ABSTRACT Objective To evaluate the influence of onco-hematological pathologies on seroconversion to COVID-19 vaccines, in addition to the effects of chemotherapy treatment on this response. Methods The present study evaluated the immunogenic response of 76 patients with onco-hematological diseases to multiple vaccine platforms compared to 25 control individuals. Results Our results showed positive response rates of 74.02% in patients with onco-hematological diseases and 100% in controls. When analyzed according to etiological group, patients with lymphoproliferative disorders achieved a positive vaccine response rate of 58.7%, whereas those with myeloproliferative diseases achieved a 100% response rate. We also observed that patients previously exposed to COVID-19 presented a 75% increase in their antibody values after vaccination, and these values were 37% higher than those of patients who did not have such exposure. We found that patients who underwent B-lymphocyte-depleting therapy in the last 2 years before vaccination had a worse response rate of 18.75%. BACKGROUND: Haptoglobin genotypes, and interleukin-6 and -8 participate in the pathophysiology of sickle cell anemia. The expression of cytokines is regulated by genetic mechanisms however the effect of haptoglobin polymorphisms on these cytokines is not fully understood. This study aimed to compare the frequency of haptoglobin genotypes and the interleukin-6 and -8 concentrations in sickle cell anemia patients and controls to investigate the association between haptoglobin genotypes and cytokine levels.METHODS: Sixty sickle cell anemia patients and 74 healthy individuals were analyzed. Haptoglobin genotypes were determined by multiplex polymerase chain reaction, and the interleukin-6 and -8 levels by enzyme linked immunosorbent assay. The association between haptoglobin genotypes and cytokines was investigated by statistical tests.RESULTS:Hp2-1 was the most common genotype in both the cases and controls while Hp1-1 was less frequent among sickle cell anemia patients. Interleukin-6 and -8 levels were higher in patients than controls (p-value <0.0001). There was no significant difference in interleukin-6 and -8 concentrations between the genotypes (p-value >0.05). A similar trend was observed among the controls.CONCLUSION: Although, levels of interleukin-6 and -8 were higher in the sickle cell anemia patients, they appeared not to be related to the haptoglobin genotypes. Further investigations are necessary to identify factors responsible for increased secretion of the interleukin-6 and -8 pro-inflammatory cytokines in patients with sickle cell anemia.

A deficiência de ferro é considerada a patologia hematológica mais prevalente no homem. Assim, é fundamental a adequada identificação de suas causas, bem como a diferenciação com outras patologias distintas para adequada abordagem da deficiência de ferro. Neste artigo são brevemente descritas outras condições que podem cursar com anemia microcítica, tais como: talassemias, anemia de doença crônica, anemia sideroblástica e envenenamento por chumbo, patologias estas que devem ser afastadas durante investigação de anemia ferropriva.

Iron deficiency is considered to be the commonest hematological pathology in humans. Thus, the essential steps in an adequate approach of iron deficiency include: the proper identification of its causes and the differentiation between iron deficiency and other conditions. This article briefly describes other conditions that may present with microcytic anemia such as thalassemia, anemia of chronic diseases, sideroblastic anemia and lead intoxication. These diseases should be considered during the investigation of iron deficiency anemia.

A osteoporose, caracterizada por aumento da fragilidade óssea e suscetibilidade a fraturas, é inversamente proporcional ao pico de massa óssea adquirido na infância. Por outro lado, a doença óssea é uma importante causa de morbidade em pacientes portadores de beta-talassemia maior (TM). Apesar de intensamente descrita em pacientes talassêmicos adultos, não existem estudos sobre as alterações de densidade óssea em crianças talassêmicas brasileiras. Foram avaliados 11 pacientes (idade mediana de 10,0, variando de 5 a 12 anos), portadores de TM, e 24 crianças (idade mediana de 9,5, variando de 6 a 12 anos) saudáveis, utilizando medida de emissão dupla de raios-X para avaliar a densidade mineral óssea (DMO). A análise de marcadores bioquímicos tais como concentração de ferritina sérica, cálcio ionizado, fosfatase alcalina, fósforo, albumina, tempo de protrombina e fator V foi realizada. A estatura foi significativamente diferente entre os dois grupos estudados, p<0,05. Os pacientes talassêmicos mostraram valores significativamente inferiores de DMO (mediana 0,61 g/cm²) quando comparados aos indivíduos controles (mediana 0,69 g/cm²), p < 0,05. A relevante perda óssea encontrada na maioria das crianças talassêmicas estudadas reforça a necessidade de identificação e tratamento adequado da osteopenia, reduzindo a morbidade destes indivíduos. Este é o primeiro estudo, descrito na literatura, que avalia a DMO em crianças talassêmicas brasileiras.

Osteoporosis is characterized by low bone mass and disruption of bone architecture, resulting in greater bone fragility with increased risk of fractures. Bone disease is an important cause of morbidity in beta thalassemia major patients. Osteoporosis has been described extensively in adult thalassemia. However, there are no studies describing Brazilian thalassemic children. We evaluated eleven patients with beta thalassemia major (median age of 10.0 years, range from 5 to 12 years) and twenty-four healthy children (median age of 9.5 years, range from 6 to 12 years), using dual X-ray absorptiometry to assess bone mineral density (BMD). Analysis of biochemical markers such as serum ferritin concentration, ionized calcium, alkaline phosphatase, phosphorus, albumin, prothrombin time and factor V was performed. The height was very different between the groups, p<0.05. The thalassemic patients showed significantly lower BMD (median 0.61 g/cm²) than control subjects (median 0.69 g/cm²) - p < 0.05. The relevant bone loss in the majority of thalassemic children studied emphasizes the need for identification and appropriate treatment of osteopenia, thereby reducing the morbidity of these patients. This is the first study described in the literature that determined bone mineral loss in Brazilian thalassemic children.

As causas de anemia aplástica adquirida (AAA) incluem mecanismos imunológicos e oxidativos de lesão ao DNA. Glutathiona S-transferase (GST) é fundamental na detoxicação celular. Em humanos, os genes da GST são codificados por quatro clones: alpha (A), mu (M), pi (P) e theta (T). Entre os genes da GST, GST M1 e T1 possuem um genótipo nulo que resulta em ausência de atividade enzimática. O objetivo deste estudo foi investigar os polimorfismos das enzimas GSTM1 e GSTT1 em pacientes brasileiros portadores de AAA. O alelo nulo da GSTM1 foi observado em três (16.6%) pacientes e o GSTT1 nulo foi observado somente em um (5.5%) paciente. Este estudo não encontrou associação entre os polimorfismos genéticos das enzimas de detoxicação GSTM1/ GSTT1 e a patogênese da AAA.

The causes of acquired aplastic anemia (AAA) include immunologic mechanisms and oxidative DNA damage. Glutathione S-transferase (GST) plays an important role in detoxification. In humans, GST genes encode four main clones: alpha (A), mu (M), pi (P) and theta (T). Among GST genes, GST M1 and T1 have null genotypes that result in a lack of activity. The aim of this study was to investigate polymorphisms of the GSTM1 and GSTT1 enzyme in Brazilian patients with AAA. The null allele of GSTM1 was observed in 3 (16.6%) patients and the GSTT1 null genotype was observed in only one (5.5%) patient. This study did not find any association between genetic polymorphisms of the GSTM1/GSTT1 detoxifying enzymes and the pathogenesis of AAA.

Priapismo é a complicação relativamente freqüente na doença falciforme. Consiste de ereção peniana prolongada e dolorosa, não acompanhada de desejo ou estímulo sexual, usualmente persistente por mais de quatro horas. A disfunção erétil é seqüela comum no tratamento inadequado. A forma típica de priapismo nestes pacientes é a de baixo fluxo, ocorrendo, ainda, a forma de priapismo recorrente ou stuttering. O tratamento inicial para esta complicação ainda não está bem estabelecido. São propostas várias opções medicamentosas, tais como agonistas adrenérgicos, hormônios análogos à gonadotrofina, dietil-estilbestrol, hidroxiuréia entre outras. Nos casos de falha com as medidas conservadoras e medicamentosas, a intervenção cirúrgica, com confecção de shunt cavernoso, é necessária. Este estudo revisa a incidência, patogênese e opções terapêuticas desta complicação na doença falciforme.

Priapism is a common complication of sickle cell disease. It is defined as a painful and persistent penil erection not accompanied by sexual desire or stimulation, usually lasting for more than 4 hours. The typical forms of priapism in sickle cell disease are low-flow and recurrent priapism (stuttering). The first-line treatment for this complication is not totally clear. Several treatments have been proposed such as adrenergic agents, gonadotropin-releasing hormone analogues, diethylstilbestrol, hydroxyurea. If conservative treatments fail, surgical intervention is required with cavernous shunts. This study reviewed the incidence, pathogenesis and management of sickle cell priapism.

A excessiva eritropoese ineficaz na talassemia pode causar hemato-poese extramedular (HEM), resultando em hepatomegalia, esplenomegalia e massas de tecido hematopoético em diversos tecidos. Localizações de HEM compensatória menos freqüentes são rins, glândulas adrenais, canal medular, pleura, pericárdio, duramáter, tecido adiposo e pele. Entretanto, HEM intratorácica é condição rara. Estratégias terapêuticas incluem radiação e transfusões sanguíneas. O uso de hidroxiuréia concomitante a terapêutica transfusional foi associado à regressão clínica da HEM na talassemia. Nós descrevemos um caso de HEM intratorácica em paciente portadora de talassemia intermédia, com significante regressão do quadro após terapêutica isolada com hidroxiuréia.

Excessive ineffective erythropoiesis in thalassemia intermedia may cause extramedullary hematopoiesis (EMH), resulting in spleen and liver enlargement or masses in several tissues, mainly paravertebrally. Other less frequent locations of diffuse compensatory EMH are kidneys, adrenal glands, breasts, spinal cord, pleura, pericardium, duramater, adipose tissue and skin, although intrathoracic extramedullary hematopoiesis is a rare condition. Management strategies have included radiation and transfusion therapy. Hydroxyurea with transfusion therapy has been associated with clinical regression of EMH in thalassemia. We report an uncommon case of intrathoracic EMH in a patient with beta-thalassemia intermedia, that showed significant recovery with HU therapy.

CONTEXTO E OBJETIVO: O aumento adquirido da hemoglobina fetal (HbF) já foi implicado como fator prognóstico em distúrbios diseritropoiéticos. Nossos objetivos foram de examinar elevações adquiridas na HbF em pacientes com anemia aplástica (AA) e hemoglobinúria paroxística noturna (PNH), e de avaliar se há associação entre a presença de polimorfismos XmnI e de região de controle de locus gênico 5' (LCR-HS2) e os níveis de HbF. TIPO DE ESTUDO E LOCAL: Estudo longitudinal no Serviço de Hematologia e Transfusão de Sangue da Universidade Federal de São Paulo - Escola Paulista de Medicina. MÉTODOS: Estudamos um grupo de 37 pacientes com AA e/ou PNH. Reação de polimerase em cadeia (PCR) e digestão enzimática foram usadas para analisar polimorfismos XmnI; e PCR para clonagem e sequenciamento automático dos polimorfismos HS2. RESULTADOS: O nível médio de HbF foi de 2,32%, mas não houve diferença significativa entre o nível de HbF dos pacientes AA e PNH (p = 0.46). Os níveis de HbF menores que 1,0% mostraram correlação estatisticamente significativa com ausência do polimorfismo XmnI (+) (p = 0.007). CONCLUSÕES: Ausência de polimorfismo XmnI está associado com diminuição de HbF. Mais estudos são necessários para confirmar estas observações e fazer comparações sobre tratamento, prognóstico e sobrevida.

CONTEXT AND OBJECTIVE: Acquired fetal hemoglobin (HbF) elevation has been implicated as a prognostic factor in dyserythropoietic disorders. Our objectives were to examine acquired HbF increases in aplastic anemia (AA) and paroxysmal nocturnal hemoglobinuria (PNH) patients, and to evaluate whether there is an association between the presence of XmnI and 5' hypersensitive site locus control region (LCR-HS2) polymorphisms and the HbF levels. DESIGN AND SETTING: Cross-sectional study at the Hematology and Blood Transfusion Service of Universidade Federal de São Paulo - Escola Paulista de Medicina. METHODS: We studied a group of 37 patients with AA and/or PNH. Polymerase chain reaction (PCR) and enzymatic digestion were utilized to analyze XmnI polymorphisms; and PCR, cloning and automated sequencing for the HS2 polymorphisms. RESULTS: The mean HbF level was 2.32%, but there was no significant difference in HbF level between the AA and PNH groups (p = 0.46). HbF levels of less than 1.0% showed a significant correlation with absence of the XmnI (+) polymorphism (p = 0.02). The presence of the XmnI allele was greater in the AA group (p = 0.007). CONCLUSIONS: XmnI polymorphism absence reduction is associated with acquired HbF elevation. Further studies are required to confirm these observations and make treatment, prognosis and survival comparisons.

CONTEXTO: O prognóstico da anemia aplástica grave melhorou com o advento do transplante de medula óssea e do tratamento imunossupressor com globulina antitimocitária. Em contraste com o sucesso destes protocolos, os estudos com seguimento a longo prazo mostraram a ocorrência de doenças clonais, tais como: hemoglobinúria paroxística noturna, síndrome mielodisplásica e leucemia aguda. RELATO DE CASO: Nós relatamos o primeiro caso descrito no Brasil de um paciente com anemia aplástica que evoluiu para síndrome mielodisplásica e leucemia mielóide aguda associada a presença de hemoglobina H e aumento da hemoglobina fetal.

CONTEXT: The prognosis of severe aplastic anemia has improved since the introduction of bone marrow transplantation and treatment with antithymocyte globulin. In contrast to the success of these protocols, studies with long term follow-up have shown the occurrence of clonal diseases such as paroxysmal nocturnal hemoglobinuria, myelodysplastic syndrome and acute leukemia in aplastic anemia. CASE REPORT: We report the first case of a Brazilian patient with aplastic anemia who developed myelodysplastic syndrome and acute myeloid leukemia showing acquired hemoglobin H and increased fetal hemoglobin.

Doença da aglutinina a frio é uma patologia caracterizada pela produção de anticorpos contra antígenos específicos da membrana eritrocitária, geralmente de classe IgM, que aglutinam hemácias em temperaturas abaixo de 32º C. Relatamos um caso de gangrena e insuficiência renal em uma mulher de 77 anos com pneumonia atípica. Anemia hemolítica secundária a crioaglutininas foi observada. O tratamento com antibióticos e corticóides foi eficaz com regressão do quadro, porém foi necessária a amputação de todos os dedos do pé (falanges) bilateralmente, preservando o restante dos pés (tarsos e metatarsos). Anemia hemolítica secundária a crioaglutininas manifesta-se freqüentemente por acrocianose e fenômeno de Raynaud. Gangrena é uma complicação incomum dessa doença, principalmente quando secundária a etiologia infecciosa, sendo observada apenas em pacientes com altos títulos persistentes. Está correlacionada freqüentemente com causa infecciosa (Mycoplasma spp, vírus Epstein-Barr, citomegalovírus, vírus Influenza A, adenovírus, Legionella spp), linfoproliferativas B e idiopáticas. Suspeita clínica de crioaglutininemia deve ser confirmada e tratada prontamente em função da risco de possíveis complicações graves.

The cold agglutinin syndrome is a haemolytic disorder usually manifested by acrocyanosis and Raynaud's phenomenon. Gangrene is an uncommon complication, usually associated with infections or B-cell lymphoproliferative diseases. We present a case of fulminant gangrene of fingers, toes, and nose in a 77-year-old woman with atypical pneumonia and acute renal failure. The diagnosis of haemolytic anaemia with cold agglutinin syndrome was done and the treatment with antibiotics and corticosteroids was effective. However, amputation of all the toes on both feet could not be avoided. Clinical aspects of cold agglutinemia, transient or chronic, are briefly discussed. We conclude that, in the presence of cold agglutinin syndrome, the treatment should be promptly initiated in order to avoid complications such as extensive gangrene.

Fungal infections caused by Candida species have increased in incidence during the past two decades in England, North America and Europe. Candidal arthritis is rare in patients who are not intravenous drug users or are who not using a prostheses. We report the case of a 24-year-old man with acute lymphoid leukemia, who developed Candida tropicalis arthritis during an aplastic period after chemotherapy. This is the eighth case described in the literature of C. tropicalis causing arthritis without intra-articular inoculation. We call attention to an unusual first sign of fungal infection: septic arthritis without intra-articular inoculation. However, this case differs from the other seven, since despite therapy a fast and lethal evolution was observed. We reviewed reported cases, incidence, risk factors, mortality and treatment of neutropenic patients with fungal infections.
