Abstract The CRISPR/Cas9 system is a simpler and more versatile method compared to other engineered nucleases such as Zinc Finger Nucleases (ZFNs) and Transcription Activator-Like Effector Nucleases (TALENs), and since its discovery, the efficiency of CRISPR-based genome editing has increased to the point that multiple and different types of edits can be made simultaneously. These advances in gene editing have revolutionized biotechnology by enabling precise genome editing with greater simplicity and efficacy than ever before. This tool has been successfully applied to a wide range of animal species, including cattle, pigs, dogs, and other small animals. Engineered nucleases cut the genome at specific target positions, triggering the cell's mechanisms to repair the damage and introduce a mutation to a specific genomic site. Over the years it has been possible to notice the growth of applications for educational purposes available on digital platforms. Many of these apps are used in teaching and learning processes, however few are evaluated regarding their theoretical and practical aspects. ABSTRACT Introduction: Immune checkpoints are regulators of the immune system response that allow self-tolerance. Molecules such as Programmed Cell Death Protein 1 (PD-1) and its Ligand (PD-L1) participate in the immune checkpoint by signaling co-inhibition of lymphocyte responses. In cancers, PD-L1 expression is associated with the immune evasion mechanism, which favors tumor growth. The use of anti-PD-1/PD-L1 drugs is already well described in solid tumors, but still not fully understood in hematologic malignancies. Myelodysplastic neoplasms (MDSs) are heterogeneous bone marrow disorders with an increased risk of progression to Acute Myeloid Leukemia (AML). The MDS affects hematopoietic stem cells and its pathogenesis is linked to genetic and epigenetic defects, in addition to immune dysregulation. The influence of the PD-L1 on the MDS remains unknown. Methods: In this study, we evaluated the mRNA expression of the PD-L1 in 53 patients with MDS, classified according to the WHO 2016 Classification. Results: Patients with dyserythropoiesis presented significantly higher PD-L1 expression than patients without dyserythropoiesis (p = 0.050). Patients classified as having MDS with an excess of blasts 2 (MDS-EB2) presented a significant upregulation in the mRNA expression of the PD-L1 compared to the MDS with an excess of blasts 1 (MDS-EB1) (p = 0.050). Furthermore, we detected three patients with very high levels of PD-L1 expression, being statistically classified as outliers. Conclusion: We suggested that the high expression of the PD-L1 is associated with a worse prognosis in the MDS and functional studies are necessary to evaluate the possible use of anti-PD-L1 therapies for high-risk MDS, such as the MDS-EBs. Recognizing the significant effects of the New Information and Communication Technologies (NICTs), the work reports the experience of a reflection group on the following question: does the massive and pervasive use of technologies in life, and therefore in health, deserve its own concept for analytical and sociopolitical emphasis? Through the experience systematization proposed by Holliday (2006), we propose the concept of Digital Determinants of Health (DDH) as it reflects the need to better highlight NICTs’ actions in the production of health - both from an epistemological and sociopolitical point of view. By suggesting the DDH concept, we urge the academic community to a more specific debate on the consequences of NICTs in contemporary life, in order to guide actions capable of mitigating negative effects and leveraging the benefits of new technologies in healthcare.

Reconociendo los efectos significativos de las Nuevas Tecnologías de la Información y la Comunicación (NTIC), este artículo relata la experiencia de un grupo sobre la cuestión: ¿merece el uso masivo y omnipresente de las tecnologías en la vida, y en consecuencia en la salud, un concepto propio para su prominencia analítica y sociopolítica? Utilizando la sistematización de Holliday (2006), el grupo propuso el concepto de Determinantes Digitales de la Salud (DDS) por la necesidad de destacar la acción de las NTIC en la salud, tanto desde el punto de vista epistemológico como sociopolítico. Con el concepto de DDS, instamos a la comunidad académica a mantener un debate más específico sobre las consecuencias de las NTIC en la vida contemporánea para orientar acciones capaces de mitigar los efectos negativos y maximizar los beneficios de las nuevas tecnologías en la salud.

Por meio da sistematização de experiências de Holliday (2006), o grupo propõe o conceito de Determinantes Digitais da Saúde (DDS) por entender a necessidade de evidenciar com mais força a ação das NTICs na produção da saúde — tanto do ponto de vista epistemológico, quanto do sociopolítico. Com a sugestão do conceito de DDS, exortamos a comunidade acadêmica a um debate mais específico sobre as consequências das NTICs na vida contemporânea para a orientação de ações capazes de mitigar os efeitos negativos e potencializar os benefícios das novas tecnologias na saúde.

RESUMO Analisaram-se, na percepção de profissionais da Atenção Primária à Saúde (APS), as dificuldades no cuidado em Saúde Mental relacionadas ao uso de Substâncias Psicoativas (SPA), à luz de reflexões sobre os tensionamentos presentes na saúde pública no que se refere ao seu uso. Foi conduzido estudo exploratório qualitativo em 11 municípios da Macrorregião de Saúde Oeste do estado de Minas Gerais, com entrevistas semiestruturadas e Grupos Focais com 145 profissionais da APS. O material foi analisado a partir do referencial de Análise de Conteúdo. As principais dificuldades identificadas estavam relacionadas à organização e à gestão do trabalho em saúde, ao estigma sobre usuários com demandas relacionadas ao uso de SPA, à falta de reflexões e práticas em saúde que considerem a realidade dos usuários em uso de SPA, à ‘cultura da medicação’ e à desarticulação da Rede de Atenção Psicossocial. É fundamental o desenvolvimento de políticas e práticas que permitam aos trabalhadores e às estruturas de saúde uma maior reflexão acerca da relação entre o uso de SPA e as possibilidades de lidar com o sofrimento, expresso de diferentes formas pelos usuários dos serviços de saúde.

Abstract This work aims to investigate the drivers of the intensity of the interaction between small and medium-sized (SMEs) Brazilian enterprises and universities and public research institutes. Data from the Directory of Research Groups in Brazil in the areas of Engineering and Agricultural Sciences and the Annual Report of Social Information were used to estimate four econometric models that explore the drivers related to the structural and behavioural characteristics of the interactive companies, their relationships with research groups, and the science, technology, and innovation policy. The results show that the size, previous experience in collaboration, and absorptive capacity, which reflect the company’s capabilities and internal resources to cooperate, in addition to access to public funding, are factors that influence the intensity of the interaction of SMEs. Resumo O objetivo deste trabalho é investigar os fatores direcionadores (drivers) da intensidade da interação entre pequenas e médias empresas (PMEs) brasileiras e universidades e institutos públicos de pesquisa. Considering the chemical and pharmacological properties of semicarbazones, an interdisciplinary didactic sequence was developed to be applied in experimental classes of undergraduate courses in Pharmacy, Chemistry and related areas. This didactic sequence was planned in three steps, starting from the planning of the target molecule, a semicarbazone derived from helional, to its synthesis, physicochemical characterization (CCD, melting point, FTIR and UV-Vis) and in silico tests of its pharmacokinetic properties using the free software ADMETlab 2.0. The proposed target molecule has as molecular scaffold the 1,3-benzodiaxole nucleus and the semicarbazone function, aiming to obtain a molecule with promising anticonvulsant action and that can be obtained by a low cost and easy to execute methodology, which makes it attractive for use in experimental classes. Abstract The current scenario of neoliberal political maneuvers in Brazil brought several legislative changes regarding health care policies which alter many services’ organizational forms and funding. These changes also come through as precarious work and threaten advances and accomplishments related to the Brazilian Psychiatric Reform and mental health care. This study aimed to identify and analyse difficulties regarding the construction of a mental health (MH) policy in Primary Health Care (PHC) according to professionals working at 11 teams of the Family Health Support Center (NASF). We developed a qualitative study through 11 semi-structured interviews conducted with 11 MH workers in PHC and explored the results using content analysis. The identified difficulties were precarious forms of employment; high professional turnover; insufficient working hours; low wages; working hours concentrated in care activities; lack of integration and communication among services and workers and disarticulation of the psychosocial attention network (RAPS). Matrix support was not present and there were no Permanent Health Education policies in the municipalities studied. Nevertheless, the NASF contributed to improve mental healthcare. Abstract Objective: To descriptively analyze the epidemiological data, clinical stage, and outcomes of oropharyngeal squamous cell carcinoma in the state of São Paulo, Brazil, and to estimate the influence of clinical stage and treatment type on overall and disease-free survival. Methods: We retrospectively analyzed epidemiological data from the São Paulo Cancer Center Foundation database relative to patients with oropharyngeal squamous cell carcinoma diagnosed between 2004 and 2014 in the state of São Paulo. Univariate and multivariate Cox regression analyses were performed to assess factors associated with the outcomes. A forward stepwise selection procedure was used. Survival curves were estimated by the Kaplan-Meier method and compared by the Gehan-Breslow-Wilcoxon test. Results: A total of 8075 individuals with oropharyngeal squamous cell carcinoma were identified. Of these, 86.3% were diagnosed at an advanced stage and 13.7% at an early stage. Only 27.2% of patients were treated surgically, whereas 57.5% were treated medically. Patients undergoing surgery had longer overall survival than those receiving medical treatment in both early- and advanced-stage oropharyngeal squamous cell carcinoma. However, there was no significant difference in disease-free survival between surgical and medical treatment. Conclusion: No significant difference in disease-free survival between medical and surgical treatment suggests similar complete remission rates with both approaches. Patients receiving medical treatment had shorter overall survival, which may be due to complications from chemotherapy and radiotherapy. However, we cannot confirm this relationship based on the data provided by the São Paulo Cancer Center Foundation. Prospective studies are warranted to assess whether the lower overall survival rate in patients receiving medical treatment is secondary to complications from chemotherapy and radiotherapy. Objetivo analizar la percepción de los profesionales de la Atención Primaria a la Salud sobre los retos, así como elaborar estrategias de superación para actuar en la Red Cigüeña. Métodos: Grupos focales con 34 profesionales de las Estrategias de Salud de la Familia (ESF) y Unidades Básicas de Salud (UBS) que monitorizan a las embarazadas. El análisis realizado fue por medio del análisis de contenido, modalidad temática. Resultados: Se han tratado dos temas en este artículo: Retos y propuestas para la actuación de los profesionales de la ESF en la Red de Atención a la Salud y la Red Cigüeña; y, Retos y propuestas para la actuación de los profesionales de las UBS en la Red de Atención a la Salud y la Red Cigüeña. Conclusión: Se identificó la fragilidad en el trabajo en red, como el establecimiento de los flujos, la baja capacidad de resolución de las situaciones frente a los procesos de comunicación formal y la rotación de los profesionales de la salud. Las propuestas de superación de los retos pasan por la reconstrucción de los flujos entre los servicios y la implantación de la Red Cigüeña en el movimiento de cogestión.

Abstract Introduction The Stork Network (RC) aims to articulate maternal and child actions, seeking a humanized and comprehensive care. However, there are challenges to be understood in the process of implementing this public policy in Brazil. Objective To analyze the perception of Primary Health Care professionals about the challenges and their overcoming strategies in the implementation of care in the Health Care Network (RAS) and in the Stork Network. Methods Focus groups were performed with two Family Health Teams (FHTs) and two Basic Health Units (BHUs) that follow pregnant women, totaling 35 participants. The content analysis of the collected data was performed, using the thematic modality. Results It was observed that networking has not shown a good performance in the municipality, showing a deficiency in relation to the establishment of flows, with little capacity to resolve situations in the face of formal communication processes. The proposals for overcoming the challenges go through the reconstruction of flows, protocols and communication between services, but also include a co-management movement. Conclusion Further studies are necessary considering the new public policies implemented in Brazil, which creates challenges in the face of the proposed integrality in health care.

Resumo Introdução: A Rede Cegonha (RC) visa articular as ações materno-infantil, buscando um atendimento humanizado e integral. Porém, há desafios a serem compreendidos no processo de implementação dessa política púplica no Brasil. Objetivo: Analisar a percepção dos profissionais da Atenção Primária à Saúde sobre os desafios e suas estratégias de superação na implementação do cuidado na Rede de Atenção à Saúde e na Rede Cegonha. Métodos: Grupos focais com duas equipes Saúde da Família (eSF) e duas de Unidade Básica de Saúde (UBS) que acompanham as gestantes, totalizando 35 participantes. Realizada análise de conteúdo, modalidade temática com os dados coletados. Resultados: Percebe-se que o trabalho em rede não tem funcionado no município, tendo deficiência com relação ao estabelecimento dos fluxos, com pouca capacidade de resolução das situações frente aos processos de comunicação formal. As propostas de superação dos desafios passam pela reconstrução dos fluxos, protocolos e comunicação entre os serviços, mas também num movimento de cogestão. Conclusão: Outras investigações são necessárias frente às novas políticas públicas implementadas no Brasil, o que gera desafios frente à proposta de integralidade no cuidado em saúde.

ABSTRACT Background: Dysphagia is characterized by difficulty in the swallowing pattern at any stage of this neuromuscular process. It is a frequent symptom after stroke. Objective: This study aimed to investigate the most commonly used phonoaudiological interventions as therapy for the treatment of swallowing disorders in patients with dysphagia after stroke. Methods: We performed a review of studies indexed in MEDLINE-PubMed, LILACS, Cochrane, and Clinical trials.gov focusing on speech-language interventions for adult dysphagic patients after stroke between January 2008 and January 2021. Results: Thirty-six articles of clinical trials were selected. Eleven different types of therapies have been studied. Studies on the efficacy of therapeutic interventions for the rehabilitation of adult patients with dysphagia after stroke are still scarce. Most techniques are combined with conventional therapy, so the effectiveness of the other techniques alone cannot be assessed. Conclusions: Therapeutic interventions should be selected in accordance with the possibilities and limitations of the patients, and especially with the findings of the clinical evaluation and with its objective. RESUMO Antecedentes: A disfagia é caracterizada como uma dificuldade no padrão de deglutição em qualquer fase desse processo neuromuscular. É um sintoma frequente após o Acidente Vascular Cerebral. Objetivos: O objetivo deste estudo foi investigar as intervenções fonoaudiológicas mais utilizadas como terapia para o tratamento dos distúrbios da deglutição em pacientes com disfagia pós AVC. Métodos: Realizamos uma revisão dos estudos indexados no MEDLINE-PubMed, LILACS, Cochrane e Clinical trials.gov com foco nas intervenções fonoaudiológicas em pacientes adultos e disfágicos após AVC entre janeiro de 2008 e janeiro de 2021. Resultados: Foram selecionados trinta e seis artigos de ensaios clínicos e estudados onze tipos de terapia. Os estudos sobre a eficácia de intervenções terapêuticas para a reabilitação destes pacientes adultos ainda são restritos. A maioria das técnicas é aplicada em combinação com a terapia convencional, tornando inconclusiva a medição da eficácia de outras técnicas isoladamente. Conclusões: As intervenções terapêuticas devem ser escolhidas de acordo com as possibilidades e limitações dos pacientes e, principalmente, com os achados da avaliação clínica e seu objetivo.
