Abstract Background and aim: Malignant hilar obstruction usually presents in advanced-stage disease with a poor prognosis. Effective biliary drainage is essential for the beginning of palliative chemotherapy. There is a debate on the amount of liver parenchyma that should be drained to achieve clinical success. This study aimed to correlate the volume of liver drained with clinical success rate. Methods: The authors conducted a retrospective study including patients with malignant hilar biliary obstruction who underwent retrograde endoscopic cholangiography for biliary drainage from January 2014 to December 2018. The main outcome was a correlation of clinical success rate with hepatic volume drained. Secondary outcomes were correlation of clinical success rate with the quantity of liver sectors drained and unilateral versus bilateral drainage. Results: 82 patients met inclusion criteria (58.5 % female), with a mean age of 60±13 years. The main cause of hilar obstruction was cholangiocarcinoma (32.9 %) followed by lymph node metastasis (23.2 %). Technical success was achieved in 75 patients (91.5 %), and clinical success in 45 patients (60 %). The authors found a significant correlation between clinical success rate when at least 50 % of viable parenchyma was drained (p = 0.016; OR = 4.15, 95 % CI 1.4–12.5). Considering liver sectors, higher clinical success rates were found when at least 2 sectors were drained (p < 0.001; OR = 8.50, 95 % CI 2.7–26.7). The correlation between unilateral versus bilateral drainage and clinical success was not statistically significant. Conclusion: Drainage of at least 50 % of volume hepatic parenchyma was associated with better outcomes as well as drainage of at least 2 hepatic sectors, regardless of if unilateral or bilateral.

RESUMO RACIONAL: Estimar os desfechos de curto e longo prazo das terapias contra o câncer é crucial para o desenvolvimento de estratégias de tratamento eficazes. A desnutrição e o estado imunológico do hospedeiro afetam significativamente os desfechos em cirurgias de grande porte. OBJETIVOS: Avaliar o valor do índice nutricional prognóstico pré-operatório (INP) na predição de desfechos em pacientes com câncer gástrico. MÉTODOS: Foi realizada uma análise de coorte retrospectiva de pacientes submetidos à cirurgia com intenção curativa para adenocarcinoma gástrico entre 2009 e 2020. O INP foi calculado da seguinte forma: INP=(10 x albumina [g/dL])+(0.005 x linfócitos [nº/mm3]). O valor de corte ideal foi determinado pela curva característica de operação do receptor (ponto de corte do INP=52), e os pacientes foram agrupados em INP baixo ou alto. RESULTADOS: Dos 529 pacientes incluídos, 315 (59,5%) foram classificados como grupo de baixo INP (INP<52) e 214 (40,5%) como grupo de alto INP (INP>52). Idade mais avançada (p=0,050), sexo masculino (p=0,003), escore da Sociedade Americana de Anestesiologistas (ASA) III/IV (p=0,001), menor nível de hemoglobina (p<0,001), menor índice de massa corpórea (p=0,001), maior relação neutrófilos-linfócitos (p<0,001), linfadenectomia D1, estágio pT avançado, pN+ e estágio pTNM mais avançado foram relacionados ao paciente com baixo INP. Além disso, as taxas de mortalidade em 30 dias (1,4 vs. 4,8%; p=0,036) e em 90 dias (3,3 vs. 10,5%; p=0,002) foram maiores no grupo com baixo PNI em comparação ao grupo com alto INP. A sobrevida livre de doença e a sobrevida global foram piores em pacientes com baixo INP em comparação com pacientes com alto INP (p<0,001 para ambos). Escore ASA III/IV, baixo INP, pT3/T4 e pN+ foram fatores de risco independentes para pior sobrevida. CONCLUSÕES: O INP pré-operatório pode predizer desfechos de curto e longo prazo de pacientes com câncer gástrico após gastrectomia curativa. Baixo INP é um fator independente relacionado a piores sobrevida livre de doença e sobrevida global.

ABSTRACT BACKGROUND: Predicting short- and long-term outcomes of oncological therapies is crucial for developing effective treatment strategies. Malnutrition and the host immune status significantly affect outcomes in major surgeries. AIMS: To assess the value of preoperative prognostic nutritional index (PNI) in predicting outcomes in gastric cancer patients. METHODS: A retrospective cohort analysis was conducted on patients undergoing curative-intent surgery for gastric adenocarcinoma between 2009 and 2020. PNI was calculated as follows: PNI=(10 x albumin [g/dL])+(0.005 x lymphocytes [nº/mm3]). The optimal cutoff value was determined by the receiver operating characteristic curve (PNI cutoff=52), and patients were grouped into low and high PNI. RESULTS: Of the 529 patients included, 315 (59.5%) were classified as a low-PNI group (PNI<52) and 214 (40.5%) as a high-PNI group (PNI≥52). Older age (p=0.050), male sex (p=0.003), American Society of Anesthesiologists score (ASA) III/IV (p=0.001), lower hemoglobin level (p<0.001), lower body mass index (p=0.001), higher neutrophil-lymphocyte ratio (p<0.001), D1 lymphadenectomy, advanced pT stage, pN+ and more advanced pTNM stage were related to low-PNI patient. Furthermore, 30-day (1.4 vs. 4.8%; p=0.036) and 90-day (3.3 vs. 10.5%; p=0.002) mortality rates were higher in low-PNI compared to high-PNI group. Disease-free and overall survival were worse in low-PNI patients compared to high-PNI (p<0.001 for both). ASA III/IV score, low-PNI, pT3/T4, and pN+ were independent risk factors for worse survival. CONCLUSIONS: Preoperative PNI can predict short- and long-term outcomes of patients with gastric cancer after curative gastrectomy. Low PNI is an independent factor related to worse disease-free and overall survival.

RESUMO RACIONAL: O status linfonodal é vital para o prognóstico do câncer gástrico (CG), mas o estágio pN convencional pode ser limitado por variações na linfadenectomia e migração do estágio. O N-Ratio, que avalia a proporção entre linfonodos metastáticos e linfonodos ressecados, surge como uma ferramenta prognóstica promissora. OBJETIVOS: Avaliar o valor prognóstico do N-Ratio no CG, particularmente em pacientes com <25 linfonodos ressecados. MÉTODOS: Foram analisados, retrospectivamente, pacientes submetidos à gastrectomia com intenção curativa para CG. As categorias do N-Ratio foram determinadas pelo método da curva ROC, e a área sob a curva (AUC) foi utilizada como medida de desempenho na previsão de recorrência/óbito. RESULTADOS: Foram incluídos no estudo 561 pacientes com CG, 57% tinham status pN+ e 17,5% tinham <25 linfonodos ressecados. O N-Ratio, com média de 0,12, previu a sobrevida com precisão de 74% (AUC=0,74; IC95% 0,70–0,78, p<0,001). As categorias N-Ratio incluíram: N-Ratio 0 (43%); N-Ratio 1 (12,3%); N-Ratio 2 (31,6%); e N-Ratio 3 (13,2%). A sobrevida livre de doença (SLD) variou entre todos os grupos N-Ratio, com o N-Ratio 3 apresentando pior sobrevida do que os casos pN3 (SLD=21,8 vs.11 meses, p=0,022, p<0,05). Nos casos com <25 linfonodos ressecados, a SLD não foi significativamente pior nos grupos N-Ratio 0 (68,8 vs. 81,9%, p=0,061, p>0,05) e N-Ratio 1 (66,2 vs. 50%, p=0,504, p>0,05). A SLD dos casos de N-Ratio 0 com <25 linfonodos foi semelhante aos casos de N-Ratio 1. CONCLUSÕES: O N-Ratio influenciou a sobrevida em pacientes com CG, especialmente na doença linfonodal avançada (N-Ratio 3). Considerando que a proporção N não afeta os casos pN0, a avaliação prognóstica individualizada é essencial para pacientes com <25 linfonodos ressecados.

ABSTRACT BACKGROUND: Lymph node status is vital for gastric cancer (GC) prognosis, but the conventional pN stage may be limited by variations in lymphadenectomy and stage migration. The N-Ratio, which assesses the ratio of metastatic to resected lymph nodes, emerges as a promising prognostic tool. AIMS: To assess N-Ratios prognostic value in GC, particularly in patients with <25 resected lymph nodes. METHODS: Patients who underwent gastrectomy with curative intent for GC were retrospectively evaluated. The N-Ratio categories were determined using the ROC curve method, and the area under the curve (AUC) was used as a measure of performance in predicting recurrence/death. RESULTS: A total of 561 GC patients were included in the study, 57% had pN+ status, and 17.5% had <25 resected lymph nodes. N-Ratio, with a mean of 0.12, predicted survival with 74% accuracy (AUC=0.74; 95%CI 0.70–0.78, p<0.001). N-Ratio categories included: N-Ratio 0 (43%); N-Ratio 1 (12.3%); N-Ratio 2 (31.6%); and N-Ratio 3 (13.2%). Disease-free survival (DFS) varied among all N-Ratio groups, with N-Ratio 3 showing worse survival than pN3 cases (DFS=21.8 vs. 11 months, p=0.022, p<0.05). In cases with <25 resected lymph nodes, DFS was not significantly worse in N-Ratio 0 (68.8 vs. 81.9%, p=0.061, p>0.05) and N-Ratio 1 (66.2 vs. 50%, p=0.504, p>0.05) groups. The DFS of N-Ratio-0 cases with <25 lymph nodes was similar to N-Ratio 1 cases. CONCLUSIONS: N-Ratio influenced survival in GC patients, especially in advanced lymph node disease (N-Ratio 3). Considering that N-Ratio does not impact pN0 cases, individualized prognosis assessment is essential for patients with <25 resected lymph nodes.

RESUMO RACIONAL: O conceito introduzido pelos protocolos de recuperação após a cirurgia modifica os cuidados perioperatórios tradicionais em cirurgia digestiva. A integração desses componentes modernos de recomendações, durante o período perioperatório, é de grande importância para garantir menos complicações pós-operatórias, redução do tempo de internação hospitalar e diminuição dos custos cirúrgicos. OBJETIVOS: Enfatizar os pontos mais importantes de um protocolo multimodal de cuidados perioperatórios. MÉTODOS: Análise criteriosa de cada recomendação dos protocolos ERAS e ACERTO, justificando sua inclusão no atendimento multimodal recomendado para pacientes de cirurgia digestiva. RESULTADOS: Os programas de recuperação avançada (PRAs), tais como os protocolos ERAS e ACERTO, são a base dos cuidados perioperatórios modernos. A terapia nutricional é de grande importância na cirurgia digestiva e, portanto, tanto os cuidados nutricionais pré-operatórios, quanto pós-operatórios são fundamentais para garantir menos complicações pós-operatórias e reduzir o tempo de internação hospitalar. O conceito de pré-habilitação é outro elemento-chave nos PRAs. O manuseio de fluidos cristalóides em perfeito equilíbrio é vital. A sobrecarga de fluidos pode atrasar a recuperação dos pacientes e aumentar as complicações pós-operatórias. A abreviação do jejum pré-operatório para duas horas antes da anestesia é agora aceita por diversas diretrizes das sociedades cirúrgicas e de anestesiologia. Combinadas com a realimentação pós-operatória precoce, essas prescrições não são apenas seguras, mas também podem melhorar a recuperação de pacientes submetidos a procedimentos digestivos. CONCLUSÕES: Este posicionamento do Colégio Brasileiro de Cirurgia Digestiva enfatiza fortemente que a implementação de PRAs em cirurgia digestive, representa uma mudança de paradigma no cuidado perioperatório, transcendendo as práticas tradicionais e adotando uma abordagem inteligente para o bem-estar do paciente.

ABSTRACT BACKGROUND: The concept introduced by protocols of enhanced recovery after surgery modifies perioperative traditional care in digestive surgery. The integration of these modern recommendations components during the perioperative period is of great importance to ensure fewer postoperative complications, reduced length of hospital stay, and decreased surgical costs. AIMS: To emphasize the most important points of a multimodal perioperative care protocol. METHODS: Careful analysis of each recommendation of both ERAS and ACERTO protocols, justifying their inclusion in the multimodal care recommended for digestive surgery patients. RESULTS: Enhanced recovery programs (ERPs) such as ERAS and ACERTO protocols are a cornerstone in modern perioperative care. Nutritional therapy is fundamental in digestive surgery, and thus, both preoperative and postoperative nutrition care are key to ensuring fewer postoperative complications and reducing the length of hospital stay. The concept of prehabilitation is another key element in ERPs. The handling of crystalloid fluids in a perfect balance is vital. Fluid overload can delay the recovery of patients and increase postoperative complications. Abbreviation of preoperative fasting for two hours before anesthesia is now accepted by various guidelines of both surgical and anesthesiology societies. Combined with early postoperative refeeding, these prescriptions are not only safe but can also enhance the recovery of patients undergoing digestive procedures. CONCLUSIONS: This position paper from the Brazilian College of Digestive Surgery strongly emphasizes that the implementation of ERPs in digestive surgery represents a paradigm shift in perioperative care, transcending traditional practices and embracing an intelligent approach to patient well-being.

RESUMO RACIONAL: Os tumores irressecáveis da cabeça do pâncreas desenvolvem icterícia por obstrução e colestase durante o acompanhamento. A colestase está associada a complicações e as opções de tratamento são colocação de prótese endoscópica (PE) e a cirurgia biliodigestiva (CB). OBJETIVOS: O objetivo do presente estudo é comparar a segurança e eficácia da cirurgia biliodigestiva (CB) com a prótese endoscópica (PE) para colestase em câncer de pâncreas avançado. MÉTODOS: Este é um estudo coorte retrospectivo de pacientes com colestase e câncer de pâncreas irressecável ou metastático, tratados com CB ou PE. Os resultados de curto e longo prazo foram avaliados. Consideramos a necessidade de readmissão hospitalar por complicações biliares falha no tratamento. RESULTADOS: Foram incluídos 93 pacientes (CB=43; PE=50). A CB foi associada a maior demanda por cuidados intensivos pós-operatórios (37 vs. 10%; p=0,002, p<0.05), maior tempo de permanência na UTI (1,44±2,47 vs. 0,66±2,24 dias; p=0,004, p<0.05) e maior tempo de internação hospitalar (7,95±2,99 vs. 4,29±5,5 dias. A CB apresentou maior risco de complicações relacionadas ao procedimento (23 vs. 8%; p=0,049, p<0.05). Não houve diferença na sobrevida global entre CB e PE (p=0,089). A PE foi independentemente associada a um maior risco de falha do tratamento do que a CCB na análise multivariada (HR=3,97; p=0,009). CONCLUSÕES: A CB está associada a uma eficácia mais prolongada do que a PE no tratamento da colestase no câncer pancreático avançado. Entretanto, a CB está associada a internações prolongadas em UTI e hospitalares e maior demanda por cuidados intensivos.

ABSTRACT BACKGROUND: The unresectable pancreatic head tumors develop obstructive jaundice and cholestasis during follow-up. Cholestasis is associated with complications and treatment options are endoscopic stenting (ES) and biliary bypass surgery (BBS). AIMS: The aim of the current study was to compare the safety and efficacy of biliary bypass surgery (BBS) and endoscopic stenting (ES) for cholestasis in advanced pancreas cancer. METHODS: This is a retrospective cohort of patients with cholestasis and unresectable or metastatic pancreas cancer, treated with BBS or ES. Short and long-term outcomes were evaluated. We considered the need for hospital readmission due to biliary complications as treatment failure. RESULTS: A total of 93 patients (BBS=43; ES=50) were included in the study. BBS was associated with a higher demand for postoperative intensive care (37 vs.10%; p=0.002, p<0.050), longer intensive care unit stay (1.44 standard deviation±2.47 vs. 0.66±2.24 days; p=0.004, p<0.050), and longer length of hospital stay (7.95±2.99 vs. 4.29±5.50 days; p<0.001, p<0.050). BBS had a higher risk for procedure-related complications (23 vs. 8%; p=0.049, p<0.050). There was no difference in overall survival between BBS and ES (p=0.089, p>0.050). ES was independently associated with a higher risk for treatment failure than BBS on multivariate analysis (hazard ratio 3.97; p=0.009, p<0.050). CONCLUSIONS: BBS is associated with longer efficacy than ES for treating cholestasis in advanced pancreatic cancer. However, the BBS is associated with prolonged intensive care unit and hospital stays and higher demand for intensive care.

RESUMO O objetivo do presente manuscrito foi fazer uma revisão histórica do desenvolvimento e das mais importantes contribuições em relação à Síndrome de Lynch, desde sua primeira descrição há mais de um século atrás. Em 1895, o reputado patologista Dr. Aldred Scott Warthin ficou intrigado com a história familiar de uma costureira local, chamada Pauline Gross. De acordo com a sua previsão, ela morreria precocemente devido a um câncer, o que realmente aconteceu (do útero). Historicamente, sua família foi designada como Família “G”, caracterizando um grupo reconhecido como a maior e mais longa árvore genealógica relacionada ao câncer familiar jamais estudada. Warthin concluiu que os membros dessa família tinham susceptibilidade genética para câncer, e ainda hoje são considerados a primeira família com Síndrome de Lynch reportada na literatura. Entretanto, naquela época a comunidade médica oncológica não era receptiva à associação entre hereditariedade e câncer, a despeito da descrição de outras famílias com heredogramas similares. Infelizmente, esse fato histórico permaneceu esquecido até que outro investigador inaugurou uma nova era para a melhor compreensão da agregação familiar do câncer. Após diversas descrições dessa mesma agregação de casos de câncer em outras famílias, essa condição inicialmente denominada Síndrome de Câncer Familial foi mudada para o epônimo Síndrome de Lynch. Esse foi um reconhecimento ao extenso e dedicado trabalho desenvolvido pelo Dr. Henry Lynch na descrição de diversas características da doença e seus esforços para estabelecer as recomendações corretas para o seu diagnóstico e tratamento. Embora o futuro anuncie que ainda teremos um longo caminho a percorrer para a completa compreensão da Síndrome de Lynch, as contribuições extraordinárias da intuição de Pauline, da perseverança de Warthin e da consistência do trabalho de Lynch nunca devem ser esquecidas por àqueles que já se beneficiaram, bem como os que ainda irão se beneficiar de todo esse conhecimento.

ABSTRACT The present manuscript aimed to review the historical development and most important contributions regarding Lynch Syndrome since its first description, more than a century ago. In 1895, a reputed pathologist from Michigan University, Dr. Aldred Scott Warthin, got intrigued by the family history of a local seamstress called Pauline Gross. According to her prevision, she would present an early death due to cancer, which actually happened (from the uterus). Historically, her family was designated “Family G”, comprising a group recognized as the longest and most detailed cancer genealogy that has ever been studied. Warthin concluded that its members had genetic susceptibility for cancer, and they are, nowadays, considered the first reported Lynch Syndrome family. At that time, however, the medical cancer community was far less receptive to the association between heredity and cancer, despite the description of other families with similar heredograms. Unfortunately, this historical fact remained somewhat dormant until another investigator inaugurated a new era in the understanding of family cancer clusters. After reports and studies from this family and many others, the condition initially called Cancer Family Syndrome was changed to the eponym Lynch Syndrome. This was a recognition of the extensive and dedicated work developed by Dr. Henry Lynch in describing various characteristics of the disease, and his efforts to establish the correct recommendations for its diagnosis and treatment. Although the future announces there is still far to go for a complete understanding of Lynch Syndrome, the remarkable contributions of Pauline’s intuition, Warthin’s perseverance, and Lynch’s work consistency must never be forgotten by those who already have or will still benefit from this knowledge.

RESUMO RACIONAL: Identificação de fatores de risco epidemiológicos no esôfago de Barrett resultando em displasia e adenocarcinoma e seu impacto na prevenção e detecção precoce. OBJETIVOS: Avaliar fatores de risco epidemiológicos envolvidos no desenvolvimento de displasia e adenocarcinoma a partir do Barrett em população específica. Realizar análise crítica do período de vigilância, objetivando individualizar o tempo de seguimento conforme riscos identificados. MÉTODOS: Estudo caso-controle retrospectivo em centro terciário com pacientes com esôfago de Barrett diagnosticados e seguidos neste centro. Pacientes com Barrett que apresentaram adenocarcinoma e/ou displasia foram comparados aos que não apresentaram, levando em consideração as variáveis sexo, idade, tabagismo, IMC, etnia e extensão do Barrett. Posteriormente, foi realizada regressão logística para mensuração da razão de chances entre fatores de risco para o desfecho adenocarcinoma e desfecho displasia. Foi correlacionada a presença de fatores epidemiológicos de risco nessa população com o tempo de desenvolvimento de adenocarcinoma a partir da metaplasia. RESULTADOS: Houve diferença estatisticamente significante entre as variáveis tabagismo, raça, sexo, extensão do Barrett e idade no grupo com adenocarcinoma em relação ao sem adenocarcinoma; tabagistas e ex-tabagistas apresentaram risco 4,309 vezes maior de desenvolver adenocarcinoma; a extensão do Barrett aumentou o risco em 1,193 vezes a cada centímetro. No grupo com displasia, as variáveis tabagismo, extensão do Barrett e idade se mostraram significantes estatisticamente; extensão do Barrett aumentou 1,128 vezes a cada centímetro o risco de displasia e idade aumentou 1,023 a cada ano o risco desse desfecho. Pacientes sem fatores de risco não desenvolveram adenocarcinoma em menos de 12 meses, mesmo com displasia anteriormente. CONCLUSÕES: O estudo confirmou maior risco de desenvolver displasia e adenocarcinoma em grupos epidemiológicos específicos, podendo direcionar o seguimento em pacientes com Esôfago de Barrett de forma mais custo efetiva.

ABSTRACT BACKGROUND: Identification of epidemiological risk factors in Barrett esophagus resulting in dysplasia and adenocarcinoma and its impact on prevention and early detection. AIMS: To evaluate epidemiological risk factors involved in the development of dysplasia and esophageal adenocarcinoma from Barrett esophagus in a specific population. To critically analyze the surveillance period, aiming to individualize follow-up time according to identified risks. METHODS: A retrospective case-control study was carried out at a tertiary center involving patients diagnosed and followed up for Barrett esophagus. Patients who developed esophageal adenocarcinoma and/or dysplasia were compared to those who did not, considering variables such as gender, age, smoking status, body mass index, ethnicity, and Barrett esophagus extension. Logistic regression was performed to measure the odds ratio for risk factors associated with the outcome of adenocarcinoma and dysplasia. The presence of epidemiological risk factors in this population was correlated with the time taken to develop esophageal adenocarcinoma from metaplasia. RESULTS: A statistically significant difference was observed in smoking status, race, gender, Barrett esophagus extension, and age between the group with esophageal adenocarcinoma and the group without it. Smokers and former smokers had a 4.309 times higher risk of developing esophageal adenocarcinoma, and each additional centimeter of Barrett esophagus increased the risk by 1.193 times. In the dysplasia group, smoking status, Barrett esophagus extension, and age were statistically significant factors; each additional centimeter of Barrett esophagus extension increased the risk of dysplasia by 1.128 times, and each additional year of age increased the risk by 1.023 times. Patients without risk factors did not develop esophageal adenocarcinoma within 12 months, even with prior dysplasia. CONCLUSIONS: The study confirmed a higher risk of developing dysplasia and esophageal adenocarcinoma in specific epidemiological groups, allowing for more cost-effective monitorization for patients with Barrett esophagus.

Abstract Prognostic factors for local recurrence in patients with rectal cancer submitted to neoadjuvant chemoradiotherapy and total mesorectal excision. Background: The standard curative treatment for locally advanced rectal cancer of the middle and lower thirds is long-course chemoradiotherapy followed by total mesorectal excision. Purpose: To evaluate the prognostic factors associated with local recurrence in patients with rectal cancer submitted to neoadjuvant chemoradiotherapy and total mesorectal excision. Methods: Retrospective study including patients with rectal cancer T3–4N0M0 or T (any)N + M0 located within 10 cm from the anal border, or patients with T2N0M0 located within 5 cm, treated by long course chemoradio-therapy followed by total mesorectal excision with curative intent. Clinical, demographic, radiologic, surgical, and anatomopathological data were collected. Local recurrence was estimated using the Kaplan-Meier function, and risk was estimated according to each characteristic using univariate and multivariate analyses. Results: 270 patients were included, 57.8% male and mean age 61.7 (30‒88) years. At initial staging, 6.7% of patients were stage I, 21.5% stage II, and 71.8% stage III. Open surgery was performed in 65.2%, with sphincter preservation in 78.1%. Mortality within 30 postoperative days was 0.7%. After 49.4 (0.5‒86.1) months of median follow-up, overall and local recurrences were 26.3% and 5.9%. On multivariate analyses, local recurrence was associated with involvement of the mesorectal fascia on restaging MRI (HR = 9.11, p = 0.001) and with pathologic involvement of radial surgical margin (HR = 8.19, p < 0.001). Conclusion: Local recurrence of rectal cancer treated with long-course chemoradiation and total mesorectal excision is low and is associated with pathologic involvement of the radial surgical margin and can be predicted on restaging MRI.

RESUMO RACIONAL: O uso de tela em reparo de grandes hérnias hiatais ainda é controverso. Um dos eventos adversos mais temidos, relacionados com o uso da tela é a erosão para a parede esofágica e gástrica. OBJETIVOS: Registrar o tratamento endoscópico de tela migrada para o lúmen gástrico, após tratamento cirúrgico de hérnia de hiato. MÉTODOS: A opção técnica de aguardar a migração progressiva da tela para o lúmen gástrico, monitorando com endoscopia digestiva alta, com remoção por tração no melhor momento, com auxílio de uma pinça de corpo estranho. RESULTADOS: A remoção da tela foi completa, e a evolução foi satisfatória, sem complicações. CONCLUSÕES: Em pacientes com migração de tela para o interior do estômago, oligossintomáticos e que não apresentam sinais de complicação, vigilância endoscópica e posterior remoção do corpo estranho pode ser realizada com sucesso, quando a tela não aderida à parede gástrica, evitando procedimentos cirúrgicos com grande morbidade e mortalidade.

ABSTRACT BACKGROUND: The use of mesh in the repair of large hiatal hernias is still controversial. One of the most feared adverse events related to the use of mesh is erosion into the esophageal and gastric walls. AIMS: To record the endoscopic treatment of mesh that has migrated into the gastric lumen after surgical treatment of hiatal hernia. METHODS: The technical option was to wait for the progressive migration of the mesh into the gastric lumen, monitoring with upper digestive endoscopy, with removal by traction at the best time, with the aid of foreign body forceps. RESULTS: The mesh was completely removed, and the evolution was satisfactory, without complications. CONCLUSIONs: In patients with mesh migration into the stomach who are oligosymptomatic and do not show signs of complications, endoscopic surveillance and subsequent removal of the foreign body can be successfully performed when the mesh is not adhered to the gastric wall, avoiding surgical procedures with high morbidity and mortality.

RESUMO RACIONAL: O volume do Hospital, especialização, disponibilidade serviços oncológicos e experiência na realização de cirurgias complexas têm impacto favorável no tratamento do câncer gástrico (CG). OBJETIVOS: Comparar resultados do tratamento do CG de acordo com o tipo de Hospital oncológico no Estado de São Paulo. MÉTODOS: Pacientes diagnosticados com CG entre 2000 e 2022 foram avaliados por dados extraídos do Registro Hospitalar de Câncer, de acordo com o tipo de Unidade de tratamento: Unidade de Assistência de Alta Complexidade Oncológica (UNACON) e Centro de Assistência de Alta Complexidade Oncológica (CACON), que tem maior complexidade. RESULTADOS: Entre os 33.774 pacientes, 23.387 (69,2%) foram tratados em CACONs e 10.387 (30,8%) em UNACONs. Pacientes no CACON eram mais jovens, tinham maior nível de escolaridade e cTNM mais avançado em comparação ao UNACON (todos p<0,001). O tempo do diagnóstico ao tratamento foi superior a 60 dias em 49,8% dos pacientes do CACON e 39,4% dos do UNACON (p<0,001). O tratamento cirúrgico foi realizado em 18.314 (54,2%) pacientes. A frequência de pN0 (40,3 vs 32,4%) e pTNM I (23 vs 19,5%) foi maior no CACON. Não houve diferença na sobrevida global (SG) entre todos os casos de adenocarcinoma tratados no CACON e no UNACON (9,3 vs 10,3 meses, p=0,462, p>0.05). No entanto, considerando apenas os pacientes submetidos à cirurgia, a SG daqueles tratados no CACON foi melhor (24,4 vs 18 meses, p<0,001, p<0.05). CONCLUSÕES: Pacientes com GC submetidos à gastrectomia em CACONs apresentaram melhores resultados de sobrevida, sugerindo que a centralização de cirurgias oncológicas complexas pode ser benéfica.

ABSTRACT BACKGROUND: The hospitals’ volume, specialization, availability of all oncological services, and experience in performing complex surgeries have a favorable impact on gastric cancer (GC) treatment. AIMS: The aim of this study was to compare the results of GC treatment according to the type of oncological hospital in the State of São Paulo. METHODS: Patients diagnosed with GC between 2000 and 2022 in qualified hospitals for cancer treatment were evaluated by data extracted from the hospital cancer registry. Patients were assessed according to the type of hospital for cancer treatment: Oncology High Complexity Assistance Unit (UNACON) and Oncology High Complexity Care Center (CACON), which has greater complexity. RESULTS: Among the 33,774 patients, 23,387 (69.2%) were treated at CACONs and 10,387 (30.8%) in UNACONs. CACON patients were younger, had a higher level of education, and had a more advanced cTNM stage compared to UNACON (all p<0.001, p<0.05). The time from diagnosis to treatment was over 60 days in 49.8% of CACON’s patients and 39.4% of UNACON’s (p<0.001, p<0.05). Surgical treatment was performed in 18,314 (54.2%) patients. The frequency pN0 (40.3 vs 32.4%) and pTNM stage I (23 vs 19.5%) were higher in CACON. There was no difference in overall survival (OS) between all adenocarcinoma cases treated at CACON and UNACON (9.3 vs 10.3 months, p=0.462, p>0.05). However, considering only patients who underwent curative surgery, the OS of patients treated at CACON was better (24.4 vs 18 months, p<0.001). CONCLUSIONS: Patients with GC who underwent gastrectomy at CACONs had better survival outcomes, suggesting that the centralization of complex cancer surgery may be beneficial.

ABSTRACT BACKGROUND: The main treatment modality for gastric cancer is surgical resection with lymphadenectomy. Despite advances in perioperative care, major surgical complications can occur in up to 20% of cases. To determine the quality of surgical care employed, a new indicator called failure to rescue (FTR) was proposed, which assesses the percentage of patients who die after complications occur. AIMS: To assess the rate of FTR after gastrectomy and factors associated with its occurrence. METHODS: Patients with gastric cancer who underwent gastrectomy with curative intent were retrospectively evaluated. According to the occurrence of postoperative complications, patients were divided into FTR group (grade V complications) and rescued group (grade III/IV complications). RESULTS: Among the 731 patients, 114 had major complications. Of these patients, 76 (66.7%) were successfully treated for the complication (rescued group), while 38 (33.3%) died (FTR group). Patients in the FTR group were older (p=0.008; p<0.05), had lower levels of hemoglobin (p=0.021; p<0.05) and albumin (p=0.002; p<0.05), and a higher frequency of ASA III/IV (p=0.033; p<0.05). There were no differences between the groups regarding surgical and pathological characteristics. Clinical complications had a higher mortality rate (40.0% vs 30.4%), with pulmonary complications (50.2%) and infections (46.2%) being the most lethal. Patients with major complications grade III/IV had worse survival than those without complications. CONCLUSIONS: The FTR rate was 33.3%. Advanced age, worse performance, and nutritional parameters were associated with FTR.

RESUMO RACIONAL: A principal modalidade de tratamento do câncer gástrico (CG) é a ressecção cirúrgica associada a linfadenectomia. Apesar dos avanços no cuidado peri-operatório, as complicações cirúrgicas maiores podem ocorrer em até 20% dos casos. Para determinar a qualidade do atendimento cirúrgico empregado, foi proposto um novo indicador denominado failure to rescue (FTR), que avalia a porcentagem dos pacientes que vão a óbito após a ocorrência de complicações. OBJETIVOS: Avaliar a taxa de FTR após gastrectomia e fatores associados com a sua ocorrência. MÉTODOS: Foram avaliados retrospectivamente os pacientes com CG submetidos à gastrectomia com intenção curativa. De acordo com a ocorrência de complicações pós-operatórias, os pacientes foram divididos em grupo FTR (complicação grau V) e grupo resgatado (complicações grau 3-4). RESULTADOS: Entre os 731 pacientes, 114 tiveram maiores complicações. Desses pacientes, 76 casos (66,7%) obtiveram sucesso no tratamento da complicação (grupo resgatados), enquanto 38 casos (33,3%) foram a óbito (grupo FTR). Os pacientes do grupo FTR apresentaram idade mais avançada (p=0,008, p<0,05), menores níveis de hemoglobina (p=0,021, p<0,05) e de albumina (p=0,002, p<0,05), e maior frequência de ASA III/IV (p=0,033, p<0,05). Não houve diferença entre os grupos quanto às características cirúrgicas e patológicas. As complicações clínicas apresentaram maior taxa de mortalidade (40.0% vs 30,4%), sendo complicações pulmonares (50,2%) e infecções (46,2%) as mais letais. Pacientes com complicações maiores grau III-IV tiveram pior sobrevida comparado aqueles sem complicações. CONCLUSÕES: A taxa de FTR foi de 33,3%. Idade avançada, pior performance, e parâmetros nutricionais associam-se ao FTR.

ABSTRACT BACKGROUND: Minimally invasive distal pancreatectomy (MIDP) is associated with less blood loss and faster functional recovery. However, the benefits of robotic assisted distal pancreatectomy (RDP) over laparoscopic distal pancreatectomy (LDP) are unknown. AIMS: To compare RDP versus LDP for surgical treatment of benign lesions, pre-malignant and borderline malignant pancreatic neoplasias. METHODS: This is a retrospective study comparing LDP with RDP. Main outcomes were overall morbidity and overall costs. Secondary outcomes were pancreatic fistula (PF), infectious complications, readmission, operative time (OT) and length of hospital stay (LOS). RESULTS: Thirty patients submitted to LDP and 29 submitted to RDP were included in the study. There was no difference regarding preoperative characteristics. There was no difference regarding overall complications (RDP – 72,4% versus LDP – 80%, p=0,49). Costs were superior for patients submitted to RDP (RDP=US$ 6,688 versus LDP=US$ 6,149, p=0,02), mostly due to higher costs of surgical materials (RDP=US$ 2,364 versus LDP=1,421, p=0,00005). Twenty-one patients submitted to RDP and 24 to LDP developed pancreatic fistula (PF), but only 4 RDP and 7 LDP experienced infectious complications associated with PF. OT (RDP=224 min. versus LDP=213 min., p=0.36) was similar, as well as conversion to open procedure (1 RDP and 2 LDP). CONCLUSIONS: The postoperative morbidity of robotic distal pancreatectomy is comparable to laparoscopic distal pancreatectomy. However, the costs of robotic distal pancreatectomy are slightly higher.

RESUMO RACIONAL: A pancreatectomia distal minimamente invasiva (PDMI) está associada a menos perda sanguínea e recuperação funcional mais rápida, no entanto, os benefícios da pancreatectomia distal robótica (PDR) são desconhecidos quando comparada a pancreatectomia distal laparoscópica (PDL). OBJETIVOS: Comparar PDR versus PDL no tratamento cirúrgico de lesões benignas, neoplasias pancreáticas malignas, pré-malignas e limítrofes. MÉTODOS: Estudo retrospectivo comparando PDL com PDR. Os desfechos primários avaliados foram morbidade e custos hospitalares. Os desfechos secundários foram fístula pancreática (FP), complicações infecciosas, readmissão, tempo cirúrgico e tempo de internação hospitalar (TIH). RESULTADOS: Trinta pacientes submetidos a PDL e 29 submetidos a PDR foram incluídos no estudo. Não houve diferença em relação às características pré-operatórias. Não houve diferença em relação às complicações gerais (PDL – 72,4% versus PRD – 80%, p=0,49). Os custos foram superiores para PDR (PDR=US$ 6688 versus PDL=US$ 6149, p=0,02), principalmente devido aos custos mais elevados de materiais cirúrgicos (PDR=US$ 2364 versus PDL=1421, p=0,00005). Vinte e um pacientes submetidos a PDR e 24 submetidos a PDL desenvolveram fístula pancreática (PF), no entanto, apenas 4 submetidos PDR e 7 a PDL apresentaram complicações infecciosas associadas a FP. O tempo cirúrgico (PDR=224 min. versus PDL=213 min., p=0,36) e a conversão para cirurgia aberta (1 PDR e 2 PDL) não tiveram diferença estatística. CONCLUSÕES: A morbidade pós operatória da pancreatectomia distal robótica é comparável à pancreatectomia distal laparoscópica. Entretando, os custos da pancreatectomia distal robótica são mais elevados.

ABSTRACT BACKGROUND: Surgical resection remains the main curative therapeutic modality for advanced gastric cancer. Recently, the association of preoperative chemotherapy has allowed the improvement of results without increasing surgical complications. AIMS: To evaluate the surgical and oncological outcomes of preoperative chemotherapy in a real-world setting. METHODS: A retrospective review of gastric cancer patients who underwent gastrectomy was performed. Patients were divided into two groups for analysis: upfront surgery and preoperative chemotherapy. The propensity score matching analysis, including 9 variables, was applied to adjust for potential confounding factors. RESULTS: Of the 536 patients included, 112 (20.9%) were referred for preoperative chemotherapy. Before the propensity score matching analysis, the groups were different in terms of age, hemoglobin level, node metastasis at clinical stage- status, and extent of gastrectomy. After the analysis, 112 patients were stratified for each group. Both were similar for all variables assigned in the score. Patients in the preoperative chemotherapy group had less advanced postoperative p staging (p=0.010), postoperative n staging (p<0.001), and pTNM stage (p<0.001). Postoperative complications, 30- and 90-days mortality were similar between both groups. Before the propensity score matching analysis, there was no difference in survival between the groups. After the analysis, patients in the preoperative chemotherapy group had better overall survival compared to upfront surgery group (p=0.012). Multivariate analyses demonstrated that American Society of Anesthesiologists III/IV category and the presence of lymph node metastasis were factors significantly associated with worse overall survival. CONCLUSIONS: Preoperative chemotherapy was associated with increased survival in gastric cancer. There was no difference in the postoperative complication rate and mortality compared to upfront surgery.

RESUMO RACIONAL: A ressecção cirúrgica continua sendo a principal modalidade terapêutica curativa para o câncer gástrico avançado. Recentemente, a associação de quimioterapia pré-operatória tem permitido a melhora dos resultados sem aumentar as complicações cirúrgicas. OBJETIVOS: Avaliar os resultados cirúrgicos e oncológicos da quimioterapia pré-operatória em um cenário do mundo real. MÉTODOS: Realizou-se uma revisão retrospectiva de pacientes com câncer gástrico submetidos à gastrectomia. Os pacientes foram divididos em dois grupos para análise: cirurgia inicial e quimioterapia pré-operatória. A análise por escore de propensão, incluindo 9 variáveis, foi aplicada para ajustar possíveis fatores de confusão. RESULTADOS: Dos 536 pacientes incluídos, 112 (20,9%) foram encaminhados para quimioterapia pré-operatória. Antes da análise por escore de propensão, os grupos eram diferentes em termos de idade, nível de hemoglobina, status de node metastasis at clinical stage e extensão da gastrectomia. Após a análise, 112 pacientes foram estratificados para cada grupo. Ambos foram semelhantes para todas as variáveis atribuídas no escore. O grupo da quimioterapia pré-operatória apresentou estágios postoperative p staging (p=0,010), postoperative n staging (p<0,001) e pTNM menos avançados (p<0,001). As complicações pós-operatórias e a mortalidade em 30 e 90 dias foram semelhantes entre os grupos. Antes da análise por escore de propensão, não houve diferença na sobrevida entre os dois grupos. Após a análise, o grupo da quimioterapia pré-operatória apresentou melhor sobrevida global em comparação ao grupo da cirurgia inicial (p=0,012). As análises multivariadas demostraram que a categoria American Society of Anesthesiologists III/IV e a metástase linfonodal foram fatores significativamente associados à pior sobrevida global. CONCLUSÕES: A quimioterapia pré-operatória foi associada à maior sobrevida no câncer gástrico. Não houve diferença na taxa de complicações pós-operatórias e mortalidade em comparação com a cirurgia inicial.

ABSTRACT BACKGROUND: Peritoneal carcinomatosis in gastric cancer is considered a fatal disease, without expectation of definitive cure. As systemic chemotherapy is not sufficient to contain the disease, a multimodal approach associating intraperitoneal chemotherapy with surgery may represent an alternative for these cases. AIMS: The aim of this study was to investigate the role of intraperitoneal chemotherapy in stage IV gastric cancer patients with peritoneal metastasis. METHODS: This study is a single institutional single-arm prospective clinical trial phase II (NCT05541146). Patients with the following inclusion criteria undergo implantation of a peritoneal catheter for intraperitoneal chemotherapy: Stage IV gastric adenocarcinoma; age 18–75 years; Peritoneal carcinomatosis with peritoneal cancer index<12; Eastern Cooperative Oncology Group 0/1; good clinical status; and lab exams within normal limits. The study protocol consists of four cycles of intraperitoneal chemotherapy with paclitaxel associated with systemic chemotherapy. After treatment, patients with peritoneal response assessed by staging laparoscopy undergo conversion gastrectomy. RESULTS: The primary outcome is the rate of complete peritoneal response. Progression-free and overall survivals are other outcomes evaluated. The study started in July 2022, and patients will be screened for inclusion until 30 are enrolled. CONCLUSIONS: Therapies for advanced gastric cancer patients have been evaluated in clinical trials but without success in patients with peritoneal metastasis. The treatment proposed in this trial can be promising, with easy catheter implantation and ambulatory intraperitoneal chemotherapy regime. Verifying the efficacy and safety of paclitaxel with systemic chemotherapy is an important progress that this study intends to investigate.

RESUMO RACIONAL: A carcinomatose peritoneal no câncer gástrico é considerada uma doença fatal, sem expectativa de cura definitiva. Como a quimioterapia sistêmica não é suficiente para conter a doença, uma abordagem multimodal associando a quimioterapia intraperitoneal à cirurgia pode representar uma alternativa para esses casos. OBJETIVOS: Investigar o papel da quimioterapia intraperitoneal em pacientes com câncer gástrico estágio IV com metástases peritoneais. MÉTODOS: Trata-se de um ensaio clínico prospectivo unicêntrico, braço único, fase II (NCT05541146). Pacientes com os seguintes critérios de inclusão serão submetidos à implantação de cateter peritoneal para quimioterapia intraperitoneal: adenocarcinoma gástrico estágio IV; idade 18-75 anos; carcinomatose peritoneal com índice de câncer peritoneal<12; ECOG 0/1; bom estado clínico e exames laboratoriais dentro da normalidade. O protocolo do estudo consiste em 4 ciclos de quimioterapia intraperitoneal com Paclitaxel associado à quimioterapia sistêmica. Após o tratamento, os pacientes com resposta peritoneal avaliada por laparoscopia serão submetidos à gastrectomia de conversão. RESULTADOS: O desfecho primário é a taxa de resposta peritoneal completa. A sobrevida livre de progressão e global são outros desfechos avaliados. O estudo foi iniciado em julho de 2022 e os pacientes serão selecionados para inclusão até que 30 sejam inscritos. CONCLUSIONS: Terapias para pacientes com câncer gástrico avançado foram avaliadas em ensaios clínicos, mas sem sucesso em pacientes com metástase peritoneal. O tratamento proposto neste estudo pode ser promissor, com fácil implantação do cateter e regime de quimioterapia intraperitoneal ambulatorial. Verificar a eficácia e segurança do Paclitaxel associado à quimioterapia sistêmica é um progresso importante que o presente estudo pretende investigar.

RESUMO RACIONAL: Pacientes com câncer gástrico estádio clínico IV podem necessitar de procedimentos paliativos para o manejo de complicações como a obstrução. Contudo, não há consenso se a realização de gastrectomia paliativa em comparação à derivação gástrica traz benefícios em termos de sobrevida. OBJETIVOS: Comparar a sobrevida global de pacientes com câncer gástrico obstrutivo distal submetidos a tratamento cirúrgico paliativo, empregando a análise com pareamento por escore de propensão. MÉTODOS: Foram selecionados retrospectivamente pacientes submetidos à cirurgia paliativa de derivação (gastrojejunostomia ou partição) e ressecção entre os anos de 2009 e 2023. Variáveis clínico-patológicas iniciais e pós-operatórias foram coletadas. RESULTADOS: Foram inicialmente incluídos 150 pacientes. O grupo derivado (n=91) apresentou mais doença localmente invasiva (p<0,01), maior garu de obstrução (p<0,01) e pior status clínico (p<0,01), enquanto os gastrectomizados (n=59) mais metástase à distância (p<0,01). Após o pareamento, restaram 35 pacientes em cada grupo. Não houve diferença na incidência de complicações pós-operatórias, mas o grupo derivado apresentou maior mortalidade em 90 dias (p<0,01). A sobrevida global foi de 16,9 e 4,5 meses para o grupo ressecado e derivado, respectivamente (p<0,01). Após análise multivariada, hipoalbuminemia (HR=2,02; IC95% 1,17–3,48; p=0,01), ausência de quimioterapia adjuvante (HR=5,97; IC95% 3,03–11,7; p<0,01) e bypass gástrico (HR =3,28; IC95% 1,8–5,95; p<0,01) foram associados a pior sobrevida. CONCLUSÕES: A gastrectomia paliativa esteve associada a maior sobrevida e menor morbidade pós-operatória quando comparada à derivação gástrica. Isto pode se dever a um melhor controle local da doença, com menores riscos de complicações e melhor efetividade da quimioterapia.

ABSTRACT BACKGROUND: Patients with clinical stage IV gastric cancer may require palliative procedures to manage complications such as obstruction. However, there is no consensus on whether performing palliative gastrectomy compared to gastric bypass brings benefits in terms of survival. AIMS: To compare the overall survival of patients with distal obstructive gastric cancer undergoing palliative surgical treatment, using propensity score matching analysis. METHODS: Patients who underwent palliative bypass surgery (gastrojejunostomy or partitioning) and resection between the years 2009 and 2023 were retrospectively selected. Initial and postoperative clinicopathological variables were collected. RESULTS: 150 patients were initially included. The derived group (n=91) presented more locally invasive disease (p<0.01), greater degree of obstruction (p<0.01), and worse clinical status (p<0.01), while the resected ones (n= 59) presented more distant metastasis (p<0.01). After matching, 35 patients remained in each group. There was no difference in the incidence of postoperative complications, but the derived group had higher 90-day mortality (p<0.01). Overall survival was 16.9 and 4.5 months for the resected and derived groups, respectively (p<0.01). After multivariate analysis, hypoalbuminemia (hazard ratio — HR=2.02, 95% confidence interval — 95%CI 1.17–3.48; p=0.01), absence of adjuvant chemotherapy (HR=5.97; 95%CI 3.03–11.7; p<0.01), and gastric bypass (HR=3,28; 95%CI 1.8–5.95; p<0.01) were associated with worse survival. CONCLUSIONS: Palliative gastrectomy was associated with greater survival and lower postoperative morbidity compared to gastric bypass. This may be due to better local control of the disease, with lower risks of complications and better effectiveness of chemotherapy.
