ABSTRACT Introduction: Body composition assessment (BCA) using anthropometric measurements (AM) is used to monitor the nutritional and health status of the elderly. As predictor variables, MAs must be valid, practical, and quick, as they favor adherence and avoid possible resistance and embarrassment on the part of those being assessed and being minimally invasive. Objective: To develop and validate equations using accessible and minimally invasive anthropometric measurements for BCA in elderly women. Methods: 100 women (68.1±6.15 years) were randomly assigned to two groups: validation (n=40; 68.1±6.15 years); and estimation (n=60; 68.4±6.70 years). DXA was selected as the criterion measure, and MAs (body mass, height, skinfolds, circumferences) were selected as predictor variables. Means were compared using the paired Student's t-test; correlations were verified using Pearson's r-test; equations using Multiple Linear Regression. The level of agreement between the groups’ results was checked using the Bland-Altman technique. Results: Two equations developed and tested (E3 and E4) met the validation criteria as they showed adequate correlation coefficients (E3: r=0.73; E4: r=0.70), low constant errors (E3: EC= −0.56; E4: EC=-0.90), total error (E3: ET=3.22; E4: ET=3.06) lower than the Standard Error of Estimate (E3: EPE=3.24; E4: EPE=3.21), indicating no statistically significant difference between the two BCA techniques observed (p>0.05). The Bland-Altman technique showed good agreement between the results of the two techniques. Conclusion: Two were validated: E3 (%Gdxa= −41.556 + 4.041(BMI) + 0.165(DcCox) − 0.440(CircCox) + 0.269(CircQuad) − 0.053(BMI)²); and E4 (%GdxaE4= 15.329 + 1.044(BMI) −1.055(CircAbra) + 0.282(CircQuad) + 0.164(DcCox) − 0.262(CircCox)). Notably, the small number of measurements were located in areas of the body that are easily accessible and have little body exposure, which minimizes possible embarrassment and favors adherence by the elderly. RESUMEN Introducción: La evaluación de la composición corporal (ECC) mediante medidas antropométricas (MA) se utiliza para monitorizar el estado nutricional y de salud de las personas mayores. Las MA, como variables predictoras, deben ser válidas, prácticas y rápidas, ya que favorecen la adherencia y evitan posibles resistencias y vergüenzas por parte de los evaluados, además de ser mínimamente invasivas. Objetivo: Desarrollar y validar ecuaciones utilizando medidas antropométricas de fácil acceso y mínimamente invasivas para la ECC en mujeres de edad avanzada. Métodos: 100 mujeres (68,1±6,15 años) asignadas aleatoriamente a dos grupos: validación (n=40; 68,1±6,15 años); estimación (n=60; 68,4±6,70 años). Se seleccionó la DXA como medida criterio, y las MA (masa corporal, estatura, pliegues cutáneos, circunferencias) como variables predictoras. Las medias se compararon mediante la prueba t de Student emparejada; las correlaciones se comprobaron mediante la prueba r de Pearson; las ecuaciones mediante Regresión Lineal Múltiple. El nivel de concordancia entre los resultados de los grupos se comprobó mediante la técnica de Bland-Altman. Resultados: Dos ecuaciones desarrolladas y probadas (E3 y E4) cumplieron los criterios de validación, ya que mostraron coeficientes de correlación adecuados (E3: r=0,73; E4: r=0,70), errores constantes bajos (E3: EC= −0,56; E4: EC=-0,90), error total (E3: ET=3,22; E4: ET=3,06) inferior al error estándar de estimación (E3: EPE=3,24; E4: EPE=3,21), lo que indica que no se observaron diferencias estadísticamente significativas entre las dos técnicas de ECC (p>0,05). La técnica de Bland-Altman mostró una buena concordancia entre los resultados de las dos técnicas. Conclusión: Se validaron dos: E3 (%Gdxa= −41,556 + 4,041(IMC) + 0,165(DcCox) − 0,440(CircCox) + 0,269(CircQuad) − 0,053(IMC)²); y E4 (%GdxaE4= 15,329 + 1,044(IMC) −1,055(CircAbra) + 0,282(CircQuad) + 0,164(DcCox) − 0,262(CircCox)). Es de destacar que el reducido número de mediciones se localizaron en zonas del cuerpo de fácil acceso y con poca exposición corporal, lo que minimiza posibles situaciones embarazosas y favorece la adherencia por parte de los ancianos. Nivel de Evidencia IV; Estudio correlacional para construir una ecuación predictiva.

RESUMO Introdução: A avaliação da composição corporal (ACC) com auxílio das medidas antropométricas (MA) é utilizado no acompanhamento do estado nutricional e de saúde das populações idosas. As MAs como variáveis preditoras, devem ser válidas, práticas e rápidas, pois favorecem a adesão e evitam possíveis resistências e constrangimento das avaliadas, além de minimamente invasivas. Objetivo: Desenvolver e validar equações utilizando medidas antropométricas de fácil acesso e minimamente invasivas, para ACC de idosas. Métodos: 100 mulheres (68,1±6,15 anos) distribuídas aleatoriamente por dois grupos: validação (n=40; 68,1±6,15 anos); estimativa (n=60; 68,4±6,70 anos). Como medida critério selecionou-se a DXA, como variáveis preditoras as MAs (massa corporal, estatura, dobras cutâneas, circunferências). As médias foram comparadas com o teste t de Student pareado; as correlações verificadas com o teste r Pearson; as equações com a Regressão Linear Múltipla. O nível de concordância entre os resultados dos grupos foi verificado com a técnica Bland-Altman. Resultados: Duas equações desenvolvidas e testadas (E3 e E4) atenderam aos critérios de validação, pois, apresentaram coeficientes de correlação adequados (E3: r=0,73; E4: r=0,70), erros constantes baixos (E3: EC= −0,56; E4: EC=-0,90), erro total (E3: ET=3,22; E4: ET=3,06) menores que o Erro Padrão de Estimativa (E3: EPE=3,24; E4: EPE=3,21), indicando não existir diferença estatisticamente significativa entre as duas técnicas de ACC observadas (p>0,05). A técnica Bland-Altman demonstrou boa concordância entre os resultados das duas técnicas. Conclusão: Duas equações foram validadas: E3 (%Gdxa= −41,556 + 4,041(IMC) + 0,165(DcCox) – 0,440(CircCox) + 0,269(CircQuad) – 0,053(IMC)²); e E4 (%GdxaE4= 15,329 + 1,044(IMC) −1,055(CircAbra) + 0,282(CircQuad) + 0,164(DcCox) – 0,262(CircCox)). Destaca-se o fato do reduzido número de medidas situarem-se em áreas corporais de fácil acesso e de pouca exposição corporal, que minimizam possíveis constrangimentos e favorecem a adesão das idosas. ABSTRACT Introduction: Due to Brazilian population aging, prevalence of aortic stenosis, and limited number of scores in literature, it is essential to develop risk scores adapted to our reality and created in the specific context of this disease. Methods: This is an observational historical cohort study with analysis of 802 aortic stenosis patients who underwent valve replacement at Hospital São Lucas, Pontifícia Universidade Católica do Rio Grande do Sul, from 1996 to 2018. With the aid of logistic regression, a weighted risk score was constructed based on the magnitude of the coeficients β of the logistic equation. Two performance statistics were obtained: area under the receiver operating characteristic curve and the chi-square (χ2) of Hosmer-Lemeshow goodness-of-fit with Pearson’s correlation coeficient between the observed events and predicted as a model calibration estimate. Results: The risk predictors that composed the score were valve replacement surgery combined with coronary artery bypass grafting, prior renal failure, New York Heart Association class III/IV heart failure, age > 70 years, and ejection fraction < 50%. The receiver operating characteristic curve area was 0.77 (95% confidence interval: 0.72-0.82); regarding the model calibration estimated between observed/predicted mortality, Hosmer-Lemeshow test χ2 = 3,70 (P=0.594) and Pearson’s coeficient r = 0.98 (P<0.001). Conclusion: We propose the creation of a simple score, adapted to the Brazilian reality, with good performance and which can be validated in other institutions.

Abstract Background Some studies have suggested the HLA-B27 gene may protect against some infections, as well as it could play a benefit role on the viral clearance, including hepatitis C and HIV. However, there is lack of SARS-CoV-2 pandemic data in spondyloarthritis (SpA) patients. Aim To evaluate the impact of HLA-B27 gene positivity on the susceptibility and severity of COVID-19 and disease activity in axial SpA patients. Methods The ReumaCoV-Brasil is a multicenter, observational, prospective cohort designed to monitor immunemediated rheumatic diseases patients during SARS-CoV-2 pandemic in Brazil. Axial SpA patients, according to the ASAS classification criteria (2009), and only those with known HLA-B27 status, were included in this ReumaCov-Brasil's subanalysis. After pairing them to sex and age, they were divided in two groups: with (cases) and without (control group) COVID-19 diagnosis. Other immunodeficiency diseases, past organ or bone marrow transplantation, neoplasms and current chemotherapy were excluded. Demographic data, managing of COVID-19 (diagnosis, treatment, and outcomes, including hospitalization, mechanical ventilation, and death), comorbidities, clinical details (disease activity and concomitant medication) were collected using the Research Electronic Data Capture (REDCap) database. Data are presented as descriptive analysis and multiple regression models, using SPSS program, version 20. P level was set as 5%. Results From May 24th, 2020 to Jan 24th, 2021, a total of 153 axial SpA patients were included, of whom 85 (55.5%) with COVID-19 and 68 (44.4%) without COVID-19. Most of them were men (N = 92; 60.1%) with mean age of 44.0 ± 11.1 years and long-term disease (11.7 ± 9.9 years). Regarding the HLA-B27 status, 112 (73.2%) patients tested positive. There were no significant statistical differences concerning social distancing, smoking, BMI (body mass index), waist circumference and comorbidities. Regarding biological DMARDs, 110 (71.8%) were on TNF inhibitors and 14 (9.15%) on IL-17 antagonists. Comparing those patients with and without COVID-19, the HLA-B27 positivity was not different between groups (n = 64, 75.3% vs. n = 48, 48%, respectively; p = 0.514). In addition, disease activity was similar before and after the infection. Interestingly, no new episodes of arthritis, enthesitis or extra-musculoskeletal manifestations were reported after the COVID-19. The mean time from the first symptoms to hospitalization was 7.1 ± 3.4 days, and although the number of hospitalization days was numerically higher in the B27 positive group, no statistically significant difference was observed (5.7 ± 4.11 for B27 negative patients and 13.5 ± 14.8 for B27 positive patients; p = 0.594). Only one HLA-B27 negative patient died. No significant difference was found regarding concomitant medications, including conventional or biologic DMARDs between the groups. Conclusions No significant difference of COVID-19 frequency rate was observed in patients with axial SpA regarding the HLA-B27 positivity, suggesting a lack of protective effect with SARS-CoV-2 infection. In addition, the disease activity was similar before and after the infection. Trial registration This study was approved by the Brazilian Committee of Ethics in Human Research (CONEP), CAAE 30186820.2.1001.8807, and was registered at the Brazilian Registry of Clinical Trials – REBEC, RBR-33YTQC. All patients read and signed the informed consent form before inclusion.

Resumo Fundamento A COVID-19 adicionou um fardo enorme sobre os médicos ao redor do mundo, especialmente as mulheres médicas, que são afetadas pelo aumento da carga de trabalho e pela perda da qualidade de vida. Objetivo Avaliar os efeitos da pandemia de COVID-19 na qualidade de vida, burnout e espiritualidade de médicas brasileiras que atendem pacientes com COVID-19 direta ou indiretamente. Método Estudo prospectivo, observacional realizado de 28 de julho a 27 de setembro de 2020, no Brasil, com mulheres médicas de 47 especialidades, a mais frequente sendo a cardiologia (22,8%), sem restrição de idade. Elas responderam voluntariamente um questionário online com questões sobre características demográficas e socioeconômicas, qualidade de vida (WHOQOL-brief) e espiritualidade (WHOQOL-SRPB) e enunciados do Oldenburg Burnout Inventory. A análise estatística utilizou o software R, regressão beta, árvores de classificação e matriz de correlação policórica, com nível de significância de 5%. Resultados Das 769 respondentes, 61,6% relataram sinais de burnout. Cerca de 64% relataram perda salarial de até 50% durante a pandemia. Algumas relataram falta de energia para as tarefas diárias, sentimentos negativos frequentes, insatisfação com a capacidade para o trabalho, e que cuidar de outras pessoas não agregava sentido às suas vidas. Os sentimentos negativos correlacionaram-se negativamente com a satisfação com a vida sexual, a satisfação com as relações pessoais e a energia para as tarefas diárias. A incapacidade de permanecer otimista em tempos de incerteza correlacionou-se positivamente com a sensação de insegurança no dia a dia e com o não reconhecimento de que cuidar de outras pessoas trouxesse sentido à vida. Conclusão O presente estudo mostrou uma alta frequência de burnout entre as médicas brasileiras que responderam ao questionário durante a pandemia de COVID-19. Apesar disso, apresentavam uma qualidade de vida relativamente boa e acreditavam que a espiritualidade trazia-lhes conforto e segurança nos momentos difíceis.

Abstract Background COVID-19 has placed a tremendous burden on physicians worldwide, especially women physicians, affected by increased workload and loss of quality of life. Objective To assess the effects of the COVID-19 pandemic on the quality of life, burnout and spirituality of Brazilian women physicians directly or indirectly providing care to COVID-19 patients. Methods Prospective, observational study performed from July 28 to September 27, 2020, in Brazil, with women physicians from 47 specialities, the most frequent being cardiology (22.8%), with no age restriction. They voluntarily answered an online survey with questions on demographic and socioeconomic characteristics, quality of life (WHOQOL-brief), spirituality (WHOQOL-SRPB), and statements from the Oldenburg Burnout Inventory. Statistical analysis used the R software, beta regression, classification trees, and polychoric correlation matrix, with a 5% of significance level. Results Of the 769 respondents, 61.6% reported signs of burnout. About 64% reported wage loss of up to 50% during the pandemic. Some reported lack of energy for daily tasks, frequent negative feelings, dissatisfaction with capability for work, and caring for others not adding meaning to their lives. Negative feelings correlated negatively with satisfaction with sexual life and personal relations, and energy for daily tasks. The inability to remain optimistic in times of uncertainty correlated positively with feeling unsafe daily and not acknowledging that caring for others brings meaning to life. Conclusion This study showed a high frequency of burnout among Brazilian women physicians who answered the survey during the COVID-19 pandemic. Nevertheless, they presented with a relatively good quality of life and believed that spirituality comforted and reassured them in hard times.

ABSTRACT This study simulates an unusual extreme rainfall event that occurred in Salvador city, Bahia, Brazil, on December 9, 2017, which was named subtropical storm Guará and had precipitation of approximately 24 mm within less than 1 h. Numerical simulations were conducted using the Weather Research and Forecasting (WRF) model over three domains with horizontal resolutions of 9, 3, and 1 km. Different combinations of seven microphysics, three cumulus, and three planetary boundary layer schemes were evaluated based on their ability to simulate the hourly precipitation during this rainfall event. Statistical indices (MB = -0.69; RMSE = 4.11; MAGE = 1.74; r = 0.55; IOA = 0.66; FAC2 = 0.58) and time series plots showed that the most suitable configurations for this weather event were the Mellor-Yamada-Janjić, Grell-Freitas, and Lin formulations for the planetary boundary layer, cumulus, and microphysics schemes, respectively. The results were compared with the data measured at meteorological stations located in Salvador city. The WRF model simulated well the arrival and occurrence of this extreme weather event in a tropical and coastal region, considering that the region already has intense convective characteristics and is constantly influenced by sea breezes, which could interfere in the model results and compromise the performance of the simulations.

RESUMEN El presente estudio simula un evento inusual de lluvia extrema ocurrido en la ciudad de Salvador, Bahía, Brasil, el 9 de diciembre de 2017, que se denominó tormenta subtropical Guará y tuvo una precipitación de aproximadamente 24 mm en menos de 1 h. Se realizaron simulaciones numéricas utilizando el modelo Weather Research and Forecasting (WRF) en tres dominios con resoluciones horizontales de 9, 3 y 1 km. Se evaluaron diferentes combinaciones de siete esquemas de microfísica, tres cumulus y tres capas límites planetarias en función de su capacidad para simular la precipitación horaria durante este evento de lluvia. Los índices estadísticos (MB = -0.69; RMSE = 4.11; MAGE = 1.74; r = 0.55; IOA = 0.66, y FAC2 = 0.58) y los gráficos de series temporales mostraron que las configuraciones más adecuadas para este evento meteorológico fueron las de Mellor-Yamada-Janjić, Grell-Freitas y Lin para los esquemas de capa límite planetaria, cumulus y microfísica, respectivamente. Los resultados se compararon con los datos medidos en las estaciones meteorológicas ubicadas en la ciudad de Salvador. El modelo WRF simuló bien la llegada y ocurrencia de este evento climático extremo en una región tropical y costera, considerando que la región ya tiene características convectivas intensas y está constantemente influenciada por brisas marinas, las cuales podrían interferir con los resultados del modelo y comprometer el desempeño de las simulaciones.

Abstract: The FACE-Q SFAOS is an instrument developed for esthetic facial procedures and has been validated in patients undergoing rhinoplasty. It consists of ten items that assess the current perception of facial appearance regarding symmetry, harmony, proportion, freshness or vitality, and self-perception of appearance; these items are perhaps important to guide demands in orofacial harmonization (OFH). We aimed to validate the FACE-Q SFAOS scale among professionals working with OFH and verify its acceptance for use in clinical practice. Altogether, 25 OFH specialists were included to evaluate the FACE-Q SFAOS scale and the new technology acceptability model (TAM). Internal validity was measured using Cronbach's α coefficient. Both scales were associated with clinical experience and perceived usefulness using Spearman's correlation and Fisher's exact or chi-square tests (SPSS 20.0, p < 0.05). The FACE-Q SFAOS (α = 0.927) and TAM (α = 0.941) scales demonstrated good internal validity, and most professionals demonstrated good acceptance (TAM >70 = 64%). The mean FACE-Q SFAOS and TAM scores were 64.00±17.03 and 74.60±20.66, respectively. Practitioners with more than two years of experience (p=0.032) who believed the scale was useful for lip filler evaluation (p = 0.017) demonstrated greater acceptance. The number of indications on the scale was directly correlated with higher levels of acceptance (p = 0.002, r = 0.594). Thus, the FACE-Q SFAOS scale has good reproducibility and acceptance among OFH specialists; however, acceptability was shown to be dependent on perceived usability.

Resumo Fundamento: O teste do degrau de seis minutos (TD6) é uma forma simples de avaliar a capacidade funcional, embora tenha sido pouco estudado em pacientes com doença arterial coronariana (DAC) ou insuficiência cardíaca (IC). Objetivo: Analisar a associação entre o TD6 e o consumo de oxigênio de pico (VO2pico) e desenvolver uma equação que estime o VO2pico com base no TD6, bem como determinar um ponto de corte para o TD6 que preveja um VO2pico ≥ 20 mL.kg-1.min-1 Métodos: Nos 171 pacientes submetidos ao TD6 e a um teste de exercício cardiopulmonar, análises da curva ROC, de regressão e de correlação foram usadas, e um p < 0,05 foi admitido como significativo. Resultados: A idade média foi 60±14 anos, e 74% eram do sexo masculino. A média da fração de ejeção ventricular esquerda foi 57±16%; 74% apresentavam DAC, e 28%, IC. A média do VO2pico foi 19±6 mL.kg-1.min-1, e o desempenho médio do TD6 foi 87±45 passos. A associação entre o TD6 e o VO2pico foi r 0,69 (p < 0,001). Os modelos VO2pico = 19,6 + (0,075 x TD6) – (0,10 x idade) para homens e VO2pico = 19,6 + (0,075 x TD6) – (0,10 x idade) – 2 para mulheres poderiam prever o VO2pico com base nos resultados do TD6 (R ajustado 0,72; R2 ajustado 0,53). O ponto de corte mais acurado para que o TD6 preveja um VO2pico ≥ 20 mL.kg-1.min-1 foi de > 105 passos [área sob a curva 0,85; intervalo de confiança de 95% 0,79 - 0,90; p < 0,001]. Conclusão: Uma equação que preveja o VO2pico com base nos resultados do TD6 foi derivada, e foi encontrada uma associação significativa entre o TD6 e o VO2pico. O ponto de corte do TD6, que prevê um VO2pico ≥ 20 mL.kg-1.min-1, foi > 105 passos. (Arq Bras Cardiol. 2021; 116(5):889-895)

Abstract Background: Six-minute step test (6MST) is a simple way to evaluate functional capacity, although it has not been well studied in patients with coronary artery disease (CAD) or heart failure (HF). Objective: Analyze the association between the 6MST and peak oxygen uptake (VO2peak) and develop an equation for estimating VO2peak based on the 6MST, as well as to determine a cutoff point for the 6MST that predicts a VO2peak ≥20 mL.Kg-1.min-1 Methods: In 171 patients who underwent the 6MST and a cardiopulmonary exercise test, correlation, regression, and ROC analysis were used and a p < 0.05 was admitted as significant. Abstract Background Prolonged mechanical ventilation (MV) after cardiac surgery imposes a significant burden on the patient in terms of morbidity and financial hospital costs. Objective To develop a risk score model to predict prolonged MV in patients undergoing coronary artery bypass grafting (CABG) surgery. Methods This was a historical cohort study of 4165 adult patients undergoing CABG between January 1996 and December 2016. MV for periods ≥ 12 hours was considered prolonged. Logistic regression was used to examine the relationship between risk predictors and prolonged MV. The variables were scored according to the odds ratio. To build the risk score, the database was randomly divided into 2 parts: development data set (2/3) with 2746 patients and internal validation data set (1/3) with 1419 patients. The final score was validated in the total database and the model's accuracy was tested by performance statistics. Significance was established at p < 0.05. Results Prolonged MV was observed in 783 (18.8%) patients. Predictors of risk were age ≥ 65 years, urgent/emergency surgery, body mass index ≥ 30 kg/m2, chronic kidney disease, chronic obstructive pulmonary disease, and cardiopulmonary bypass time ≥ 120 minutes. The area under the ROC curve was 0.66 (95% CI, 0.64-0.68; p<0.001), the Hosmer-Lemeshow chi-square test was χ2: 3.38 (p=0.642), and Pearson's correlation was r = 0.99 (p<0.001), indicating the model’s satisfactory ability to predict the occurrence of prolonged MV. Conclusion Selected variables allowed the construction of a simplified risk score for daily practice, which may classify the patients as having low, moderate, high, and very high risk. RESUMO A murcha bacteriana limita a produção do tomateiro sob condições de alta temperatura e umidade, e porta-enxertos resistentes são importantes para o manejo integrado da doença. Visto que existe interação entre isolados locais e genótipos de tomateiros, objetivou-se avaliar 14 porta-enxertos de tomateiro à murcha bacteriana na mesorregião da Mata de Pernambuco, Brasil. A reação dos porta-enxertos a dois sequevares de Ralstonia solanacearum e dois de R. pseudosolanacearum foi avaliada em quatro experimentos realizados em casa de vegetação em delineamento inteiramente casualizado, com quatro repetições, com quatro plantas cada. Sete genótipos foram selecionados para avaliar a reação à doença como porta-enxertos de tomateiro ‘Tomini F1’ em uma área de produção com histórico da doença em Chã Grande, Pernambuco, Brasil, utilizando delineamento em blocos casualizados, com quatro plantas por tratamento por bloco. Não foram observados sintomas da doença nas plantas enxertadas em ‘Guardião’, ‘Woodstock’ e ‘Yoshimatsu’. Considerando todos os experimentos, ‘Guardião’ e ‘Muralha’ apresentaram os melhores níveis de resistência, podendo ser utilizados no manejo integrado da murcha bacteriana e estudados em programas de melhoramento.

ABSTRACT Bacterial wilt limits tomato production and resistant rootstocks could be important for the integrated management of the disease. Since there is an interaction between local bacterial strains and tomato genotype, this study aimed to evaluate 14 tomato rootstocks to bacterial wilt in the Mata mesoregion of Pernambuco state, Brazil. The rootstocks reaction to two sequevars of Ralstonia solanacearum and two of R. pseudosolanacearum was evaluated in four experiments carried out in the greenhouse using the completely randomized experimental design, with four replications composed of four plants each. Seven genotypes were selected to evaluate the reaction to bacterial wilt as rootstocks grafting in tomato plants ‘Tomini F1’ in a production area with disease history in the Chã Grande municipality, using randomized block design with four plants per treatment in each block. In the field experiment, disease symptoms were not observed in the grafted plants in ‘Guardião’, ‘Woodstock’, and ‘Yoshimatsu’. Regarding all experiments, ‘Guardião’ and ‘Muralha’ showed the best resistance levels and could be used in the integrated management of bacterial wilt and studied in plant breeding programs.

ABSTRACT Objective: to investigate the factors that are present in the work environment of the nursing team and that contribute to emergence of psychic burden in Psychosocial Care Centers III. Methods: this is a descriptive and qualitative study carried out in three services located in Paraíba State. Data were obtained from an interview with a semi-structured script, and textual content was treated using the software Interface de R pour Analyzes Multidimensionnelles de Textes et de Questionneires. Results: the factors that trigger psychic burden arise primarily from pace of work, precarious physical structure, work with users in mental distress, lack of management support, insufficient multidisciplinary team and lack of clinical supervision. Conclusion: the difficulties faced by nursing professionals in the work environment cause suffering at work and hinder the effectiveness and, consequently, the quality of nursing care.

RESUMEN Objetivo: investigar los factores que se encuentran presentes en el ambiente laboral del equipo de enfermería y que contribuyen al surgimiento de cargas de trabajo psíquicas en los Centros de Atención Psicosocial III. Métodos: estudio descriptivo con abordaje cualitativo, realizado en tres servicios ubicados en el estado de Paraíba. Los datos se obtuvieron de una entrevista con un guión semiestructurado, y el contenido textual se trató con el software Interface de R pour Analyzes Multidimensionnelles de Textes et de Questionneires. Resultados: los factores desencadenantes de cargas psíquicas surgen principalmente del ritmo de trabajo, la precaria estructura física, el trabajo con usuarios en distrés mental, la falta de apoyo gerencial, el insuficiente equipo multidisciplinario y la falta de supervisión clínica. Conclusión: las dificultades que enfrentan los profesionales de enfermería en el ámbito laboral provocan sufrimiento en el trabajo, afectando la efectividad del cuidado y la calidad del cuidado de enfermería.

Métodos: estudo descritivo, de abordagem qualitativa, realizado em três serviços localizados no estado da Paraíba. Os dados foram obtidos a partir de entrevista com roteiro semiestruturado, e o conteúdo textual foi tratado por meio do software Interface de R pour Analyses Multidimensionnelles de Textes et de Questionneires. Resultados: os fatores desencadeantes das cargas psíquicas surgem prioritariamente a partir do ritmo de trabalho, da estrutura física precária, do trabalho com usuário em sofrimento mental, da falta de apoio da gestão, da equipe multidisciplinar insuficiente e da falta de supervisão clínica. Conclusão: as dificuldades enfrentadas pelos profissionais de enfermagem no ambiente laboral ocasionam sofrimento no trabalho, prejudicando a efetividade do atendimento e, consequentemente, a qualidade da assistência de enfermagem.

OBJECTIVES: This observational, cross-sectional study based aimed to test whether heart failure (HF)-disease management program (DMP) components are influencing care and clinical decision-making in Brazil. METHODS: The survey respondents were cardiologists recommended by experts in the field and invited to participate in the survey via printed form or email. The survey consisted of 29 questions addressing site demographics, public versus private infrastructure, HF baseline data of patients, clinical management of HF, performance indicators, and perceptions about HF treatment. RESULTS: Data were obtained from 98 centers (58% public and 42% private practice) distributed across Brazil. Public HF-DMPs compared to private HF-DMP were associated with a higher percentage of HF-DMP-dedicated services (79% vs 24%; OR: 12, 95% CI: 94-34), multidisciplinary HF (MHF)-DMP [84% vs 65%; OR: 3; 95% CI: 1-8), HF educational programs (49% vs 18%; OR: 4; 95% CI: 1-2), written instructions before hospital discharge (83% vs 76%; OR: 1; 95% CI: 0-5), rehabilitation (69% vs 39%; OR: 3; 95% CI: 1-9), monitoring (44% vs 29%; OR: 2; 95% CI: 1-5), guideline-directed medical therapy-HF use (94% vs 85%; OR: 3; 95% CI: 0-15), and less B-type natriuretic peptide (BNP) dosage (73% vs 88%; OR: 3; 95% CI: 1-9), and key performance indicators (37% vs 60%; OR: 3; 95% CI: 1-7). In comparison to non- MHF-DMP, MHF-DMP was associated with more educational initiatives (42% vs 6%; OR: 12; 95% CI: 1-97), written instructions (83% vs 68%; OR: 2: 95% CI: 1-7), rehabilitation (69% vs 17%; OR: 11; 95% CI: 3-44), monitoring (47% vs 6%; OR: 14; 95% CI: 2-115), GDMT-HF (92% vs 83%; OR: 3; 95% CI: 0-15). In addition, there were less use of BNP as a biomarker (70% vs 84%; OR: 2; 95% CI: 1-8) and key performance indicators (35% vs 51%; OR: 2; 95% CI: 91,6) in the non-MHF group. Physicians considered changing or introducing new medications mostly when patients were hospitalized or when observing worsening disease and/or symptoms. Adherence to drug treatment and non-drug treatment factors were the greatest medical problems associated with HF treatment. CONCLUSION: HF-DMPs are highly heterogeneous. New strategies for HF care should consider the present study highlights and clinical decision-making processes to improve HF patient care.

ASSESSMENT OF THE ELECTRONIC STRUCTURE OF THE MONOCLINIC PHASE OF NIOBIUM OXIDE BASED ON THE USE OF DIFFERENT DENSITY FUNCTIONALS. Niobium oxides, Nb2O5, are considered semiconductor materials with very attractive physical and chemical properties for applications in many areas, such as catalysis, sensors, medical, aerospace, etc. Especially, the characterization of Nb2O5-based nanostructures with monoclinic structure has received much attention in recent years. However, despite the great importance of this system, some of its fundamentals properties are still not fully understood. Hence, this work aims to apply the theoretical methodologies through Density Functional Theory (DFT) calculations in periodic models based on the use of different density functionals (like B1WC, B3PW, B3LYP, PBE0, PBESOL0, SOGGAXC, and WC1LYP) to investigate the physical and chemical properties of the monoclinic structure of Nb2O5. The band structures, energy bandgap, density of state, and vibrational properties, as well as order-disorder effects on the monoclinic structure of Nb2O5 are investigated in this study. Our theoretical results show a better agreement with experimental data for the B3LYP functional and hence lead to new perspectives on the deeper physicochemical understanding of the monoclinic Nb2O5. From these computational tools, it is possible to unravel the relations between structure and properties, which may contribute to the future development of new devices and applications based on these materials.

RESUMO O milho é bastante cultivado na região semiárida brasileira e contém alto valor energético na dieta animal. Sabendo disto, objetivou-se caracterizar diferentes classes genéticas de milho quanto ao seu potencial forrageiro nas condições semiáridas de Sergipe. Para isso, foram avaliados 36 genótipos em blocos incompletos em Látice quadrado reticulado 6x6, com duas repetições, na fazenda experimental da Embrapa – Semiárido, em Graccho Cardoso-SE. As variáveis analisadas foram: altura da planta, diâmetro do colmo, peso de planta, massa fresca, massa seca, porcentagem de massa seca e massa de forragem. Com os dados obtidos foi realizada análise de variância com auxílio do software SAS 9,4 e, para a geração dos componentes principais (CP) e gráficos, foi utilizado o software R com o pacote “GGEBiplotGUI”. Todas as variáveis apresentaram diferenças significativas entre classes genéticas. Os dois primeiros componentes principais contemplaram 83,9% da variação total. O autovalor do primeiro CP (CP1) foi de 4,11, considerado muito significativo, já que a soma de todos os autovalores foi de 6,99. A porcentagem de massa seca, diâmetro do colmo, peso de planta, massa fresca, seca e de forragem obtiveram maior capacidade de discriminar as classes genotípicas. Os híbridos intervarietais ficaram mais próximos do genótipo “ideal”. A maior correlação positiva foi entre massa de forragem e massa seca. Os híbridos simples e triplos destacaram-se individualmente na altura da planta e porcentagem de massa. A massa seca e de forragem obtiveram maiores médias em todas as classes, exceto nos híbridos simples. As variedades foram as menos estáveis nas avaliações. Os híbridos intervarietais apresentaram maior potencial forrageiro e os duplos, topcross e variedades maior variabilidade genética, as demais classes não são indicadas para agricultores de baixa e média tecnologia quando cultivados em região semiárida para forragem de milho.

ABSTRACT Corn is widely grown in the semi-arid region and contains high energy value in the animal diet. Thus, the aim of this study was to characterize different genetic classes of maize cultivated for forage potential in the semiarid conditions the Sergipe. For this, 36 genotypes were evaluated in incomplete blocks in lattice square reticulate 6x6, with two replications, in the experimental farm of Embrapa - Semiárido, in Graccho Cardoso-SE. The variables analyzed were: plant height, stem diameter, plant weight, fresh weight, dry weight, percentage of dry weight and forage weight. With the data obtained was accomplished an analysis of variance with the help of software SAS 9,4 and, for generation the principal component analysis (PCA) and graphics, was used the software R with the package “GGEBiplotGUI”. All the variables showed significant differences between genetic classes. The first two main components included 83.9% of the total variation. The eigenvalues of the first PC (PC1) were 4,11, considering more significant, since the sum of all was of 6,99. The percentage of dry mass, stem diameter, plant weight, fresh, dry and forage mass obtained greater hability to discriminate genotypic classes. The intervarietal hybrids were closer to the “ideal” genotype. The greatest positive correlation was between forage weight and dry weight. The single and triple hybrids stood out individually in plant height and percentage of mass. The dry and forage mass obtained higher averages in all classes, except for simple hybrids. The varieties were the least stable in the evaluations. The intervarietal hybrids showed greater forage potential and the double, topcross and varieties had greater genetic variability, already to the genetic classes they are not indicated for farmers of low and mean tech in the semiarid region for forage of maize.
