Introduction: Invasive ductal carcinoma corresponds to the most common histological type of the breast, coexisting with different forms of clinical evolution, histological grading, expression of certain tissue markers and genomic profiles that seek a better understanding of the disease. Objectives: To analyze the correlation of β-catenin and AXL markers with tumor aggressiveness, with reference to overall survival, tumor progression and histopathological prognostic factors. Methods: A study of 101 samples of invasive ductal mammary carcinoma was performed. Those with a diagnosis of ductal type, initially submitted to biopsy or definitive surgical treatment, were included. For control purposes, 20 samples of intraductal carcinoma, 35 of breast fibroadenoma and 10 of breast tissue without any alteration were included. Those undergoing neoadjuvant chemotherapy, those without a tumor sample prior to chemotherapy, those lost to follow-up, and those with incomplete data, were excluded. Results: When the β-catenin expression was analyzed, it was negative. As for AXL, different degrees of expression were observed without statistical significance between them. Conclusion: When analyzing invasive ductal breast adenocarcinoma in TMA, there was no correlation in the expression of ß-Catenin and AXL when compared to overall survival, tumor progression and histological grade.

Introdução: O carcinoma ductal invasor corresponde ao tipo histológico mais comum da mama coexistindo com formas diferentes de evolução clínica, graduação histológica, expressão de determinados marcadores teciduais e perfis genômicos que procuram melhor entendimento da doença. Objetivos: Analisar a correlação dos marcadores β-catenina e AXL com a agressividade tumoral, tendo como referência a sobrevida global, progressão tumoral e fatores prognósticos histopatológicos. Métodos: Foi realizado estudo de 101 amostras de carcinoma mamário ductal invasor. Foram incluídas aquelas com diagnóstico do tipo ductal, submetidas inicialmente à biópsia ou tratamento cirúrgico definitivo. Incluiu-se para fins de controle 20 amostras de carcinoma intraductal, 35 de fibroadenoma mamário e 10 de tecido mamário sem qualquer alteração. Foram excluídos os submetidos à quimioterapia neoadjuvante, que não tivessem amostra tumoral prévia ao tratamento quimioterápico, que perderam o seguimento, e com dados incompletos. Resultados: Quando analisada a expressão da β-catenina, foi negativa. Quanto ao AXL foram observados diferentes graus de expressão sem significância estatística entre eles. Conclusão: Quando analisados adenocarcinoma mamário do tipo ductal invasor em TMA não houve correlação na expressão de ß-catenina e AXL quando comparados a sobrevida global, progressão tumoral e grau histológico.

RESUMO Racional: Atualmente a infecção persistente pelo papilomavírus humano (HPV) tem sido relacionada em algumas regiões geográficas como fator de risco para o carcinoma epidermoide do esôfago. Ela resulta na imunoexpressão da proteína p16, que tem sido utilizada como marcadora da linhagem oncogênica por este agente etiológico. Objetivo: Correlacionar aspectos epidemiológicos do carcinoma epidermoide do esôfago com a prevalência de infecção pelo HPV. Métodos: Foram analisados 58 casos buscando-se perfil epidemiológico dos pacientes, com suas peças submetidas à análise histopatológica e imunoistoquímica pelo p16. Resultado: Dos 58 casos avaliados, 40 eram homens e 18 mulheres, com idade média de 63,2 anos. A imunoexpressão pelo p16 foi de 46,55%. Conclusão: A prevalência de infecção pelo HPV é alta no carcinoma epidermoide de esôfago apresentando-se em quase a metade dos casos (46,55%), sem diferenciação de idade quanto aos gêneros.

ABSTRACT Background: Currently, persistent human papillomavirus (HPV) infection has been related in some geographic regions as a risk factor for esophageal squamous cell carcinoma. It results in the immunoexpression of the p16 protein, which has been used as marker of the oncogenic lineage by this etiological agent. Aim: To correlate epidemiological aspects of esophageal squamous cell carcinoma with the prevalence of HPV infection. Methods: Fifty-eight cases were analyzed and submitted to histopathological and immunohistochemical analysis by p16. Results: Of the 58 cases evaluated, 40 were men and 18 women, with a mean age of 63.2 years. p16 immunoexpression was positive in 46.55%. Conclusion: The prevalence of HPV infection is high in esophageal squamous cell carcinoma presenting in almost half of the cases (46.55%), without gender differentiation.

RESUMO Racional: A criação de um banco de dados clínicos informatizado com a capacidade de coletar informações dos pacientes de forma prospectiva e com possibilidade de resgate e cruzamento viabiliza estudos científicos de maior qualidade e credibilidade em menor tempo. Objetivos: Validar em único protocolo mestre os dados clínicos referentes à Cirurgia do Aparelho Digestivo de forma multiprofissional incorporando-o na plataforma SINPE©, e verificar a incidência das doenças digestivas com base nas coletas prospectivamente realizadas. Método: Organizar no software em estrutura padronizada todos os itens pré-existentes no banco de dados do SINPE©, informatizar a base teórica através do módulo MIGRASINPE© criando-se um único protocolo mestre multiprofissional para uso como um todo. Resultados: Foram criados e/ou adaptados os protocolos específicos existentes que correspondem às doenças digestivas mais prevalentes unificando-os. Criou-se a possibilidade de uso multiprofissional integrando todos os dados coletados da medicina, enfermagem, fisioterapia, nutrição e gestão em saúde de maneira prospectiva. O total foi de 4.281 coletas assim distribuídas: vias biliares extra-hepáticas, n=1.786; esôfago, n=1015; anorretais, n=736; cólon, n=550; intestino delgado, n=86; pâncreas, n=71; estômago, n=23; fígado, n=14. Conclusões: A validação da unificação e estruturação em único protocolo mestre dos dados clínicos referentes à Cirurgia do Aparelho Digestivo de forma multiprofissional e prospectiva foi possível e o estudo epidemiológico realizado permitiu identificar as doenças mais prevalentes nesse aparelho.

Abstract Background: The creation of a computerized clinical database with the ability to collect prospective information from patients and with the possibility of rescue and crossing data enables scientific studies of higher quality and credibility in less time. Aim: To validate, in a single master protocol, the clinical data referring to Surgery of Digestive System in a multidisciplinary way, incorporating in the SINPE© platform, and to verify the incidence of digestive diseases based on the prospectively performed collections. Method: Organize in one software, in a standardized structure, all the pre-existing items in the SINPE© database; the theoretical basis was computerized through the MIGRASINPE© module creating a single multiprofessional master protocol for use as a whole. Results: The existing specific protocols were created and/or adapted - they correspond to the most prevalent digestive diseases - unifying them. The possibility of multiprofessional use was created by integrating all data collected from medicine, nursing, physiotherapy, nutrition and health management in a prospective way. The total was 4,281 collections, distributed as follows: extrahepatic biliary tract, n=1,786; esophagus, n=1015; anorectal, n=736; colon, n=550; small intestine, n=86; pancreas, n=71; stomach, n=23; liver, n=14. Conclusions: The validation of the unification and structuring in a single master protocol of the clinical data referring to the Surgery of the Digestive System in a multiprofessional and prospective way was possible and the epidemiological study carried out allowed to identify the most prevalent digestive diseases.

RESUMO Racional: A resposta metabólica ao trauma cirúrgico é potencializada pelo jejum pré-operatório prolongado que contribui para o aumento da resistência à insulina. Esta manifestação é mais intensa no 1º e 2º dias de pós-operatório e é diretamente proporcional ao porte da operação. Objetivo: Comparar se a abreviação do jejum pré-operatório e a realimentação precoce no pós-operatório associado à restrição hídrica no trans e pós-operatório interferem na evolução dos pacientes submetidos ao bypass gastrojejunal. Métodos: Foram recrutados 80 pacientes indicados ao bypass gastrojejunal em Y-de-Roux. Eles foram distribuídos randomicamente em dois grupos: ringer lactato (RL) que fizeram jejum de 6 h para sólidos, administrando 50 g de maltodextrina em 100 ml de água mineral 2 h antes do início da anestesia e de soro fisiológico (SF) que fizeram jejum de 12 h para sólidos e líquidos. A anestesia foi padronizada para os dois grupos. Durante o procedimento operatório no RL foi administrado 1500 ml solução de ringer lactato e no SF 2500 ml de soro fisiológico (0,9% de cloreto de sódio). Em ambos os grupos foram analisados durante a intubação a ocorrência ou não de bronco-aspiração e mensurado o volume gástrico residual após abertura da cavidade abdominal. No pós-operatório do Grupo RL, os pacientes iniciaram dieta liquida após 24 h do término do procedimento operatório; no Grupo SF foi mantido jejum nas primeiras 24 h, prescrição de 2000 ml de soro fisiológico e início da dieta líquida restrita com 36 h. Cada paciente realizou no pós-operatório imediato, ainda na sala de cirurgia, a dosagem de CPK, insulina, sódio, potássio, ureia, creatinina, PaCO2, pH e bicarbonato e em 48 h repetiu-se a coleta destes exames. Resultados: Não houve episódios de broncoaspiração e fístulas gastrojejunais em ambos os grupos. Na análise do volume residual gástrico dos grupos SF e RL, as médias de volume foram respectivamente 16,5 e 8,8 apresentando significância estatística entre os grupos. Nos exames laboratoriais não houve diferença entre os grupos no sódio; nível sérico de potássio no SF foi maior (p=0,029); ureia e creatinina maiores no RL; CPK não apresentou diferenças; insulina no grupo SF foram maiores; pH foi maior no SF; bicarbonato de sódio evidenciou diferença significativa em todos o momentos; PaCO2 no RL foi maior. Na análise de incidência de náusea e flatos não foram observados significância estatística entre os grupos. Conclusões: A abreviação do jejum pré-operatório e a realimentação precoce no pós-operatório de bypass gastrojejunal em Y-de-Roux com a aplicação de programas como ERAS ou Projeto Acerto aceleram a recuperação do paciente, diminuindo o volume gástrico residual e o nível de insulina, e não predispõem complicações.

ABSTRACT Rational: The metabolic response to surgical trauma is enhanced by prolonged preoperative fasting, contributing to increased insulin resistance. This manifestation is more intense on the 1st and 2nd postoperative days and is directly proportional to the size of the operation. Aim: To compare whether preoperative fasting abbreviation and early postoperative refeeding associated with intraoperative and postoperative fluid restriction interfere in the evolution of patients undergoing gastrojejunal bypass. Methods: Eighty patients indicated for Roux-en-Y gastrojejunal bypass were selected. They were randomly divided into two groups: Ringer Lactate (RL) group, who underwent a 6 hours solids fasting, with the administration of 50 g of maltodextrin in 100 ml of mineral water 2 hours before the beginning of anesthesia; and Physiologic Solution (PS) group, who underwent a 12 hours solids and liquids fasting. Anesthesia was standardized for both groups. During the surgical procedure, 1500 ml of ringer lactate solution was administered in the RL and 2500 ml of physiological solution (0.9% sodium chloride) in the PS. In both groups, the occurrence of bronchoaspiration was analyzed during intubation, and the residual gastric volume was measured after opening the abdominal cavity. In the postoperative period in Group RL, patients started a liquid diet 24 hours after the end of the operative procedure; whilst for PS group, fasting was maintained for the first 24 hours, it was prescripted 2000 ml of physiological solution and a restricted liquid diet after 36 hours. Each patient underwent CPK, insulin, sodium, potassium, urea, creatinine, PaCO2, pH and bicarbonate dosage in the immediate postoperative period, and 48 hours later, the exams were repeated. Results: There were no episodes of bronchoaspiration and gastrojejunal fistulas in either group. In the analysis of the residual gastric volume of the PS and RL groups, the mean volumes were respectively 16.5 and 8.8, which shows statistical significance between the groups. In laboratory tests, there was no difference between groups in sodium; PS group showed a higher level of serum potassium (p=0.029); whilst RL group showed a higher urea and creatinine values; CPK values were even for both; PS group demonstrated a higher insulin level; pH was higher in PS group; sodium bicarbonate showed a significant difference at all times; PaCO2 values in RL group was higher than in PS. In the analysis of the incidence of nausea and flatus, no statistical significance was observed between the groups. Conclusions: The abbreviation of preoperative fasting and early postoperative refeeding of Roux-en-Y gastrojejunal bypass with the application of ERAS or ACERTO Project accelerated the patient’s recovery, reducing residual gastric volume and insulin level, and do not predispose to complications.

RESUMO Racional: O aumento da prevalência da obesidade levou ao aumento significativo da ocorrência de síndrome metabólica, fator de risco reconhecido para aumento da morbimortalidade por doenças cardiovasculares. A hiperglicemia ou diabetes mellitus do tipo 2, dislipidemia e hipertensão arterial são seus principais componentes. Desde 2015, diretrizes internacionais reconheceram os benefícios da cirurgia bariátrica em cada fator isolado desta síndrome. Objetivos: Avaliar o impacto do bypass gástrico em Y-de-Roux nesta síndrome comparando períodos pré e pós-operatório com análise laboratorial, e comparar a razão cintura/estatura e o IMC em relação a determinação do perfil de risco cardiometabólico. Métodos: Realizou-se um estudo retrospectivo com base prospectiva selecionando 80 pacientes submetidos à bypass gástrico em Y-de-Roux. Foram analisados o colesterol total, HDL, LDL, triglicerídeos, glicemia de jejum, hemoglobina glicada, insulina, índice de massa corpórea (IMC), vitamina D, vitamina B12, perímetro abdominal e relação cintura/estatura em três períodos: o pré-operatório de 1 a 6 meses, pós-operatório de 1 a 6 meses e pós-operatório de 1 a 2 anos. Resultados: Houve melhora em todos os parâmetros das análises clínicas. O IMC, no pré-operatório, teve a média dos valores de 39,8, no pré-operatório de 1 a 6 meses, os valores caíram para 33,2 e no pós-operatório de 1 ano média foi de 26. O perímetro abdominal teve média dos valores de 118,5, no pré-operatório, 105,2 no pós-operatório de 1 a 6 meses e 90,3 no pós-operatório de 1 a 2 anos. A relação cintura/estatura teve 0,73, 0,65 e 0,56 no pré, pós 1 a 6 meses e 1 a 2 anos respectivamente. Conclusão: O bypass gástrico em Y-de-Roux melhora a síndrome metabólica e a relação cintura/estatura é superior ao IMC na avaliação do perfil do risco cardiometabólico.

ABSTRACT Background: The increased prevalence of obesity has led to a significant increase in the occurrence of metabolic syndrome, a recognized risk factor for increased morbidity and mortality from cardiovascular diseases. Hyperglycemia or type 2 diabetes mellitus, dyslipidemia and arterial hypertension are its main components. Since 2015, international guidelines have recognized the benefits of bariatric surgery in each isolated factor of this syndrome. Aim: To evaluate the impact of Roux-en-Y gastric bypass in this syndrome comparing pre- and postoperative periods with laboratory analysis and to compare waist/height ratio and BMI in relation to the determination of the cardiometabolic risk profile. Methods: A retrospective study was carried out, selecting 80 patients undergoing Roux-en-Y gastric bypass. Total cholesterol, HDL, LDL, triglycerides, fasting glucose, glycated hemoglobin, insulin, body mass index (BMI), vitamin D, vitamin B12, waist circumference and waist/height ratio in three periods were analyzed: the preoperative period from 1 to 6 months, postoperative from 1 to 6 months and postoperative from 1 to 2 years. Results: There was an improvement in all parameters of the clinical analyses. The preoperative BMI had a mean value of 39.8, in the preoperative period from 1 to 6 months, the values dropped to 33.2 and in the postoperative period of 1 year, the mean was 26. The perimeter mean values of 118.5 preoperatively, 105.2 postoperatively from 1 to 6 months and 90.3 postoperatively from 1 to 2 years. Waist/height ratio was 0.73, 0.65 and 0.56 in pre, post 1 to 6 months and 1 to 2 years respectively. Conclusion: Roux-en-Y gastric bypass improves metabolic syndrome and waist-to-height ratio is superior to BMI in the assessment of the cardiometabolic risk profile.

RESUMO Racional: As derivações intestinais revolucionaram o tratamento da obesidade mórbida pela sua viabilidade e resposta sustentada. Porém, estudos experimentais sugerem, após estas derivações, risco maior de câncer de cólon. Objetivo: Analisar as alterações histológicas e imunoistológicas que a derivação jejunojejunal possa produzir no jejuno, íleo e cólon ascendente. Método: Foram utilizados 24 ratos Wistar machos randomicamente divididos em dois grupos, controle (n=12) e experimento (n=12) e subdivididos em grupos de quatro. Nove semanas após a derivação jejunojejunal procedeu-se a ressecção segmentar do jejuno excluso, íleo terminal e cólon ascendente. Análise histológica focou na espessura da mucosa, altura dos vilos, profundidade das criptas e a imunoistoquímica na expressão do Ki-67 e p53. Resultados: Foram encontradas diferenças significativas entre os grupos experimento e controle em relação à espessura da mucosa no jejuno (p=0,011), no íleo (p<0,001) e no cólon (p=0,027). Também houve diferença significativa em relação à altura dos vilos no íleo (p<0,001) e profundidade das criptas no jejuno (p<0,001). Os resultados indicaram que existe diferença significativa entre os grupos em relação à expressão do Ki-67 no cólon (p<0,001). Não foram encontradas diferenças significativas entre os grupos em relação à expressão do Ki-67 no jejuno e no íleo. Na avaliação do P53, foi encontrada coloração nuclear negativa em todos os casos. Conclusão: O desvio realizado na derivação gastrojejunal em Y-de-Roux, predispõem efeitos proliferativos epiteliais, causando aumento da espessura da mucosa, altura dos vilos e profundidade das criptas do jejuno, íleo e cólon ascendente.

ABSTRACT Background: Intestinal diversions have revolutionized the treatment of morbid obesity due to its viability and sustained response. However, experimental studies suggest, after these derivations, a higher risk of colon cancer. Aim: To analyze the histological and immunohistological changes that the jejunojejunal shunt can produce in the jejunum, ileum and ascending colon. Method: Twenty-four male Wistar rats were randomly divided into two groups, control (n=12) and experiment (n=12) and subdivided into groups of four. Nine weeks after the jejunojejunal shunt, segmental resection of the excluded jejunum, terminal ileum and ascending colon was performed. Histological analysis focused on the thickness of the mucosa, height of the villi, depth of the crypts and immunohistochemistry in the expression of Ki-67 and p53. Results: Significant differences were found between the experiment and control groups in relation to the thickness of the mucosa in the jejunum (p=0.011), in the ileum (p<0.001) and in the colon (p=0.027). There was also a significant difference in relation to the height of the villus in the ileum (p<0.001) and the depth of the crypts in the jejunum (p0.001). The results indicated that there is a significant difference between the groups regarding the expression of Ki-67 in the colon (p<0.001). No significant differences were found between the groups regarding the expression of Ki-67 in the jejunum and ileum. In the P53 evaluation, negative nuclear staining was found in all cases. Conclusion: The jejunojejunal deviation performed in the Roux-in-Y gastrojejunal bypass, predispose epithelial proliferative effects, causing an increase in the thickness of the mucosa, height of the villi and depth of the crypts of the jejunum, ileum and ascending colon.

RESUMO Racional: A obesidade modifica a anatomia de seu portador. Além da mudança estética o alto percentual de gordura determina evidentes alterações funcionais. A normalidade antropométrica na mensuração do perímetro abdominal e estatura pode servir de base na mensuração dos riscos cardiometabólicos da obesidade. Objetivo: Verificar se é possível determinar parâmetros de normalidade entre cintura e estatura em pessoas que apresentem IMC e percentuais de gordura normais, para servir de base na avaliação de riscos para co-morbidades da obesidade. Métodos: Foi extraída amostra de 454 indivíduos com IMC e percentuais de gordura considerados dentro da normalidade. Ela foi dividida em faixas de idade tanto para homens como para mulheres entre 18 a 25; 26 a 35; 36 a 45; 46 a 55; 56 a 65. Totalizou 249 homens e 205 mulheres. Resultados: Em relação ao percentual da estatura como medida do perímetro abdominal, a amostra total feminina apresentou média de 44,2±1,1% e a masculina 45,3%+1,5. Para as mulheres este percentual determinou a equação da relação cintura-estatura representado por X=(idade+217)/5,875, e para homens X= (idade+190,89) /5,2222. “X” representa o percentual da medida da estatura para que o indivíduo se enquadre na categoria de adequados percentual de gordura e IMC. Conclusão: Entre a estatura de homens e mulheres adultos possuidores de percentuais de gordura e IMC normais, existe relação numérica comum, sendo em média 44% para mulheres e 45% para homens.

ABSTRACT Background: Obesity changes the anatomy of the patient. In addition to the aesthetic change, the high percentage of fat determines evident functional changes. Anthropometric normality in measuring abdominal circumference and height can serve as a basis for measuring cardiometabolic risks of obesity. Aim: To verify if it is possible to determine parameters of normality between waist and height in people with normal BMI and fat percentages, to serve as a basis for assessing risks for obesity comorbidities. Methods: A sample of 454 individuals with BMI and percentages of fat considered within the normal range was extracted. It was divided into age groups for both men and women between 18 and 25; 26 to 35; 36 to 45; 46 to 55; 56 to 65. A total of 249 men and 205 women were included. Results: Regarding the percentage of height as a measure of the abdominal perimeter, the total female sample had an average of 44.2±1.1% and the male 45.3%+1.5. For women, this percentage determined the equation of the waist-height ratio represented by X=(age+217) / 5.875, and for men X=(age+190.89) / 5.2222. “X” represents the percentage of the height measurement so that the individual falls into the category of adequate percentage of fat and BMI. Conclusion: Between the stature of adult men and women with normal fat percentage and BMI, there is a common numerical relationship, with is on average 44% for women and 45% for men.

RESUMO Objetivo: avaliar a eficácia de três marcadores imunoistoquímicos envolvidos no processo de cicatrização de ferida cirúrgica. Métodos: estudo experimental em 40 ratos da raça Wistar, dos marcadores metaloproteinases e metaloproteinase da matriz 9 (MMP-9), fator de transformação do crescimento beta (TGF-β) e miofibroblasto e alfa actina de músculo liso (α-AML), estudados a partir de fragmentos de cicatriz cirúrgica de incisão abdominal envolvendo pele, aponeurose e peritônio. Os animais foram distribuídos em quatro subgrupos de dez de acordo com o dia da morte, programada em três, sete, 14 e 21 dias. Resultados: na expressão da MMP-9 ocorreu aumento progressivo de sua concentração, mais evidente do 7º ao 14º dias variando a imuno-expressão tecidual entre 2,65% e 11,50%.TGF- β mostrou expressão em nível alto no 3º dia, caiu no 7º, voltando a subir no 14º, com pequena queda no 21º dia variando a imuno-expressão tecidual entre 0,03% e 2,92%. A α-AML apresentou níveis com pouca variação e discreto aumento variando a imuno-expressão tecidual entre 0,88% e 3,23%. Conclusão: a MMP-9 se apresentou como melhor marcador, seguido pela TGF-β. Já o α-AML não se mostrou um bom sinalizador da evolução da reparação tissular.

ABSTRACT Objective: to evaluate the efficacy of three immunohistochemical markers involved in the wound healing process. Methods: experimental study of 40 Wistar rats of the markers metalloproteinases and matrix metalloproteinase 9 (MMP-9), beta transforming growth factor (TGF-β) and myofibroblasts and smooth muscle actin alpha (α-MLA) markers, studied from fragments of surgical scar of abdominal incision involving skin, aponeurosis and peritoneum. The animals were divided into four subgroups of ten according to the day of death, scheduled in three, seven, 14 and 21 days. Results: MMP-9 expression showed a progressive increase of its concentration, more evident from 7th to 14th days, varying the tissue immunoexpression between 2.65% and 11.50% . TGF- β showed expression at high level on the 3rd day, fell in the 7th, rising again in the 14th, with a small decrease in the 21st day, varying the tissue immunoexpression between 0.03% and 2.92%. The α-AML presented levels with little variation and a slight increase, varying the tissue immunoexpression between 0.88% and 3.23%. Conclusion: MMP-9 presented as the best marker, followed by TGF-β. However, α-AML was not a good indicator of the evolution of tissue repair.

RESUMO Racional: Os estudos relacionados à obesidade têm evidenciado sua associação com a síndrome metabólica. A descoberta que a obesidade é capaz de promover inflamação, sem os sinais clássicos, tem levado vários grupos de pesquisa a caracterizar os tipos celulares que agem e o mecanismo envolvido neste processo. Objetivo: Avaliar a variação da hemoglobina glicada e a secreção da citocina inflamatória, interleucina-6, em indivíduos diabéticos com IMC<35 kg/m² no pré e pós-operatório da técnica de desvio duodenal parcial. Métodos: Nove pacientes foram avaliados antes e um ano após a operação e a variação da concentração da interleucina-6 foi avaliada pela metodologia de Elisa. Também foi avaliada a variação da HbA1c. Resultados: A quantificação de interleucina-6 apresentou no pré-operatório valor de 65,50436±2,911993 pg/ml e de 39,47739+3,410057 pg/ml após um ano da operação e a hemoglobina glicada apresentou média de 10,67 no pré-operatório e de 5,8 após um ano da operação. Conclusão: O desvio duodenal parcial foi capaz de, um ano após o procedimento, diminuir os efeitos da inflamação crônica demonstrada pela diminuição da concentração da interleucina-6 plasmática e normalizar a hemoglobina glicada em pacientes diabéticos com IMC<35 kg/m2.

ABSTRACT Background: Studies related to obesity have shown association with metabolic syndrome. Data showing that obesity is capable to cause low grade chronic inflammation, without its classic signs and symptoms, call attention to researches to study different cells types and the mechanism of the inflammatory process. Aim: To evaluate the variation of glycated hemoglobin (HbA1c) and the pro-inflammatory cytokine interleukin-6 (IL6) in diabetic patients with BMI <35 kg/m2 in the pre and postoperative of partial duodenal switch. Method: Nine patients were studied before and one year after the operation and the variation of the serum IL6 was measured by Elisa. The changes of HbA1c were also registered. Results: The pre-operative IL6 levels reached 65,50436±2,911993 pg/ml and one year after de operation 39,47739±3,410057 and the HbA1c average of 10,67 and 5.8 in the same period. Conclusion: The partial duodenal switch was efficient to control one year after the procedure the chronic inflammatory process caused by the diabetes mellitus type 2 with BMI <35 by dropping the IL6 levels and bringing the HbA1c to normal.

Introdução: Concomitantemente com a pandemia da obesidade, observa-se fenômeno de envelhecimento global, com aumento significativo na população de idosos obesos. Objetivo: Revisar as indicações da cirurgia bariátrica para os idosos, focando principalmente a morbimortalidade dos procedimentos. Método : Revisão da literatura nas bases PubMed/Medline e Scielo com foco na relação dos fatores de risco com diversas técnicas de cirurgia bariátrica em idosos. Foram cruzados os seguintes descritores na forma de AND/OR: Obesidade; Cirurgia bariátrica; Complicações; Idosos. Conclusão: Nas pessoas com idade superior a 60 anos os procedimentos bariátricos representam opção aceitável e efetiva de tratamento. Os idosos devem ser tratados em centros especializados, de grande movimento cirúrgico e baixos índices de morbidade e mortalidade13. Com isso, eles experimentam os benefícios da cirurgia bariátrica com aceitável morbimortalidade. Contudo, a idade por si só não deve funcionar como um impeditivo absoluto na indicação cirúrgica.

Introduction : Concurrently with the pandemic obesity is observed global aging phenomenon, with a significant increase of obesity in the elderly population. Aim : To review the indications for bariatric surgery for the elderly, mainly focusing on the morbidity and mortality of procedures. Method : Review of the literature in PubMed/Medline and Scielo focusing on the relationship of risk factors with different techniques of bariatric surgery in the elderly. The following descriptors were crossed in the form of AND/OR: Obesity; Bariatric surgery; Complications; Elderly. Conclusion : In people older than 60 years bariatric procedures represent acceptable and effective treatment option. The elderly should be treated in specialized centers with experience in major surgical procedures and low morbimortality. Going in this way, they experience the benefits of bariatric surgery with acceptable morbidity and mortality. However, age alone should not be considered as an absolute impediment for surgical indication.

Objetivo:verificar a prevalência da STC em pacientes obesos candidatos à cirurgia bariátrica comparada com a prevalência em indivíduos não obesos e em pacientes já submetidos ao procedimento cirúrgico para verificar se as medidas de perda de peso influem na prevalência e gravidade dos sintomas.Métodos: três grupos de indivíduos foram estudados: 1) candidatos à cirurgia bariátrica (pré-operatório); 2) já submetidos ao tratamento cirúrgico bariátrico (pós-operatório) e 3) grupo controle. Foram coletados dados demográficos e clínicos referentes à síndrome do túnel do carpo. Foi realizada ultrassonografia para medição da área da secção transversa do nervo mediano para o diagnóstico da síndrome. Resultados:foram incluídos 329 indivíduos (114 no grupo pré-operatório, 90 no grupo pós-operatório e 125 controles). Houve maior prevalência de parestesias quando se comparou o grupo pré-operatório com o controle (p<0,00001). Houve diminuição das parestesias (p=0,0002) e da área da secção transversa do nervo mediano (p=0.04) nos pacientes do pós-operatório, mas não houve diferença significativa na prevalência geral da síndrome do túnel do carpo. Foi observada diferença significativa entre os grupos pré e pós-operatório (p=0,05) nos indivíduos que realizavam trabalho não manual. Conclusão: houve maior prevalência da síndrome do túnel do carpo entre o grupo pré-operatório comparado com o controle, mas não se observou diferença significativa entre os grupos pré e pós-operatório no geral. Houve diferença entre os grupos pré e pós-operatório dentre os trabalhadores não manuais.

Objective:To evaluate the prevalence of carpal tunnel syndrome in candidates for bariatric surgery comparing with the non-obese population and verify the effects on it of bariatric treatment. Methods:We studiedthree groups of individuals: 1) patients waiting for bariatric surgery (preoperative); 2) individuals who had already undergone the procedure (postoperative); and 3) control group. We collected demographic and clinical data of carpal tunnel syndrome. The Ultrasound examination was carried out to diagnose the syndrome by measuring the median nerve area. Results:We included 329 individuals (114 in the preoperative group, 90 in the postoperative group and 125 controls). There was a higher prevalence of paresthesias (p=0.0003), clinical tests (p=0.0083) on the preoperative group when compared with controls (p<0.00001). There were lowe levels of paresthesias (p=0.0002) and median nerve area (p=0.04) in postoperative patients but with no significant difference in general. A significant difference was found between the preoperative and postoperative groups (p=0.05) in those who performed non-manual work.Conclusion: There was a higher prevalence of carpal tunnel syndrome in the preoperative group compared with the control one, but no significant difference was observed between the pre and postoperative groups in general. There was difference between pre and postoperative groups for non-manual workers.

INTRODUÇÃO: A associação entre obesidade e doença do refluxo gastroesofágico apresenta alta incidência e pode estar presente em metade dos obesos com indicação cirúrgica. Operações bariátricas podem também induzir refluxo por si só - de modo diferente do fator IMC -, e seus mecanismos são dependentes do tipo do procedimento realizado. OBJETIVO: Efetuar revisão bibliográfica comparando os dois procedimentos atualmente mais utilizados para tratamento cirúrgico da obesidade e analisar a relação deles com o advento de doença do refluxo gastroesofágico pré-existente ou de surgimento somente no pós-operatório. MÉTODO: Foi realizada revisão bibliográfica nas bases virtuais Medline/Pubmed, Scielo, Lilacs, Embase e Cochrane cruzando os seguintes descritores MeSH: gastric bypass AND/OR anastomosis, Roux-en-Y AND/OR gastroesophageal reflux AND/OR gastroenterostomy AND/OR gastrectomy AND/OR obesity AND/OR bariatric surgery AND/OR postoperative period. Foram consideradas pertinentes 135 referências e utilizadas 30 neste artigo. Também foi adicionada a experiência dos autores deste artigo no manuseio dessas técnicas com esse mister. CONCLUSÃO: As alterações estruturais causadas pela técnica operatória na gastrectomia vertical apresenta maior comprometimento dos mecanismos anti-refluxo predispondo a indução da DRGE no pós-operatório quando comparado à técnica operatória realizada no Bypass gastrointestinal em Y-de-Roux.

BACKGROUND: The association between obesity and gastroesophageal reflux disease has a high incidence and may be present in half of obese patients with surgical indication. Bariatric operations can also induce reflux alone - differently from BMI factors - and its mechanisms are dependent on the type of procedure performed. OBJECTIVE: To perform a literature review comparing the two procedures currently most used for surgical treatment of obesity and analyze their relationship with the advent of pre-existing reflux disease or its appearance only in postoperative period. METHOD: The literature was reviewed in virtual database Medline/PubMed, SciELO, Lilacs, Embase and Cochrane crossing the following MeSH descriptors: gastric bypass AND / OR anastomosis, Roux-en-Y AND / OR gastroesophageal reflux AND / OR gastroenterostomy AND / OR gastrectomy AND / OR obesity AND / OR bariatric surgery AND / OR postoperative period. A total of 135 relevant references were considered but only 30 were used in this article. Also was added the experience of the authors of this article in handling these techniques on this field. CONCLUSION: The structural changes caused by surgical technique in vertical gastrectomy shows greater commitment of antireflux mechanisms predisposing the induction of GERD postoperatively compared to the surgical technique performed in the gastrointestinal Bypass Roux-en-Y.

RACIONAL: Embora seja muito complexo identificar o que levou ao grande crescimento da obesidade, as causas principais são o aumento do consumo de alimentos energéticos e ricos em gorduras saturadas e açucares, e a redução das atividades físicas. OBJETIVOS: Comparar a massa corpórea, o peso da gordura retroperitoneal e gonadal e o índice de Lee em ratos submetidos à dieta normal e à suplementada com sacarose. MÉTODOS: A amostra foi de 40 animais, divididos em dois grupos: 20 do grupo controle que receberam dieta de ração normal e água por três meses e 20 do grupo experimento que receberam a mesma ração suplementada com sacarose 300 g/l na água. Os animais foram pesados uma vez por semana durante 91 dias. Na data da morte foram aferidos o comprimento nasoanal, peso corporal e foi calculado o índice de Lee. Após, foram submetidos à laparotomia e a gordura retroperitoneal e gonadal foram individualizadas, ressecadas e avaliada a porcentagem do peso dela em relação ao corporal na data da morte. RESULTADOS: Verificou-se diferença estatisticamente significativa entre o 14º até o 78º dia, quando comparados os grupos indicando que a sacarose interfere no ganho de peso dos ratos. A média de peso foi maior no grupo experimento em todos os períodos, tendo como referência o peso inicial. Verificou-se diferença significativa a favor do grupo experimento no peso da gordura gonadal e retroperitoneal. Não houve diferença significativa da comparação do índice de Lee. CONCLUSÃO: A massa corpórea foi maior nos animais submetidos à dieta suplementada, com maior gordura retroperitoneal e gonadal e sem diferença no índice de Lee.

BACKGROUND: Although is complex to identify the factors responsible for the important growth in obesity all over the world, the main causes are increased consumption of energy, highly saturated fats and sugars, and reduced physical activity. AIM: To compare rats with normal and supplemented diet with sucrose in relationship to body mass, weight of gonadal and retroperitoneal fat and Lee index. METHODS: Forty rats were divided into two groups: 20 in the control group that received normal chow diet and water for three months, and 20 animals in the experimental group who received the same diet but supplemented with sucrose 300 g/l of water. The animals were weighed once a week during 91 days. At scheduled death, they had measured the naso-anal length, body weight and Lee index. After laparotomy, retroperitoneal and gonadal fat were isolated, dried and the percentage of weight in relation to body weight at the date of death was evaluated. RESULTS: There was a statistic significant difference between the 14th and 78th day favoring the experiment group indicating that sucrose interferes with weight gain in rats. The average weight was higher in the experimental group in all periods in comparison to initial weight. There was also significant difference in the weight of the gonadal and retroperitoneal fat. There was no significant difference comparing the Lee index. CONCLUSION: The body mass index was higher in animals treated with diet supplemented with sucrose and had higher gonadal and retroperitoneal fat, but no difference in the Lee index.
