Resumo O presente artigo tem como escopo analisar a proteção jurídica à dignidade da pessoa humana no seio da sociedade de dados. O objetivo principal é tratar de como preservar a dignidade da pessoa humana no contexto em que as interações humanas estão cada mais permeadas pelas novas tecnologias de big data pelo e ambiente digital. O presente trabalho tem sua importância justificada na assimilação dos novos padrões de interação social e a proteção jurídica a ser conferida no novo contexto social. A metodologia empregada na construção deste artigo foi a lógico-dedutiva, com base no procedimento de análise bibliográfica sobre os pontos centrais envolvendo a sociedade de dados, o campo digital e a defesa da dignidade da pessoa humana.

Abstract The present article aims to analyze the legal protection of human dignity within the data society. The main objective is to address how to preserve the dignity of the human person in the context where human interactions are increasingly permeated by new technologies such as big data and the digital environment. This work is justified by the assimilation of new patterns of social interaction and the legal protection to be conferred in the new social context. The methodology employed in the construction of this article was logical-deductive, based on a bibliographic analysis procedure on the central points involving the data society, the digital field, and the defense of human dignity.

Abstract Background The extent of cardiac damage associated with aortic stenosis has important prognostic implications after transcatheter aortic valve replacement (TAVR). However, the role of tricuspid regurgitation (TR) in this clinical setting is still unclear. Objectives To explore the association between TR and mortality in patients undergoing TAVR and assess changes in TR severity post TAVR and its relationship with short and mid-term mortality. Methods Relevant databases were searched for articles published from inception until August 2020. Out of 414 screened studies, we selected 24 that reported the degree of TR pre or post TAVR. The primary outcome was all-cause mortality, and random effects meta-analysis models were conducted (at a significance level of 5%). Results Seventeen studies reported associations between pre-TAVR TR and all-cause mortality (> 45,000 participants) and thirteen accessed TR severity post TAVR (709 participants). Moderate/severe baseline TR was associated to higher all-cause mortality both at 30 days (HR 1.65; 95% CI, 1.20-2.29) and 1.2 years (HR 1.56; 95% CI, 1.31-1.84). After TAVR, 43% of patients presented a decrease of at least one grade in TR (30 days, 95% CI, 30-56%), sustained at 12.5 months in 44% of participants (95% CI, 35-52%). Persistence of significant TR was associated with a two-fold increase in all-cause mortality (HR 2.12; 95% CI, 1.53-2.92). Conclusions Significant TR pre TAVR is associated with higher mortality. Although TR severity may improve, the persistence of significant TR post TAVR is strongly associated with increased mortality. Our findings highlight the importance of a detailed assessment of TR pre and post TAVR and might help identify patients who may benefit from more careful surveillance in this scenario.

Resumo Fundamento A extensão do dano cardíaco associada à estenose aórtica tem importantes implicações prognósticas após a substituição da valva aórtica transcateter (TAVR). Contudo, ainda não está claro qual é o papel da insuficiência tricúspide (IT) nesse cenário clínico. Objetivos Explorar a associação entre IT e mortalidade em pacientes submetidos a TAVR e avaliar as alterações na gravidade da IT após a TAVR e sua relação com mortalidade de curto e médio prazo. Métodos Foram feitas pesquisas em bases de dados relevantes de artigos publicados do início até agosto de 2020. Dos 414 estudos triados, selecionamos 24 que relataram o grau de IT pré- ou pós-TAVR. O desfecho primário foi mortalidade por todas as causas, e foram conduzidos modelos de metanálise de efeitos aleatórios (a um nível de significância de 5%). Resultados Dezessete estudos relataram associações entre IT pré-TAVR e mortalidade por todas as causas (> 45.000 participantes), e 13 avaliaram a gravidade da IT pós-TAVR (709 participantes). A IT basal moderada/grave foi associada a maior mortalidade por todas as causas em 30 dias [razão de risco (RR) 1,65; intervalo de confiança (IC) 95% 1,20-2,29] e 1,2 ano (RR 1,56; IC95% 1,31-1,84). Após a TAVR, 43% dos pacientes apresentaram redução de pelo menos um grau na IT (30 dias, IC95% 30-56%), que se sustentou em 12,5 meses em 44% dos participantes (IC95% 35-52%).A persistência de IT significativa foi associada a um aumento de duas vezes na mortalidade por todas as causas (RR 2,12; IC95% 1,53-2,92). Conclusões A IT significativa pré-TAVR está associada a maior mortalidade. Ainda que a gravidade da IT possa melhorar, a persistência de IT significativa após a TAVR está fortemente associada ao aumento da mortalidade. Nossos achados destacam a importância de uma avaliação detalhada da IT pré- e pós-TAVR e podem ajudar a identificar pacientes que possam se beneficiar de uma vigilância mais cuidadosa nesse cenário.

SUMMARY OBJECTIVE: Sickle cell disease is the most frequent of the hereditary hemoglobinopathies and it presents multisystemic effects. A manifestation that is commonly found in sickle cell disease is upper airway obstruction, particularly adenotonsillar hypertrophy. This study aims to evaluate the peak nasal inspiratory flow measurements of children and adolescents with sickle cell disease. METHODS: This is a case-control study on children aged between 8 and 15 years who were diagnosed with sickle cell disease. Peak nasal inspiratory flow measurements were obtained from patients. RESULTS: A total of 279 patients were enrolled in this study, with 93 in the case group and 186 in the control group. The case group had an 82.83% chance of having lower peak nasal inspiratory flow values than the control group. In the case group, 75% of the peak nasal inspiratory flow values were in the lower standards, whereas in the control group, only 25% were in the lower standards. CONCLUSION: This study showed a high prevalence of reduced peak nasal inspiratory flow values in children with sickle cell disease and could certainly be incorporated into the day-to-day clinical evaluation of patients as a screening instrument.

RESUMO A radiação é um dos aspectos que influenciam a composição de espécies, sendo assim, a luz é um fator limitante para o crescimento das árvores. O estudo objetivou avaliar as variações morfológicas foliares entre indivíduos de Myrcia splendens (Sw.) DC., sob diferentes condições de luminosidade, em uma floresta urbana. O fragmento caracteriza-se por possuir uma área sem formação de dossel e uma com regeneração mais avançada. Foram coletados ramos de cinco indivíduos, totalizando 100 folhas. Avaliou-se a área foliar, massa fresca, massa seca, conteúdo de água, área foliar específica e teor de matéria seca das folhas. A análise de variância mostrou que a maioria das características (massa seca, área foliar específica, teor de matéria seca das folhas) variaram significativamente. A massa seca, conteúdo de água e área foliar apresentaram alta correlação com a massa fresca em ambos os ambientes. A variação morfológica encontrada em M. splendens permite a criação de estratégias de crescimento e estabelecimento da espécie de acordo com as condições ambientais.

ABSTRACT Solar radiation is one of the aspects which most influence species composition, so light is a limiting factor to tree growth in forests. This study aimed to evaluate the leaf morphological variations between individuals of the Myrcia splendens (Sw.) DC. species under different light conditions in an urban forest. The fragment is characterized by having an open canopy area and an advanced regeneration stage in which branches of five individuals were collected, totaling 100 leaves. The traits evaluated were leaf area, fresh biomass, dry biomass, water content, specific leaf area and leaf dry matter content. An analysis of variance showed that most of the morphological traits (dry biomass, specific leaf area, leaf dry matter content) varied significantly. Furthermore, variables such as dry biomass, water content and leaf area showed a high correlation with fresh biomass in both areas. The morphological variation found in M. splendens allows for creating growth and establishment strategies according to the environmental conditions.

Resumo Introdução A prescrição de medicamentos em idosos é complexa, envolvendo polifarmácia, interação medicamentosa e medicamentos inapropriados. Entre os medicamentos, destacam-se os benzodiazepínicos, cujo uso a longo prazo é comum e crescente entre idosos. Objetivo Avaliar as interações medicamentosas potenciais (IMP) em idosos que usam benzodiazepínicos e vivem na comunidade. Método Estudo transversal, realizado por meio de inquérito domiciliar, com 423 idosos de Juiz de Fora, Minas Gerais, Brasil. A busca e a classificação das IMP foram realizadas no Sistema Micromedex®. Resultados A prevalência do uso de benzodiazepínicos na população foi de 18,3% (IC 95%: 15,2-21,6). O uso de benzodiazepínicos se mostrou associado à polifarmácia (p < 0,01; RP = 3,03; IC 95%: 1,79-5,26). Foram encontradas 157 ocorrências de IMP em 69,9% dos idosos, das quais 25 foram relacionadas aos benzodiazepínicos. Nos idosos que usavam benzodiazepínicos, a polifarmácia esteve associada às IMP (p < 0,01; RP = 16,13; IC 95%: 4,67-55,55). A ocorrência de IMP foi elevada nos idosos que usavam benzodiazepínicos, abrangendo interações de valor clínico altamente significativo. Conclusão As interações detectadas podem apresentar ou induzir eventos adversos, comprometendo a segurança da farmacoterapia e demonstrando a importância de avaliar o processo de uso de medicamentos.

Abstract Background Prescription of medications for elderly individuals is complex, potentially involving polypharmacy, drug interactions, and inappropriate medication. Notable among the medications are benzodiazepines, whose long-term use is common and growing among the elderly population. Objective This study aimed to evaluate the occurrence of potential drug interactions (PDI) in elderly community-dwelling patients who use benzodiazepines. Method Cross-sectional study, conducted via household survey, with 73 elderly persons in Juiz de Fora, MG, Brazil. The search and classification of PDIs was carried out using the Micromedex® system. Results The prevalence of benzodiazepine use in this population was 18.3% (95% CI, 15.2-21.6). The use of benzodiazepines was associated with polypharmacy (p<0.01; PR 3.03; 95% CI, 1.79-5.26). A total of 157 PDI occurrences were found in 69.9% of the elderly people assessed. Of these, 25 were related to benzodiazepines. In the elderly individuals who use benzodiazepines, polypharmacy was associated with PDI (p<0.01; PR = 16.13; 95% CI, 4.67-55.55). PDI occurrence was high in the elderly people who use benzodiazepines, including interactions of highly significant clinical value. Conclusion In general, PDI is associated with polypharmacy, which demonstrates the need to rationalize drug use in the elderly population through continuous monitoring, seeking the simplest and safest possible therapeutic regimen.

O texto parte de uma crítica a três ilusões correntes, acentuadas pela resposta à Covid-19, que difundem as ideias de que as aprendizagens acontecem “naturalmente” numa diversidade de tempos e ambientes, de que a escola física vai dar lugar à “escola virtual” e de que a pedagogia pode ser substituída pelas tecnologias. Em alternativa a essas “ilusões”, sublinhamos que a educação implica sempre uma intencionalidade, o que nos conduz a valorizar o papel dos professores na construção de um espaço público comum da educação, na criação de novos ambientes escolares e na composição de uma pedagogia do encontro.

ABSTRACT The title ‘Teachers after the pandemic’ draws attention to the need for profound changes in education and teaching work. The text starts from a critique of three current illusions, accentuated by the response to Covid-19, which spread the ideas that learning happens ‘naturally’ in a diversity of times and spaces, that the physical school will give way to the ‘virtual school’, and that pedagogy can be replaced by technologies. As an alternative to these ‘illusions,’ we emphasize that education always implies an intentionality, which leads us to value the role of teachers in the construction of a common public space for education, in the creation of new school environments, and in the composition of a ‘relationship pedagogy.’

RESUMEN El título “Profesores después de la pandemia” llama la atención sobre la necesidad de cambios profundos en la educación y el trabajo docente. El texto parte de una crítica a tres ilusiones actuales, acentuadas por la respuesta al Covid-19, que difunden las ideas de que el aprendizaje ocurre “naturalmente” en una diversidad de tiempos y espacios, que la escuela física dará paso a la “escuela virtual” y que la pedagogía puede ser reemplazada por tecnologías. Como alternativa a esas “ilusiones”, destacamos que la educación siempre implica una intencionalidad, lo que nos lleva a valorar el papel de los profesores en la construcción de un espacio público común para la educación, en la creación de nuevos entornos escolares y en la composición de una pedagogía del encuentro.

Resumo Este artigo busca identificar os principais elementos da envoltória que prejudicam o desempenho térmico da autoconstrução em um contexto de tendência de elevação das temperaturas, importante efeito da mudança climática. Com base em um conjunto de moradias autoconstruídas, localizadas no Cantinho do Céu, loteamento irregular da zona sul de São Paulo, aplica-se um método de avaliação que combina a análise das envoltórias das habitações, com pesquisa de campo, entrevistas e fotografias termográficas. Os resultados indicam que o aquecimento das habitações se relaciona, predominantemente, com o uso inadequado de materiais e a disposição equivocada dos elementos construtivos. Recomenda-se a assessoria técnica como instrumento atuante sobre as deficiências construtivas das habitações precárias, contribuindo para a melhor adaptação da população mais vulnerável aos efeitos do clima.

Abstract This article seeks to identify the main envelope elements that hinder the thermal performance of self-construction in a context of rising temperatures, an important effect of climate change. Based on a set of self-built houses located in Cantinho do Céu, an irregular settlement in the southern region of São Paulo, we used an evaluation method that combines the analysis of the building envelopes with field research, interviews, and thermographic photographs. The results indicate that the heating of the dwellings is predominantly related to the inappropriate use of materials and to the wrong disposition of construction elements. Technical advice is recommended as a tool that acts on the construction defects of precarious housing, contributing to a better adaptation of the most vulnerable population to climate effects.

Resumo Objetivo: Estimar o número de óbitos por asma e a tendência temporal da taxa de mortalidade por asma em crianças e adolescentes no Brasil. Métodos: Estudo ecológico de séries temporais em que foram avaliados os óbitos por asma ocorridos no Brasil entre 1996 e 2015, na população com até 19 anos. Foram analisadas a taxa de mortalidade específica por asma e a sua tendência temporal. Resultados: Ocorreram 5.014 óbitos, a maioria (68,1%) registrada em menores de cinco anos. Observou-se uma oscilação da taxa de mortalidade específica por asma entre 0,57 e 0,21/100.000 habitantes, o que correspondeu a uma redução de 59,8% no período. Quanto ao local dos óbitos, 79,4% ocorreram em ambiente hospitalar. Nesta amostra o adolescente teve 1,5 vez mais chance de óbito por asma fora do ambiente hospitalar do que as crianças até nove anos. Não foi observada diferença significativa da redução da tendência temporal entre os sexos e dos óbitos fora do ambiente hospitalar. Conclusões: Houve tendência temporal de redução da mortalidade por asma em crianças e adolescentes nos 20 anos avaliados. As taxas de mortalidade variaram nas diversas regiões geográficas do país, foram mais elevadas na Região Nordeste. O predomínio dos óbitos abaixo de cinco anos pode se associar a maior vulnerabilidade desse grupo em países de baixa renda. Na adolescência, ressaltam-se óbitos fora do ambiente hospitalar. As mortes por asma são eventos inaceitáveis, considera-se o caráter tratável da doença e a presença de fatores evitáveis na maioria dos desfechos fatais.

Abstract Objective: To evaluate the number of asthma deaths and the temporal trend of the asthma-specific mortality rate in children and adolescents up to 19 years of age in Brazil. Methods: This is an ecological time-series study of asthma deaths reported in Brazil, in the population up to 19 years of age, between 1996 and 2015. The specific asthma mortality rate and its temporal trend were analyzed. Results: There were 5014 deaths during the 20 years evaluated, with the majority, 68.1%, being recorded in children under 5 years of age. The specific asthma mortality rate ranged from 0.57/100,000 in 1997 to 0.21/100,000 in 2014, with a significant reduction of 59.8%. Regarding the place of death, 79.4% occurred in a hospital setting. In this sample, the adolescents had a 1.5-fold higher chance of death out-of-hospital than children up to nine years of age. There was no significant difference in the temporal trend between the genders and no significant decrease in out-of-hospital deaths. Conclusions: This study found a temporal trend for a reduction in asthma deaths over 20 years in children and adolescents in Brazil. Mortality rates varied across the geographic regions of the country and were higher in the Northeast. The prevalence of deaths under 5 years of age may be associated with the greater vulnerability of this age group in low-income countries. In adolescence, deaths outside the hospital environment are noteworthy. Asthma deaths are rare but unacceptable events, considering the treatable nature of the disease and the presence of avoidable factors in most of fatal outcomes.

Abstract Studies have shown improvement of the cardiac autonomic balance in post-bariatric patients. Most of these studies included patients using drugs interfering in the autonomic nervous system. This study assessed the time course of changes in the sympathovagal balance after bariatric surgery (Roux-en-Y gastric bypass) in 26 women free from drugs. A 10-min electrocardiographic recording was obtained at baseline and at 3 and 6 months after surgery. Temporal and spectral domains of heart rate variability were analysed. The time domain components of cardiac vagal modulation increased progressively after surgery. In the frequency domain, high frequency power increased from 24.9 at baseline (18.0 to 46.3) to 44.5 at 3 months (23.4 to 65.6), and 54.1 at 6 months after surgery (37.6 to 64.0) (median and IQR in normalized units). Low frequency spectral power decreased from 56.2 at baseline (39.6 to 74.4) to 36.8 at 6 months after surgery (24.9 to 53.9) (P= 0.036). Low frequency/high frequency ratio decreased from 2.3 at baseline (1.0 to 4.2) to 0.8 at 6 months after surgery (0.4 to 1.3) (P= 0.038). Progressive shift towards predominance of vagal tonus was detected in the follow-up. Most of the patients recovered low frequency/high frequency at 6 months after surgery.

Resumo O e-SUS é um software que instrumentaliza o processo de trabalho em saúde no Brasil. Dentre os dados de cadastro da população há o registro do uso de plantas medicinais na Atenção Básica (AB) à saúde. O objetivo do trabalho foi analisar esses registros no e-SUS em uma Unidade Básica de Saúde (UBS) no Vale do Jequitinhonha e realizar uma pesquisa com pessoas da mesma UBS que responderam sobre o uso de plantas medicinais. A hipótese do estudo é que embora o registro seja relevante, os dados gerados pelo software subestimam a realidade. Na pesquisa realizada com 77 usuários de uma UBS, selecionados de forma randomizada, 93,5% declararam fazer uso de plantas medicinais, enquanto nos relatórios do e-SUS apenas 0,8% dos indivíduos fazem este uso na mesma UBS. Portanto, os dados registrados ainda não podem ser utilizados para desenvolvimento de políticas de plantas medicinais na AB. O registro confiável do uso de plantas medicinais no e-SUS ainda é um desafio. É importante conscientizar os profissionais dos benefícios do preenchimento correto dos formulários para o desenvolvimento de políticas públicas que incluam o que é preconizado na Política Nacional de Plantas Medicinais visando reconhecer, orientar e valorizar o uso de plantas medicinais.

Abstract The e-SUS is a software used to computerize the health work process in Brazil. Among the population registration data is the record about the use of medicinal plants in Primary Health Care (AB). Thus, our objective was to analyze the records of the e-SUS in a Basic Health Unit (UBS) in the Jequitinhonha Valley and conduct a survey with individuals from the same UBS, where they were asked about the use of medicinal plants. The hypothesis of the study is that although the registration is relevant, the software data underestimate the reality about medicinal plants use. In a survey with 77 users of an UBS randomly selected, 93.5% reported using medicinal plants, while in e-SUS reports only 0.8% of the individuals from the same UBS use medicinal plants. Therefore, the recorded data cannot yet be used to develop medicinal plant policies in the AB. Reliable record of medicinal plant use in e-SUS is still a challenge. It is important to make professionals aware of the benefits of correctly completing the forms for the development of public policies that include what is recommended by the National Medicinal Plants Policy in order to recognize, guide and value the use of medicinal plants.

Resumo Neste trabalho são apresentadas as espécies de Passifloraceae stricto sensu do estado de Minas Gerais, Brasil. A família encontra-se representada por dois gêneros, Mitostemma, apenas com M. glaziovii e Passiflora com 52 espécies representantes dos quatro subgêneros: três espécies pertencentes a P. subg. Astrophea, 12 a P. subg. Decaloba, duas a P. subg. Deidamioides e 35 a P. subg. Passiflora, totalizando 53 espécies, das quais, quatro novas citações para o estado. São apresentadas descrições, fotos, dados de distribuição geográfica e comentários taxonômicos das espécies de Passifloraceae ocorrentes em Minas Gerais.

Abstract In this work, we presented the species of Passifloraceae stricto sensu from the state of Minas Gerais, Brazil. The family is represented by two genera: Mitostemma, only with M. glaziovii, and Passiflora with 52 species in four subgenera: three species belonging to P. subg. Astrophea, 12 to P. subg. Decaloba, two to P. subg. Deidamioides, and 35 to P. subg. Passiflora, with a total of the 53 species, with four new citations for the state. Descriptions, photos, geographic distribution data, and taxonomic comments of Passifloraceae species occurring in Minas Gerais are presented.

Resumo Objetivo Há comprovação do importante papel das alterações no sistema imunológico no desencadeamento e manutenção da síndrome nefrótica idiopática (SNI). O objetivo deste estudo foi investigar a expressão das citocinas em populações de linfócitos de pacientes com SNI em comparação a indivíduos saudáveis e de acordo com a proteinúria. Métodos Este estudo transversal incluiu 44 pacientes com SNI e oito crianças saudáveis, pareados por idade e sexo (controles). Os pacientes foram subdivididos de acordo com a proteinúria: proteinúria persistente ou remissão parcial (PP ≥ 300 mg/24 h, n = 17) e proteinúria baixa ou remissão completa (PB < 300 mg/24 h, n = 27). A análise ex vivo de leucócitos no sangue periférico por citometria de fluxo foi feita utilizando marcadores de superfície para linfócitos T, TCD4, TCD8, células natural killer (NK), linfócitos NKT e B. As frequências das citocinas intracelulares foram analisadas nessas células. Resultados A frequência dos linfócitos B, células NK e células NKT foi menor em pacientes com SNI do que nos controles, ao passo que os pacientes com SNI apresentaram maior frequência de células CD4+fator de necrose tumoral (TNF)-α+ do que nos controles. Os linfócitos T citotóxicos que expressam interferon (IFN)-γ foram menores nos pacientes com SNI do que nos controles. Os pacientes com PP mostraram maiores frequências de linfócitos T CD4 que expressam IFN-γ e TNF-α que os controles. Os linfócitos CD8 que expressam TNF-α apresentaram aumento no grupo com PP, em comparação aos com PB e os controles, apesar de as células CD8+IFN-γ+ serem mais baixas nos pacientes com PB e nos controles. Conclusão Com relação ao nível de proteinúria, os pacientes com SNI apresentaram aumento na expressão de TNF-α nos linfócitos CD4 e expressão reduzida de IFN-γ nos linfócitos CD8. A persistência da proteinúria foi associada a maiores níveis de marcadores inflamatórios.

Abstract Objective: There is evidence of an important role of immune system changes in the triggering and maintenance of idiopathic nephrotic syndrome (INS). The aim of this study was to investigate the expression of cytokines in lymphocyte populations of patients with INS in comparison to healthy individuals, according to proteinuria. Methods: This cross-sectional study included 44 patients with INS and eight healthy children, matched for age and sex (controls). Patients were subdivided according to proteinuria: persistent proteinuria or partial remission (PP ≥ 300 mg/24 h, n = 17) and low proteinuria or complete remission (LP < 300 mg/24 h, n = 27). Ex vivo analysis of peripheral blood leukocytes by flow cytometry was performed using surface markers for T-lymphocytes, TCD4, TCD8, natural killer (NK) cells, NKT, and B-lymphocytes. Frequencies of intracellular cytokines were analyzed in these cells. Results: The frequencies of B-lymphocytes, NK cells, and NKT cells were lower in INS than in controls, whereas INS patients had a higher frequency of CD4+tumor necrosis factor (TNF)-α+ cells than controls. Cytotoxic-T-lymphocytes expressing IFN-γ were lower in INS than in controls. Patients with PP showed higher frequencies of CD4-T-lymphocytes expressing IFN-γ and TNF-α than controls. CD8-lymphocytes expressing TNF-α were increased in PP group when compared with LP and controls, while CD8+interferon (IFN)-γ+ cells were lower than in LP and in controls. Conclusion: Regardless the level of proteinuria, INS patients had increased expression of TNF-α in CD4-lymphocytes and reduced expression of IFN-γ in CD8-lymphocytes. Persistence of proteinuria was associated with higher levels of inflammatory markers.

ABSTRACT Beachrocks are a common feature along the Brazilian coast and although their occurrence in intertidal zones is concentrated in tropical regions, similar formations have been described submerged on the continental shelf in subtropical regions. In the state of Paraná, submerged sandstones are present on the continental shelf and their formation could be associated with the cementation of beach sediments. This would provide an excellent indicator of the stabilization of the coastline during lower sea-level periods. In this study, samples were identified and collected in Paraná State, Southern Brazil, at depths between 18 and 33 meters in the continental shelf and at 6 meters depth in the Paranaguá Bay inlet. As in situ observations proved problematic, analysis relied mainly on their petrography, mineralogy, ages and isotopic values (δ13C and δ18O). Whole rock dating demonstrated that the oldest acquired sample was formed 28109-26406 cal. years BP, being exposed to atmospheric conditions during the Last Glacial Maximum. This exposure is reflected in its cements, composed uniquely of microcrystalline and spar calcite, and on its δ18O/δ13C values, which indicates formation on a fresh water environment. Contrarily to most carbonate cemented products described along Brazilian coastline, δ13C values ranged between -26.36 and -51.07‰ on all other samples, interpreted as a result of cement precipitation prompted by methane, either: in a paleo-estuarine/ paleo-lagunar to transgressive environment buried under a pile of coarser material or; after transgression and drowning of organic-rich sediments by coarser sediments, and due to upward migration of methane on the sedimentary column. Both processes would result in cements identified on these samples, apart from the one collected closer to shore which indicates some freshwater influence. Two of the samples separated by 15km distance perpendicularly to the actual coastline and 11m depth difference yielded max and minimum ages of 7913 and 7452 cal. years BP. Such a short time span between these samples could be either the result of their formation on a linked environment or older carbon signature from drowned environments being present on younger cements.

Resumo As Diretrizes Curriculares Nacionais brasileiras do Curso de Graduação em Medicina direcionam recomendações para as instituições de ensino superior, com foco na formação baseada no desenvolvimento de competências. Tal apontamento implica em desenvolver no discente a capacidade de mobilizar não apenas conhecimentos, mas também habilidades e atitudes para lidar com situações e dilemas reais, preparando-o para o cenário de atuação profissional do médico no país. Os autores apresentam um novo modelo conceitual, integrativo, não hierárquico e representado por um átomo, cujos atributos: saber, aplicar, demonstrar, fazer, ser e interagir orbitam em torno do desenvolvimento de competências essenciais. Tal concepção, uma remodelação da tradicional pirâmide proposta por Miller, é fruto das experiências avaliativas desenvolvidas na instituição, de forma a favorecer a reflexão sobre a complexidade do processo da formação médica.

Abstract The Brazilian National Curriculum Guidelines for Undergraduate Medicine Courses indicates recommendations for higher education institutions, in the context of competence-based approach. Such approach implies developing in the medicine students the capacity to mobilize, not only knowledge but also expertise and attitudes to deal with real situations and dilemmas, preparing them for the scenario of professional practice of physician’s in the country. The authors present a new conceptual model, not hierarchical and represented by an atom, whose attributes: knowing, applying, demonstrating, doing, being and interacting orbited around developing the core competencies. This conception, a remodeling of the traditional pyramid model proposed by Miller, is the result of the evaluative experiences developed in the institution, to favor reflection on the complexity of the medical training process.
