Introduction: Surgical procedures were systematized and scientifically based at the end of the 19th century by William Stewart Halsted. Through scientific studies, anatomical principles, knowledge of physiology and pathology, strict aseptic techniques, gentle tissue manipulation, among others, were systematized. This lasted for centuries. But the modern technological era invented videosurgery and complicated the previously used teaching method. Objective: Review previously published teaching methods using simulators. Method: This is a narrative review carried out in the PubMed and Scielo databases. The search used the following keywords: videosurgery, teaching, simulators, training. The inclusion criteria comprised review articles and experimental studies, written in Portuguese and English and available in full text. The selected articles were analyzed based on the technologies covered, future perspectives and challenges mentioned. Result: 28 articles were included. Conclusion: As the results of this review on simulators show, once trained residents begin learning videolaparoscopic operations much more easily and safely.

Introdução: Os procedimentos cirúrgicos foram sistematizados e embasados cientificamente no final do século XIX por William Stewart Halsted. Por meio de estudos científicos, princípios anatômicos, conhecimento de fisiologia e patologia que permitiram sistematizar técnica asséptica estrita, manipulação tecidual gentil, dentro de outros. Isto perdurou por séculos. Mas a moderna era tecnológica inventou a videocirurgia e complicou o método de ensino anteriormente usado. Objetivo: Revisar os métodos de ensino feito por simuladores já publicados. Método: Trata-se de revisão narrativa realizada nas bases de dados PubMed e Scielo. A busca utilizou as seguintes palavras-chave: videocirurgia, ensino, simuladores, treinamento. Os critérios de inclusão compreenderam artigos de revisão e estudos experimentais, escritos em português e inglês e disponíveis em texto completo. Os trabalhos selecionados foram analisados com base nas tecnologias abordadas, perspectivas futuras e desafios mencionados. Resultado: Foram incluídos 28 artigos. Conclusão: Conforme mostram os resultados desta revisão sobre simuladores, os residentes depois de treinados, iniciariam o aprendizado de operações videolaparoscópicas com muito mais facilidade e segurança.

Introduction: Colorectal cancer is among the most common malignant neoplasms worldwide and the pre-malignant lesions that lead to its appearance are polyps in their various types. As a minimally invasive method, endoscopic resection emerges as the preferred method for focused lesions. Objective: To carry out a review verifying whether endoscopic therapy is safe and efficient in the treatment of lesions located in the rectum. Method: Narrative review collecting information published on virtual platforms (SciELO, Google Scholar, Pubmed and Scopus). Initially, a search was carried out for descriptors related to the topic, which were identified through DeCS/MeSH, namely: “colorectal cancer, adenoma, mucosectomy, dysplasia, polyp” with AND or OR search, considering the title and/or abstract. Afterwards, only those that were most similar were chosen, and the works were read in full. Results: 41 articles were evaluated. Conclusion: With the evolution of concepts and technological improvements, there is an increasing possibility of diagnosing non-polypoid or superficial lesions (superficially elevated, flat or depressed) and laterally spreading or laterally growing lesions or tumors (LST). ) that, by definition, have a diameter greater than 10 mm. Mucosectomy may be indicated for minimally invasive treatment or prevention in cases that have not yet advanced and can be cured endoscopically.

Introdução: O câncer colorretal está entre as neoplasias malignas mais comuns em todo mundo e as lesões pré-malignas que levam ao seu surgimento são os pólipos em seus variados tipos. Como forma minimamente invasiva a ressecção endoscópica desponta como método preferencial nas lesões focadas. Objetivo: Efetuar revisão verificando se a terapêutica endoscópica é segura e eficiente no tratamento de lesões localizadas no reto. Método: Revisão narrativa colhendo informações publicadas em plataformas virtuais (SciELO, Google Scholar, Pubmed e Scopus). Inicialmente foi realizada busca por descritores relacionados ao tema, os quais foram identificados por meio do DeCS/MeSH, a saber: “câncer colorretal, adenoma, mucosectomia, displasia, pólipo” e seus equivalentes em inglês “colorectal cancer, adenoma, mucosectomy, dysplasia, polyp” com busca AND ou OR, considerando o título e/ou resumo. Após, foram escolhidos somente os que tinham maior similitude, e realizada a leitura na íntegra dos trabalhos. Resultados: Foram avaliados 41 artigos. Conclusão: Com a evolução dos conceitos e com a melhora tecnológica, há possibilidade de diagnosticar cada vez maior lesões não-polipoides ou superficiais (superficialmente elevadas, planas ou deprimidas) e as lesões ou tumores de espraiamento ou crescimento lateral (LST, Laterally Spreading Tumor) que por definição apresentem diâmetro maior que 10 mm. A mucosectomia pode ser indicada para o tratamento minimanente invasivo ou prevenção nos casos ainda não avançados e que possam ser curados endoscopicamente.

Introduction: Serrated lesions are considered precursors of colorectal adenocarcinomas. Studying them in their initial phases is important when thinking about cancer prevention, when they are hyperplastic lesions, or sessile serrated lesions. Objective: To carry out an integrative review of the literature to compare the endoscopic and histopathological characteristics in the initial phases of the disease, with the intention to help understand colorectal cancer. Methods: Integrative literature review based on existing analysis material on the digital platforms SciELO – Scientific Electronic Library Online, Google Scholar, Pubmed and Scopus. The beginning was the search with the following descriptors: “serrated sessile lesions, colorectal carcinoma, superficial lesions”, with AND or OR search, considering the title and/or abstract , and subsequently including only those with the highest correlation for reading the full texts. Results: 39 articles were included. Conclusion: The majority of superficially elevated serrated lesions measuring more than 5 mm and resected by colonoscopies are hyperplastic. They occur throughout the colon and rectum, while the serrated sessiles are preferentially located in the proximal colon. Hyperplastic ones generally do not present dysplasias and sessile serrated ones can have them intensely.

Introdução: Lesões serrilhadas são consideradas precursoras dos adenocarcinomas colorretais. Estudá-las em suas fases iniciais é importante ao pensar na prevenção do câncer, quando, então, são lesões hiperplásicas, ou sésseis serrilhadas Objetivo: Realizar revisão integrativa da literatura para comparar as características endoscópicas e histopatológicas nas fases iniciais da doença, com a intenção de auxiliar no entendimento do câncer colorretal. Métodos: Revisão integrativa da literatura baseada em material de análise existente nas plataformas digitais SciELO – Scientific Electronic Library Online, Google Scholar, Pubmed e Scopus.  O início foi a busca com os seguintes descritores: “lesões sésseis serrilhadas, carcinoma colorretal, lesões superficiais”, e seus equivalentes em inglês “serrated sessile lesions, colorectal carcinoma, superficial lesions” com busca AND ou OR, considerando o título e/ou resumo, e incluindo-se posteriormente somente  aqueles com maior correlação para leitura dos textos completos. Resultados:  Incluíram-se 39 artigos. Conclusão: A maioria das lesões serrilhadas superficialmente elevadas com mais de 5 mm e ressecadas por colonoscopias, são hiperplásicas. Elas ocorrem por todo o cólon e reto, enquanto as sésseis serrilhadas localizam-se preferencialmente no cólon proximal. As hiperplásicas geralmente não apresentam displasias e as sésseis serrilhadas podem tê-las de forma intensa.

Introduction: Papillary carcinomas are the most prevalent and least aggressive thyroid carcinomas (PTC). In some cases, the diagnosis is doubtfull and the prognosis is poor. The search for tissue biomarkers that ensure both the diagnosis for indeterminate cases and the prognosis, identifying the most aggressive cases, has been studied in recent decades. Objective: To review the literature in search of cyclin D1 as a marker of papillary thyroid carcinomas and multinodular goiters and evaluate whether its expression correlates with the clinicopathological characteristics of papillary thyroid carcinomas. Methods: Narrative review carried out by collecting information for reading and analysis from online research on virtual platforms. Initially, a search was carried out from MESH descriptors related to the topic, using the following terms: “papillary thyroid carcinoma, cyclin D1, immunohistochemistry, diagnosis, prognosis.” with AND or OR search, considering the title and/or abstract and those chosen were read in full. Results: The search included 77 articles that were compiled in this review. Conclusion: Cyclin D1 was expressed in the vast majority of PTCs, with diffuse distribution being predominant. There was no correlation between its expression and any clinicopathological characteristic of PTC.

Racional - Os carcinomas papilíferos são os mais prevalentes e menos agressivos de tireoide (CPT). Em alguns casos, o diagnóstico é duvidoso e o prognóstico ruim. A busca de biomarcadores teciduais que permitam assegurar tanto o diagnóstico para casos indeterminados, quanto o prognóstico, identificando os casos de maior agressividade, têm sido estudadas nas últimas décadas. Objetivo: Revisar na literatura na busca da ciclina D1 como marcador dos carcinomas papilíferos de tireoide e nos bócios multinodulares, e avaliar se a expressão dela apresenta correlação com as características clínico-patológicas dos carcinomas papilíferos de tireoide. Métodos: Revisão narrativa feita colhendo informações para leitura e análise a partir de pesquisa online em platoformas virtuais. Inicialmente foi realizada busca por descritores DECs relacionados ao tema, utilizando os seguintes termos: “carcinoma papilífero de tireoide, ciclina D1, imunoistoquímica, diagnóstico, prognóstico.” com busca AND ou OR, considerando o título e/ou resumo e os escolhidos foram lidos na íntegra. Resultados: A busca incluiu 77 artigos que foram compilados nesta revisão.  Conclusão: A ciclina D1 foi expressa na grande maioria dos CPT sendo a distribuição difusa predominante. Não houve correlação entre a expressão dela com qualquer característica clinicopatológica dos CPT.

Introduction: Celiac disease is an autoimmune condition triggered by the ingestion of and contact with gluten in genetically predisposed individuals. Dyspeptic syndrome, on the other hand, is characterized by chronic upper digestive symptoms. The relationship between these two conditions is the main focus of this review. Objective: To investigate the prevalence of celiac disease in patients with dyspeptic syndrome based on the analysis of epidemiological characteristics, upper digestive endoscopy, duodenal histology and serology. Method: Narrative review carried out with information published on virtual platforms in Portuguese and English and analyzed during the period from January 2022 to August 2023. The material for reading and analysis was selected from the SciELO platforms – Scientific Electronic Library Online, Google Scholar, Pubmed and Scopus using the following terms: “dyspepsia, celiac disease, gluten, prevalence” with AND or OR search, considering the title and/or abstract. Afterwards, the full texts were read, including 18 articles. Results: In dyspeptic patients, the average age was 45.13 years and the female gender was predominant. Symptoms associated with gluten were reported in 6%. The antitransglutaminase antibody was positive, with an estimated prevalence of 1.5%. Considering the Brazilian sample of 100 patients, the diagnosis of celiac disease was made with a prevalence of 3%. Conclusion: Although the prevalence of celiac disease in dyspeptic patients may be higher than in the general population, results are variable and depend on several factors, including testing methodologies and regional characteristics. This review also highlights the importance of an individualized approach in the investigation of celiac disease in dyspeptic patients, considering aspects such as family history, gluten-related symptoms and autoimmune comorbidities.

Introdução: A doença celíaca é uma condição autoimune desencadeada pela ingestão e/ou contato com o glúten em indivíduos geneticamente predispostos. A síndrome dispéptica, por outro lado, é caracterizada por sintomas digestivos superiores crônicos. A relação entre estas duas condições é o foco principal deste estudo. Objetivo: Investigar a prevalência da doença celíaca em pacientes com síndrome dispéptica com base da análise de características epidemiológicas, endoscopia digestiva alta, histologia duodenal e sorologia. Método: Revisão narrativa feita com informações publicadas em plataformas virtuais em português e inglês, e analisada durante o período de janeiro de 2022 a novembro de 2023. O material para leitura e análise foi selecionado das plataformas SciELO – Scientific Electronic Library Online, Google Scholar, Pubmed e Scopus utilizando os seguintes termos: “dispepsia, doença celíaca, glúten, prevalência” com busca AND ou OR, considerando o título e/ou resumo. Após, foi realizada a leitura da íntegra dos textos incluindo 18 artigos. Resultados: A idade média dos pacientes com dispepsia foi de 45,13 anos e o sexo feminino foi predominante. Os sintomas associados ao glúten foram relatados em 6%. O anticorpo antitransglutaminase foi positivo com prevalência estimada de 1,5%.  Considerando a amostra brasileira de 100 pacientes, o diagnóstico de doença celíaca foi prevalente em 3%. Conclusão: Embora a prevalência de doença celíaca em pacientes dispépticos possa ser maior do que na população em geral, os resultados são variáveis e dependem de vários fatores, incluindo metodologias de teste e características regionais. Esta revisão também ressalta a importância de abordagem individualizada na investigação da doença celíaca em pacientes dispépticos, considerando aspectos como história familiar, sintomas relacionados ao glúten e comorbidades autoimunes.

Introduction: Improved endoscopic imaging allows the differential diagnosis of colorectal lesions in real time through the study of the microvasculature. Objective: To review the use of Blue Laser Imaging (BLI) in analyzing the capillary patern for the differential diagnosis between neoplastic and non-neoplastic lesions. Method: This is an integrative review of the literature that collected information published on virtual platforms in Portuguese and English. The search for reading and analysis was carried out on the SciELO – Scientific Electronic Library Online, Google Scholar, Pubmed and Scopus platforms. The descriptors taken from DeCS/MESH were: “Blue laser imaging. Endoscopy. Chromoendoscopy, Colorectal polyps. Magnification” and its English counterparts “Blue laser imaging. Image-enhanced endoscopy. Chromoendoscopy. Colorectal polyps. Magnification” with AND or OR search, considering the title and/or abstract. Only those that had the greatest correlation to the topic were included, and the review was based on them, with reading, in full, of the texts. Results: In the end, 22 articles resulted that made up this review. Conclusion: BLI associated with magnification showed good results in real-time predictive histological diagnosis for colon and rectal lesions.

Introdução: Imagem endoscópica aprimorada permite o diagnóstico diferencial das lesões colorretais em tempo real através do estudo da microvasculatura. Objetivo: Revisar o uso do Blue Laser Imaging (BLI) na análise do padrão dos capilares para o diagnóstico diferencial entre lesões neoplásicas e não neoplásicas. Método: Esta é revisão integrativa da literatura  que colheu informações publicadas em plataformas virtuais em português e inglês. A busca para leitura e análise foi realizada nas plataformas SciELO – Scientific Electronic Library Online, Google Scholar, Pubmed e Scopus.  Os descritores retirados do DeCS/MESH foram: “Blue laser imaging. Endoscopia. Cromoendoscopia, Pólipos colorretais. Magnificação” e seus correspondentes em inglês “Blue laser imaging. Image-enhanced endoscopy. Chromoendoscopy. Colorectal polyps. Magnification” com busca AND ou OR, considerando o título e/ou resumo. Foram incluídos somente os que tivessem maior correlação ao tema, e neles foi baseada a revisão com leitura, na íntegra, dos textos. Resultados: Ao final resultaram  22 artigos que compuseram esta revisão. Conclusão: BLI associada à magnificação mostrou bons resultados no diagnóstico histológico preditivo em tempo real para as lesões de cólon e reto.

Introduction: The Charlson comorbidities index (CCI) assesses a person's chances of survival over the next 10 years. In systemic lupus erythematosus (SLE), multiple comorbidities and complications affect patient survival. Objetive: Analize the variables that influence the CCI of a group of females with SLE. Methods: Retrospective study of medical records of 100 lupus patients for CCI, clinical, epidemiological and serological variables. Results: No epidemiological variable interfered in CCI. Regarding clinical manifestations, patients with glomerulonephritis had a worse CCI than those without (p<0.0001) and those with central nervous system manifestations had a tendency to worse CCI (p=0.09). Patients with anti-Ro antibodies (p=0.02) and rheumatoid factor or RF (p=0.002) were associated with a lower CCI. Conclusions: The presence of glomerulonephritis is associated with lower survival and of the anti-Ro and RF antibodies with longer survival in SLE.

Introdução : O índice de comorbidades de Charlson (ICC) avalia as chances de sobrevivência de uma pessoa nos próximos 10 anos. No lúpus eritematoso sistêmico (LES) múltiplas comorbidades e complicações afetam a sobrevida. Objetivo : Verificar as variáveis ​​que influenciam no ICC de um grupo de mulheres com LES. Métodos : Estudo retrospectivo de 100 pacientes lúpicas para o ICC, variáveis ​​clínicas, epidemiológicas e sorológicas. Resultados : Nenhuma variável epidemiológica interferiu no ICC. Quanto à clínica, pacientes com glomerulonefrite tiveram pior ICC do que os sem (p<0,0001) e os com manifestações de sistema nervoso central tiveram tendência para pior ICC (p=0,09). Portadores de anticorpos anti-Ro (p=0,02) e fator reumatoide (FR; p=0,002) se associaram com ICC menor. Conclusões : A presença de glomerulonefrite se associa com menor sobrevida, e a dos anticorpos anti-Ro e FR com maior sobrevida no LES.

ABSTRACT BACKGROUND: Lipomatous pseudohypertrophy of the pancreas, pancreatic lipomatosis, pancreatic steatosis, non-alcoholic fatty pancreatic disease, or fatty pancreas is an extremely rare disease, characterized by the organ enlargement and a localized or diffuse replacement of pancreatic acinar cells by mature adipose tissue, preserving the pancreatic ductal system and islets of Langerhans. AIMS: To report a rare case of lipomatous pseudohypertrophy of the pancreas in a symptomatic patient and the surgical treatment employed. METHODS: A 24-year-old male patient with weight loss (10 kilograms in 8 months), hyperglycemia, severe and recurrent acute abdominal pain, epigastric discomfort associated with nausea, vomiting, and jaundice for 40 days. Magnetic resonance imaging was performed, revealing an irregular lipomatous pseudohypertrophy of the pancreas, measuring 6.0 × 5.6 cm in the head, uncinate process, and part of the body of the pancreas. The pancreatic duct dilation was diffuse and irregular, associated with atrophy of the remnant parenchyma, particularly in the tail of the pancreas. The patient underwent pancreatoduodenectomy without total mesopancreas excision followed by pancreatojejunostomy. RESULTS: The postoperative course was uneventful, the length of stay in the ICU was two days, and the patient was discharged on the seventh postoperative day. CONCLUSIONS: The disease treatment depends on the signs and symptoms at presentation and a pancreatoduodenectomy is indicated in patients with severe and recurrent abdominal pain.

RESUMO RACIONAL: Pseudo-hipertrofia lipomatosa do pâncreas, lipomatose pancreática, esteatose pancreática, doença pancreática gordurosa não alcoólica ou pâncreas gorduroso é uma doença extremamente rara, caracterizada por aumento do órgão e substituição localizada ou difusa de células acinares pancreáticas por tecido adiposo, preservando o sistema ductal pancreático e as ilhotas de Langerhans. OBJETIVOS: Relatar um caso raro de pseudo-hipertrofia lipomatosa do pâncreas, em paciente sintomático e o tratamento cirúrgico empregado. MÉTODOS: Paciente do sexo masculino, 24 anos, com queixas de emagrecimento (10 quilos nos últimos 8 meses), hiperglicemia, dor abdominal aguda intensa e recorrente, desconforto epigástrico associado a náuseas, vômitos e icterícia há 40 dias. A ressonância magnética (RM) revelou pseudo-hipertrofia lipomatosa irregular do pâncreas, medindo 6,0 × 5,6 cm na cabeça, processo uncinado e parte do corpo do pâncreas. A dilatação do ducto pancreático era difusa e irregular, associada à atrofia do parênquima remanescente, principalmente na cauda do pâncreas. O paciente foi submetido à duodenopancreatectomia sem excisão total do mesopâncreas seguida de pancreatojejunostomia. RESULTADOS: O pós-operatório transcorreu sem intercorrências, o tempo de internação na UTI foi de 2 dias, com alta hospitalar no sétimo dia. CONCLUSÕES: O tratamento da doença depende dos sinais e sintomas na apresentação e a duodenopancreatectomia é indicada em pacientes com dor abdominal intensa e recorrente.

ABSTRACT BACKGROUND: Complete surgical resection is the treatment of choice for patients with liver metastases, but in some patients, it is not possible to obtain a complete R0 resection. Moreover, the recurrence rate is up to 75% after three years. After the experience of the Oslo group with cadaveric liver transplant, some centers are starting their experience with liver transplant for colorectal liver metastasis. AIMS: To present our initial experience with living donor liver transplant for colorectal liver metastasis. METHODS: From 2019 to 2022, four liver transplants were performed in patients with colorectal liver metastases according to the Oslo criteria. RESULTS: Four patients underwent living donor liver transplants, male/female ratio was 3:1, mean age 52.5 (42–68 years). All patients were included in Oslo criteria for liver transplant. Two patients had already been submitted to liver resection. The decision for liver transplant occurred after discussion with a multidisciplinary team. Three patients recurred after the procedure and the patient number 3 died after chemotherapy. CONCLUSIONS: Living donor liver transplant is a viable treatment option for colorectal liver metastasis in Brazil, due to a shortage of donors.

RESUMO RACIONAL: A ressecção cirúrgica completa é o tratamento de escolha para pacientes com metástases hepáticas, mas em alguns pacientes não é possível obter uma completa ressecção R0. Além disso, a taxa de recorrência é de até 75% após 3 anos. Após a experiência do grupo de Oslo com transplante hepático cadavérico, alguns centros estão iniciando sua experiência com transplante hepático para metástase hepática colorretal. OBJETIVOS: Apresentar a experiência inicial com transplante de fígado de doador vivo para metástase hepática colorretal. MÉTODOS: De 2019 a 2022, foram quatro transplantes hepáticos em pacientes com metástases hepáticas colorretais, de acordo com os critérios de Oslo. RESULTADOS: Quatro pacientes foram submetidos a transplante hepático de doador vivo, a relação homem/mulher de 3:1, a idade média foi de 52,5 (42-68 anos). Todos os pacientes foram incluídos nos critérios de Oslo para transplante de fígado. Dois pacientes já haviam sido submetidos à ressecção hepática. A decisão pelo transplante hepático ocorreu após discussão com equipe multidisciplinar. Três pacientes recidivaram após o procedimento e o paciente número 3 morreu após a quimioterapia. CONCLUSÕES: O transplante de fígado com doador vivo é uma opção viável de tratamento para metástase hepática colorretal no Brasil, devido à escassez de doadores.

RESUMO A colecistite aguda (CA) é um processo inflamatório agudo da vesícula biliar que pode estar associado a complicações potencialmente graves, como empiema, gangrena, perfuração da vesícula biliar e sepse. O tratamento padrão para a CA é a colecistectomia laparoscópica. No entanto, para um pequeno grupo de pacientes com CA, o risco de colecistectomia laparoscópica pode ser muito alto, principalmente em idosos com doenças graves associadas. Nestes pacientes críticos, a colecistectomia percutânea ou a drenagem endoscópica da vesícula biliar guiada por ultrassom podem ser uma opção terapêutica temporária, como ponte para a colecistectomia. O objetivo deste artigo de posicionamento do Colégio Brasileiro de Cirurgia Digestiva é apresentar novos avanços no tratamento da CA em pacientes cirúrgicos de alto risco, para auxiliar cirurgiões, endoscopistas e clínicos a selecionar o melhor tratamento para os seus pacientes. A eficácia, segurança, vantagens, desvantagens e resultados de cada procedimento são discutidos. As principais conclusões são: a) Pacientes com CA e risco cirúrgico elevado devem ser tratados preferencialmente em hospitais terciários onde a experiência e os recursos cirúrgicos, radiológicos e endoscópicos estão disponíveis. b) A modalidade de tratamento ideal para pacientes com elevado risco cirúrgico, deve ser individualizada, com base nas condições clínicas e na experiência disponível. c) A colecistectomia laparoscópica continua sendo uma excelente opção de tratamento, principalmente em hospitais em que a drenagem da vesícula biliar percutânea ou endoscópica não está disponível. d) A colecistostomia percutânea e a drenagem endoscópica da vesícula biliar devem ser realizadas apenas em hospitais bem equipados e com radiologista intervencionista e/ou endoscopista experientes. e) O cateter de colecistostomia deve ser removido após a resolução da CA. No entanto, em pacientes que não têm condição clínica para realizar colecistectomia, o cateter pode ser mantido por um período prolongado ou mesmo definitivamente. f) Se o cateter de colecistostomia for mantido por longo período de tempo podem ocorrer várias complicações, como sangramento, fístula biliar, obstrução, dor no local de inserção, remoção acidental do cateter e CA recorrente. g) O tempo de espera ideal entre a colecistostomia e a colecistectomia ainda não foi estabelecido, e vai desde imediatamente após a melhoria clínica, até meses após. h) Longos períodos de espera entre colecistostomia e colecistectomia podem estar associados a novos episódios de CA, múltiplas readmissões hospitalares e aumento dos custos. ABSTRACT Acute cholecystitis (AC) is an acute inflammatory process of the gallbladder that may be associated with potentially severe complications, such as empyema, gangrene, perforation of the gallbladder, and sepsis. The gold standard treatment for AC is laparoscopic cholecystectomy. However, for a small group of AC patients, the risk of laparoscopic cholecystectomy can be very high, mainly in the elderly with associated severe diseases. In these critically ill patients, percutaneous cholecystostomy or endoscopic ultrasound gallbladder drainage may be a temporary therapeutic option, a bridge to cholecystectomy. The objective of this Brazilian College of Digestive Surgery Position Paper is to present new advances in AC treatment in high-risk surgical patients to help surgeons, endoscopists, and physicians select the best treatment for their patients. The effectiveness, safety, advantages, disadvantages, and outcomes of each procedure are discussed. The main conclusions are: a) AC patients with elevated surgical risk must be preferably treated in tertiary hospitals where surgical, radiological, and endoscopic expertise and resources are available; b) The optimal treatment modality for high-surgical-risk patients should be individualized based on clinical conditions and available expertise; c) Laparoscopic cholecystectomy remains an excellent option of treatment, mainly in hospitals in which percutaneous or endoscopic gallbladder drainage is not available; d) Percutaneous cholecystostomy and endoscopic gallbladder drainage should be performed only in well-equipped hospitals with experienced interventional radiologist and/or endoscopist; e) Cholecystostomy catheter should be removed after resolution of AC. However, in patients who have no clinical condition to undergo cholecystectomy, the catheter may be maintained for a prolonged period or even definitively; f) If the cholecystostomy catheter is maintained for a long period of time several complications may occur, such as bleeding, bile leakage, obstruction, pain at the insertion site, accidental removal of the catheter, and recurrent AC; g) The ideal waiting time between cholecystostomy and cholecystectomy has not yet been established and ranges from immediately after clinical improvement to months. h) Long waiting periods between cholecystostomy and cholecystectomy may be associated with new episodes of acute cholecystitis, multiple hospital readmissions, and increased costs. Finally, when selecting the best treatment option other aspects should also be considered, such as costs, procedures available at the medical center, and the patient’s desire. ABSTRACT Purpose To compare tissue inflammatory response, foreign body reaction, fibroplasia, and proportion of type I/III collagen between closure of abdominal wall aponeurosis using polyglactin suture and intraperitoneal implant of polypropylene, polypropylene/polyglecaprone, and polyester/porcine collagen meshes to repair defects in the abdominal wall of rats. Methods Forty Wistar rats were placed in four groups, ten animals each, for the intraperitoneal implant of polypropylene, polypropylene/polyglecaprone, and polyester/porcine collagen meshes or suture with polyglactin (sham) after creation of defect in the abdominal wall. Twenty-one days later, histological analysis was performed after staining with hematoxylin-eosin and picrosirius red. Results The groups with meshes had a higher inflammation score (p < 0.05) and higher number of gigantocytes (p < 0.05) than the sham group, which had a better fibroplasia with a higher proportion of type I/III collagen than the tissue separating meshes (p < 0.05). There were no statistically significant differences between the three groups with meshes. Conclusions The intraperitoneal implant of polypropylene/polyglecaprone and polyester/porcine collagen meshes determined a more intense tissue inflammatory response with exuberant foreign body reaction, immature fibroplasia and low tissue proportion of type I/III collagen compared to suture with polyglactin of abdominal aponeurosis. However, there were no significant differences in relation to the polypropylene mesh group.

RESUMO Racional: No Brasil, a abordagem baseada em metas foi nomeada de Projeto ACERTO e tem obtido bons resultados quando aplicada em operações eletivas com diminuição do tempo de internação, retorno mais precoce as atividades sem incremento de morbimortalidade. Objetivo: Analisar o impacto do ACERTO na assistência prestada em operações de emergência. Métodos: Foi realizado um estudo de intervenção em hospital de trauma. Foram comparados 452 pacientes submetidos à operações de emergência e acompanhados pelo serviço de cirurgia geral no período de outubro a dezembro de 2018 (fase pré-ACERTO, n=243) e no período de março a junho de 2019 (fase pós-ACERTO, n=209). Foram avaliados: reintrodução da dieta, volume de hidratação venosa pós-operatória infundido, tempo de uso de catéteres, sondas e drenos, analgesia pós-operatória, prevenção de vômitos pós-operatórios, mobilização precoce e fisioterapia. Resultados: Após a implantação do ACERTO houve reintrodução mais precoce da dieta, foi atingido o aporte calórico ideal mais precocemente, retirada mais precoce da hidratação venosa, maior prescrição de analgesia pós-operatória, de profilaxia de vômitos pós-operatórios e maior prescrição de fisioterapia e mobilização precoce em todos (p<0,01); na análise multivariada não houve alteração nas taxas de complicações observadas pré e pós-ACERTO (10,7% vs. 7,7% (p=0,268) e houve diminuição do tempo de internação pós-ACERTO (8,5 vs. 6,1 dias (p=0.008). Conclusão: A implantação do projeto ACERTO diminuiu o tempo de internação hospitalar, melhorou a assistência médica prestada sem incremento das taxas de complicações avaliadas.

ABSTRACT Background: In Brazil, the goal-based approach was named Project ACERTO and has obtained good results when applied in elective surgeries with shorter hospitalization time, earlier return to activities without increased morbidity and mortality. Aim: To analyze the impact of ACERTO on emergency surgery care. Methods: An intervention study was performed at a trauma hospital. Were compared 452 patients undergoing emergency surgery and followed up by the general surgery service from October to December 2018 (pre-ACERTO, n=243) and from March to June 2019 (post-ACERTO, n=209). Dietary reintroduction, volume of infused postoperative venous hydration, duration of use of catheters, probes and drains, postoperative analgesia, prevention of postoperative vomiting, early mobilization and physiotherapy were evaluated. Results: After the ACERTO implantation there was earlier reintroduction of the diet, the earlier optimal caloric intake, earlier venous hydration withdrawal, higher postoperative analgesia prescription, postoperative vomiting prophylaxis and higher physiotherapy and mobilization prescription were achieved early in all (p<0.01); in the multivariate analysis there was no change in the complication rates observed before and after ACERTO (10.7% vs. 7.7% (p=0.268) and there was a decrease in the length of hospitalization after ACERTO (8,5 vs. 6,1 dias (p=0.008). Conclusion: The implementation of the ACERTO project decreased the length of hospital stay, improved medical care provided without increasing the rates of complications evaluated.

RESUMO Racional: Trombose de artéria hepática é importante causa de falência de enxerto e complicações biliares. Fatores de risco para trombose estão relacionados aos aspectos técnicos da anastomose arterial e fatores não cirúrgicos. Objetivo: Avaliar os fatores de risco para o desenvolvimento de trombose de artéria hepática. Métodos: A amostra consta de 1050 casos de transplante hepático. Foi realizado estudo retrospectivo e transversal, e as variáveis foram avaliadas em doadores e receptores. Resultados: A análise univariada mostrou que as variáveis relacionadas a trombose de artéria hepática são: MELD e tempo de isquemia quente. Na análise multivariada, o MELD=14.5 e tempo de isquemia quente =35 min foram fatores de risco independentes para trombose de artéria hepática. No teste de prevalência para avaliação do tipo de anastomose como variável, foi observado que a sutura contínua tem maior risco de trombose quando comparada com aquela em pontos separados. Conclusão: Tempo de isquemia quente prolongado, MELD calculado e idade do recipiente foram fatores de risco independentes para trombose de artéria hepática após transplante de fígado em adultos. Pacientes submetidos à anastomose com sutura contínua apresentaram mais trombose quando comparados com a em pontos separados. Retransplante por trombose está associado com maior mortalidade.

ABSTRACT Background: Hepatic artery thrombosis is an important cause of graft loss and ischemic biliary complications. The risk factors have been related to technical aspects of arterial anastomosis and non-surgical ones. Aim: To evaluate the risk factors for the development of hepatic artery thrombosis. Methods: The sample consisted of 1050 cases of liver transplant. A retrospective and cross-sectional study was carried out, and the variables studied in both donor and recipient. Results: Univariate analysis indicated that the variables related to hepatic artery thrombosis are: MELD (p=0.04) and warm time ischemia (p=0.005). In the multivariate analysis MELD=14.5 and warm ischemia time =35 min were independent risk factors for hepatic artery thrombosis. In the prevalence ratio test for analysis of the anastomosis as a variable, it was observed that patients with continuous suture had an increase in thrombosis when compared to interrupted suture. Conclusions: Prolonged warm ischemia time, calculated MELD and recipient age were independent risk factors for hepatic artery thrombosis after liver transplantation in adults. Transplanted patients with continuous suture had an increase in thrombosis when compared to interrupted suture. Re-transplantation due to hepatic artery thrombosis was associated with higher recipient mortality.
