Abstract Objective To investigate the clinicopathological significance and prognosis of the expression of the anterior gradient 3 (AGR3) protein in women with breast cancer. Data Sources The PubMed, CINAHL, EMBASE, Scopus, and Web of Science databases were searched for studies published in English and without restrictions regarding the year of publication. The search terms were: breast cancer AND anterior gradient 3 OR AGR3 expression. Study Selection We included observational or interventional studies, studies on AGR3 protein expression by immunohistochemistry, and studies on invasive breast cancer. Case reports, studies with animals, and reviews were excluded. In total, 4 studies were included, containing 713 cases of breast cancer. Data Collection Data were extracted on clinicopathological characteristics and survival. A meta-analysis of the prevalence of AGR3 expression was performed according to the clinicopathological characteristics, hazard ratios (HRs), and overall survival and disease-free survival. Data Synthesis The expression of AGR3 was found in 62% of the cases, and it was associated with histological grade II, positivity of estrogen and progesterone receptors, low expression of ki67, recurrence or distant metastasis, and lumen subtypes. In patients with low and intermediate histological grades, AGR3 expression was associated with worse overall survival (HR: 2.39; 95% confidence interval [95%CI]: 0.628–4.159; p= 0.008) and worse disease-free survival (HR: 3.856; 95%CI: 1.026–6.686; p= 0.008). Conclusion The AGR3 protein may be a biomarker for the early detection of breast cancer and predict prognosis in luminal subtypes. In addition, in patients with low and intermediate histological grades, AGR3 protein expression may indicate an unfavorable prognosis in relation to survival.

Resumo Objetivo Investigar o significado clinicopatológico e prognóstico da expressão da proteína anterior gradient 3 (AGR3) em mulheres com câncer de mama. Fontes de Dados Utilizamos as bases de dados PubMed, CINAHL, EMBASE, Scopus e Web of Science para pesquisar estudos em inglês, sem restrições quanto ao ano de publicação. Os termos buscados foram: breast cancer AND anterior gradient 3 OR AGR3 expression. Seleção dos Estudos Foram incluídos estudos observacionais ou intervencionais, estudos sobre a expressão da proteína AGR3 por imuno-histoquímica, e estudos sobre câncer de mama invasivo. Excluíram-se relatos de casos, estudos com animais e revisões. Quatro estudos foram selecionados, que continham 713 casos de câncer de mama. Coleta de Dados Foram extraídos dados relativos a características clinicopatológicas e sobrevida. A metanálise da prevalência da expressão de AGR3 foi realizada conforme as características clinicopatológicas, razões de risco (RRs) e sobrevida global (SG) e sobrevida livre de doença (SLD). Síntese dos Dados Encontrou-se expressão de AGR3 em 62% dos casos, que se associou com grau histológico II, positividade de receptores de estrogênio e progesterona, baixa expressão de ki67, recorrência ou metástase à distância e subtipos luminais. Em pacientes com graus histológicos baixo e intermediário, a expressão de AGR3 conferiu pior SG (RR: 2,39; intervalo de confiança de 95% [IC95%]: 0,628–4,159; p= 0,008) e pior SLD (RR: 3,856; IC95%: 1,026–6,686; p= 0,008). Conclusão A AGR3 pode ser um biomarcador para a detecção precoce do câncer de mama e predizer o prognóstico em subtipos luminais. Em graus histológicos baixo e intermediário, a expressão da proteína AGR3 pode indicar um prognóstico desfavorável em relação à sobrevida.

ABSTRACT BACKGROUND AND OBJECTIVES: Inadequate pain assessment in preterm infants (PI) is a persistent problem. Currently, a precise pain assessment is one of the main challenges for health professionals in the intensive care units (NICU). The objective of this study was to verify the correlation between the Neonatal Infant Pain Scale (NIPS) and Premature Infant Pain Prole - Revised (PIPP-R), internal consistency, and inter-evaluator reliability on pain assessment during aspiration in PI. METHODS: Prospective observational study with low birth weight PI (< 2500 g), hemodynamically stable, minimal or no sedation, under mechanical ventilation, continuous positive airway pressure, nasal cannula oxygen, or ambient air, and needing aspiration during hospitalization in the neonatal intensive care unit in the period from 2019 to 2020.. PI were evaluated during three different aspiration procedures: without intervention (1), using gentle touch (2), and using sucrose (3). NIPS and PIPP-R instruments were applied, while internal consistency was determined using Cronbach’s alpha, reliability using the intraclass correlation coefficient, and concurrent validity using Spearman’s correlation coefficient. RESULTS: Fifty PIs requiring tracheal aspiration were evaluated. NIPS and PIPP-R showed high (Cronbach α: 0.824) and moderate (Cronbach α: 0.655) internal consistency. Inter-e- valuation reliability was excellent in all aspiration procedures for NIPS (1: 0.991, 2: 0.987, and 3: 0.993) and PIPP-R (1: 0.997, 2: 0.986, and 3: 0.977). Concurrent validity was observed only for aspiration without intervention. CONCLUSION: Although NIPS may have better clinical utility than PIPP-R, both instruments presented good internal consistency and inter-evaluator reliability and may be used for assessing pain during aspiration in PI.

RESUMO JUSTIFICATIVA E OBJETIVOS: A avaliação inadequada da dor em recém-nascidos prematuros (RNPT) é um problema persistente. A avaliação precisa da dor é um dos principais desafios para profissionais de saúde nas Unidades de Terapia Intensiva Neonatais (UTIN). O objetivo deste estudo foi verificar a associação entre a Escala de Dor Neonatal (Neonatal Infant Pain Scale - NIPS) e o Perfil da Dor do Bebê Prematuro - Revisado (Premature Infant Pain Profile Revised - PIPP-R), assim como a consistência interna e a confiabilidade inter-avaliadores na aferição da dor durante a aspiração do RNPT. MÉTODOS: Estudo transversal prospectivo com RNPT de baixo peso ao nascer (<2500 g), hemodinamicamente estáveis, com mínima ou nenhuma sedação, sob ventilação mecânica, apresentando pressão positiva contínua nas vias aéreas, oxigênio na cânula nasal ou ar ambiente, e precisando de aspiração durante a internação na UTIN no período de 2019 a 2020. Os RNPT foram avaliados durante três diferentes procedimentos de aspiração: sem intervenção (1), toque gentil (2) e administração de sacarose (3). Os instrumentos NIPS e PIPP-R foram aplicados durante a avaliação. A consistência interna foi determinada pelo alfa de Cronbach, a confiabilidade pelo coeficiente de associação intraclasse e a validade concorrente pelo teste de associação de Spearman. RESULTADOS: Foram avaliados 50 RNPT que necessitaram de aspiração traqueal. A NIPS e a PIPP-R mostraram consistência interna alta (Cronbach α: 0,824) e moderada (Cronbach α: 0,655), respectivamente. A confiabilidade inter-avaliadores foi excelente em todos os procedimentos de aspiração para NIPS (1: 0,991, 2: 0,987 e 3: 0,993) e PIPP-R (1: 0,997, 2: 0,986, e 3: 0,977). A validade concorrente foi observada apenas para aspiração sem intervenção. CONCLUSÃO: Embora a NIPS possa ter melhor utilidade clínica do que o PIPP-R, ambos os instrumentos apresentaram boa consistência interna e confiabilidade inter-avaliadores, e podem ser usados para avaliar a dor durante a aspiração em RNPT.

Resumo Objetivo Avaliar a frequência de anomalias congênitas (ACs) estruturais no centro-oeste brasileiro e a associação com fatores de risco maternos. Métodos Estudo prospectivo, observacional, caso-controle, baseado em uma população hospitalar. Foram analisadas gestantes atendidas em um serviço de medicina fetal no Brasil, no período de outubro de 2014 a fevereiro de 2016. Foram analisadas 357 gestantes, dentre as quais 223 tiveram fetos com ACs estruturais (grupo controle) e 134 tiveram fetos estruturalmente normais (grupo controle). A história clínica foi feita antes da consulta de pré-natal, e o diagnóstico da AC estrutural foi realizado por ultrassonografia. Resultados Observou-se uma frequência de 64,27% (n = 223) de gestantes com fetos com ACs estruturais. As ACs estruturais mais frequentes foram as do sistema nervoso central (30,94%), seguidas das anomalias do sistema gênito-urinário (23,80%), e, por fim, das ACs múltiplas (16,60%). Antecedentes de crianças com AC (razão de probabiliade [RP]: 3,85; p = 0,022), antecedentes familiares (RP: 6,03; p = < 0,001), e consanguinidade entre os grupos progenitores (RP: 4,43; p = 0,034) influenciaram a ocorrência de AC estrutural. Conclusão As ACs mais frequentes foram as do sistema nervoso central, as do sistema gênito-urinário, e as ACs múltiplas. Os fatores de risco maternos que podem ter influenciado a ocorrência de AC estrutural foram antecedentes de crianças com AC, história familiar, e a consanguinidade entre os pais.

Abstract Objective To evaluate the frequency of structural congenital anomalies (CAs) in the midwest of Brazil and its association with maternal risk factors. Methods This was a prospective, observational, case-control study based on a hospital population. Pregnant women attended at a fetal medicine service in Brazil were analyzed in the period from October 2014 to February 2016.A total of 357 pregnant women were included, 223 of whom had fetuses with structural anomalies (group case), and 134 of whom had structurally normal fetuses (control group). The clinical history was made previous to prenatal consultation, and the diagnosis of the structural CA was performed through ultrasound. Results A frequency of 64.27% (n = 223) of pregnant women with fetuses with structural anomalies was observed. The most frequent structural CAs were those of the central nervous system (30.94%), followed by anomalies of the genitourinary system (23.80%), and, finally, by multiple CAs (16.60%). The background of previous children with CAs (odds ratio [OR]: 3.85; p = 0.022), family history (OR: 6.03; p = < 0.001), and consanguinity between the progenitors (OR: 4.43; p = 0.034) influenced the occurrence of structural CA. Conclusion The most frequent CAs are those of the central nervous system, followed by those of the genitourinary system, and then multiple anomalies. The maternal risk factors that may have influenced the occurrence of structural CA were previous children with CA, family history, and consanguinity among the parents.

Resumo Objetivo Avaliar e comparar a prevalência de anomalias congênitas (ACs) estruturais de acordo com o índice de massa corporal (IMC) materno. Métodos Estudo transversal envolvendo gestantes com fetos diagnosticados com ACs estruturais por ultrassonografia morfológica entre novembro de 2014 e janeiro de 2016. O estado nutricional das gestantes foi classificado de acordo como valor bruto do índice de massa corporal. As gestantes foram categorizadas em quatro grupos: baixo peso, peso adequado, sobrepeso e obesidade. A análise estatística foi realizada no programa Stata/SE versão 12.0 (Stata Corporation, College Station, TX), comvalores de p ≤ 0,05 considerados estatisticamente significantes. Resultados Um total de 223 gestantes tiveram fetos diagnosticados com ACs. A prevalência de AC estrutural emgestantes combaixo peso foi de 20,18%, emgestantes com peso adequado foi de 43,50%, em gestantes com sobrepeso foi de 22,87%, e em gestantes com obesidade foi de 13,45%. A prevalência de anomalias do sistema nervoso central (SNC) e do sistema geniturinário foi alta para os quatro grupos. Observou-se associação positiva entre múltiplas anomalias em gestantes com peso adequado (razão de prevalência [RP] = 1,65; p ≤ 0,004) e entre anomalias do sistema linfático em gestantes obesas (RP = 4,04, p ≤ 0,000). Conclusão A prevalência das anomalias do SNC e do sistema geniturinário foi alta em todas as categorias de IMC. Gestantes obesas foram associadas a anomalias do sistema linfático. Portanto, o rastreamento e a identificação dos fatores de risco para as AC são importantes, independentemente do IMC materno. Nossos achados reforçam a importância de discutir com gestantes sobre a nutrição materna e seu efeito no desenvolvimento fetal e no desfecho neonatal.

Abstract Objective To evaluate and compare the prevalence of structural congenital anomalies (CAs) according to maternal body mass index (BMI). Methods The present cross-sectional study involved pregnant women with fetuses diagnosed with structural CAs through morphological ultrasonography between November 2014 and January 2016. The nutritional status of the pregnant women was classified according to the gross value of the body mass index. The pregnant women were categorized into four groups: low weight, adequate weight, overweight, and obesity. Statistical analysis was performed using Stata/SE version 12.0 (Stata Corporation, College Station, TX), with values of p ≤ 0.05 considered statistically significant. Results A total of 223 pregnant women had fetuses diagnosed with CAs. The prevalence of structural CAs in pregnant women with lowweight was of 20.18%, of 43.50% in pregnant women with adequate weight, of 22.87% in pregnant women with overweight, and of 13.45% in pregnant women with obesity. The prevalence of central nervous system (CNS) anomalies and of genitourinary systemanomalieswas high for the four groups of pregnant women. A positive association was observed between multiple anomalies in pregnant women with adequate weight (prevalence ratio [PR] = 1.65; p ≤ 0.004) and between anomalies of the lymphatic system in obese pregnant women (PR = 4.04, p ≤ 0.000). Conclusion The prevalence of CNS and genitourinary systemanomalies was high in all of the BMI categories. Obese pregnancies were associated with lymphatic system anomalies. Therefore, screening and identification of the risk factors for CAs are important, regardless of the maternal BMI. Our findings reinforce the importance of discussing with pregnant women maternal nutrition and its effect on fetal development and on neonatal outcome.

ABSTRACT Toxoplasma gondii can cross the placental barrier, causing fetal infection with potentially severe sequelae. The aim of this study was to evaluate whether the serological screening for toxoplasmosis should be included in the basic neonatal heel prick test in order to establish criteria for the confirmation and/or exclusion of the diagnosis of congenital infection in newborns treated at three public health units in the metropolitan region of Goiania, Goias State, Brazil. Blood samples were collected on filter paper from newborns and later, peripheral blood samples from the mothers and their respective children were obtained to confirm or exclude the diagnosis of suspected congenital infection, by means of an enzyme-linked immunosorbent assay (IgM and IgG) and a polymerase chain reaction assay. From a total of 1,159 blood samples collected on filter paper, 43.92% were reactive to IgG and 0.17% to anti-T. gondii IgM and IgG. One hundred and twenty-seven paired samples (mother and child) were collected following consensual protocols for peripheral blood collection. Results obtained from the filter paper and peripheral blood of the newborns were 90.55% concordant. A comparison of the mother and child blood test results showed agreement regarding the detection of IgG in 90.48% of the samples. The parasite DNA was detected in the peripheral blood of one child. In view of the results obtained in this study, the inclusion of the serological screening for toxoplasmosis in the newborn heel prick test proved to be effective for the early detection of congenital T. gondii infection.

Abstract INTRODUCTION: Knowledge of the epidemiological profile and risk factors for Toxoplasma gondii infection among postpartum women is a relevant issue, because this protozoan can be vertically transmitted to the developing fetus, which can cause severe and debilitating disease. The aim of this study was to assess the risk factors associated with T. gondii infection in postpartum women in Goiânia, GO, Brazil. METHODS: This cross-sectional study comprised 229 postpartum women, among whom 204 were chronically infected (IgG+/IgM-), and 25 were seronegative (IgG-/IgM-; control group). All the patients were asked to complete a form to provide sociodemographic, clinical, dietary, and cultural information. The data were analyzed to compare seropositivity and risk factors based on the odds ratio (OR) thereof. RESULTS: The sociodemographic characteristics associated with the risk for toxoplasmosis were: education ≤ 8 years [OR: 2.521, confidence interval (CI): 1.01-6.301, p=0.049], and age ≥ 30 years (OR: 4.090; CI: 1.180-14.112, p=0.023). Clinical and behavioral characteristics related to eating raw and undercooked meat, were not found to be risk factors associated with a positive test for toxoplasmosis. CONCLUSIONS: Our findings concur with the results of other studies conducted in Brazil and abroad, where variables such as low levels of schooling, and advanced age (≥ 30 years) are major risk factors for pregnant women to become infected with T. gondii.

Resumo Objetivos avaliar a percepção dos profissionais de saúde envolvidos no processo do trabalho de parto quanto à assistência ao parto normal, e comparar duas maternidades de Goiânia quanto à percepção desses profissionais na realização das rotinas e práticas recomendadas pela Organização Mundial de Saúde (OMS). Métodos Trata-se de um estudo analítico, comparativo, com abordagem quantitativa, realizado em duas maternidades públicas da cidade de Goiânia-GO. Participaram do estudo 86 profissionais que atuavam na assistência ao trabalho de parto imediato nas duas maternidades. Foi utilizado um questionário contendo 40 questões relacionadas ao programa de humanização no pré-natal e nascimento (PHPN) do Ministério da Saúde (MS), presença do acompanhante, e os procedimentos realizados. Para a análise dos dados, utilizou-se os testes Qui-quadrado e Exato de Fisher. Resultados A maioria dos profissionais afirmou conhecer sobre o PHPN proposto pelo MS nas duas maternidades. Com relação às boas práticas recomendadas, a maioria dos profissionais afirmou que elas são aplicadas na maternidade 2, enquanto, na maternidade 1, apesar de muitas delas estarem presentes, ainda há muitas intervenções desnecessárias. Conclusão quando comparadas as duas maternidades, a maternidade 2, que foi criada como modelo de humanização, a rotina se adéqua mais às recomendações da OMS. Já na maternidade 1, observou-se uma série de intervenções consideradas pela OMS como ineficazes ou utilizadas de forma inadequada.

Abstract Purpose To evaluate the perception of health professionals involved in the labor process and theassistanceto normal delivery, comparing two hospitals in the cityof Goiânia, Brazil, regarding the perception of theseprofessionals when they are performing the routines and practices recommended by the World Health Organization (WHO). Methods This is an analytical comparative study with a quantitative approach, performed in two public hospitals in the city of Goiânia, in the state of Goiás, Brazil. The study included 86 professionals working in assistance to immediate labor in two hospitals. A questionnaire containing 40 questions was applied. The questionnaire related to the Program for the Humanization of Prenatal and Childbirth Care (PHPN, in the Portuguese acronym) of the Brazilian Ministry of Health, the presence of a companion, and the procedures performed. For the data analysis, we used the chisquare and Fisher’s exact tests. Results Most of the professionals claimed to know about the PHPN proposed by Brazilian Ministry of Health in the two hospitals. With regard to good practices, most professionals said that they are performed in maternity ward 2, while on maternity 1, although many of them are present, there are still many unnecessary interventions. Conclusion When comparing the two maternity hospitals, maternity 2, which was created as a routine humanization model, manages to better adhere to the WHO recommendations. In maternity 1, there was a series of interventions considered by the WHO as ineffective, or used in an inappropriate manner.

ABSTRACT Toxoplasmosis is caused by Toxoplasma gondii and the probability of this infection occurring in the first months of life is usually low because its transmission is related to eating habits. A 6-month-old nursing infant was diagnosed with acute toxoplasmosis, which was identified through anti- T. gondii IgA, IgM and low-avidity IgG serologic assays, polymerase chain reaction (PCR) and mouse bioassay test although its mother was seronegative. This serological divergence between mother and child led us to interview the mother regarding epidemiological factors. During this interview, she reported that she had given her 2-month-old baby a piece of undercooked beef to suck on. After some time, the baby presented fever and cervical lymphadenitis. This report emphasizes the importance of serological surveys of toxoplasmosis in nursing infants presenting with fever and lymphadenitis, in view of the possible acquisition of toxoplasmosis in the first months of life.

Objetivos Avaliar a variabilidade da contagem automática tridimensional dos folículos ovarianos que mediram 2 a 6 mm e 2 a 10 mm durante o ciclo menstrual. Verificar se este exame pode ser aplicado fora da fase folicular precoce do ciclo. Método Prospectivo observacional. Foram incluídas todas as pacientes inférteis submetidas à monitorização da ovulação de 20 de abril de 2013 a 30 de outubro de 2014, com18 a 35 anos; IMC de 18 a 25 kg/m2, eumenorréicas; semhistória de cirurgia ovariana e sem alterações nas dosagens do TSH, prolactina, insulina e glicemia. Foram excluídas aquelas que apresentaram cistos ovarianos e as que faltaram algum dia da monitorização. A contagem ultrassonográfica dos folículos foi feita pelo modo 3D com Sono AVC na fase folicular precoce, folicular media, periovulatória e lútea do ciclo. Resultados Quarenta e cinco mulheres foram incluídas. Houve diferença entre as médias das contagens dos folículos com 2 a 6 mm (p = 0,001) e 2 a 10 mm (p = 0,003) pelo teste de Friedman que avaliou conjuntamente as quatro fases do ciclo. Quando se aplicou o teste t-Student pareado, houve aumento significativo na contagem dos folículos de 2 a 6 mm quando se comparou a contagem desses folículos na fase folicular média e periovulatória com a contagem da fase lútea. Não houve diferença significante entre a contagem destes folículos pequenos nas fases folicular precoce, média e periovulatória. Conclusões A variação da contagem automática tridimensional dos folículos de 2 a 6 mm, nas fases folicular precoce, folicular média e periovulatória, não mostrou significância estatística. Houve uma variação significativa da contagem automática 3D dos folículos ovarianos de 2 a 10 mmdurante o ciclo. A variabilidade significativa da contagem dos folículos de 2 a 10 mm durante o ciclo não permite que este exame seja realizado fora da fase folicular precoce.

Objective To evaluate the variability of three-dimensional automatic counts of ovarian follicles measuring 2-6 to 2-10 mm during the menstrual cycle and to determine if this test can be applied outside the early follicular phase of the menstrual cycle. Methods in a prospective observational study, serial transvaginal ultrasound scans were performed from April 20, 2013, to October 30, 2014, on infertile patients. Inclusion criteria: age between 18 and 35 years, BMI 18-25 kg/m2, regular menstrual cycles, no history of ovarian surgery and no hormonal changes in TSH, prolactin, fasting insulin or glucose. We excluded patients with ovarian cysts or who did not complete one or more days of the serial transvaginal ultrasound scans. The follicle count was performed in 3D mode ultrasound with a Sono AVC system. Visits were scheduled for the early follicular, mid-follicular, periovulatory and luteal phases of the menstrual cycle. Results Forty-five women were included. The Friedman test showed that the total number of follicles measuring 2-6 mmvaried significantly (p = 0.001) across the four periods of the menstrual cycle. The Paired Student t-test showed a significant increase in 2-6 mm follicle count from the mid-follicular and periovulatory phase to the luteal phase. We found no significant intra-cycle variation between the small follicles (2-6 mm) in the early follicular, mid-follicular and periovulatory phases. The Friedman test showed that the total number of follicles measuring 2-10 mm varied significantly (p = 0.003) across the menstrual cycle. Conclusions The variation of three-dimensional automatic counts of 2-6 mm follicles in the early follicular, mid-follicular and periovulatory phases was not statistically significant. The significant variability in the counts of follicles measuring 2-10 mm across the menstrual cycle does not permit this examination to be performed side the early follicular phase.

Abstract: BACKGROUND: During pregnancy there is immunological, metabolic, endocrine and vascular changes responsible for physiological and pathological skin changes. OBJECTIVES: determine the prevalence of specific physiological changes and pregnancy, comparing the period of gestation of their appearances and compare type of prenatal care as the skin changes. METHODS: A cross-sectional study with 905 pregnant women. RESULTS: The prevalence of physiological skin changes was 88.95% and the most common was pigment. The prevalence of specific dermatoses was 8.72% and atopic eruption was the most common. CONCLUSION: Physiological changes were seen more in the 3rd quarter, as well as the specific dermatoses. No statistical difference in prenatal low risk compared to high risk was observed, whereas the cutaneous physiological changes and specific pregnancy dermatoses.

OBJETIVO:Avaliar a função sexual de mulheres submetidas a técnicas de reprodução assistida.MÉTODOS:Estudo caso-controle com 278 mulheres atendidas no Hospital das Clínicas da Universidade Federal de Goiás nos serviços de Reprodução Humana e no Ambulatório de Ginecologia. As mulheres foram distribuídas em grupo caso (168 mulheres inférteis) e grupo controle (110 mulheres férteis), e responderam ao questionário Índice de Função Sexual Feminina (IFSF) para avaliação da função sexual. Calculou-se a odds ratio (OR)para a chance de disfunção sexual nas mulheres inférteis (p<0,05).RESULTADOS:Do total de mulheres analisadas, 33,09% apresentaram disfunção sexual e não houve diferença no escore do IFSF (p=0,29). A prevalência de disfunção sexual entre as mulheres inférteis foi de 36,30%, e entre as férteis, 28,18%; contudo, não houve diferença entre os escores do IFSF (p=0,36). Nos domínios desejo e excitação houve diferença significativa nas mulheres inférteis (p=0,01). Mulheres inférteis apresentam a mesma chance de disfunção sexual em relação às mulheres férteis (OR=1,4; IC95% 0,8–2,4; p=0,2).CONCLUSÃO:Não houve diferenças entre as mulheres inférteis quando comparadas às férteis. Mulheres inférteis submetidas a técnicas de reprodução assistida carecem da abordagem profissional sobre a saúde sexual em relação ao desejo e à excitação.

PURPOSE:To evaluate sexual function in women undergoing assisted reproductive techniques.METHODS:This is a case-control study including 278 women assisted in Human Reproduction services and at the Gynecology Clinic of the University Hospital, Federal University of Goiás, Brazil. The women were divided into a study group (168 infertile women) and a control group (110 fertile women), and they answered the Female Sexual Function Index (FSFI) questionnaire used the assess the sexual function. We calculated the odds ratio (OR) for the chance of sexual dysfunction in infertile women (p<0.05).RESULTS:Out of the analyzed women, 33.09% reported sexual dysfunction, with no difference in the FSFI score between groups (p=0.29). The prevalence of sexual dysfunction was of 36.30% among infertile women and 28.18% among fertile women; however, there was no difference between FSFI scores (p=0.36). The desire and arousal domains were significantly different among infertile women (p=0.01). Infertile women had the same chances of having sexual dysfunction as fertile women (OR=1.4, 95%CI 0.8–2.4; p=0.2).CONCLUSION:There were no differences between infertile and fertile women. Infertile women undergoing assisted reproduction techniques require professional approach to sexual health regarding desire and arousal.

OBJETIVO: Analisar os resultados sorológicos, anatomopatológicos e parasitológicos de material abortivo para infecções com risco de transmissão vertical, com ênfase na toxoplasmose. MÉTODOS: Foi realizado um estudo coorte-transversal tratando da prevalência das doenças infectoparasitárias. Participaram da pesquisa 105 mulheres que sofreram aborto espontâneo completo e/ou incompleto; elas foram entrevistadas por meio de um questionário, e foram coletadas amostras de sangue e material abortivo. Foram realizados testes imunológicos para toxoplasmose, doença de Chagas, rubéola, citomegalovírus e sífilis e análise anatomopatológica nos restos ovulares. RESULTADOS: 55% das mulheres tinham entre 20 e 30 anos de idade. A maioria (68%) apresentou idade gestacional entre a 7ª e a 14ª semanas. 54,3% das mulheres tinham o ensino médio completo ou incompleto. Pela análise da sorologia, a infecção com risco de transmissão vertical mais frequente foi o citomegalovírus (CMV) com 97,1% de positividade, e em seguida a rubéola, com 95,2%. A toxoplasmose teve um percentual de 54,3%, a doença de Chagas, de 1,9% e a sífilis, de 0,95%. A análise dos laudos de biópsia demonstrou que 63,1% apresentaram inflamação e 34%, ausência de inflamação. Das análises sorológica, anatomopatológica e parasitológica das 105 mulheres, 57 foram soropositivas para T. gondii, e nenhuma teve resultado positivo para a Reação em Cadeia da Polimerase (PCR) e para inoculação em camundongos. CONCLUSÕES: A prevalência de doenças com risco de transmissão congênita nas mulheres com abortamento espontâneo é importante, sendo necessárias pesquisas visando esclarecer a etiologia do aborto.

PURPOSE: To analyze the serological, anatomopathological and parasitological results obtained from abortive material in order to detect infections with the risk of vertical transmission, with emphasis on toxoplasmosis. METHODS: A cross-sectional cohort study was conducted in order to determine the prevalence of infectoparasitic diseases. A total of 105 women who suffered spontaneous complete or incomplete abortion participated in the study. The women were interviewed, answered a questionnaire and had their blood and abortive material collected. Immunological tests were carried out in order to detect toxoplasmosis, Chagas disease, rubeola, cytomegalovirus and syphilis, and anatomopathological analysis of the ovular remains was performed. RESULTS: 55% of the women studied were 20 to 30 years old. Most of them (68%) presented a gestational age between the 7th and 14th week. 54.3% of the women had complete or incomplete high school education. Serological analysis showed cytomegalovirus (CMV) as the most common vertically transmitted infection with 97.1% positivity, followed by rubeola with 95.2%. Toxoplasmosis showed 54.3% positivity, Chagas disease 1.9% and syphilis 0.95%. Anatomopathological analysis showed inflammation in 63.1% of the cases and absence of inflammation in 34%. The results of the serological, anatomopathological and parasitological analysis of the 105 participants showed that 57 women were T. gondii positive. However, none showed positivity in the polymerase chain reaction (PCR) or in mouse inoculation. CONCLUSIONS: The prevalence of diseases with the risk of vertical transmission is important in women with spontaneous abortion, indicating the need for more research in order to investigate the etiology of abortion.

OBJETIVOS: Investigar o efeito de um programa individualizado e supervisionado de exercícios para os músculos do assoalho pélvico (MAP) no pós-parto de multíparas e verificar a correlação entre dois métodos de medida de força dos MAP. MÉTODOS: Conduziu-se um ensaio clínico aberto em que foram incluídas puérperas, multíparas com idade entre 18 e 35 anos. A amostra foi de 23 puérperas divididas em dois grupos: Grupo Intervenção (GI, n=11) e Grupo Controle (GC, n=12). As puérperas do GI participaram de um programa de exercícios para os MAP durante oito semanas, com frequência de duas vezes por semana. As puérperas do GC não receberam orientação quanto à prática de exercícios. A força dos MAP foi medida em duas oportunidades, utilizando-se a palpação vaginal digital e o perineômetro. A análise estatística foi realizada através dos seguintes testes: exato de Fisher, do c², t de Student, Kolmogov-Smirnov para duas amostras e coeficiente de correlação de Pearson. Foi considerado como significativo p<0,05. RESULTADOS: A média de idade das participantes do GI foi de 24±4,5 anos e do GC foi de 25,3±4 anos (p=0,4). Após o programa de exercícios, verificou-se diferença significativa entre os grupos nas duas medidas da força muscular (p<0,001). Os dois métodos de medida da força muscular apresentaram correlação significativa nas duas avaliações (1ª avaliação: r=0,889, p<0,001; 2ª avaliação: r=0,925, p<0,001). CONCLUSÕES: O programa de exercícios resultou em aumento significativo da força dos MAP. Verificou-se boa correlação entre a palpação vaginal digital e o perineômetro, indicando que a palpação vaginal pode ser utilizada na prática clínica por ser um método de baixo custo e que demonstrou uma correlação significativa com um método objetivo, o perineômetro.

PURPOSES: To investigate the effect of an individualized and supervised exercise program for the pelvic floor muscles (PFM) in the postpartum period of multiparous women, and to verify the correlation between two methods used to assess PFM strength. METHODS: An open clinical trial was performed with puerperal, multiparous women aged 18 to 35 years. The sample consisted of 23 puerperal women divided into two groups: Intervention Group (IG, n=11) and Control Group (CG, n=12). The puerperal women in IG participated in an eight-week PFM exercise program, twice a week. The puerperal women in CG did not receive any recommendations regarding exercise. PFM strength was assessed using digital vaginal palpation and a perineometer. The statistical analysis was performed using the following tests: Fisher's exact, c², Student's t, Kolmogorov-Smirnov for two samples, and Pearson's correlation coefficient. Significance was defined as p<0.05. RESULTS: The participants' mean age was 24±4.5 years in IG and 25.3±4 years in CG (p=0.4). After the exercise program, a significant difference was found between the groups in both modalities of muscle strength assessment (p<0.001). The two muscle strength assessment methods showed a significant correlation in both assessments (1st assessment: r=0.889, p<0.001; 2nd assessment: r=0.925, p<0.001). CONCLUSIONS: The exercise program promoted a significant improvement in PFM strength. Good correlation was observed between digital vaginal palpation and a perineometer, which indicates that vaginal palpation can be used in clinical practice, since it is an inexpensive method that demonstrated significant correlation with an objective method, i.e. the use of a perioneometer.

OBJETIVOS: Descrever o processo de inicialização da marcha de gestantes e comparar o comportamento do centro de pressão nos três trimestres gestacionais. MÉTODOS: Foram avaliadas 57 gestantes de baixo risco, com idades de 18 a 35 anos, em três trimestres gestacionais, selecionadas por conveniência. Foram dividas em três grupos por idade gestacional - 1º trimestre (4-12 semanas), 2º trimestre (13-28 semanas) e 3º trimestre (29-42 semanas) -, sendo 19 gestantes para cada trimestre. Cada gestante foi posicionada em posição ortostática, com um pé em cada plataforma de força AMTI, ficando assim até ouvir um sinal sonoro para iniciar a marcha em um percurso de quatro metros. Os dados obtidos foram submetidos à análise estatística com o programa SPSS. Para a comparação dos grupos utilizou-se os teste de Kolmogorov Smirnov, teste de Tukey e o coeficiente de correlação de Spearman. Em todas as análises considerou-se o nível de significância de 5%. RESULTADOS: Foram encontradas diferenças significativas quando comparados os grupos 1º trimestre (GPT) e 3º trimestre (GTT), para as variáveis amplitude de oscilação médio lateral GPT (0,4 cm) e GTT (0,2 cm) e velocidade de deslocamento médio-lateral - GPT (0,9 cm/s) e GTT (0,4 cm/s). Observou-se uma diminuição gradativa nas variáveis de amplitude de oscilação anteroposterior e médio-lateral, bem como, na velocidade de deslocamento do primeiro trimestre na plataforma 1 em relação à plataforma 2. Houve diferenças significativas nas variáveis de amplitude de oscilação médio-lateral e velocidade de deslocamento médio-lateral comparando GPT e GTT. CONCLUSÃO: As variáveis analisadas apresentaram pequenas diferenças e não constituem risco eminente para a estabilidade dinâmica da gestante.

PURPOSES: To describe the process of gait initiation of pregnant women and to compare the behavior of the pressure center in the three trimesters of pregnancy. METHODS: Fifty-seven low-risk pregnant women were evaluated, aged 18 to 35 years, selected for convenience location during the three trimesters of pregnancy. The women were divided into three groups of 19 subjects each, according to gestational age - 1st quarter (4-12 weeks), 2nd quarter (13-28 weeks), and 3rd quarter (29-42 weeks,). Each patient was positioned standing up with one foot on each AMTI force platform until she heard a beep indicating that she should start walking a distance of four meter. Data were analyzed using the SPSS software. The Kolmogorov Smirnov test, Tukey's test and Spearman correlation coefficient were used for group comparisons, with 5% significance level in all tests. RESULTS: Significant differences were found between the 1st quarter (GFT) and 3rd quarter (GTT) groups regarding mediolateral oscillation amplitude (GFT: 0.4 cm and GTT: 0.2 cm) and mediolateral displacement rate (GFT: 0.9 cm/s and GTT: 0.4 cm/s). There was a gradual decrease in anteroposterior and mediolateral oscillation rate, and in the speed of displacement from platform 1 to platform 2 in GFT. There was a significant difference in oscillation amplitude and mediolateral displacement speed between GFT and GTT. CONCLUSION: The variables analyzed showed minor differences and do not constitute an imminent risk for the stability dynamics of pregnant woman.

OBJETIVO: Construir um nomograma correlacionando a idade com diferentes valores dos percentis da contagem dos folículos antrais (CFA) em mulheres inférteis. MÉTODOS: Foi feito um estudo transversal de todas as pacientes atendidas, no centro de reprodução assistida Fêmina, no período de 1º de março de 2010 a 1º de outubro de 2011. As pacientes foram submetidas à ultrassonografia transvaginal do 2º ao 4º dias de seu ciclo menstrual. Foram incluídas as pacientes de 21 a 45 anos, com ciclos regulares, dois ovários íntegros, sem evidência de endocrinopatias e que assinaram o consentimento. Foram excluídas as tabagistas, portadoras de galactosemia, cistos ovarianos, com antecedente de hepatopatia, cirurgia ginecológico-ovariana e tratamento com quimioterapia ou radioterapia. Com o intuito de se verificar a correlação da CFA com a idade das pacientes, foram utilizados os percentis 5, 25, 50, 75 e 95. Com o uso dos percentis foi feita uma regressão linear que possibilitasse perceber o efeito da idade sobre a CFA. Foi utilizado como nível de significância o valor de 5% (p<0,05). RESULTADOS: Cento e setenta e duas pacientes foram incluídas no estudo e a média de idade foi de 32,7 anos. Dentre as causas de infertilidade, os fatores masculino e tubário foram as principais etiologias, contribuindo com 65% dos casos. O nomograma correlacionando a idade com os percentis 5, 25, 50, 75 e 95 da CFA foi melhor ajustado por uma função linear. Os percentis que apresentaram as correlações mais altas foram o P25 (r=-0,9; p<0,001), o P50 (r=-0,9; p<0,001) e o P75 (r=-0,9; p<0,001). CONCLUSÃO: Construiu-se um nomograma correlacionando a idade com os diferentes valores dos percentis da CFA em mulheres inférteis sem endocrinopatias. Esse apresentou um padrão linear de redução da CFA com a idade, em todos os percentis. Esse nomograma pode ser uma referência para o clínico; no entanto, uma validação futura, com dados longitudinais, ainda é necessária.

PURPOSE: To produce age-related nomograms for ovarian antral follicle count (AFC) in infertile women. METHODS: It was done a cross-sectional study of patients attended in the center of assisted reproduction Fêmina, from March 2010 to October 2011. The patients were submitted to transvaginal ultrasonography from day 2 to day 4 of their menstrual period. Patients included were between 21 to 45 years old, with regular menses, two healthy ovaries, without any evidence of endocrinopathies and who gave written informed consent. Patients excluded were smokers, with galactosemia or ovarian cysts, with antecedents of liver disease, ovarian surgeries or who were treated with chemotherapy or radiotherapy. In order to check the evolution of the AFC in relation to patient age, we used the 5th, 25th, 50th, 75th and 95th percentiles. Linear regression was carried out using these percentiles, permitting us to determine the effect of age on the CFA. RESULTS: A total of 172 patients with a mean age of 32.7 years were included in the trial. The male and tubal factors were the main causes of infertility, accounting for 65% of cases. The age-related nomogram for the 5th, 25th, 50th, 75th and 95th percentiles of AFC revealed that changes were best fitted by a linear function. The percentiles that showed the highest correlations were 25 (r=-0.9; p<0.001), 50 (r=-0.9; p<0.001) and 75 (r=-0.9; p<0.001). CONCLUSION: A nomogram was constructed correlating age with the different AFC percentiles in infertile women without endocrinopathies. This showed a linear pattern of decline in AFC with age in all percentiles. These nomograms could provide a reference guide for the clinician. However, future validation, with longitudinal data, still is needed.
