RESUMO Introdução: os expansores teciduais são de importante relevância na cirurgia plástica, e dentre suas indicações destaca-se seu uso no tratamento de sequelas de queimaduras. Ainda que apresente bons resultados, seu uso requer cuidados especiais pois a incidência de complicações não é desprezível. Objetivo: relatar a experiência do Hospital das Clínicas da FMUSP com a utilização de expansores teciduais para tratamento de sequelas de queimaduras, estabelecendo a incidência de complicações e identificando possíveis fatores de risco para ocorrência das mesmas. Método: estudo retrospectivo, observacional e analítico, avaliando o uso de expansores no tratamento de sequelas de queimaduras no período de 2009 a 2018 no Hospital das Clínicas da FMUSP. Resultados: no período de análise estabelecido, foram colocados 245 expansores, sendo 215 em pacientes do sexo feminino, com idade média de 19,96 anos, sendo 40% em tronco e 20% em couro cabeludo, predominando uso do retangular em 76,7% dos casos. As complicações foram divididas em maiores e menores. As complicações ocorreram em 17,95% dos casos, sendo extrusão e infecção as de maior incidência. Houve maior incidência de complicação em expansores utilizados nos membros superiores e inferiores bem como naqueles que não realizaram expansão concomitante (p<0,05), havendo ainda maior chance de complicações absolutas nos pacientes submetidos à expansão adicional. Observou-se também diminuição no número de complicações entre 2009 e 2018. Conclusão: a taxa de complicação (17,95%) é semelhante a da literatura, observando-se maior taxa de complicação nos expansores colocados em membros e maior taxa de complicações absolutas quando realizada expansão adicional.

ABSTRACT Background: tissue expanders have high relevance in plastic surgery and among indications it is worth mentioning their use in the treatment of burn reconstruction. Although it shows good results, its use requires special care because some complications can interrupt the reconstruction process. The objective of this study was to report the experience of the Clinics Hospital (University of Sao Paulo) with the use of tissue expanders to treat burn sequelae, establishing the incidence of complications, and identifying risk factors for their occurrence. Methods: a retrospective, observational, and analytical study, evaluating the use of expanders in burns sequelae treatment from 2009 to 2018. Results: 245 expanders were placed in 84 patients, 215 were female, with a mean age of 19.96 years, being 40% in the trunk and 20% in the scalp, with a predominance of rectangular shape in 76.7% of cases. Complications were classified as major and minor.Complications occurred in 17.95% of cases, and extrusion and infection were the most common. There was a higher incidence of complications in expanders used in the upper and lower limbs as well as in those who did not undergo concomitant expansion (p <0.05), with an even higher chance of major complications in patients submitted to additional expansion. From 2009 to 2018, we observed a decrease in the incidence of complications. Conclusion: the complication rate (17.95%) is similar to other studies of the literature, there was a higher rate of complication with expanders placed in the limbs and a higher rate of major complications when additional expansion was done.

Abstract Background and objective We define burn sequelae as any abnormal body condition related to or resulting from a burn, functional when lead to any limitation of function and non-functional when the sequel still exists but there is no limitation of function, formerly called aesthetic sequelae. The present study proposes a systematization of reasoning in the treatment of burn sequelae in order to facilitate and establish a standardization of care and of the surgical schedule of patients. Methods We conduct a study in the burn sequelae outpatient clinic of the Hospital das Clinicas at the Faculty of Medicine of the University of São Paulo (Brazil), treating 640 patients and generating a systematization of care for patients with burn sequelae. Results Due to the difficulty in treating burn sequelae, we developed an algorithm where the teaching of concepts and logical reasoning became easy. The standardization of procedures made the treatment of burn sequelae reproducible, easy to understand by plastic surgeons and even improving the results. In this way, we believe that academic training becomes easier and more effective. Conclusions Standardized medical reasoning and algorithms are a powerful weapon and simple and effective auxiliary methods to obtain consistent results in the daily practice of medicine.

Resumen Introdução e objectivo Definimos sequela de queimaduras como qualquer condição corporal anormal relacionada ou decorrente de uma queimadura, funcional as que levam a qualquer limitação da função e não-funcional quando ainda existe a sequela, porém não existe limitação da função, antigamente chamada de sequela estética. O presente estudo propõe uma sistematização de raciocínio no tratamento de sequelas de queimaduras de maneira para facilitar e estabelecer uma padronização do atendimento e da programação cirúrgica dos pacientes. Material e métodos Estudo realizado no ambulatório de sequelas de queimaduras do hospital das clínicas da Faculdade de Medicina da Universidade de São Paulo (Brasil). Foram tratados 640 pacientes e geraram esta sistematização do atendimento de pacientes com sequelas de queimaduras. Resultados Em razão da dificuldade no tratamento das sequelas de queimaduras desenvolvemos um algoritmo onde se tornasse fácil o ensino dos conceitos e do raciocínio lógico. A padronização dos procedimentos e das condutas tornou o atendimento de sequelas de queimaduras reprodutível, de fácil compreensão por parte dos cirurgiões plásticos e ainda melhoraram os resultados. Desta maneira, acreditamos que a formação acadêmica se torna mais fácil e efetiva. Conclusões O raciocínio médico padronizado é os algoritmos são uma arma poderosa e métodos auxiliares simples e eficazes na obtenção de resultados consistentes na prática diária da medicina.

RESUMO Objetivo Verificar a correlação entre duas escalas para avaliação das cicatrizes pós-queimaduras com as alterações miofuncionais orofaciais em pacientes queimados. Método Participaram do estudo 16 adultos com sequelas de queimaduras de terceiro grau em cabeça e pescoço. As etapas de coleta de dados envolveram: aplicação das escalas de avaliação da cicatrização Patient and Observer Scar Assessment Scales (POSAS) e Vancouver Scar Scale, aplicação da Avaliação Miofuncional Orofacial com Escores Expandidos (AMIOFE-E) e avaliação da mobilidade mandibular. Resultados Os resultados indicaram correlação negativa moderada entre os itens de deglutição, respiração, escore total de funções e escore total na AMIOFE-E e as escalas de cicatriz, indicando que, quanto mais grave a pontuação nessas escalas, menor a pontuação nos itens do AMIOFE-E (indicativo de maior alteração). Não foram observadas correlações entre os itens da avaliação clínica da motricidade orofacial e a escala de gravidade da cicatriz preenchida pelos pacientes. Conclusão Os resultados do presente estudo sugerem que existe uma correlação entre a gravidade da cicatriz de pacientes queimados, medida por meio de escalas médicas, e as alterações miofuncionais orofaciais. Pacientes que apresentarem pontuação indicativa de cicatrizes patológicas em região de cabeça e pescoço devem ser imediatamente encaminhados para avaliação miofuncional orofacial.

ABSTRACT Purpose Verify the correlation between two scar assessment scales and the presence of orofacial myofunctional disorders (OMD) in patients with head and neck (H&N) burns. Methods Participants of this study were 16 adult individuals with H&N full-thickness burns. Data were collected through assessment of mandibular range of movement and application of the following instruments: Patient and Observer Scar Assessment Scale (POSAS), Vancouver Scar Scale, and Orofacial Myofunctional Evaluation with Scores (OMES). Results Results showed moderate negative correlation between the variables deglutition, breathing, total score of the functions, total score on the OMES and scores on the scar assessment scales, indicating that the higher (more severe) the scores on these scales, the lower the scores on the items of the OMES (indicative of greater OMD severity). No correlations were observed between the items of the OMES and the POSAS Patient scale. Conclusion Results suggest that there is correlation between scar severity in burn patients, measured through clinical scales, and presence of OMD. Patients who present scores indicative of H&N pathological scars should be immediately referred to orofacial myofunctional assessment.

OBJECTIVES: The purpose of this study was to analyze the characteristics of oral-motor movements and facial mimic in patients with head and neck burns. METHODS: An observational descriptive cross-sectional study was conducted with patients who suffered burns to the head and neck and who were referred to the Division of Orofacial Myology of a public hospital for assessment and rehabilitation. Only patients presenting deep partial-thickness and full-thickness burns to areas of the face and neck were included in the study. Patients underwent clinical assessment that involved an oral-motor evaluation, mandibular range of movement assessment, and facial mimic assessment. Patients were divided into two groups: G1 - patients with deep partial-thickness burns; G2 - patients with full-thickness burns. RESULTS: Our final study sample comprised 40 patients: G1 with 19 individuals and G2 with 21 individuals. The overall scores obtained in the clinical assessment of oral-motor organs indicated that patients with both second- and third-degree burns presented deficits related to posture, position and mobility of the oral-motor organs. Considering facial mimic, groups significantly differed when performing voluntary facial movements. Patients also presented limited maximal incisor opening. Deficits were greater for individuals in G2 in all assessments. CONCLUSION: Patients with head and neck burns present significant deficits related to posture, position and mobility of the oral myofunctional structures, including facial movements.

ABSTRACT Introduction: Reconstruction of the nipple-areola complex is the final stage of breast reconstruction. The most common nipple reconstruction techniques use local flaps or grafts. However, these techniques in cases of burns produce undesirable outcomes due to the decreased vascularization of damaged skin. The objective of this work was to evaluate the use of the autonomized star flap in the nipple reconstruction of burned breasts. Methods: Nipples were reconstructed in eight female patients in two surgeries each. Results: There were no complications such as necrosis, dehiscence, complete loss of projection of the new nipple, or infection. The mean projection at the end of surgery was 15.25 mm; 6 months after reconstruction, it was 3 mm, showing stability. All patients were satisfied with their results. Conclusion: We conclude that autonomization leads to safe reconstruction of the nipple on burned breasts and maintains a satisfactory minimal projection of the reconstructed nipple. RESUMO Introdução: A reconstrução do complexo areolomamilar é a fase final da reconstrução mamária. As técnicas de reconstrução do mamilo mais utilizadas são com retalhos locais ou enxertos. A utilização destas técnicas em mamas que sofreram queimaduras apresenta resultados indesejados, em decorrência da menor vascularização da pele lesada. O objetivo deste trabalho foi avaliar a utilização do retalho trilobado autonomizado na reconstrução do mamilo em mamas queimadas. Métodos: Oito pacientes do sexo feminino tiveram seus mamilos reconstruídas em dois tempos cirúrgicos. Resultados: Não ocorreram complicações, como necrose, deiscência, perda completa da projeção do novo mamilo ou infecção. A projeção média ao término da cirurgia foi de 15,25 mm e, após seis meses de reconstrução, foi de 3 mm, permanecendo estável. Todas as pacientes ficaram satisfeitas com os resultados. Conclusão: Concluímos que a autonomização deu segurança à reconstrução do mamilo em mamas queimadas e manteve projeção mínima satisfatória do mamilo reconstruído. INTRODUÇÃO: O aperfeiçoamento no tratamento inicial do paciente queimado, por meio da reposição volêmica e, principalmente, com excisão e enxertia precoce das lesões, resultou em profundo impacto na evolução dos indivíduos queimados, ocorrendo aumento da taxa de sobrevida. Com a maior sobrevida desses pacientes surgiu um novo desafio para a cirurgia reparadora, o tratamento das sequelas de queimaduras, principalmente compostas pelas contraturas. O objetivo deste estudo é demonstrar o uso da matriz dérmica artificial associado a terapia por pressão negativa no tratamento de sequelas de queimaduras. MÉTODO: Foram selecionados 10 pacientes com contratura por queimadura. Os pacientes selecionados foram submetidos a liberação da contratura cicatricial e colocação de matriz dérmica artificial (Integra®), conforme técnica padrão, e curativo por pressão negativa para cobertura. A cada 5 dias, foram realizadas trocas do curativo até completar período de 3 semanas a 4 semanas. Após esse período, o leito foi submetido a enxertia de pele. Os pacientes foram questionados, no pré e pós-operatório, quanto a sua satisfação com os aspectos estético e funcional da região abordada. RESULTADOS: Houve integração de cerca de 98% da matriz de regeneração dérmica na área em que a contratura foi ressecada. Também ocorreu integração de aproximadamente 85% dos enxertos utilizados. Todos os pacientes abordados referiram significativa melhora estética e, principalmente, funcional da região abordada. CONCLUSÕES: O uso da matriz de regeneração dérmica associado a terapia por pressão negativa promove maior taxa de sucesso na abordagem da contratura cicatricial, proporcionando melhor resultado tanto funcional como estético nos pacientes com sequelas graves de queimadura.

BACKGROUND: Burn patients are initially treated with volume replacement and, importantly, subsequent excision and early grafting of their injuries. These first aid measures improve the progress and survival rates of burned individuals. However, this treatment creates a new challenge in reparative surgery with regard to the management of burn sequelae, which mainly include contractures. In this study, we aimed to assess the effect of artificial dermal matrix application and negative pressure therapy on the treatment of burn sequelae. METHODS: Ten patients with burn contractures were selected for this study and underwent scar contracture release, wound coverage with an artificial dermal matrix (Integra®), and negative pressure dressing according to the standard procedures. Dressing changes were performed every 5 days for 3-4 weeks. A skin graft was then laid on the wound bed. In the pre- and postoperative period, the patients were asked to rate their satisfaction with the aesthetic and functional aspects of the treated region. RESULTS: In the areas in which the contractures were resected, integration of almost 98% of the dermal regeneration matrix was observed. Also integration was observed in 85% of the grafts used. All the patients reported a significant aesthetic and, in particular, functional improvement of the treated region. CONCLUSIONS: The use of dermal regeneration matrix and negative pressure therapy for the treatment of scar contractures improves the success rate and the aesthetic and functional outcomes of patients with severe burn sequelae.
