Este artigo analisou a gestão federal da Política Nacional de Atenção Integral Saúde do Homem (PNAISH) sob uma perspectiva histórico-crítica. Foi realizada análise documental do processo de implementação da política entre 2009 e 2024 contido no arquivo da Coordenação de Atenção à Saúde do Homem (COSAH). A análise de conteúdo resultou nas seguintes categorias temáticas: organização e gestão da COSAH; processo de revisão da PNAISH; e desafios e frentes estratégicas para a implementação da PNAISH. Foram encontradas fragilidades na institucionalização da coordenação, além de uma revisão da política, em 2021, marcada pelo discurso neoconservador, que excluiu as menções a gênero e direitos sexuais e reprodutivos. Esses princípios epistemológicos da PNAISH foram retomados em 2023 com ações de qualificação da Atenção Primária para abordagem das repercussões do machismo e do patriarcado sobre a saúde de homens, mulheres e crianças.

Esos principios epistemológicos de la PNAISH se retomaron en 2023 a partir de acciones de calificación de la atención primaria para el abordaje de las repercusiones del machismo y del patriarcado sobre la salud de hombres, mujeres y niños.

This study analyzed federal government management of the National Policy for Comprehensive Men's Health Care (PNAISH) from a critical historical perspective. We analyzed documents addressing the policy implementation process between 2009 and 2024 contained in the archives of the National Office for the Coordination of Men’s Healthcare (COSAH). The content analysis resulted in the following thematic categories: COSAH organization and management; the PNAISH review process; and challenges and key strategic areas for implementing the PNAISH. We found weaknesses in the institutionalization of coordination, in addition to a policy review in 2021 marked by neoconservative discourse excluding gender and sexual and reproductive rights. These epistemological principles of the PNAISH were revisited in 2023, stemming from actions to improve the quality of primary care addressing the effects of machismo and patriarchy on men’s, women’s and children’s health.

ABSTRACT Objective: To evaluate the association of TSH, free T3 (FT3), free T4 (FT4), and conversion (FT3:FT4) ratio values with incident hypertension. Materials and methods: The study included data from participants of the ELSA-Brasil study without baseline hypertension. Serum TSH, FT4 and FT3 levels, and FT3:FT4 ratio values were assessed at baseline, and incident hypertension (defined by blood pressure levels ≥ 140/90 mmHg) was estimated over a median of 8.2 years of follow-up. The risk of incident hypertension was evaluated considering a 1-unit increase in TSH, FT4, FT3, and conversion ratio values and after dividing these variables into quintiles for further analysis using Poisson regression with robust variance. The results are presented as relative risks (RR) and 95% confidence intervals (CIs) before and after adjustment for multiple variables. Results: The primary analysis incorporated data from 5,915 euthyroid individuals, and the secondary analysis combined data from all euthyroid individuals, 587 individuals with subclinical hypothyroidism, and 31 individuals with subclinical hyperthyroidism. The rate of incident hypertension was 28% (95% CI: 27%-29.3%). The FT4 levels in the first quintile (0.18-1.06 ng/dL) were significantly associated with incident hypertension (RR: 1.03, 95% CI: 1.01-1.06) at follow-up. The association between FT4 levels in the first quintile and incident hypertension was also observed in the analysis of combined data from euthyroid individuals and participants with subclinical thyroid dysfunction (RR: 1.04, 95% CI: 1.01-1.07). The associations were predominantly observed with systolic blood pressure levels in euthyroid individuals. However, in the combined analysis incorporating euthyroid participants and individuals with subclinical thyroid dysfunction, the associations were more pronounced with diastolic blood pressure levels. Conclusion: Low FT4 levels may be a mild risk factor for incident hypertension in euthyroid individuals and persons with subclinical thyroid dysfunction. Abstract The aim of this study was to evaluate the impact of oral conditions and health-related quality of life (HRQoL) on oral health-related quality of life (OHRQoL) in children and adolescents with blood coagulation disorders and hemoglobinopathies (BCDH). The study was cross-sectional and included 61 individuals aged 2 to 18 years with BCDH. Exams for dental caries (dmft/DMFT index), oral hygiene (simplified oral hygiene index – OHI-S), and gingival health (modified gingival index – MGI) were performed. The pediatric quality of life inventory™ (PedsQL™) generic core scale and oral health scale were used to measure HRQoL and OHRQoL. Spearman’s correlation coefficient (ρ) and the Mann-Whitney test (α = 0.05) were conducted to assess the relationship between covariates and the PedsQL™ oral health scale. The mean PedsQL™ oral health scale score was 76.66 (SD = 21.36). Worse OHRQoL was correlated with poor oral hygiene (ρ = -0.383; p: 0.004), poor gingival health (ρ = -0.327; p = 0.014), and better HRQoL (ρ = 0.488; p < 0.001). Greater untreated dental caries experience was associated with worse OHRQoL (p = 0.009). ABSTRACT Objective: To understand the bioethical issues involved in the care management of nurses working in the Family Health Strategy. Method: A qualitative study was conducted through five focus groups with 36 nurses selected in the sample. Thematic content analysis was performed based on the bioethical framework, and the synthesis was presented in a conceptual map. Results: Bioethical issues were identified in the nursing care practice, related to both specific bioethical themes and the peculiarities of the work. Additionally, challenges and facilitators that interfere with addressing these issues were identified. Final considerations: Understanding the bioethical issues involved in the care management of nurses was possible with the theoretical support of different bioethical perspectives. The identified issues relate to persistent and current themes in the field of bioethics. However, some aspects intrinsic to daily practice are still imperceptible to professionals, contributing to the difficulty of discussing bioethics in this care model. ABSTRACT Post-acute COVID-19 syndrome, or long COVID, presents with persistent symptoms, including cough, dyspnea, and fatigue, extending beyond one month after SARS-CoV-2 infection. Cardiac complications such as chest pain and arrhythmias have raised concerns, with chronotropic incompetence (CI), an inadequate heart rate increase during exercise, emerging as a significant condition contributing to diminished exercise tolerance and quality of life. This study estimated the prevalence of CI and explored its association with aerobic capacity and physical activity levels in long COVID patients. A cross-sectional study was conducted at a private hospital in Sergipe, Brazil, involving 93 patients over 18 years old with persistent post-COVID-19 symptoms after confirmed SARS-CoV-2 infections. Exclusion criteria included beta-blocker use, inadequate respiratory exchange ratio, and inability to complete cardiopulmonary exercise testing (CPET). Clinical histories, CPET results, and chronotropic index calculation were used to identify CI, with logistic regression analyzing associated factors. Of the participants (mean age 45 years; average duration since COVID-19 diagnosis 120 days), 20.4% were diagnosed with CI. Logistic regression identified a strong association between CI and sedentary behavior (OR 11.80; 95% CI 2.54 to 54.78; p=0.001). Patients with CI showed lower predicted peak heart rates and maximal oxygen uptake. The prevalence of CI among long COVID patients in this study was approximately 20%, associated with decreased aerobic capacity and increased sedentary behavior. These findings highlight the need for timely diagnosis and therapeutic interventions, including cardiopulmonary rehabilitation, to enhance the quality of life in post-COVID patients with CI. The study’s cross-sectional design and its specific context have limited causality inference and generalizability, underscoring the importance of further research in diverse settings. ABSTRACT The use of tank mixtures has long been a practice in the phytosanitary management of agricultural crops. However, when different molecules are mixed in a tank, interactions may occur that reduce the efficiency of the products and cause a loss of selectivity in the crops. The aim of this study was to evaluate selectivity in maize, with a tank mixture of different phytosanitary products recommended for use with the crop. The following products were used: herbicides - Sanson®, Soberan®, Callisto®, Zapp Pro®; fungicides - Priori Xtra®, Nativo®, Tebufort®, Aproach Prima®; insecticides - Connect®, Lorsban® 480 BR, Karate Zeon®; and nutrition - Kellus Manganese®, Kellus Blindex®. The experimental units consisted of 5-litre pots, containing two plants per pot, with the products applied at phenological stage V6. Assessments were made of phytotoxicity, transient chlorophyll a fluorescence, chlorophyll content and shoot dry weight. The data obtained in the experiment were submitted to ANOVA (p ≤ 0.05), and when significant, to the Scott-Knott test (p ≤ 0.05). The maize crop suffered phytotoxicity of up to 30%, with the plants recovering from 28 DAA. The phytotoxicity was caused by mixing the different products in the spraying solution, mainly due to the synergism caused by the mixture of the herbicides nicosulfuron and tembotrione with the insecticide chlorpyrifos and the fertiliser Kellus Manganese®, which aggravated the incompatibility of some treatments. RESUMEN Objetivo: evaluar el proceso de trabajo de los profesionales de la salud en la atención primaria para enfrentar el Covid-19. Método: estudio cualitativo, que utilizó como base conceptual el proceso de trabajo en salud, y como referente metodológico la investigación evaluativa. La recolección se realizó con 23 profesionales de la salud, entre julio y septiembre de 2021, en unidades básicas de salud de un municipio del noroeste del estado de Paraná, Brasil. Para el análisis, los datos se organizaron utilizando el software MAXQDA, y cada segmento de datos se organizó en función de los significados. RESUMO Objetivo: avaliar o processo de trabalho dos profissionais de saúde na atenção primária no enfrentamento à Covid-19. Método: estudo qualitativo, que utilizou como base conceitual o processo de trabalho em saúde, e, como referencial metodológico, a pesquisa avaliativa. A coleta foi realizada com 23 profissionais de saúde, entre julho e setembro de 2021, nas unidades básicas de saúde em um município no noroeste do Paraná-Brasil. Para a análise organizaram-se os dados por meio do software MAXQDA, e cada segmento de dado foi organizado conforme os significados. Resultados: emergiram duas categorias, a saber: Dificuldades enfrentadas na reorganização da ambiência, atividades programáticas e dimensionamento dos profissionais da atenção primária à saúde; e Organização do fluxo de atendimento ao usuário com Covid-19 na atenção primária à saúde. ABSTRACT Objective: to assess the work process of Primary Health Care professionals in coping with COVID-19. Method: a qualitative study that used the health work process as conceptual basis and qualitative research as methodological framework. The collection procedure was carried out with 23 health professionals from July to September 2021, in the basic health units from a municipality in northwestern Paraná, Brazil. ABSTRACT Objective: To estimate the prevalence of Sexually Transmitted Infections (STIs) in immigrants and refugees living in the metropolitan region of Goiânia, Goiás. Method: This is a cross-sectional and analytical study. Data collection was carried out from July 2019 to January 2020 and 308 immigrants and refugees were included in the sample. All were underwent face-to-face interviews and were tested for HIV, Syphilis, and Hepatitis B, using rapid tests. Results: The general prevalence for any of the STIs investigated was 8.8% (95%CI 6.0% – 12.3%), being 5.8% (95%CI 3.6% – 8.9%) for Hepatitis B, 2.3% for Syphilis (95%CI 1.00% – 4.4%) and 0.7% for HIV (95%CI 0.1% – 2.1%). Multiple analysis, using logistic regression, showed that the variables male gender (OR = 2.7) and length of time living in Brazil (OR = 2.6) were significantly associated with STIs (p < 0.05). Conclusion: The results of this study suggest that STIs are a health problem in immigrants/refugees, which appear to be enhanced with the length of migration in the country. Public policies that guarantee health care for this population shall be considered. RESUMEN Objetivo: Estimar la prevalencia de Enfermedades de Transmisión Sexual (ETS) en inmigrantes y refugiados residentes en la región metropolitana de Goiânia, Goiás. Todos fueron entrevistados cara a cara y sometidos a pruebas de VIH, Sífilis y Hepatitis B, mediante pruebas rápidas.. Resultados: La prevalencia general para cualquiera de las ETS investigadas fue de 8,8% (IC95% 6,0% – 12,3%), siendo 5,8% (IC95% 3,6% – 8,9%) para Hepatitis B, 2,3% para Sífilis (IC95% 1,00% – 4,4%) y 0,7% para VIH (IC95% 0,1% – 2,1%). Resumo Introdução A pandemia por SARS-CoV-2, iniciada em dezembro de 2019, foi declarada pela Organização Mundial da Saúde (OMS) como emergência de saúde pública internacional em janeiro de 2020, com impacto sobre o acesso a serviços de saúde sexual e reprodutiva para as mulheres. Objetivo Discutir a contracepção no contexto pandêmico, a partir das políticas públicas vigentes e do cenário mundial e brasileiro. Método Trata-se de um artigo de opinião, onde se descreve as barreiras e possíveis soluções para o acesso à assistência de saúde sexual e reprodutiva qualificada para as mulheres. Resultados Explorou-se temas relevantes como: mudança na logística dos serviços, redução do número de consultas, dificuldade na importação de contraceptivos, falta de profissionais capacitados, burocratização do acesso a métodos contraceptivos. O uso da telemedicina e o fortalecimento da Atenção Primária à Saúde são consideradas estratégias para garantia do acesso e modificação da realidade das mulheres. Conclusão Faz-se necessário nesse contexto, a manutenção das ações dos serviços de planejamento sexual e reprodutivo para prevenir agravos provenientes de gestações não planejadas e aumento na morbimortalidade materna.

Abstract Background The SARS-CoV-2 pandemic, which started in December 2019, was declared by the World Health Organization (WHO) as an international public health emergency in January 2020, with an impact on access to sexual and reproductive health services for women. Objective To discuss contraception in the pandemic context, based on current public policies and the world and Brazilian scenario. Method This is an opinion article, which describes the barriers and possible solutions for access to qualified sexual and reproductive health care for women. Results Relevant topics were explored, such as: the change in the logistics of services, the reduction in the number of consultations, the difficulty in importing contraceptives, the lack of trained professionals, and the bureaucratization of access to contraceptives methods. The use of telemedicine and the strengthening of Primary Health Care are considered strategies to guarantee access and change the reality of women. Conclusion In this context, it is necessary to maintain the actions of sexual and reproductive planning services to prevent injuries from unplanned pregnancies and increase maternal morbidity and mortality.

Resumo O preconceito e a discriminação relacionados à obesidade podem vir de profissionais de saúde e estudantes. O objetivo foi avaliar as atitudes negativas em relação à obesidade entre universitários brasileiros de nutrição que relataram dados demográficos, peso, altura e responderam o Antifat Attitudes Test (AFAT) e a Escala de Silhuetas Brasileira para avaliar a satisfação e percepção da imagem corporal. Os escores total e das subescalas da AFAT foram comparados entre as categorias usando o teste U de Mann-Whitney. As associações das características dos participantes com a AFAT foram analisadas por meio de regressão linear múltipla. A pontuação total da AFAT foi positivamente associada ao sexo masculino (ß: 0,13; p < 0,001), idade (ß: 0,06; p < 0,001), instituições de ensino fora da capital (ß: 0,03; p < 0,05) e instituições privadas (ß: 0,08; p < 0,001); e negativamente associada à renda (ß: -0,05; p = 0,006), participantes que se percebiam acima do IMC real (ß: -0,15; p < 0,001) e do terceiro ano do curso (ß: -0,05; p = 0,041). As pontuações das subescalas foram positivamente associadas com sexo masculino e idade; e negativamente associadas com aqueles que se percebiam mais pesados. Os estudantes tinham atitudes antigordura especialmente se eram homens, mais velhos, de instituições privadas, no começo do curso e baixa renda - e menos se percebiam seu IMC maior.

Abstract Obesity-related prejudice and discrimination may have a source in health professionals and students. The objective was to assess anti-fat attitudes among Brazilian nutrition undergraduates who reported demographic data, weight, height and responded the Antifat Attitudes Test (AFAT) and the Brazilian Silhouette Scales to assess body image satisfaction and perception. Total and subscales of AFAT scores were compared among categories using the Mann-Whitney U test. Associations of participants’ characteristics with the AFAT were analyzed using multiple linear regression. Total AFAT score was positively associated with male sex (ß: .13; p < .001), age (ß: .06; p < .001), educational institution outside capital (ß: .03; p < .05), private institutions (ß: .08; p < .001); and negatively associated with income (ß: -.05; p = .006), participants who perceived themselves with increased BMI (ß: -.15; p < .001) and those at the third year of course (ß: -.05; p = .041). Subscales scores were positively associated with male sex and age; and negatively associated with those who perceived themselves heavier. They have anti-fat attitudes especially if they were man, older, from private institutions, are at the beginning of the course, and have lower household income - and less weight bias if they perceived with increased BMI.

resumo Crianças com déficit de crescimento por desnutrição perinatal apresentam especificidades na distribuição do percentual de gordura corporal (%GC) que não puderam ser detectadas por equações anteriores baseadas no %GC. Este estudo desenvolveu e validou equações preditivas do %GC derivadas de variáveis antropométricas em crianças de 7 a 10 anos residentes no Nordeste do Brasil, utilizando como referência a absorciometria radiológica de dupla energia (DXA). Foram utilizados dados de composição corporal de 58 crianças. O DXA foi usado como modelo de referência. Um modelo estatístico de regressão múltipla stepwise (forward) foi usado para desenvolver as equações. A análise de Bland-Altman (IC: 95%), teste t de Student pareado e o coeficiente de correlação intraclasse (CCI) foram utilizados para validar e comparar as equações. Duas novas equações foram desenvolvidas para ambos os sexos: meninos: %GC: 13,642 + (1,527*IMC) + (-0,345*Altura) + (0,875*Tríceps) + (0,290* Circunferência da cintura) e meninas: %GC: - 13,445 + (2,061*coxa). A análise de Bland-Altman mostrou boa concordância, com limites variando de -1,33 a 1,24% para meninos e -3,35 a 4,08% para meninas. O teste t de Student pareado não mostrou diferença entre %GC-DXA e as duas novas equações (p>0,05), e o CCI foi de 0,948 e 0,915, respectivamente.

abstract Children with a deficit of growth because of perinatal malnutrition present specificities in the percentage of body fat (%BF) that could not be detected by previous fat mass-based equations. This study developed and validated predictive equations of the %BF derived from anthropometric variables in children aged 7 to 10 living in Northeast Brazil, using dual-energy x-ray absorptiometry (DXA) as a reference. Body composition data from 58 children were utilized. DXA was used as a reference. A stepwise (forward) multiple regression statistical model was used to develop the new equations. The Bland-Altman analysis (CI: 95%), paired Student's t-test, and the intraclass correlation coefficient (ICC) was used to validate and compare the developed equations. Two new equations were developed for either gender: boys: %BF: 13.642 + (1.527*BMI) + (-0.345*Height) + (0.875*Triceps) + (0.290* Waist Circumference) and girls: %BF: -13.445 + (2.061*Tight). The Bland-Altman analysis showed good agreement, with limits ranging from -1.33 to 1.24% for boys and -3.35 to 4.08% for girls. The paired Student’s t-test showed no difference between %BF-DXA and the two new equations (p> 0.05), and the ICC was 0.948 and 0.915, respectively. DXA-based anthropometric equations provide an accurate and noninvasive method to measure changes in the %BF in children.
