European health literacy survey questionnaire short form (HLS-Q12): adaptation and evidence of validity for the Brazilian context

Abstract Health literacy (HL) refers to knowledge, motivation and skills to understand, evaluate and apply health information, enabling appropriate decision making in daily life on health care and health promotion. Studies show that HL is associated with several social determinants, health outcomes, and health promotion. In Brazil, studies on the thematic are still scarce. Thus, the present study aimed to adapt, seek evidence of validity, reliability and estimate the parameters of the items of the European Health Literacy Survey Questionnaire Short Form (HLS-Q12) for the Brazilian context. 770 individuals participated, recruited through advertisements in the media and social networks, 82.1% female, aged between 18 and 83 (M = 35.5, SD = 13.52), from 21 Federative Units of Brazil and the Federal District. The subjects answered the HLS-Q12 and a sociodemographic questionnaire. Exploratory factor analysis indicated a unifactorial structure with good psychometric characteristics (GFI = 0.98; CFI = 0.98; RMSEA = 0.08; RMSR = 0.07). Cronbach's alpha, Guttman's lambda 2 and McDonald's omega reliability indicators were equal to 0.87. We conclude that the HLS-Q12 is an adequate instrument to assess the level of HL in the Brazilian population.

OBJECTIVES: This observational, cross-sectional study based aimed to test whether heart failure (HF)-disease management program (DMP) components are influencing care and clinical decision-making in Brazil. METHODS: The survey respondents were cardiologists recommended by experts in the field and invited to participate in the survey via printed form or email. The survey consisted of 29 questions addressing site demographics, public versus private infrastructure, HF baseline data of patients, clinical management of HF, performance indicators, and perceptions about HF treatment. RESULTS: Data were obtained from 98 centers (58% public and 42% private practice) distributed across Brazil. Public HF-DMPs compared to private HF-DMP were associated with a higher percentage of HF-DMP-dedicated services (79% vs 24%; OR: 12, 95% CI: 94-34), multidisciplinary HF (MHF)-DMP [84% vs 65%; OR: 3; 95% CI: 1-8), HF educational programs (49% vs 18%; OR: 4; 95% CI: 1-2), written instructions before hospital discharge (83% vs 76%; OR: 1; 95% CI: 0-5), rehabilitation (69% vs 39%; OR: 3; 95% CI: 1-9), monitoring (44% vs 29%; OR: 2; 95% CI: 1-5), guideline-directed medical therapy-HF use (94% vs 85%; OR: 3; 95% CI: 0-15), and less B-type natriuretic peptide (BNP) dosage (73% vs 88%; OR: 3; 95% CI: 1-9), and key performance indicators (37% vs 60%; OR: 3; 95% CI: 1-7). In comparison to non- MHF-DMP, MHF-DMP was associated with more educational initiatives (42% vs 6%; OR: 12; 95% CI: 1-97), written instructions (83% vs 68%; OR: 2: 95% CI: 1-7), rehabilitation (69% vs 17%; OR: 11; 95% CI: 3-44), monitoring (47% vs 6%; OR: 14; 95% CI: 2-115), GDMT-HF (92% vs 83%; OR: 3; 95% CI: 0-15). In addition, there were less use of BNP as a biomarker (70% vs 84%; OR: 2; 95% CI: 1-8) and key performance indicators (35% vs 51%; OR: 2; 95% CI: 91,6) in the non-MHF group. Physicians considered changing or introducing new medications mostly when patients were hospitalized or when observing worsening disease and/or symptoms. Adherence to drug treatment and non-drug treatment factors were the greatest medical problems associated with HF treatment. CONCLUSION: HF-DMPs are highly heterogeneous. New strategies for HF care should consider the present study highlights and clinical decision-making processes to improve HF patient care.

RESUMO Introdução: A clientela crítica encontra-se exposta à riscos. Assim, compreender as relações entre os elementos que envolvem a segurança e proteção torna-se fundamental. Objetivo: verificar a associação entre os diagnósticos de enfermagem do domínio segurança e proteção da NANDA Internacional com seus componentes em pacientes críticos internados em unidade de terapia intensiva. Métodos: estudo transversal, realizado com 86 pacientes críticos de um hospital no Nordeste do Brasil, por meio do formulário. Na análise estatística, foram aplicados os testes de Qui-quadrado e exato de Fisher. Resultados: O diagnóstico Risco de resposta adversa ao meio de contraste com iodo apresentou associação estatística com os componentes: Propriedades físicas e químicas dos meios de contraste, Ansiedade, Uso concomitante de medicamentos, Veias fragilizadas, Doença subjacente e Extremos de idade. O diagnóstico Risco de desequilíbrio na temperatura corporal associou-se à taxa metabólica alterada, extremos de idade e sedação. Conclusão: Esses achados proporcionam avanço científico para a enfermagem ao fornecerem auxílio na inferência diagnóstica, na previsão dos cuidados e na tomada de decisão do enfermeiro.
ABSTRACT Introduction: Critically-ill patients, admitted to an intensive care unit, are exposed to risks. Therefore, understanding the relationships between the elements involving their safety and protection becomes fundamental. Objective: To verify the association between the nursing diagnoses within the domain safety and protection of NANDA (North American Nursing Diagnosis Association) International with its components in critically-ill patients hospitalized in an intensive care unit. Methods: Cross-sectional study carried out through a form conducted on 86 critically-ill patients of a hospital in the northeast of Brazil. The chi-square and Fisher's exact tests were applied in the statistical analysis. Results: The diagnosis of the risk of adverse response to the contrast medium with iodine presented statistical association with the components: physical and chemical properties of the contrast media, anxiety, concomitant use of medications, fragile veins, underlying disease, and age extremes. The diagnosis of the risk of imbalance in body temperature was associated with the altered metabolic rate, age extremes, and sedation. Conclusion: These findings provide scientific advances for nursing by providing help in diagnostic inference, in the anticipation of care, and in the decision-making by the nursing staff.
RESUMEN Introducción: Los pacientes en estado crítico, internados en una unidad de terapia intensiva, se encuentran expuestos a riesgos. Así, comprender las relaciones entre los elementos que involucran su seguridad y su protección se vuelve fundamental. Objetivo: Verificar la asociación entre los diagnósticos de enfermería del dominio seguridad y protección de la NANDA Internacional con sus componentes en pacientes críticos internados en unidad de terapia intensiva. Métodos: Estudio transversal, realizado con 86 pacientes críticos de un hospital en el Nordeste de Brasil, a través del formulario. En el análisis estadístico, se aplicaron las pruebas de Qui-cuadrado y exacto de Fisher. Resultados: El diagnóstico Riesgo de respuesta adversa al medio de contraste con yodo presentó asociación estadística con los componentes: Propiedades físicas y químicas de los medios de contraste, Ansiedad, Uso concomitante de medicamentos, Venas fragilizadas, Enfermedad subyacente y Extremos de edad. Desde 1990, intensificou-se o movimento das organizações internacionais na construção do sub-regime internacional de pesquisa, desenvolvimento e inovação em saúde. Este artigo analisa as prioridades dos países em desenvolvimento para essa área desde a década de 1990. Estudo descritivo-analítico, estabelece um diálogo entre a literatura e as contribuições de informantes-chave. Defendendo a superação da iniquidade mundial no fomento científico-tecnológico em saúde, as organizações internacionais recomendam aos países em desenvolvimento focalizar suas ações em doenças negligenciadas e pesquisa operacional - agenda insuficiente para a cooperação científico-tecnológica ser fator de superação de vulnerabilidades entre países
Since 1990, international organizations have been increasingly involved in building an international sub-regime for research, development and innovation in health. This article analyzes the priorities of developing countries in health since the 1990s. It is a descriptive and analytical study that investigates the literature and contributions of key informants. Calling for the end of global inequities in the support for science and technology in health, international organizations recommend that developing countries focus their efforts on neglected diseases and operational research, an insufficient agenda for science and technology cooperation to effectively overcome the vulnerabilities between countries

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Processing and characterization of extruded breakfast meal formulated with broken rice and bean flour

O objetivo deste trabalho foi elaborar um produto matinal extrusado de quirera de arroz e bandinha de feijão, além de verificar a influência do processo de extrusão nas suas características físico-químicas, nutricionais, tecnológicas e sensoriais. O produto final apresentou teor considerável de proteínas (9,9 g.100 g-1), podendo ser considerado uma boa fonte desse nutriente para crianças e adolescentes. Para a fibra alimentar, observou-se teor de 3,71 g.100 g-1 do produto pronto para o consumo. Dessa forma, o floco matinal de arroz e feijão pode receber a alegação de alimento fonte de fibras, de acordo com a legislação brasileira. Com relação às propriedades tecnológicas, o extrusado estudado apresentou índice de expansão de 8,89 e densidade aparente de 0,25 g.cm-3. Quanto à análise sensorial, o floco matinal avaliado obteve notas médias de aceitação, situadas no intervalo de 6,8 a 7,7, que corresponde às categorias "gostei ligeiramente" e "gostei muito". Para a intenção de compra, 79% dos provadores opinaram que certamente ou possivelmente comprariam o produto. O emprego de quirera de arroz e bandinha de feijão é uma interessante alternativa para a elaboração de produto matinal extrusado, apresentando boas qualidades de ordem nutricional, tecnológica e sensorial.
The objective of this work was to develop an extruded breakfast product containing broken rice and split old beans and to verify the influence of the extrusion process on their physicochemical, technological, and sensory characteristic. The final product had a protein content of 9.9 g.100 g-1, and therefore it can be considered a good source of proteins for children and teenagers. The dietary fiber content of the final edible product was 3.71 g.100 g-1. Therefore, the breakfast meal may be considered as a source of dietary fiber according to Brazilian law . As for the technological properties, the extruded product presented an expansion index of 8.89 and apparent density of 0.25 g.cm-3. With regard to the sensory analysis, the acceptance average was ranked between 6.8 and 7.7, corresponding to the categories "liked slightly" and "liked very much". With regard to purchase intention, 79% of the panelists said they would certainly or possibly purchase the product. Broken rice and split old beans are interesting alternatives for the elaboration of extruded breakfast products presenting good nutritional, technological, and sensory qualities.

OBJETIVOS: avaliar o perfil da atividade eletromiográfica do músculo bíceps braquial de recém nascidos pré-termo (RNPT) antes de realizarem a posição canguru e até 48h após a permanência nesta posição. MÉTODOS: coorte prospectivo, avaliando 20 RNPT internados no alojamento mãe canguru do Instituto de Medicina Integral Prof. Fernando Figueira (IMIP), Recife, PE. A atividade eletromiográfica do músculo bíceps braquial foi avaliada imediatamente antes de sua introdução à posição canguru, sendo acompanhados a cada 24h durante três dias consecutivos. O registro era feito no período de um minuto com eletromiógrafo. Eram colocados eletrodos auto-colantes entre o ponto motor e a junção miotendínea. Para comparação dos valores médios foi usado análise de variância para medidas repetidas. RESULTADOS: a atividade eletromiográfica diferiu estatisticamente entre os intervalos [F(3,67)=6,01; p=0,005], sendo que em 48 h foi maior do que em 0h, mas não em 24 h. CONCLUSÕES: a posição canguru é capaz de induzir um aumento da atividade eletromiográfica em crianças pré-termo.
OBJECTIVES: to evaluate the profile of the electromyographic activity of the brachial biceps in preterm newborns (PTNBs) before being placed in the Kangaroo position and after 48 hours in this position. METHODS: a propspective cohort evaluating 20 PTNBs admitted to the Kangaroo Mother Unit of the Instituto de Medicina Integral Prof. Fernando Figueira (IMIP), Recife, PE. The electromyographic activity of the brachial biceps muscle was evaluated immediately before being placed in the Kangaroo position and checked every 24h for three consecutive days. The measurement was taken for one minute using an electromyograph. Self-adhesive electrodes were placed between the motor point and the myotendinous junction. To compare the mean values, analysis of variance for repeated measurements was used. RESULTS: electromyographic activity differed to a statistically significant degree from one interval to another [F(3,67)=6,01; p=0,005], being greater after 48 h than at 0 h, but not after 24 h. CONCLUSIONS: the Kangaroo position is capable of bringing about an increase in electromyographic activity in preterm children.

Karyotype differentiation of four Cestrum species (Solanaceae) revealed by fluorescent chromosome banding and FISH

The karyotypes of four South American species of Cestrum (C. capsulare, C. corymbosum, C. laevigatum and C. megalophylum) were studied using conventional staining, C-CMA/DAPI chromosome banding and FISH with 45S and 5S rDNA probes. The karyotypes showed a chromosome number of 2n = 2x = 16, with metacentric chromosomes, except for the eighth submeta- to acrocentric pair. Several types of heterochromatin were detected, which varied in size, number, distribution and base composition. The C-CMA+ bands and 45S rDNA were located predominantly in terminal regions. The C-CMA+/DAPI+ bands appeared in interstitial and terminal regions, and the C-DAPI+ bands were found in all chromosome regions. The 5S rDNA sites were observed on the long arm of pair 8 in all species except C. capsulare, where they were found in the paracentromeric region of the long arm of pair 4. The differences in band patterns among the species studied here, along with data from other nine species reported in the literature, suggest that the bands are dispersed in an equilocal and non-equilocal manner and that structural rearrangements can be responsible for internal karyotype diversification. However, it is important to point out that the structural changes involving repetitive segments did not culminate in substantial changes in the general karyotype structure concerning chromosome size and morphology.
