INTRODUCTION: Depressive disorders (DDs) are very prevalent disorders particularly in women, a high-risk gender group. Determining the risk and protective factors associated with the development of DDs is fundamental to planning preventive and therapeutic strategies. In this study, we evaluated the correlations between healthy maternal attachment and the development of DDs in adulthood. METHODS: We evaluated 52 women at 6 months to 1 year after premature childbirth at Maternidade Vila Nova Cachoeirinha. They were evaluated using the following instruments: Brazilian Criteria of Economic Classification,Parental Bonding Inventory (PBI),Center for Epidemiologic Studies Depression Scale (CES-D), and Edinburgh Postnatal Depression Scale (EPDS). Cut-off scores on the CES-D and EPDS were used to classifythe subjects as currently having a DD or having probable postpartum disorder (PPD) after childbirth. Multiple logistic regression was used to evaluate the risk factors associated with DDs. RESULTS: We found that 49.1% of the sample had a current depressive episode, and 73.6% had probable PPD. Based on logistic regression, current depression (odds ratio = 1.092 [confidence interval: 1.005; 1.186]), and a PPD (odds ratio = 1.108 [confidence interval: 1.011; 1.21]) were negatively correlated with affective maternal relationships. CONCLUSIONS: Women who reported healthy attachment with their mothers did not develop DDs when faced with stressful situations such as premature childbirth.

Transtornos do sono e insônia são mais freqüentemente observados entre as mulheres do que entre os homens durante a vida adulta. Entre outros fatores, especula-se que alta comorbidade psiquiátrica para transtornos afetivos e ansiosos entre as mulheres contribua para maior ocorrência de insônia. Além disso, períodos de intensa variabilidade hormonal (por exemplo, gestação e puerpério, perimenopausa) parecem estar relacionados com maior incidência de insônia e de transtornos respiratórios do sono entre as mulheres. Estudos populacionais sugerem que a transição menopausal constitue um período de maior risco para o desenvolvimento de sintomas depressivos, vasomotores e de insônia. Este artigo examina os fatores clínicos e hormonais que contribuem para a ocorrência de insônia durante a peri e a pós-menopausa. Opções terapêuticas são discutidas, incluindo técnicas cognitivo-comportamentais, tratamentos hormonais, agentes hipnótico-sedativos, antidepressivos e tratamentos de medicina alternativa.

Sleep disturbances and insomnia are more commonly observed in adult females than in adult males. It has been speculated that comorbid depression and anxiety among females may contribute to a higher incidence of insomnia. In addition, periods of intense hormonal variability (e.g.; pregnancy/postpartum, menopause transition) appear to be associated with higher incidence of sleep disturbances in females. Epidemiologic studies suggest that the menopausal transition constitutes a period of heightened risk for the occurrence of depressive symptoms and vasomotor complaints, as well as insomnia. This article reviews the clinical and hormone factors that may play a role for the development of insomnia during the menopausal transition and postmenopause. Treatment options are critically reviewed, including cognitive-behavioral techniques, hormonal treatments, hypnotics, antidepressants, and complimentary/alternative medicine.

Mulheres estão sujeitas a um maior risco para o desenvolvimento de transtorno de estresse pós-traumático (TEPT) do que os homens, o que acarreta prejuízos e custos significativos do ponto de vista psicossocial e de saúde pública. Estudos recentes mostram interações complexas entre o impacto de experiências traumáticas e o ciclo reprodutivo feminino. Por exemplo, mulheres com transtorno disfórico pré-menstrual (TDPM), que também relatam histórico de trauma ou abuso físico, estão mais sujeitas a apresentar uma resposta neuroendócrina diferenciada após exposição a um fator ou evento estressante, quando comparadas a mulheres com TDPM e sem história de abuso ou mulheres sem TDPM. Além disso, mulheres com histórico de trauma ou abuso podem apresentar recidiva de sintomas durante a gravidez. Por fim, aquelas que sofrem abortamento estão mais sujeitas ao desenvolvimento de sintomas do transtorno de estresse pós-traumático. Neste artigo, examinamos os dados existentes sobre diferenças de gênero e transtorno de estresse pós-traumático, com enfoque especial nos fatores psicológicos e fisiológicos mais relevantes para o surgimento de sintomas após exposição a eventos traumáticos relacionados ao ciclo reprodutivo feminino. Opções terapêuticas existentes são criticamente revistas, incluído terapias de aconselhamento e a técnica de debriefing psicológico.

Women are at significantly higher risk for developing post-traumatic stress disorder (PTSD) than men, resulting in increased psychosocial burden and healthcare related costs. Recent research has shown complex interactions between the impact of traumatic experiences, and the reproductive lifecycle in women. For example, women suffering from premenstrual dysphoric disorder (PMDD) who also report a history of sexual or physical abuse are more likely to present with different neuroendocrine reactivity to stressors, when compared to premenstrual dysphoric disorder subjects without prior history of trauma or abuse or non-premenstrual dysphoric disorder subjects. In addition, women with a history of abuse or trauma may experience re-emergence of symptoms during pregnancy. Lastly, females who experience miscarriage may present with even higher prevalence rates of post-traumatic stress disorder symptoms. In this manuscript we examine the existing data on gender differences in post-traumatic stress disorder, with particular focus on psychological and physiological factors that might be relevant to the development of symptoms after exposure to traumatic events associated with the reproductive life cycle. Current options available for the treatment of such symptoms, including group and counselling therapies and debriefing are critically reviewed.

O manejo de distúrbios psiquiátricos durante a gestação, particularmente de quadros depressivos e de transtornos bipolares, é uma tarefa complexa. Este artigo revisa o conhecimento atual sobre o possível impacto dos transtornos de humor não-tratados sobre o desenvolvimento fetal. Também são reexaminados os possíveis riscos de teratogenicidade associados ao uso dos diversos antidepressivos, estabilizadores de humor, antipsicóticos e benzodiazepínicos durante a gestação. Dá-se ênfase ao fato de que nenhuma decisão clínica é isenta de risco. Desse modo, psiquiatras e demais profissionais atuantes na área de saúde mental devem buscar uma estratégia terapêutica que reduza ao máximo o risco de exposição da/do mãe/feto ao transtorno psíquico, bem como aos possíveis efeitos teratogênicos dos psicofármacos utilizados.

The management of psychiatric disturbances during pregnancy, particularly depression and bipolar disorders, is complex. This article reviews the existing data regarding the impact of an untreated psychiatric illness on the infant's development. In addition, the potential risks to the fetus due to prenatal exposure to different psychotropic agents, including antidepressants, mood stabilizers, antipsychotics, and benzodiazepines, are summarized. Moreover, this article emphasizes that no decision is risk-free, and the ultimate goal is to reduce the exposure to both the illness and the potential teratogenic effects of the treatment. Therefore, clinicians should seek a treatment strategy, which poses the least risk for both mother and infant.
