ABSTRACT BACKGROUND AND OBJECTIVES: Home-based exercises can improve function and quality of life in shoulder pain patients. Knowing the facilitators and barriers is crucial for adherence to shoulder pain treatment. It is believed that individuals who adhere to home exercises have fewer environmental barriers, pain intensity and shoulder disability. The aim of this study is to identify facilitators and barriers for adherence to a home-based exercise program in shoulder pain individuals, and to investigate the influence of environmental barriers, shoulder pain and disability, and kinesiophobia on adherence to a program. METHODS: This is a cross-sectional study. Shoulder pain individuals performed home-based exercises for eight weeks based on a booklet in addition to supervised physical therapy or as their sole therapy. They reported the perceived facilitators and barriers to home exercises adherence and answered the Craig Hospital Inventory of Environmental Factors (CHIEF), Shoulder Pain and Disability Index (SPADI), and the Tampa Scale for Kinesiophobia (TSK) questionnaires. Logistic regression models analyzed the scores of the CHIEF, SPADI, and TSK as predictors of adherence. RESULTS: Fifty individuals participated in this study and 88% adhered to the program. The most frequent facilitator and barrier were “having guidance from a professional,” and “lack of available time,” respectively. Pain and disability were the only predictor of adherence (p=0.044), and an increase of one point in SPADI reduced the likelihood of adherence to home exercises by 14% (Exp(B) =0.86). CONCLUSION: Individuals reported facilitators and barriers to the exercise program. Environmental barriers assessed using the CHIEF and kinesiophobia were not predictors of adherence.

RESUMO JUSTIFICATIVA E OBJETIVOS: Os exercícios domiciliares podem melhorar a função e a qualidade de vida de pacientes com dor no ombro. Conhecer facilitadores e barreiras é fundamental para adesão ao tratamento da dor no ombro. Acredita-se que indivíduos que aderem aos exercícios domiciliares apresentem poucas barreiras ambientais, baixa intensidade de dor e incapacidade do ombro. O objetivo deste estudo foi identificar facilitadores e barreiras para a adesão a um programa de exercícios domiciliares em indivíduos com dor no ombro e investigar a influência de barreiras ambientais, da dor e incapacidade no ombro, e da cinesiofobia na adesão ao programa. MÉTODOS: Neste estudo transversal, indivíduos com dor no ombro realizaram exercícios domiciliares por oito semanas com base em uma cartilha além da fisioterapia supervisionada ou como sua única terapia. Os indivíduos relataram facilitadores e barreiras percebidos para a adesão aos exercícios em casa e responderam ao Craig Hospital Inventory of Environmental Factors-Brasil (CHIEF-Br), Shoulder Pain and Disability Index-Brasil (SPADI-Br) e Escala Tampa de Cinesiofobia (TAMPA). Os escores do CHIEF-Br, SPADI-Br e TAMPA foram analisados como preditores de adesão por meio de modelos de regressão logística. RESULTADOS: Participaram do estudo 50 indivíduos e 88% aderiram ao programa. O facilitador e a barreira mais frequentes foram “receber orientação de um profissional” e “falta de tempo disponível”, respectivamente. Dor e incapacidade foram os únicos preditores de adesão (p=0,044), e o aumento de um ponto no SPADI-Br reduziu a probabilidade de adesão aos exercícios domiciliares em 14% (Exp(B) =0,86). CONCLUSÃO: Os indivíduos relataram facilitadores e barreiras ao programa de exercícios. A barreiras ambientais avaliadas por meio do CHIEF-Br e a cinesiofobia não foram preditores da adesão.

RESUMO Durante a pandemia de COVID-19, observou-se um menor nível de prática de atividade física pela população, o que pode influenciar o nível de autoeficácia para dor na população com dor musculoesquelética. Neste contexto, o objetivo deste estudo foi analisar se existe associação entre o tempo de prática de atividade física e o nível de autoeficácia para dor na população com dor musculoesquelética durante a pandemia de COVID-19 no estado de São Paulo, controlada por variáveis biopsicossociais. Realizou-se um estudo através de um formulário online com questões sobre aspectos sociodemográficos, tempo semanal de prática de atividade física, níveis de estresse e ansiedade, intensidade de dor e autoeficácia para dor (PSEQ-10 - Pain Self-Efficacy Questionnaire). A análise estatística ocorreu por meio de dois modelos de regressão linear múltipla, com (modelo A) e sem (modelo B) o controle dos dados por fatores psicoemocionais (ansiedade e estresse) em 150 pessoas. Foi encontrada associação entre o tempo de prática de atividade física semanal e o nível de autoeficácia para dor no modelo A (p=0,0271, β=1,914) e no modelo B (p=0,0333, β=1,826). Intensidade de dor durante a pandemia, índice de massa corporal (IMC) e sexo, dentre as variáveis de controle, também foram associadas ao nível de autoeficácia para dor. Maior tempo de prática de atividade física foi associado a maior nível de autoeficácia para dor na população com dor musculoesquelética durante a pandemia de COVID-19. Intensidade de dor durante a pandemia, IMC e sexo também foram associados ao nível de autoeficácia para dor.

ABSTRACT During the COVID-19 pandemic, a lower level of physical activity was observed in the population, which may influence the level of pain self-efficacy in the population with musculoskeletal pain. In this context, this study sought to analyze whether there is an association between the time of physical activity practice and the level of pain self-efficacy in the population with pain during the COVID-19 pandemic in the state of São Paulo, controlled by biopsychosocial variables. This study was conducted via an online form with questions about sociodemographic aspects, weekly physical activity practice, stress and anxiety levels, pain intensity and pain self-efficacy (Pain Self-Efficacy Questionnaire - PSEQ-10). The analysis was determined by two models of multiple linear regression, with (Model A) and without (Model B) the control of data by psycho-emotional factors (anxiety and stress) in 150 subjects. An association was found between weekly physical activity practice time and pain self-efficacy level, Model A (p=0.0271, β=1.914) and Model B (p=0.0333, β=1.826). Pain intensity during the pandemic, body mass index (BMI) and sex, among the control variables, also were associated with the pain self-efficacy level. A higher time of physical activity practice was associated with a higher pain self-efficacy level in the population with musculoskeletal pain during the COVID-19 pandemic. Pain intensity during the pandemic, BMI and sex also were associated with pain self-efficacy level.

RESUMEN Durante la pandemia del COVID-19 se observó un menor nivel de actividad física en la población, lo que puede influir en el nivel de autoeficacia para el dolor en la población con dolor musculoesquelético. En ese contexto, el objetivo de este estudio fue analizar si existe asociación entre el tiempo de práctica de actividad física y el nivel de autoeficacia para el dolor en la población con dolor musculoesquelético durante la pandemia del COVID-19 en el estado de São Paulo, controlado por variables biopsicosociales. Se realizó un estudio mediante un formulario por Internet con preguntas sobre los aspectos sociodemográficos, el tiempo semanal de actividad física, los niveles de estrés y ansiedad, la intensidad del dolor y la autoeficacia del dolor (PSEQ-10 - Pain Self-Efficacy Questionnaire). El análisis estadístico se realizó utilizando dos modelos de regresión lineal múltiple, con (modelo A) y sin (modelo B) control de datos para factores psicoemocionales (ansiedad y estrés) en 150 personas. Se encontró asociación entre el tiempo de práctica de actividad física semanal y el nivel de autoeficacia para el dolor en el modelo A (p=0,0271, β=1,914) y en el modelo B (p=0,0333, β=1,826). La intensidad del dolor durante la pandemia, el índice de masa corporal (IMC) y el sexo, entre las variables de control, también se asociaron con el nivel de autoeficacia para el dolor. La práctica de actividad física durante más tiempo se asoció con un mayor nivel de autoeficacia para el dolor en la población con dolor musculoesquelético durante la pandemia del COVID-19. La intensidad del dolor durante la pandemia, el IMC y el género también se asociaron con el nivel de autoeficacia para el dolor.

RESUMO JUSTIFICATIVA E OBJETIVOS: A pandemia de COVID-19 impactou a prática de atividade física da população com dor musculoesquelética (DME), e um dos modos de ampliar a realização da atividade física é estimulando a autoeficácia para dor. Entretanto, até o momento, não se observou na literatura qualquer estudo que trouxesse a relação entre autoeficácia e atividade física nessa população. O objetivo deste estudo foi analisar a associação do nível de autoeficácia e do tempo semanal de prática de atividade física na população com dor durante o distanciamento social relacionado à pandemia de COVID-19 nos estados do Ceará e São Paulo. MÉTODOS: Este estudo se caracteriza como um estudo transversal com abordagem quantitativa. A coleta de dados foi realizada a partir de um formulário eletrônico. Para identificação da associação entre autoeficácia (Pain Self-Efficacy Questionnaire) e tempo semanal de prática de atividade física (variável dependente) na população com dor, foi realizada uma regressão linear múltipla. RESULTADOS: A autoeficácia apresentou relação direta (ß = 0,015; p = 0,0016) com tempo de atividade física praticada durante a pandemia. Outras variáveis apresentaram associação com maior tempo de prática de atividade física, como menor intensidade de dor durante a pandemia (ß = -0,064; p = 0,0223), menor índice de massa corporal (ß = -0,036; p=0,0004), ser do sexo masculino (ß = -0,441; p<0,0001) e relatar ausência de comorbidades (ß = -0,297; p = 0,0116). O histórico de diagnóstico positivo para COVID-19 não apresentou associação com tempo de atividade física (p = 0,5347). CONCLUSÃO: Identificou-se associação direta entre autoeficácia para dor e tempo de prática de atividade física na população com dor musculoesquelética durante o distanciamento social relacionado à COVID-19.

ABSTRACT BACKGROUND AND OBJECTIVES: The COVID-19 pandemic has impacted the practice of physical activity in the population with musculoskeletal pain, and one of the ways to increase the performance of physical activity is to stimulate pain self-efficacy. However, to date, no study that brought the association between self-efficacy and physical activity in this population has been observed in the literature. The aim of this study was to analyze the association of the level of pain self-efficacy and physical activity weekly time in the population with pain during social distancing related to the COVID-19 pandemic in the states of Ceará and São Paulo. METHODS: This study is characterized as a cross-sectional study with a quantitative approach. Data collection was performed using an electronic form. To identify the association between pain self-efficacy (Pain Self-Efficacy Questionnaire) and physical activity weekly time (dependent variable) in the population with pain, a multiple linear regression was performed. RESULTS: Self-efficacy showed a direct association (ß = 0.015; p = 0.0016) with the time of physical activity practiced during the pandemic. Other variables were associated with longer time of physical activity, such as lower pain intensity during the pandemic (ß = -0.064; p = 0.0223), lower body mass index (ß = -0.036; p = 0.0004), the male gender (ß = -0.441; p<0.0001) and absence of reported comorbidities (ß = -0.297; p = 0.0116). The history of positive diagnosis for COVID-19 was not associated with the physical activity duration (p = 0.5347) in an alternative model. CONCLUSION: A direct association between pain self-efficacy and time of physical activity in the population with musculoskeletal pain during COVID-19-related social distancing was identified.

RESUMO JUSTIFICATIVA E OBJETIVOS: As abordagens cognitivo-comportamentais têm sido aplicadas em pacientes com dor crônica como estratégia de tratamento para redução de sintomas e incapacidade, uma vez que o medo relacionado a dor, a cinesiofobia e a catastrofização podem representar barreiras psicossociais importantes para a recuperação. A relevância do medo e de comportamentos de evitação no desenvolvimento e manutenção de processos de dor crônica incapacitante já é bem estabelecida na literatura científica. CONTEÚDO: A abordagem cognitivo-comportamental fundamenta a relação da dor com comportamentos de defesa e a contribuição para a incapacidade funcional, auxiliando os clínicos a reconhecer sinais destes comportamentos defensivos na prática, além de fornecer estratégias para prática clínica, destacando as abordagens que podem ser utilizadas, como a educação em neurociência da dor e as terapias de exposição. Dessa maneira, é possível fornecer um guia para facilitar a implementação desses conceitos na prática clínica para a abordagem de pessoas com dor musculoesquelética crônica, ajudando os clínicos a fundamentar as teorias de aprendizado do medo e evitação no contexto da dor e a lidar com fatores psicossociais dos pacientes que apresentam associação entre dor e movimento. CONCLUSÃO: O medo relacionado à dor e os pensamentos catastróficos influenciam na intensidade da dor e na incapacidade funcional. O reconhecimento da dor dentro do contexto multidimensional auxilia no estabelecimento de abordagens direcionadas. As abordagens cognitivo-comportamentais baseadas em terapia de exposição têm como enfoque o olhar para além das estruturas do corpo.

ABSTRACT BACKGROUND AND OBJECTIVES: Cognitive-behavioral approaches have been applied in patients with chronic pain as a treatment strategy to reduce symptoms and disability, since fear related to pain, kinesiophobia and catastrophizing may be important psychosocial barriers for recovery. The relevance of fear and avoidance behaviors in the development and maintenance of processes of disabling chronic pain is already well established in the scientific literature. CONTENTS: The cognitive-behavioral approach defines the relationship of pain with defense behaviors and the contribution to functional disability, assisting clinicians to recognize signs of these defensive behaviors during practice, as well as providing strategies for clinical practice, highlighting approaches that can be used, such as pain neuroscience education and exposure therapy. Therefore, it is possible to provide a guide to facilitate the implementation of these concepts in clinical practice for the management of chronic musculoskeletal pain, helping clinicians to ground the theories of fear learning and avoidance in the context of pain and to address the psychosocial factors of patients who present an association between pain and movement. CONCLUSION: Pain-related fear and catastrophic thoughts influence pain intensity and functional disability. Recognizing pain within a multidimensional context assists in establishing targeted approaches. Cognitive-behavioral approaches based on exposure therapy focus on the view beyond body structures.

Abstract Aims: To describe reference values for the Closed Kinetic Chain Upper Extremity Stability Test (CKCUEST) and Upper Quarter Y Balance Test (UQYBT) in young adults, and to determine whether there were differences in both tests based on gender and age. Intrarater reliability and measurement error were also assessed. Methods: Test-retest design with a sample of 146 young adults. The CKCUEST (number of touches, normalized score, and power) and the UQYBT (normalized reach in the medial, superolateral, and inferolateral direction, and the composite score) were used. Mean and standard deviation were calculated for both tests. A linear and a mixed regression model were applied to determine significant differences in test scores. Reliability was determined using Intraclass Correlation Coefficient (ICC) and error measurement through Standard Error of Measurement (SEM) and Smallest Detectable Change (SDC). Results: Normative values were established and divided by gender and age. Men presented greater scores regarding the number of touches, power score, superolateral and inferolateral reaches. Good intrarater reliability was found for both tests. In the CKCUEST, the SEM and SDC values were 1 and 3 touches. In the UQYBT, the SEM values ranged from 3 to 7 cm, while the SDC ranged from 8 to 19 cm. Conclusion: Normative data were provided and men presented greater scores than women. Good intrarater reliability was found and values of SEM and SDC were established. Clinicians must use both tests in clinical practice since different aspects of the upper quarter are assessed.

RESUMO JUSTIFICATIVA E OBJETIVOS: A dor crônica é considerada um problema de saúde pública mundial, pode levar ao estresse físico e emocional, além de altos custos financeiros e sociais para a população. O objetivo deste estudo foi produzir uma revisão sistemática para identificar a prevalência da dor crônica no Brasil, considerando suas regiões geográficas e subclassificações de mecanismos pela International Association for the Study of Pain (IASP). CONTEÚDO: Foi realizada uma revisão sistemática nas seguintes bases de dados: Scielo, Pubmed, Periódicos Capes, Science Direct e Biblioteca Virtual em Saúde. Foram incluídos 35 estudos que investigavam a prevalência de dor crônica no Brasil. A prevalência variou de 23,02 a 76,17%, apresentando média nacional de 45,59% entre os estudos, afetando mais o sexo feminino. A região do Brasil com maior prevalência dentre os estudos incluídos foi a região centro-oeste (56,25%), porém a região com mais estudos e maior população analisada foi a região sudeste (42,2%). Quanto às classificações de mecanismos da IASP, a dor possivelmente nociceptiva obteve prevalência de 36,70%, já a neuropática foi de 14,5% e a dor nociplástica de 12,5%. CONCLUSÃO: O presente estudo identificou alta prevalência de dor crônica no Brasil, maior em mulheres. Em relação aos seus mecanismos, a dor de predomínio possivelmente nociceptivo foi a mais prevalente. Quanto a região geográfica nacional, o destaque de maior prevalência foi para a região Centro-Oeste, porém a região com mais estudos e maior população analisada foi a região Sudeste.

ABSTRACT BACKGROUND AND OBJECTIVES: Chronic pain is considered a worldwide public health problem, can lead to physical and emotional stress, in addition to high financial and social costs for the population. The aim of this study was to produce a systematic review to identify the prevalence of chronic pain in Brazil, considering its geographical regions and mechanisms subclassifications by the International Association for the Study of Pain (IASP). CONTENTS: A systematic review was carried out on the following databases: Scielo, Pubmed, Periódicos Capes, Science Direct and Virtual Health Library. 35 studies that investigated the prevalence of chronic pain in Brazil were included. The prevalence ranged from 23.02 to 76.17%, presenting a national average of 45.33% between studies, affecting more women. The Brazilian region with the highest prevalence among the included studies was the Midwest region (56.25%), however the region with the most studies and the largest population analyzed was the Southeast region (42.2%). Regarding the classifications of IASP mechanisms, possibly nociceptive pain had a prevalence of 36,70%, whereas neuropathic pain was 14,5% and nociplastic pain 12,5%. CONCLUSION: The present study observed a high prevalence of chronic pain in Brazil, being the majority in women. Regarding chronic pain mechanisms, the possibly nociceptive predominance was the most prevalent. As for the national geographic region, the highlight of the highest prevalence of chronic pain was for the Midwest region, however the region with the most studies and the largest population analyzed was the Southeast region.

RESUMO JUSTIFICATIVA E OBJETIVOS: A síndrome do impacto subacromial é uma disfunção da articulação glenoumeral, podendo gerar dor, incapacidade funcional, dependência e baixa qualidade de vida. O objetivo deste estudo foi comparar a força muscular isométrica entre ombro doloroso e não doloroso e verificar a relação entre dor e incapacidade no ombro e força muscular isométrica do ombro e nível de cinesiofobia em indivíduos com sintomas de síndrome do impacto subacromial. MÉTODOS: Participaram do estudo 20 voluntários com sintomas de síndrome do impacto subacromial. A avaliação foi realizada por meio de ficha de avaliação, questionário Shoulder Pain and Disability Index (SPADI - BRASIL) para avaliação da incapacidade e dor no ombro e questionário TAMPA para analisar o nível de cinesiofobia. A avaliação da força muscular isométrica do ombro foi realizada utilizando o instrumento Manual Hand-Held Dynamometer estabilizado por faixa rígida nos movimentos de flexão, extensão, abdução, rotação interna e externa do ombro. RESULTADOS: Na comparação de força muscular isométrica entre lado sintomático e assintomático foi verificada evidência de diferença significativa (p<0,05) para todos os movimentos avaliados. Houve relação negativa significativa entre índice de dor e incapacidade no ombro e força muscular isométrica no movimento de flexão e rotação interna, além de relação positiva significativa com nível de cinesiofobia. CONCLUSÃO: Indivíduos com sintomas de síndrome do impacto subacromial apresentaram redução significativa da força muscular isométrica do ombro sintomático. O índice de dor e incapacidade no ombro está relacionado com força muscular isométrica do ombro e com nível de cinesiofobia.

ABSTRACT BACKGROUND AND OBJECTIVES: The subacromial impingement syndrome is a dysfunction of the glenohumeral joint which can cause pain, functional disability, dependence, and low quality of life. The aim of this study was to compare the muscle strength between painful and non-painful shoulders and to verify the relationship between shoulder pain and disability and isometric muscle strength of the upper limb and kinesiophobia level in individuals with symptoms of subacromial impingement syndrome. METHODS: Twenty volunteers with symptoms of subacromial impingement syndrome participated in the study. The evaluation was carried out by means of an evaluation form, the Shoulder Pain and Disability Index questionnaire (SPADI - BRAZIL) to assess disability and shoulder pain and the TAMPA questionnaire to analyze the level of kinesiophobia. The evaluation of shoulder isometric muscle strength was performed using the Manual Hand-Held Dynamometer stabilized by a rigid band in the movements of flexion, extension, abduction and internal and external rotation of the shoulder. RESULTS: In the comparison of muscle strength between symptomatic and asymptomatic side, evidence of significant difference (p<0.05) was verified for all movements evaluated. There was a significant negative relationship between shoulder pain and disability index and isometric muscle strength of flexion and internal rotation, and a significant positive relationship with the level of kinesiophobia. CONCLUSION: Individuals with symptoms of subacromial impingement syndrome showed a significant reduction in muscle strength of the symptomatic shoulder. The shoulder pain and disability index is related to the isometric muscle strength of the shoulder and to the level of kinesiophobia.

RESUMO Fatores intrínsecos, como nível de escolaridade, idade e gênero, se relacionam com dor e disfunção, assim como a autoeficácia pode modificar o efeito que a dor e disfunção têm sobre resultados clínicos dos pacientes. Este estudo transversal investigou se o escore no Shoulder Pain and Disability Index (SPADI), idade, nível de escolaridade e gênero são preditivos de autoeficácia em pacientes com dor no ombro e se há diferença nos níveis de dor e incapacidade entre grupos de idade e gênero. Foram analisados dados sociodemográficos e pontuações do SPADI e do Chronic Pain Self- Efficacy Scale (CPSS) de um banco de dados de pacientes atendidos em um ambulatório de fisioterapia público especializado em ombro, um total de 123 pacientes com média de idade de 54 (±11,54), SPADI de 67,56 (±22,54) e CPSS de 182,22 (±61,76). A análise de regressão linear múltipla mostrou o SPADI como único fator preditivo de autoeficácia (β=-1,39 [IC95%=-1,84 a -0,93], p<0,001), explicando 23% de sua variância (r2=0,23). A análise de variância (ANOVA) mostrou que a pontuação do SPADI foi significantemente diferente entre gêneros (diferença média=22,27; p<0,001), mas similar entre grupos de idade (diferença média=7,04; p=0,16). Concluiu-se que os pacientes que se queixaram de dor no ombro em um ambulatório de fisioterapia público especializado em ombro foram a maioria mulheres de meia-idade, que cursaram apenas o ensino fundamental, apresentaram dor e incapacidade importantes e uma alta autoeficácia. Sendo que a pontuação no questionário SPADI foi capaz de prever parcialmente a autoeficácia.

ABSTRACT Factors such as schooling level, age and gender are associated with a more intense pain and a higher level of dysfunction in the shoulder and self-efficacy can modify the effect that pain and dysfunction have on patients’ clinical outcomes. Our study investigated if the score on the Shoulder Pain and Disability Index (SPADI), age, schooling level and gender are predictive of self-efficacy in patients with shoulder pain. It also verified if there are differences in levels of pain and disability between age groups and genders. Sociodemographic data and scores from the SPADI and the Chronic Pain Self-Efficacy Scale (CPSS) from a database of patients treated at a public physical therapy clinic specialized in shoulder were analyzed. In total, 123 patients with a mean age of 54 (±11.54), SPADI of 67.56 (±22.54) and CPSS of 182.22 (±61.76) were analyzed. Multiple linear regression analysis showed SPADI as the only predictive factor of self-efficacy (β=-1.39 [95%CI=-1.84 to -0.93], p<0.001), explaining 23% of its variance (r2=0.23). ANOVA showed that the SPADI score was significantly different between genders (mean difference=22.27; p<0.001), but was similar between age groups (mean difference=7.04, p=0.16). We concluded that patients that complained of shoulder pain in a public shoulder physical therapy clinic were middle-aged women, who attended only up to middle school, had significant pain and disability, and high self-efficacy. The SPADI score can partially predict self-efficacy.

RESUMEN Los factores intrínsecos, como el nivel educativo, la edad y el género, están relacionados con dolor y disfunción, así como la autoeficacia puede modificar el efecto que el dolor y la disfunción tienen en los resultados clínicos de los pacientes. Este estudio transversal tuvo como objetivo evaluar si la puntuación del Índice de discapacidad y dolor de hombro (SPADI), la edad, el nivel educativo y el género son predictores de la autoeficacia en pacientes con dolor de hombro y si existe una diferencia en los niveles de dolor y discapacidad entre grupos de edad y género. Se analizaron los datos sociodemográficos y las puntuaciones de SPADI y la Chronic Pain Self- Efficacy Scale (CPSS) de una base de datos de pacientes que recibieron atención en una clínica pública de fisioterapia especializada en hombro; un total de 123 pacientes con promedio de edad de 54 (±11,54), SPADI de 67,56 (±22,54) y CPSS de 182,22 (±61,76). El análisis de regresión lineal múltiple reveló el SPADI como el único predictor de autoeficacia (β=-1,39 [IC95%=-1,84 a -0,93], p<0,001), lo que explica el 23% de su varianza (r2=0,23). El análisis de varianza (Anova) demostró que la puntuación SPADI fue significativamente distinta entre los géneros (diferencia media=22,27; p<0,001), pero similar entre los grupos de edad (diferencia media=7,04; p=0,16). Se concluyó que los pacientes que se quejaban de dolor de hombro en una clínica pública de fisioterapia, especializada en hombro, eran en su mayoría mujeres de mediana edad, que tenían cursada la escuela primaria, presentaban dolor y discapacidad significativos y alta autoeficacia. La puntuación en el cuestionario SPADI fue capaz de predecir parcialmente la autoeficacia.

Abstract Aims: To investigate the factors associated with shoulder rotational range of motion and its correlation with the self-reported function scale and functional assessment of the shoulder (closed kinetic chain upper extremity stability, peak torque of shoulder rotators, glenohumeral internal rotation deficit - GIRD, and pectoralis minor length index) in competitive water polo players. Methods: Thirty-four competitive water polo players (age: 15 +/- 1.6 years) participated in this study. The shoulder rotational range of motion, closed kinetic chain upper extremity stability, pectoralis minor length index, isokinetic torque of shoulder internal and external rotations, and self-reported upper limb function were assessed. The Pearson correlation coefficient was used to test the linear correlation before the multiple linear regressions, that were used to predict the variables associated with the external rotation and internal rotation range of motion ratio (ER/IR ratio) of the throwing limb. Results: A significant association was identified between the throwing limb ER/IR ratio and GIRD, and this observation allows us to infer that GIRD was responsible for 18.4% (p=0.01) of the variation. No associations between ER/IR ratio and the variables closed kinetic chain upper extremity stability, peak torque of shoulder internal rotation, and pectoralis minor length index were identified. Conclusion: Anatomical GIRD was the only factor associated with the ER/IR ratio in asymptomatic competitive water polo players from high school, indicating the beginning of the typical adaptations to increase performance without significantly altering the self-perception and functionality of their upper limbs.

RESUMO Objetivo: Comparar a função do membro superior e a qualidade de vida entre crianças com paralisia obstétrica do plexo braquial e aquelas sem paralisia do plexo braquial (crianças usuais). Métodos: Foram avaliadas 24 crianças com paralisia obstétrica do plexo braquial e 24 crianças usuais, ambos os grupos com 10±3 anos. A função do membro superior foi avaliada pela Escala Mallet Modificada e Active Movement Scale, já a qualidade de vida foi analisada por meio das escalas Pediatric Outcome Data Collection Instrument e Child Health Questionnaire. Foram realizados testes U de Mann-Whitney para investigar diferenças entre os grupos nas escalas. Resultados: Crianças com paralisia obstétrica do plexo braquial apresentaram menor função do membro superior quando comparadas às crianças usuais, em ambas as escalas utilizadas. Essas crianças também apresentaram menores pontuações para a maioria dos domínios do Pediatric Outcome Data Collection, exceto para conforto/dor. Além disso, apresentaram escores inferiores nos seguintes domínios do Child Health Questionnaire: função física, dor, comportamento, saúde mental, percepção da saúde em geral, impacto emocional nos pais e pontuação psicossocial resumida. Conclusões: A paralisia obstétrica do plexo braquial tem uma influência negativa na função do membro superior e na qualidade de vida, principalmente em relação à saúde geral, mobilidade básica, funções física e psicossocial, felicidade, dor, comportamento, saúde mental, funcionalidade do membro superior e impacto emocional nos pais.

ABSTRACT Objective: To compare the upper limb function and quality of life between children with neonatal brachial plexus palsy and controls with unaffected brachial plexus (typical children). Methods: Twenty-four children with neonatal brachial plexus palsy and 24 typical ones were evaluated, both groups with 10±3 years of age. The upper limb function was assessed by the Modified Mallet Scale and the Active Movement Scale, whereas quality of life was analyzed by the Pediatric Outcome Data Collection Instrument and the Child Health Questionnaire. Mann-Whitney U tests investigated the differences between groups in such scales. Results: Children with neonatal brachial plexus palsy presented lower limb function compared to typical children in both scales. These children also presented lower scores for most of the Pediatric Outcome Data Collection Instrument domains, except for comfort/pain. In addition, they had lower scores in the following domains of the Child Health Questionnaire: physical functioning, pain, behavior, mental health, overall health perception, emotional impact on parents, and psychosocial summarized score. Conclusions: Neonatal brachial plexus palsy has a negative influence on upper limb function and quality of life, mainly considering overall health, basic mobility, physical and psychosocial functions, happiness, pain, behavior, mental health, upper limb function, and emotional impact on their parents.

RESUMO A dor no ombro representa a terceira principal queixa musculoesquelética da população. Afeta fatores físicos, psicológicos e econômicos do indivíduo. A atenção primária à saúde é essencial para a eficácia do cuidado dos pacientes acometidos. Este estudo é observacional transversal e obteve um perfil dos usuários e das consultas médicas com relato de dor no ombro durante um ano na atenção primária do município de Ribeirão Preto (SP). Foram analisados em prontuários os registros das consultas médicas agendadas e sem agendamento prévio. Nestes registros foram coletados dados dos pacientes que apresentaram queixas de dor musculoesquelética no ombro (dados sociodemográficos e características das consultas), que foram analisados de forma descritiva e pelos testes qui-quadrado de Pearson, razão de chance e regressão logística múltipla. A frequência de consultas médicas por queixa de dor no ombro foi de 9,2%. O perfil dos indivíduos que se queixaram de dor no ombro se caracterizava por mulheres, com idade avançada, casadas, alfabetizadas e que apresentavam alguma ocupação. As consultas em sua maioria tiveram retornos agendados, oferecimento de orientações terapêuticas e poucos encaminhamentos.

ABSTRACT Shoulder pain is the third most common musculoskeletal complaint of the world population. It affects the physical, psychological and financial situation of the individual. Primary care is essential to an effective health care for affected patients. This cross-sectional and observational study has the purpose of characterizing the profile of users of the primary health care service, and analyzing the medical records with shoulder pain reports over an one-year period in the primary care service of Ribeirão Preto - SP. Medical records of patients with scheduled and unscheduled medical consultations were analyzed. Data from patients with musculoskeletal shoulder pain - sociodemographic data and the consultation characteristics - were collected and analyzed descriptively and by Pearson’s Chi-squared test, Odds Ratio and Multiple Logistic Regression. The frequency of shoulder pain in the primary care was 9.2%. The profile of individuals who complained about shoulder pain was advanced age, married, literate and working women. Most consultations had scheduled follow-up consultations, therapeutic guidance and few referrals.

RESUMEN El dolor en el hombro representa la tercera principal queja musculoesquelética de la población. Acomete los factores físicos, psicológicos y económicos del individuo. La atención primaria a la salud es esencial para la eficacia del cuidado de los pacientes afectados. Este estudio de tipo observacional transversal obtuvo un perfil de los usuarios y de las consultas médicas en que había relato de dolor en el hombro durante un año en la atención primaria del municipio de Ribeirão Preto (SP). Se analizaron los registros de las consultas médicas programadas y sin programación previa. En estos registros se recolectaron los datos de los pacientes que se quejaban de dolor musculoesquelético en el hombro (sus datos sociodemográficos y las características de las consultas), los cuales fueron analizados de forma descriptiva y por la prueba chi-cuadrado de Pearson, por las razones de prevalencia y por la regresión logística múltiple. La frecuencia de consultas médicas por queja de dolor en el hombro fue del 9,2%. El perfil de los individuos que se quejaron de dolor en el hombro fue de mujeres, con edad avanzada, casadas, alfabetizadas y que se dedicaban a alguna actividad. Las consultas en su mayoría tuvieron retornos programados, ofrecimiento de orientaciones terapéuticas y pocos encaminamientos.

RESUMO O objetivo do estudo foi testar todas as propriedades de confiabilidade do Perfil do Estilo de Vida Individual (PEVI) em pacientes com dor musculoesquelética. Participaram desse estudo 105 pacientes com dor musculoesquelética, recrutados de um serviço público de fisioterapia de nível secundário. O PEVI foi aplicado pelo Examinador 1 na avaliação inicial e pelo Examinador 2 após uma hora. Após três a sete dias da avaliação inicial, o Examinador 1 reaplicou o PEVI. O PEVI e seus componentes apresentaram consistência interna de 0,27 a 0,61 e valores de confiabilidade intra e interexaminador de moderado a excelente (CCI=0,68 a 0,90) e de moderado a quase perfeito (K=0,59 a 0,83). A porcentagem de concordância foi de 61% a 98%, o EPM do PEVI foi de 4,1 pontos e a MMD foi de 5,7 pontos. Concluímos que o PEVI é um instrumento confiável para avaliar o estilo de vida de pacientes com dor musculoesquelética.

ABSTRACT The aim of this study was to test all the reliability properties of the Individual Lifestyle Profile (ILP) in patients with musculoskeletal pain. A total of 105 patients with musculoskeletal pain participated in this study after being recruited from a public secondary physical therapy care. ILP was applied by Rater 1 at the initial assessment and by Rater 2 after one hour. After three to seven days of the initial assessment, Rater 1 reapplied ILP. ILP and its components showed internal consistency from 0.27 to 0.61 and intra- and inter-rater reliability values from moderate to excellent (ICC=0.68 to 0.90) and from moderate to almost perfect (K=0.59 to 0.83). Percent agreement was 61% to 98%, the SEM of ILP was 4.1 points and the MDC was 5.7 points. We conclude that ILP is a reliable instrument to assess the lifestyle of patients with musculoskeletal pain.

RESUMEN El objetivo del estudio fue poner a prueba todas las propiedades de confiabilidad del Perfil de Estilo de Vida Individual (PEVI) en pacientes con dolor musculoesquelético. Participaron en este estudio 105 pacientes con dolor musculoesquelético, provenientes de servicios públicos de fisioterapia de nivel secundario. El evaluador 1 aplicó el PEVI en la evaluación inicial, y el evaluador 2 lo aplicó después de una hora. Después de 3 a 7 días de evaluación inicial, el evaluador 1 reaplicó el PEVI. La consistencia interna del PEVI y sus componentes fue de 0,27 a 0,61; y los valores de confiabilidad intra e interevaluador fueron de moderada a excelente (CCI=0,68 a 0,9), y de moderada a casi perfecta (K=0,59 a 0,83). El porcentaje de concordancia fue de entre un 61% y un 98%, el EPM del PEVI fue de 4,1 puntos, y el CMD fue de 5,7 puntos. Se concluye que el PEVI es un instrumento confiable para evaluar los estilos de vida de pacientes con dolor musculoesquelético.

<sec><title>Aim:</title><p>To compare the electromyographic activity and the asymmetry index among degrees of severity of temporomandibular disorders (TMD).</p></sec><sec><title>Methods:</title><p>Surface electromyography (EMG) of the right and left masseter and temporalis muscles was performed in 126 undergraduate students at rest and at maximal voluntary contraction. Three measurements were performed for five seconds of maximal contraction and mandibular rest. The degree of temporomandibular dysfunction was identified according to the Fonseca anamnestic index. The analysis of the asymmetry index for two pairs of muscles during maximal voluntary contraction was based on the asymmetry index proposed by Naeije, McCarrol and Weijs (1989).</p></sec><sec><title>Results:</title><p>48.41% of the sample had mild TMD, followed by volunteers without TMD (26.98%), moderate TMD (19.05%) and severe (5.56%). The survey results show absence of correlation between the Fonseca anamnestic index and electromyographic activity at rest and at maximal voluntary contraction in undergraduate students in both muscles (p>0.05) and the asymmetry index did not differ between the analyzed groups.</p></sec><sec><title>Conclusions:</title><p>For the population of undergraduate students, there is no evidence that the presence and severity of TMD influence the EMG activity of masseter and temporalis muscles and the muscle asymmetry index at rest and maximal voluntary contraction.</p></sec>

A dor musculoesquelética pode ser caracterizada como aguda ou crônica e é o sintoma mais prevalente na população mundial. É possível afirmar que estará presente na vida de todos os adultos. A incidência tem aumentado muito nos últimos anos em função das mudanças nos hábitos de vida, meio ambiente, além do estresse e aumento das cobranças no mundo corporativo. Os gastos com esse agravo são cada vez maiores e suas consequências físicas, psicológicas e sociais são evidentes. Embora com significativo impacto na vida das pessoas com dor musculoesquelética, poucas são as reflexões que relacionam ensino, demandas de serviços e as doenças musculoesqueléticas. Trata-se de um estudo epidemiológico quantitativo descritivo, tipo levantamento, que teve como objetivo descrever o perfil dos encaminhamentos aos serviços de Fisioterapia e procedimentos relacionados a Classificação Estatística Internacional de Doenças, CID-10, na cidade de Ribeirão Preto e fornecer reflexões para a formação profissional no Brasil. Os resultados apontaram uma maior frequência de encaminhamentos do sexo feminino (70,52%), a dor lombar baixa representou a CID-10 mais frequente (12,14%) e os profissionais Fisioterapeutas, Fisiatras e Médicos Clínicos foram os que mais encaminharam, com 48,57% dos casos. A maioria dos encaminhamentos foi para serviços ambulatoriais (57,55%). Com os resultados, almejou-se fornecer aos gestores dos serviços de saúde e aos responsáveis pela formação em saúde elementos para organizar a demanda de cuidado aos usuários, capacitação de servidores e fundamentar iniciativas de pesquisa, acompanhamento, prevenção desse agravo, além de atualizar as estratégias de ensino.

Musculoskeletal pain can be characterized as acute or chronic and is the most prevalent symptom in the population. It might be assumed that pain will be present in the lives of all adults. Its incidence has greatly increased in recent years due to changes in lifestyle, environment as well as stress and the increasing demands of the cor porate world. Expenses with this injury are increasing and their physical, psychological and social consequences are evident. Although with significant impact on the lives of people with musculoskeletal pain, there are few reflec tions that relate teaching, service demands and muscu loskeletal diseases. This is a descriptive quantitative epi demiological study type survey, which aimed to describe the profile of referrals for physical therapy procedures and related services to International Statistical Classification of Diseases, ICD-10, in the city of Ribeirão Preto and provide reflections for professional training in Brazil. The results showed a higher frequency of referrals of female patients (70.52%), low back pain represented the most frequent ICD-10 (12.14%) and Physical Therapists, Physiatrists and Clinical Doctors were responsible for most of the referrals, with 48.57% of cases. Most referrals were for outpatient services (57.55%). It is aimed to provide to health service managers and those responsible for health training with the results of this research some elements to organize the demand for care users, server training and support re search initiatives, monitoring, preventing these events and updating the teaching strategies.

El dolor musculo esquelético puede caracterizarse como agudo o crónico y es el síntoma más frecuente en la población mundial. Es posible afirmar que estará presente en la vida de todos los adultos. Su incidencia como aguda o crónica ha aumentado mucho en los últimos años en función de cambios en el estilo de vida, del medioambiente, además del estrés y aumento de los cobros en el mundo laboral. Se gasta mucho con este problema y sus consecuencias físicas, psicologías y sociales son evidentes. Aunque con significativo impacto en la vida de los sujetos con dolor musculo esquelético, las reflexiones que relacionan enseñanza, demanda por servicios a las enfermedades musculo esqueléticas son pocas. Se trata de un estudio epidemiológico cuantitativo descriptivo, de tipo encuesta, que tuvo por objetivo describir el perfil de las remisiones a los servicios de fisioterapia y los procedimientos relacionados a la Clasificación Estadística Internacional de Enfermedades, CIE-10, en la ciudad de Ribeirão Preto, SP, con el fin de producir reflexiones a la formación profesional en el Brasil. Los resultados muestran una mayor frecuencia de remisiones del género femenino (70,52%), el dolor lumbar baja representó la CIE-10 más frecuente (12,14%) y los profesionales fisioterapeutas, fisiatras y médicos clínicos fueron los que más encaminaron la gente, con el 48,57% de los casos. La mayoría de las remisiones fueron para servicios de ambulatorio (57,55%). Con los resultados, se anheló proporcionar a los gestores de salud y a los responsables por la formación en salud elementos para organizar la demanda de cuidado a los sujetos, capacitación de los empleados públicos y fundamentación de iniciativas de investigación, supervisión, prevención del problema, además de actualización de estrategias de enseñanza.

Objetivo Avaliar a confiabilidade e responsividade do duty factor à dor provinda da desordem temporomandibular Métodos Participaram 20 voluntárias, com média de idade de 29 anos e 6 meses (±9,2), portadoras de desordem temporomandibular miogênica, segundo o critério diagnóstico para pesquisa em Desordem Temporomandibular (RDC/TMD). Foram coletados sinais eletromiográficos nas condições de repouso dos músculos mastigatórios, na presença de dor moderada a severa e na diminuição ou eliminação desta dor após aplicação de Estimulação Elétrica Nervosa Transcutânea durante 45 minutos. Os sinais eletromiográficos de repouso foram processados para obtenção dos valores do tempo de ativação muscular acima de 10% do valor da máxima força de apertamento dentário. A confiabilidade foi testada pelo coeficiente de correlação intraclasse em dados repetidos antes da analgesia. Também foi calculado o erro padrão de medida e mínima mudança detectável. A responsividade da variável foi analisada entre as coletas eletromiográficas realizadas antes e após a analgesia pelo tamanho de efeito e média de resposta padronizada. Resultados Duty factor apresentou valores de coeficiente de correlação intraclasse acima de 0,75 para todos os músculos. O erro padrão foi entre 4% e 8% e a mínima mudança detectável entre 5% e 12%. Na responsividade da variável para a dor, o tamanho de efeito obteve valores entre 0,2 e 0,5 e a média da resposta padronizada, valores acima de 0,8. Conclusão O duty factor apresentou confiabilidade excelente e responsividade à dor da desordem temporomandibular baixa para tamanho de efeito e excelente para média da resposta padronizada.

Purpose To assess the reliability and responsiveness of the duty factor variable for assessing pain originating from temporomandibular disorders. Methods The sample comprised 20 female volunteers, mean age 29 years 6 months (± 9.2), with a diagnosis of myogenic temporomandibular dysfunction according to the Research Diagnostic Criteria for Temporomandibular Disorders. Electromyographic (EMG) signals were collected at three times: during mandibular rest; in the presence of moderate to severe pain; and when pain was reduced to mild or absent after 45 minutes of transcutaneous electrical nerve stimulation (TENS). Electromyographic signals during mandibular rest were processed to obtain values for muscle activation time greater than 10% of maximum bite force. Reliability was tested with intraclass correlation for repeated data before analgesia. Standard error of measurement (SEM) and minimum detectable change (MDC) were also computed to determine reliability. Responsiveness of duty factor was analyzed between EMG recordings, before and after analgesia, by calculating effect size (ES) and standardized response mean (SRM). Results Duty factor presented intraclass correlation coefficient above 0.75 for all muscles. Standard error of measurement ranged from 4% to 8% and minimum detectable change from 5% to 12%. Regarding the responsiveness of duty factor to pain, effect size values fell between 0.2 and 0.5 and SRM values were greater than 0.8. Conclusion Duty factor showed excellent reliability. However, responsiveness to TMD-related pain was low as expressed by effect size and excellent as expressed by standardized response mean.
