Abstract Objective: To evaluate the association between clinical and imaging with surgical and pathological findings in patients with suspected neuroendocrine tumor of appendix and/or appendix endometriosis. Methods: Retrospective descriptive study conducted at the Teaching and Research Institute of Hospital Israelita Albert Einstein, in which medical records and databases of patients with suspected neuroendocrine tumor of appendix and/or endometriosis of appendix were analyzed by imaging. Results: Twenty-eight patients were included, all of which had some type of appendix alteration on the ultrasound examination. The pathological outcome of the appendix found 25 (89.3%) lesions compatible with endometriosis and three (10.7%) neuroendocrine tumors. The clinical findings of imaging and surgery were compared with the result of pathological anatomy by means of relative frequency. Conclusion: It was possible to observe a higher prevalence of appendix endometriosis when the patient presented more intense pain symptoms. The image observed on ultrasound obtained a high positive predictive value for appendicular endometriosis. SUMMARY OBJECTIVE: Robotic surgery is currently on the rise and has been widely applied all over the world. Gynecology offers great opportunities for the development of innovative techniques due to the magnitude of surgical needs. The aim of this study was to correlate perioperative complications, surgical time, and length of hospital stay with surgical diagnosis, procedure performed, and surgeon experience in robot-assisted gynecological surgeries in a 10-year period. METHODS: This was a retrospective, transversal, cross-sectional study involving 632 patients who underwent robotic gynecological surgery from January 2008 to December 2017 in a community hospital in Sao Paulo, Brazil. Medical records of robot-assisted gynecological operations were searched for perioperative complications, operative time, and length of hospital stay, correlating these outcomes with surgical diagnosis, procedure performed, and surgeon experience, considering those with 20 or less robotic procedures and surgeons with more than 20 cases in their career as in-training or qualified surgeons, respectively. RESULTS: Endometriosis (381 cases) was the most common surgical indication, followed by uterine myoma (171 patients). Qualified surgeons had 64% less complications than in-training surgeons (p=0.03) and achieved 20% lower surgical time and 15% shorter length of hospital stay. CONCLUSION: In this study, qualified surgeons with more than 20 robotic procedures had better perioperative outcomes and less complications than in-training surgeons during their first 20 robotic surgeries.

Resumo Introdução A endometriose é uma doença inflamatória que afeta mulheres em idade reprodutiva, causando dor e possibilidade de infertilidade. A endometriose foi associada a baixa qualidade de vida e pesquisas mostram o impacto da endometriose emdiversas áreas da vida, justificando como tais pacientes têmmaior probabilidade de desenvolver depressão, ansiedade e estresse. Objetivo O objetivo da presente revisão sistemática foi explorar o campo da psicologia na endometriose, identificando estudos que usaram a técnica da terapia cognitiva comportamental como tratamento da endometriose e da dor pélvica crônica. Métodos As palavras chaves utilizadas foram Endometriose AND Terapia comportamental; Disciplinas e atividades comportamentais; Terapia cognitiva comportamental; Saúde mental; Técnicas psicológicas; Psicologia; Psicoterapia; Serviços de saúde mental, e a busca foi realizada nos bancos de dados PubMed / Medline, SCIELO, LILACS e CAPES. O estudo seguiu as diretrizes dos Principais Itens para Relatar Revisões Sistemáticas e Metanálises (PRISMA, na sigla em inglês) e foram considerados todos os estudos cuja estratégia de intervenção utilizada estava relacionada à terapia cognitivocomportamental. Resultados Dos 129 artigos encontrados, somente 5 foram selecionados, e foi possível identificar que a intervenção psicológica cuja abordagem trouxe técnicas da terapia cognitivo-comportamental promoveu diminuição na sensação de dor, melhora nos escores de depressão e estresse e mudanças significativas em aspectos da qualidade de vida como vitalidade, funcionalidade física e social, bem-estar emocional, controle e autonomia. Conclusão A terapia cognitivo-comportamental pode ser muito promissora para o tratamento psicológico/emocional de quem tem endometriose. No entanto, a presente revisão sistemática destaca a necessidade de desenvolver estudos mais estruturados com métodos consistentes, claros e replicáveis para se chegar a um protocolo de intervenção psicológica para pacientes que convivem com esse quadro ginecológico-físico-emocional.

Abstract Introduction Endometriosis is an inflammatory disease that affects women of reproductive age, causing pain and the possibility of infertility. Endometriosis was associated to low life quality and research shows the impact of endometriosis in several areas of life, justifying how these patients are more likely to develop depression, anxiety, and stress. Objective The aim of the present systematic review was to explore the field of psychology in endometriosis, identifying studies that used the cognitive behavioral therapy technique as a treatment for endometriosis and chronic pelvic pain. Methods The keywords used were Endometriosis and Behavioral Therapy; Behavioral Disciplines and Activities; Cognitive Behavioral Therapy; Mental Health; Psychological Techniques; Psychology; Psychotherapy; Mental Health Services; and the search was performed in the following databases: PubMed/Medline, Scielo, Lilacs, and Capes. The study followed the PRISMA guidelines and all studies whose intervention strategy used was related to cognitive-behavioral therapy were considered. Results Of the 129 articles found, only 5 were selected, and it was possible to identify that the psychological intervention whose approach brought cognitive-behavioral therapy techniques promoted a decrease in the sensation of pain, improvements in the scores of depression and stress, and significant changes in aspects of quality of life such as vitality, physical and social functioning, emotional well-being, control, and autonomy. Conclusion Cognitive-behavioral therapy can be very promising to take care of the emotional side of those who have endometriosis However, the present systematic review highlights the need to develop more structured studies with consistent, clear and replicablemethods to reach a psychological intervention protocol for patients who live with this gynecological-physical-emotional condition.

SUMMARY OBJECTIVE: To analyze the public data of hysterectomies performed in the only health system in the city of São Paulo between 2008 and 2018. METHODS: The following public health system data were extracted and analyzed: age, technique, number of surgeries, mortality during hospitalization, length of stay in the establishment (days), and amounts paid by the public network. RESULTS: A total of 20,119 procedures were analyzed. The most prevalent procedure was total hysterectomy (43.2%), followed by vaginal hysterectomy (26.7%), subtotal hysterectomy (24.3%), and laparoscopic hysterectomy (5.8%). Early discharge (hospital stay of up to 1 day) was more prevalent in cases of vaginal hysterectomy (39%). We observed a marked downward trend in the number of total hysterectomies. Total hysterectomy was the most expensive procedure; no significant difference was noted in the cost of vaginal versus laparoscopic hysterectomy. We noticed a trend of rising costs over the years. The most frequent hospital admission code was that of leiomyoma of the uterus in cases of total, subtotal, and laparoscopic hysterectomy. CONCLUSION: Despite the decrease in the number of hysterectomies over the 11-year study period in São Paulo, it remains in high demand mainly for the treatment of uterine leiomyomatosis. Laparoscopic hysterectomy has been gaining ground and showed a slightly upward trend with a shorter hospital stay. Laparoscopic and vaginal hysterectomy required less financial support from the health system than open surgery.

Resumo Estima-se que cerca de 28 milhões de cirurgias sejam postergadas ou canceladas nomundo em decorrência desta pandemia, causando atraso no diagnóstico e tratamento de mais de 2 milhões de casos oncológicos. No Brasil, tanto a ANS (Agencia Nacional de Saúde) comoa ANVISA (Agencia Nacional de Vigilância Sanitária) orientaram o adiamento das cirurgias eletivas e não essenciais, tendo um impacto considerável no número de procedimentos cirúrgicos comdiminuição de 33,4% neste período no Brasil.No entanto, algumasmulheres necessitam de tratamento para várias doenças ginecológicas, algumas das quais não podem ser adiadas. O objetivo deste artigo é apresentar recomendações sobre o tratamento cirúrgico durante a pandemia de COVID-19.

Abstract It is estimated that around 28 million surgeries will be postponed or canceled worldwide as a result of this pandemic, causing a delay in the diagnosis and treatment of more than 2 million cancer cases. In Brazil, both the National Health Agency (ANS) and National Health Surveillance Agency (ANVISA) advised the postponement of elective and non-essential surgeries, causing a considerable impact on the number of surgical procedures that decreased by 33.4% in this period. However, some women need treatment for various gynecological diseases that cannot be postponed. The purpose of this article is to present recommendations on surgical treatment during the COVID-19 pandemic.

Resumo Objetivo Analisar a aplicabilidade e eficiência de um treinamento em laparoscopia com múltiplas abordagens, em melhorar as habilidades laparoscópicas básicas de residentes de ginecologia e obstetrícia (GO). Métodos Estudo transversal, observacional e descritivo, desenvolvido no Centro de Treinamento em Experimentação e Cirurgia (CETEC) do Instituto de Pesquisa do Hospital Albert Einstein com residentes de GO. Foram aplicadas avaliações teóricas e práticas a 24 residentes de GO com o objetivo de avaliar suas habilidades laparoscópicas antes e após sua participação em um curso de 8 semanas. O curso envolveu palestras teóricas e exercícios práticos de cirurgia laparoscópica através de modelos de borracha, caixas pretas, simuladores virtuais e modelos animais (porcos). Resultados Houve uma melhora geral na habilidade dos residentes, comaumento do número de respostas corretas na avaliação teórica e diminuição do tempo na execução dos testes práticos (montagem do porta-agulha e realização de nó laparoscópico). O curso foi avaliado pelos alunos como altamente relevante por melhorar suas habilidades cirúrgicas e motivá-los a continuar praticando. Conclusão O treinamento laparoscópico utilizando múltiplas abordagens resultou em melhora significativa das habilidades cirúrgicas atrelado a alto nível de satisfação dos participantes. Novos estudos ainda são necessários para mensurar a retenção destas habilidades adquiridas a longo prazo.

Abstract Objective To analyze the applicability and efficiency of amulti-approach laparoscopic training in improving basic laparoscopic skills of obstetrics and gynecology (OBGYN) residents. Methods Cross-sectional, observational and descriptive study, developed at the Experimentation and Surgery Training Center (CETEC, in the Portuguese acronym) of the Hospital Israelita Albert Einstein with OBGYN residents. Theoretical and practical tests were applied to 24 OBGYN residents to assess their laparoscopic skills before and after their participation in an 8-week course. The course involved theoretical lectures and practical laparoscopic surgery exercises developed using rubber models, black boxes, virtual simulators and animal models (pigs). Results There was an overall improvement in the ability of the residents, with an increase in the number of correct answers in the theoretical evaluation and decrease in the time needed to perform practical tests (needle holder assembly and laparoscopic node). The course was evaluated by the students as highly relevant for both improving their surgical skills and motivating them to continue practicing. Conclusion Laparoscopic training using multiple approaches resulted in significant improvement of surgical skills with a high satisfaction level of the participants. Further studies are still needed to measure the long-term retention of these acquired skills.

Abstract Graft-versus-host disease is one of the main causes of morbidity and mortality in patients submitted to hematopoietic stem cell transplantation. This study reviewed the prevalence of lower female genital tract graft-versus-host disease following hematopoietic stem cell transplantation. A systematic search of the literature for articles published between 1982 and 2015 was performed. A growing number of young women suffering from malignant and benign hematological diseases are receiving allogeneic hematopoietic stem cell transplantation with very satisfactory results in relation to the disease itself. However, these patients face gynecological problems due to graft-versus-host disease. Correct diagnosis and early management are needed to avoid irreversible complications.

Resumo Introdução: A disfunção sexual é altamente prevalente, afetando 40% da população feminina. A incidência de tal disfunção é conhecida por ser maior entre as mulheres com câncer de mama e pacientes com ansiedade e depressão. No entanto, existem poucos dados sobre a prevalência de disfunção sexual entre mulheres com doença benigna da mama (BBD). Objetivo: Avaliar a incidência de disfunção sexual, depressão e ansiedade em mulheres com BBD, em comparação a mulheres saudáveis. Método: Avaliamos a incidência de disfunção sexual em 60 pacientes com doença benigna da mama (fibroadenomas, cistos mamários, dor mamária e tumor phyllodes) e 69 mulheres saudáveis (grupo controle). As participantes completaram o Questionário de Quociente Sexual para Mulheres (SQQ-F), o Inventário de Depressão de Beck e o Inventário de Ansiedade de Beck. A análise estatística revelou que a depressão e a ansiedade eram comparáveis entre os grupos BBD e controle (10,3 vs. 20,3% e 38,7 vs. 34,3%, respectivamente, p>0,05). O escore médio de SQQ-F (65,6±22,7 vs. 70,1±16,8; p>0,05) e a disfunção sexual (33,3 vs. 25,4%; p=0,324) foram semelhantes entre os grupos BBD e controle. Conclusão: Não encontramos diferenças entre mulheres com BBD e mulheres saudáveis em termos de incidência de disfunção sexual, ansiedade e depressão. No entanto, dada a alta prevalência dessa condição, é importante avaliar a qualidade de vida sexual, bem como a qualidade de vida global, em mulheres com BBD.

Summary Introduction: Sexual dysfunction is highly prevalent, affecting 40% of the female population. The incidence of such dysfunction is known to be higher among women with malignant breast disease and in patients with depression or anxiety. However, there are few data regarding the prevalence of sexual dysfunction among women with benign breast disease (BBD). Objective: To evaluate the incidence of sexual dysfunction, depression and anxiety among women with BBD, in comparison with that observed for healthy women. Method: We evaluated the incidence of sexual dysfunction in 60 patients with benign breast disease (fibroadenomas, breast cysts, breast pain and phyllodes tumor) and 69 healthy women (control group). Participants completed the Sexual Quotient Questionnaire for Females (SQQ-F), the Beck Depression Inventory and the Beck Anxiety Inventory. Statistical analysis revealed that depression and anxiety were comparable between BBD and control groups (10.3 vs. 20.3% and 38.7 vs. 34.3%, respectively, p>0.05). The mean SQQ-F score (65.6±22.7 vs. 70.1±16.8; p>0.05) and sexual dysfunction (33.3 vs. 25.4%; p=0.324) were similar between BBD and control groups. Conclusion: We found no differences between women with BBD and healthy women in terms of the incidence of sexual dysfunction, anxiety and depression. Nevertheless, given the high prevalence of this condition, it is important to assess sexual quality of life, as well as overall quality of life, in women with BBD.

Objetivo: avaliar o perfil epidemiológico e as complicações operatórias das pacientes submetidas à operações ginecológicas realizadas devido à presença de doenças benignas em um hospital público terciário no Estado de Roraima, Brasil. Métodos: foi realizado um levantamento retrospectivo, por meio da análise de prontuários de 518 pacientes submetidas à operações ginecológicas entre os meses de janeiro e junho de 2012. Foram incluídas as três principais operações realizadas nesse período (n=175): histerectomia, colpoperineoplastia e colocação de sling suburetral. Foram excluídos 236 casos de laqueadura tubária e 25 casos em que não foi possível acesso ao prontuário médico. Resultados: A média etária foi 47,6 anos, predominaram pacientes com ensino fundamental (36,6%), provenientes da capital (77%), com relações estáveis (47,4%) e ocupação prevalente "do lar" (26,3%). As pacientes, em sua maioria, tinham antecedentes três ou mais partos (86,6%), com via vaginal prévia em 50,2% e parto cesáreo prévio em 21%. Os principais diagnósticos indicativos de tratamento cirúrgico foram: mioma uterino (46,3%), incontinência urinária de esforço (27,4%) e distopias genitais (17,7%). Foram encontrados três casos (1,7%) de lesões intraepiteliais de alto grau na colpocitologia oncótica. A operação mais realizada foi a histerectomia total (19,8%), sendo 15,5% por via vaginal. A complicação mais frequente foi a infecção de ferida operatória (2,2%). Conclusão: as mulheres submetidas às operações ginecológicas devido à doenças benignas apresentavam média etária de 47 anos, a maioria tinha nível de escolaridade fundamental, eram provenientes da capital, tinham união estável, eram predominantemente do lar, multíparas e apresentaram taxas de complicações operatórias baixas.

Objective: To evaluate the epidemiological profile and the operative complications of patients undergoing gynecological operations for benign diseases in a tertiary public hospital in the state of Roraima, Brazil. Methods: We conducted a retrospective survey through the analysis of 518 records of patients submitted to gynecological operations between January and June 2012. We included the three major operations during this period (n = 175): hysterectomy, colpoperineoplasty and suburethral sling placement. We excluded 236 cases of tubal ligation and 25 cases where it was not possible to access to medical records. Results: The mean age was 47.6 years; the education level of most patients was completed junior high (36.6%); 77% were from the State capital, 47.4% were in stable relationships and 26.3% were housewives. The majority of patients had given birth three or more times (86.6%), with previous vaginal delivery in 50.2%, and cesarean delivery, 21%. The main diagnostic indications for surgical treatment were uterine myoma (46.3%), urinary incontinence (27.4%) and genital dystopias (17.7%). We found three cases (1.7%) of high-grade intraepithelial lesions on Pap smear. The most common procedure was total hysterectomy (19.8%), 15.5% vaginally. The most common complication was wound infection (2.2%). Conclusion: Women undergoing gynecological operations due to benign disease had a mean age of 47 years, most had levels of basic education, came from the capital, were in stable relationships, predominantly housewives, multiparous and showed low operative complication rates.

Objectivo: embora a endometriose seja uma doença prevalente, cujo diagnóstico precoce é fundamental para a prevenção de sua progressão, é uma condição frequentemente negligenciada em adolescentes. O objetivo deste estudo é levantar as características clínicas das pacientes adolescentes com endometriose acompanhadas em um hospital terciário. Métodos: levantamento retrospectivo de 394 pacientes submetidas à cirurgia com diagnóstico histológico de endometriose no Setor de Endometriose da Divisão de Clínica de Ginecologia do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, de 2008 a 2013. Foram incluídas 21 adolescentes (idade menor de 21 anos). Resulados: a idade média foi de 17,95±1,48 anos, a média de tempo para a confirmação diagnóstica foi de 2,96±2,93 anos e a idade do início dos sintomas foi em média de 15,28±3,03 anos. Os locais de acometimento foram ovariano (38%), peritoneal (47,6%) e retrocervical (23,8%). Dismenorreia esteve presente em 80,9% das adolescentes (sendo severa em 33,3% dos casos) e dor pélvica crônica em 66,6%. Conclusão: endometriose em adolescentes é um importante diagnóstico diferencial de dor pélvica e cistos anexiais, principalmente entre aquelas sem resposta ao tratamento convencional. As principais formas de acometimento são peritoneais e ovarianas. Apesar do início dos sintomas na adolescência e dos avanços nos métodos de imagem, ainda se observa demora no diagnóstico dessa doença.

Objective: despite endometriosis being a common disease, where early detection is key to preventing its progression, it is a condition often overlooked in adolescents. The aim of this study was to report the clinical characteristics of adolescent patients with endometriosis monitored in a tertiary hospital. Methods: a retrospective study of 394 patients undergoing surgery with a histological diagnosis of endometriosis at the Endometriosis Division of the Gynecology Department at the Hospital das Clínicas of the University of São Paulo Medical School from 2008 to 2013. 21 adolescents were included (aged under 21 years). Results: the age ranged from 17.95 ± 1.48 years, the average time for diagnostic confirmation was 2.96 ± 2.93 years, and the age at the onset of symptoms was 15.28 ± 3.03 years on average. The sites affected were ovarian (38%), peritoneal (47.6%) and retrocervical (23.8%). Dysmenorrhea was found in 80.9 % of adolescents (severe in 33.3% of cases) and chronic pelvic pain in 66.6%. Conclusion: endometriosis in adolescents is an important differential diagnosis from pelvic pain and ovarian cysts, mainly among those with no response to conventional treatment. The main forms of involvement are peritoneal and ovarian. Despite the onset of symptoms in adolescence and advances in imaging methods, the diagnosis of this disease is still delayed.

A endometriose representa uma afecção ginecológica comum, atingindo de 5%-15% das mulheres no período reprodutivo e até 3%-5% na fase pós-menopausa. Essa doença é definida pelo implante de estroma e/ou epitélio glandular endometrial em localização extrauterina, podendo comprometer diversos locais. Humanos e animais são expostos diariamente a poluentes químicos que têm a capacidade de influenciar negativamente processos fisiológicos e, potencialmente, causar doenças, dentre elas a endometriose. Com esta revisão tivemos por objetivo relacionar a influência dos fatores ambientais e dietéticos na gênese da endometriose. O mecanismo pelo qual a dioxina e seus símiles (TCDD/PCBs) atuam na alteração da fisiologia endometrial permanence incerta e é especulativa devido à dificuldade em se avaliar a exposição na vida intraútero, infância e vida adulta e suas reais consequências, além das limitações de sua reprodução in vitro. Devemos entender melhor o mecanismo de ação desses poluentes amibentais não só na saúde reprodutiva, mas na saúde em geral do indivíduo, para se promover estratégias de prevenção que devem incluir não só a educação populacional, mas o estabelecimento de limites de exposição, técnicas menos poluentes e melhor aproveitamento dos nossos recursos naturais.

Endometriosis represents a common gynecological condition affecting 5%-15% of childbearing age women and up to 3% 5% of post-menopausal women. This disease is defined by the presence of stromal and/or endometrial glandular epithelium implants in extra-uterine locations possibly compromising several sites. Humans and animals are daily exposed to chemical pollutants that could adversely influence physiological processes and potentially cause diseases, including endometriosis. In this review, the authors aimed at settling the influence of environmental and dietary factors on endometriosis pathogenesis. The mechanism by which dioxin and its similes (TCDD/PCBs) act changing the endometrial physiology remains uncertain and is speculative due to the difficulty in assessing the exposure over intrauterine life, childhood and adulthood and its actual consequences, in addition to the limitations to its in vitro reproducibility. We need to better understand the mechanism of action of these environmental pollutants, not only on reproductive health, but also on overall health of individuals and so prevention strategies, including not only population education, but setting exposure limits, less polluting techniques and a better use of our natural resources, could be promoted.

OBJETIVO: Este estudo analisa a relação entre as características clínicas da endometriose e os padrões da resposta imune Th1/Th2. MÉTODOS: Estudo prospectivo realizado com 65 pacientes com endometriose (Grupo A) e 33 pacientes sem a doença (Grupo B). Foram realizadas avaliação no fluido peritoneal e sangue periférico de IL 2, 4 e 10, TNF-alfa e IFN-gama. A significância foi estabelecida em p < 0,05. RESULTADOS: TNF-alfa encontrava-se elevado em pacientes com endometriose que apresentavam dispareunia de profundidade comparado com controle (média 4,5 pg/ml e 2,3 pg/ml, p< 0,05). Dentre essas pacientes (n=32), 65,5% apresentavam endometriose profunda. Pacientes com endometriose e infertilidade apresentavam concentrações maiores de IL-2 no fluido peritoneal quando comparadas com controle (média 5,9 pg/ml e 0,2 pg/ml, p< 0,05), sendo que neste grupo, 63,5% (n=14) apresentavam endometriose profunda. Foi observada também maior concentração de IL-10 nas pacientes que apresentavam endometriose ovariana quando comparadas às sem esse tipo de endometriose, assim como quando comparadas às pacientes do grupo controle (média 50pg/ml, 18,7pg/ml e 25,7pg/ml, p<0,05). CONCLUSÃO: Estes resultados sugerem que quando dados clínicos específicos associam-se a uma produção elevada de certas citocinas, ocorre um padrão de resposta Th1 que pode estar associado à endometriose profunda.

OBJECTIVE: This study analyzed the relationship between clinical characteristics of endometriosis and Th1/Th2 immune response patterns. METHODS: A prospective study was performed with 65 patients with endometriosis (Group A) and 33 without the disease (Group B). Measurement of IL 2, 4 and 10, TNF-alpha and IFN-gamma was carried out in peripheral blood and peritoneal fluid. RESULTS: Serum TNF-alpha was higher in patients with endometriosis who had deep dyspareunia compared to controls (mean 4.5 pg/ml and 2.3 pg/ml, p<0.05). Among these patients (n=32), 65.5% had deep endometriosis. Patients with endometriosis and infertility had higher IL-2 concentrations in peritoneal fluid than controls (mean 5.9 pg/ml and 0.2 pg/ml, p<0.05). Among these patients (n=22), 63.5% (n=14) had deep endometriosis. A higher concentration of IL-10 was also observed in patients with ovarian endometriosis when compared to those without this type of disease, as well as when compared to control group patients (mean 50pg/ml, 18.7pg/ml and 25.7pg/ml, p<0.05). CONCLUSIONS: These results suggest that when specific clinical data are associated with a higher production of certain cytokines, there is a Th1 response pattern that may be related to deep infiltrating endometriosis.

OBJETIVO: Descrever aspectos clínicos e epidemiológicos das pacientes portadoras de endometriose pélvica operadas em nosso serviço. MÉTODOS: Estudo retrospectivo de 892 pacientes submetidas a videolaparoscopia com confirmação histológica do diagnóstico de endometriose. RESULTADOS: A média de idade foi de 33,2 ± 6,3 anos, sendo 78,7% brancas. Observamos 76,9% de mulheres com 2º ou 3º graus completos; 56,5% das pacientes eram nulíparas e 62,2% relataram dismenorreia como principal queixa. A dor pélvica crônica foi o sintoma mais prevalente, seguido pela dispareunia de profundidade, sendo referidos por 56,8% e 54,7% das pacientes, respectivamente. A infertilidade foi referida por 39,8% das 892 pacientes. CONCLUSÃO: A endometriose é uma doença geralmente diagnosticada na 4º década da vida das pacientes, as quais apresentam queixas clínicas relacionadas com frequência à dor pélvica e infertilidade, que devem sempre ser questionadas para orientar a hipótese diagnóstica.

OBJECTIVE: To describe clinical and epidemiological aspects of patients with pelvic endometriosis who were operated in our service. METHODS: A retrospective study was made of 892 patients submitted to laparoscopy with histological confirmation of diagnosis of endometriosis. RESULTS: The mean age was 33.2 ± 6.3 years and 78.7% were Caucasian. We found that 76.9% of women had higher education. 56.5% of patients were nulliparous and 62.2% reported dysmenorrhea as the main complaint. Chronic pelvic pain was the most prevalent symptom, followed by deep dyspareunia, mentioned by 56.8% and 54.7% of patients, respectively. Infertility was reported by 39.8% of the 892 patients. CONCLUSION: Endometriosis is a disease diagnosed in the 4th decade of life, of patients who have multiple complaints . They must always be questioned to properly orient diagnosis and monitor results of treatment.

CONTEXTO E OBJETIVO: A endometriose é uma doença freqüente que pode acometer o trato intestinal. O objetivo deste relato de caso é descrever forma pouco usual de apresentação clínica. RELATO DE CASO: Paciente de 35 anos tinha queixa de dor em região epigástrica somente durante o período menstrual e antecedente de endometriose vesical. Endoscopia digestiva alta era normal, e ultra-sonografia transvaginal e ressonância nuclear magnética da pelve demonstravam lesão em transição íleo-ceco-apendicular de 30 x 22 x 13 mm sugestiva de endometriose. A paciente foi submetida a ressecção laparoscópica do segmento intestinal afetado pela doença com imediata reconstrução do trânsito intestinal através da confecção de anastomose íleo-cólon ascendente. A análise histológica confirmou o diagnóstico de endometriose. CONCLUSÕES: Dor epigástrica recorrente deve ser caracterizada quanto à sua relação com o período menstrual em mulheres jovens, especialmente se houver antecedente de endometriose, pois pode ser um sinal clínico da doença acometendo o trato intestinal.

CONTEXT AND OBJECTIVE: Endometriosis is a common affliction that may affect the intestinal tract. The objective of this case report was to describe an unusual clinical presentation of this form of the disease. CASE REPORT: The patient was a 35-year-old woman with epigastric pain that only occurred during menstruation, who had a history of bladder endometriosis. Endoscopy of the upper digestive tract showed normal results. Transvaginal ultrasound and nuclear magnetic resonance of the pelvis showed a lesion involving the ileocecal junction and appendix, measuring 30 x 22/x/13/mm, that was suggestive of endometriosis. The patient underwent laparoscopic resection of the bowel segment affected by the disease, followed by anastomosis of the ileum and ascending colon for immediate restoration of intestinal transit. Histological analysis confirmed the diagnosis of endometriosis. CONCLUSIONS: In young women, recurrent epigastric pain should be evaluated with regard to its relationship to menstruation, particularly if there is a history of endometriosis, since this may be a clinical sign that the disease is affecting the intestinal transit.
