitos, elementos clínicos de una alta morbilidad y mortalidad infantil. Objetivo: Describir los desenlaces clínicos de los pacientes con síndrome de Down sometidos a cirugía e intervencionismo como tratamiento de las cardiopatías congénitas en esta institución. Material y métodos: Estudio retrospectivo que incluyó pacientes menores de 18 años con síndrome de Down y patología cardiaca asociada durante los últimos 10 años. Las variables estudiadas fueron: peso, talla, sexo, edad, tipo de cardiopatía, procedimiento correctivo, tiempo de estancia intrahospitalaria y en unidad de terapia intensiva, morbilidad y mortalidad. Resultados: 368 pacientes pediátricos fueron llevados a corrección quirúrgica o intervencionista, de los cuales 197 (54%) pertenecían al sexo femenino, la mediana de edad fue de 24 meses (rango intercuartílico [RIQ]: 14-48) en el grupo quirúrgico y de 36 meses (RIQ: 17-85) en el intervencionista. Las cardiopatías congénitas más frecuentes fueron: persistencia del conducto arterioso (31%), comunicación interventricular (28%), canal atrioventricular (CAV) (20%), comunicación interauricular (16%) y tetralogía de Fallot con el 4% respectivamente. La estancia hospitalaria fue de 9 días (RIQ: 7-15) en el grupo quirúrgico y de 3 días (RIQ: 2-5) en el hemodinámico. Las morbilidades fueron infección postoperatoria en 30 pacientes (14%) y en 19 pacientes (9%) bloqueo atrioventricular completo. La mortalidad global incluyendo tanto el quirúrgico como el intervencionista fue del 2%. Conclusiones: Los resultados terapéuticos, quirúrgicos e intervencionistas, en los niños con síndrome de Down y cardiopatías congénitas han mejorado en forma muy satisfactoria. Es de destacar la menor prevalencia del CAV en la población mexicana. Es indispensable realizar evaluación cardiológica a los niños con síndrome de Down y aquellos con cardiopatías congénitas llevarlos a corrección de manera oportuna para favorecer la sobrevida y calidad de vida.

Abstract Background: Down syndrome is the most common chromosomal abnormality, it is associated with a wide variety of congenital heart defects, being considered as clinical elements of high infant morbidity and mortality. Objective: To describe the clinical outcomes of patients with Down syndrome undergoing surgery and interventionism as treatment for congenital heart disease at this Institution. Material and methods: 368 patients with Down syndrome and associated congenital heart disease were diagnosed. The variables studied were weight, stature, sex, age, type of heart disease, corrective procedure, length of stay in the hospital and intensive care unit, morbidity and mortality. Results: 368 pediatric patients underwent surgical or interventional correction. Of which 197 (54%) were female, the median age was 24 months (interquartile range [IQR]: 14-48) in the surgical group and 36 months (IQR: 17-85) in the interventional group. The most frequent congenital heart diseases were: PCA (31%), IVC (28%), CAV (20%), ASD (16%) and tetralogy of Fallot with 4% respectively. Hospital stay was 9 days (IQR: 7-15) in the surgical group and 3 days (IQR: 2-5) in the hemodynamic group. Morbidities were postoperative infection in 30 patients (14%) and complete atrioventricular block in 19 patients (9%). Overall mortality including both surgical and interventional was 2%. Conclusions: The therapeutic, surgical and interventional results in children with Down syndrome and congenital heart disease have improved very satisfactorily. The lower prevalence of the atrioventricular canal in the Mexican population is noteworthy. It is essential to carry out a cardiological evaluation of children with Down syndrome and those with congenital heart disease to correct them in a timely manner to promote survival and quality of life.

ABSTRACT Introduction: Medical students are vulnerable to psychological distress and face a number of stressors such as academic pressures, economic concerns, and difficulties in interpersonal relationships. The quality of their educational training may also impact their mental health and subjective well-being. Objective: to describe their perception of subjective well-being and the impact of medical training on their own mental health. Methods: We surveyed 119 medical students and administered the “Perception survey on vocation, living and recreational habits, training and professional attitudes,” the CAGE questionnaire, the Generalized Anxiety Disorder-7 (GAD-7) and the Patient Health Questionnaire-2 (PHQ-2). Results: Students’ mean age was 22.5±2.28 years old and 59.7 % of participants were women, their lifestyle habits were scored as acceptable, and the employment of cell-phones and social networks ranged 76-99%; 24.1 % recognized a problematic alcohol, 67.2 % reported anxiety according to the GAD-7, and 49.6 % depression according to PHQ-2. Symptoms of anxiety were frequent among females and were higher when the perceived quality of training and student/teacher ratio were lower. Discussion: Students’ perceptions of lower quality of training and resources seemed to be associated with higher levels of anxiety, which may suggest that educational policies and resources should be carefully revised and implemented to improve students’ health and well-being.

RESUMEN Introducción: los estudiantes de medicina son vulnerables al malestar psicológico y se enfrentan a una serie de factores estresantes como las presiones académicas, las preocupaciones económicas y las dificultades en las relaciones interpersonales. La calidad de su formación educativa también puede influir en su salud mental y bienestar subjetivo. Objetivo: describir su percepción de bienestar subjetivo y el impacto de la formación médica en su propia salud mental. Metodología: se encuestó a 119 estudiantes de medicina y se les administró la "Encuesta de percepción sobre vocación, hábitos de vida y recreación, formación y actitudes profesionales", el cuestionario CAGE, el Trastorno de Ansiedad Generalizada-7 (TAG-7) y el Cuestionario de Salud del Paciente-2 (PHQ-2). Resultados: la edad media de los estudiantes fue de 22,5±2,28 años y el 59,7 % de los participantes eran mujeres; sus hábitos de vida se calificaron como aceptables, y el empleo de teléfonos móviles y redes sociales osciló entre el 76 y el 99 %; el 24,1 % reconoció un consumo problemático de alcohol, el 67,2 % manifestó ansiedad según el TAG-7, y el 49,6 % depresión según el PHQ-2. Los síntomas de ansiedad eran frecuentes entre las mujeres y eran mayores cuando la calidad percibida de la formación y la proporción alumnos/profesor eran menores. Discusión: la percepción por parte de los estudiantes de una menor calidad de la formación y de los recursos parecía estar asociada a mayores niveles de ansiedad, lo que puede sugerir que las políticas y los recursos educativos deberían revisarse y aplicarse cuidadosamente para mejorar la salud y el bienestar de los estudiantes.

ABSTRACT Introduction: Self-regulation of learning refers to the modulation of affective, cognitive and behavioral processes throughout a learning experience to reach a desired level of achievement. Students who have a better management of their cognitive and metacognitive skills learn in a better way than those who do not, therefore, it is essential to study at what level university students are capable of learning to learn and to be managers of their learning. Objectives: To analyze the relationship between self-regulation of learning and academic performance in medical students from three universities in the year 2022. Materials and methods: This research followed a quantitative approach; the type of research was observational and descriptive. A previously validated instrument was used to identify types of self-regulation, the Self-Regulation of Learning from Texts (ARATEX-R). Results: No significant relationship was found between the academic profile of medical students and the total ARATEX-R score. When studying the relationship between the dimensions of self-regulation, a relationship was found between academic performance and motivation planning and management, which implies that those who have better planning skills and who are able to manage their motivation have better academic performance. Conclusion: The main dimensions that characterize the self-regulation of learning in medical students were: context management, cognition management and comprehension evaluation.

RESUMEN Introducción: La autorregulación del aprendizaje se refiere a la modulación de los procesos afectivos, cognitivos y conductuales a lo largo de una experiencia de aprendizaje para alcanzar un nivel de logro deseado. Los estudiantes que tienen un mejor manejo de sus habilidades cognitivas y metacognitivas aprenden de una mejor forma que aquellos que no, por lo tanto, es fundamental estudiar a qué nivel los estudiantes universitarios son capaces de aprender a aprender y a ser gestores de su aprendizaje. Objetivos: Analizar la relación entre la autorregulación del aprendizaje y el rendimiento académico en estudiantes de la carrera medicina de tres universidades en el año 2022. Materiales y métodos: Esta investigación siguió un enfoque cuantitativo, el tipo de investigación fue observacional de tipo descriptivo. Se utilizó un instrumento previamente validado para identificar los tipos de autorregulación, la Autorregulación del Aprendizaje a partir de Textos (ARATEX-R). Resultados: No se encontró relación significativa entre el perfil académico de los estudiantes de medicina y el puntaje total del ARATEX-R. Al estudiar la relación entre las dimensiones de la autorregulación se encontró relación entre el rendimiento académico y la planificación y la gestión de la motivación, lo que implica que los que tienen mejores habilidades de planificación y que son capaces de gestionar su motivación tienen mejor rendimiento académico. Conclusión: Las principales dimensiones que caracterizan la autorregulación del aprendizaje en estudiantes de medicina fueron: gestión del contexto, gestión de la cognición y evaluación de la comprensión.

RESUMO: Espondilomielopatia cervical (EMC) é uma doença que geralmente afeta cães de raças grandes e gigantes. Ela é caracterizada por anormalidades da coluna cervical que podem causar danos à medula espinhal ou às raízes nervosas. Artroplastia cervical com prótese de disco tem sido proposta como opção de tratamento na medicina veterinária. O presente estudo teve como objetivo avaliar os principais estresses em uma nova prótese de disco intervertebral canina e corpos vertebrais por meio da análise de elementos finitos. Foram compostos dois grupos experimentais que representaram o material constituinte da nova prótese: grupos aço inoxidável (SSG) e o liga de titânio (TAG). Tensões equivalente de von-Mises (VMS) e tensão principal mínima (MiPS) média foram avaliadas sob forças de compressão, tração e torção para vértebras e prótese. O grupo SSG teve maior distribuição de VMS para todos parâmetros avaliados, enquanto o grupo TAG teve maior MiPS. Estresses estiveram mais concentrados nas superfícies lateral e ventral dos corpos vertebrais do que nas placas terminais. Os estresses médios da prótese foram mais concentrados na superfície de contato osso/implante do que na interface prótese/parafuso. Estresses máximos foram concentrados na superfície cranial do parafuso. A nova prótese permitiu distribuição uniforme do estresse ao longo do corpo vertebral. Comparando os materiais da prótese, a liga de titânio foi marginalmente superior quanto aos estresses médios em todas as direções, sendo menos provável que sofra afundamento da prótese.

ABSTRACT: Cervical spondylomyelopathy (CSM) is a disease that affects mostly large- and giant-breed dogs. It is characterized by abnormalities of the cervical spine that may cause damage to the spinal cord and nerve roots. Cervical disc arthroplasty has been proposed as a treatment option in veterinary medicine. The current study evaluated the main stresses in a novel canine vertebral disc prosthesis and vertebral bodies using finite element analysis. Two experimental groups were created based on the material used for the prosthesis: stainless steel group (SSG) and titanium alloy group (TAG). Vertebral and prosthetic average equivalents von-Mises stress (VMS) and minimum principal stress (MiPS) were assessed while compressive, tensile, and lateral bending shear loads were applied. The stainless steel group had greater VMS distribution on all the evaluated parameters while the titanium alloy group had greater MiPS. Stresses were more concentrated on the lateral and ventral surfaces of the vertebral bodies than on their endplates. The average prosthetic stresses were more concentrated on the bone/implant contact surface than on the prosthesis/screw interface. Maximum stresses were concentrated in the screws’ cranial surface. The novel prosthesis allows even distribution along the vertebral body. Comparing prosthesis materials, titanium alloy was marginally superior regarding average stresses in all directions and should be less likely to suffer subsidence.

Colorectal cancer survival in Manizales, Colombia, 2008-2017: a population-based study

RESUMEN Objetivo: Estimar la supervivencia a 5 años en pacientes con cáncer colorrectal (CCR) según características del paciente y del tumor. Métodos: Estudio longitudinal a partir de los casos de CCR invasivo diagnosticados entre 2008 y 2017 captados por el Registro Poblacional de Cáncer de Manizales (n=850). Se realizó seguimiento hasta el 24 de agosto de 2021. Se calculó la supervivencia causa-específica y la supervivencia neta para las características sociodemográficas y del tumor, y análisis multivariado de Cox. Resultados: El 55,5% de los casos se presentaron en mujeres. El tipo histológico más frecuente (78,2%) fue el adenocarcinoma. Las localizaciones más frecuentes fueron recto (32,0%), colon ascendente (16,6%) y sigmoides (16,2%). El 25,5% de los casos fue diagnosticado en estadio IV. Ocurrieron 567 muertes por CCR. La supervivencia específica a 5 años fue del 45,8% (IC95% 42,4–49,3), con efectos independientes para la edad (HR=1,83; IC95% 1,26–2,65 edad >75 años vs. edad <50 años) y el estadio clínico avanzado (HR=2,5 y HR 5,7 para estadios III y IV vs. estadio I). Se observó una menor supervivencia en pacientes de nivel socioeconómico medio en comparación con el grupo de mayor posición socioeconómica (HR=1,52; IC95% 1,08–2,14), pero no en los pacientes de posición socioeconómica baja. No se observaron efectos independientes para el régimen de aseguramiento en salud. Conclusiones: En Manizales, aproximadamente 5 de cada 10 pacientes con CCR invasivo fallecen en los primeros cinco años del diagnóstico, con sobrevida menor en pacientes mayores de 75 años, de nivel socioeconómico medio y diagnosticados en estadio clínico avanzado.

ABSTRACT Objective: To determine 5-year survival in patients with colorectal cancer (CRC) according to patient and tumor characteristics. Methods: Longitudinal study based on incident cases of invasive CRC between 2008 and 2017 captured by the Manizales Population-based Cancer Registry (n=850). Patients were followed up to August 24th, 2021. Cause-specific survival and net survival were calculated for sociodemographic and tumor characteristics, and Cox multivariate was fitted. Results: Fifty-five percent of cases occurred in women. The most frequent histological type was adenocarcinoma (78.2%). The most frequent locations were rectum (32.0%), ascending colon (16.6%), and sigmoid (16.2%). Twenty-five percent of cases were diagnosed in stage IV. There were 567 deaths due to CRC. The 5-year specific survival was 45.8% (95%CI 42.4–49.3), with independent effects for age (HR=1.83; 95%CI 1.26–2.65 age >75 years vs. <50 years) and advanced clinical stage (HR=2.5 and HR 5.7 for stages III and IV vs. stage I, respectively). Lower survival was observed in patients of medium socioeconomic status compared with higher socioeconomic status (HR=1.52; 95%CI 1.08–2.14), but not in patients of low socioeconomic status. No independent effects were observed for the health insurance regime. Conclusions: In Manizales, approximately 5 out of 10 patients with invasive CRC die in the first five years after diagnosis, with a lower survival in patients older than 75 years, from medium socioeconomic level and diagnosed in advanced clinical stages.

ABSTRACT: Cervical arthroplasty with disc prosthesis has been proposed as a treatment option for dogs with Cervical Spondylomyelopathy. The present study developed a novel vertebral disc prosthesis for dogs. Sixteen Functional Spinal Units (C5-C6) were collected from dog cadavers with body weights ranging between 25 and 35 kg, and their vertebral measurements were used to design a prosthetic disc. The sizing of the prosthesis was performed based on the averages of the measurements of width, height, and length of the vertebral bodies from C5-C6 of all specimens. The prosthesis was developed using the Rhinoceros 3D® and SolidWorks® programs, and 3D prototyping was carried out to define the best design. The developed prosthesis consisted of two independent parts that are fixed to the cranial and caudal vertebral bodies, in the intervertebral space, and fitted together by metal-to-metal surfaces capable of moving in the lateral, ventral, and dorsal directions. Each part of the prosthesis is angled in two portions: vertically, in the intervertebral space, and horizontally, in contact with the ventral surface of the vertebral bodies, both of which are fixed by means of monocortical locking screws. The design of the developed prototype allowed a good fit in the intervertebral space between C4-C5, C5-C6, and C6-C7.

RESUMO: A artroplastia cervical com prótese de disco tem sido proposta como uma opção de tratamento para cães com Espondilomielopatia Cervical. O presente estudo teve como objetivo desenvolver uma nova prótese de disco intervertebral para cães. Dezesseis Unidades Funcionais de Coluna Vertebral (C5-C6) foram coletadas de cadáveres de cães com peso corporal variando entre 25 e 35 kg, e suas medidas vertebrais foram usadas para projetar um disco intervertebral protético. O dimensionamento da prótese foi realizado com base nas médias das mensurações da largura, altura e comprimento dos corpos vertebrais de C5-C6 de todos os espécimes. A prótese foi desenvolvida nos programas Rhinoceros 3D® e SolidWorks® e utilizou-se prototipagem em 3D para a definição do melhor design. A prótese desenvolvida é formada por duas partes independentes que se fixam às espífises dos corpos vertebrais cranial e caudalmente ao espaço intervertebral, e se encaixam entre si por superfícies de metal-metal com capacidade de movimentação nas direções lateral, ventral e dorsal. Cada parte da prótese é angulada em duas porções: a vertical, que fica no espaço intervertebral, e a horizontal, que fica em contato com a superfície ventral dos corpos vertebrais, ambas as quais são fixadas por meio de parafusos bloqueados monocorticais. O design do protótipo desenvolvido permitiu bom encaixe no espaço intervertebral entre C4-C5, C5-C6 e C6-C7.

Abstract Objectives The present study aimed to evaluate the measurement invariance of a general measure of the perception of governmental responses to COVID-|19 (COVID-SCORE-10) in the general population of 13 Latin American countries. Methods A total of 5780 individuals from 13 Latin American and Caribbean countries selected by non-probabilistic snowball sampling participated. A confirmatory factor analysis was performed and the alignment method was used to evaluate invariance. Additionally, a graded response model was used for the assessment of item characteristics. Results The results indicate that there is approximate measurement invariance of the COVID-SCORE-10 among the participating countries. Furthermore, IRT results suggest that the COVID-SCORE-10 measures with good psychometric ability a broad spectrum of the construct assessed, especially around average levels. Comparison of COVID-SCORE-10 scores indicated that participants from Cuba, Uruguay and El Salvador had the most positive perceptions of government actions to address the pandemic. Thus, the underlying construct of perception of government actions was equivalent in all countries. Conclusion The results show the importance of initially establishing the fundamental measurement properties and MI before inferring the cross-cultural universality of the construct to be measured.

Abstract Background: Hybridization in nature occurs in numerous botanical families. In particular, the Cactaceae family contains lots of genera in which hybridization is reported. Questions: What are the patterns of reported natural hybridization in Cactaceae and their probable causes? Are there phylogenetic and evolutionary implications related to hybridization, particularly in Opuntioideae? Data description: A total of 62 articles about natural hybridization and classical Cactaceae literature were reviewed. Study site and dates: From 1900 to June 2021 Methods: A search for articles was performed in Web of Science and Google Scholar with the keywords "Cactaceae hybridization", for time span "1900 to 2021" and included information from classic family-specific monographs. Results: Natural hybrids in Cactaceae occur in subfamilies, Cactoideae and Opuntioideae. There is evidence of nonselective mechanisms of reproductive isolation, but only for few taxa. For Cactoideae members the main approach used was morphological description, and the tribe with the highest number of natural hybrids was Trichocereeae. In Opuntioideae, the reviewed articles performed mostly chromosome counts, morphometric and phylogenetic analyses, and showed the highest number of natural hybrids. Conclusions: It has been suggested that hybridization impacts the evolution of Cactoideae and Opuntioideae, but few studies have formally tested this hypothesis. In Cactoideae, we found only descriptive evidences of hybridization; therefore, previous statements suggesting an important role of hybridization in the evolution of Cactoideae should be supported by performing formal analyses. For the postulation that hybridization impacts the evolution of Opuntioideae, we found formal evidence supporting hybridization hypothesis unlike what we found in Cactoideae.

Resumen Antecedentes: La hibridación natural se presenta en numerosas familias botánicas. En particular Cactaceae presenta muchos géneros con reportes de hibridación. Preguntas: ¿Cuáles son los patrones reportados de hibridación natural en Cactaceae y sus probables causas? ¿Existen implicaciones filogenéticas y evolutivas relacionadas a la hibridación, particularmente en Opuntioideae? Descripción de datos: Se revisaron 62 artículos sobre hibridación natural y literatura clásica de Cactaceae. Sitio y años de estudio: De 1900 a junio de 2021 Métodos: Una búsqueda en Web of Science y Google Scholar con las palabras clave "Cactaceae hybridization", un lapso de tiempo de " 1900 a 2021", así como información de monografías clásicas. Resultados: Los híbridos naturales en Cactaceae se encuentran en Cactoideae y Opuntioideae. Hay evidencia de mecanismos no selectivos de aislamiento reproductivo, pero solo para pocos taxones. En Cactoideae el enfoque principal fue la descripción morfológica, y la tribu con más híbridos naturales fue Trichocereeae. En Opuntioideae, los artículos revisados realizaron principalmente conteos cromosómicos, análisis morfométricos y filogenéticos, y mostraron el mayor número de híbridos naturales. Conclusiones: Se había sugerido que la hibridación impacta la evolución de Cactoideae y Opuntioideae, pero pocos estudios han probado formalmente esta hipótesis. En Cactoideae, encontramos evidencias descriptivas de hibridación; por lo tanto, las declaraciones previas que sugieren un papel importante de la hibridación en la evolución de Cactoideae deben ser apoyadas mediante análisis formales. Para la postulación de que la hibridación impacta la evolución de Opuntioideae, encontramos evidencia formal que apoya las hipótesis de hibridación diferente de lo encontrado en Cactoideae.

Resumen Introducción: En Colombia, la investigación sobre el VIH en Personas que se Inyectan Drogas (PID) es escaso, pese a ser un grupo clave para el control de la infección. Objetivo: Determinar la prevalencia de VIH y sus factores asociados en PID atendidos en una Institución Prestadora de Servicios de salud (IPS) de Medellín-Colombia. Métodos: Estudio de prevalencia en 293 PID en una IPS especializada en la atención de enfermedades infecciosas. Se aplicó una encuesta para recolectar datos sociodemográficos y de salud, una inmunocromatografía para la tamización de la infección y los casos positivos se confirmaron con Western blot. Los análisis se realizaron en SPSS 27.0. Resultados: La edad mediana 29 años, 84% hombres, 52% sin escolaridad y alta frecuencia de factores de riesgo sexual. La prevalencia del VIH fue de 5,5%, de estos 87,5% coinfectados con virus de la hepatitis C y 53,6% con la B. La prevalencia de VIH fue estadísticamente más elevada en sujetos mayores de 29 años de edad (10,4%), quienes viven en condición de calle (25,5%), sin escolaridad (9,9%) y quienes reciben dinero o drogas a cambio de sexo (11,6%). Conclusión: Se obtuvo una prevalencia de VIH mayor a la reportada en estudios previos de la ciudad, alta proporción de coinfección con virus de las hepatitis B y C, y se identificaron grupos con mayor ocurrencia de VIH. Estos resultados, además de permitir disponer de una línea base para evaluar intervenciones posteriores en este grupo, demuestra la interseccionalidad en el riesgo de VIH, consumo de drogas inyectables, hepatitis virales, baja escolaridad, condición de calle, y otros que muestran la complejidad clínica, epidemiológica y social para impactar el VIH en este grupo.

Abstract Introduction: In Colombia, research on HIV in people who inject drugs (PID) is scarce, despite being a key group for infection control. Objective: To determine the prevalence of HIV and its associated factors in PID treated in a health service provider institution (IPS) in Medellín-Colombia. Methods: Prevalence study in 293 PID from an IPS specialized in the care of infectious diseases. A survey was applied to collect sociodemographic and health data, immunochromatography for infection screening, and positive cases were confirmed with Western blot. Analyzes were performed in SPSS 27.0. Results: The median age was 29 years, 84% men, 52% without schooling and high frequency of sexual risk factors. HIV prevalence was 5.5%, of these 87.5% co-infected with hepatitis C virus and 53.6% with hepatitis B virus. HIV prevalence was statistically higher in subjects older than 29 years of age (10.4%), those who live on the streets (25.5%), without schooling (9.9%) and those who receive money or drugs in exchange for sex (11.6%). Conclusion: A higher prevalence of HIV was obtained in comparison with previous studies of the city; a high proportion of co-infection with hepatitis B and C viruses, and groups with a higher occurrence of HIV were identified. These results, in addition to providing a baseline to evaluate subsequent interventions in this group, demonstrate the intersectionality in the risk of HIV, injecting drug use, viral hepatitis, low schooling, homelessness, and others social and health conditions, that show the clinical, epidemiological and social complexity to impacting HIV in this group.

Resumen Introducción: El instrumento de calidad de vida de la Organización Mundial de la Salud es ampliamente utilizado en personas sanas y enfermas; sin embargo, son pocos los estudios colombianos que han analizado sus dimensiones en población sana. Objetivo: Evaluar las propiedades psicométricas del WHOQOL-BREF en población sana de Medellín, y estimar los intervalos de referencia para sus cuatro dimensiones. Métodos: Estudio transversal y de evaluación psicométrica en 1938 personas. Se evaluó fiabilidad con el α de Cronbach, consistencia interna y poder discriminante con Rho de Spearman, validez de contenido con coeficientes λ y varianza explicada mediante un análisis factorial exploratorio con extracción por componentes principales. Se eliminaron valores aberrantes mediante el estadístico de Grubbs, se estimaron los intervalos de referencia globales para cada dimensión del WHOQOL-BREF y los específicos, según variables demográficas y socioeconómicas, mediante intervalos de confianza del 95% para la media, y comparaciones mediante T- Student y Anova. Los análisis se realizaron en SPPS 25.0®. Resultados: Se halló una excelente fiabilidad, consistencia interna, poder discriminante y validez de contenido en las cuatro dimensiones del instrumento en población sana. Los valores de referencia fueron 64,3-65,6 en salud física, 70,8-72,0 en salud psicológica, 64,3-65,9 en salud social y 60,5-61,6 en salud ambiental, con diferencias estadísticamente significativas según las características demográficas y socioeconómicas Conclusión: El excelente desempeño psicométrico de la escala en población sana, permitió la estimación de los intervalos de referencia para las cuatro dimensiones, así como los valores para subgrupos conformados por variables demográficas y socioeconómicas, lo que resulta determinante para incluir los desenlaces centrados en las personas en los programas de medicina y salud pública, así como comparadores adecuados para población enferma.

Abstract Introduction: The Quality of Life Instrument of the World Health Organization is widely used in healthy and sick people; however, few Colombian studies have analyzed its dimensions in a healthy population. Objective: To evaluate the psychometric properties of the WHOQOL-BREF in a healthy population of Medellín and to estimate the reference intervals for it’s four dimensions. Methods: Cross-sectional study and psychometric evaluation in 1938 people. Reliability was evaluated with Cronbach's α, internal consistency and discriminant power with Spearman's Rho, content validity with λ coefficients and variance explained by means of an exploratory factor analysis with extraction by principal components. Aberrant values were eliminated using the Grubbs statistic, the global reference intervals were estimated for each dimension of the WHOQOL-BREF and the specific ones, according to demographic and socioeconomic variables, using 95% confidence intervals for the mean, and comparisons using Student's t and Anova. The analyzes were carried out in SPPS 27.0®. Results: Excellent reliability, internal consistency, discriminating power, and content validity were found in the four dimensions of the instrument in a healthy population. The reference values were 64.3-65.6 in physical health, 70.8-72.0 in psychological health, 64.3-65.9 in social health and 60.5-61.6 in environmental health, with statistically significant differences according to demographic and socioeconomic characteristics Conclusion: The excellent psychometric performance of the scale in the healthy population allowed the estimation of the reference intervals for the four dimensions, as well as the values for subgroups made up of demographic and socioeconomic variables, which is decisive for including the outcomes centered on people in medicine and public health programs, as well as suitable comparators for the sick population.

RESUMEN Objetivos. Evaluar la exactitud de gota gruesa (GG) frente a la reacción en cadena de la polimerasa (PCR) cuantitativa para la malaria asociada al embarazo (MAE). Materiales y métodos. Se realizó una revisión sistemática de pruebas diagnósticas en nueve bases de datos. Se evaluó la calidad metodológica con QUADAS. Se estimó sensibilidad, especificidad, cociente de probabilidad positivo (CPP) y negativo (CPN), razón de odds diagnóstica (ORD) y área bajo la curva ROC. Se determinó la heterogeneidad con el estadístico Q de Der Simonian-Laird y la incertidumbre con el porcentaje de peso de cada estudio sobre el resultado global. Resultados. Se incluyeron diez estudios con 5691 gestantes, 1415 placentas y 84 neonatos. En los estudios con nPCR (PCR anidada) y qPCR (PCR cuantitativa) como estándar, los resultados de exactitud diagnóstica fueron estadísticamente similares, con sensibilidad muy baja (50 y 54%, respectivamente), alta especificidad (99% en ambos casos), alto CPP y deficiente CPN. Usando nPCR la OR diagnóstica fue 162 (IC95%=66-401) y el área bajo la curva ROC fue 95%, mientras que con qPCR fueron 231 (IC95%=27-1951) y 78%, respectivamente. Conclusiones. Mediante un protocolo exhaustivo se demostró el bajo desarrollo de investigaciones sobre la exactitud diagnóstica de la GG en MAE. Se demostró que la microscopía tiene un desempeño deficiente para el diagnóstico de infecciones asintomáticas o de baja parasitemia, lo que afianza la importancia de implementar otro tipo de técnicas en el seguimiento y control de las infecciones por malaria en las gestantes, con el fin de lograr el control y posible eliminación de la MAE.

ABSTRACT Objective. To evaluate the accuracy of thick smear (TS) versus quantitative polymerase chain reaction (PCR) for pregnancy-associated malaria (PAM). Materials and methods. We carried out a systematic review of diagnostic tests in nine databases. Methodological quality was evaluated with QUADAS. Sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), diagnostic odds ratio (DOR) and area under the ROC curve were estimated. Heterogeneity was determined with the Der Simonian-Laird Q method and uncertainty with the weighted percentage of each study on the overall result. Results. We included 10 studies with 5691 pregnant women, 1415 placentas and 84 neonates. In the studies with nested PCR (nPCR) and quantitative PCR (qPCR) as the standard, the diagnostic accuracy results were statistically similar, with very low sensitivity (50 and 54%, respectively), high specificity (99% in both cases), high PLR and poor NLR. When nPCR was used, the DOR was 162 (95%CI=66-401) and the area under the ROC curve was 95%, while with qPCR it was 231 (95%CI=27-1951) and 78%, respectively. Conclusions. We demonstrated that research on the diagnostic accuracy of TS in PAM is limited. Microscopy showed poor performance in the diagnosis of asymptomatic or low parasitemia infections, which reinforces the importance of implementing other types of techniques for the follow-up and control of malaria infections in pregnant women, in order to achieve the control and possible elimination of PAM.

SUMMARY OBJECTIVE: This study aimed to compare pain intensity, stiffness, functionality, central sensitization, and self-efficacy, between individuals with bilateral knee osteoarthritis and unilateral knee osteoarthritis. METHODS: We included sedentary participants with knee osteoarthritis. The diagnosis was defined by a specialist, in which there was a complaint of pain and/or altered function in the lower limbs (duration ≥3 months); morning stiffness; pain intensity ≥3; Kellgren-Lawrence 2–3° associated with X-ray; persistence of symptoms >3 months. We used the following tools: Western Ontario and McMaster Universities Arthritis Index, Numerical Pain Scale, Central Sensitization Inventory, and Pain Self-Efficacy Questionnaire. Intergroup comparisons were performed using the t-test. RESULTS: The sample consisted of 118 adult individuals, divided into two groups: bilateral knee osteoarthritis (n=59) and unilateral knee osteoarthritis (n=59). We observed a significant difference (p<0.05) and a large effect size (d≥0.8), in the comparisons between: stature, body mass index, physical function, central sensitization, and self-efficacy. CONCLUSION: Individuals with bilateral knee osteoarthritis have higher levels of central sensitization, impaired functionality, and a lower level of self-efficacy.
