Las alteraciones de la diferenciación sexual (ADS) son patologías originadas por trastornos en una de las tres etapas sucesivas de dicha diferenciación: cromosómica (XX, XY), gonadal (testículo, ovario) o fenotípica. El objetivo del trabajo fue dar a conocer la forma de presentación de las ADS en pacientes provenientes de las regiones Capital y Centro Occidental de Venezuela. Se incluyeron diecisiete pacientes y se elaboraron los árboles genealógicos, se realizó evaluación clínica, estudios hormonales, citogenéticos, imagenología y determinación de marcadores del gen SRY y microsatélites del cromosoma Y. En función de la evaluación clínica y los datos obtenidos de los exámenes practicados, se efectuaron los diagnósticos siguientes: a) Doce corresponden a ADS 46,XX , de los cuales siete pacientes tienen ADS por exceso de andrógenos, un caso con reversión sexual, un ADS ovotesticular, un caso con síndrome malformativo, uno con disgenesia gonadal y uno con hipogonadismo, b) Cuatro presentan ADS 46,XY (un paciente con síndrome de Smith-Lemli-Opitz II, uno con síndrome malformativo y dos casos con hipogonadísmo), c) Un caso de ADS por alteración cromosómica 46,XXY (síndrome de Klinefelter). En relación a la edad de la primera consulta, la mayor parte (47,1%) se realizó en menores de 5 años, referidos por ambigüedad sexual con necesidad de resolver la identificación del sexo; en la pubertad los pacientes consultan por alteraciones en los caracteres sexuales secundarios y amenorrea (adolescentes); en la adultez por infertilidad. Los resultados permitieron realizar un mejor asesoramiento genético y contribuir a mejorar la calidad de vida de los pacientes y sus grupos familiares.

Disorders of sexual differentiation (DSD) are pathologies characterized by an atypical development of chromosomal (XX, XY) gonadal (testis, ovary) or phenotypical sex. The objective of this work was to inform the presentation forms of SDS in patients from the Capital and West Center regions of Venezuela. Seventeen patients were included and the pedigrees, clinical evaluation, hormonal studies, cytogenetic, imaging and identification of SRY gene markers and Y chromosome microsatellites were made. Depending on the clinical evaluation and data from examinations carried out, the following diagnoses were made: a)Twelve patients correspond to DSD 46, XX, of which seven patients have DSD by androgen excess, a case with sex reversal a ovotesticular DSD, a case with malformation syndrome, one with gonadal dysgenesis and one hypogonadism; b) Four patients presented DSD 46, XY (a patient with Smith-Lemli-Opitz syndrome II, one malformation syndrome and two cases with hypogonadism) c) A case of ADSs by chromosomal abnormality 46,XXY (Klinefelter syndrome). In relation to age of first consultation, the majority (47.1%) was performed in children under 5 years, referred by sexual ambiguity with need to address sex identification. In puberty, the patients consult due to alterations in secondary sexual characteristics and amenorrhea in teenagers, in adulthood due to infertility. The results helped to make a better genetic counseling and to improve the quality of life of patients and their families.

La Fibrosis Quística (FQ) es una enfermedad genética, causada por mutaciones en el gen de la proteína reguladora de la conductancia transmembrana (CFTR). Actualmente se han identificado más de 1900 mutaciones, la más común es pF508del predominante en Europa. En Venezuela se han reportado pocas mutaciones: pF508del, pG542X, pR553X, 3120+1G'A. El objetivo del trabajo fue determinar la frecuencia de seis mutaciones del gen CFTR en pacientes de la región Centro-Occidental de Venezuela e identificar focos geográficos de la enfermedad. Se incluyeron sesenta pacientes no consanguíneos, con características clínicas consistentes con diagnóstico de FQ y sesenta familiares sanos. Se revisaron las historias clínicas para investigar las variables: sexo, edad al momento del diagnóstico, concentración de cloruros en sudor, lugar de nacimiento del paciente y los abuelos. Una vez obtenido el consentimiento informado, se tomó una muestra de 2 mL de sangre para extraer ADN y se indagaron las mutaciones: pF508del, pG542X, pR553X, pG551D, pN1303K y pG85E. Las mutaciones encontradas y sus frecuencias fueron: pF508del (39,16 %), pG542X (4,17 %) y pR553X (2,50%); el 54,17% son mutaciones diferentes a las seis investigadas y se les denominó OTRA. Se identificaron siete genotipos, pero sólo 25% son completos (pF508del/pF508del, pF508del/pG542X y pF508del/pR553X). Se identificaron quince focos geográficos de FQ ubicados en las regiones siguientes: a)Centro-Occidental (05), b)Centro-Norte Costera (03), c)Central (01), d)Sur-Oeste (03), e) Sur-Este (01) y f)Oriental (02). Estos resultados contribuyen a mejorar la calidad de vida de los pacientes y sus familias y son importantes para el Programa Nacional de FQ.

The Cystic Fibrosis (CF) is a genetic disease caused by mutations in the CF transmembrane conductance regulator gene (CFTR). At present, more than 1900 mutations have been identified. The most common CFTR mutation is pF508del, it is predominant in Europe. Few mutations have been reported in Venezuela: pF508del, pG542X, pR553X, 3120+1G'A. The objective of this work was to determine the frequency of six CFTR mutations in patients from the Western Central Region of Venezuela and to identify CF geographical focuses. Sixty unrelated patients with clinical features consistent with CF diagnosis and sixty healthy relatives were included. The clinical records were reviewed to investigate: sex, age at diagnosis, sweat chloride values and birthplace of patients and grandparents. Two mL of whole blood samples were drawn from patients, after informed consent and DNA was extracted; the pF508del, pG542X, pR553X, pG551D, pN1303K and pG85E mutations were investigated. The mutations found and their frequency were: pF508del (39.16 %), pG542X (4.17 %) and pR553X (2.50%). There were 54.17% different mutations from the six investigated and they were called OTHER. Seven genotypes were identified, but only 25% are complete (pF508del/pF508del, pF508del/pG542X and pF508del/pR553X). Fifteen CF geographical focuses were identified, which are located in the following regions: a) Western Central (05), b) North Central Coast (03), c) Central (01), d) South-West (03), e) South-East (01), f) Oriental (02). The results of this research contribute to improve the quality of life in patients and their families, and they are important for the National CF Program.

La Fibrosis Quística (FQ) es una enfermedad autosómica recesiva multisistémica de evolución crónica progresiva y letal. Varios estudios de correlación fenotipo-genotipo han identificado mutaciones relacionadas con formas clínicas severas y otras leves, esta asociación aún no ha sido investigada en los pacientes FQ diagnosticados en la Unidad Proyecto Aragua (UPA) de la Universidad de Carabobo Sede Aragua. Por tal motivo esta investigación es realizada a partir de la revisión de diez historias clínicas de pacientes con diagnóstico de FQ realizado en la UPA y atendidos en el Hospital Central de Maracay durante el período 1997-2007. El objetivo del trabajo fue determinar las características fenotípicas y relacionarlas con el genotipo. Los genotipos presentes fueron DeltaF508/Otra (33,3%), G542X/Otra (11,5%), Otra/Otra (55,6%); se formaron dos grupos según el genotipo: El Nº1, integrado por individuos con genotipo compuesto, donde uno de los alelos corresponde a una mutación severa; y en el Nº 2 se incluyeron los pacientes con mutaciones no identificadas y un paciente fallecido antes de investigar el genotipo. El promedio de la edad al diagnóstico es 1,92 años en el grupo 1 y 4,26 años en el grupo 2. La insuficiencia pancreática (IP) y las complicaciones digestivas son más severas en el grupo 1; los pacientes del grupo 2 presentan mayor heterogeneidad clínica. Se concluye que el genotipo es un buen predictor de la función exocrina pancreática ya que todos los pacientes con IP, tienen al menos una mutación severa. Se manifiesta la necesidad de indagar otras mutaciones y sugerimos un diagnóstico precoz y selectivo, que ayude a prevenir las complicaciones de esta patología y mejorar la calidad de vida de pacientes y sus familias.

Cystic fibrosis is an autosomal recessive and multisystemic disease of chronic progressive and lethal evolution. Several studies of fenotype-genotype correlations have identified mutations related to clinical severe and mild forms, this association still has not been investigated in the FQ patients diagnosed in the Unidad Proyecto Aragua (UPA) of Universidad de Carabobo Sede Aragua (University of Carabobo, Aragua Campus). This is a descriptive and retrospective investigation carried out from the review of ten patients’ case histories with FQ diagnosed in the UPA and attended at the Central Hospital of Maracay during the period 1997-2007. The aim of this work was to determine the phenotypic characteristics and to relate them to the genotype. The genotypes found were DeltaF508/Other (33.3%), G542X/Other (11.5%), Other / Other (55.6%); the patients were classified according to genotype in two groups: Group Nº 1 integrated by individuals with composed genotype, where one of the allele corresponds to a severe mutation; and in group Nº 2 were included the patients with not identified mutations and a deceased patient before investigating the genotype. The mean age at diagnosis was 1.92 years in group 1 and of 4.26 years in group 2. The pancreatic insufficiency (IP) and the digestive complications are severe in group 1; the patients of group 2 showed major clinical heterogeneity. It is concluded that the genotype is a good predictor of the exocrine pancreatic function, since all the patients with IP, have at least a severe mutation. It demonstrates the need to investigate other mutations and we suggest a selective and early diagnosis, to help prevent the complications of this pathology and improve the quality of patients’ life and his families.

Seven polymorphic sites in the beta-globin gene cluster were analyzed on a sample of 96 chromosomes of Venezuelan sickle cell patients from the State of Aragua. The Benin haplotype was predominant with a frequency of 0.479, followed by the Bantu haplotype (0.406); a minority of cases with other haplotypes was also identified: atypical Bantu A2 (0.042), Senegal (0.031), atypical Bantu A7 (0.021) and Saudi Arabia/Indian (0.021) haplotypes; however, the Cameroon haplotype was not identified in this study. Our results are in agreement with the historical records that establish Sudanese and Bantu origins for the African slaves brought into Venezuela.
