COVID-19 and Acute Kidney Injury ― Direct and Indirect Pathophysiological Mechanisms Underlying Lesion Development

Abstract COVID-19 is a pandemic disease caused by the SARS-CoV-2 (Severe Acute Respiratory Syndrome Coronavirus 2) responsible for millions of deaths worldwide. Although the respiratory system is the main target of COVID-19, the disease can affect other organs, including the kidneys. Acute Kidney Injury (AKI), commonly seen in patients infected with COVID-19, has a multifactorial cause. Several studies associate this injury with the direct involvement of the virus in renal cells and the indirect damage stimulated by the infection. The direct cytopathic effects of SARS-CoV-2 are due to the entry and replication of the virus in renal cells, changing several regulatory pathways, especially the renin-angiotensin-aldosterone system (RAAS), with repercussions on the kallikrein-kinin system (KKS). Furthermore, the virus can deregulate the immune system, leading to an exaggerated response of inflammatory cells, characterizing the state of hypercytokinemia. The such exaggerated inflammatory response is commonly associated with hemodynamic changes, reduced renal perfusion, tissue hypoxia, generation of reactive oxygen species (ROS), endothelial damage, and coagulopathies, which can result in severe damage to the renal parenchyma. Thereby, understanding the molecular mechanisms and pathophysiology of kidney injuries induced by SARS-COV-2 is of fundamental importance to obtaining new therapeutic insights for the prevention and management of AKI.

RESUMO: O objetivo desta pesquisa foi determinar um intervalo de referência para a relação proteína C reativa (PCR)/albumina (R:PCR/ALB) na espécie canina e analisar o potencial de PCR, albumina e a relação entre ambas como indicadores de gravidade de doença, tempo de internação (TI) e mortalidade nesta espécie. Para isso, um estudo foi realizado em um Hospital Veterinário Escola no sul do Brasil. Cento e noventa cães foram incluídos aleatoriamente, sem distinção de sexo, idade ou raça, de junho de 2013 a novembro de 2016. O plasma foi coletado e analisado para avaliação da PCR e albumina. O intervalo de referência estipulado para o R:PCR/ALB em cães foi de 0,36-0,60, conforme determinado pelo intervalo de confiança da média das reamostragens (em percentis). Foram calculadas as frequências, médias e desvios-padrões das variáveis, análises de correlação e análises comparativas (Kruskal-Wallis em α = 5%). Notou-se elevação (acima da referência) da R:PCR/ALB proporcional à gravidade da doença de base, sendo normais as médias da PCR. Adicionalmente, a hipoalbuminemia foi indicadora de doença sistêmica, mas, não de gravidade. Dessa forma, a R:PCR/ALB foi melhor indicadora de gravidade de doença do que a PCR e albumina, analisadas separadamente. Em relação ao TI, houve correlação positiva com a R:PCR/ALB (p<0,01) em doentes, não sendo observado o mesmo com a PCR e albumina. Em relação à mortalidade, a hipoalbuminemia foi a única marcadora válida em animais com doenças críticas (p=0,04). Conclui-se, portanto, que a R:PCR/ALB é melhor marcadora de gravidade de doença e TI em cães do que a PCR e albumina analisadas separadamente.
ABSTRACT: The objective of this research was to creates a reference interval for C-reactive protein (CRP)/albumin ratio (CAR) in the canine species and to analyze the potential of CRP, albumin and the relationship between both, to serve as indicators of disease severity, length of hospital stay (LoS) and mortality in this species. For this, an outcome study was conducted in a Veterinary Teaching Hospital in southern Brazil. One hundred ninety dogs were included randomly, without distinction of gender, age, or breed, from June 2013 to November 2016. Plasma was collected from them and analyzed for assessment of CRP and albumin. The reference range stipulated for CAR in dogs was 0.36-0.60, as determined by the confidence interval of mean resamplings (in percentiles). The frequencies mean, and standard deviations of the variables, correlation analysis, and comparative analysis (Kruskal-Wallis in α = 5%) were calculated. Elevation (above reference) of CAR was determined to be proportional to the severity of the underlying disease, and CRP means were reasonable. Besides, hypoalbuminemia was indicative of systemic disease, but not of severity. Thus, CAR was a better marker of disease severity than were CRP and albumin, analyzed separately. Concerning LoS, there was a positive correlation with CAR (p<0.01) in patients, and the same was not observed with CRP and albumin. Concerning mortality, hypoalbuminemia was the only marker valid in animals with a critical illness (p=0.04). In conclusion, CAR is a better marker of disease severity and LoS in dogs than are CRP and albumin analyzed separately.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Durante 24 meses foram capturados, inspecionados e liberados no mesmo espaço do Parque Estadual da Pedra Branca, Rio de Janeiro, 96 marsupiais e 64 roedores. Neles foram recolhidos manualmente 105 carrapatos, de 10 espécies em duas famílias. A espécie de carrapato dominante entre os roedores foi Amblyomma longirostre e entre os marsupiais foi Ixodes loricatus. Houve correlação direta significativa entre a temperatura e a intensidade de parasitismo por carrapatos.
During 24 months 96 marsupials and 64 rodents were captured, inspected and set free in the same space of the Pedra Branca State Park, Rio de Janeiro. From them, 105 ticks from 10 species in two families were manually collected. The dominant tick species on the rodents was Amblyomma longirostre and on the marsupials was Ixodes loricatus. There was a significant correlation of the temperature in relation to intensity of parasitism for ticks, demonstrating that the higher the temperature, greater is the number of ticks.

Avaliou-se o efeito do ambiente de produção sobre o desempenho produtivo e respostas fisiológicas de frangos de corte demarca comercial sexados, machos e fêmeas. Os frangos foram criados em duas alas separadas no interior de um galpão comercial com sistemas de ventilação convencional e nebulização. O ambiente produtivo foi avaliado por meio do índice de temperatura do globo negro e umidade, da intensidade de ruído e do nível de iluminância. A avaliação dos animais foi feita por meio das respostas fisiológicas - frequência respiratória, temperatura retal, temperatura da pele e temperatura da pena - e produtivas - ganho de peso semanal, massa corporal e mortalidade. Os machos apresentaram desempenho produtivo superior às fêmeas (P<0,05). A massa corporal média dos machos foi 214,6g maior que a das fêmeas aos 35 dias de vida, a qual se igualou à massa corporal dos machos somente aos 38,47 dias de vida. As respostas fisiológicas não se relacionaram com o ambiente.
The effect of the production environment on the productive performance and physiological responses of sexed broiler chickens, male and female, was evaluated. The birds were raised in two separate regions inside the broiler house. The productive environment was determined using the black-globe temperature humidity index, as well as noise and luminance levels. The physiological responses (breath rate, rectal temperature, skin temperature and feather temperature) and productive responses (weekly weight gain, body mass and mortality) were used to evaluate the effect of the production environment on the broilers raised in a house equipped with conventional and misting ventilation systems. The mean body mass of the males was 214.6 g greater than females at 35 days of life, which became equal to the male mean body mass only at 38.47 days of life. The physiological responses were not related to the environment.

Distonia dopa-responsiva (DRD), classificada como DYT5, é um erro inato do metabolismo que pode ser causado por dois diferentes tipos de defeito bioquímico: deficiência de GTP ciclo-hidrolase 1 (GCH1) (autossômica dominante) ou de tirosina hidroxilase (autossômica recessiva). Descrevemos o caso de menina de 10 anos com distonia generalizada progressiva e alteração da marcha com importante melhora após uso de levodopa. A relação fenilalanina/tirosina estava aumentada após teste de sobrecarga com fenilalanina. O estudo molecular mostrou que o paciente apresenta uma combinação hererozigótica de mutação no gene GCH1: a já conhecida mutação P23L e uma nova mutação Q182E. Discutem-se as características da DRD e as alterações genéticas possíveis.
Dopa-responsive dystonia (DRD) is an inherited metabolic disorder now classified as DYT5 with two different biochemical defects: autosomal dominant GTP cyclohydrolase 1 (GCH1) deficiency or autosomal recessive tyrosine hydroxylase deficiency. We report the case of a 10-years-old girl with progressive generalized dystonia and gait disorder who presented dramatic response to levodopa. The phenylalanine to tyrosine ratio was significantly higher after phenylalanine loading test. This condition had two different heterozygous mutations in the GCH1 gene: the previously reported P23L mutation and a new Q182E mutation. The characteristics of the DRD and the molecular genetic findings are discussed.
