ABSTRACT Hepatocellular carcinoma (HCC) is the third most common cause of cancer-related mortality worldwide. The Brazilian Society of Hepatology (SBH) published in 2020 the updated recommendations for the diagnosis and treatment of HCC. Since then, new data have emerged in the literature, including new drugs approved for the systemic treatment of HCC that were not available at the time. The SBH board conducted an online single-topic meeting to discuss and review the recommendations on the systemic treatment of HCC. The invited experts were asked to conduct a systematic review of the literature on each topic related to systemic treatment and to present the summary data and recommendations during the meeting. All panelists gathered together for discussion of the topics and elaboration of the updated recommendations. The present document is the final version of the reviewed manuscript containing the recommendations of SBH and its aim is to assist healthcare professionals, policy-makers, and planners in Brazil and Latin America with systemic treatment decision-making of patients with HCC.

Abstract Introduction: goldenhar syndrome is a rare congenital syndrome that affects the craniofacial morphogenesis. It is a complex syndrome, with heterogeneous presentation which the diagnosis can still be performed in the intrauterine through morphological ultrasound. Description: a case report of a 4-year-old male patient diagnosed with Goldenhar syndrome, along with its clinical presentation, diagnostic investigation and follow-up. Discussion: the follow-up on these patients remains a challenge, since it can affect different systems and with different presentations. The earlier the diagnosis is performed, the greater the patient’s chances of having a favorable prognosis with multidisciplinary stimulation. The objective of this article is to contribute to the medical literature, in order to assist in the diagnosis and management of future cases.

ABSTRACT: The downer cow syndrome (DCS) is characterized by an alert cow showing inability or reluctance to stand for 12 hours or more. This paper reported clinical, laboratory, and pathological findings in a Guzerá heifer with rhabdomyolysis, pigmenturia and acute renal failure following DCS. A 17-month-old Guzerá heifer was transported via a 350-km ride in a truck and showed sternal recumbency and severe difficulty in standing and walking. Neurological examination was unremarkable, and the heifer presented normal response to cranial nerves and spinal cord tests. Rectal palpation revealed a 5-month gravid uterus. No other abnormalities were noted in the pelvis or around the coxofemoral joints. Biochemical abnormalities included extremely high muscular enzyme activities (creatine phosphokinase and aspartate aminotransferase) and high creatinine levels. Urinalysis revealed blackish and cloudy urine, proteinuria, and a positive occult blood test. Spinal cord ultrasonography showed no abnormalities. This report highlighted an uncommon clinical presentation (myoglobinuria) and pathological findings in a heifer with DCS as a consequence of severe compressive muscle damage. Practitioners and producers must be aware of the risk of careless road transportation for long distances of cattle, especially obese cows, avoiding unnecessary suffering and expenses due to DCS.

ABSTRACT Prune belly syndrome is a rare congenital disease of unknown etiology that is present in one in every 40 thousand live births, and predominantly affects males, at a ratio of 4:1. In males, it presents with anomalies in the urinary system, absence of abdominal muscles, bilateral cryptorchidism, and infertility. In women, the syndrome has variable presentations, but fertility is preserved. Searching the medical literature, we found only one case of prune belly syndrome in pregnant women. Therefore, the patient in this report is the second case. She was primiparous, 25-years-old, with no abdominal muscles, severe congenital kyphoscoliosis, and pulmonary restriction. Elective cesarean section was performed at 37 weeks of gestation due to maternal risk of uterine rupture by transverse presentation and fetal risk of intrauterine growth restriction. The pre-anesthetic approach defined that general anesthesia might have more risks for the patient due to severe maternal lung disease compared to ultrasound-guided locoregional anesthesia. During prenatal care, there were some maternal complications, such as asthma exacerbations, abdominal pain, and constipation. The newborn was born small for gestational age and this can possibly be explained by maternal restrictive lung capacity. The newborn presented with Apgar score 8/9 and tachypnea, but improved after two hours of life.

ABSTRACT The muscular performance of the lumbar spine is frequently assessed in studies that compare populations with and without low back pain. However, the gold standard equipment used to evaluate the strength of these muscles has a high cost. Therefore, rarely used in the clinical practice of health professionals, being the handheld dynamometer an accessible alternative for use in clinical practice. Therefore, this study aimed to determine the reliability of the handheld dynamometer in the measurement of maximum voluntary isometric contraction of trunk extensors in healthy adults. Twenty-six healthy adults of both sexes (21.53± 1.88 years old) performed the test and retest with a 1-week interval. The comparison between test-retest demonstrated high and very high intraclass correlation coefficient for peak strength (0.82; 0.60 to 0.92) and mean strength (0.90; 0.76 to 0.95), respectively. Also, the Bland-Altman analyses indicate an error of 5.10 kg for peak strength and 5.7 kg for mean strength. In addition, the minimum detectable change for peak strength was 6.03kg (16.18%) and 5.92 kg for mean strength (20.95%). In conclusion, the test presents high reliability for the extensor muscles of the trunk in healthy adults. Therefore, the use of the handheld dynamometer can be indicated for clinical practice.

Abstract Herpetic whitlow is a viral infection of the fingers caused by the herpes simplex virus. The disease has a bimodal age distribution, affecting children under 10 years of age and young adults between 20 and 30 years old. It can be easily mistaken for panaritium or bacterial cellulitis. In patients with AIDS, atypical, chronic and recurrent ulcerated lesions occur. The Tzanck test allows a quick and low-cost diagnosis of herpes simplex virus infection. The authors report the case of a child with AIDS with painful finger ulcers in which the diagnosis was confirmed by the Tzanck test.

Abstract Background: There is a lack of information on the role of chronic use of hydroxychloroquine during the SARS-CoV-2 outbreak. Our aim was to compare the occurrence of COVID-19 between rheumatic disease patients on hydroxychloroquine with individuals from the same household not taking the drug during the first 8 weeks of community viral transmission in Brazil. Methods: This baseline cross-sectional analysis is part of a 24-week observational multi-center study involving 22 Brazilian academic outpatient centers. All information regarding COVID-19 symptoms, epidemiological, clinical, and demographic data were recorded on a specific web-based platform using telephone calls from physicians and medical students. COVID-19 was defined according to the Brazilian Ministry of Health (BMH) criteria. Mann-Whitney, Chi-square and Exact Fisher tests were used for statistical analysis and two binary Final Logistic Regression Model by Wald test were developed using a backward-stepwise method for the presence of COVID-19. Results: From March 29th to May 17st, 2020, a total of 10,443 participants were enrolled, including 5166 (53.9%) rheumatic disease patients, of whom 82.5% had systemic erythematosus lupus, 7.8% rheumatoid arthritis, 3.7% Sjögren’s syndrome and 0.8% systemic sclerosis. In total, 1822 (19.1%) participants reported flu symptoms within the 30 days prior to enrollment, of which 3.1% fulfilled the BMH criteria, but with no significant difference between rheumatic disease patients (4.03%) and controls (3.25%). After adjustments for multiple confounders, the main risk factor significantly associated with a COVID-19 diagnosis was lung disease (OR 1.63; 95% CI 1.03-2.58); and for rheumatic disease patients were diagnosis of systemic sclerosis (OR 2.8; 95% CI 1.19-6.63) and glucocorticoids above 10 mg/ day (OR 2.05; 95% CI 1.31-3.19). In addition, a recent influenza vaccination had a protective effect (OR 0.674; 95% CI 0.46-0.98). Conclusion: Patients with rheumatic disease on hydroxychloroquine presented a similar occurrence of COVID-19 to household cohabitants, suggesting a lack of any protective role against SARS-CoV-2 infection. Trial registration Brazilian Registry of Clinical Trials (ReBEC; RBR - 9KTWX6).

Aims: To identify distinct asthma control clusters based on Control of Allergic Rhinitis and Asthma Test (CARAT) and to compare patients’ characteristics among these clusters. Methods: Adults and adolescents (≥13 years) with persistent asthma were recruited at 29 Portuguese hospital outpatient clinics, in the context of two observational studies of the INSPIRERS project. Demographic and clinical characteristics, adherence to inhaled medication, beliefs about inhaled medication, anxiety and depression, quality of life, and asthma control (CARAT, >24 good control) were collected. Hierarchical cluster analysis was performed using CARAT total score (CARAT-T). Results: 410 patients (68% adults), with a median (percentile 25-percentile 75) age of 28 (16-46) years, were analysed. Three clusters were identified [mean CARAT-T (min-max)]: cluster 1 [27(24-30)], cluster 2 [19(14-23)] and cluster 3 [10(2-13)]. Patients in cluster 1 (34%) were characterised by better asthma control, better quality of life, higher inhaler adherence and use of a single inhaler. Patients in clusters 2 (50%) and 3 (16%) had uncontrolled asthma, lower inhaler adherence, more symptoms of anxiety and depression and more than half had at least one exacerbation in the previous year. Furthermore, patients in cluster 3 were predominantly female, had more unscheduled medical visits and more anxiety symptoms, perceived a higher necessity of their prescribed inhalers but also higher levels of concern about taking these inhalers. There were no differences in age, body mass index, lung function, smoking status, hospital admissions or specialist physician follow-up time among the three clusters. Conclusion: An unsupervised method based on CARAT-T, identified 3 clusters of patients with distinct, clinically meaningful characteristics. The cluster with better asthma control had a cut-off similar to the established in the validation study of CARAT and an additional cut-off seems to distinguish more severe disease. Further research is necessary to validate the asthma control clusters identified.

ABSTRACT: This study produced pectin microcapsules containing Lactobacillus acidophilus by external ionic gelation, followed by the adsorption of whey protein and pectin to form multilayers. The viability of free and microencapsulated lactobacilli was evaluated after in vitro exposure to gastrointestinal conditions. They were also assessed by heat treatment, and stability was examined at -18 °C, 5 °C and 25 °C for 120 days. Exposure to different pHs, simulating passage through the gastrointestinal tract, showed that treatment of the microcapsules with only pectin (LA/P0) and with one and two layers of whey protein (treatments LA/P1 and LA/P3, respectively), were able to protect Lactobacillus acidophilus , with microcapsules increasing the release of probiotics from the stomach into the intestines. Free cells showed a decrease in their counts over the course of the simulated gastrointestinal system. Regarding heat treatments, microcapsules with a layer of whey protein (LA/P1) maintained the viability of their encapsulated Lactobacillus acidophilus (9.57 log CFU/g-1). The best storage viability was at -18 °C, with a count of 7.86 log CFU/g-1at 120 days for microcapsule LA/P1,with those consisting of two layers of whey protein (LA/P3)having a 6.55 log CFU/g-1 at 105 days. This study indicated that external ionic gelation was effective and could be used for the production of pectin microcapsules, with multilayer whey protein promoting greater protection and viability of Lactobacillus acidophilus.

ABSTRACT Since few studies have addressed the embryonic development of Guinea fowl (Numida meleagris), the objective of the present study was to evaluate its embryonic development in the Cerrado region of Brazil and compare the results to published descriptions of the embryonic development of other domestic bird species. The commercialized weight for Guinea fowl eggs used in the experiment was found to be 37.57 g, while egg fertility was 92%. Embryo growth rate (%) was higher on the sixth day of incubation relative to other days. The heart began beating on the third day of development, while eye pigmentation and upper and lower limb buds appeared on the sixth day. The yolk sac was incorporated on the 24th day followed by chick external pipping and hatching on the 28th day. Comparisons revealed that the embryonic development of Guinea fowl exhibits both similarities to, and differences from, the embryonic development of other domestic birds. From three days of embryonic development, differences were observed among the compared species. Despite the same incubation period, Guinea fowl embryos develop faster than turkeys. All species begin internal pecking two days before hatching and internal pecking 24 h before hatching.

ABSTRACT Smoking is the leading cause of respiratory disease (RD). The harmful effects of smoking on the respiratory system begin in utero and influence immune responses throughout childhood and adult life. In comparison with “healthy” smokers, smokers with RD have peculiarities that can impede smoking cessation, such as a higher level of nicotine dependence; nicotine withdrawal; higher levels of exhaled carbon monoxide; low motivation and low self-efficacy; greater concern about weight gain; and a high prevalence of anxiety and depression. In addition, they require more intensive, prolonged treatment. It is always necessary to educate such individuals about the fact that quitting smoking is the only measure that will reduce the progression of RD and improve their quality of life, regardless of the duration and severity of the disease. Physicians should always offer smoking cessation treatment. Outpatient or inpatient smoking cessation treatment should be multidisciplinary, based on behavioral interventions and pharmacotherapy. It will thus be more effective and cost-effective, doubling the chances of success.

The objective of this study was to evaluate the structural, productive and nutritional value of Tifton 85 grass fertilized with two nitrogen doses at four regrowth ages. The experimental design was a randomized block in a factorial 4 x 2, with four regrowth ages (28, 35, 42 and 49 days) and two levels of nitrogen (100 and 300 kg ha-1) in six replicates, totaling 48 experimental units, of 25 m² (5x5m) each. The blocking criterion was the variation in the experimental area. Dry matter production increased linearly, on average 56.4%, as the regrowth age increased from 28 to 49 days (P<0.01). Among the N rates, there was a 17.2% increase in dry matter yield when fertilized with 300 kg / ha of nitrogen. The height of the plants (P <0.01), as well as the number of live leaves (P <0.01), number of expanded leaves (P <0.01) and the final leaf length (P <0.01) increased linearly with the advancement of cut age. Fertilization of Tifton 85 grass with 100 kg ha-1 of N, managed every 35 days improves the dry matter production, structural characteristics and nutritional value of forage for cutting and / or grazing.

ABSTRACT Objectives To identify the offering of complementary foods to children aged below two years old, and the conformity of this with the Ministry of Health’s recommendations. Method a study undertaken with 52 children in a Primary Healthcare Center in the North East of Brazil, between December 2015 and February 2016. Interviews were held with the mothers, who were given a socioeconomic questionnaire which was also related to the children’s dietary profile. The data were analyzed using the statistical program R®. The foods consumed were classified as either “not appropriate” or “appropriate”, according to the Ministry of Health. Results of the children older than one year, one (3.3%) was receiving complementary breastfeeding, while those younger than one year old (n=22/100%) were eating baby porridge (‘mingau’*) and 20 (90.9%) had already drunk soda. Soda, mingau, salted snacks and cookies were the foods consumed most. “Inappropriate” food was prevalent in the children younger than one year old. Conclusion most of the food given consisted of industrialized products, and is thus a problem of food insecurity for the study population.

ABSTRACT BACKGROUND AND OBJECTIVES: Temporomandibular joint dysfunction is a chronic condition that affects a large part of the college population, deserving greater importance in public health due to the negative consequences in students’ performance and in the quality of life of these individuals. The objective of this study is to check the association and correlation of temporomandibular dysfunction symptoms and signs with depression in students from the School of Science and Technology of Maranhão. METHODS: Epidemiological, descriptive and cross-sectional study with the quantitative approach with 199 undergraduate students. For data collection, we used Fonseca’s Anamnestic Index and the Research Diagnostic Criteria for Temporomandibular Disorders, Axis II. Data were analyzed using Pearson’s Chi-square test, Fisher’s Exact test, and Spearman’s Correlation, considering a statistical significance of 5%. RESULTS: 37.7% students showed signs and symptoms of mild temporomandibular dysfunction, 19.6% moderate and 9% severe. Temporomandibular dysfunction was more frequent in female students (70.2%) between the ages of 19 to 24 years (70.2%). When assessing the level of depression symptoms, it was found that 25.1% students had moderate depression and 17.1% severe. There was no statistically significant association (p<0.001) and weak positive correlation (r=0.38; p<0.001) between the presence of temporomandibular dysfunction signs and symptoms and depression. CONCLUSION: The results of this study suggest that there is a relationship between temporomandibular dysfunction and depression in undergraduate students, with higher prevalence in women, however, more specific diagnostic methods are necessary to confirm this relationship and the need for treatment.

Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.
