Abstract Aim: Cardiovascular physiology learned by exercise science students is often quickly forgotten. We tested whether a state rotation model would help students to recall key principles of Cardiovascular Physiology (CV). Methods: Seventy-one undergraduate students enrolled in the Exercise Physiology Course at the School of Physical Education and Sport, University of São Paulo, participated in the study. The students were randomly assigned into one of 4 stations, dedicated to recalling the concepts of the heart as a pump (e.g. preload, post-load, and contractility; station 1) and hemodynamics (e.g. serial and parallel conductance; station 2) by using the educational tool. Heart rate (HR) control by sympathetic nervous system activation (station 3) and HR control by vagal activation (station 4) were assessed by quantifying HR response to the Stroop color and word test and during face immersion in cold water, respectively. To evaluate the efficacy of the intervention, we used a Socrative app to launch eight multiple-choice questions before (PRE) and after (POST) the student's station rotation. The questions were related to the basic principles of exercise physiology and its consequences on the cardiovascular system. Results: The 4-station average score (% of corrected answers) achieved after the station rotation was higher than the score achieved before (71.21%, SD 14.50 vs. 31.07%, SD 18.04; for POST and PRE, respectively p < 0.005). Considering specific stations, the lowest score of corrected answers before the rotation was observed at station 2- hemodynamics when compared with station 1-heart as a pump and station 3/4 - autonomic control (18.9%, SD 0.9 vs. 46.5, SD 24.1 and 34.8, SD 2.1 for hemodynamics, heart as a pump and autonomic control, respectively). Interestingly, after the rotation, there was a significant increase in corrected scores for all stations (33.9, SD 9.8; 80.5, SD 4.6 and 90.2, SD 2.3, for hemodynamics, heart as a pump, and autonomic control, respectively). Conclusion: Our results suggest that the use of the educational tool was effective to recall CV principles that are essential to a better understanding of the CV responses to exercise and applying the concepts in exercise testing and prescription for different populations.

Abstract Aim: To evaluate the use of CMAP tools® software as a teaching strategy of skeletal muscle components and contraction in undergraduates students of physical education. Methods: Nineteen students from the discipline of fundamentals of human physiology of the undergraduate course of physical education of the Centro Universitário Padre Anchieta were engaged in this study. Students were asked to divide into groups of three people and to bring a notebook with CMAP tools® software installed. During class, students were responsible for creating conceptual maps in CMAP tools® software describing each step of skeletal muscle components and contraction. To assess the effectiveness of this strategy in assisting learning related to skeletal muscle components and contraction, we created a visual learning scale (ranging from 0 (did not help) to 10 (helped a lot)) and applied to students at the end of the class. Results: From the students assessed, 47% scored 10 on the visual learning scale. Overall, 74% scored higher than 8 on the scale. Conclusion: The use of CMAP tools® software was effective in helping students understand concepts related to skeletal muscle components and contraction.

OBJETIVO: Avaliar a insulinemia em pacientes com dermatomiosite virgens de glicocorticoide e avaliar a resistência insulínica, utilizando o modelo de avaliação da homeostase de resistência insulínica (HOMA2-IR). MÉTODOS: Este estudo transversal incluiu 25 pacientes com dermatomiosites, não-diabéticos e sem uso prévio de glicocorticoides. Para o grupo de controle, 50 voluntários foram pareados por idade, gênero, etnia, peso e estatura. O índice HOMA2-IR foi calculado a partir de dados basais de insulina e glicose. Os parâmetros do International Myositis Assessment & Clinical Studies Group (IMACS) foram utilizados para avaliar o status da doença. RESULTADOS: A méda de idade dos pacientes foi de 43,5 anos, predominantemente do sexo feminino. Os pacientes apresentaram baixa atividade de doença de acordo com os parâmetros do IMACS. O índice de massa corporal e a circunferência da cintura foram maiores no grupo da dermatomiosite em comparação com o grupo controle. O nível de insulina e o HOMA2-IR também foram maiores em pacientes com dermatomiosite. Além disso, analisando a dermatomiosite isoladamente, o índice HOMA2-IR correlacionou-se positivamente com o peso, o índice de massa corporal e a circunferência da cintura e foi independente dos parâmetros de status da doença. CONCLUSÕES: Pacientes com dermatomiosite apresentam valores mais elevados de insulinemia basal, resistência à insulina, índice de massa corporal e circunferência da cintura. Além disso, o HOMA2-IR está moderadamente correlacionado com esses parâmetros antropométricos. Essas anormalidades metabólicas estão relacionadas ao desenvolvimento da síndrome metabólica, uma das principais comorbidades observadas na dermatomiosite.

OBJECTIVE: To evaluate insulinemia in glucocorticoid naïve patients with dermatomyositis and to evaluate insulin resistance using the homeostatic model assessment of insulin resistance (HOMA2-IR). METHODS: This cross-sectional study included 25 dermatomyositis, non-diabetic glucocorticoid naïve patients. The control group consisted of 50 volunteers matched for age, gender, ethnicity, weight and height. The HOMA2-IR index was calculated from baseline insulin and glucose data. The International Myositis Assessment & Clinical Studies Group (IMACS) parameters were used to evaluate disease status. RESULTS: Mean age of the patients was 43.5 years and these were predominantly females. Patients had low disease activity according to IMACS parameters. Higher body mass index and waist circumference were observed in the dermatomyositis group compared to the control group. Insulin level and HOMA2-IR were also higher in patients with dermatomyositis. Moreover, analyzing dermatomyositis alone, the HOMA2-IR index correlated positively with weight, body mass index and waist circumference and was independent on disease status parameters. CONCLUSIONS: Patients with dermatomyositis had higher values for basal insulinemia, insulin resistance, body mass index and waist circumference. Moreover, HOMA2-IR moderately correlated with these anthropometric parameters. These metabolic abnormalities are related to the development of metabolic syndrome, one of the main comorbidities observed in dermatomyositis.

Abstract Systemic autoimmune myopathies (SAMs) are a heterogeneous group of rare systemic autoimmune diseases that primarily affect skeletal muscles. Patients with SAMs show progressive skeletal muscle weakness and consequent functional disabilities, low health quality, and sedentary lifestyles. In this context, exercise training emerges as a non-pharmacological therapy to improve muscle strength and function as well as the clinical aspects of these diseases. Because many have feared that physical exercise exacerbates inflammation and consequently worsens the clinical manifestations of SAMs, it is necessary to evaluate the possible benefits and safety of exercise training among these patients. The present study systematically reviews the evidence associated with physical training among patients with SAMs.
