ABSTRACT Objective: The objective of this narrative review is to discuss the current state of research funding in Brazil. Materials and Methods: This study is based on the most recent edition of the course Funding for Research and Innovation in the University of Sao Paulo School of Medicine which was a three-day course with 12 hours of instruction. The course brought together leading experts in the field to comprehensively discuss the current state of research funding in Brazil. Each speaker provided a presentation on a specific topic related to research funding. After the workshop, speakers assembled relevant topics in this manuscript. Results: collaborative research is critical for securing research funding. It optimizes proposal competitiveness, amplifies societal impact, and manages risks effectively. As such, fostering and supporting these collaborations is paramount for both researchers and funding agencies. To maintain the highest integrity in research, investigators involved in these collaborations must disclose any relationships that could potentially influence the outcomes or interpretation of their projects. Conclusions: In Brazil, the mainstay of research funding stems from public entities, with agencies such as CNPq, CAPES, and state bodies like FAPESP, FAPERJ, FAPEMIG and others at the forefront. Concurrently, industry funding offers viable pathways, especially through industry-sponsored studies, investigator-led projects, and collaborative initiatives. The Brazilian funding landscape is further enriched by innovative platforms, including crowdfunding and the contributions of institutions like the Serrapilheira Institute. Internationally, esteemed organizations such as the National Institutes of Health (NIH) and the Bill & Melinda Gates Foundation stand out as potential funders. RESUMO OBJETIVO Avaliar a associação entre a idade gestacional e as áreas verdes, áreas construídas urbanas e a concentração de material particulado 2,5 (MP2,5) em São Paulo, analisando a distribuição irregular dessas áreas e os níveis de poluição acima do recomendado. MÉTODOS A população utilizada no estudo foi a dos nascidos vivos no ano de 2012, com os dados do Sistema de Informações sobre Nascidos Vivo (Sinasc) na cidade de São Paulo. ABSTRACT OBJECTIVE To evaluate the association between gestational age and green areas, urban built areas, and the concentration of particulate matter 2.5 (PM2.5) in the city of São Paulo, analyzing the irregular distribution of these areas and pollution levels above the recommended level. METHODS The study population consisted of a cohort of live births from 2012, and data from the Live Birth Information System (Sinasc) of the city of São Paulo were used. Using satellite images and supervised classification, the distribution and quantity of green areas and built areas in the city of São Paulo was obtained, as well as the concentrations of PM2.5. Logistic regressions were used to obtain possible associations. RESULTS The results of the study show that a lower percentage of green areas is significantly associated with a higher chance of preterm births. A higher building density was positively associated with the odds ratio for preterm birth. We did not find any significant associations between air pollution (PM2.5) and preterm births. CONCLUSIONS The results of this study show that greener areas are less associated with preterm births when compared with less green areas.

Resumo Objetivo: Estabelecer protocolo para realização de autópsias minimamente invasivas (AMIs) na detecção de óbitos por arboviroses e relatar dados preliminares desse protocolo no Ceará, Brasil. Métodos: Médicos patologistas foram treinados em AMI. Resultados: Estabeleceu-se protocolo para AMI, definindo-se critérios para amostras a serem coletadas, suas formas de armazenamento e diagnóstico, segundo o tipo de amostra biológica; em três meses, foram realizadas 43 AMIs, das quais 21 (48,8%) chegaram ao Serviço de Verificação de Óbito (SVO) com hipótese diagnóstica de alguma arbovirose e sete (16,3%) foram confirmados (seis de chikungunya; uma de dengue); também foram confirmados casos de covid-19 (n = 9), tuberculose (n = 5), meningite (n = 4), criptococose (n = 1), doença de Creutzfeldt-Jakob (n = 1), neoplasia de mama (n = 1) e raiva humana (n = 1). ABSTRACT The reduction of infectious diseases and longer life favored the greater prevalence of chronic diseases such as cancer, diabetes, cardiovascular and respiratory diseases. In general, the geographical scale of studies on socioeconomic conditions and health problems is the municipality. In this geographical scale there is no clear separation between the types of settlement. Intraurban studies by type of settlement can contribute to a more faithful portrait on inequalities in living conditions. In this study, data from residents of the municipality of São Paulo were analyzed who were admitted and those who died, by diseases of the circulatory system from 2010 to 2016 for deaths and from 2011 to 2016, for hospitalizations. Each hospitalization and death was attributed to the settlement of housing according to type: non-regular settlement, precarious or regular. The following calculations were made: proportion of hospital admissions by the Unified Health System (SUS) by groups of causes, standardized rates by gender and age group, hospitalization/inhabitant according to sex, age group and type of settlement and standardized rates by gender and age standardized mortality rates due to circulatory diseases. The results found an even more wicked situation in precarious settlements for all age groups, both sexes. The difference in cardiovascular health between the three types of settlements, evaluated through the proportions of hospital hospitalizations and mortality rates, show that almost 1,700,000 people in São Paulo are in a major disadvantage compared to the group formed by 85% of the population. RESUMO A redução das doenças infecciosas e vida mais longa favoreceram a maior prevalência das doenças crônicas como câncer, diabetes, doenças cardiovasculares e doenças respiratórias. Em geral, a escala geográfica dos estudos sobre condições socioeconômicas e problemas de saúde é o município. Nesta escala geográfica não há separação nítida entre os tipos de assentamento. Estudos intraurbanos por tipo de assentamento podem contribuir para um retrato mais fiel sobre as desigualdades nas condições de vida. Neste estudo, foram analisados dados de residentes do município de São Paulo que foram internados e os que foram a óbito, por doenças do aparelho circulatório de 2010 a 2016, para óbitos, e de 2011 a 2016, para internações. Cada internação e óbito foi atribuída ao assentamento de moradia segundo tipo: aglomerado subnormal (AGSN), precário ou regular. Brazil’s ministry of health published guidelines on the management of the disease using "early treatment”. The aim of this study was to create a visual timeline from January 2020 to April 2021 based on the review of relevant literature and documents examined using document analysis. We synthesized the main items of news on early treatment published on the Ministry of Health website, scientific evidence on the medications used, and facts related to the disease in Brazil. The timeline evidenced the insistent promotion of early treatment by the Ministry of Health within the context of a “disinfodemic” despite the existence of evidence against this type of intervention.

ABSTRACT Visceral leishmaniasis (VL) is a chronic vector-borne zoonotic disease caused by trypanosomatids, considered endemic in 98 countries, mainly associated with poverty. About 50,000–90,000 cases of VL occur annually worldwide, and Brazil has the second largest number of cases in the world. The clinical picture of VL is fever, hepatosplenomegaly, and pancytopenia, progressing to death in 90% of cases due to secondary infections and multi-organ failure, if left untreated. We describe the case of a 25-year-old female who lived in the metropolitan area of Sao Paulo, who had recently taken touristic trips to several rural areas in Southeastern Brazil and was diagnosed post-mortem. During the hospitalization in a hospital reference for the treatment of COVID-19, the patient developed acute respiratory failure, with chest radiographic changes, and died due to refractory shock. Abstract Objective: In this brief review, the authors focus on the effects of gestational exposures to urban air pollution on fetal development and neonatal outcomes. Source of data: In this review the authors used PubMed, Web of Science and SciELO research platforms, analyzing papers from the last 30 years. Summary of the findings: Epidemiological and experimental evidence agree that gestational exposure to air pollution in urban increases the risks for low birth weight, preterm birth, congenital malformation, intrauterine growth restriction, and neonatal mortality. Furthermore, exposures are associated with increased risks for preeclampsia, hypertension, gestational diabetes. Conclusions: Therefore, it is time for greater involvement and engagement of the health sector in the discussion of public policies that may affect the quality of the environment, and that directly or indirectly impact the health of those who were not yet born.

RESUMO Este artigo descreve as condições atuais da rede de monitoramento de qualidade do ar no Brasil. Os resultados revelam que apenas dez estados e o DF realizam o monitoramento através de 371 estações ativas - 80% delas na Região Sudeste. Outras informações relevantes são: (i) 41,2% das estações nacionais são privadas; no estado do Rio de Janeiro elas representam 60% do total de suas estações, enquanto no estado de São Paulo, 100% das estações são públicas; (ii) o MP10 é o poluente mais monitorado em 62,8% das estações e o MP2,5 em apenas 25,9% delas; e, (iii) a comunicação dos dados de monitoramento em tempo real à população ocorre em cinco estados. Após trinta anos de sua criação, a Rede Nacional de Qualidade do Ar encontra-se incompleta, e insuficientemente implantada, inviabilizando uma adequada gestão da qualidade do ar pelos órgãos ambientais.

ABSTRACT This article describes the current conditions of the air quality monitoring network in Brazil. The results reveal that only ten states and the Federal District carry out the monitoring through 371 active stations - 80% in the Southeast region. Other relevant information include: (i) 41.2% of the national stations are private - in the state of Rio de Janeiro they represent 60% of the its total of stations, while in the state of São Paulo, 100% of the stations are public; (ii) MP10 is the most monitored pollutant in 62.8% of stations and MP2.5 in only 25.9% of them; and, (iii) the communication of monitoring data in real time to the population occurs in five states. Thirty years after its creation, the National Air Quality Network, is incomplete and insufficiently implemented, preventing adequate air quality management by environmental agencies.

RESUMO O Material Particulado (MP) está entre os principais contaminantes do ar. A Infraestrutura Verde (IV) vem sendo reconhecida como alternativa para melhorar a qualidade do ar. As árvores podem afetar diretamente os níveis de MP, interceptando partículas em sua superfície. Devido a essa capacidade, esta pesquisa teve como objetivo contribuir com informações sobre a viabilidade da vegetação verde para interceptar e identificar as principais fontes de poluentes atmosféricos. Amostras de cascas de árvores de parques urbanos de São Paulo (SP) foram utilizadas como monitor biológico de poluentes atmosféricos. Foi observado um decaimento exponencial das concentrações e aprisionamento de poluentes em direção à região mais interna dos parques. Além disso, foi possível distinguir a emissão de veículos leves das emissões de veículos pesados. O baixo custo do monitoramento e sua eficácia mostram que a IV ocupa papel de destaque na melhoria da saúde e a qualidade de vida das pessoas, bem como nas ações que buscam atenuar os efeitos da poluição do ar em cidades de médio e grande porte.

ABSTRACT Particulate Matter (PM) ranks among the main air contaminants. A Green Infrastructure (GI) has been recognized as an alternative to improve air quality. Trees can directly affect PM levels by trapping it on their surface. This study aimes to provide extra information on the feasibility of using green vegetation to trap pollution and also to identify the main sources of air pollutants in urban areas. Tree bark samples from urban parks in São Paulo, Brazil, were used as biological monitors of air pollutants. The trapping of pollutants and an exponential decay of their concentrations were observed towards the more inner sites of the parks. Furthermore, it was possible to discriminate light-duty from heavy-duty vehicle emissions. The low cost of monitoring and its effectiveness emphasize that a GI has a pivotal position in enhancing health and quality of life and in dealing with the effects of air pollution in medium-size and large cities.

Abstract COVID-19 comprises clinical outcomes of SARS-CoV-2 infection and is highly heterogeneous, ranging from asymptomatic individuals to deceased young adults without comorbidities. There is growing evidence that host genetics play an important role in COVID-19 severity, including inborn errors of immunity, age-related inflammation and immunosenescence. Here we present a brief review on the known order of events from infection to severe system-wide disturbance due to COVID-19 and summarize potential candidate genes and pathways. Finally, we propose a strategy of subject’s ascertainment based on phenotypic extremes to take part in genomic studies and elucidate intrinsic risk factors involved in COVID-19 severe outcomes.

OBJECTIVES: Ultrasound-guided minimally invasive autopsies (MIA-US) are an alternative to conventional autopsies and have been used in our institution to investigate the pathophysiology of COVID-19 since the beginning of the pandemic. Owing to the limitations of post-mortem biopsies for evaluating cardiopulmonary events involving large vessels, we continuously improved the technique during this period. Objectives: To demonstrate the usefulness of an extended MIA-US technique (EMIA-US) for the study of thoracic involvement in COVID-19. METHOD: US-guided percutaneous tissue sampling was combined with a small thoracic incision (≤5 cm), allowing for the sampling of larger tissue samples or even the entire organ (lungs and heart). RESULTS: EMIA-US was performed for eight patients who died of COVID-19 in 2021. We demonstrate cardiopulmonary events, mainly thromboembolism and myocardial infarction, that could be evaluated using EMIA-US. CONCLUSIONS: Minimally invasive image-guided post-mortem tissue sampling is a flexible and practical method to conduct post-mortem studies of human diseases, mainly in areas that do not have autopsy facilities or, alternatively, when autopsy is not possible owing to financial constraints, cultural and religious values, or for safety reasons, such as in the case of highly contagious infectious diseases. We present evidence that EMIA-US is feasible and can be used as an alternative to increase the accuracy of MIA-US in detecting cardiopulmonary events involving large vessels, which may not be assessed through post-mortem biopsies.
