SUMMARY The aging process occurs due to the decline of vital physiological functions and adaptability of the body, being influenced by genetics and lifestyle. With advances in genetics, biological aging can be calculated by telomere length. Telomeres are regions at the ends of chromosomes that play a role in the maintenance and integrity of DNA. With biological aging, telomere shortening occurs, causing cellular senescence. Several studies show that shorter telomeres are associated with acute and chronic diseases, stress, addictions, and intoxications. Even in the current COVID-19 pandemic, telomere shortening is proposed as a marker of severity in individuals infected by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). On the other hand, healthy lifestyle habits increase telomere length and balance of various cellular functions, preventing diseases.

Resumo O tratamento homeopático está fundamentado no princípio da similitude terapêutica, empregando medicamentos que causam determinados distúrbios para tratar manifestações semelhantes, estimulando uma reação do organismo contra seus próprios transtornos. A ocorrência dessa reação secundária do organismo, de natureza oposta à ação primária dos medicamentos, está evidenciada no estudo do efeito rebote (paradoxal) de inúmeras classes de fármacos modernos. Neste trabalho, além de fundamentar o princípio da similitude perante a farmacologia clínica e experimental, sugerimos uma proposta para empregar centenas de drogas convencionais segundo o método homeopático, aplicando a similitude terapêutica entre os eventos adversos dos medicamentos e as manifestações clínicas dos pacientes. Descrevendo linhas de pesquisa existentes e um método específico para o uso terapêutico do efeito rebote dos fármacos modernos (http://www.novosmedicamentoshomeopaticos.com), esperamos minimizar preconceitos relativos à homeopatia e contribuir para uma ampliação da arte de curar.

Summary The homeopathic treatment is based on the principle of therapeutic similitude, employing medicines that cause certain disorders to treat similar manifestations, stimulating a reaction of the organism against its own ailments. The occurrence of this secondary reaction of the organism, opposite in nature to the primary action of the medicines, is evidenced in the study of the rebound (paradoxical) effect of several classes of modern drugs. In this work, in addition to substantiate the principle of similitude before the experimental and clinical pharmacology, we suggest a proposal to employ hundreds of conventional drugs according to homeopathic method, applying the therapeutic similitude between the adverse events of medicines and the clinical manifestations of patients. Describing existing lines of research and a specific method for the therapeutic use of the rebound effect of modern drugs (http://www.newhomeopathicmedicines.com), we hope to minimize prejudices related to the homeopathy and contribute to a broadening of the healing art.

OBJETIVO: Apoiado no aforismo hipocrático primum non nocere, o princípio bioético da não maleficência roga que o ato médico cause o menor dano ou agravo à saúde do paciente, incumbindo ao médico avaliar os riscos de determinada terapêutica por meio do conhecimento dos possíveis eventos adversos das drogas. Dentre esses, o efeito rebote representa um efeito colateral comum a inúmeras classes de fármacos modernos, podendo causar transtornos graves e fatais nos pacientes. Esta revisão tem o objetivo de esclarecer os profissionais da saúde sobre os aspectos clínicos e epidemiológicos do fenômeno rebote. MÉTODOS: Uma revisão qualitativa, exploratória e bibliográfica foi realizada na base de dados PubMed utilizando os unitermos 'rebound', 'withdrawal', 'paradoxical', 'acetylsalicylic acid', 'anti-inflammatory', 'bronchodilator', 'antidepressant', 'statin', 'proton pump inhibitor' and 'bisphosphonate' RESULTADOS: O efeito rebote ocorre após a descontinuação de inúmeras classes de fármacos com ação contrária aos distúrbios da doença, exacerbando-os a níveis superiores aos anteriores do tratamento. Independente da doença,dadrogaedaduração do tratamento, o fenômeno se manifesta numa pequena proporção de indivíduos suscetíveis. No entanto, pode causar eventos adversos graves e fatais, devendo ser considerado um problema de saúde pública em vista do enorme consumo de fármacos pela população CONCLUSÃO: Reunindo um corpo de evidências crescente e inquestionável, o médico precisa ter conhecimento das consequências do efeito rebote e de como minimizá-lo, aumentando a segurança no manejo das drogas modernas. Por outro lado, este efeito rebote pode ser utilizado de forma curativa, ampliando o espectro da terapêutica moderna.

OBJECTIVE: Supported in the Hippocratic aphorism primum non nocere, the bioethical principle of non-maleficence pray that the medical act cause the least damage or injury to the health of the patient, leaving it to the doctor to assess the risks of a particular therapy through knowledge of possible adverse events of drugs. Among these, the rebound effect represents a common side effect to numerous classes of modern drugs, may cause serious and fatal disorders in patients. This review aims to clarify the health professionals on clinical and epidemiological aspects of rebound phenomenon. METHODS: A qualitative, exploratory and bibliographic review was held in the PubMed database using the keywords 'rebound', 'withdrawal', 'paradoxical', 'acetylsalicylic acid', 'anti-inflammatory', 'bronchodilator', 'antidepressant', 'statin', 'proton pump inhibitor' and 'bisphosphonate'. RESULTS: The rebound effect occurs after discontinuation of numerous classes of drugs that act contrary to the disease disorders, exacerbating them at levels above those prior to treatment. Regardless of the disease, the drug and duration of treatment, the phenomenon manifests itself in a small proportion of susceptible individuals. However, it may cause serious and fatal adverse events should be considered a public health problem in view of the enormous consumption of drugs by population. CONCLUSION: Bringing together a growing and unquestionable body of evidence, the physician needs to have knowledge of the consequences of the rebound effect and how to minimize it, increasing safety in the management of modern drugs. On the other hand, this rebound can be used in a curative way, broadening the spectrum of the modern therapeutics.

OBJETIVO: Após ter sido introduzida como disciplina eletiva (não obrigatória) no segundo semestre de 2002, a acupuntura tem sido ensinada na Faculdade de Medicina da Universidade de São Paulo (FMUSP) desde então. Esse estudo foi realizado com o intuito de verificar o interesse e a aceitação da acupuntura entre os estudantes de medicina da FMUSP e se, de alguma forma, ela poderia influenciá-los positivamente, contribuindo para melhorar as suas competências. MÉTODOS: O estudo foi conduzido durante o período de 2002 a 2007, utilizando-se métodos de estatística descritiva. RESULTADOS: A média de todos os estudantes que concluíram a disciplina foi de 24,9 alunos por semestre, sendo que a média de estudantes de medicina por semestre é de 87,5 (28% de todos os estudantes de medicina nos 7º e 8º semestres). Após o primeiro semestre de formação básica em acupuntura, apenas oito alunos por grupo são promovidos (após seleção) para a Liga Médica Acadêmica de Acupuntura, que tem a duração de dois anos. Em todos os semestres, as oito vagas são preenchidas. Entre os estudantes que concluíram o curso, 98% consideraram-no de boa ou muito boa qualidade, 85% consideravam-se parcialmente capazes para empregar a acupuntura e 79% consideraram que o curso contribuiu para a sua educação médica. CONCLUSÃO: Existe um real interesse para a aprendizagem da acupuntura, pois em torno de 30% de todos os alunos quiseram passar por um breve treinamento em acupuntura, alguns deles optando por uma formação de dois anos em reconhecimento à importância da acupuntura como parte da capacitação profissional. Os resultados sugerem que a acupuntura deve ser incluída como uma disciplina eletiva para alunos de medicina.

OBJECTIVE: Since being introduced as an elective discipline (non-obligatory) in the second semester of 2002, acupuncture has been taught in the University of São Paulo School of Medicine. We conducted this study to verify interest and acceptance among under graduate medical students and to discern a positive influence contributing to improveing their skills. METHODS: The study was conducted from 2002 to 2007 using descriptive statistical methods. RESULTS: The average number of students who concluded the discipline is 24.9 students per semester with an average number of medical students per semester of 87.5 (28% of all medical students in the 7th and 8th semesters). After a semester of basic training in acupuncture, only eight students per group are promoted (after selection) to the Medical Academic League of Acupuncture which lasts for two years. Every semester, all eight vacancies are filled. Among the students who concluded the course, 98% described course quality as good or very good, 85% considered themselves at least partially able to use acupuncture and 79% stated that the course influenced them in their medical education. CONCLUSION: There is a genuine interest to learn acupuncture, almost 30% of all students volunteer for a brief training in acupuncture while some of them choose additional training of two years indue to the importance of acupuncture as part of professional skills. Results indicate that acupuncture should be included as an elective discipline for medical students.

Em virtude da valorização dos aspectos técnico-científicos no modelo biomédico, aspectos subjetivos da individualidade foram desprezados, tornando a medicina moderna desumanizada e reducionista na abordagem do processo de adoecimento humano. Com o intuito de resgatar a relação médico-paciente e a atenção integral ao indivíduo enfermo, com nítidos reflexos na qualidade e eficácia dos serviços prestados, a preocupação com a humanização da saúde passou a ser uma prioridade dos sistemas públicos, com a implantação de iniciativas nos diversos níveis de atenção. Desta forma, propostas humanísticas estão sendo incorporadas à formação médica nos cursos de graduação em Medicina e na rede de Atenção Básica em Saúde, visando contemplar, entre outros aspectos, "a compreensão dos determinantes sociais, culturais, comportamentais, psicológicos, ecológicos, éticos e legais, nos níveis individual e coletivo do processo saúde-doença". Neste contexto, o modelo homeopático, por aplicar em sua prática clínica diária uma abordagem semiológico-terapêutica individualizante e humanística na compreensão do processo saúde-doença e no tratamento das enfermidades, valorizando os determinantes éticos, filosóficos, antropológicos, psicológicos e socioambientais, poderia contribuir de forma adjuvante e complementar a humanização da formação médica, desde que seus preceitos sejam incorporados de forma regular e gradativa ao processo ensino-aprendizagem das instituições públicas e acadêmicas.

To the extent that technical and scientific aspects of the biomedical model assumed increasing importance, subjective characteristics of individuality were neglected, and modern medicine became dehumanized and narrow-minded in its approach to the processes involved in human illness. A current priority for government-run health systems is the determination to reclaim the physician-patient relationship, with comprehensive concern for patients and a clear effect on the quality and effectiveness of the services provided in an effort to achieve a more humane health system. Several new initiatives have been implemented at different levels of care. Humane propositions are thus being incorporated into medical training, in both undergraduate education and the public primary care system, with the aim of understanding the social, cultural, behavioral, psychological, environmental, ethical, and legal determinants of the health-disease process at the individual and community levels. In daily clinical practice, the homeopathic model applies a humane and individualized semiological-therapeutic approach to understanding the health-disease process and treatment of diseases, with attention to the value of ethical, philosophical, anthropological, and socio-environmental determinants and thus the potential for contributing positively to assisting and complementing medical education, as long as the model's principles are progressively incorporated into the teaching and learning process of public and academic institutions.
