Devido à importância do estudo das causas de dificuldade de aprendizado, delineou-se estudo caso controle, para avaliar o volume das cavidades nasais, tonsilas faríngeas e tonsilas palatinas em crianças com e sem dificuldade de aprendizado. MÉTODO: Foram estudadas 48 crianças, 24 oriundas do Centro de Avaliação e Estimulação Precoce (CADEP), no qual o critério é a repetência escolar de no mínimo dois anos consecutivos, e 24 escolares com aprendizado dentro dos padrões de normalidade, que constituíram o grupo controle. As crianças foram submetidas a exame otorrinolaringológico (anamnese, exame físico) e exames específicos (rinometria acústica, Rx de cavum). RESULTADOS: Os resultados mostraram que os escolares com deficiência de aprendizado possuem prevalência maior de hipertrofia de tonsila faríngea, p < 0,001, e palatina, p < 0,001. A média do volume das cavidades nasais não mostrou associação estatisticamente significativa com dificuldade de aprendizado (p = 0,75). CONCLUSÃO: Com base neste estudo, conclui-se que crianças com hipertrofia adenotonsilar possuem mais dificuldade no aprendizado quando comparadas com crianças sem hipertrofia. Given the importance of studying the causes of learning disorders, we designed this case-control study to assess the nasal cavity volume, pharyngeal and palatine tonsils in children with and without learning disabilities. METHOD: A total of forty-eight children were enrolled in the study: twenty-four coming from the Center for Evaluation and Early Stimulation (CADEP), in which the criterion is the school failure of at least two consecutive years; and twenty-four students with normal learning - which made up the control group. The children were submitted to ENT examination (history, physical examination) and specific tests (acoustic rhinometry, cavum radiography). RESULTS: The results showed that students with learning disabilities have a higher prevalence of pharyngeal tonsil hypertrophy: p < 0.001, and palatine tonsil hypertrophy: p < 0.001. The average volume of the nasal cavities showed no statistically significant association with learning difficulties (p = 0.75). CONCLUSION: Based on this study, we concluded that children with adenotonsillar hypertrophy have more learning difficulties when compared to children without such hypertrophy.

OBJECTIVE: To evaluate laryngeal sequelae from surgical treatment of recurrent respiratory papillomatosis in children, as well as associated risk factors. METHODS: Case-control study. Medical record data analysis of 50 children with recurrent respiratory papillomatosis, divided into two groups: with and without laryngeal sequelae. The group of patients with laryngeal sequelae was compared to those without sequelae in regard to the onset of disease, age at first surgery, number and frequency of surgeries, disease stage, and type of surgery (CO2 laser, cold forceps). RESULTS: 23 patients (46%) sustained laryngeal sequelae. The most frequent sequela was anterior commissure synechia (17 patients [34%]), followed by glottic stenosis (six patients [12%]). There was no statistically significant difference between groups with and without laryngeal sequelae regarding the disease onset (p = 0.93), age at first surgery (p = 0.68), number of surgeries (p = 0.22), annual frequency of surgery (p = 0.93), presence of papilloma in anterior (p = 0.430) or posterior commissure (p = 0.39), and type of surgery (p = 0.27). The Derkay anatomical score (a staging system that assesses the extent of the disease in the aerodigestive tract) was significantly higher in the laryngeal sequelae group (p = 0.04). CONCLUSION: Laryngeal sequelae are a frequent complication of recurrent respiratory papillomatosis surgical treatment in children, particularly anterior commissure synechiae and glottic stenosis. Advanced stages are associated with increased risk of laryngeal sequelae after surgery.

A hipertrofia obstrutiva das tonsilas palatinas e faríngeas está associada à respiração oral e pode levar a desequilíbrios faciais. A adenotonsilectomia parece ser insuficiente para o tratamento quando ocorreram alterações anatômicas. Técnicas ortopédicas faciais auxiliam no restabelecimento morfofuncional. Estudo clínico longitudinal prospectivo objetivou observar alterações craniofaciais após adenotonsilectomia e verificar a importância de associar ortopedia ao tratamento. MATERIAL E MÉTODO: Cinquenta e três crianças de ambos os gêneros, entre 6 e 12 anos, divididas em: Grupo 1, 20 crianças com respiração nasal; Grupo 2, 33 crianças com hipertrofia obstrutiva das tonsilas faríngeas e/ou palatinas, submetidas à adenotonsilectomia. Após a cirurgia, este grupo foi subdividido: Grupo 2A, 16 pacientes não tratados com expansão rápida da maxila; Grupo 2B, 17 pacientes tratados com disjunção maxilar. Foram realizadas medidas cefalométricas em norma frontal e lateral anteriores à cirurgia e após 14 meses. Foram utilizados os testes Kruskal-Wallis e Wilcoxon, com nível de significância de 5%. RESULTADOS: A adenotonsilectomia equilibrou o crescimento transversal, sagital e vertical em ambos os grupos, sendo mais efetiva no grupo submetido ao tratamento combinado. CONCLUSÕES: A adenotonsilectomia favoreceu o crescimento facial das crianças com hipertrofia obstrutiva, sendo mais evidente quando associada à expansão maxilar.

Obstructive hypertrophy of the tonsils and/or adenoids is associated with mouth breathing and can lead to facial imbalances. Adenotonsillectomy is not enough to treat the anatomic changes. Facial orthopedic techniques aid in morphological and functional recovery. This prospective longitudinal clinical study aimed to observe craniofacial changes after adenotonsillectomy and to verify the importance of linking rapid maxillary expansion to treatment. METHOD: Fifty-three children of both genders, aged 6 to 12 years, were allocated to: Group 1, 20 children with nasal breathing; and group 2, 33 children with obstructive hypertrophy of pharyngeal and/or palate undergoing adenotonsillectomy. After surgery, this group was subdivided into Group 2A, 16 patients not treated with rapid maxillary expansion; and Group 2B, 17 patients treated with maxillary rapid expansion. Frontal and lateral cephalometric measurements were made prior to surgery and after 14 months. Statistical analysis used the Kruskal-Wallis and Wilcoxon tests - significance level of 5%. RESULTS: Adenotonsillectomy balanced transversal, sagittal and vertical growth in both groups, and was more effective in the group undergoing combined treatment. CONCLUSIONS: Adenotonsillectomy improved the facial growth of children with obstructive hypertrophy, which was more evident when associated with rapid maxillary expansion.

Crianças com tonsilas e adenoides hipertróficas podem apresentar efeitos desfavoráveis na oclusão dentária, que tendem a agravar no período de crescimento, tornando imprescindível seu diagnóstico e tratamento precoce. OBJETIVO: Este estudo clínico prospectivo comparou medidas cefalométricas dos incisivos antes e após a adenotonsilectomia, em respiradores orais. MATERIAL E MÉTODO: A amostra foi de 38 pacientes de ambos os gêneros, entre 7 e 11 anos, dividida em: grupo oral, com 18 pacientes com hipertrofia obstrutiva da tonsila faríngea e/ou palatinas grau 3 ou 4; grupo controle, com 20 pacientes com respiração nasal. Medidas dentárias angulares e lineares foram comparadas entre os grupos, antes e após adenotonsilectomia, em um intervalo de 14 meses. A análise estatística utilizou os testes t-Student e Wilcoxon para amostras não pareadas, ao nível de significância de 5%. RESULTADOS: A inclinação axial e a posição sagital dos incisivos inferiores aumentaram significativamente no grupo com respiração oral; o posicionamento sagital dos incisivos superiores aumentou significativamente no grupo oral, que ainda obteve aumento significativo de sobremordida. CONCLUSÃO: A adenoamigdalectomia se mostrou bastante eficaz na melhora de algumas medidas dentárias, com benefícios aos pacientes em crescimento, prevenindo que más oclusões dentárias tenham difícil tratamento ou se tornem definitivas.

Children with hypertrophic tonsils and adenoids may have adverse effects on dental occlusion, which tend to worsen during the growth period. Diagnosis and early treatment is essential. AIM: Prospective clinical study to compare the cephalometric measurements before and after adenotonsillectomy in mouth breathing patients. MATERIAL AND METHOD: We had 38 patients of both genders, aged between 7 and 11 years in our sample, broken down into: oral group, 18 patients with obstructive hypertrophy of pharyngeal tonsil and/or palate grade 3 or 4; control group, 20 patients with normal breathing. Angular and linear dental measurements were compared between the groups in a 14 months interval. We used the "t" Student and Wilcoxon tests for unpaired samples, at 5% significance, for statistical purposes. RESULTS: The sagittal position and axial angle of the lower incisors increased significantly in the group with oral breathing, the sagittal position of the upper incisors increased significantly in the oral group, which still had a significant increase in overbite. CONCLUSION: Adenotonsillectomy was very effective in improving some dental measurements, with benefits to growing patients preventing malocclusions from becoming difficult to treat or permanent.

Estudo de coorte contemporânea com corte transversal. Há evidências da influência da percepção auditiva sobre o desenvolvimento da linguagem oral e escrita e da autopercepção das condições vocais. A maturação do sistema auditivo pode interferir nesse processo. OBJETIVO: Caracterizar habilidades auditivas de Localização e de Ordenação Temporal em crianças disfônicas. MATERIAL E MÉTODO: Avaliaram-se 42 crianças (4 a 8 anos). Grupo Pesquisa: 31 crianças disfônicas, Grupo de Comparação: 11 crianças sem queixas de alterações vocais. Todas apresentaram limiares auditivos normais e reflexo cócleo-palpebral. Foram submetidas à Avaliação Simplificada do Processamento Auditivo (Pereira, 1993). Para comparar os grupos utilizaram-se os testes estatísticos de Mann-Whitney e Kruskal-Wallis. Nível de significância: 0,05 (5%). RESULTADOS: À Avaliação Simplificada, 100% do Grupo Controle e 61,29% do Grupo Pesquisa apresentaram resultados normais. Nas provas de Localização e Memória Sequencial Verbal, os grupos mostraram-se semelhantes. A Memória Sequencial Não Verbal mostrou piores resultados nas crianças disfônicas. Nesse grupo, o desempenho foi pior dos quatro aos seis anos. CONCLUSÃO: As crianças disfônicas apresentaram alterações das habilidades de localização ou ordenação temporal, a habilidade de ordenação temporal de sons não verbais diferenciou o grupo disfônico. Nesse grupo, a Localização Sonora melhorou com a idade.

Contemporary cross-sectional cohort study. There is evidence of the auditory perception influence on the development of oral and written language, as well as on the self-perception of vocal conditions. The auditory system maturation can impact on this process. OBJECTIVE: To characterize the auditory skills of temporal ordering and localization in dysphonic children. MATERIALS AND METHODS: We assessed 42 children (4 to 8 years). Study group: 31 dysphonic children; Comparison group: 11 children without vocal change complaints. They all had normal auditory thresholds and also normal cochleo-eyelid reflexes. They were submitted to a Simplified assessment of the auditory process (Pereira, 1993). In order to compare the groups, we used the Mann-Whitney and Kruskal-Wallis statistical tests. Level of significance: 0.05 (5%). RESULTS: Upon simplified assessment, 100% of the Control Group and 61.29% of the Study Group had normal results. The groups were similar in the localization and verbal sequential memory tests. The nonverbal sequential memory showed worse results on dysphonic children. In this group, the performance was worse among the four to six years. CONCLUSION: The dysphonic children showed changes on the localization or temporal ordering skills, the skill of non-verbal temporal ordering differentiated the dysphonic group. In this group, the Sound Location improved with age.

A respiração oral pode causar deformações na arcada dentária e representar risco a cáries e doenças periodontais, podendo ser agravado pela utilização de aparelhos fixos. OBJETIVO: Avaliar o grau de mineralização do esmalte dentário e a microbiota cariogênica bucal de respiradores orais que utilizaram disjuntores maxilares. MATERIAL E MÉTODO: Estudo prospectivo com 20 pacientes respiradores orais com atresia maxilar, idades entre 9 e 13 anos. A mineralização do esmalte dentário foi medida pela técnica de fluorescência, antes da instalação do disjuntor maxilar e após sua remoção. A microbiota cariogênica foi avaliada pelo No Caries®. Na análise estatística utilizamos o teste "t" (p<0,05), e a microbiota oral analisada por incidência. RESULTADOS: Houve diferença estatisticamente significante no grau de mineralização do esmalte dentário após a disjunção maxilar, com valor médio de 3,08. O teste colorimétrico demonstrou que 45% diminuiu e 15% aumentou o potencial à cárie dentária, sendo que 40% permaneceu inalterado após o uso do disjuntor maxilar. CONCLUSÃO: Houve diferença estatisticamente significante no grau de mineralização do esmalte dentário nos pacientes respiradores orais após a utilização de disjuntor, porém dentro da faixa de normalidade clínica, e um número pequeno de pacientes aumentou o potencial cariogênico durante o tratamento ortodôntico.

Mouth breathing may cause deformities on the dental arch and be a risk factor for caries and periodontal disease; fixed orthodontic appliances compound the problem. AIM: to evaluate mineralization of tooth enamel and the oral cariogenic microbiota of mouth breathers that are using maxillary expanders. MATERIAL AND METHOD: a prospective study of 20 mouth-breathing patients with maxillary atresia, aged from 09 to 13 years. Enamel mineralization was measured using a fluorescence technique, before installing the expander and after its removal. The cariogenic microbiota was evaluated by the No Caries®. The t test (p<0.05) was applied for the statistical analysis, and the oral microbiota was analyzed by incidence. RESULTS: there was a statistically significant difference in the enamel mineralization level after maxillary expansion; the mean value was 3.08. The colorimetric test showed that the caries development potential was reduced in 45%, increased in 15%, and unaltered in 40% after maxillary expander use. CONCLUSION: there was a statistically significant difference in enamel mineralization after maxillary expansion; this difference was within the clinically normal range; the cariogenic potential increased in a small number of patients during orthodontic treatment.

OBJETIVO: Avaliar a eficácia e a segurança do levamisol no tratamento profilático da afta recorrente, utilizando um protocolo de estudo duplo-cego. MÉTODOS: Quatorze pacientes receberam doses decrescentes de levamisol por via oral por seis meses (dose inicial de 150mg três vezes por semana). Dez pacientes receberam placebo. As avaliações foram mensais. RESULTADOS: Houve tendência à diminuição do número de crises nos dois grupos, mas sem diferenças entre ambos. O número de lesões diminuiu significantemente nos grupos levamisol e placebo, mas na comparação entre eles a diferença não foi significante. A duração das lesões diminuiu significantemente no grupo placebo, porém ao compará-lo com o grupo levamisol a diferença não foi significante durante todo o tratamento. A intensidade da dor foi significantemente menor nos dois grupos, mas ao compará-los a dor foi significantemente menor no grupo placebo. A avaliação global final mostrou melhora em 50% dos pacientes do grupo levamisol e em 70% do Placebo, sem diferença significante entre os dois tratamentos. Não foi observada diferença na frequência de efeitos colaterais entre os grupos. CONCLUSÃO: Levamisol, como usado nesse protocolo, é uma droga segura. Comparado ao placebo, levamisol não é efetivo no tratamento profilático da afta recorrente. O efeito placebo é importante em desordens nas quais fatores emocionais afetam a recorrência ou a expressão de sintomas.

OBJECTIVE: to utilize a double-blind protocol to provide clarification about the safety and effectiveness of levamisole in the treatment of recurrent aphthous stomatitis. METHODS: Fourteen patients took a decreasing dose of oral levamisole for six months (initial dose 150mg three times a week) and ten others were placebo control patients. All were evaluated monthly. RESULTS: The number of crises had a tendency to decrease in both groups, but without a difference between groups. The number of lesions diminished significantly in the two groups, but upon comparison the difference was not significant. Duration of the lesions diminished significantly in the placebo, however when compared to the levamisole group, difference was not significant during treatment. The intensity of pain was significantly lower in the two groups, but upon comparison, pain was significantly lower in the placebo group. The final global evaluation showed improvement in 50% of patients of the levamisole group and in 70% of the placebo, without a significant difference between treatments. No difference in the frequency of collateral effects was observed between groups. CONCLUSIONS: Levamisole, as used in this protocol, is a safe drug. When compared with the placebo, levamisole is not effective in the prophylactic treatment of recurrent aphthous stomatitis. The placebo effect is important in diseases where emotional factors affect recurrence or expression of symptoms.

A criação de um banco de dados sobre as demandas otorrinolaringológicas no SUS poderá fornecer informações para organizar estratégias de saúde e criar ou otimizar recursos do governo ou privados. OBJETIVOS: Determinar em pacientes até 17 anos de idade: 1) As principais doenças otorrinolaringológicas diagnosticadas; 2) Exames complementares mais solicitados; 3) Medicamentos mais prescritos; 4) Procedimentos ambulatoriais realizados e as indicações cirúrgicas; 5) Estado empregatício dos pais e o número de irmãos dependentes; 6) Comparar e analisar estatisticamente os dados nos locais estudados. MATERIAL E MÉTODO: Estudo prospectivo e análise estatística das variáveis obtidas nas primeiras-consultas na Policlínica de Mariana, na sede Consórcio Intermunicipal de Saúde da Microrregião de Sete Lagoas e na Triagem da Otorrinolaringologia da UNIFESP-EPM. RESULTADOS: Há diferenças estatisticamente significativas quanto às variáveis idade, média salarial dos pais empregados, número de irmãos até 17 anos, rinite, doenças otológicas, exames solicitados, prescrições e indicações de cirurgias micro-otológicas. CONCLUSÕES: O diagnóstico mais encontrado foi respiração oral. O exame complementar e a cirurgia mais indicados foram a radiografia de cavum e a adenoamigdalectomia. O nível de desemprego, a baixa média salarial e o número de dependentes na família dificultam tratamentos que não estejam disponíveis na rede pública de saúde.

The data base of ENT care in the Brazilian public health system (Sistema Unico de Saude - SUS) will help organize public health programs. AIM: The following items were investigated in patients aged up to 17 years attended in public health system outpatient units in the city of Mariana, in the ENT screening unit, UNIFESP-EPM, and in CISMISEL: 1) The main otorhinolaryngological diagnoses; 2) The most frequently required exams, drugs, and surgical procedures and their indications; 3) The jobs of parents; the number of siblings; and 4) A statistical analysis and comparison of data in each location. MATERIAL AND METHODS: We undertook a prospective study and a statistical analysis of variables that were gathered during the first visit. RESULTS: The age, the parents' salary, the number of siblings aged below 18 years, the presence of rhinitis, ears diseases, the exams, drugs and otological surgeries that were indicated were all statistically significant. CONCLUSION: The most common diagnosis was mouth breathing. The most common surgery was adenotonsillectomy. The most frequently requested exam was a lateral cranial radiograph. The number of unemployed parents, their poor salaries, and the number of siblings make if difficult for these patients to be treated in any facility other than the public heath system.

OBJETIVO: Avaliar os seios paranasais em pacientes com asma estável ou asma aguda para determinar a prevalência de rinossinusite bacteriana aguda. MÉTODOS: Estudo transversal incluindo 30 pacientes com asma aguda (73% do sexo feminino) tratados na sala de emergência e 30 pacientes com asma estável (80% do sexo feminino) regularmente acompanhados em ambulatório. Todos os pacientes responderam a um questionário sobre sinais e sintomas respiratórios e foram submetidos a exame otorrinolaringológico e a radiograma e tomografia computadorizada de seios paranasais. RESULTADOS: Com base no diagnóstico clínico, a prevalência de rinossinusite bacteriana aguda foi de 40% nos pacientes com asma aguda e de 3% nos com asma estável. O exame otorrinolaringológico e os exames de imagem isoladamente não foram úteis para a confirmação diagnóstica. CONCLUSÕES: O exame otorrinolaringológico e o radiograma e a tomografia de seios paranasais por si só não foram úteis para o diagnóstico de rinossinusite bacteriana aguda. Nossos resultados confirmam a evidência de que o diagnóstico clínico de rinossinusite aguda deve ser dado com cautela.

OBJECTIVE: To evaluate paranasal sinuses in patients with stable or acute asthma in order to determine the prevalence of acute bacterial rhinosinusitis. METHODS: A cross-sectional study including 30 patients with acute asthma (73% females) treated in the emergency room and 30 patients with stable asthma (80% females) regularly monitored as outpatients. All patients completed a questionnaire on respiratory signs and symptoms and were submitted to ear, nose and throat (ENT) examination, as well as to X-ray and computed tomography (CT) imaging of the sinuses. RESULTS: Based on the clinical diagnosis, the prevalence of acute bacterial rhinosinusitis was 40% in the patients with acute asthma and 3% in those with stable asthma. The ENT examination findings and the imaging findings in isolation were not useful to confirm the diagnosis. CONCLUSIONS: In themselves, ENT examination findings, X-ray findings and CT findings were not useful for the diagnosis of acute bacterial rhinosinusitis. Our results provide further evidence that a clinical diagnosis of bacterial rhinosinusitis should be made with caution.

A cirurgia dos tumores selares é tradicionalmente um campo de atuação dos neurocirurgiões. O uso do endoscópio permitiu acesso transnasal direto ao seio esfenoidal sem a necessidade de descolamento do septo nasal, com menor desconforto e morbidade pós-operatória inferior aos métodos tradicionais. OBJETIVO: Verificar as dificuldades técnicas, intercorrências e complicações pós-operatórias, no manejo otorrinolaringológico do acesso endoscópico transnasal à sela túrcica. MATERIAL E MÉTODO: Foram analisados retrospectivamente os prontuários dos pacientes submetidos à cirurgia da região selar, entre março de 2001 e dezembro de 2005. Foram incluídos 91 pacientes submetidos a um total de 95 procedimentos por via transnasal endoscópica. Desenho científico: Clínico retrospectivo. RESULTADOS: Foi possível a realização da técnica endoscópica transnasal em todos os pacientes estudados. Não houve necessidade de remoção da concha média ou de desvios septais em nenhum dos casos. A principal intercorrência foi fístula liquórica durante a remoção de tumores (13,68%). As complicações pós-operatórias foram: sangramento nasal (8,42%), fístula liquórica (8,42%), e meningite (2,11%). CONCLUSÃO: O acesso endoscópico transnasal aos tumores selares pôde ser realizado de forma minimamente invasiva, preservando-se as estruturas nasais nos 95 procedimentos estudados, independente da idade do paciente, características e etiologia do tumor.

Transsphenoidal surgery for sellar region tumors is traditionally done only by neurosurgeons. The use of endoscopes has permitted a direct transnasal approach to the sphenoidal sinus, without dissection of the septal mucosa, reducing postoperative morbidity. AIM: The purpose of this study was to assess the technical difficulties, and per and postoperative complications of the otolaryngological management of the endoscopic transnasal approach to the sellar region. MATERIAL AND METHOD: 159 patients undergoing sellar region surgery between March 2001 and December 2006 were assessed retrospectively. 91 patients who underwent 95 endoscopic transnasal procedures were included in this study. Study design: a clinical retrospective study. RESULTS: The endoscopic transnasal technique was feasible for every patient, independent of age, anatomical variations, tumor characteristics, tumor etiology, and previous surgical history. There was no need to remove the middle turbinate or septal deviations in any of the cases. The most significant peroperative complication was CSF leak during tumor removal (13.68%). Postoperative complications were: nasal bleeding (8.42%), CSF leak (8.42%), and meningitis (2.19). CONCLUSION: The transnasal endoscopic approach was accomplished with minimal invasion, preserving nasal structures in all 95 procedures, independent of age, anatomical variations, tumor characteristics,tumor etiology, and previous surgical history.

Muitos estudos demonstram associação entre queilite actínica e carcinoma espinocelular do lábio. OBJETIVO: Verificar a relação da queilite actínica com o prognóstico dessa neoplasia. MATERIAIS E MÉTODOS: Elaborou-se um estudo de coorte retrospectivo com corte transversal em carcinoma espinocelular do lábio. Cortes histológicos desse tumor, levantados entre 1993-2000, nos arquivos do Departamento de Patologia/Universidade Federal de São Paulo, foram revisados para evidenciar presença ou ausência de queilite actínica no vermelhão adjacente ao tumor. Os prontuários dos pacientes foram revisados à procura de informações sobre exposição solar, metástase e recidiva. A ocorrência ou ausência de recidiva e metástase foi correlacionada com a presença ou ausência de queilite actínica no vermelhão. Os dados obtidos foram analisados pelo teste exato de Fisher. RESULTADOS: Dos 31 pacientes selecionados predominou o sexo masculino, cor da pele branca e localização no lábio inferior. Constatou-se: independência entre a ocorrência de metástase e recidiva com sexo, cor dos pacientes e localizações no lábio superior ou inferior; dependência entre a presença de queilite actínica e elastose solar, dependência entre a ausência de queilite actínica e presença de metástase; independência entre a ausência de queilite actínica e presença de recidiva. CONCLUSÃO: Os tumores originários de queilite actínica têm melhor prognóstico.

Many studies have shown an association between actinic cheilitis and squamous carcinoma of the lips. AIM: The aim of the study was to observe the relation between actinic cheilitis and the prognosis of squamous carcinoma of the lips. MATERIALS AND METHODS This is a retrospective cross-sectional cohort study of squamous carcinoma of the lips. Histological sections of squamous carcinoma tumors done at the the Departament of Pathology of the Sao Paulo Federal University between 1993 and 2000 were reviewed for evidence of actinic cheilitis in the lip vermillion adjacent to the tumor. Patient reports were reviewed to find information about exposure to sun, metastases and relapses. The occurrence or absence of relapses and metastases was correlated with the presence or absence of actinic cheilitis in the lip vermillion. Data was analyzed by Fisher’s Exact test. RESULTS: Of the 31 selected patients, most were caucasian, males and with lower lip involvement. Statistical analysis demonstrated independence between the occurrence of metastases and relapse and gender, skin color and site (lower or upper lips). There was dependence between actinic cheilitis and solar elastosis, and between the absence of actinic cheilitis and the occurrence of metastases. There was no dependence between the absence of actinic cheilitis and the occurrence of relapses. CONCLUSION: It may be concluded that tumors originating from actinic cheilitis have a better prognosis.

Trinta por cento das faringotonsilites agudas são de etiologia estreptocócica com potencial de complicações como a glomerulonefrite difusa aguda e febre reumática. Crianças de creches apresentam maior incidência destas infecções. OBJETIVO: Identificar e comparar a prevalência do Streptococcus pyogenes na orofaringe entre crianças que freqüentam creches e crianças não-institucionalizadas, em duas regiões do Brasil. CASUÍSTICA E MÉTODO: Estudo prospectivo com 200 crianças, provenientes da cidade de Porto Velho/RO e São Paulo/SP, em quatro grupos, freqüentadoras ou não de creches. Realizou-se swab de orofaringe e cultura para identificação do Streptococcus pyogenes. RESULTADOS: Prevalência de 8% e 2% entre as crianças de São Paulo que atendem a creches e do grupo controle, respectivamente, apresentando valor estatístico (p=0,02). Prevalência de 24% e 16% nos grupos de Porto Velho/RO que freqüentam creche e controle respectivamente, não caracterizando diferença estatisticamente significante (p=0,18). Observou-se diferença estatisticamente significante entre os grupos creche e controle de São Paulo/SP aos seus correspondentes de Porto Velho/RO (p < 0,01). CONCLUSÃO: Os resultados deste estudo nos permitem sugerir que freqüência em creches representa um fator de risco para a colonização de orofaringe pelo Streptococcus pyogenes, fato observado em populações distintas, porém com significância estatística em apenas uma das duas amostras.

Thirty percent of acute pharyngotonsillitis is caused by Streptococcus pyogenes, which increased the risk of glomerulonephritis and rheumatic fever. Children attending daycare centers have a higher incidence of these infections. AIM: to identify and compare the prevalence of Streptococcus pyogenes in the oropharynx of children who are enrolled and who are not enrolled in daycare centers in different regions of Brazil. MATERIALS AND METHODS: A prospective study of two hundred children from Sao Paulo/SP and Porto Velho/RO. Children from each city were divided into two groups: those attending, and those not attending daycare centers. Swabs of the oropharynx were taken for bacteriological culture and identification. RESULTS: The prevalence of Streptococcus pyogenes in the Sao Paulo groups were 8% and 2% for daycare and control groups, which was statistically significant (p=0.02). The prevalence in children from Porto Velho/RO was 24% and 16% for daycare and control groups, which was statistically significant (p=0.015). Statistical analysis also showed a significant difference between the corresponding groups in the two locations (p<0.01). CONCLUSION: These results show that daycare attendance is a risk factor for oropharyngeal streptococcal colonization; this was seen in different populations, but was statistically significance in only one of the two samples.

Os casos de introdução de corpos estranhos acidentais em crianças são freqüentes nos pronto-atendimentos. O otorrinolaringologista pode atuar nos casos localizados no esôfago. A experiência é fundamental para o sucesso das intervenções. OBJETIVO: Descrever o atendimento de crianças que ingeriram moedas no Setor de Otorrinolaringologia do Hospital João XXIII. FORMA DE ESTUDO: Clínico prospectivo. MATERIAL E MÉTODO: Foram avaliados sete casos de ingestão de moedas (idade, sexo, status na família, tamanho da moeda, conduta/evolução). RESULTADOS: A idade variou de um até nove anos. Dois pacientes eram filhos únicos e cinco eram mais novos. O tamanho da moeda variou de 1,9cm até 2,5cm. Após oito horas de observação, três casos necessitaram de remoção no centro cirúrgico porque a radiografia mostrava a moeda na cricofaringe e em quatro casos houve a descida espontaneamente para o intestino. CONCLUSÃO: O Setor de Otorrinolaringologia tem bons resultados usando laringoscópio de lâmina reta e pinça nos casos alojados na cricofaringe e esofagoscopia rígida para os casos distais. A amostra não permite concluir se o tamanho da moeda e a idade influenciam a descida espontânea para o trato gastrointestinal e se os pacientes filhos únicos ou os mais novos são mais predispostos a este acidente.

The ingestion of foreign bodies by children is frequently seen in emergency departments. ENTs can manage those lodged in the esophagus but experience is important for a successful intervention. AIM: Describe seven cases of children that ingested coins, managed at the ENT Department of João XXIII Hospital. STUDY DESIGN: Clinical/prospective. MATERIALS AND METHODS: We describe seven cases (gender, age, family status, coin size and treatment/evolution). RESULTS: Age ranged from one to nine years. Two patients were only children and five were the youngest in their families. Coins sizes ranged from 1.9 to 2.5 cm. After eight hours of observation, three cases were treated in the surgery room because the foreign body was lodged in the cricopharynx. Four cases resolved spontaneously. CONCLUSION: The ENT department has good results removing coins lodged in the upper esophagus using forceps and laryngoscopy; and also using rigid esophagoscopy for the lower esophagus. In this study it was not possible to establish the importance of coin size and patient age in attempting to predict spontaneous resolution, nor if the child being an only child or the youngest in the family may have some predisposition in this kind of accident.

Poucos estudos sobre os efeitos da desnutrição protéico-calórica na mucosa oral são encontrados na literatura. OBJETIVOS: O objetivo deste trabalho é verificar os efeitos da desnutrição protéico-calórica na mucosa da língua de ratos adultos, envolvendo microscopia óptica e eletrônica de varredura. MATERIAIS E MÉTODOS: Um estudo experimental foi realizado em trinta ratos Wistar, 15 controles e 15 com desnutrição protéico-calórica. O último grupo recebeu ração em pequena quantidade, com menor conteúdo de caseína, durante 45 dias. Os ratos foram pesados a cada 3 dias, do primeiro (90 dias de vida) ao último dia (45 dias de dieta), quando foram sacrificados. Foi realizada eletroforese de proteínas plasmáticas e as línguas foram preparadas para microscopia óptica comum e eletrônica. Para análise estatística utilizou-se análise de variância e teste T de Student. RESULTADOS: Foi observada uma significante diminuição no peso e nas proteínas plasmáticas dos ratos com desnutrição protéico-calórica em relação ao grupo controle. A análise histológica não mostrou diferenças entre os dois grupos, e os resultados, com respeito à contagem das papilas filiformes da mucosa lingual pela microscopia eletrônica, não revelou diferenças estatisticamente significantes entre os grupos. CONCLUSÃO: A desnutrição protéico-calórica não causa alterações na mucosa da língua de ratos adultos.

There are few published studies on the effects of protein-caloric undernourishment on the oral mucosa. OBJECTIVES: The objective of this study is to verify the histological and ultrastructural aspects of the tongue mucosa in protein-caloric undernourished adult rats. MATERIAL AND METHODS: A clinical experimental study was done in thirty Wistar rats, 15 controls and 15 with protein-caloric undernourishment. The last group received ration in small amounts, with a reduced casein content, during 45 days. Rats were weighed every 3 days, from the first (90 days of life) to the last day of a 45-day dietary period, when they were sacrificed. Plasma was used for protein electrophoresis and their tongues were prepared for light and scanning electron microscopy. Analysus of variance and Student’s t-test were used for statistical analysis. RESULTS: A significant decrease in weight and in plasma proteins was found in protein-caloric undernourished rats compared to the control group. Histological findings revealed no differences between the two groups and there were no statistically significant differences in the filiform papilla count under the scanning electron microscopy. CONCLUSION: Protein-caloric undernourishment does not cause alterations in the tongue mucosa of adult rats.
