Resumo Introdução: As patologias músculo-esqueléticas constituem um dos motivos mais frequentes de consulta em medicina geral e familiar. A elevada incidência destas patologias traduz-se também numa elevada necessidade de cuidados de reabilitação pelo que, em 2021, a prescrição de medicina física e reabilitação (MFR) destacou-se como o segundo maior gasto relativamente ao total da despesa com meios complementares de diagnóstico e terapêutica na região Norte. Assim, importa caracterizar os principais motivos de prescrição. Objetivos: Caracterizar a prescrição de consultas de MFR relativas a patologia osteoarticular e músculo-esquelética no Agrupamento de Centros de Saúde de Matosinhos (ACeS). Métodos: Estudo observacional e retrospetivo de todas as prescrições de MFR realizadas de janeiro a dezembro/2021 em todas as unidades de saúde constituintes de um ACeS, com caracterização sociodemográfica dos utentes e análise do número de prescrições e códigos de Classificação Internacional de Cuidados de Saúde Primários associados às mesmas. Resultados: O número médio de prescrições por USF foi de 1710,07. Verificou-se que 47,94% das prescrições corresponderam a primeira consulta de MFR, enquanto 52,06% corresponderam a consulta subsequente. A maioria das prescrições foi feita a utentes do sexo feminino (65,54%), com isenção do pagamento de taxas moderadoras (58,65%) e idade média de 57,15 anos. O problema de saúde mais frequentemente codificado foi L86 - Síndrome da Coluna com irradiação de dor, seguindo-se L92 - Síndrome do Ombro Doloroso, quer para primeira consulta quer para consulta subsequente. As patologias que originaram maior número de prescrições subsequentes/utente foram esclerose múltipla e parkinsonismo. Observou-se ainda que 46,76% das prescrições se acompanhavam de código ICPC-2 correspondente a diagnóstico passível de prescrição de fisioterapia. Conclusão: Trata-se de um estudo relevante que caracterizou as prescrições de MFR, permitindo conhecer as patologias que levaram à requisição destes cuidados, podendo constituir um ponto de partida para melhorar a adequação dos recursos em saúde.

Abstract Introduction: Musculoskeletal disorders are one of the most frequent reasons for medical appointments in primary care. The high incidence of these pathologies also translates into a high need for rehabilitation care. This is why, in 2021, the prescription of physical and rehabilitation medicine (PRM) stood out as the second highest expense in the total costs of complementary means of diagnostic and therapeutic, in the North of Portugal. Thus, it is important to describe the main reasons for prescription. Objective: To describe the prescriptions of PRM appointments related to osteoarticular and musculoskeletal pathology, in the Matosinhos group of Family Health Units (FHUs). Methods: Observational and retrospective study of all PRM prescriptions carried out from January to December 2021 in 14 FHUs, with sociodemographic characterization of users and analysis of the number of prescriptions and codes of the International Classification of Primary Health Care associated with them. Results: The mean number of prescriptions per FHU was 1710.07. It was found that 47.94% of the prescriptions corresponded to the first PRM appointment, while 52.06% corresponded to the subsequent appointments. Most prescriptions were made to female patients (65.54%), exempted from paying user fees (58.65%), and mean age of 57.15 years. The most frequently coded health problem was L86 - Painful Spine Syndrome, followed by L92 - Painful Shoulder Syndrome, both for first and subsequent appointments. The pathologies that originated the highest number of subsequent prescriptions per user were multiple sclerosis and parkinsonism. It was also observed that 46.76% of the prescriptions were accompanied by an ICPC-2 code corresponding to a valid diagnosis for physiotherapy prescription. Conclusion: This is a relevant study that characterized PRM prescriptions and allowed us to understand the pathologies that led to the need for this care, which may constitute a starting point for improving the adequacy of health resources.

Resumo A sexagem das espécies é uma ferramenta importante para o manejo populacional. Em tartarugas, cágados e jabutis, a sexagem geralmente é feita de acordo com características secundárias, como morfologia do plastrão ou técnicas de eversão do falo. Neste artigo, apresentamos o método de eversão do falo e comparamos sua eficiência com outras técnicas existentes. Este novo método visa encurtar o tempo de manejo e reduzir os efeitos colaterais físicos nos animais, promovendo o manejo clínico e reprodutivo do cágado-de-barbicha (Phrynops geoffroanus) e espécies relacionadas. Uma amostra de cinquenta indivíduos foi coletada na região central do estado do Piauí, Nordeste do Brasil. O método proposto consiste em remover a cabeça de sua posição de repouso. Os métodos que obtiveram índice de sucesso superior a 80% foram comparados entre si quanto ao tempo de estimulação necessário para a eversão do falo. O método aqui proposto alcançou 100% de eficiência na determinação do sexo de espécimes com comprimento de carapaça linear entre 10 e 34 cm. Além disso, quando comparado ao segundo método mais eficiente, reduziu substancialmente o tempo necessário para o manejo dos espécimes (91%), evitando possíveis sequelas nos indivíduos manuseados.

Abstract The sexing of species is an important tool for population management. In tortoises and turtles, sexing is usually done according to secondary characteristics, such as plastron morphology or phallus eversion techniques. In this paper, we present the phallus eversion method and compare its efficiency to other existing techniques. This new method aims at shortening the management time and reducing the physical aftereffects on the animals, fostering the clinical and reproductive management of Geoffroy’s side-necked turtle (Phrynops geoffroanus) and related species. A sample of fifty individuals was taken in the central region of Piauí state, Northeastern Brazil. The proposed method consisted of moving the head from its resting position. The methods achieving a success rate higher than 80% were compared to each other as for the stimulation time required for phallus eversion. The method as here proposed achieved 100% efficiency in the sex determination of specimens with a linear carapace length between 10 and 34 cm. Also, when compared to the second most efficient method, it substantially reduced the time needed for managing the specimens (91%), avoiding possible sequelae in the individuals handled.

Abstract Introduction: The articulation between primary care and secondary care and the Pulmonology Diagnostic Centers is particularly important in the management of an insidious disease with a great impact on public health such as tuberculosis. The present case highlights the role of the family doctor in the articulation of care and its importance in the identification and guidance of a possible tuberculosis condition. Case description: Female adolescent, 16 years old, with a history of autoimmune thyroiditis under levothyroxine, iron deficiency anemia under supplementation, and anxiety disorder. Refers to a consultation with the family doctor in 2019 due to a cervical adenomegaly with two weeks of evolution, without warning signs, and an anti-inflammatory was prescribed. Referral to pediatric surgery consultation at a central hospital through urgent pediatric care at the referral hospital, due to persistent adenomegaly after empirical antibiotic treatment and normal analytical study. An aspiration biopsy was performed five months later, with discharge from the consultation due to the absence of malignancy, with no other relevant information. Observed in an emergency context at the referral hospital, six months later, due to new cervical adenomegaly, an alert was made by the family doctor after the detection of isolation of Mycobacterium tuberculosis in a lymph node biopsy performed at the central hospital. Consequent follow-up at the Pneumological Diagnosis Center for lymph node and pulmonary tuberculosis, having undergone lymph node excision and under anti-bacillary treatment, with a favorable clinical outcome. Comment: Tuberculosis is a particular clinical challenge due to the diversity of presentations. This case demonstrates the complexity of managing these patients and the importance of creating alerts in case of a positive mycobacteriological result. The role of the family doctor, as it is often the patient's first contact with healthcare and the bridge to other services, entails an important responsibility in identifying these patients and preventing the chain of transmission.

Resumo Introdução: A articulação entre os cuidados de saúde primários e secundários e os Centros de Diagnóstico Pneumológico reveste-se de particular importância na gestão de uma doença insidiosa e com grande impacto na saúde pública como a tuberculose. O presente caso destaca o papel do médico de família na articulação de cuidados e a importância desta para a identificação e orientação de um possível quadro de tuberculose. Descrição do caso: Adolescente do sexo feminino, 16 anos de idade, com antecedentes de tiroidite autoimune sob levotiroxina, anemia ferropénica sob suplementação e perturbação da ansiedade. Recorre a consulta com a médica de família em 2019 por adenomegalia cervical com duas semanas de evolução, sem sinais de alarme, tendo sido prescrito anti-inflamatório. Referenciação a consulta de cirurgia pediátrica de um hospital central, por indicação do atendimento pediátrico urgente do hospital de referência, por persistência da adenomegalia após tratamento antibiótico empírico e estudo analítico normal. Realizada biópsia aspirativa cinco meses depois, com alta da consulta por ausência de malignidade, sem outras informações relevantes. Observada em contexto de urgência no hospital de referência, seis meses depois, por adenomegalias cervicais de novo, tendo sido feito alerta pela médica de família após deteção de isolamento de Mycobacterium tuberculosis em biópsia ganglionar. Consequente seguimento no Centro de Diagnóstico Pneumológico por tuberculose ganglionar e pulmonar, tendo sido submetida a exérese ganglionar e encontrando-se sob tratamento antibacilar, com evolução clínica favorável. Comentário: A tuberculose constitui um particular desafio clínico pela diversidade de apresentações. Este caso demonstra a complexidade da gestão destes doentes e a importância da criação de alertas em caso de resultado micobacteriológico positivo. O papel do médico de família, dado ser frequentemente o primeiro contacto do doente com os cuidados de saúde e a ponte com os restantes serviços, acarreta uma importante responsabilidade na identificação destes doentes e na prevenção da cadeia de transmissão.

Abstract This work describes the circulation over the continental slope and the São Paulo Plateau in the Santos Basin during the SANSED winter 2019 survey. The cruise consisted of four legs in the period between June, 11 and August, 03 2019. The observed circulation is dominated by the Atlantic southwestern boundary current regime and remotely-generated anticyclones and cyclones. The former is composed by the Brazil Current, the Intermediate Western Boundary Current and their mesoscale meanders; the latter are 300km vortical rings with origin in the eastern side of the South Atlantic Basin. A Lagrangian scheme applied over satellite altimeter maps indicate that the origin of these rings is primarily the Cape Basin of South Africa. The interaction between the boundary currents, their cyclonic meanders, and the anticyclonic rings is complex, and varies widely. During the SANSED winter 2019 survey period, three anticyclones interacted with the Brazil Current, instabilizing it, forming dipoles with the current cyclonic meanders, leading to their downstream propagation. Ancienter cyclonic eddies within Santos Basin may interfere with the propagation of the large anticyclones further south. In addition, the continuous arrival of remotely-originated anticyclones, the larger portion over the São Paulo Plateau presented a tendency of counter-clockwise circulation during the whole cruise period.

Resumen: El objetivo fue identificar patrones del síndrome metabólico en mujeres, estimar sus prevalencias y relaciones con características sociodemográficas y biológicas. Este estudio examinó 5.836 mujeres utilizando datos de la línea de base del Estudio Longitudinal de Salud del Adulto (ELSA-Brasil). Los patrones de síndrome metabólico se definieron a través del análisis de clase latente, utilizando las siguientes anormalidades metabólicas como indicadores: obesidad abdominal, hiperglucemia, hipertensión, hipertrigliceridemia y colesterol HDL reducido. Las relaciones entre los patrones de síndrome metabólico y las características individuales se evaluaron a través del análisis de clases latentes con covariables. Se identificaron tres patrones de síndrome metabólico, denominados “alta expresión metabólica”, “expresión metabólica moderada” y “baja expresión metabólica”. Los primeros dos patrones representan la mayoría (el 53,8%) de las mujeres del estudio. Las mujeres que tenían un nivel de escolaridad primario o secundario y que pertenecían a la clase social baja tuvieron una mayor probabilidad de presentar una expresión metabólica más alta. Los negros y pardos tuvieron una probabilidad más alta de presentar “expresión metabólica moderada”. Las mujeres en la menopausia que tenían 50 años o más presentaron una probabilidad más alta de tener patrones de mayor riesgo para la salud. Este estudio abordó la naturaleza heterogénea del síndrome metabólico, identificando tres perfiles diferentes para el síndrome entre las mujeres. La combinación de obesidad abdominal, hiperglucemia e hipertensión representa el principal perfil metabólico encontrado entre los participantes del ELSA-Brasil. Factores sociodemográficos y biológicos fueron importantes predictores para los patrones de síndrome metabólico.

Abstract: This study aimed to identify patterns of metabolic syndrome among women and estimate their prevalence and relationship with sociodemographic and biological characteristics. In total, 5,836 women were evaluated using baseline data from the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil). Patterns of metabolic syndrome were defined via latent class analysis, using the following metabolic abnormalities as indicators: abdominal obesity, hyperglycemia, hypertension, hypertriglyceridemia, and reduced HDL cholesterol. The relationship between these patterns and individual characteristics was assessed using latent class analysis with covariates. Three patterns of metabolic syndrome were identified: high metabolic expression, moderate metabolic expression, and low metabolic expression. The first two patterns represented most women (53.8%) in the study. Women with complete primary or secondary education and belonging to lower social classes were more likely to have higher metabolic expression. Black and mixed-race women were more likely to have moderate metabolic expression. Menopausal women aged 50 years and older were more often classified into patterns of greater health risk. This study addressed the heterogeneous nature of metabolic syndrome, identifying three distinct profiles for the syndrome among women. The combination of abdominal obesity, hyperglycemia, and hypertension represents the main metabolic profile found among ELSA-Brasil participants. Sociodemographic and biological factors were important predictors of patterns of metabolic syndrome.

Resumo: O objetivo foi identificar padrões de síndrome metabólica em mulheres, estimar suas prevalências e relações com características sociodemográficas e biológicas. Este estudo examinou 5.836 mulheres utilizando dados da linha de base do Estudo Longitudinal de Saúde do Adulto (ELSA-Brasil). Os padrões de síndrome metabólica foram definidos por meio de análise de classe latente, usando as seguintes anormalidades metabólicas como indicadores: obesidade abdominal, hiperglicemia, hipertensão, hipertrigliceridemia e colesterol HDL reduzido. As relações entre os padrões de síndrome metabólica e as características individuais foram avaliadas por meio da análise de classes latentes com covariáveis. Foram identificados três padrões de síndrome metabólica, denominados “alta expressão metabólica”, “expressão metabólica moderada” e “baixa expressão metabólica”. Os dois primeiros padrões representaram a maioria (53,8%) das mulheres do estudo. As mulheres com nível de escolaridade primário ou secundário e pertencentes à classe social baixa tiveram maior chance de apresentar maior expressão metabólica. Negros e pardos tiveram maior chance de apresentar “expressão metabólica moderada”. Mulheres na menopausa com 50 anos ou mais apresentaram maior chance de ter padrões de maior risco à saúde. Este estudo abordou a natureza heterogênea da síndrome metabólica, identificando três perfis distintos para a síndrome entre as mulheres. A combinação de obesidade abdominal, hiperglicemia e hipertensão representa o principal perfil metabólico encontrado entre os participantes do ELSA-Brasil. Fatores sociodemográficos e biológicos foram importantes preditores para os padrões de síndrome metabólica.

ABSTRACT Objective: Osteosarcoma lung metastases have a wide variety of CT presentations, representing a challenge for radiologists. Knowledge of atypical CT patterns of lung metastasis is important to differentiate it from benign lung disease and synchronous lung cancer, as well as to determine the extent of primary disease. The objective of this study was to analyze CT features of osteosarcoma lung metastasis before and during chemotherapy. Methods: Two radiologists independently reviewed chest CT images of 127 patients with histopathologically confirmed osteosarcoma treated between May 10, 2012 and November 13, 2020. The images were divided into two groups for analysis: images obtained before chemotherapy and images obtained during chemotherapy (initial CT examination). Results: Seventy-five patients were diagnosed with synchronous or metachronous lung metastases. The most common CT findings were nodules (in 95% of the patients), distributed bilaterally (in 86%), with no predominance regarding craniocaudal distribution (in 71%). Calcification was observed in 47%. Less common findings included intravascular lesions (in 16%), cavitation (in 7%), and the halo sign (in 5%). The primary tumor size was significantly greater (i.e., > 10 cm) in patients with lung metastasis. Conclusions: On CT scans, osteosarcoma lung metastases typically appear as bilateral solid nodules. However, they can have atypical presentations, with calcification being the most common. Knowledge of the typical and atypical CT features of osteosarcoma lung metastasis could play a key role in improving image interpretation in these cases.

RESUMO Objetivo: As metástases pulmonares do osteossarcoma têm uma grande variedade de apresentações tomográficas, o que é um desafio para os radiologistas. É importante conhecer os padrões tomográficos atípicos da metástase pulmonar para diferenciá-la de doença pulmonar benigna e câncer sincrônico de pulmão, bem como para determinar a extensão da doença primária. O objetivo deste estudo foi analisar as características tomográficas da metástase pulmonar do osteossarcoma antes da quimioterapia e durante a quimioterapia. Métodos: Dois radiologistas analisaram de modo independente as imagens de TC de tórax de 127 pacientes com osteossarcoma confirmado histopatologicamente, tratados entre 10 de maio de 2012 e 13 de novembro de 2020. As imagens foram divididas em dois grupos para a análise: imagens obtidas antes da quimioterapia e imagens obtidas durante a quimioterapia (TC inicial). Resultados: Setenta e cinco pacientes receberam diagnóstico de metástase pulmonar sincrônica ou metacrônica. Os achados tomográficos mais comuns foram nódulos (em 95% dos pacientes), distribuídos bilateralmente (em 86%), sem predominância de distribuição craniocaudal (em 71%). Observou-se calcificação em 47%. Alguns dos achados menos comuns foram lesões intravasculares (em 16%), escavação (em 7%) e o sinal do halo (em 5%). O tumor primário foi significativamente maior (> 10 cm) em pacientes com metástase pulmonar. Conclusões: Em imagens de TC, as metástases pulmonares do osteossarcoma tipicamente aparecem em forma de nódulos sólidos bilaterais. No entanto, podem ter apresentações atípicas, sendo a calcificação a mais comum. Conhecer as características tomográficas típicas e atípicas da metástase pulmonar do osteossarcoma pode ser fundamental para interpretar melhor as imagens nesses casos.

Abstract Many biological activities are described for the Clusiaceae family. Clusia fluminensis, a species from Brazilian flora, is mainly employed for ornamental purposes. This review aimed to depict the current knowledge of C. fluminensis from a bioprospecting standpoint. “Clusia fluminensis” search term was applied in Scopus, Web of Science, PubMed and Bireme databases according to PRISMA-ScR statement. Selected papers on Phytochemistry or Bioactivity followed hand searching procedures. Bioactivity preclinical studies considered in vitro or in vivo biological systems, treated with plant extracts or isolated compounds. The outcomes were compared with standard or no treatment control groups. Critical appraisal of individual trials considered completeness in the research fields. Our results showed that 81% of the selected papers presented high level of completeness, 69% revealed phytochemical parameters and 31% biological applications of plant extracts and isolated compounds. Polyisoprenylated benzophenones, terpenoids, sterols and phenolic compounds were identified. Antiviral, insecticidal and snake antivenom activities were reported. In conclusion, the phytochemical data reinforce the reported activities. Potential applications in personal care, nutritional supplementation and pharmaceutical, food, chemical or textile industries were also identified. Toxicological and phytochemical complementary studies may be required.

ABSTRACT: São Paulo state is one of the country’s largest producers of beef and milk, and the midwestern region plays a key role in this production, as half of São Paulo’s cattle herd is found in this region. These numbers alone demonstrate the importance of livestock in this region. Therefore, this study aimed to describe the main epidemiological and clinical signs in cattle cases at the Large Animal Hospital at FMVZ-Unesp, located in the midwestern region of São Paulo state. The present retrospective study assessed 638 clinical cases of cattle treated from January 2010 to December 2019 (10 years). Digestive system diseases were the most prevalent, diagnosed in 30.3% of patients, followed by neurological diseases (19.1%) and respiratory diseases (10.5%). The other diseases were distributed in decreasing order as follows: musculoskeletal (7.8%), hematopoietic (6.1%), genitourinary (5.6%), metabolic and nutritional (5.5%), neonatal (4.7%), cutaneous (2.6%), poisoning (2.5%), lymphatic (2.2%), cardiovascular (1.6%) and other diseases (1.4%). Rabies, a fatal zoonotic disease, was the main cause of death in this study and the main disease associated with neurological signs (23.7%). Recognizing the main diseases of cattle in this region will promote the adoption of prophylactic measures to minimize their occurrence and manage treatment to avoid economic losses and decreased productivity of herds.

RESUMO: O estado de São Paulo é um dos maiores produtores de carne e leite bovino do país e a região centro-oeste do estado tem papel fundamental nessa produção, pois possui a metade do rebanho de bovinos do estado. Portanto, este estudo descreve os principais achados clínicos e epidemiológicos de bovinos atendidos no Hospital de Grandes Animais da FMVZ/Unesp, localizado na região centro-oeste paulista. Foi realizado um levantamento nos arquivos da Clínica de Grandes Animais do Hospital Veterinário da FMVZ-Unesp, Botucatu/SP, dos 638 casos clínicos de bovinos atendidos de janeiro de 2010 a dezembro de 2019. Enfermidades do sistema digestório foram as mais prevalentes (30,3%), seguidas das doenças neurológicas (19,1%) e respiratórias (10,5%). As demais enfermidades foram distribuídas, em ordem decrescente, em: musculoesquelético (7,8%), haematopoiético (6,1%), geniturinário (5,6%), cutânea (2,6%), linfático (2,2%) e cardiovascular (1,6%) ou foram classificadas em doenças metabólicas e nutricionais (5,5%), neonatais (4,7%), tóxicas (2,5%) ou outros distúrbios (1,4%). A raiva, uma zoonose fatal, foi a principal causa de óbito neste estudo e a principal doença neurológica (23,7%). O reconhecimento das principais doenças dos bovinos desta região permite a adoção de medidas profiláticas e de manejo para minimizar sua ocorrência e evitar perdas econômicas com o tratamento e menor produtividade do rebanho.

Abstract Onychomycosis is the most common disease affecting the nail unit and accounts for at least 50% of all nail diseases. In addition, Candida albicans is responsible for approximately 70% of onychomycoses caused by yeasts. This study investigated the antifungal effect of (R) and (S)-citronellal enantiomers, as well as its predictive mechanism of action on C. albicans from voriconazole-resistant onychomycoses. For this purpose, in vitro broth microdilution and molecular docking techniques were applied in a predictive and complementary manner to the mechanisms of action. The main results of this study indicate that C. albicans was resistant to voriconazole and sensitive to the enantiomers (R) and (S)-citronellal at a dose of 256 and 32 µg/mL respectively. In addition, there was an increase in the minimum inhibitory concentration (MIC) of the enantiomers in the presence of sorbitol and ergosterol, indicating that these molecules possibly affect the integrity of the cell wall and cell membrane of C. albicans. Molecular docking with key biosynthesis proteins and maintenance of the fungal cell wall and plasma membrane demonstrated the possibility of (R) and (S)-citronellal interacting with two important enzymes: 1,3-β-glucan synthase and lanosterol 14α-demethylase. Therefore, the findings of this study indicate that the (R) and (S)-citronellal enantiomers are fungicidal on C. albicans from onychomycoses and probably these substances cause damage to the cell wall and cell membrane of these micro-organisms possibly by interacting with enzymes in the biosynthesis of these fungal structures.

Resumo A onicomicose é a doença mais comum que afeta a unidade ungueal e representa pelo menos 50% de todas as doenças ungueais. Além disso, a Candida albicans é responsável por aproximadamente 70% das onicomicoses causadas por leveduras. Nesse estudo, foi investigado o efeito antifúngico dos enantiômeros (R) e (S)-citronelal, bem como seu mecanismo de ação preditivo sobre C. albicans de onicomicoses resistentes ao voriconazol. Para este propósito, foram aplicadas técnicas in vitro de microdiluição em caldo e docking molecular de forma preditiva e complementar para os mecanismos de ação. Os principais resultados deste estudo indicam que C. albicans foi resistente ao voriconazol e sensível aos enantiômeros (R) e (S)-citronelal na dose de 256 e 32 µg/mL respectivamente. Além disso, houve aumento da concentração inibitória mínima (CIM) dos enantiômeros na presença do sorbitol e do ergosterol, indicando que estas moléculas possivelmente afetem a integridade da parede e da membrana celular de C. albicans. O docking molecular com proteínas chave da biossíntese e manutenção da parede celular e da membrana plasmática fúngica, demonstraram a possibilidade do (R) e (S)-citronelal interagir com duas importantes enzimas: 1,3-β-glucan sintase e lanosterol 14α-demetilase. Portanto, os achados desse estudo indicam que os enantiômeros (R) e (S)-citronelal são fungicidas sobre C. albicans de onicomicoses e provavelmente essas substâncias causem danos a parede e a membrana celular desses microrganismos possivelmente por interagir com as enzimas da biossíntese destas estruturas fúngicas.

ABSTRACT Objective To evaluate the accuracy of corneal tomography with Scheimpflug images (Pentacam® AXL, OCULUS, Wetzlar, Germany) in scanning patterns with 25 and 50 images, by analyzing their repeatability and reproducibility in normal eyes, eyes with keratoconus and with keratoconus after crosslinking. Methods Prospective, cross-sectional, mixed study including patients who had normal corneas, or who had keratoconus, who had not undergone corneal surgery, with the exception of crosslinking. Patients who had undergone surgery for intracorneal ring implantation or corneal transplantation were excluded. Three measurements were performed with 25 images and three measurements were performed with 50 images on each eye included in the study. Results The study evaluated a total of 54 eyes of 41 patients. Of these, 10 (18.52%) eyes had normal corneas, without previous eye surgery, 22 (40.74%) were eyes with keratoconus, without previous eye surgery, and 22 (40.74%) were eyes with keratoconus, with crosslinking for keratoconus. In the study of repeatability, we noticed a greater variation of values in all indices in patients with KC compared to those with normal eyes, in most of the parameters analyzed. Conclusion There is no significant difference between the scanning methods with 25 and 50 images. However, 25 scans has advantages that make it the main method suggested. The presence of keratoconus has a negative impact on the repeatability of both types of examination, and there is great agreement between the methods in the three groups studied. The high repeatability coefficient in some parameters suggests that these data should be used with care in the assessment of keratoconus progression.

RESUMO Objetivo Avaliar a precisão da tomografia corneana com imagens de Scheimpflug (Pentacam® AXL, OCULUS, Wetzlar, Alemanha) nos padrões de escaneamento com 25 e 50 imagens, verificando sua repetibilidade e reprodutibilidade em olhos normais, com ceratocone e com ceratocone após crosslinking. Métodos Estudo prospectivo, transversal, misto, no qual foram incluídos os pacientes que tinham córneas normais, ou com ceratocone; e não tinham realizado cirurgias corneanas, com exceção do crosslinking. Foram excluídos os pacientes que tivessem realizado outras cirurgias, como implante de anel intracorneano ou transplante de córnea. Foram realizadas três medidas com 25 imagens e três medidas com 50 imagens em cada olho incluído no estudo. Resultados O estudo avaliou 54 olhos de 41 pacientes. Destes, dez (18,52%) olhos eram com córneas normais e sem cirurgia ocular prévia aleatoriamente selecionados de 5 pacientes; 22 (40,74%) olhos aleatoriamente selecionados de 18 pacientes com ceratocone, sem cirurgia ocular prévia; e 22 (40,74%) olhos com ceratocone de 18 pacientes operados por crosslinking. No estudo da repetibilidade, percebemos uma maior variação de valores em todos os índices nos pacientes com ceratocone comparados aos de pacientes com olhos normais, em grande parte dos parâmetros analisados. Conclusão Não houve diferença significativa entre os métodos de escaneamento por 25 e 50 imagens, sendo o escaneamento por 25 imagens mais conveniente. A presença de ceratocone teve impacto negativo na repetibilidade de ambos os tipos de exame, e foi grande a concordância entre os métodos nos três grupos estudados. O coeficiente de repetibilidade permite considerar a relevância clínica dos dados para avaliar a progressão de ceratocone.

ABSTRACT: Vagal indigestion (VI) is a digestive disorder characterized by the dysfunction of the vagus nerve. This disorder leads to changes in forestomach motility. The causes of VI are varied. Failure to transport digestive contents may occur due to mechanical obstruction or a neurogenic origin. There are few reports in the literature regarding this disease in mini-cattle. These cattle seem to be predisposed to the development of VI. The aim of the present study was to analyze the epidemiological, clinical, laboratory, and electrocardiographic aspects of mini-cattle diagnosed with VI. The medical records of nine mini-cattle diagnosed with VI were retrospectively reviewed. Mini-cattle were referred to the Large Animal Hospital in Botucatu, Brazil, from 2002 to 2021. It was observed that VI affected young mini-cattle. The predominant clinical manifestations were intermittent gas ruminoreticular tympanism with increased volume in the left paralumbar fossa and increased volume in the right ventral region (apple/pear aspect), hypomotility in ruminal auscultation and the presence of pasty stools. The mean heart rate of the animals was 75 beats per minute. In one case, bradycardia was observed by electrocardiogram and Holter monitor system. In this case, the predominance of parasympathetic activity of the autonomic nervous system was observed. The main hematological findings were leukocytosis and lymphocytosis. The therapeutic approach included ruminal fistulation and the implantation of a permanent rumen cannula. All animals undergoing the procedure were discharged with daily care instructions.

RESUMO: A indigestão vagal (IV) é um distúrbio digestivo caracterizado pela disfunção do nervo vago. Esse distúrbio leva a alterações na motilidade dos pré-estômagos. As causas da IV são variadas. A falha no transporte do conteúdo digestivo pode ocorrer por obstrução mecânica ou pode ser de origem neurogênica. Há poucos relatos na literatura a respeito dessa enfermidade em mini-bovinos. Esses bovinos parecem estar predispostos ao desenvolvimento de IV. O objetivo do presente estudo foi analisar os aspectos epidemiológicos, clínicos, laboratoriais e eletrocardiográficos de mini-bovinos diagnosticados com IV. Os prontuários de nove mini-bovinos diagnosticados com IV foram revisados retrospectivamente. Os mini-bovinos foram encaminhados ao Hospital de Grandes Animais de Botucatu, Brasil, de 2002 a 2021. Observou-se que a IV acometeu mini-bovinos jovens. As manifestações clínicas predominantes foram timpanismo ruminoreticular gasoso intermitente com aumento de volume na fossa paralombar esquerda e aumento de volume na região ventral direita (aspecto maçã/pêra), hipomotilidade na ausculta ruminal e presença de fezes pastosas. A frequência cardíaca média dos animais foi de 75 batimentos por minuto. Em um caso, foi observada bradicardia por meio do eletrocardiograma e do sistema Holter. Neste caso observou-se a predominância da atividade parassimpática do sistema nervoso autônomo. Os principais achados hematológicos foram leucocitose e linfocitose. A abordagem terapêutica incluiu fistulação ruminal e implante de cânula ruminal permanente. Todos os animais submetidos ao procedimento receberam alta hospitalar com orientações de cuidados diários.

Abstract Low density sugarcane plantation (LDSP) has been implemented by some sugarcane producers in Brazil, aiming to save seeds and operational costs. The study was carried out in the municipality of Areia, Paraíba, Brazil. Five planting densities were used, varying from 5 to 25 m-2 of buds arranged in randomized blocks, with four replications. Data were measured annually over three cultivation cycles (2017 to 2020), during which the field was fertilized with NPK and the harvests were carried out manually without prior burning. The lower planting density presents higher productivity only in the cane plant (101.03 t ha-1) due to the higher plant height (2.37 m) and the higher number of stalks (11 stalks m-2), suggesting that these variables are due to the greater availability of light, water and photosynthate. However, there is a drastic reduction in sugarcane yield for this lower population in the 2nd ratoon by up to 65.62%, which is correlated with number of stalks per meter. We demonstrate the agronomic viability of LDSP in the population of 10 buds m-2 in relation to conventional planting of sugarcane until the 2nd ratoon. Data are important for future studies to present additional considerations for other production factors, such as the effects of mechanized harvesting and the management of nutrients and water, assessing the sustainability of this large-scale planting system.

Resumo O método de plantio de cana-de-açúcar de baixa densidade (LDSP) vem sendo implementado por alguns produtores de cana-de-açúcar no Brasil, visando economia de sementes e custos operacionais. O estudo foi realizado no município de Areia, Paraíba, Brasil. Utilizou-se cinco densidades de plantio, variando de 5 a 25 m-2 de gemas dispostas em blocos casualizados, com quatro repetições. Os dados foram medidos anualmente ao longo de três ciclos de cultivo (2017 a 2020). Menor densidade de plantio mostrou maior produtividade (101,03 t ha-1) devido à maior altura de planta (2,37 m) e ao maior número de hastes (NS) (11 hastes m-2), sugerindo que essas variáveis são devido à maior disponibilidade de luz, água e fotoassimilados. No entanto, houve redução drástica na produtividade da cana-de-açúcar para esta população menor na 2ª soca em 65,62%, estando correlacionado com o número de colmos por metro. Demonstramos a viabilidade agronômica do LDSP em uma população de 10 gemas m-2 em relação ao plantio convencional de cana-de-açúcar até a 2ª soca. Os dados são importantes para que estudos futuros apresentem considerações adicionais sobre outros fatores de produção, como os efeitos da colheita mecanizada e do manejo de nutrientes e água, avaliando a sustentabilidade desse sistema de plantio em larga escala.

Abstract: This study aimed to identify patterns of metabolic syndrome among women and estimate their prevalence and relationship with sociodemographic and biological characteristics. In total, 5,836 women were evaluated using baseline data from the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil). Patterns of metabolic syndrome were defined via latent class analysis, using the following metabolic abnormalities as indicators: abdominal obesity, hyperglycemia, hypertension, hypertriglyceridemia, and reduced HDL cholesterol. The relationship between these patterns and individual characteristics was assessed using latent class analysis with covariates. Three patterns of metabolic syndrome were identified: high metabolic expression, moderate metabolic expression, and low metabolic expression. The first two patterns represented most women (53.8%) in the study. Women with complete primary or secondary education and belonging to lower social classes were more likely to have higher metabolic expression. Black and mixed-race women were more likely to have moderate metabolic expression. Menopausal women aged 50 years and older were more often classified into patterns of greater health risk. This study addressed the heterogeneous nature of metabolic syndrome, identifying three distinct profiles for the syndrome among women. The combination of abdominal obesity, hyperglycemia, and hypertension represents the main metabolic profile found among ELSA-Brasil participants. Sociodemographic and biological factors were important predictors of patterns of metabolic syndrome.

Resumen: El objetivo fue identificar patrones del síndrome metabólico en mujeres, estimar sus prevalencias y relaciones con características sociodemográficas y biológicas. Este estudio examinó 5.836 mujeres utilizando datos de la línea de base del Estudio Longitudinal de Salud del Adulto (ELSA-Brasil). Los patrones de síndrome metabólico se definieron a través del análisis de clase latente, utilizando las siguientes anormalidades metabólicas como indicadores: obesidad abdominal, hiperglucemia, hipertensión, hipertrigliceridemia y colesterol HDL reducido. Las relaciones entre los patrones de síndrome metabólico y las características individuales se evaluaron a través del análisis de clases latentes con covariables. Se identificaron tres patrones de síndrome metabólico, denominados “alta expresión metabólica”, “expresión metabólica moderada” y “baja expresión metabólica”. Los primeros dos patrones representan la mayoría (el 53,8%) de las mujeres del estudio. Las mujeres que tenían un nivel de escolaridad primario o secundario y que pertenecían a la clase social baja tuvieron una mayor probabilidad de presentar una expresión metabólica más alta. Los negros y pardos tuvieron una probabilidad más alta de presentar “expresión metabólica moderada”. Las mujeres en la menopausia que tenían 50 años o más presentaron una probabilidad más alta de tener patrones de mayor riesgo para la salud. Este estudio abordó la naturaleza heterogénea del síndrome metabólico, identificando tres perfiles diferentes para el síndrome entre las mujeres. La combinación de obesidad abdominal, hiperglucemia e hipertensión representa el principal perfil metabólico encontrado entre los participantes del ELSA-Brasil. Factores sociodemográficos y biológicos fueron importantes predictores para los patrones de síndrome metabólico.

Este estudo examinou 5.836 mulheres utilizando dados da linha de base do Estudo Longitudinal de Saúde do Adulto (ELSA-Brasil). Os padrões de síndrome metabólica foram definidos por meio de análise de classe latente, usando as seguintes anormalidades metabólicas como indicadores: obesidade abdominal, hiperglicemia, hipertensão, hipertrigliceridemia e colesterol HDL reduzido. As relações entre os padrões de síndrome metabólica e as características individuais foram avaliadas por meio da análise de classes latentes com covariáveis. Foram identificados três padrões de síndrome metabólica, denominados “alta expressão metabólica”, “expressão metabólica moderada” e “baixa expressão metabólica”. Os dois primeiros padrões representaram a maioria (53,8%) das mulheres do estudo. As mulheres com nível de escolaridade primário ou secundário e pertencentes à classe social baixa tiveram maior chance de apresentar maior expressão metabólica. Negros e pardos tiveram maior chance de apresentar “expressão metabólica moderada”. Mulheres na menopausa com 50 anos ou mais apresentaram maior chance de ter padrões de maior risco à saúde. Este estudo abordou a natureza heterogênea da síndrome metabólica, identificando três perfis distintos para a síndrome entre as mulheres. A combinação de obesidade abdominal, hiperglicemia e hipertensão representa o principal perfil metabólico encontrado entre os participantes do ELSA-Brasil. Fatores sociodemográficos e biológicos foram importantes preditores para os padrões de síndrome metabólica.

Abstract The toxic potential of dithiocarbamates fungicides widely used in world agriculture is well known, among which Mancozeb is one of the most used. This study aimed to evaluate the toxicity of Mancozeb, determining the LC50% of the product and the behavioral and histological changes observed in fish of the Pacamã species through acute and sublethal toxicity tests. The first experiment was carried out on Pacamã fingerlings exposed to dosages of 0.5, 1, 2, 4, and 8mg/L of Mancozeb under the form ManzateWG®, for a total period of 96 hours in the acute experiment, and in the second experiment, fish were subjected to concentrations of 1/10 of those used in the acute experiment (0.05, 0.1, 0.2, 0.4 and 0.8mg/L, respectively), for 15 days in total. The 50% lethal concentration of ManzateWG® was calculated at the end of the acute experiment, presenting a value of 2.29mg/L at 96h for Pacamã fingerlings. A behavioral assessment was carried out through daily observation of the fish during both experiments, and an increase in mucus production was observed, as well as atypical social behavior in those exposed to the toxic agent. Histopathological evaluation was performed on livers collected after the end of the sublethal experiment, and the main hepatic alterations observed were cytoplasmic vacuolization, inflammatory infiltrate, and necrosis. Mancozeb has toxic potential and is capable of generating behavioral changes, as well as increasing the risk of liver damage in Pacamãs exposed to this compound.

Resumo O potencial tóxico dos fungicidas ditiocarbamatos amplamente usados na agricultura mundial é bem conhecido, e entre estes, o Mancozebe é um dos mais utilizados. Esse estudo teve como objetivo avaliar a toxicidade do Mancozebe, determinando a CL50% do produto e as alterações comportamentais e histológicas, observadas em peixes da espécie pacamã através de testes de toxicidade aguda e subletal. O primeiro experimento foi realizado em alevinos de pacamã expostos a dosagens de 0,5, 1, 2, 4 e 8mg/L de Mancozebe sob a forma ManzateWG®, por um período total de 96 horas no experimento agudo, e no segundo experimento os peixes foram submetidos a concentrações de 1/10 daquelas utilizadas no experimento agudo (0,05, 0,1, 0,2, 0,4 e 0,8mg/L, respectivamente), a exposição no segundo experimento foi de 15 dias. A concentração letal a 50% do ManzateWG® foi calculada ao fim do experimento agudo, apresentando um valor de 2,29mg/L às 96h para alevinos de pacamã. Avaliou-se o comportamento por meio da observação diária dos peixes, notando-se o aumento da produção de muco assim como comportamento social atípico naqueles indivíduos expostos ao agente tóxico. A análise histopatológica foi conduzida nos fígados coletados após a conclusão do experimento subletal, identificando as principais alterações hepáticas como vacuolização citoplasmática, infiltrado inflamatório e necrose. O Mancozebe apresenta potencial tóxico e é capaz de gerar alterações comportamentais, assim como aumentar o risco de lesões hepáticas em pacamãs expostos a este composto.

Abstract The campos rupestres (rupestrian grasslands) of the Brazilian Espinhaço Range are renowned for harboring extraordinary plant diversity and endemism, yet many groups representative of this habitat remain taxonomically understudied. Sauvagesia, a predominantly neotropical genus of Sauvagesieae (Ochnaceae), is a classic example. In this study, we refine the long-neglected and problematic taxonomy of the Espinhaço-Range-inhabiting species of Sauvagesia. Our taxonomic treatment includes 24 species of which seven are derived from recently described taxa or new combinations presented herein. For each of the three new combinations from variety into species status, we also provide new lectotypifications. The studied species are remarkably variable in habit, leaf, and floral architectures, and their distribution is often narrowly restricted to particular mountains across the Espinhaço Range, a pattern especially observed in the highly endemic S. subsect. Vellozianae. We provide an updated identification key, distribution maps, photographs, and a discussion of diagnostic morphological features.

Resumo Os campos rupestres da Cadeia do Espinhaço são marcados por possuírem uma extraordinária diversidade vegetal e endemismo, onde grande quantidade de grupos ainda são pouco estudados do ponto de vista taxonômico. É o caso do gênero Sauvagesia, predominantemente neotropical, o maior e mais heterogêneo de Sauvagesieae (Ochnaceae). Neste estudo, tentamos desvendar a taxonomia problemática, há muito negligenciada, das espécies de Sauvagesia no Espinhaço. Nosso tratamento taxonômico registrou 24 espécies, das quais sete provêm de táxons recentemente descritos ou novas combinações aqui apresentadas. Para cada uma das três novas combinações de status de variedade em espécie, nós também fornecemos novas lectotipificações. As espécies estudadas são notavelmente variáveis no hábito, folhas e arquitetura floral e sua distribuição é restrita a algumas montanhas do Espinhaço, um padrão observado especialmente na endêmica S. subseção Vellozianae. Nós fornecemos uma chave de identificação atualizada, mapas de distribuição, fotos e uma discussão de características morfológicas diagnósticas.
