ABSTRACT BACKGROUND: In the surgical correction of large incisional hernias, the use of a prosthesis is essential in most cases regardless of the technique chosen. The preference is for the polypropylene prosthesis. AIMS: To compare the onlay and Rives-Stoppa techniques in the correction of incisional hernias, their immediate results, complications, advantages, and disadvantages. METHODS: Two groups of patients with incisional hernias were analyzed, submitted to the onlay (19 patients) and Rives-Stoppa (17 patients) techniques, and that used polypropylene prostheses. General epidemiological variables, perioperative data variables, and postoperative complications were assessed. RESULTS: The patients’ epidemiologic profile was similar between both groups. The majority were women (58.4%), with a mean age of 65.5 years and a previous mean body mass index of 41.5 kg/m². The Rives-Stoppa technique was employed in most patients (52.7%). Those submitted to the onlay technique had longer abdominal drainage time and longer hospital stay, as well as a higher incidence of seromas and surgical wound infection. CONCLUSIONS: The incisional herniorrhaphy technique with the placement of a pre-peritoneal polypropylene mesh by the Rives-Stoppa technique was superior to the onlay due to lower rates of drain use, hospital stay, and postoperative complications.

RESUMO RACIONAL: Na correção cirúrgica das grandes hérnias incisionais, independente da técnica escolhida, na maioria das vezes, o uso de uma prótese é imprescindível. A preferência é pelas próteses de polipropileno. OBJETIVOS: Comparar a técnicas onlay e Rives-Stoppa na correção de hérnias incisionais, os seus resultados imediatos, as complicações, as vantagens e desvantagens. MÉTODOS: Foram analisados dois grupos de portadores de hérnias incisionais, submetidos à técnica onlay (19 pacientes) e técnica Rives-Stoppa (17 pacientes), utilizando prótese de polipropileno. As variáveis gerais epidemiológicas, variáveis de dados perioperatórios e complicações pós-operatórias foram analisados. RESULTADOS: O perfil epidemiológico dos pacientes foi semelhante entre os grupos, a maioria eram mulheres (58.4%), com média de idade de 65.5 anos e com índice de massa corpórea (IMC) médio prévio de 41,5kg/m². A técnica de Rives-Stoppa foi utilizada na maioria dos pacientes (52.7%). Pacientes submetidos a técnica onlay apresentaram maior tempo de uso de dreno abdominal e maior tempo de internação hospitalar, além de maior incidência de seromas e infecção de ferida operatória. CONCLUSÕES: A técnica de herniorrafia incisional com colocação de tela de polipropileno pré-peritoneal, pela técnica de Rives-Stoppa apresentou-se superior à técnica onlay, devido menor taxa de uso de drenos, menor taxa de tempo de internação hospitalar, bem como menor taxa de complicações pós-operatórias.

ABSTRACT Objectives: to characterize the outline of a project for health education and its contributions to the propagation of information on the Instagram social network. Methods: exploratory and descriptive research on an Instagram profile called “@resenhadasaude”;. Data collection: from July 23, 2020, to April 21, 2021. Interaction metrics were generated on 36 posts. Simple and percentage statistical analysis were applied. Results: there are 1,016 followers in Brazil, with a 206.02% growth. The largest audience is teenagers, young people, and women, with a gender difference of 41.8%. The greatest interest was about covid-19, sexual health, and drugs. Followers’ misconceptions reinforce the need for the dissemination of quality information. Conclusions: Instagram metrics point to the project’s validation in terms of audience interest, mostly adolescents and youth. Instagram proved to be powerful for educational purposes and information dissemination, as well as an autonomous field for nursing.

RESUMEN Objetivos: caracterizar perfil de un proyecto de educación en salud y suyas contribuciones a la difusión de información en la red social Instagram. Métodos: investigación exploratoria y descriptiva sobre un perfil del Instagram denominado “@resenhadasaude”;. Recogida de datos: de 23 de julio de 2020 a 21 de abril de 2021. Generadas métricas de interacción sobre 36 publicaciones. Aplicado análisis estadístico simple y porcentual. Resultados: son 1.016 seguidores en Brasil, con crecimiento de 206,02%. Mayor público es adolescente, joven y femenino, con diferencia entre los géneros de 41,8%. Mayor interés fue sobre covid-19, salud sexual y drogas. Equívocos de conocimiento de seguidores refuerzan la necesidad de difusión de informaciones de calidad. Conclusiones: las métricas del Instagram apuntan la validación del proyecto en términos de interés del público, mayoritariamente de adolescentes y jóvenes. El Instagram se mostró potente para fines educativos y difusión de información, así como campo autónomo para a enfermería.

Abstract Objective To evaluate clinical characteristics and outcomes of COVID-19 patients infected with HIV, and to compare with a paired sample without HIV infection. Methods This is a substudy of a Brazilian multicentric cohort that comprised two periods (2020 and 2021). Data was obtained through the retrospective review of medical records. Primary outcomes were admission to the intensive care unit, invasive mechanical ventilation, and death. Patients with HIV and controls were matched for age, sex, number of comorbidities, and hospital of origin using the technique of propensity score matching (up to 4:1). They were compared using the Chi-Square or Fisher's Exact tests for categorical variables and the Wilcoxon for numerical variables. Results Throughout the study, 17,101 COVID-19 patients were hospitalized, and 130 (0.76%) of those were infected with HIV. The median age was 54 (IQR: 43.0;64.0) years in 2020 and 53 (IQR: 46.0;63.5) years in 2021, with a predominance of females in both periods. People Living with HIV (PLHIV) and their controls showed similar prevalence for admission to the ICU and invasive mechanical ventilation requirement in the two periods, with no significant differences. In 2020, in-hospital mortality was higher in the PLHIV compared to the controls (27.9% vs. 17.7%; p = 0.049), but there was no difference in mortality between groups in 2021 (25.0% vs. 25.1%; p > 0.999). Conclusions Our results reiterate that PLHIV were at higher risk of COVID-19 mortality in the early stages of the pandemic, however, this finding did not sustain in 2021, when the mortality rate is similar to the control group.

Abstract This study aimed to analyze the 100 most-cited papers in Dentistry, with a focus on female leadership in dental research. Papers were retrieved from the Web of Science Core Collection (WoS- CC) in the category ‘Dentistry, Oral Surgery & Medicine’. Gender was assessed through WoS-CC, Scopus, ResearchGate, social media, institutional websites, and software that assigns gender according to first names (https://genderapi.io). Characteristics of authors in leadership roles were retrieved, such as affiliation, publication history, citations, H factor, and i500. The 100 most-cited papers in Dentistry were authored by 394 researchers, 326 (82.7%) men, and 68 (17.3%) women - there were 4.8 male authors for each female. Among the lead authors, there were 11.3 males for each female. Among female senior authors, there were 7 males for each female. Among lead/senior authors of the 100 most-cited papers (first and last authors, respectively), 18 were women. There was an increase in the participation of women in the top cited papers regardless of authorship role across the six decades, with a peak of two female authors in the first decade of the 21st century. For female authors in leadership roles, their publication history shows the time between their first and last papers in WoS-CC ranged from 4 to 42 years for lead authors and 1 to 39 years for senior authors. Women were found to be largely underrepresented as leaders of the 100 most-cited papers, highlighting pervasive gender inequalities in dental research publications.

Resumo Objetivou-se analisar os 100 artigos mais citados em Odontologia, com foco na liderança feminina na pesquisa odontológica. Os artigos foram identificados utilizando-se a base de dados Web of Science Core Collection (WoS-CC) na categoria ‘Dentistry, Oral Surgery & Medicine’. O gênero dos autores foi avaliado por meio do WoS-CC, Scopus, ResearchGate, mídias sociais, sites institucionais e um software que atribui gênero de acordo com o primeiro nome (https://genderapi.io). As características dos autores em papéis de liderança foram identificadas, como afiliação, histórico de publicação, número de citações, fator H e i500. Os 100 trabalhos mais citados em Odontologia foram de autoria de 394 pesquisadores, sendo 326 (82,7%) homens e 68 (17,3%) mulheres - foram 4,8 autores homens para cada mulher. Entre os primeiros autores, havia 11,3 homens para cada mulher. Entre os últimos autores, havia 7 homens para cada mulher. Entre os primeiros/últimos autores dos 100 artigos mais citados, 18 eram mulheres. Houve um aumento na participação de mulheres nos artigos mais citados, independentemente do papel da autoria ao longo das seis décadas, com um pico de duas autoras na primeira década do século XXI. Para autoras em papéis de liderança, seu histórico de publicação mostra que o tempo entre o primeiro e o último artigo no WoS-CC variou de 4 a 42 anos para primeiro autor principais e de 1 a 39 anos para último autor. Verificou-se que as mulheres estão sub-representadas como líderes dos 100 artigos mais citados, destacando-se desigualdades de gênero generalizadas nas publicações em pesquisa odontológica.

Resumo A doença renal crônica é um problema de saúde pública global e em seu estágio terminal está associada à necessidade de terapia dialítica. Abstract Chronic kidney disease is a worldwide public health problem, and end-stage renal disease requires dialysis. Most patients requiring renal replacement therapy have to undergo hemodialysis. Therefore, vascular access is extremely important for the dialysis population, directly affecting the quality of life and the morbidity and mortality of this patient population. Since making, managing and salvaging of vascular accesses falls within the purview of the vascular surgeon, developing guideline to help specialists better manage vascular accesses for hemodialysis if of great importance. RESUMO Objetivo: Linfo-histiocitose hemofagocítica (LHH) é uma síndrome caracterizada pela hiperativação de linfócitos T citotóxicos, células natural killer e macrófagos, levando à secreção de citocinas pró-inflamatórias. Linfo-histiocitose hemofagocítica associada a Leishmaniose Visceral é um quadro de grande desafio para o diagnóstico, considerando que os sintomas são semelhantes, podendo levar ao óbito pacientes não tratados. Nosso objetivo é descrever uma série de casos de Leishmaniose Visceral com LHH admitidos em um hospital de referência para doenças infecciosas. Descrição do caso: Todos os 115 casos de Leishmaniose Visceral encaminhados ao centro de referência em doenças infecciosas pediátricas foram revistos para identificar os casos de LHH. Cinco casos (4,5%) foram confirmados com LHH e apresentaram febre, esplenomegalia, citopenia, hipertrigliceridemia ou hipofibrinogenemia, aumento de ferritina e hemafagocitose em medula óssea. Comentários: Deve-se suspeitar de LHH em crianças com doenças infecciosas que não respondem adequadamente ao tratamento ou em pacientes com sintomas graves, principalmente em áreas endêmicas da leishmaniose.%

ABSTRACT Objective: Hemophagocytic lymphohistiocytosis syndrome (HLHS) is characterized by an immunological hyperactivation of cytotoxic T cells, natural killer cells, and macrophages, leading to the secretion of proinflammatory cytokines. HLHS associated with Visceral Leishmaniasis might be difficult to diagnose once symptoms are similar, resulting in the death of untreated patients. Our aim is to describe a series of cases of Visceral Leishmaniasis with HLHS admitted to a referral hospital for infectious diseases. Case description: All 115 cases of Visceral Leishmaniasis referred to a referral center for pediatric infectious diseases were reviewed to identify the cases of HLHS. Five cases (4.5%) were confirmed with HLHS and they presented fever, splenomegaly, cytopenia, hypertriglyceridemia or hypofibrinogenemia, increased ferritin and hemophagocytosis in the bone marrow. Comments: It important to rule out HLHS in children with infectious diseases that do not respond adequately to treatment or in patients with severe symptoms, especially in leishmaniasis endemic areas.%

RESUMO Objetivo: estimar a prevalência de sífilis e fatores associados em comunidades ribeirinhas. Método: estudo transversal e analítico realizado com 250 ribeirinhos residentes em cinco comunidades no município de João Pessoa, estado da Paraíba. Os dados foram coletados por meio de entrevista e testes rápidos de triagem para investigação da sífilis. Realizaram-se análise bivariada, de regressão logística e análise do peso da evidência (Weight of Evidence) para identificar a associação entre as variáveis de fatores e de comportamento de risco e a positividade do teste rápido. Resultados: a prevalência de sífilis foi de 11,6% (IC95%: 7,5–15,6). Ribeirinhos que possuem história prévia de Infecção Sexualmente Transmissível (OR 8,00; IC95%: 2,76–23,2), histórico de prisão (OR 7,39; IC95%: 1,61–33,7) e que relataram ter mais de dois parceiros sexuais nos últimos 12 meses (OR 4,31; IC95%: 1,55–11,9) apresentaram maiores chances de apresentar resultado positivo para sífilis. Conclusão: alta prevalência de sífilis entre os ribeirinhos e presença de fatores comportamentais que aumentam a vulnerabilidade à aquisição da infecção. Reforça-se a necessidade do investimento de estratégias preventivas e de rastreamento de sífilis em populações consideradas vulnerabilizadas.

Abstract Objective: To estimate the prevalence of syphilis and associated factors in riverine communities. Method: This is a cross-sectional and analytical study carried out with 250 riverside dwellers living in five communities in the city of João Pessoa, state of Paraíba. Data were collected through interviews and rapid screening tests to investigate syphilis. Bivariate, logistic regression and weight of evidence analysis were performed to identify the association between risk factors and behavior variables and rapid test positivity. Results: he prevalence of syphilis was 11.6% (95%CI: 7.5–15.6). Riverside dwellers who have a previous history of Sexually Transmitted Infection (OR 8.00; 95%CI: 2.76–23.2), history of imprisonment (OR 7.39; 95%CI: 1.61–33.7) and who reported having more than two sexual partners in the last 12 months (OR 4.31; 95%CI: 1.55–11.9) were more likely to be positive for syphilis. Conclusion: High prevalence of syphilis among riverside dwellers and the presence of behavioral factors that increase vulnerability to acquiring the infection. The need to invest in preventive and screening strategies for syphilis in populations considered vulnerable is highlighted.

Se realizaron análisis bivariado, análisis de regresión logística y análisis del peso de la evidencia (Weight of Evidence) para identificar la asociación entre las variables de factores y de comportamiento de riesgo y la positividad de la prueba serológica rápida. Resultados: la prevalencia de sífilis fue un 11,6% (IC95%: 7,5–15,6). Ribereños que poseen historia anterior de Infección Sexualmente Transmisible (OR 8,00; IC95%: 2,76–23,2), histórico de cárcel (OR 7,39; IC95%: 1,61–33,7) y que relataron tener más de dos parejas sexuales en los últimos 12 meses (OR 4,31; IC95%: 1,55–11,9) demostraron mayores riesgos de presentar resultado positivo para la enfermedad. Conclusión: alta prevalencia de sífilis entre los ribereños y presencia de factores comportamentales que aumentan la vulnerabilidad a la adquisición de la infección. Se refuerza la necesidad de inversión en estrategias preventivas y de identificación de sífilis en poblaciones consideradas vulnerables.

RESUMO A lagarta-parda do eucalipto, Thyrinteina arnobia (Stoll, 1782) (Lepidoptera: Geometridae), vem sendo relatada de forma significativa em povoamentos florestais. A importância econômica de Thyrinteina arnobia para o setor florestal na atualidade evidencia a necessidade da consolidação do Manejo Integrado de Pragas Florestais (MIP Floresta), incluindo métodos de monitoramento e controle através de semioquímicos, como o emprego de feromônios. Os compostos químicos voláteis (COVs) em substâncias cuticulares de insetos podem contribuir na regulação da interação entre os indivíduos, sendo objeto de estudo da ecologia química. Dessa forma, objetivou-se realizar uma prospecção dos compostos orgânicos voláteis (COVs) cuticulares em Thyrinteina arnobia e avaliar o papel desses compostos como possíveis feromônios de curta distância. Para o estudo químico de substâncias feromonais, foi realizada a extração de compostos cuticulares (asas e pernas) em Thyrinteina arnobia de machos e fêmeas. Os extratos obtidos foram analisados por cromatografia gasosa com detector de ionização de chamas (GC-FID). Foi realizado um espectro de ação por meio da eletroantenografia acoplada à cromatografia gasosa (CG-EAG) dos extratos de asas e pernas de fêmeas virgens sobre a antena de machos de Thyrinteina arnobia. Contudo, não foi possível observar diferença significativa nas respostas das antenas de machos a nenhum dos estímulos oferecidos. Assim, é possível inferir que Thyrinteina arnobia não faz uso de compostos cuticulares para reconhecimento a curta distância, sendo necessários estudos complementares de comportamento sexual, incluindo observação de corte e cópula.

ABSTRACT The eucalyptus brown caterpillar, Thyrinteina arnobia (Stoll, 1782) (Lepidoptera: Geometridae), has been significantly reported in forest stands. The economic importance of Thyrinteina arnobia for the forestry sector today highlights the need to consolidate the Forest Integrated Pest Management (Forest IPM), including methods of monitoring and control through the use of semiochemicals, such as pheromones. Volatile chemical compounds (VOCs) in cuticular substances from insects, may contribute to the regulation in interaction between individuals, as object of study for chemical ecology. Thus, it was aimed the prospection for cuticular volatile organic compounds (VOCs) in Thyrinteina arnobia and the evaluation of the role of these compounds as possible short-distance pheromones for the species. For the study of pheromone chemical substances, cuticular compounds were extracted from wings and legs of Thyrinteina arnobia.The selected extracts were analyzed by gas chromatography with flame ionization detector (GC-FID). An action spectrum was carried out by electroantenography coupled to gas chromatography (GC-EAG) of wings and legs extracts from virgin female upon Thyrinteina arnobia male antenna, however, it was not possible to observe significant differences at the responses of the male antennae to any of the offered stimuli. Therefore, it is possible to infer that Thyrinteina arnobia does not use cuticular compounds for short distance recognition, thus complementary studies of sexual behavior, including observation of courtship and copulation are needed.

Resumo A osteonecrose é uma doença que raramente afeta os ossos do pé. Quando presente, ela é mais comum no tálus e no navicular. Casos de osteonecrose do cuneiforme intermédio são extremamente raros, e após uma revisão bibliográfica minuciosa encontramos apenas cinco relatos na literatura, sendo todos eles em pacientes pediátricos. A seguir, apresentamos o caso de um paciente adulto com osteonecrose do cuneiforme intermédio resistente ao tratamento conservador. Portanto, propusemos uma abordagem cirúrgica com bom resultado. Nível de Evidência V; Estudos Terapêuticos; Opinião de Especialista.

Abstract Osteonecrosis is a disease that rarely affects the bones of the foot. When present, it is more common in the talus and in the navicular. Cases of osteonecrosis of the intermediate cuneiform are extremely rare, and after a thorough bibliographic review, we found only five reports in the literature, all of them in pediatric patients. Below, we present the case of an adult patient with osteonecrosis of the intermediate cuneiform that was resistant to conservative treatment. Thus, we proposed a surgical approach with good results. RESUMO Os canabinoides compreendem os endocanabinoides, fitocanabinoides e os canabinoides sintéticos e desempenham ações no sistema nervoso central e periférico. Uma quantidade enorme de publicações tem sido lançada nos últimos anos, embora a cannabis seja conhecida por milênios. Os Departamentos Científicos da Academia Brasileira de Neurologia descreveram as evidências do uso médico em suas áreas. A literatura está em constantes mudanças e possíveis novas evidências podem surgir nos próximos dias ou meses. A prescrição dessas substâncias deve ser discutida com os pacientes e suas famílias, com conhecimento sobre eventos adversos e sua eficácia.

ABSTRACT Cannabinoids comprehend endocannabinoids, phytocannabinoids, and synthetic cannabinoids, with actions both in the central and peripherical nervous systems. A considerable amount of publications have been made in recent years, although cannabis has been known for over a thousand years. Scientific Departments from the Brazilian Academy of Neurology described evidence for medical use in their areas. Literature is constantly changing, and possible new evidence can emerge in the next days or months. Prescription of these substances must be discussed with patients and their families, with knowledge about adverse events and their efficacy.

Vitamin D is a fat-soluble vitamin that plays a role not only in calcium homeostasis, but also in several other functions, including cell growth and immune functions, and is considered a neurosteroid. Vitamin D deficiency is highly prevalent worldwide and has been suggested to be associated with an increased risk of emotional disorders. Therefore, the association between vitamin D levels and psychophysiological disorders, such as depression, anxiety, and mood, has been investigated. To list these variables, a bibliographical literature research was conducted in the MEDLINE/PubMed, Web of Science, Scopus, Science Direct and PsycINFO databases, between November and December 2020, with no year limits of publication. The studies involved humans aged between 18 and 59 years without associated diseases. This review presents evidence of the main variables involved in this association, main tools used to verify these variables, and methods used to verify circulating vitamin D levels in populations. Most studies have indicated that the main psychophysiological variables involved with vitamin D levels are depression and anxiety followed by mood, and an association has been observed between increased serum vitamin D levels and reduction in symptoms of depression, anxiety, and mood, and there is a heterogeneity of methods for assessing vitamin D. More studies are clearly needed to improve our understanding of their role in modulating the psychophysiological aspects of vitamin D levels.

Resumo Objetivo Determinar a concordância entre os instrumentos de mensuração short test of functional health literacy in adults (S-TOFHLA) e short assessment of health literacy for Portuguese-speaking adults (SAHLPA-18) como estratégia para estimar a validade concorrente. Métodos Estudo transversal, com usuários do Sistema Único de Saúde. Para testar a validade concorrente, aplicou-se abordagem de concordância com teste de Kappa ponderado para dados qualitativos. Resultados Participaram 372 indivíduos, dos quais 66% e 62% não apresentaram nível de letramento adequado, segundo o SAHLPA-18 e o S-TOFHLA, respectivamente. Observou-se correlação forte entre os instrumentos (p<0,001; r=0,60); e a concordância de acertos encontrada, 65,3% (Kappa=0,35; p<0,001), foi considerada fraca. Conclusão Os instrumentos SAHLPA-18 e S-TOFHLA apresentam constructos diferentes e fraca concordância. É indicado o uso de diferentes instrumentos em pesquisas de mensuração do nível de letramento; e desenvolvimento de instrumentos específicos às condições de saúde que permitam obter resultado próximo ao real contexto dos indivíduos.

Abstract Objetivo To determine agreement between the Short Test of Functional Health Literacy in Adults (S-TOFHLA) and the Short Assessment of Health Literacy for Portuguese-speaking Adults (SAHLPA-18) measurement instruments as a strategy for estimating concurrent validity. Methods This was a cross-sectional study conducted with users of the Brazilian National Health System. An agreement approach using a weighted Kappa test for qualitative data was applied in order to test for concurrent validity. Results 372 individuals participated. It was found that 66% and 62% of them did not have an adequate level of literacy according to SAHLPA-18 and S-TOFHLA, respectively. There was strong correlation between the instruments (p<0.001; r=0.60), although the 65.3% agreement of correct answers found was considered weak (Kappa=0.35; p<0.001). Conclusion The SAHLPA-18 and S-TOFHLA instruments have different constructs and poor agreement. Use of different instruments is indicated in research intended to measure level of literacy, as is the development of instruments specific to health conditions that allow results close to the real context of individuals to be obtained.

Se encontró que 66% y 62% de estos no tenía un nivel adecuado de letramiento según SAHLPA-18 y S-TOFHLA, respectivamente. Hubo una fuerte correlación entre los instrumentos (p<0.001; r=0.60), sin embargo 65.3% de concordancia, se consideró débil (Kappa=0.35; p<0.001). Conclusión Los instrumentos SAHLPA-18 y S-TOFHLA tienen diferentes construcciones y escasa concordancia. En la investigación se indica el uso de diferentes instrumentos para medir el nivel de letramiento y el desarrollo de instrumentos específicos a las condiciones de salud que permitan obtener resultados cercanos al contexto real de los individuos.

Evidence shows that metabolic syndrome (MS) is associated with a greater risk of developing Parkinson's disease (PD) because of the increase in oxidative stress levels along with other factors such as neuroinflammation and mitochondrial dysfunction. However, because some studies have reported that MS is associated with a lower risk of PD, the relationship between MS and PD should be investigated. This study aimed to investigate the effect of MS on PD. Two authors searched five electronic databases, namely, MEDLINE, PubMed, Scopus, PsycINFO, Web of Science, and Science Direct, for relevant articles between September and October 2020. After screening the title and abstract of all articles, 34 articles were selected for full-text review. Finally, 11 articles meeting the eligibility criteria were included in the study. The quality of articles was critically evaluated using the Joanna Briggs Institute. Overall, we evaluated data from 23,586,349 individuals (including healthy individuals, with MS and PD) aged 30 years or more. In cohort studies, the follow-up period varied between 2 and 30 years. MS contributed considerably to the increase in the incidence of PD. In addition, obesity, a component of MS, alone can increase the probability of developing neurodegenerative diseases. However, despite few studies on MS and PD, changes in cognitive function and more rapid progression of PD disease has been documented in patients with MS using methods commonly used in research.
