Resumo Introdução A força extrabucal é a estratégia mais comum para corrigir a má oclusão de Classe II de Angle, restringindo e redirecionando o crescimento maxilar. Objetivo Avaliar as alterações esqueléticas decorrentes do uso do aparelho extrabucal, com ancoragens cervical e parietal, associado ao aparelho ortodôntico fixo, em pacientes em crescimento com má oclusão de Classe II (Angle) tratados na clínica de um centro de treinamento ortodôntico. Material e método As idades ao início e ao término do tratamento, o tipo de ancoragem, e os valores de algumas variáveis cefalométricas foram obtidos dos arquivos clínicos. Após a aplicação dos critérios de inclusão e exclusão, foram selecionados 56 prontuários. Em seguida, a amostra foi dividida em dois grupos, de acordo com a ancoragem: cervical (n= 30) e parietal (n=26). As diferenças entre os valores inicial (T1) e final (T2) das variáveis cefalométricas SNA, SNB, ANB, AO-BO, GoGn.SN e AFI (porcentagem da altura facial inferior em relação à altura facial total) foram avaliadas em ambos os grupos. Resultado Diferenças significativas entre T1 e T2 foram encontradas em relação às variáveis SNB e ANB em ambos os grupos. A variável AO-BO apresentou diferença estatisticamente significante apenas no grupo cervical. As demais variáveis não apresentaram diferenças significantes entre T1 e T2. Conclusão As alterações esqueléticas decorrentes do uso das ancoragens cervical e parietal foram muito semelhantes. Houve diminuição da discrepância anteroposterior entre a maxila e a mandíbula devido ao deslocamento anterior da mandíbula, sem alterações verticais significativas.

Abstract Introduction Extraoral strength is the most common strategy to correct Angle Class II malocclusion, restricting and redirecting the maxillary growth. Objective To evaluate the skeletal changes resulting from the use of headgear, with cervical and parietal anchorages, associated with a full fixed orthodontic appliance, in growing patients with Class II malocclusion (Angle) treated in the clinic of an orthodontic training center. Material and method The ages at the beginning and end of the treatment, anchorage type, and the values of some cephalometric variables were obtained from the clinical files. After applying the inclusion and exclusion criteria, 56 patient files were selected. Then, the sample was divided into two groups, according to the anchorage type: cervical (n=30) and parietal (n=26). The differences between the initial (T1) and final (T2) values of cephalometric measurements SNA, SNB, ANB, AO-BO, GoGn.SN and LHF (percentage of lower facial height to total facial height) were evaluated in both groups. Result Significant differences between T1 and T2 were found relative to SNB and ANB variables in both groups. The AO-BO variable presented a statistically significant difference only in the cervical group. The remaining variables did not show significant differences between T1 and T2. Conclusion The skeletal changes resulting from the use of cervical or parietal anchorage were very similar. There was a decrease in the sagittal discrepancy between the maxillary bones due to forward displacement of the mandible, without significant vertical changes.

RESUMO Objetivo: Avaliar as alterações dentárias e esqueléticas decorrentes do uso exclusivo do aparelho extrabucal durante 15 ± 4 meses para tratamento de pacientes com má oclusão de Classe II divisão 1 (Grupo Experimental, GE). Métodos: As diferenças entre o início (T1) e imediatamente após o término da terapia (T2) com o aparelho extrabucal de tração cervical (Grupo Experimental, GE, n = 23) foram comparadas àquelas apresentadas por um grupo composto por indivíduos não tratados (Grupo Controle, GC, n = 22), com má oclusão e faixa etária cronológica compatíveis. As variáveis cefalométricas avaliadas foram: ANB, GoGn.SN, AO-BO, S’-ENA, S’-A, S’-B, S’-Pog e S’-U6 (primeiro molar superior). Os testes de Shapiro-Wilk e Levene foram aplicados para avaliar os resultados. Resultados: Diferenças significativas entre T1 e T2 foram encontradas para as variáveis ANB, S’-U6, AO-BO, S’-ENA, S’-A, S’-B e S’-Pog, quando comparados os dois grupos. Nenhuma diferença estatisticamente significativa foi encontrada em relação ao ângulo GoGn.SN. Conclusão: O uso do aparelho extrabucal com tração cervical promoveu movimento para distal do primeiro molar superior e restringiu o deslocamento anterior da maxila, sem afetar significativamente o ângulo GoGn.SN.

ABSTRACT Objective: Evaluate dental and skeletal changes resulting from the exclusive use of the cervical headgear for 15 ± 4 months in the treatment of patients with Class II division 1 malocclusion. Methods: Differences between the beginning (T1) and immediately after the end of the therapy (T2) with the cervical headgear in growing patients (Experimental Group, EG, n = 23) were examined and compared, during compatible periods, with those presented by a group of untreated individuals (Control Group, CG, n =22) with similar malocclusions and chronological age. The cephalometric variables evaluated were: ANB, GoGn.SN, AO-BO, S'-ANS, S'-A, S'-B, S'-Pog and S'-U6 (maxillary first molar). The Shapiro-Wilk and Levene tests were used to evaluate the results. Results: Significant differences were found relative to the ANB, S'-U6, AO-BO, S'-ANS, S'-A, S'-B and S'-Pog variables between T1 and T2 when comparing both groups. No statistically significant variation was found regarding the GoGn.SN angle. Conclusions: The use of cervical headgear promoted distal movement of the maxillary first molars and restricted the anterior displacement of the maxilla, without significantly affecting the GoGn.SN angle.

RESUMO Objetivos: a acurácia e a confiabilidade de modelos de gesso e modelos digitais adquiridos por dois diferentes scanners de superfície a laser foram testadas com três métodos: medição com paquímetro, medição com programa específico e sobreposição de modelos digitais. Métodos: trinta modelos de gesso com dentição permanente que preencheram os critérios de inclusão predeterminados foram selecionados e escaneados com dois diferentes scanners a laser (R700 e Xcad). Três examinadores mediram distâncias selecionadas nos modelos de gesso usando um paquímetro digital e, nos modelos digitais, usando o programa Ortho Analyzer. Os modelos digitais também foram comparados por sobreposição de modelos, com o programa Geomagic Qualify. A confiabilidade intra e interexaminadores das medições foi avaliada por meio do coeficiente de correlação intraclasse (ICC). O teste t pareado foi utilizado para avaliar a acurácia das medições nos modelos de gesso e digitais. Resultados: as medições nos modelos de gesso e nos modelos digitais escaneados pelos dois diferentes scanners apresentaram valores elevados para o ICC. As diferenças estatisticamente significativas encontradas entre as medições nos modelos de gesso e nos modelos digitais não foram consideradas clinicamente relevantes. O método de sobreposição de modelos com o programa Geomagic Qualify demonstrou que os dois tipos de modelos digitais não foram significativamente diferentes. Conclusões: os modelos digitais criados a partir do escaneamento dos modelos de gesso com os scanners R700 e Xcad foram clinicamente precisos, de acordo com os dois métodos de comparação utilizados.

ABSTRACT Objectives: The accuracy and reliability of plaster models and digital models acquired with two different surface laser scanners were tested by means of three methods: measurement with calipers, digital measurement with proper software and superimposition of the digital models. Methods: Thirty plaster models with permanent dentition that met the inclusion criteria were selected and scanned with two laser scanners (R700 and Xcad). Three examiners measured distances on plaster models with a digital caliper and on digital models using Ortho Analyzer software. The digital models were also compared by means of superimposition of the models using the Geomagic Qualify software. The intra and inter-examiner reliability of the measurements were evaluated using the ICC. Paired t test was used to test the accuracy of the measurements on digital and plaster models. Results: The measurements on plaster and digital models acquired by two different scanners showed high values for the ICC. Although statistically significant differences between the measurements on plaster and digital models have been found, these discrepancies were not considered clinically relevant. The superimposition method with Geomagic Qualify software showed that the two digital models were not significantly different. Conclusions: Digital models created from scanned plaster models using the R700 or Xcad scanners were clinically accurate according to the two methods of comparison used.

RESUMO Objetivo: o objetivo deste estudo foi comparar as doses equivalentes e efetivas absorvidas pelo mesmo receptor, quando exposto a diferentes exames radiográficos digitais (panorâmica, telerradiografia lateral e periapicais) e à tomografia computadorizada de feixe cônico (TCFC). Métodos: dosímetros termoluminescentes pré-calibrados foram colocados em 24 locais em um phantom antropomórfico (Alderson Rando Phantom, Laboratórios de Pesquisas de Alderson, New York, NY, EUA), o que representa um adulto de tamanho médio. Os seguintes equipamentos foram avaliados: Heliodent Plus (Sirona Dental Systems, Bernsheim, Alemanha), Orthophos XG 5 (Sirona Dental Systems, Bernsheim, Alemanha) e i-CAT (Imaging Sciences International, Hatfield, PA, EUA). As doses equivalentes e as doses efetivas foram calculadas tendo-se em conta as recomendações da Comissão Internacional de Proteção Radiológica (International Commission on Radiological Protection, ICRP), publicadas em 1990 e 2007. Resultados: embora a dose eficaz do set radiográfico tenha correspondido a 17,5% (ICRP 1990) e 47,2% (ICRP 2007) da dose da TCFC, as doses equivalentes na pele e superfícies ósseas e musculares desse set foram maiores, quando comparadas às da TCFC. Em algumas áreas, a radiação produzida pelo set de radiografias foi maior, devido à radiação do exame periapical completo. Conclusão: considerando-se o princípio da otimização da proteção radiológica, a tomografia computadorizada adquirida no tomógrafo i-CAT deve ser usada apenas em circunstâncias específicas e justificadas. Além disso, seguindo o princípio ALARA, radiografias periapicais unitárias de áreas pré-selecionadas são mais apropriadas do que o exame periapical completo.

ABSTRACT Objective: The aim of this study was to compare the equivalent and effective doses of different digital radiographic methods (panoramic, lateral cephalometric and periapical) with cone-beam computed tomography (CBCT). Methods: Precalibrated thermoluminescent dosimeters were placed at 24 locations in an anthropomorphic phantom (Alderson Rando Phantom, Alderson Research Laboratories, New York, NY, USA), representing a medium sized adult. The following devices were tested: Heliodent Plus (Sirona Dental Systems, Bernsheim, Germany), Orthophos XG 5 (Sirona Dental Systems, Bernsheim, Germany) and i-CAT (Imaging Sciences International, Hatfield, PA, USA). The equivalent doses and effective doses were calculated considering the recommendations of the International Commission of Radiological Protection (ICRP) issued in 1990 and 2007. Results: Although the effective dose of the radiographic set corresponded to 17.5% (ICRP 1990) and 47.2% (ICRP 2007) of the CBCT dose, the equivalent doses of skin, bone surface and muscle obtained by the radiographic set were higher when compared to CBCT. However, in some areas, the radiation produced by the orthodontic set was higher due to the complete periapical examination. Conclusion: Considering the optimization principle of radiation protection, i-CAT tomography should be used only in specific and justified circumstances. Additionally, following the ALARA principle, single periapical radiographies covering restricted areas are more suitable than the complete periapical examination.

OBJETIVO: avaliar a prevalência de reabsorções radiculares externas severas e identificar prováveis fatores de risco decorrentes do tratamento ortodôntico. MÉTODOS: utilizou-se uma amostra selecionada aleatoriamente, composta de radiografias periapicais de incisivos superiores e inferiores, obtidas no mesmo centro radiológico, de pré- e pós-tratamento ortodôntico ativo, de 129 pacientes, de ambos os sexos, tratados por meio da técnica Edgewise Standard. Dois examinadores mensuraram e definiram a reabsorção radicular de acordo com índice proposto por Levander et al., e o grau de reabsorção foi registrado, definindo a reabsorção em quatro graus de severidade. Para avaliar a reprodutibilidade intra- e interexaminadores, adotou-se o índice de coeficiente kappa ponderado. O teste chi-quadrado (χ2) foi adotado para avaliar a relação entre a quantidade de reabsorção radicular e o sexo dos pacientes, arcada dentária (superior ou inferior), tratamentos com ou sem extrações, duração do tratamento, forma radicular, estágio do ápice radicular (aberto ou fechado), overjet e overbite no início do tratamento. RESULTADOS: os incisivos centrais superiores apresentaram a maior porcentagem de reabsorção radicular severa, seguidos dos incisivos laterais superiores e dos incisivos laterais inferiores. Entre 959 dentes avaliados, 28 (2,9%) apresentaram reabsorção radicular severa. Os fatores de risco relacionados foram: dentes localizados na região anterossuperior, overjet maior ou igual a 5mm ao início do tratamento, tratamentos envolvendo extrações dentárias, tempo prolongado de terapia e formação radicular completa à época do início do tratamento ortodôntico. CONCLUSÃO: o estudo demonstrou que cuidados devem ser tomados em tratamentos ortodônticos envolvendo extrações, com grande retração de incisivos superiores, tratamentos prolongados e/ou ápice radicular completamente formado no início da terapia ortodôntica.

OBJECTIVE: To assess the prevalence of severe external root resorption and its potential risk factors resulting from orthodontic treatment. METHODS: A randomly selected sample was used. It comprised conventional periapical radiographs taken in the same radiology center for maxillary and mandibular incisors before and after active orthodontic treatment of 129 patients, males and females, treated by means of the Standard Edgewise technique. Two examiners measured and defined root resorption according to the index proposed by Levander et al. The degree of external apical root resorption was registered defining resorption in four degrees of severity. To assess intra and inter-rater reproducibility, kappa coefficient was used. Chi-square test was used to assess the relationship between the amount of root resorption and patient's sex, dental arch (maxillary or mandibular), treatment with or without extractions, treatment duration, root apex stage (open or closed), root shape, as well as overjet and overbite at treatment onset. RESULTS: Maxillary central incisors had the highest percentage of severe root resorption, followed by maxillary lateral incisors and mandibular lateral incisors. Out of 959 teeth, 28 (2.9%) presented severe root resorption. The following risk factors were observed: anterior maxillary teeth, overjet greater than or equal to 5 mm at treatment onset, treatment with extractions, prolonged therapy, and degree of apex formation at treatment onset. CONCLUSION: This study showed that care must be taken in orthodontic treatment involving extractions, great retraction of maxillary incisors, prolonged therapy, and/or completely formed apex at orthodontic treatment onset.

AIM: In order to determine the reliability between two of these methodologically different method, this study evaluated the systematic and random errors of the method proposed by Tanaka and Johnston, which is based on the sum of mandibular permanent incisors, and the Huckaba method, which uses radiographs. METHODS: In a random sample of 28 plaster models of mandibular dental arches belonging to individuals of both genders, aged six to eleven years old, a single investigator performed the measurement of required space, according to the two methods evaluated. After 15 days, the measurements were repeated, and each of them was performed twice in sequence to calculate the repeatability and reproducibility conditions, and the systematic and random errors for each method. RESULTS: The random error of the method proposed by Huckaba was larger in terms of reproducibility (1.53 mm) and repeatability (0.57 mm) compared with the analysis proposed by Tanaka and Johnston (0.20 mm and 0.12 mm, respectively). CONCLUSIONS: The method proposed by Huckaba was proved to be inadequate in relation to reproducibility, with respect to the random error, and should be used with caution to measure the required space in the mandibular arch.

Objective: To compare the influence of archwire material (NiTi, beta-Ti and stainless steel) and brackets design (self-ligating and conventional) on the frictional force resistance. Material and Methods: Two types of brackets (self-ligating brackets - Smartclip, 3M/Unitek - and conventional brackets - Gemini, 3M/Unitek) with three (0, 5, and 10 degrees) slot angulation attached with elastomeric ligatures (TP Orthodontics) were tested. All brackets were tested with archwire 0.019"x0.025" nickel-titanium, beta-titanium, and stainless steel (Unitek/3M). The mechanical testing was performed with a universal testing machine eMIC DL 10000 (eMIC Co, Brazil). The wires were pulled from the bracket slots at a cross-head speed of 3 mm/min until 2 mm displacement. Results: Self-ligating brackets produced significantly lower friction values compared with those of conventional brackets. Frictional force resistance values were directly proportional to the increase in the bracket/ wire angulation. With regard to conventional brackets, stainless steel wires had the lowest friction force values, followed by nickel-titanium and beta-titanium ones. With regard to self-ligating brackets, the nickel-titanium wires had the lowest friction values, significantly lower than those of other materials. Conclusion: even at different angulations, the self-ligating brackets showed significantly lower friction force values than the conventional brackets. Combined with nickel-titanium wires, the self-ligating brackets exhibit much lower friction, possibly due to the contact between nickel-titanium clips and wires of the same material.

OBJETIVO: Avaliar a influência dos erros quando dois métodos para avaliação do espaço presente no arco dentário são utilizados. MÉTODOS: Um examinador único realizou a aferição do espaço presente em 30 modelos de gesso através de duas técnicas: 1) mensuração realizada com paquímetro digital; e 2) mensuração feita através do contorno do arco com fio de latão e medição deste com régua milimetrada. Após 15 dias, esta análise foi novamente realizada, utilizando os dois métodos, sendo que cada um deles foi empregado duas vezes consecutivas em cada modelo. Assim, foram obtidos dados em condição de repetibilidade e de reprodutibilidade e os erros sistemáticos e aleatórios de cada método nestas condições foram calculados. RESULTADOS: Os erros sistemáticos foram significativos (P<0,05) apenas na condição de reprodutibilidade para o método com fio de latão. Com paquímetro, a análise dos erros aleatórios mostrou que os erros presentes foram inferiores a 1 mm nas duas condições. Com fio de latão estes erros não existiram em condição de repetibilidade, porém foram de 1,69 mm em condição de reprodutibilidade. CONCLUSÕES: No método com paquímetro digital os erros sistemático e aleatório foram considerados aceitáveis. O método de medição com fio de latão não apresentou erro para repetibilidade. Para reprodutibilidade, no entanto, o erro sistemático foi significante e o erro aleatório encontrado fez com que este método fosse considerado no presente estudo como inapropriado.

PURPOSE: To evaluate the influence of errors when two methods for assessing the available space in the dental arch are used. METHODS: A single investigator measured the available space in 30 plaster models by using two techniques: 1) with a digital caliper; and 2) by contouring the arch with a brass wire and subsequently straightening and measuring this wire with a millimeter ruler. Fifteen days later, data were collected using both methods with duplicate measurements, so that each method was used twice for each model. The random and systematic errors of each method under the conditions of repeatability and reproducibility were calculated. RESULTS: For method reproducibility, systematic errors were statistically significant (P<0.05) only when the brass wire method was used. The random errors were less than 1 mm under both conditions. With the brass wire, random errors were not significant for repeatability; however, for reproducibility, the mean value was 1.69 mm. CONCLUSION: In the digital caliper method, systematic and random errors were considered acceptable. The brass wire measurement method did not present errors under the condition of repeatability; for reproducibility, however, the systematic error was significant and the random error indicates that this method should be considered inappropriate to measure space in the dental arch.

OBJETIVO: Avaliar a aplicabilidade da análise de Tanaka e Johnston em indivíduos brasileiros de cor de pele branca e de cor de pele negra. METODOLOGIA: Um total de 650 modelos de gesso de arcadas dentárias foram examinados e 95 foram selecionados para compor a amostra. Mediu-se a maior distância entre os pontos de contato dos incisivos inferiores e caninos e pré-molares, superiores e inferiores do lado direito, com o paquímetro paralelo ao plano oclusal e perpendicular ao longo eixo do dente. A predição do comprimento dos caninos, primeiros e segundos pré-molares em ambos os arcos foi feita através da análise de Tanaka e Johnston. Comparou-se a predição e o valor real do tamanho dos dentes em relação ao gênero e à cor da pele dos sujeitos da amostra. RESULTADOS: A análise de Tanaka e Johnston superestimou o somatório dos diâmetros mésio-distais dos caninos e pré-molares apenas no grupo de indivíduos de cor de pele branca do gênero feminino. Para os outros grupos não foram observadas diferenças estatisticamente significantes. CONCLUSÃO: A análise de Tanaka e Johnston permite uma predição aceitável do somatório dos diâmetros mésio-distais dos caninos e pré-molares dos indivíduos brasileiros de cor de pele negra e de cor de pele branca do gênero masculino. Com relação aos indivíduos de cor de pele branca do gênero feminino, os valores foram superestimados.

PURPOSE: To assess the applicability of the Tanaka-Johnston analysis in Brazilian subjects. METHODS: A total of 650 plaster casts of dental arches were examined, and 95 were selected to comprise the study sample. The largest distance between the contact points of mandibular and maxillary incisors as well as mandibular canines and premolars on the right side were measured with a calliper held parallel to occlusal plane and perpendicular to the long axis of the tooth. A prediction of the length of canines, and first and second premolars in both arches was made by using the Tanaka-Johnston analysis. Predicted and actual dental sizes were compared in relation to the gender and ethnic group of the sample subjects. RESULTS: The Tanaka-Johnston analysis overestimated the sum of mesiodistal widths of maxillary and mandibular canines and premolars only in the white female group. The difference between the Tanaka-Johnston prediction and the actual measurement was significant. For the other groups studied no statistical difference was observed. CONCLUSION: The Tanaka-Johnston analysis provided an acceptable prediction of the sum of mesiodistal widths of maxillary and mandibular canines and premolars in black and white Brazilian men, but not in white Brazilian women.

OBJETIVO: comparar a confiabilidade de identificação dos pontos visualizados sobre radiografias cefalométricas convencionais e sobre imagens geradas pela Tomografia Computadorizada Cone-Beam em 2D e 3D. MÉTODOS: o material constou de imagens obtidas através do tomógrafo computadorizado Cone-Beam, em norma lateral, em 2D e 3D, impressas em papel fotográfico; e radiografias cefalométricas laterais, realizadas na mesma clínica radiológica e no mesmo dia, de dois pacientes pertencentes aos arquivos do Curso de Especialização em Ortodontia da Faculdade de Odontologia da Universidade Federal Fluminense (UFF). Dez alunos do Curso de Especialização em Ortodontia da UFF identificaram pontos de referência sobre papel de acetato transparente e foram feitas medições das seguintes variáveis cefalométricas: ANB, FMIA, IMPA, FMA, ângulo interincisal, 1-NA (mm) e ¯1-NB (mm). Em seguida, foram calculadas médias aritméticas, desvios-padrão e coeficientes de variância de cada variável para os dois pacientes. RESULTADOS E CONCLUSÃO: os valores das medições realizadas a partir de imagens em 3D apresentaram menor dispersão, sugerindo que essas imagens são mais confiáveis quanto à identificação de alguns pontos cefalométricos. Entretanto, como as imagens em 3D impressas utilizadas no presente estudo não permitiram a visualização de pontos intracranianos, torna-se necessário que softwares específicos sejam elaborados para que esse tipo de exame possa se tornar rotineiro na clínica ortodôntica.

OBJECTIVE: To compare the reliability of two different methods used for viewing and identifying cephalometric landmarks, i.e., (a) using conventional cephalometric radiographs, and (b) using 2D and 3D images generated by Cone-Beam Computed Tomography. METHODS: The material consisted of lateral view 2D and 3D images obtained by Cone-Beam Computed Tomography printed on photo paper, and lateral cephalometric radiographs, taken in the same radiology clinic and on the same day, of two patients selected from the archives of the Specialization Program in Orthodontics, at the School of Dentistry, Fluminense Federal University (UFF). Ten students from the Specialization Program in Orthodontics at UFF identified landmarks on transparent acetate paper and measurements were made of the following cephalometric variables: ANB, FMIA, IMPA, FMA, interincisal angle, 1-NA (mm) and ¯1 -NB (mm). Arithmetic means were then calculated, standard deviations and coefficients of variance of each variable for both patients. RESULTS AND CONCLUSIONS: The values of the measurements taken from 3D images showed less dispersion, suggesting greater reliability when identifying some cephalometric landmarks. However, since the printed 3D images used in this study did not allow us to view intracranial landmarks, the development of specific software is required before this type of examination can be used in routine orthodontic practice.
